Abstract
Det er beskrevet LH-RH peptidanaloger med utmerket affinitet for LH-RH reseptorer, med formel (1): Al -A2-A3-A4-A5-A6-HAA-A7-Pro-Z hvor Z, Al til A7 er spesifisert i beskrivelsen og HAA er en ikke-aromatisk hydrofob aminosyre med fra 7 til 20 karbonatomer. Det er videre beskrevet anvendelser av nevnte peptidanaloger og farmasøytiske blandinger som inneholdende disse.
Description
Foreliggende oppfinnelse vedrører LH-RH peptidanaloger, peptidagonist eller antagonist av LH-RH samt anvendelser derav og farmasøytiske blandinger inneholdende disse.
LH-RH eller luteiniserende hormon-frigjørende hormon, er et neurohumoralt hormon produsert i hypothalamus som stimulerer sekresjon av gonadotrophiner, LH (luteiniserende hormon) og FSH (foltikkel-stimulerende hormon), som igjen regulerer endokrine og eksokrine funksjoner i eggstokkene til kvinner og testiklene til menn. Det har følgende strukturelle formel:
E. Hazum et al., FEBS Letters, Vol. 123, no. 2,1981, s. 300-302 beskriver LH-RH analoger modifisert i posisjon 7 med cykloheksylalanin, cykloleucin og fenylalanin.
Det demonstreres deri at erstatning av Leu<7> med cykloheksylalanin (Cha) i LH-RH resulterer i en reduksjon i biologisk aktivitet ned til 66% av gjenværende aktivitet i forhold til LH-RH satt til 100%, til tross for en noe høyere motstand mot enzymatisk degradering.
DE 2 424 287 vedrører LH-RH eller FSH-RH peptider. Det er beskrevet deri at L-aminosyrer med beskyttede hydroksyl, karboksyl, eller aminogrupper: SertBu<1>), Thr(Bu<l>), Asp(OBu*), Glu(OBul), Orn(Boc) eller Lys(Boc) forøkte med minst 2-ganger biopotensen til LH-RH og dets mere potente agonist analoger. Dette ble derimot ikke understøttet ved biologiske data.
WO 91/05563 beskriver terapeutiske LH-RH-dekapeptider med substitusjoner spesielt i posisjonene 3 og 7. Det ble angitt at p-X-Phe i posisjon A7 er spesielt fordelaktig med hensyn på aktivitet.
Historisk (Karten and Rivter, Endocr. Rev., 1986, 7(1), 44-66), er syntetisk forbedring av LH-RH aktiviteten blitt oppnådd først, ved erstatning av C-terminalt glycinamid med et etylamid direkte bundet til Pro<9> og deretter, ved innføring av D-ala i stilling 6. Begge uavhengige gjennombrudd ga analoger som hver var ca.
5 ganger mer aktiv enn LH-RH. Alle terapeutisk nyttige agonister er et resultat av ytterligere vesentlig forbedring i stilling 6 med innføring av hydrofobe alifatisk eller aromatisk D-aminosyrer istedenfor D-ala, med eller uten samlet Pro<9->N-etylamid modifikasjon. På denne C-terminale enden, er det bare oppnådd små forbedringer med fluorerte amider eller med azaglycinamid. Erstatning av Trp i stilling 3 med 1Nal er rapportert (Karten og Rivier, 1986, cf ovenfor), hvilket gir en agonist to ganger så kraftig som LH-RH, uten ytterligere syntetiske eller terapeutiske utviklinger. Eneste andre individuelle aminosyremodifikasjon oppdaget for å øke den biologiske aktiviteten til noen agonister, ble funnet i stilling 7. Således, økte ikke kun N-metylering av Leu<7> i LH-RH dets potens, men økte aktiviteten til noen allerede kraftige syntetiske agonister med visse D-aminosyrer i stilling 6 så som D-Trp (Karten og Rivier, 1986, cf ovenfor); videre, ladete og mer omfangsrike L-aminosyrer enn leucin (SerfOBu<4>), Asp(O-but), Glu(O-but), BocLys) forbedret aktiviteten til [des-Gly<10>; Pro<9->N-etylamid]-LH-RH noe, men redusert styrken av 6-modifiserte agonister (Karten og Rivier, 1986, cf ovenfor).
