Description
Foreliggende oppfinnelse vedrører forbindelser som er i stand til å modulere cannabinoidreseptorer.
Bakgrunn for oppfinnelsen
Det er i dag en fornyet interesse for de terapeutiske anvendelsene av medisinsk cannabis og syntetiske cannabinoider, slik som A<9->tetrahydrocannabiol (THC) [1], som er den aktive komponenten i cannabis.
THC kan være terapeutisk fordelaktig på flere hovedområder i medisin inkludert kontroll av akutt og spesielt kronisk/neuropatisk smerte, kvalme, anoreksi, AIDS, glaukom, astma og ved multiple sklerose [Baker, D. et al., Nature 2000, 404, 84-87; Baker, D. et al., FASEB J. 2001, 15, 300-302; Schnelle, M. et al., Forsch. Komplementarmed., 1999, 6 Suppl. 3, 28-36].
Et antall cannabinoidligander har blitt rapportert i litteraturen. WO 00116756 beskriver N-acyl-vanillin-amidderivater som er i stand til å aktivere perifere cannabinoid-reseptorer. US 2003/0191069 beskriver 2-oksokinolin forbindelser som er i stand til å virke selektivt på cannabinoid-reseptorer. Til slutt beskriver US 5.342.971 dibenzo[b,d]pyraner og fremgangsmåter for fremstilling av forløperne til THC.
Grovt sett kan cannabinoidligander bli delt i tre hovedgrupper som består av (i) klassiske cannabinoider slik som (-)-A<9->tetrahydrocannabinol, A<9->THC [1] og CP55,940 [9];
(ii) endocannabinoider, slik som anandamid [2] og 2-arakidonylglyserol [3]; og (iii) ikke-klassiske heterosykliske analoger som typisk er heterosykler slik som WIN 55,212 [7] og den selektive CBi-antagonisten SR141716A [8] [Pertwee, R.G., Pharmacology & Therapeutics 1997, 74, 129-180]. Konformasjonelt avgrensede anandamidanaloger har også blitt rapportert [Berglund, B.A. et al., Drug Design and Discovery 2000, 16, 281-294]. Per i dag er likevel den terapeutiske nyttigheten av cannabinoidlegemidler begrenset ved deres uønskede psykoaktive egenskaper.
Cannabinoider er kjent for å modulere nociceptiv prosessering i modeller med akutt, inflammatorisk og neuropatisk smerte [Pertwee, R.G., Prog. Neurobiol. 2001, 63, 569-611]. Mer spesifikt har forskning blitt sentrert rundt rollen til cannabinoider I modeller med neuropatisk hyperalgesi [Herzberg, U. et al., Neurosci. Lett. 1997, 221, 157-160] og inflammatorisk hyperalgesi [Richardson, J.D., Pain 1998, 75, 111-119; Jaggar, S.I. et al., Pain 1998, 76, 189-199; Calignano, A. et al., Nature 1998, 394, 277-281; Hanus, L. et al, Proe. Nati. Acad. Sei. USA 1999, 96, 14228-14233]. Det har også blitt foreslått at cannabinoidreseptoruttrykking og nivået på endogene cannabinoider kan endres i løpet av inflammasjon og hyperalgesi [Pertwee, R.G., Prog. Naurobiol. 2001, 63, 569-611].
Cannabinoidsignaliseringssystemet er antatt å involvere to klonede cannabinoid-reseptorer (CBiog CB2), endocannabinoidligander slik som anandamid [2] og 2-arakidonylglyserol [3] og et endocannabinoid degraderingssystem [Howlett, A.C. et al., International Union of Pharmacology XXVII, Pharmacol. Rev. 2002, 54, 161-202; Pertwee, R.G., Pharmacology of cannabinoid receptor ligands. Curr. Med. Chem. 1999, 6, 635-664].
