Description
Anuloplastiering
Die Erfindung betrifft einen insbesondere für die Aortenklappenanuloplastie geeigneten Anu- loplastring mit einem den Anuloplastiering umgebenden Textilüberzug.
Anuloplastieringe werden für die intravasale und extravasale Implantation zur Wiederherstellung pathologisch veränderter Herzklappen verwendet. Durch Vernähen dieser Prothesen mit dem Herzgewebe kann man einer insuffizienten Herzklappe ihre funktionell richtige Ringstruktur wiedergeben, sodass die Herzklappensegel bzw. Herzklappentaschen wieder zu einem exakten Schluss zusammengeführt werden.
Anuloplastieringe sind in DE 694 03 482 T2 und EP 0 338 994 AI beschrieben.
Ein Problem bekannter Anuloplastieringe, insbesondere bei Anuloplastieringen für die Aortenklappenanuloplastie, ist, dass die Ringe direkt unterhalb der Klappe eingebracht werden müssen und im Falle einer Aortenklappe die Klappentaschen während der Diastole, d.h. wenn diese voll entfaltet unter Druck sind, den oberen Rand des Anuloplastierings berühren und damit grundsätzlich - mit fatalen Folgen - verletzt werden können.
Aufgabe der Erfindung ist es daher, einen Anuloplastiering, insbesondere einen für die Aortenklappenanuloplastie geeigneten Anuloplastring zu schaffen, der die Gefahr von Verletzungen der Klappensegel bzw. -taschen verringert.
Die Aufgabe wird durch den Anuloplastiering mit den Merkmalen von Anspruch 1 gelöst. Die Unteransprüche geben vorteilhafte Ausgestaltungen der Erfindung wieder.
Grundgedanke der Erfindung ist es, den bei Anuloplastieringen bekannten Textilüberzug derart auszugestalten, dass dieser einen sich im Wesentlichen vertikal aus der Ringebene erstreckenden Abschnitt aufweist, der einen Nahtsaum ausbildet.
Durch diesen (Naht-) Saum wird zum einen ein vorteilhafter Abstand des Anuloplastierings zur Herzklappenbasis erzwungen und bei im Querschnitt bevorzugt kegelförmiger Ausbildung des Saums vermieden, dass die Herzklappensegel oder -taschen mit dem Anuloplastiering nach der Erfindung in Berührung kommen.
Zum anderen dient der Saum als Nahtring zur Befestigung des Anuloplastierings am Herzgewebe, sodass die Befestigung des Anuloplastierings erleichtert wird.
Neben der im Querschnitt kegelförmigen Ausbildung des Nahtsaums sind auch andere Formen, beispielsweise oval oder rechteckig, denkbar, wobei darauf geachtet werden muss, dass der Nahtsaum in situ nicht mit der Herzklappe in Berührung kommen kann.
Der Anuloplastiering kann aus jedem geeigneten Material, beispielsweise Metall oder Kunststoff hergestellt sein. Der textile Überzug besteht bevorzugt aus Polyethylenterephthalat (PET; Dacron®).
Die Erfindung wird anhand von in den beiliegenden Zeichnungen dargestellten besonders bevorzugt ausgebildeten Ausfuhrungsbeispielen eines Aortenklappenanuloplastierings näher erläutert, wobei die Erfindung auch bei für andere Herzklappen geeigneten Anuloplastierin- gen angewendet werden kann. Es zeigen:

Fig. 1 zeigt eine perspektivische Ansicht eines aus dem Stand der Technik bekannten Aortenklappenanuloplastierings. Der vorbekannte Aortenklappenanuloplastiering 10 kann von Außen oder von Innen am Aortenklappenring angenäht werden und ist daher leicht implantierbar. Um Schädigungen des Reizleitungssystems des Herzens beim Annähen des Anuloplastierings 10 durch direkte Verletzungen oder Verziehungen zu vermeiden, ist vorgesehen, dass sich zwei zu einer (abgerundeten) Spitze aufeinander zulaufende Abschnitte derart weit aus der Ringebene erstrecken, dass der kritische Bereich des Reizleitungssystems ausgespart wird und diese Abschnitte problemlos mit der Aortenwandung vernäht werden können.