Når det gjelder antagonister, er det blitt forsøkt en rekke modifikasjoner i alle stillinger untatt Pro<9> , og en rekke kombinasjoner blant dem, er blitt forsøkt med varierende hell for å oppnå hemning av endogen LH-RH aktivitet (Dutta, Drugs of the Future, 1988,13(8), 761-787 ; Karten and Rivier, Endoc. Rev., 1986, 7(1), 44-66). For eksempel antid, en standard kraftig LH-RH antagonist, er et resultat av aminosyre- endringer i stillingene 1, 2, 3, 5, 6, 8 og 10. N-metylering av Leu<7> førte til en reduksjon i potens og eneste endringer i denne stilling raportert for å øke den (2-ganger maksimum) ble erstatning av Leu<7> med Trp<7> eller Phe<7>.
Det er nå funnet at erstatning av Leu<7> med sterkt hydrofobe aminosyrer, øker aktiviteten til LH-RH eller til kjente meget aktive analoger (agonister eller antagonister) av LH-RH.
Spesielt er det funnet at erstatning av Leu<7> med adamantylalanin (Ada) eller neopentylglycin (Npg) øker aktiviteten til LH-RH og gjør det mulig å oppnå analoger med en høy affinitet for LH-RH reseptorer. Mer spesifikt, er [Npg<7>]-LH-RH analoger ifølge foreliggende oppfinnelse kraftige LH-RH agonister/antagonister in vivo.

Det er videre beskrevet peptidagonist eller antagonist av LH-RH ifølge krav 1, kjennetegnet ved at den har formel:

Eksempler på saltene med farmasøytisk godtagbare syrer er de med mineralsyrer, så som for eksempel hydrokloridet, hydrobromid, sulfat, fosfat, borat, hydrogensulfat, dihydrogenfosfat eller nitrat og de med organiske syrer, så som for eksempel acetat, oksalat, tartrat, succinat, maleat, fumarat, glukonat, citrat, pamoat, malat, ascorbat, benzoat, p-toluensulfonat eller naphtalenesulfonat.
Eksempler på saltene med farmasøytisk godtagbare baser er de med alkali eller jordalkalimetaller så som natrium, kalium, kalsium eller magnesium og de med organiske baser så som aminer, trometamol, N-metylglutamin og lignende.
Peptidene ifølge foreliggende oppfinnelse kan fremstilles ved velkjente teknikker innen peptidkjemi så som for eksempel peptid syntese i løsning eller faststoff-fase peptidsyntese. Generelt, involverer disse teknikker trinnvis tilsetning av én eller flere aminosyrer - som kan være hensiktsmessig beskyttet - til en peptidkjede som er i ferd med å bli dannet.
Fortrinnsvis blir peptidene ifølge oppfinnelsen syntetisert ved anvendelse av trinnvis faststoff- fase syntese (1,2) med N-oc-Fmoc beskyttelse. For eksempel blir peptidene bragt på en 4-metylbenzylhydrylaminharpiks (Peninsula Laboratories, UK) eller på en aminometyl-harpiks (Peninsula Laboratories, UK). C-terminal prolin blir innført som 4-(Boc-Prolyloksymetyl)fenyl. Påfølgende fjernelse av Boe beskyttelsesgruppen blir oppnådd med trifluoreddiksyre fulgt av diklormetan og dimetylformamid (DMF) vasking så vel som diisopropyletylamin nøytralisering. Det er også mulig å anvende en "Rink" harpiks (4-{2\4'-dimetoksyfenyl)-Fmoc-amino-metylfenoksyharpiks) ved anvendelse av Fmoc syntesestrategi (2).
