Antecedentes do invento
Campo do invento 0 presente invento refere-se, de um modo geral, a elementos de ligação para dispositivos de accionamento electromecânicos, e de modo mais especifico, a um elemento de ligação para um dispositivo de accionamento electromecânico que pode ser utilizado para agrafar tecido.
Descrição da arte anterior
Após a identificação de uma região de musculatura com hérnia na parede abdominal, é quase sempre necessária uma intervenção cirúrgica. 0 procedimento é fácil de compreender em conceito, mas inclui um número de caracteristicas tecnicamente sofisticadas, em especial na medida em que é, habitualmente, concretizada de forma remota, i.e., através de pequenos orifícios formados na parte inferior do tronco do paciente. Com maior particularidade, a técnica utilizada com maior frequência envolve a formação de dois ou três orifícios de cirurgia no abdómen inferior do paciente, a inserção de um tubo cilíndrico correspondente através de cada um dos orifícios, a passagem de instrumentos alongados através dos tubos, a retracção de tecidos moles para fora do sítio da hérnia e agrafar o tecido fissurado fechado. Esta fase de fecho final muitas vezes inclui, também, a utilização de uma rede sintética, que é colocada sobre o orifício com a finalidade de suportar os tecidos moles e órgãos dispostos por cima do orifício em cicatrização. A rede é agrafada directamente à musculatura, na vizinhança do defeito. O presente invento é dirigido à instrumentação que é o componente agrafador.
Os mecanismos agrafadores tradicionais funcionam de uma forma simples. 0 agrafo é inicialmente colocado num formato de U. O componente de accionamento de agrafos contacta com a 2 ΕΡ 1 257 206 / PT porção de base do agrafo e faz avançar as duas pontas do agrafo que se estendem para cima através dos elementos que se destinam a ser unidos e em direcção a uma porção de bigorna estacionária. Com frequência, os elementos que se destinam a ser unidos necessitam que a porção de bigorna seja colocada para além dos mesmos para proporcionar suporte adequado para que as pontas do agrafo possam atravessar. Logo que as pontas do agrafo tenham avançado através dos elementos que se destinam a ser unidos, as pontas contactam com a bigorna. Com frequência, a bigorna compreende ranhuras guia para virar o agrafo ai formadas para guiar a deformação plástica dos agrafos à medida que os mesmos avançam sob a motivação do accionador de agrafos. Esta deformação plástica vira (ou dobra) os agrafos, um em direcção ao outro, a fim de formar o formato caixa tradicional a partir do formato U original. Os dispositivos agrafadores utilizados em cirurgia de laparoscopia e endoscopia são significativamente diferentes dos mecanismos agrafadores tradicionais em concepção e função. Ao contrário dos mecanismos agrafadores tradicionais, uma porção de bigorna espacialmente distinta e oposta não é prática e não é, por conseguinte, utilizada. Em alternativa, os dispositivos agrafadores cirúrgicos compreendem um componente com accionador de agrafos e bigorna integrados que contém e faz avançar o agrafo para e através do tecido que se destina a ser unido. Mais em particular, como mostrado na Fig. 1, os dispositivos agrafadores de tecidos cirúrgicos internos da técnica antecedente compreendem um tubo alongado 10 que é concebido para deslizar através do tubo portal do orifício de entrada minimamente invasivo. A ponta 12 do tubo 10 inclui uma pluralidade de agrafos individuais que são engatados de forma sequencial pelo accionador de agrafos. Cada agrafo está inicialmente disposto em forma de U. O formato em U pode ser descrito de forma mais específica através da identificação das diferentes regiões do mesmo. A porção de base plana do agrafo possui uma porção central e porções laterais. As pontas que se estendem para cima, que estão dispostas nas extremidades laterais da porção de base plana, são, cada uma, aproximadamente, equivalentes em comprimento e são, 3 ΕΡ 1 257 206 / PT aproximadamente, iguais a um terço do comprimento da porção de base plana. O accionador de agrafos inicialmente impele o agrafo para a frente ao pressionar contra a porção de base do agrafo na porção central do mesmo. 0 agrafo é obrigado a avançar para a frente até que as pontas se tenham estendido para além da ponta do alojamento e tenham penetrado no tecido que se destina a ser unido. O avanço adicional do agrafo é evitado por uma porção de bigorna que está fixa na ponta do mecanismo. A bigorna, ao contrário dos mecanismos agrafadores tradicionais, apenas contacta a porção central da porção da base do agrafo, oposta ao accionador de agrafos. Dito de forma equivalente, logo que o agrafo tenha avançado completamente no interior da ponta do dispositivo, a bigorna na ponta evita que o agrafo se mova para a frente na medida em que o mesmo fica restringido entre o accionador de agrafos na parte de trás e a bigorna na frente.
