Description
O ppfinnelsens område
Foreliggende oppfinnelse angår en fremgangsmåte for forbedring av in vivo-funksjonen av rekombinant faktor VIII SQ ved å skjerme utsatte mål på polypeptidet. Oppfinnelsen er spesielt fordelaktig for konjugater hvor polypeptidet er faktor VIII med en høy spesifikk aktivitet, og hvor monometoksypolyalkylenoksid (mPEG) anvendes som den biokompatible polymer.
O ppfinnelsens bakgrunn
Det er velkjent at stabiliteten in vitro og halveringstiden in vivo av polypeptider kan økes ved kovalent binding av biokompatible polymerer (i det etterfølgende referert til som konjugasjon eller modifikasjon). Modifikasjon av polypeptidoverflaten har også fordelen ved å redusere immunogeniteten som oppvises av polypeptidet.
Pegylering, dvs. kobling av forskjellige polyetylenglykoler (PEG) til et polypeptid, er en teknikk som bredt anvendes for å øke in v/fro-stabiliteten og in vivo-halveringstiden av f.eks. proteiner. For pegylering er det foreslått mange teknikker i løpet av årene. Det refereres her til Zalipsky, S. et al., i poly(etylenglykol) Chemistry, Biotechnical and Biomedical Applications, Plenum, New York (1992), og Katre N.V., Adv. Drug Deliv. Rev., 10,91-114 (1993).
For noen polypeptider er et tap av aktivitet eller funksjon erkjent som en konsekvens av denne konjugasjon, en effekt som øker med modifikasjonsgrad (Inada, Y. et al., Trends in Biotechnology, 13, 86-91 (1995)). Det er utviklet metoder for å gjøre kobling mer selektiv for å omgå dette problem. Setedirigert mutagenese er f.eks. blitt anvendt for rekombinant interleukin-2 (rIL-2). En spesifikk målgruppe kan dannes ved innføyelse av et cystein (Goodson, R. J. og Katre, N. V. Bio/Technology 8, 344-346 (1990); Katre 1993, se ovenfor). En slik reaksjonsvei er ikke generelt anvendelig for terapeutiske proteiner, fordi aminosyresubstitusjon kan endre de opprinnelige karakteristika for molekylet, og er derfor kontroversiell. U.S. patentskrift nr. 4 904 584 angir polypeptider hvori aminosyresekvensen er forandret ved delesjon av en eller flere lysinrester for setespesifikk kovalent modifisering med f.eks. polyalkylenglykol. Alternativt kan polymeren dirigeres mot glykosyleringsseter på et protein, f.eks. faktor IX, som beskrevet i WO 94/29370 (Enzon). Dette involverer imidlertid oksidasjon av karbohydratkomponentene som kan oppnås ved å reagere glykoproteinet med natriumperjodat, eller enzymatisk ved hjelp av galaktoseoksidase. Disse betingelser er ofte ødeleggende for molekyler. I dette spesielle tilfelle var glykosyleringssetene som skulle konjugeres lokalisert i en faktor IX-peptidsekvens som fjernes under proteolytisk aktivering in vivo. Den biokompatible polymer påvirker således ikke funksjonen av det aktive polypeptid.
I WO 94/13322 og EP 0675736 (Farmitalia Carlo Erba) er det vist at pegylering kan utføres uten å svekke funksjonen av visse seter som er essensielle for funksjonen av det spesielle protein ("første stoff<1>). Dette oppnås ved beskyttelse av disse setene ved å bringe det første stoff i kontakt med et andre stoff som spesifikt bindes til disse komponenter. Nærmere bestemt utføres pegyleringen ved å immobilisere det spesielle protein på en resin med ligander med spesifikk affinitet for proteinet. Andre stoffer er f.eks. komplementære biologiske molekyler. Eksempler på par beskrevet i WO 94/13322 er antistoff (første stoff) - tilsvarende antigen (andre stoff); spesifikk inhibitor (første stoff) - enzym (andre stoff); vekstfaktor (første stoff)
- tilsvarende reseptor (andre stoff), eller de omvendte av hvert av disse par.