Syntesen omfatter oppstillings-, spaltnings- og rensningstrinn, som beskrevet nedenfor:
I. Oppstilling
For alle peptidene blir de følgende avbeskyttelses/koblings prodedyrene anvendt:
1 • DMF vasking (3 ganger -1 min.)
2 - Piperidin 25 % i DMF (1 min.)
3 - Piperidin 25 % i DMF (to ganger -15 min.)
4 - DMF vasking (7 ganger -1 min.)
For hvert trinn blir 15 ml oppløsningsmiddel pr. gram peptidharpiks anvendt.
Kobling av alle aminosyrer (tre ganger overskudd av) blir utført i DMF i nærvær av BOP, Hobt og DIEA (3). Hvert koblingstrinn blir kontrollert for fullførelse med ninhydrintest (4) og dobbelkobling blir utført hvis nødvendig. Hvis den etter den andre koblingstesten fortsatt er positiv, blir harpiksen acetylert (eddiksyre-anhydrid, 10 ganger overskudd og DIEA).
Generelt, blir en trifluoreddiksyre (TFA) behandling utført før avbeskyttelses/ spaltningstrinn.
II. Spaltning
Peptidene blir spaltet fra harpiksen og fullstendig avbeskyttet med en behandling med enten flytende hydrogenfluorid (HF) eller TFA. 10 ml HF eller TFA pr. gram peptidharpiks blir anvendt vanligvis ved 0° C i 45 min. eller 2,5 timer, i nærvær av p-cresol og etanditiol (for tryptofan-inneholdende peptider) som oppfangere.
Etter inndampning av HF, blir rå reaksjonsblanding vasket med dietyleter, oppløst i TFA, utfelt med dietyleter og tørket under redusert trykk: Om nødvendig, blir peptidet spaltet fra harpiksen før avbeskyttelse og deretter amidert med en behandling med etylamin (5 ml etylamin pr. gram petpidharpiks, - 78°C, 20 timer).
Når en benzylgruppe er til stede i sluttproduktet, blir TFA anvendt (10 ml pr. gram peptidharpiks, 0° C, 2,5 timer) ved den endelige spaltning/avbeskyttelse.
Preparatet fra TFA spaltningsblandingen i v % er som følger:
Etter filtrering av harpiksen, blir peptidet utfelt fra reaksjonsblandingen ved tilsetning av en stor mengde dietyleter. Etter mange vaskinger med dietyleter blir rå peptid tørket under redusert trykk.
III. Rensning
Alle peptidene blir renset ved revers-fase- væskekromatografi.
Generell prosedyre for rensning er identisk for hver peptid ; imidlertid blir gradienten til det organiske oppløsningsmiddelet regulert avhengig av den opprinnelige retensjonstiden til peptidet.

Alle fraksjoner inneholdende måtforbindelse blir individuelt analysert ved analytisk HPLC. Fraksjonene med en renhet høyer eenn 95 % blir samlet og fryse-tørket. Dersom ønsket renhet ikke blir nådd etter det første rensningstrinnet, blir et andre rensningstrinn og, hvis nødvendig, et tredje rensningstrinn utført. Betingelsene for rensning i andre og tredje trinn er lignende som de beskrevet ovenfor bortsett fra at gradientstigningen blir modifisert for å øke spaltningen.
Etter lyofilisering er alle rensede peptider til stede som deres trifluoracetat salt. Det endelige pulveret som tilsvarer hvert peptid blir kontrollert med analytisk HPLC. Strukturen til hver forbindelse blir også bedømt med massespektralanalyse og netto peptidinnhold blir bestemt ved UV absorpsjon.
Peptidene ifølge foreliggende oppfinnelse har en kraftig affinitet for LH-RH reseptorer.