Nesta altura, um segundo mecanismo é engatado a fim de fechar o agrafo. Este mecanismo de fecho compreende um par de braços direitos que avançam para a frente a partir do interior da ponta e contactam o agrafo nas porções laterais da porção plana da base. O avanço prolongado dos braços que viram as pontas obriga o agrafo a deformar à medida que as pontas do agrafo são curvadas, à força, uma contra a outra. No entanto, o verdadeiro sitio da viragem não é ao longo das pontas. É na união das porções lateral e central da porção de base plana. Isto obriga o agrafo a ser completamente deformado no seu dispositivo agrafador cirúrgico para obter um formato em D em oposição ao formato em B que os agrafos completamente deformados dos mecanismos agrafadores tradicionais obtêm. A libertação do agrafo, logo que o mesmo tenha sido acoplado ao tecido unido, é conseguida ao retrair, primeiro, os braços que viraram o agrafo e, depois, torcendo o veio do dispositivo para puxar o agrafo para fora da bigorna.
Uma característica adicional dos dispositivos agrafadores cirúrgicos da técnica antecedente que deverá ser apreciada à luz do presente invento é o meio pelo qual todos os elementos de avanço, i.e., o accionador de agrafos e os braços que viram as pontas, são accionados. Nos dispositivos agrafadores cirúrgicos minimamente invasivos da técnica antecedente, a porção tubular alongada termina num manipulo que inclui um 4 ΕΡ 1 257 206 / PT gatilho. O gatilho possui uma função dupla em que a compressão do gatilho faz avançar, inicialmente, o accionador de agrafos sem deslocar os braços que viram as pontas. Logo que o agrafo tenha sido completamente obrigado a avançar e esteja pronto para ser virado, os braços são obrigados a avançar através da ponta para entrar em contacto com o agrafo, por compressão prolongada do gatilho em estilo de garra.
Um exemplo deste tipo geral de dispositivo agrafador da técnica antecedente é mostrado na patente americana 5 389 098.
Será facilmente reconhecível que a necessidade de accionamento manual, bem como o mecanismo em duas fases para fazer avançar e depois formar o agrafo, estão cheios de falhas potenciais. Em adição, a necessidade de torcer o veio do dispositivo para puxar o agrafo para fora da bigorna logo que o agrafo tenha unido o tecido apresenta um risco significativo de fissurar o tecido na medida em que esta acção esforça o agrafo e o tecido.
Em adição, outro inconveniente é o mecanismo para descarga dos agrafos ser um gatilho acoplado a um veio alongado rígido que segue um movimento de translação para a frente e para trás em correspondência com a compressão e a libertação do gatilho. A rigidez do veio requer que o cirurgião manipule todo o dispositivo a fim de alinhar o agrafador de forma adequada, visto que as hérnias da cavidade abdominal ocorrem, com frequência, sobre a parede do abdómen, esta manipulação é, com frequência, difícil.
Finalmente, em relação ao utilizador dos dispositivos agrafadores cirúrgicos da técnica antecedente, os dispositivos são construídos de modo a serem completamente descartáveis. A incapacidade de reutilizar os dispositivos aumenta o custo de utilização dos dispositivos. Mais especificamente, esta característica não diminui o custo global de produção devido às óbvias necessidades de utilização médica, mas não aumenta o custo por procedimento na medida em que um novo dispositivo tem de ser utilizado em cada cirurgia. De facto, se mais agrafos são necessários para a cirurgia do que os que são disponibilizados no dispositivo, um dispositivo completamente 5
ΕΡ 1 257 206 / PT novo tem de ser utilizado devido a não existir a possibilidade de simplesmente substituir o cartucho de agrafos vazio. É, por conseguinte, um principal objecto do presente invento proporcionar um dispositivo agrafador cirúrgico que possa ser facilmente manipulado na posição adequada sem ter de deformar de forma grosseira os tecidos moles através dos quais passa o veio alongado do dispositivo. É também um objecto do presente invento proporcionar um dispositivo agrafador cirúrgico que possua um mecanismo de descarga de agrafos com uma única paragem. É um objecto relacionado do presente invento proporcionar um dispositivo agrafador cirúrgico que não esforce o tecido envolvente quando o agrafo é libertado da ponta do dispositivo. É um objecto adicional do presente invento proporcionar um dispositivo agrafador cirúrgico que não necessite que se descarte a ponta e o accionador de agrafos, de modo a minimizar o desperdício médico e o custo associado com a utilização do dispositivo quando comparado com dispositivos agrafadores cirúrgicos da técnica antecedente.
Outros objectos do presente invento serão reconhecidos de acordo com a descrição do mesmo disponibilizada de seguida e na descrição detalhada da concretização preferida em conjunção com as restantes figuras.