I en prosess ment for farmasøytisk produksjon er det imidlertid fordelaktig dersom anvendelse av stoffer med biologisk kompleksitet kan holdes på et minimum. Dette skyldes primært de strenge fordringer for dokumentasjon av biokjemisk homogenitet og sikkerhet for anvendelse av disse stoffer. Anvendelse av affinitetsligander produsert ved hjelp av organisk-kjemisk syntese ville derfor være fordelaktig.
DE 3717210 angår en fremgangsmåte for modifikasjon av biopolymerer, fortrinnsvis ladningsbærende biopolymerer, ved å immobilisere biopolymer på f.eks. en ionebytterabsorbent, omsetning med biopolymeren med reagenser, f.eks. enzymer eller andre biokjemiske reagenser, for å danne et reaksjonsprodukt og deretter atskille reaksjonsproduktet fra absorbenten. Reaksjonsproduktet er fortrinnsvis en nukleinsyre som er spaltet med et restriksjonsenzym. I DE 3717210 anvendes den adsorberte tilstand av biopolymeren for å eksponere biopolymeren bedre for reagenser og derved øke effektiviteten av modifikasjonen. Det foreligger verken indikasjon på skjerming av eksponerte mål eller en hensikt på å bibeholde aktivitet av biopolymeren som ville bli fordelaktig for en funksjon av biopolymeren in vivo. DE 3717210 er kun rettet på kartlegging av egenskaper til biomolekyler, slik som nukleinsyrer, ved å endre den originale karakter ved reell bearbeidelse eller økning av antallet funksjonelle enheter i makromolekyler, slik som inkorporering av radioaktive isotoper.
Mange proteiner tilsiktet for terapeutisk anvendelse er blitt konjugert, vanligvis ved anvendelse av forskjellige pegyleringsteknikker (Francis, G. E. et al., i Stability of Protein Pharmaceuticals/Series: Pharmaceutical Biotechnology 3, 86-91
(1995)). De fleste eksempler angår intravenøs administrering. Opptak etter subkutan administrering til plasma, lunge, lever og milt av noen mPEG-konjugerte allergener er imidlertid beskrevet, og immunterapi med mPEG-konjugerte allergener gitt subkutant har vist seg å være effektive (Dreborg, S. og Åkerblom. E. B., Crit. Rew. Therap. Drug Carr. Sys. 6(4), 315-365 (1990)). Intramuskulær administrering er dessuten blitt anvendt i kliniske forsøk med adenosin deaminase (Hershfield, M. S. et al., New Eng. J. Med. 316, 589-596 (1987). I noen få tilfeller har også pegylering blitt hevdet å være fordelaktig for den orale administrering. Pegylering av IgG for oral administrering er således beskrevet i EP-A-0 614 373, Mount Sinai School of Medicine. Pegylering av faktor VIII og faktor IX for oral administrering er beskrevet i Sakuragawa et al., Acta. Med. Biol., 34(3), 77-84 (1987) og i japansk patentsøknad nr. 44509/83, Nippon Chemifar Company. Faktor VIII er et protein som deltar i den indre blodkoagulering. Det er en kofaktor i reaksjonen hvor enzymfaktor IXa, i nærvær av fosfolipid og kaliumioner, omdanner proenzymfaktor X til den aktive form, faktor Xa, hvilket til sist fører til et fibrinkoagel. Human faktor VIII syntetiseres som et enkeltkjedet molekyl på ca. 300 kDa og består av de strukturelle domener A1-A2-B-A3-C1-C2 (Gistchier et al., 1984, Nature 312, s. 326; Wood et al., 1984, Nature, 312, s. 330: Vehar et al., 1984, Nature 312, s. 337; Toole et al., 1984, Nature, 312, s. 342). Forløperproduktet bearbeides til to polypeptidkjeder på 200 og 80 kDa i Golgi og de to kjeder som holdes sammen ved hjelp av metallioner uttrykkes i blodet (Kaufman et al., 1988, J. Biol. Chem., 263, s. 6352; Andersson et al., 1986, Proe. Nati. Acad. Sei., 83, s. 2979). B-domenet av faktor VIII synes å kunne unnværes med hensyn til faktor Vlll-kofaktorfunksjonen, mens A- og C-domenene har flere interaksjonsseter for andre makromolekyler som spiller en rolle i hemostasen (Sandberg et al., 1993, Thrombos. Håemostas., 69, s. 1204, og Lind et al., 1995, Eur. J. Biochem., 232, s. 19). von Willebrands faktor (vWf) er et multifunksjonelt polymert plasmaprotein som består av disulfidbundne subenheter på ca. 240 kDa. Subenhetene danner en heterogen populasjon av multimerer som har ett molekylvektområde fra ca. 1 MDa til 20 