En viktig funksjon for cannabinoidsystemet er å virke som en regulator for synaptisk neurotransmitterfrigjøring [Kreitzer, A.C. et al, Neuron 2001, 29, 717-727; Wilson, R.I. et al, Neuron 2001, 31, 453-462]. CBiblir uttrykt ved høye nivåer i CNS, spesielt i globus pallidus, substantia nigra, cerebellum og hippocampus [Howlett, A.C, Neurobiol. Dis. 1998, 5, 405-416]. Dette er i overensstemmelse med de kjente, skadelige effektene av cannabis på balanse og korttids hukommelsesprosessering [Howlett, A.C. et al., International Union of Pharmacology XXVII, Pharmacol. Rev. 2002, 54, 161-202]. CB2blir uttrykt av leukocytter og dens modulering utløser ikke psykoaktive effekter, og videre er den ikke et signifikant mål for symptomhåndtering der de fleste effektene er CBi-medierte.
Selv om mange cannabinoide effekter er sentralt medierte ved reseptor i CNS (Howlett, A.C. et al., International Union of Pharmacology XXVII, Pharmacol. Rev. 2002, 54, 161-202], er det på det rene at perifere CB-reseptorer også spiller en viktig rolle, spesielt ved smerte og i gastrointestinaltraktus. For eksempel blir CBiogså uttrykt i perifere vev, slik som i dorsalrotganglier, perifere nerver og neuromuskulære terminaler, for derved å tillate neurotransmisjon og bli regulert på utsiden av CNS [Pertwee, R.G., Life Sei. 1999, 65, 597-605]. I overensstemmelse med dette kan terapeutisk aktivitet ved tilstander slik som de som involverer smerte [Fox, A. et al, Pain 2001, 92, 91-100] eller tarmhypermotilitet være lokalisert på ikke-CNS-seter. Opp til i dag har likevel forskning på det perifere cannabinoidsystemet blitt hindret av mangelen på farmakologiske midler som selektivt målsøker perifere reseptorer i stedet for reseptorene i CNS.
For å kunne eliminere skadelige psykoaktive effekter er det ønskelig å ekskludere cannabinoidagonister fra CNS. Det finnes to etablerte fremgangsmåter for CNS-eksklusjon av små molekylmidler. En fremgangsmåte involverer å ekskludere stoffer fra CNS ved å nøyaktig kontrollere deres fysiokjemiske egenskaper for å forhindre dem i å krysse blod-hjerne-barrieren (BBB). BBB er dannet av hjerne endotelceller, med tette intercellulære overganger og lite muligheter til å passere gjennom disse [Tamai, I. et al., J. Pharm. Sei. 2000, 89, 1371-1388]. Dermed må stoffer komme inn i hjernen enten ved passiv diffusjon over plasmamembraner eller ved hjelp av aktive transportmekanismer. BBB danner slik en effektiv barriere for mange perifert sirkulerende stoffer.
En alternativ fremgangsmåte for å ekskludere forbindelser fra hjernen er å inkorporere strukturelle egenskaper som gjør dem i stand til å bli aktivt pumpet over BBB. Et slikt eksempel er opioidagonisten loperamid, og selv om den er lipofil inneholder loperamid strukturelle egenskaper som gjenkjennes av p-glykoproteintransporteren (MDR1) som tillater den å bli aktivt pumpet over blod-hjerne-barrieren. [Wandel, C. et al., Anesthesiology 2002, 96, 913-920; Seelig, A. et al., Eur. J. Pharm. Sei. 2000, 12, 31-40].
Foreliggende oppfinnelse søker å tilveiebringe nye cannabinoidreseptormodulatorer. Mer spesielt søker oppfinnelsen å tilveiebringe cannabinoidreseptormodulatorer som lindrer og/eller eliminerer noen av ulempene som vanligvis er assosiert med modulatorer fra den kjente teknikken, for eksempel uønskede psykoaktive bivirkninger. Mer spesifikt søker oppfinnelsen å tilveiebringe modulatorer som selektivt målsøker perifere cannabinoid-reseptorer.