Zusätzlich kann eine weitere Spitze vorgesehen sein, wobei die Spitzen der dreieckigen Struktur zwischen dem rechts koronartragenden Sinus und dem nicht koronartragenden Sinus bzw. dem rechts koronartragenden Sinus und dem links koronartragenden Sinus angepasst sind.
Im Übrigen weist der Ring 10 eine optimale Konformation in Bezug auf äußere Form und Elastizität bzw. Festigkeit auf, um den Koaptationsrand der Taschensegel zu erhöhen und die Rekonstruktion zu erleichtern und zu stabilisieren.
Die sich aus der Ringebene erstreckenden Abschnitte sind annähernd der Anatomie zwischen dem rechten koronartragenden Sinus und dem nicht koronartragenden Sinus bzw. zwischen dem rechts koronartragenden Sinus und dem links koronartragenden Sinus nachempfunden und haben vorteilhafterweise eine zum Mittelpunkt des gedachten, von den Abschnitten gebildeten Dreiecks konvexe Konfiguration. Dieses bedeutet, dass sich die Abschnitte leicht in Richtung der Ringebene (konvex) gebogen erstrecken (nicht dargestellt). Die Spitzen können derart ausgebildet sein, dass nur die Ringoberkante aus der Ringebene geführt ist, während die Ringunterkante auf derselben Höhe wie die Ringunterkante verbleibt. Es entsteht also ein flächig verfülltes Dreieck, sodass der Aortenklappenanuloplastiering 10 eine größere flächige Seitenwandung aufweist.
Der Aortenklappen- Anuloplastiering 10 nach dem Stand der Technik kann also sowohl für (funktionell) bikuspide als auch trikuspide Aortenklappen Verwendung finden.
Fig. 2 zeigt nun eine geschnittene Teilansicht eines erfindungsgemäß ausgestalteten Anulo- pastierings 10, beispielsweise des erfindungsgemäß ausgestalteten Aortenklappen- Anuloplastierings 10 aus Fig. 1. Es kann sich aber auch um einen für andere Herzklappen geeigneten anderen Anuloplastiering 10 handeln. Der Anuloplastiering 10 ist von einem erfindungsgemäß vorteilhaft ausgebildeten Textilüber- zug 20 umgeben. Insbesondere weist der Textilüberzug 20 einen sich im Wesentlichen vertikal aus der Ringebene erstreckenden Abschnitt 30 auf, der einen Nahtsaum ausbildet. Der Nahtsaum 30 ist oberhalb des Rings 10 in Strömungsrichtung, also in Richtung der Herzklappe ausgerichtet und bevorzugt ebenfalls ringförmig ausgebildet. Speziell weist der Nahtsaum eine im Querschnitt bevorzugt symmetrisch ausgebildete, insbesondere kegelförmige bzw. spitz zulaufende Geometrie auf, sodass eine Schräge entsteht, die zum einen als Nahtring, zum anderen zur Vermeidung einer Berührung der Herzklappenbasis mit der Prothese dient.
Die im Wesentlichen vertikale Erstreckung des Nahtsaums aus der Ringebene, bevorzugt in derselben Richtung wie die in Fig. 1 gezeigten sich aus der Ringebene erstreckenden Ringab- schnitte, ist wenigstens um das 1,5-fache, bevorzugt um wenigstens das 2-fache länger als die Erstreckung des Textilüberzugs 20 von der Oberfläche des Anuloplastierings 10 in andere
Richtungen, insbesondere in der Ringebene. Der Nahtsaum erstreckt sich aus der Ringebene bevorzugt über eine Länge von 1 bis 5 mm.
Der Textilüberzug 20 umgibt den Anuloplastiering 10 wenigstens abschnittsweise, ist aber bevorzugt über den gesamten Umfang des Rings 10 ausgebildet. Bevorzugt ist der Textilüberzug 20 aus Polyethylenterephthalat (PET; Dacron®) hergestellt.
Der Anuloplastiering 10 ist nach einer bevorzugten Ausgestaltung wenigstens abschnittsweise flexibel ausgebildet, wobei diese Flexibilität des Anuloplastierings 10 bei der Ausgestaltung des Textilüberzugs 20 berücksichtigt sein muss. Speziell kann vorgesehen sein, dass der Anuloplastiering 10 als offener Ring ausgebildet ist, der Textilüberzug 20 jedoch einen geschlossenen Ring bzw. Torus bildet.