MDa. Funksjonen av vWf ved primær hemostase er å fremme blodplateadhesjon til blodkarveggen ved tilstander med høy skjærhastighet. vWf binder også faktor VIII tett, men ikke kovalent. I normalt humant plasma sirkulerer faktor VIII og vWf som et kompleks med hverandre. vWf har en viktig stabiliserende effekt på faktor VIII-molekylet, ved at det beskytter molekylet mot nedbrytning av proteaser (Koedam et al., 1987, Doctoral Thesis, ICG Printing, Dordrecht, 1986, s. 83; Hamer et al., 1986, Haemostasis, 1987, 58, s. 223). Den humane in v/vo-halveringstid av faktor VIII er vanligvis 10-15 timer. Hos vWf-manglende pasienter medfølges de reduserte nivåer av vWf av reduserte nivåer av faktor VIII på grunn av en svekket frigivelse og økt nedbrytningshastighet av faktor VIII. Tuddenham et al., 1982, Br. J. Haematol., 52, s. 259 og Brinkhous et al., 1985, Proe. Nati. Acad. Sei., 82, s. 8752 viste at tilstedeværelse av vWf har en viktig effekt på in v/vo-overlevelsen av faktor VIII. Når faktor VIII ble infusert i hemofile hunder ble det oppnådd en halveringstid på 7-10 timer, mens halveringstiden var ca. én time etter infusjon i vWf-manglende hunder (Brinkhous et al., se ovenfor). Faktor IX er proenzymet for faktor IXa beskrevet nedenfor. Den er en serinprotease og er ett av de vitamin K-avhengige koagulasjonsproteiner. Molekylmassen er ca. 55 kDa (DiScipio et al., 1977, Biochemistry, 16, s. 698). Faktor IXa reagerer spesifikt med andre komponenter som deltar i faktor IX-aktiveringen (i: Haemostasis and Thrombosis, 1994, vol. 1, 3. utg., red. Bloom A. et al.). Det fremgår fra avsnittene ovenfor at faktor VIII er et protein med flere interaksjonsseter som hvert er ansvarlig for en spesifikk funksjon. Det er derfor vanskelig å modifisere faktor VIII med full bibeholdelse av biologisk funksjon. Pegyleringsteknikken er blitt anvendt tidligere på proteinblandinger inneholdende faktor VIII. Det er således beskrevet i WO 94/15625 (Enzon) at pegylering av faktor VIII ved anvendelse av en karbamat(uretan)-binding i en ubestemt modifikasjonsgrad som resulterer fra et 100 gangers molart overskudd av mPEG i forhold til faktor VIII, kan øke in v/vo-halveringstiden hos mus fra 13 timer til 55 timer. Halveringstiden hos mus blir konvensjonelt vurdert til ca. én time. 13 timer er således en uvanlig lang halveringstid hos mus. Det må dessuten tas i betraktning at, med den ekstremt lave renhet av startfaktor Vlll-preparatet (20-50 IU/mg protein), var andre proteiner enn faktor VIII dominerende under koblingsreaksjonen. Et viktig protein som vanligvis er til stede i faktor Vlll-preparater med lav renhet er von Willebrands faktor som har en stabiliserende funksjon for faktor VIII, og som også meget vel kunne bidra til beskyttelsen av det tilsvarende funksjonelle sete under konjugasjonsprosessen. Etter konjugasjon kan mPEG lokaliseres på ethvert av proteinene som er til stede i proteinblandingen som faktor VIII vanligvis kun utgjør en liten del av. Den pegylerte proteinblanding inneholdende faktor VIII ble anvendt for intravenøs administrering. Et veldefinert startmateriale er imidlertid én av de essensielle faktorer som utgjør de kontrollerte betingelser fordret for farmasøytisk produksjon. Andre polymerer er også anvendt for konjugasjon av faktor VIII. Det er således beskrevet i US patent 4 970 300 at dekstrankonjugasjon kan anvendes for å forlenge halveringstiden av faktor VIII. I de fleste koblingsteknikker reagerer polymerreagenset med å-aminogrupper på lysinrester i polypeptidet. Disse er ofte spredt over hele polypeptidoverflaten og kan meget vel resultere i en konjugasjon nærliggende et funksjonelt sete. Som en konsekvens, ved vilkårlig kobling, blir aktiviteten eller funksjonen ofte forstyrret. Det er de foreliggende patentsøkeres erfaring at, ved anvendelse av slike teknikker på meget rene preparater, slik som en B-domene-deletert rekombinant faktor VIII med koagulasjonsaktivitet, blir denne aktivitet alvorlig redusert allerede ved en modifikasjonsgrad på ca. 5 mPEG/faktor VHI-molekyl.