Sumário do invento
Os objectos precedentes do invento são proporcionados por um elemento de ligação por agrafo do presente invento que pode ser acoplado e actuado de forma remota através de um dispositivo de accionamento electromecânico. Em particular, o invento proporciona um agrafador cirúrgico como definido na reivindicação 1. Este agrafador acopla numa extremidade de acoplamento proximal a uma extremidade distai de um veio de transmissão flexível do dispositivo de accionamento electromecânico, de modo a acoplar um veio roscado do elemento de ligação a um veio de tracção flexível do veio flexível do 6 ΕΡ 1 257 206 / PT dispositivo de accionamento electromecânico. Uma vez que os gatilhos de dedo actuados no dispositivo accionador electromagnético obrigam o veio de tracção flexível a rodar, o veio roscado roda e faz avançar no alojamento do elemento de ligação um elemento de accionamento roscado do elemento de ligação que faz deslocar no alojamento. O avanço do elemento de accionamento roscado faz avançar um elemento para fechar agrafos montado sobre o elemento de accionamento roscado bem como um elemento que contém agrafos acoplado por uma mola ao elemento para fechar agrafos. Cada um dos motores 60, 62 é um motor de duplo sentido e estão também acoplados a um comutador de accionamento manual 66 montado no topo do manipulo 50, pelo qual o utilizador pode, de forma selectiva, alterar o sentido de rotação de cada motor 60, 62. Num caso deste tipo, o veio flexível 72 e o manipulo 50 deveriam ser separáveis e tais que a interface entre a extremidade proximal do veio flexível 72 e a extremidade distai do manipulo 50 inclua um meio de acoplamento para os componentes de accionamento. De modo específico, em relação aos componentes de accionamento do veio flexível 72, no interior da bainha elastomérica está um par de tubos rígidos mais pequenos 76a, 76b em que cada um contém um veio de transmissão flexível 78a, 78b que pode rodar no interior do tubo fixo correspondente 76a, 76b. Cada veio de transmissão flexível 78a, 78b pode, por si próprio, transferir um binário do motor de accionamento correspondente 60, 62 no manipulo 50 para a extremidade distai 80 do veio flexível 72, enquanto é ainda suficientemente flexível para ser virado, inclinado, curvado, etc. na medida em que o cirurgião considere necessário para “serpentear" o veio flexível 72 através do intestino do paciente. Por exemplo, os veios de transmissão flexíveis 78a, 78b podem compreender um cabo de fibra com malha de aço. Deverá ser reconhecido que outros veios de transmissão flexíveis podem ser adequados para esta finalidade. Também, como sugerido acima, em ligação com o indicador de estado remoto 68 do manipulo 50, o veio flexível 72 também contém um condutor eléctrico 86 para acoplar aos elementos de ligação. Quando a extremidade proximal de acoplamento do alojamento do elemento de ligação por agrafo está acoplada à extremidade distai do veio flexível do dispositivo de accionamento electromecânico, o veio roscado do elemento de ligação por agrafo engata no veio de tracção flexível do veio flexível. A actuação remota do veio de tracção flexível através da utilização de um primeiro gatilho de dedo do dispositivo de accionamento electromecânico obriga o veio de tracção flexível a rodar, o que, por sua vez, obriga o veio roscado a rodar, o que obriga o elemento de accionamento roscado a avançar no interior do alojamento do elemento de ligação. A actuação remota alternada do veio de tracção flexível através da utilização de um segundo gatilho de dedo obriga o veio de tracção flexível a inverter a rotação, o que, por sua vez, obriga o veio roscado a inverter a rotação, o que obriga o elemento de accionamento roscado a retrair no interior do alojamento do elemento de ligação. Assim, como referido acima, o elemento que contém agrafos e o elemento que suporta agrafos avançam e retraem com o elemento de accionamento roscado. Quando o elemento que suporta agrafos é retraído para uma 11 ΕΡ 1 257 206 / PT posição de recepção de agrafos, adjacente ao cartucho de agrafos, um agrafo é impelido pelo dispensador de agrafos pressionado por mola do cartucho de agrafos para a ranhura de suporte de agrafos do elemento que suporta agrafos. À medida que o elemento que contém agrafos e o elemento para fechar agrafos avançam no interior do alojamento, o agrafo é transportado no interior da ranhura de suporte de agrafos. Descrição detalhada das concretizações preferidas