Denne affiniteten er blitt bestemt i henhold til følgende metode:
Hypofyser fra hunn Sprague Dawley rotter ble fjernet og homogenisert med en Potter homogenisator i en 25 mM HEPES buffer (pH 7,4) inneholdende 0,32 M sukrose, 100 pg/l PMSF (fenylmetylsulfonylftuorid), 5,6 U/l aprotinin og 10 000 U/l bacitracin. Homogenateter ble sentrifugert ved 700 g i 10 minutter og superna-tantene ble ytterligere sentrifugert ved 12,500 g i 30 minutter. Peiletene ble homogenisert og sentrifugert som beskrevet ovenfor, men i samme buffer uten
sukrose.
Alle homogeniserings-, sentrifugerings- og påfølgende inkuberingstrinn ble
utført ved 4° C.
Alikvoter av membran fraksjoner ble inkubert i 2 timer i duplicat med økende konsentrasjoner av testforbindelser i nærvær av 20 til 70 pM [<12>5l]-buserelin (mellom 1000 og 2000 Ci/mmol avhengig av ligandbatcher). Forsøket ble avsluttet ved filtrering under sug (Brandel 96-brønn-høster) gjennom Whatman GF/B glass-fiberfiltere. Etter gjentatte vaskinger ble filterene plassert i tellebeholdere med scintillasjons-cocktail for å måle radioaktiviteten til 125l. For hvert forsøk ga kurve-tilpassning av gjenværende spesifikk binding mot konsentrasjoner av testforbindelse 50 % hemmende konsentrasjon (IC50). Hver forbindelse ble testet i minst 4 forsøk.
Dette LH-RH reseptorforsøket ble karakterisert ved 4 metningsforsøk ved anvendelse av økende konsentrasjon [<125>l]-buserelin i fravær eller tilstede-værelse av 1 pM umerket buserelin for uspesifikke bindingsbestemmelser. Spesifikke bindingsdata ble analysert i henhold til Scatchard's metode. Ved like-vekt (2 timer inkubering) var dissosiasjonskonstanten (Kd) og antall bindingseter for [125l]-buserelin henholdsvis lik 88 ± 6 pM og 15,6 ± 2,9 pM.
For hver testforbindelse, ble hemmende konstant (Ki) beregnet fra dets IC50
i henhold til Cheng og Prussofs ligning: Ki = IC50AI + [radioligand]/Kd). Ki ble deretter transformed inni pKi (= - log Ki) for endelig ekspresjon av affinitets-skalaene.
Den naturlige ligand, LH-RH, utviser en sterk affinitet med eksperimentell IC50 i 10 nM område, dvs. en pKi lik ca. 8.
Såkalte superagonister som buserelin, leuprorelin, tryptorelin, histrelin eller deslorelin og antagonister som antid viser en til og med sterkere binding til LH-RH reseptorer med IC50 i det subnanomolare området, dvs. pKi > 9.
Affiniteten til testpeptider ifølge foreliggende oppfinnelse for LH-RH reseptorer er gitt i Tabell 1 nedenfor:
Peptidene i henhold til generelle formel (Ila) utviser en agonist aktivitet på LH-RH reseptorer in vivo, hvilket resulterer i at hypofysen stimulerer LH sekresjon, som, i menn, fører til at testiklene stimulerer sekresjon av testosteron.
Voksne hann Sprague-Dawley rotter mottok en subkutan injeksjon av forskjellige doser LH-RH, tryptorelin eller leuprorelin eller deres respektive mot-parter idet Npg<7>erstatter Leu<7>: eksempel 1 ([Npg<7>]-LH-RH), eksempel 6 ([Npg<7>]-leuprorelin) eller eksempel 11 ([Npg<7>]-tryptorelin), oppløst i fosfat-bufret saltvann (PBS). To timer senere, ble blodprøver tatt for total plasmatestosteron bestemmelse med direkte radioimmunoanalyse (Immunotech). Eksempel 1 var noe mer enn to ganger så aktiv som LH-RH og de andre forbindelsene oppførte seg som såkalte « superagonister» ved indusering av en sterkere stimulering av testosteron sekresjon ved mye lavere doser (logaritmisk x akse) enn LH-RH (Figur 1; 8 dyr pr.punkt). Ved 20 ng/kg, var sekresjonen av testosteron likeledes maksimalt stimulert av de fire superagonistene (eksponensiell y akse skala), men ved 10 ng/kg var eksemplene 6 og 11 litt mer enn to ganger så aktiv som leuprorelin og tryptorelin.