O ppsummering av oppfinnelsen
De foreliggende oppfinnere har funnet at den spesifikke aktivitet av konjugerte polypeptider kan bibeholdes i høy grad, kun ved immobilisering av polypeptidet på en gruppespesifikk absorbent før koblingsreaksjonen, som etterfølges av desorpsjon av konjugatet ved hjelp av konvensjonelle teknikker. Dette er ganske overraskende fordi det tidligere har vært betraktet som essensielt å anvende adsorbenter med spesifikke bindingsproteiner med hensyn til det aktuelle polypeptid, for å oppnå en beskyttelse av visse domener som er viktige for de biologiske funksjoner.
Fordelen ved å modifisere biokompatible polymerer avhenger ofte av modifikasjonsgraden. En høy modifikasjonsgrad, dvs. et høyt antall biokompatible polymerer pr. polypeptidmolekyl, fordres således vanligvis for en effektiv beskyttelse mot proteolytisk aktivitet. Ved hjelp av foreliggende oppfinnelse, hvori de biokompatible polymerer innføres mer selektivt, er den spesifikke aktivitet bibeholdt i høyere grad. De foreliggende oppfinnere har således funnet at faktor VIII kan beskyttes effektivt mot nedbrytning i et in vitro- miljø som er vist å ha en nedbrytende effekt på dette molekyl. Denne effekt kan oppnås ved en modifikasjonsgrad på kun 4-5 mPEG/faktor VIII.
Anvendelsen av gruppespesifikke adsorbenter ifølge foreliggende oppfinnelse er mer økonomisk gunstig sammenlignet med adsorbentene beskrevet i kjent teknikk. Anvendelsen av gruppespesifikke adsorbenter vil også lette registrering av konjugatene som terapeutiske midler.
Den foreliggende fremgangsmåte muliggjør forbedring av in v/vo-funksjonen av polypeptider, spesielt ved å forbedre den farmakokinetiske funksjon, inkludert biotilgjengeligheten, og ved å redusere immunogeniteten som oppvises av polypeptidene.
Foreliggende oppfinnelse angår således en fremgangsmåte for forbedring av in v/vo-funksjonen av rekombinant faktor VIII SQ ved å skjerme utsatte mål på polypeptidet, kjennetegnet ved a) immobilisering av rekombinant faktor VIII SQ ved interaksjon med en gruppespesifikk adsorbent som inneholder anionebytterligander, b) aktivering av en biokompatibel polymer; c) konjugasjon av den aktiverte biokompatible polymer til eksterne seter på den immobiliserte, rekombinante faktor
VIII SQ; etterfulgt av d) eluering av konjugatet fra adsorbenten.
Detaljert beskrivelse av oppfinnelsen
Ved foreliggende oppfinnelse angår betegnelsen interaksjonssete forskjellige seter som er essensielle for den biologiske funksjon av det spesielle polypeptid.