Como definido acima, o presente invento compreende um elemento de ligação de tecido por agrafo para utilização com um dispositivo de accionamento electromecânico. Com maior particularidade, e referindo, agora, as Figs. 3 e 4, uma concretização preferida do elemento de ligação 100 compreende um alojamento 102 com uma extremidade distai de descarga de agrafos 104 de uma extremidade proximal de acoplamento 106 que possui um encaixe 114 para acoplar o elemento de ligação 100 à extremidade distai (mostrado melhor como elemento 80 da Fig. 2) do veio flexível (mostrado melhor como elemento 72 da Fig. 2) do dispositivo de accionamento electromecânico (mostrado melhor na Fig. 2). (De um modo geral, ver o pedido de patente Americano co-pendente com o número de série U.S. 09/324 452, com o título "An Electromechanical Driver Device For Use With Anastomosing, Stapling, and Resecting Instruments".) O elemento 118 que contém agrafos e o elemento 124 para fechar agrafos estão unidos por uma mola 126 e montados no elemento de accionamento roscado 116 de modo que o avanço do elemento de accionamento roscado 116 faz avançar o elemento 118 que contém agrafos e o elemento 124 para fechar agrafos. O elemento 124 para fechar agrafos compreende um par de saliências, ou braços salientes 128 que estão posicionados para engatar o agrafo 130 nas porções laterais 132 (mostrado melhor na Fig. 4) da porção de base plana 134 (mostrado melhor na Fig. 4) do agrafo 130. Em funcionamento, quando a extremidade proximal de acoplamento 106 do alojamento 102 do elemento de ligação por agrafo 100 está acoplada à extremidade distai do veio flexível do dispositivo de accionamento electromecânico (mostrado melhor na Fig. 2), o veio roscado 112 do elemento de ligação por agrafo 100 engata no veio de tracção flexível (mostrado melhor como elemento 78a ou elemento 78b na Fig. 2) do veio flexível (mostrado melhor como elemento 72 na Fig. 2). A actuação remota do veio de tracção flexível através da utilização de um gatilho de dedo (mostrado melhor como elemento 58a ou elemento 58b na Fig. 2) do dispositivo de accionamento electromecânico obriga o veio de tracção flexível a rodar, o que, por sua vez, obriga o veio roscado 112 a rodar, o que obriga o elemento de accionamento roscado 116 a avançar no interior do alojamento 102 do elemento de ligação 100. A comutação remota do comutador de accionamento manual (mostrado melhor como elemento 66 na Fig. 2) do dispositivo de accionamento electromecânico e a subsequente actuação remota do gatilho de dedo do dispositivo de accionamento electromecânico obriga o veio de tracção flexível a inverter a rotação, o que, por sua vez, obriga o veio roscado 112 a inverter a rotação, o que obriga o elemento de accionamento roscado 116 a retrair no interior do alojamento 102 do elemento de ligação 100. Deste modo, como referido acima, o elemento 118 que contém agrafos e o elemento que suporta agrafos 120 avançam e retraem com o elemento de accionamento roscado 116. Quando o elemento que suporta agrafos 120 é retraído para uma posição de recepção de agrafos, adjacente ao cartucho de agrafos 144, um agrafo da pluralidade de agrafos 146 é impelido pelo dispensador de agrafos pressionado por mola 148 do cartucho de agrafos 144 para a ranhura 140 que suporta agrafos do elemento de suporte de agrafos 120. À medida que o elemento 118 que contém agrafos e o elemento 124 para fechar agrafos avançam no interior do alojamento 102, o agrafo 130 é transportado no interior da ranhura 140 que suporta agrafos. - 3 - Agrafador cirúrgico (100) de acordo com a reivindicação 1, em que o agrafador cirúrgico (100) faz parte de um sistema cirúrgico electromecânico que também compreende: ΕΡ 1 257 206 / PT 2/3 um veio alongado (72); um segundo veio de transmissão rotativo axialmente (78a, 78b) disposto no interior do veio alongado (72); e um sistema de motorização (60, 62) configurado para accionar o segundo veio de transmissão rotativo axialmente (78a, 78b); em que o agrafador cirúrgico (100) está configurado para se acoplar de forma a separar-se a uma extremidade distai (80) do veio alongado (72), e em que o agrafador cirúrgico (100) inclui um accionador (116) disposto no alojamento (102), o elemento (124) para fechar agrafos montado no accionador (116) e seguindo um movimento de translação no interior do alojamento (102) em correspondência com o accionador (116).
- 4 - Agrafador cirúrgico (100) de acordo com a reivindicação 3, em que o accionador (116) inclui o veio de transmissão rotativo (112), estando o veio de transmissão rotativo (112) roscado, estando o veio de transmissão rotativo (112) configurado para ser acoplado ao segundo veio de transmissão de rotação axial (78a, 78b).
- 5 - Agrafador cirúrgico (100) de acordo com a reivindicação 3, em que o sistema cirúrgico electromecânico também compreende: um sistema de controlo (66) configurado para controlar o sistema motorizado (60, 62); e uma unidade de controlo remoto configurada para comunicar com o sistema de controlo para controlar o sistema motorizado através do sistema de controlo.
- 6 - Agrafador cirúrgico (100) de acordo com a reivindicação 5, em que a unidade de controlo remoto inclui uma unidade de controlo remoto por fio.