Ved denne mellom-dosen på 10 ng/kg, ble mange eksempler med Npg<7> eller Ada<7> istedenfor Leu<7> ifølge oppfinnelsen screenet for agonist aktivitet (Tabellene 2 og 3). Når tilgjengelig fra Bachem (France) eller Sigma (France), ble tilsvarende standard agonist med Leu<7> testet for sammenligning. I alle seks tilfeller, var både Npg<7-> og Ada<7-> modifisert eksempler mer aktiv, med en liten eller vid margin avhengig av Leu<7> motpart forbindelsen (Tabell 2). I to tilfeller, var Ada<7 >erstatningen mer fordelaktig enn Npg<7> (eksemplene 27 og 28). I motsetning til dette førte Npg<7> til mer kraftige agonister enn Ada<7> i to andre strukturer (eksemplene 6 og 11).
Funnene illustrerer at økning av den totale hydrofobisiteten til aminosyren
i stilling 7 til LH-RH analoger er en generell metode for å oppnå en større potens in vivo. Avhengig av resten av molekylet, kan andre karakteristika til sidekjeden i stilling 7, så som sterisk hindring, modulere den resulterende økningen i aktivitet.
En økning i hydrofobisitet, som eksemplifisert med Npg<7>, var kompatibel med mange endringer i stillingene 3 og 6, hvilket gir mange agonister i et område med potens som ligner den til leuprorelin eller tryptorelin (eksemplene 5,9,13,20, 21,22 eller 25 ; Tabellene 2 og 3). N-metylering av Npg<7> var også kompatibel med en meget sterk agonistisk aktivitet (eksempel 23; Tabell 3).
Det kan konkluderes med at eksemplene 6 og 23 på den ene siden og eksempel 28 på den andre siden, er de beste eksempler i dag på mere kraftige LH-RH agonister enn eksisterende terapeutiske LH-RH analoger, oppnådd ved økning av hydrofobisiteten til aminosyren i stilling 7, henholdsvis med Npg eller Ada.
Peptidene i henhold til generelle formel (Mb) utøver en antagonistisk aktivitet på LH-RH reseptorer in vivo, hvilket resulterer i hemning av ovulasjon i hunkjønn.
Voksne hunn Wistar rotter blir først overvåket for normal østrogen-syklisitet med daglige vaginale utstryk. Etter minst 2 regulære 4-dag sykluser, mottok de ved subkutan injeksjon enten konstituenten alene (0,5 ml av en blanding av propylen-glykol og vann : 20/80 vol/vol) eller LH-RH antagonist i henhold til formelen (Hb) oppløst i denne bæreren, rundt klokken 14.00 på dagen med proestrus. Alle untatt ett bærer-behandlet dyr ovuleret spontant, som demonstrert ved gjenvinning av en rekke ovocytter i oviductene den følgende morgen.
Når effektive, blokkere LH-RH antagonister fullstendig ovulasjonen. Antid, en kommersielt tilgjengelig standard LH-RH antagonist (fra Bachem, France) viste en dose-beslektet hemning av ovulajonen (Tabell 4). Semi-logaritmisk regresjons-analyse ga en 50 % hemmende dose (IDS0) lik 0,99 pg/rotte. Når Leu7 ble erstattet med Npg7 i strukturen til antid (eksempel 15), ble den hemmende potensen markert øket ved 0,5 og 0,25 ug/rat,, hvilket resulterer i en ID50 på 0,26 ug/rat. Variasjoner i basiske aminosyrer i stillingene 6 og 8 var kompatibel med Npg<7>, hvilket gir antagonister som er mere kraftige enn antid, som vist med maksimal eller sub-maksimal aktivitet i eksemplene 16,17,18 eller 19 ved dosen 1 ug/rotte (Tabell 4). Eksempel 17 var spesielt aktiv, men dets effekter var ikke beslektet med dosen i doseområdet studert. Derfor er innføring av en hydrofob aminosyre i stilling 7 fordelaktig for å oppnå sterkere LH-RH antagonistiske egenskaper, som best eksemplifisert med 4-ganger økning i antiovulatorisk potens til antid ved erstatning av Leu<7> med Npg<7> (eksempel 15).