Denne siste forbindelsen induserte typiske cannabinhermende effekter mens forbindelse (16) var inaktiv ved 1 mg/kg (ovenfor) og til og med ved 20 mg/kg i.v. Figur 4 viser undersøkelsen av spastisitet hos CREAE-mus. Spastisitet utviklet seg etter utviklingen av kronisk EAE indusert ved injeksjon av museryggmargshomogenat i Freunds komplette adjuvans på dag 0 og 7. Dette foregikk 60-80 dager etter induksjon i villtype-ABH.CnrJ+/+-mus og 30-40 dager etter induksjon i CBl-reseptorgen ( Cnrl)-manglende ABH.C/7r.Z-/-mus) Spastisitet ble undersøkt ved hjelp av resistens mot fult utslag i baklemmer mot en strekklapp (Baker et al. 2000), før og etter behandling med enten den fulle CB!/CB2-agonisten CP55,940 eller full CB1/CB2/"CB3"-agonist injisert intraperitonealt eller forbindelse (16) injisert intravenøst i vehikkel (alkohol:kremofor:PBS (1:1:18)). Resultatene representerer den prosentvise endringen +, SEM fra baselinje (n< 10-12 per gruppe) 10 minutter etter administrering. Statistisk analyse ble utført og rådata ble analysert parvis fra baselinjen hvor (<*>P<0,001). De anti-spastiske effektene til CNS-penetrantagonister ble tapt hos CBi-manglende mus, noe som tyder på at
CB2/"CB3" ikke er et mål for anti-spatisk aktivitet. Vehikkel alene var inaktiv (Baker et al. 2000). Forbindelse (16) oppviste signifikant antispastisk aktivitet hos villtype mus og var aktiv når den ble administrert i PBS alene (ikke vist).

Til en løsning med 3-jodbenzosyre (10,02 g, 40,30 mmol) i tørr diklormetan ved romtemperatur (180 ml) under en nitrogenatmosfære ble EDCI (7,72 g, 40,30 mmol) tilsatt etterfulgt av trietylamin (8,0 ml, 60,45 mmol) og blandingen ble rørt ved romtemperatur i ytterligere 5 minutter. DL-alaninol (3,02 g, 40,3 mmol) ble deretter tilsatt og blandingen ble rørt ved romtemperatur i 16 timer. Reaksjonsblandingen ble vasket med en blanding av mettet saltløsning og mettet natriumbikarbonat (1:1, 2x150 ml) etterfulgt av mettet saltløsning (100 ml). De organiske fasene ble separert og tørket over magnesiumsulfat og løsemiddelet ble avdampet under vakuum. Fremgangsmåte A
[Tetrahedron 2000, 56, 4777-4792], bis(trifenylfosfin)palladium(II)klorid (3,5 mol%), kobber(I)jodid (8 mol%) og trietylamin (4 mmol) ble tilsatt til en løsning av N-(2-hydroksy-l-metyletyl)-3-jodbenzamid (1) (1 mmol) i DMF (5 ml). Blandingen ble rørt i 1 time under en nitrogenatmosfære ved romtemperatur. Alkynet (1 mmol) ble tilsatt og reaksjonsblandingen ble rørt ved 60°C i 2 timer. Fremgangsmåte B
[J. org. Chem. 1999, 64, 4777-4792, J. med. Chem. 1998, 41, 420-427] Tetrakis-(trifenylfosfin)palladium(O) (2 mol%) og kobber(I)jodid (7 mol%) ble tilsatt til pyrrolidin (15 ml) i en rundbunnet flaske og rørt ved romtemperatur under en nitrogenatmosfære i 5 minutter. Til denne løsningen ble N-(2-hydroksy-l-metyletyl)-3-jodbenzamid (1 mmol) tilsatt og rørt i ytterligere 15 minutter ved romtemperatur. Alkynet (1 mmol) ble tilsatt og reaksjonsblandingen ble rørt ved 60°C i 3 timer. Reaksjonsblandingen ble konsentrert
undervann, resten ble behandlet med DOWEX50 WX80 (10 x vekt av utgangsmateriale), DOWEX50 WX80 ble vasket med acetonitril (3 x 20 ml), deretter suspendert i en blanding av acetonitril/vann (3/1). Resten ovenfor ble løst i acetonitril/vann (1:1, 20 ml) og ble tilsatt til resinsuspensjonen og ristet i 20 minutter. Resinet ble filtrert av, vasket med acetonitril/vann (3/1) og løsemiddelet ble fjernet fra filtratet under vakuum. Syntese av amider
Fremgangsmåte C:
Til en løsning av alkynsyren (1 mmol) i tørr THF (6 ml) under nitrogenatmosfære ble trietylamin (2 mmol) tilsatt og deretter avkjølt til -10°C. Til reaksjonsblandingen ble etyolklorformat (1 mmol) tilsatt og deretter rørt i ytterligere 15 minutter ved -10°C. I mellomtiden ble en løsning av aminhydroklorid (3 mmol), vann (0,88 ml), trietylamin (0,63 ml, 6 mmol) og THF (1,76 ml) fremstilt og tilsatt dråpevis til reaksjonsblandingen. Reaksjonen ble hensatt for oppvarming opp til 5°C i løpet av 1,5 time og deretter rørt ved romtemperatur i ytterligere 30 minutter. Blandingen ble helt inn i en l:l-blanding med mettet saltløsning og mettet natriumbikarbonat (50 ml) og deretter ekstrahert med DCM (5 x 50 ml). Kinolin (143 ul, 1,3 mmol), palladium på bariumsulfat redusert (5 %) (143 mg) og alkynet (1 mmol) ble kombinert i metanol (14 ml) og rørt under atmosfærisk trykk av hydrogen inntil<1>H NMR i råmaterialet viste at reduksjonen var fullstendig. Katalysatoren ble fjernet ved filtrering gjennom en pute med kiselgur, som ble vasket flere ganger med metanol. Filtratet ble avdampet under vakuum og produktet ble renset ved hjelp av preparativ HPLC.
Fremgangsmåte E:

Generell fremgangsmåte for BER/ Ni- hvdroaenerina
Polymerstøttet borhydrid (borhydrid på amberlite IRA-400, 2,5 mmol BH4"/1 g resin)
(BER) (0,750 g) og nikkelacetat-tetrahydrat (0,046 g, 1,9 mmol) ble oppslemmet i 7 ml metanol, hydrogen ble boblet gjennom suspensjonen inntil et svart belegg av nikkel fremkom på resinet, deretter ble alkyn (1 mmol) løst i 7 ml metanol tilsatt til blandingen under hydrogen. Blandingen ble ristet i 9 timer og deretter filtrert. Resinet ble vasket flere ganger med metanol og deretter ble det kombinerte filtratet avdampet under vakuum. Resten ble løst i et passende løsemiddel og filtrert gjennom kiselgur for å fjerne nikkelet. Produktet ble renset ved hjelp av preparativ reversfase-HPLC-kromatografi.

Validering som CBl- aaonister med perifer virkning
In wtro- radioligand bindin<g>sstudier
Radioligandbindingsanalyse [Ross, R.A. et al., Br. J. Pharmacol. 1999, 128, 735-743] utføres med CB^reseptorantagonisten [3H]SR141716A (0,5 nM) eller [3H]CP55940 (0,5 nM) i hjerne- og miltmembraner. Analyser utføres i analysebuffer inneholdende 1 mg/ml BSA, der det totale analysevolumet er 500 ul. Binding initieres ved tilsettingen av membraner (100^g). Vehikkelkonsentrasjonen på 0,1 % DMSO holdes konstant gjennom hele forsøket. Analyser utføres ved 37°C i 60 minutter før avslutning ved tilsetning av iskald vaskebuffer (50 mM Tris-buffer, 1 mg/ml BSA) og vakuumfiltrering ved å benytte en 12-brønners prøvemanifold (Brandel Cell Harvester) og Whatman GF/B glassfiber filtre som hadde blitt fuktet i vaskebuffer ved 4°C i 24 timer. Hvert reaksjonsrør vaskes 5 ganger med et 4 ml alikvot med buffer. Filtrene ovnstørkes i 60 minutter og deretter plasseres i 5 ml scintillasjonsvæske (Ultima Gold XR, Packard) og radioaktivitet kvantifiseres ved hjelp av flytende scintillasjonsspektrometri. Spesifikk binding er definert som forskjellen mellom bindingen som forekom i nærvær og i fravær av 1 umerket ligand og er henholdsvis 71 % og 40 % av den totale radioligand bundet i hjerne og milt. Konsentrasjonene av konkurrerende ligander (testforbindelser) som frembringer 50 % fortrengning av radioliganden (IC50) fra spesifikke bindingsseter beregnes ved å benytte GraphPad Prism (GraphPad Software, San Diego). Inhiberingskonstant (Ki)-verdier beregnes ved å benytte ligningen til Cheng & Prusoff [Cheng, Y. og Prusoff, W.H., Biochem. Pharmacol. 1973, 22, 3099-3108].