Ved foreliggende oppfinnelse angår betegnelsen utsatte mål eksterne seter på det aktuelle polypeptid som er tilbøyelige til uønskede reaksjoner in vivo. Eksempler på utsatte mål inkluderer antigene epitoper og seter for proteolytisk spaltning. Visse seter for proteolytisk spaltning refereres imidlertid til som interaksjonsseter.
Foreliggende oppfinnelse muliggjør, i en hittil uoppnåelig grad, å redusere innvirkningen av uønskede reaksjoner in vivo, f.eks. proteolytisk spaltning og mulig aggregering, mens interaksjonssetene som er fundamentale for den biologiske funksjon av polypeptidet samtidig etterlates upåvirket. Dette er beskrevet ved anvendelsen av mPEG-konjugasjon av (rekombinant) faktor VIII. Nærmere bestemt, ved immobilisering av polypeptidet ved interaksjon med en gruppespesifikk absorbent som inneholder ligander, fremstilt ved organisk-kjemisk syntese, utelukkes interaksjonssetene på polypeptidet fra konjugasjon til den biokompatible polymer. Ved konjugasjon av den aktiverte biokompatible polymer til polypeptidet på de ønskede, eksterne seter, skjules dessuten de utsatte mål for virkningen fra f.eks. proteaser.
Ved foreliggende oppfinnelse refererer polypeptider til proteiner og oligopeptider med minst 20 aminosyrer i kjeden. Antallet aminosyrer i polypeptidet fremstilt i overensstemmelse med foreliggende oppfinnelse ligger fortrinnsvis i området fra 30 og opp til 4 500 aminosyrer, og fortrinnsvis i området fra 40 opp til 3 000 aminosyrer. Polypeptidene kan stamme fra pattedyr, nærmere bestemt mennesker, eller de kan produseres ved hjelp av rekombinant DNA-teknikker. Polypeptider som kan konjugeres i overensstemmelse med foreliggende oppfinnelse inkluderer polypeptider som oppviser koagulasjonsaktivitet, eller som har en understøttende funksjon for koagulering. Polypeptidene kan være uforkortede, dvs. sekvensen av aminosyrer som er identisk med den tilsvarende sekvens funnet i pattedyr generelt, og i mennesker spesielt. Polypeptidene kan også være delesjonsderivater av de uforkortede polypeptider, hvor én eller flere aminosyrer mangler. Polypeptidet er fortrinnsvis koagulasjonsfaktor VIII, von Willebrands faktor (vWf) eller en kombinasjon derav, eller koagulasjonsfaktor IX, og fortrinnsvis koagulasjonsfaktor VIII.
Uforkortet faktor VIII til stede i humant plasma har en molekylmasse på ca. 300 kDa. Faktor Vlll-konsentrater avledet fra humant plasma inneholder flere fragmenterte, fullt ut aktive faktor VHI-former, som beskrevet av Anderson et al. (se ovenfor). Den minste aktive form har en molekylmasse på ca. 170 kDa og består av to kjeder med ca. 90 kDa og ca. 80 kDa holdt sammen med metallioner. Det refereres her til EP-A-0 197 901, Pharmacia & Upjohn AB. Den biologisk aktive faktor VIII produsert i overensstemmelse med foreliggende oppfinnelse, har derfor passende en molekylmasse i området fra ca. 170 kDa og opp til ca. 300 kDa.
Pharmacia AB, Stockholm, Sverige, har utviklet et rekombinant faktor Vin-produkt som tilsvarer den 170 kDa plasmafaktor Vlll-form i terapeutisk faktor VIII-konsentrater. Det trunkerte rekombinante faktor Vlll-molekyl er betegnet r-VIII SQ og produseres av kinesisk hamster ovarie(CHO)celler i en cellekulturprosess i serumfritt medium. Den spesifikke aktivitet av r-VIII SQ er ca. 15 000IU VIII:C pr. mg totalt protein. Strukturen og biokjemien av r-VIII SQ er beskrevet i WO-A-91/09122, tilhørende Pharmacia AB.