Som konklusjon i både agonist og antagonist studier in vivo, er det blitt vist at erstatning av Leu<7> med en mer hydrofob ikke-aromatisk aminosyre, så som Npg eller Ada, økte systematisk potensen til eksisterende analoger. Nært beslektede analoger med en hydrofob aminosyre i posisjon 7 uten direkte Leu<7> motpart for sammenligning, utviste ofte interessante nivåer av aktivitet perse.
Anvendelse av Npg, Ada eller én hvilken som helst annen hydrofob aminosyre i posisjon 7 til en LH-RH analogsekvens som tilsvarer definisjonen ifølge generell formel (I) blir krevd et generelt trekk for å oppnå nye LH-RH agonister eller antagonister med høy eller forsterket potens in vivo.
Ingen tegn på toksisitet er observert med peptidene ifølge oppfinnelsen ved farmasøytisk aktive doser.
Peptidene ifølge oppfinnelsen og deres farmasøytisk akseptable salter kan bli anvendt for behandling eller forhindring av forskjellige plager eller sykdommer hvor en LH-RH agonist- eller antagonist-aktivitet er nødvendig.
Hovedmålet for LH-RH analoger er på hypofysekjertelen, men direkte virkninger er blitt rapportert på selve gonadene (testikler og ovarier), på tymus og noen lymfoide cellelinjer, på mastceller og på bryst-, prostata- eller bukspyttkjertel-tumorer.
LH-RH agonister i følge formel (Ila) utviser på et hvilken som helst LH-RH sensitivt mål, enten en stimulatorisk aktivitet ved kort-tids akutt eller pulserende administreringer, eller en inhibitorisk effekt med gjentatt eller kontinuerlig administreringer som induserer desensibiliseringen og nedreguleringen av LH-RH reseptorene. Når det gjelder hypotalamo-hypofyse-gonadalaksen resulterer forlenget administrering til en såkalt "kjemisk" kastrering.
LH-RH antagonister ifølge formel (IIb) utviser primært en inhibitorisk effekt på et hvilket som helst LH-RH sensitivt mål, men er også nyttig for oppnåelse eller planlegging av en tilbakeslags-stimulatorisk frigjøring av LH og FSH når behandlingen blir avsluttet.
På grunn av dette ambivalente potensiale til både LH-RH agonister og antagonister kan alle analoger ifølge formel (I) finne en hensiktsmessig terapeutisk anvendelse i mennesker samt i dyr, avhengig av doser, behandlings-regimer og administreringsveier, informeringsendokrinologi og ved behandling av kjønnshormon-avhengige benigne eller maligne tumorer, alene eller i kombinasjon med andre hormonelle eller anti-tumorale midler. LH-RH sensitive kjønnshormon-avhengig benigne eller maligne tumorer kan også gå tilbake ved behandling med LH-RH analoger ifølge formel (I), alene eller i kombinasjon med anti-tumorale midler. Immunmekanismer kan også bli modifisert av LH-RH analoger ifølge formel (I), alene eller i kombinasjon med immunmodulerende eller -sensitive midler så som glukokotrikoider, cyklosporin, rapamycin, takrolimus, derivater derav og lignende. LH-RH analoger ifølge oppfinnelsen er derfor meget nyttige for behandling og forhindring av autoimmune sykdommer, podningsavstøtning eller atopiske sykdommer, og for behandling av benigne eller maligne lymfoproliferative forstyrrelser.