In wtro- cannabinoidreseptormodulerinasaktivitet
Forbindelser evalueres for cannabinoid moduleringspotensial ved å benytte et muse-vas-deferens preparat (Ward S, Mastriani D, Casiano F og Arnold R (1990) J Pharmacol Exp Ther 255:1230-1239) som tilveiebringer bevis for CB-agonisme, heller en enkel reseptor-binding som ikke alltid reflekterer agonistpotensial. Forbindelse (16) viste signifikante effekter i dette systemet (Figur 1) med en IC50på~1 nM sammenlignet med den kjente fulle agonisten R(+) WIN55,212 (IC50~5 nM). Dette ble inhibert av den selektive CB!-antagonisten SR141716A, som indikerer at den observerte sammentrekningen ble mediert via den perifere CBi-reseptoren.

Øvre qastrointestinaltransitt
Gastrointestinaltransitt måles ved å benytte trekullfremgangsmåten. Mus mottar oralt 0,1 ml (10 g/mus) med en sort markør (10 % trekullsuspensjon i 5 % arabisk gummi) og etter 20 minutter avlives musene ved innånding av C02og tynntarmen fjernes. Avstand som markøren har vandret måles og uttrykkes som en prosentandel av den totale lengden på tynntarmen fra pylorus til caecum [Izzo, A.A. et al, Br. J. Pharmacol. 2000, 129, 1627-1632]. Cannabinoidagonister gis 30 min før trekulladministrering.
Kolonfremdriftstest
Distal kolonfremdrift måles i henhold til Pinto et al. [Gastroenterology 2002, 123, 227-234]. Tretti minutter etter administreringen av cannabinoide legemidler settes en enkelt 3 mm glasskule inn 2 cm inne i den distale kolon på hver mus. Tiden som er nødvendig for utstøtning av glasskulen ble bestemt for hvert dyr. Den høyere gjennomsnittlige utstøtningstidsverdien er en indeks på en sterkere inhibering av kolonfremdrift.
Psvkotropisk aktivitet for perifert aktive cannabinoider
Mange CBi-agonister er kjent for å indusere psykotropisk assosierte "tetrade-effekter" på grunn av sentral binding til CB-reseptorer [Howlett, A.C. et al., International Union of Pharmacology, XXVII, Pharmacol. Rev. 2002, 54, 161-202]. Undersøkelser ble gjennomført for å finne ut hvor vidt forbindelsene ifølge foreliggende oppfinnelse også binder til sentrale CBi-reseptorer. Dette undersøkes ved å måle evnen til forbindelsene i å indusere avslapning, ptose, hypomotilitet, katalepsi og hypotermi i normale mus [Brooks, J. W. et al., Eur. J. Pharmacol. 2002, 439, 83-92], etter i.v., i.p. og oral administrering.
Bestemmelse av nivåer av forbindelse i hjerne
Kvantitering av permeabilitet inn i hjerne oa rvaamara
Hjerne/ryggmargspenetrering av forbindelsene kan bli målt direkte som følger. Hjerne- og ryggmargsopptak i anesteserte rotter måles ved å benytte standard fremgangsmåter som fremlagt i Ohno et al. [Ohno, K. et al., Am. J. Physiol 1978, 235, H299-H307]. Kort fortalt blir forbindelsen injisert intravenøst (femoralt) som enten en enkel bolus eller trinnvis infusjon. Flere plasmaprøver (femoral arterie) tas for å beregne plasma-konsentrasjon overtid (integral, areal under kurven). Terminale hjerne- og ryggmargs-prøver tas for å måle hjernepenetrering (korreksjon for forbindelse i restblod ved enten fysiologisk saltvannsutvasking eller ved å måle innholdt blodvolum ved å benytte kort sirkulering for en inert lav permeabilitetsmarkør slik som [<14>C]sukrose). PS (cm.s-1) er likt med Chjerne/integral Cplasma, der PS = permeabilitet x overflateareal (cm<2>) produkt for hjerneopptak og C er konsentrasjon. Alternativt måles en steady-state-vevs-/plasmaratio som en mer omtrentlig indeks, igjen med blodutvasking eller korreksjon. Sammenligning gjøres med kontrollforbindelser som er kjent for å ha lav permeabilitet over BBB, for eksempel radiomerket sukrose eller insulin, testet ved identiske betingelser.