Ved foreliggende oppfinnelse kan faktor VIII enten være plasmafaktor VIII eller rekombinant faktor VIII. Når faktor VIII er rekombinant kan den være uforkortet faktor VIII, eller fortrinnsvis et delesjonsderivat av uforkortet faktor VIII med koagulasjonsaktivitet. Mer foretrukket er delesjonsderivatet rekombinant faktor VIII SQ (r-VIII SQ). I denne sammenheng er et delesjonsderivat definert som en koagulasjonsfaktor VIII hvori hele, eller en del av, B-domenet mangler, mens koagulasjonsaktiviteten er opprettholdt. De resterende domener er passende bundet ved hjelp av en aminosyrelinker. Eksempler på forskjellige linkerkonstruksjoner er gitt i P. Lind et al., Eur. J. Biochem., vol., 232, (1995) s. 19-27.
Foreliggende oppfinnelse kan fordelaktig anvendes for selektivt å innføre biokompatible polymerer i mange forskjellige faktor Vlll-produkter. Oppfinnelsen kan således anvendes for ytterligere å stabilisere plasmafaktor VIII in vivo, der denne faktor allerede er stabilisert ved assosiasjon med dens bærerprotein, von Willebrands faktor (vWf).
Det er fordelaktig at sluttproduktet er veldefinert. Den spesifikke faktor VIII-aktivitet i koblingsprosedyren ifølge foreliggende oppfinnelse bør således være minst ca. 1 000 IU/mg totalt protein, og passende minst 2 000 IU/mg totalt protein. Den spesifikke faktor Vlll-aktivitet ligger fortrinnsvis i området fra 5 000 opp til 20 000 IU/mg totalt protein, og mer foretrukket i området fra 10 000 opp til
17 000 IU/mg totalt protein.
Faktor Vlll-aktiviteten ved koblingsprosedyren kan være minst ca.
1 000 IU/ml, og passende minst 2 000 IU/ml. Faktor Vlll-aktiviteten ligger fortrinnsvis i området fra 5 000 opp til 50 000 IU/ml, og mer foretrukket fra 20 000 opp til 40 000 IU/ml.
Den biokompatible polymer ifølge foreliggende oppfinnelse kan utvelges fra gruppen som består av homopolymerer, kopolymerer eller blokk-kopolymerer av monoalkyl-overdekkende polyalkylenoksider. De biokompatible polymerer kan være rettkjedete eller forgrenede. Det monoalkyl-overdekkende polyalkylenoksid utvelges passende fra gruppen som består av polyetylenglykol-homopolymerer og polypropylenglykol-homopolymerer. Den biokompatible polymer er fortrinnsvis en polyetylenglykol(PEG)homopolymer. Molekylvekten av PEG kan være i området fra ca. 300 opp til 20 000, passende i området fra 1 500 opp til 10 000 og fortrinnsvis i området fra 3 000 opp til 5 000.
Den biokompatible polymer bør være overdekket i én ende for å unngå kryssderivatisering. Eksempler på grupper som er egnede for dette formål er rettkjedete eller forgrenede lavere alkoksygrupper, fortrinnsvis monometoksygruppen. En spesielt foretrukket biokompatibel polymer ved foreliggende oppfinnelse er monometoksypolyetylenglykol (mPEG).
Andre biokompatible polymerer er også tenkelige, f.eks. dekstran, polyvinylpyrrolidon og DL-aminosyrer.
Den biokompatible polymer må bli aktivisert før koblingsreaksjonen for å muliggjøre dannelsen av kovalente bindinger. For dette formål bør det til enden av den biokompatible polymer som vender mot polypeptidet være bundet en hydroksylgruppe som kan aktiveres. Det finnes mange måter for å aktivere den biokompatible polymer, avhengig av den utvalgte aminosyre for kovalent binding. Egnede mPEG-derivater for kobling til aminogrupper er mPEG-succinimidylsuccinat, mPEG-succinimidylsuccinamid, mPEG-succinimidylpropionat, succinimidylkarbonat av mPEG, succinimidylester av karboksymetylert mPEG, mPEG-oksykarbonylimidazol, mPEG-nitrofenylkarbonater, mPEG-triklorfenylkarbonat, mPEG-tresylat (den sistnevnte er beskrevet av Nilsson K., Moscbach K., Methods in Enzymology, vol. 104, s. 56-69 (1984)), (alle de nevnte reagenser markedsføres av Shearwater Polymers, Inc., Huntsville, AL, USA), mPEG-maleinsyreanhydrid og mPEG-metylmaleinsyreanhydrid (Garman A, Kalindjian S. B., FEB vol. 223:2, s. 361-365), og blandede anhydrider av hhv. mPEG-succinat, mPEG-eddiksyre eller mPEG-propionsyre (Dreborg S. og Akerblom E., se ovenfor).