LH-RH analoger ifølge formel (I) er spesielt nyttige, alene eller i kombinasjon med kjønns-steroider eller gonadotrofiner, ved inhibisjon, planlegging og aktivering av ovulasjon ved in vitro fertiliseringsprogrammer, og ved behandling av han- og hun-kjønnsinfertilitet eller hypogonadiske tilstander. De kan derimot også bli anvendt for befruktningshindring av menn og kvinner eller behandling av hypergonadiske tilstander, alene eller i kombinasjon med kjønns-steroider eller gonadotrofiner. Dette gjelder for menn og kvinner, men også ville dyr og husdyr ved anvendelser så som forbedring eller kontrollering av befruktning, eller som et redskap for å optimalisere avlingsstrategier.
LH-RH analoger ifølge formel (I) er også spesielt nyttige i menn for å behandle langkommet prostata-kreft, men kan også bli anvendt som en første linje terapi i denne indikasjonen og ved benign prostata-hypertrofi, alene eller i kombinasjon med inhibitorer av androgen virkning, dvs. anti-androgener så som kyproteronacetat, osateronacetat, klormadinonacetat, flutamid, nilutamid eller bikalutamid og lignende eller 5a-reduktase inhibitorer så som finasterid, epristerid eller turosterid og lignende eller C17-20 lyase-inhibitorer så som abirateron og lignende.
LH-RH analoger i henhold til formel (I) er også spesielt nyttige for behandling eller forhindring av brystkreft hos kvinner og menn, spesielt østrogen reseptor positive tumorer, alene eller i kombinasjon med antiøstrogener så som tamoxifen, raloxifen eller droloxifen og lignende, eller med aromatase inhibitorer så som atamestan, formestan, letrozol, anastrozol og lignende eller med C17-20 lyase-inhibitorer så som abirateron og lignende, men også visse østrogen reseptor negative tumorer så som reagerer på direkte effekter til LH-RH analoger eller indirekte på deres gonadal-suppressive aktivitet.
Andre gynekologiske tilstander, så som endometrial hyperplasi, leiomyom, adenomyom, endometriose, polycystisk ovarie syndrom, hirsutism og benign brystsykdom (smerte, cyster eller fibrose), kan også bli forhindret eller dra nytte av behandling med LH-RH analoger ifølge formel (I), alene eller i kombinasjon med antiøstrogener (sitert ovenfor), progestiner så som kyproteronacetat, osateronacetat, klormadinonacetat, nomegestrolacetat, promegeston, demegeston, trimegeston og lignende, eller deres befruktningshindrende middel eller post-menopausale erstatningskombinasjonsformuleringer med østrogener så som østradiol eller etynyløstradiol. Peptidene ifølge oppfinnelsen kan også interfere med gestasjon ved å indusere abort eller ved å aktivere fødselsprosessen, alene eller i kombinasjon med østrogener (sitert ovenfor), antiprogestiner så som mifepriston- eller prostaglandin-analoger så som sulproston.
Lignende indikasjoner kan oppstå innen veterinærmedisin for hun- og han-dyr for både husdyr og ville dyr som kan trenge anvendelse av LH-RH analoger ifølge formel (I).
Et annet aspekt ifølgé foreliggende oppfinnelse er derfor farmasøytiske blandinger inneholdende en effektiv mengde av ett peptid ifølge formel (I) eller et farmasøytisk akseptabelt salt derav.
Et ytterligere aspekt ved foreliggende oppfinnelse vedrører anvendelse av et peptid ifølge et av kravene 1 til 16 for fremstilling av et medikament for behandling av infertilitet, hypogonadiske eller hypergonadiske tilstander, hvori nevnte peptid biir anvendt alene eller i kombinasjon med et kjønns-steroid eller et gonadotrofin.
Et ytterligere aspekt ved foreliggende oppfinnelse vedrører anvendelse av et peptid ifølge et av kravene 1 til 16 for fremstilling av et befruktningshindrende middel, hvori nevnte peptid blir anvendt alene eller i kombinasjon med et kjønns-steroid eller et gonadotrofin.