Preliminær karakterisering av biologien til CBi^ aaonisme
Nociceptiv aktivitet for perifert aktive cannabinoider
Det foreligger bevis på CBi-mediert nocicepsjon i periferien [Fox, A. et al., Pain 2001, 92, 91-100]. Undersøkelser på begrenset isjiasnerve ligering ble derfor gjennomført på rotter og på knockout-mus.
Undersøkelser av spastisitet
Ytterligere undersøkelser ble gjennomført ved å benytte cannabinoid-knockout-mus, inkludert CBi-, CB2-, VR-1-, FAAH- og kondisjonelle CBi-knockout-mus. Spastisitet kan bli indusert i ABH (signifikant spastisitet forekommer hos 50-60 % av dyr i 80 dager etter 3-4 sykdomsepisoder) eller ABH.CBi-/- (signifikant spastisitet forekommer hos 80-100 % av dyr i løpet av 30-40 dager etter 1-2 sykdomsepisoder). Forbindelser blir injisert først intra-venøst (for å begrense første passasjeeffekter), i.p. eller oralt. Spastisitet blir undersøkt (n=6-7/gruppe) ved resistens ovenfor baklem fleksjon ved å benytte en strekklapp [Baker, D. et al, Nature 2000, 404, 84-87]. Dyr virker som sine egne kontroller og vil bli analysert på en parvis måte. For å redusere antall dyr, innsats og kostnader, mottok disse dyrene etter en legemiddelfri periode (spastisitet returnerer i løpet av 24 timer) ulike doser og/eller vehikkel. Lave doser med CBi-agonister og CNS-aktiv CP55,940, som kontroll, blir lokalt (subkutan, intramuskulær) administrert til spastiske ABH-mus og mangelen på aktivitet i et kontralateralt lem blir analysert [Fox, A. et al., Pain 2001, 92, 91-100]. Ekspresjon av CBii det perifere nervesystemet, inkludert dorsalrotganglier, et ikke-CNS-sete for CB-mediert nocicepsjon kan fjernes ved å benytte periferin-Cre-transgene mus [Zhou, L. et al., FEBS Lett. 2002, 523, 68-72]. Disse betingede KO-musene opprettholdes på C57BL/6-bakgrunnen. Disse musene utvikler EAE etter induksjon med myelinoligodendrocytt glykoproteinresidiene 35-55 peptid [Amor, S. et al., J. Immunol. 1994, 153, 4349-4356].
In wVo- evaluerina i normale mus oa CREAE- mus
En CNS-ekskludert forbindelse tilveiebringer et redskap for å undersøke om en komponent av en cannabinoid-anti-spastisk effekt er mediert via perifere CB-reseptorer. Forbindelse (16) ble undersøkt for CNS-effekter i normale mus som vist i Figurene 2 og 3. Ved en dose på 1 mg/kg ble ingen hypotermi eller hypomotilitet observert. Hos CREAE-mus ble en markert effekt på spastisitet notert (Figur 4) og tilveiebringer sterkt bevis på at en selektiv inhibering av spastisitet er mulig å oppnå uten å frembringe CNS-effekter. Som påpekt ovenfor er det ingen etablert rolle for perifere, cannabinoid reseptorer i kontroll av spastisitet, likevel er spastisitet sannsynligvis et produkt av nerveskade i ryggmargen, i det minste i EAE [Baker, D. et al, FASEB J. 2001, 15, 300-302, Baker D. et al., J. Neuroimmunol. 1990, 28, 260-270] og avvikende signaler til og fra muskulatur bidrar i det minste delvis til muskelspasmene som forekommer ved spastisitet.