Fig. 3 zeigt eine geschnittene Teilansicht des erfindungsgemäßen Anuloplastierings in situ. Von der Herzanatomie ist lediglich die Aortenwandung AW der Sinus valsalvae, die Wandung des linken Ventrikels VW und die (geschlossene) Aortenklappentasche AKT dargestellt. Der Anuloplastiering 10 mit erfindungsgemäß ausgebildetem Textilüberzug 20 ist durch den Nahtsaum 30 zur Aortenklappenbasis beabstandet angeordnet, sodass die Gefahr von Verletzungen der Klappentasche vermindert ist. Gleichzeitig bietet die vom äußeren Umfang zur Spitze des Nahtsaums 30 verlaufende Schräge eine sanft geneigte Fläche zur Vermeidung einer Berührung der Klappenbasis mit dem Ring 10, wodurch die Gefahr von Verletzungen weiter vermindert wird. Schließlich bildet der Nahtsaum bevorzugt einen Nahtring der mit dem Nahtmaterial N durchstochen werden kann.
Fig. 4 zeigt schließlich ein besonders bevorzugt ausgestaltetes Ausführungsbeispiel in einer Detailansicht in situ. Insbesondere zeigt Fig. 4 einen Ausschnitt der anatomischen Verhältnisse im Bereich zwischen links-koronar-tragenden Sinus und rechts-koronar-tragenden Sinus. Es ist deutlich zu erkennen, dass der Textilüberzug 20 in diesem Bereich der anatomischen Form des Herzens dadurch angepasst ist, dass sich der Textilüberzug weiter aus der Ringebene erstreckt als in anderen Abschnitten - hierfür sind keine - wie noch aus dem Stand der Technik (siehe Fig. 1) bekannt - sich aus der Ringebene erstreckenden Abschnitte des Anuloplastierings 10 selbst notwendig. Es genügt, dass der Textilüberzug in Blutströmungsrichtung abschnittsweise sich unterschiedlich weit erstreckend ausgebildet ist, also die (Ober-) Kante des Nahtsaums 30 einen unterschiedlich großen Abstand zum Anuloplastiering 10 (bzw. dessen Oberkante) hat.
Diese Nahtringerhöhung ist einfach herzustellen und kann je nach der patientenspezifischen Anatomie entsprechend vernäht werden. Somit kann der Fertigungsaufwand, der üblicherweise für die passgenaue Anfertigung des Anuloplastierings 10 (also den„Kern" ohne Überzug) mit gegebenenfalls mehreren verschiedenen sich aus der Ringebene erstreckenden Abschnit- ten notwendig ist, dadurch entfallen, dass eine oder mehrere Nahtringerhöhungen vorgesehen sind.
Die Nahtringerhöhungen erfüllen dabei dieselben Aufgaben, wie der in Fig. 1 dargestellte Anuloplastiering: einerseits wird der kritische Bereich des Reizleitungssystems ausgespart und andererseits kann eine feste und formgebende Verbindung des Anuloplastierings mit der Aortenwandung erfolgen.
Claims
1. Anuloplastiering (10) mit einem den Anuloplastiering (10) umgebenden Textilüberzug (20), wobei der Textilüberzug (20) einen sich im Wesentlichen vertikal aus der Ringebene erstreckenden, einen Nahtsaum ausbildenden Abschnitt (30) aufweist.
2. Anuloplastiering (10) nach Anspruch 1, wobei der Anuloplastiering (10) selbst keine sich aus der Ringebene erstreckenden Abschnitte aufweist.
3. Anuloplastiering (10) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass sich der Nahtsaum ausbildende Abschnitt (30) abschnittsweise unterschiedlich weit aus der Ringebene erstreckt.
4. Anuloplastiering (10) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Textilüberzug (20) den Anuloplastiering (10) wenigstens abschnittsweise umgibt.
5. Anuloplastiering (10) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Textilüberzug aus Polyethylenterephthalat (PET) besteht.
6. Anuloplastiering (10) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Anuloplastiering (10) wenigstens abschnittsweise flexibel ausgebildet ist.
7. Anuloplastiering (10) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Anuloplastiering (10) als offener Ring ausgebildet ist.