Peptider ifølge oppfinnelsen blir fortrinnsvis administrert ved parenteral administrering, til tross for at orale formuleringer også er effektive forutsatt at doseringen blir hensiktsmessig øket.
Foretrukne leveringssystemer for LH-RH agonister ifølge formel (Ila) ved langtids hypofyse-gonadale suppressive indikasjoner er implanterbare innretninger med sakte frigjøring, eller injiserbare bionedbrytbare polymere mikro- eller nano-partikler eller -kapsler, eller mikro- eller nano-emulsjoner, med enhetsdoser av peptider eller av deres hensiktsmessige salter som varierer fra 1 mg til 100 mg pr. human pasient med en virkningsvarighet som varierer fra 1 måned til 1 år. Langtidsadministrering av LH-RH antagonister ifølge formel (Hb) vil generelt kreve høyere doseringer i samme sakte-frigjørende formuleringer, varierende fra 10 mg til 1 g i 1 uke til 1 år med aktivitet. Dyredoser vil bli tilpasset på grunnlag av kroppsvekt avhengig av ville etler husdyr som skal bli behandlet enten med LH-RH agonister eller antagonister ifølge formel (I).
Alle andre metoder for parenteral administrering.er egnet for øyeblikkelig, forsinket eller planlagt levering av peptidene ifølge oppfinnelsen: subkutane, intramuskulære, intravenøse, intragonadale eller intratumorale nål-bolus-injeksjoner, eller forlengede kontinuerlig, pulserende eller planlagte perfusjoner eller mikroinfusjoner ved anvendelse av hensiktsmessige pumpteknologi; gass-drivmiddel subkutane mikroinjeksjoner; vaginale kremer, geler eller pessarer; rektal-innretninger eller suppositorier; transdermale kremer, geler, vann, løsninger, plastre eller iontoforetiske innretninger; nasal spray eller tørr-pulver inhalerings-innretning; oftalmiske løsninger, geler, kremer eller kontaktlinser; lungeinhalering av mikro- eller nano-partikter eller dråper dannet manuelt eller med en hensikts- . messig pulveriserings- eller forstøvningsinnretning.
Enhetsdosen av disse parenterale administreringer vil variere i mennesker fra 0,001 mg til 10 mg/dag for LH-RH agonister ifølge formel (Ila) eller fra 0,01 til 100 mg/dag for LH-RH antagonister ifølge formel (Hb), 1 til 16 ganger pr. dag (når det gjelder pulserende administrering).
Oral administrering av peptidene ifølge oppfinnelsen blir fortrinnsvis oppnådd ved anvendelse av gastro-resistente og forsinkede enteriske eller karm-frigjørings-formuleringer som kan være belagte piller eller tabletter inneholdende to eller flere bestanddeler, harde gelatinkapsler, spesielle polymeriske makro-, mikro- eller nano-kuler som inneholder disse, eller en hvilken som helst innretning konstruert for å bli beskyttet fra mave/tarmnedbrytning og frigjøre disse når dette er nødvendig. Effektiv oral behandling kan bli oppnådd med hvilken som helst av oven-
nevnte formuleringer med enhetsdoser av peptider ifølge formel (1) varierende fra 1 mg til 1 g pr. human pasient, fra 1 til 16 ganger pr. dag (når det gjelder pulserende administrering).
Alle ovennevnte orale eller parenterale formuleringer av peptidene ifølge oppfinnelsen og deres farmasøytiske akseptable salter kan inneholde én eller flere farmasøytisk hensiktsmessige eksiptenter, én eller flere inhibitorer av proteaser, og én eller flere absorpsjonsfremmende midler etter behov ifølge spesifikke administreringsveier.
Rått pulver av rene peptider ifølge oppfinnelsen ellar farmasøytisk

1. LH-RH-peptidanalog, karakterisert ved at den har formel (SEQ IDN°:1):