Cysteinrester kan konjugeres med mPEG-maleimid, mPEG-vinylsulfon og mPEG-ortopyridylsulfid (markedsført av Shearwater Polymers, Inc., Huntsville, AL, USA). Egnede reagenser i tilfellet med konjugering av guanidingruppen i arginin er mPEG-derivater av fenylglyoksal (Zalipski, se ovenfor).
Karbohydrater må først oksideres slik at aldehydgrupper er til stede, og de
kan deretter konjugeres med mPEG-hydrazid (Zalipski, se ovenfor).
Koblingsprosedyren utføres ved en passende pH for at aminosyren skal konjugeres. Den egnede pH for polypeptidet i sin helhet må også tas i betraktning, så vel som pH-avhengigheten av reaktiviteten av den biokompatible polymer. pH for kobling ligger derfor vanligvis i området fra ca. 7 og opp til ca. 8, idet den lavere grense er satt for å unngå en omfattende koblingsprosedyre, og den øvre grense for å tilveiebringe milde betingelser for polypeptidet. Det ovenfor angitte pH-område er egnet ved konjugasjon som involverer lysiner. I eksemplene i den foreliggende patentsøknad er det derfor konjugert et molekyl som har FVIII-aktivitet under slike milde betingelser. pH-verdier utenfor dette område kan også være passende i visse tilfeller. Konjugasjon som involverer cysteiner utføres derfor passende ved en pH lavere enn ca. 7, mens argininer passende konjugeres ved en pH i området fra ca. 5,5 og opp til ca. 9,3.
Koblingsprosedyren bør utføres ved en temperatur over 0 °C, passende i området fra ca. 5 og opp til ca. 40 °C, og fortrinnsvis i området fra 10 og opp til 30 °C. Det må også her vurderes en temperatur som er egnet for hele polypeptidet, så vel som påvirkningen av temperatur på reaktiviteten av den biokompatible polymer. I eksemplene ifølge foreliggende patentsøknad ble alle koblingstrinn utført ved romtemperatur.
Adsorbenten anvendt ved foreliggende oppfinnelse er gruppespesifikk i forhold til polypeptidet som skal immobiliseres. Gruppespesifikke adsorbenter har affinitet for flere polypeptider. De bindes dessuten ofte mindre sterkt og eluering kan utføres under mildere betingelser enn med monospesifikke adsorbenter, idet de sistnevnte bindes til et enkelt, eller et meget lite antall polypeptider. Eksempler på egnede gruppespesifikke adsorbenter er gitt i Jansson, J.-C. et al., Protein Purification, Principles. High Resolution Methods, and Applications, VCH Publishers, s. 274-285
(1989), som er inkorporert heri ved referanse. Adsorbenten ifølge foreliggende oppfinnelse er dessuten kjennetegnet ved at den inneholder ligander produsert ved hjelp av organisk-kjemisk syntese. Eksempler på adsorbenter som tilfredsstiller disse kriterier er generelt anvendelige kromatografiresiner for adsorpsjon av protein, f.eks. biokompatible matrikser til hvilke ligander, slik som anioniske og kationiske utbyttergrupper, sulfhydrylgrupper, immobiliserte metallaffinitetskromatografi (IMAC) grupper, rettkjedete eller forgrenede, mettede eller umettede hydrokarbonkjeder og aromatiske grupper, er blitt koblet. Anvendelige er også ligander med blandet funksjon, f.eks. med både ioniske og hydrofobe deler, slik som aminoalkyl eller dimetylaminopropylkarbanylpentyl. Disse to ligander er blitt rapportert som anvendelige for rensing av plasmaavledet faktor VIII, beskrevet av hhv. Morgenthaler et al., Thromb. Haemostas. 47(2), 124 ff. (1982) og Te Booy, M. P. M., et al., J. Chrom. 503, 103-114 (1990). Et foretrukket eksempel på aminoalkyl er aminoheksyl. Ligander med høyere kompleksitet kan også anvendes, slik som peptider eller fosfolipider, fremstilt ved hjelp av organisk-kjemisk syntese. For fagfolk inkluderer gruppeaffinitetsligandene som produseres ved hjelp av organisk-kjemisk syntese mange forskjellige stoffer. Reaktive triazinbaserte tekstilfargestoffer har således vist seg anvendelige som adsorbenter for flere enzymer, slik som kinaser og hydrogenaser. Sukkerderivater, nukleotider og peptider, og analoger derav, og fremstilt ved hjelp av organisk-kjemisk syntese, er også anvendelige (Dechow, F. J. Separation and purification techniques in biotechnology, Noyes Publications, Parke Ridge, NJ, USA, s. 440-443 (1989)). Ligander som er anvendelige for anvendelse ved foreliggende oppfinnelse er videre anioniske utbyttergrupper, fortrinnsvis sterke, slik som kvaternær aminoetyl (QAE), trimetylaminoetyl (TMAE) eller kvaternær aminometyl (Q), mer foretrukket kvaternær aminometyl (Q). Ligandene kan dessuten kobles tett til matriksen, eller en spacer, slik som alkylamin, heksylamin, diaminodipropylamin, etylaminsuccinamid (Persson & Lagerstrom, i Packings and stationary phases in chromatographic techniques, Unger, K. K., red., Marcel Dekker Inc. 1990, s. 754), eller med en forgrenet tentakkel som ligandene er festet til (et egnet eksempel er "Fractogel" EMD, produsert av Merck, Tyskland). Etter koblingsprosedyren elueres det konjugerte polypeptid ved hjelp av konvensjonelle teknikker. I denne sammenheng er det essensielt at fysisk-kjemiske betingelser vurderes for å unngå nedbrytning av de konjugerte polypeptider. Dersom koblingsreaksjonen involverer en konjugasjonsbinding som er labil i et bestemt pH-område, bør dette således unngås. Ved koblingsprosedyren, når polypeptidet er immobilisert på adsorbenten, kan polypeptidet bringes i kontakt med et oppløselig blokkeringsmiddel, med det formål å ekskludere ytterligere seter fra konjugasjon. Et slikt blokkeringsmiddel bør være produsert ved hjelp av organisk-kjemisk syntese, og dessuten være konjugert til enhver av de ovenfor identifiserte ligander bundet til en polymer utvalgt fra f.eks. monoalkyloverdekkende polyalkylenoksider, kopolymerer eller blokk-kopolymerer av polyalkylenoksider, polyetylenglykol-homopolymerer eller polypropylenglykol-homopolymerer. Blokkeringsmidlet kan også inneholde enheter med spesifikk affinitet for polypeptidet. Etter elueringen desorberes det oppløselige blokkeringsmiddel ved anvendelse av egnede kjemiske betingelser. Dersom blokkeringsmidlet f.eks. ble adsorbert ved hydrofob interaksjon, kan det desorberes ved å redusere ionestyrken, eller ganske enkelt ved å senke temperaturen. Det oppløselige blokkeringsmiddel separeres deretter fra konjugatet ved hjelp av gelpermeasjonskromatografi under konvensjonelle betingelser. Matriksen til adsorbenten ved foreliggende oppfinnelse kan være enhver kommersiell resin kjent for å være kompatibel med biologiske molekyler. Matriksen kan således utvelges fra forskjellige sterkt hydrofile matrikser, f.eks. agarosematrikser, slik som et bredt utvalg av "Sepharose"-matrikser solgt av Pharmacia Biotech, Uppsala, Sverige, organiske polymermatrikser, slik som TSK-GEL:s solgt av Tosoh Corp., Tokyo, Japan, eller høyporøse organiske polymermatrikser solgt av Per Septive Biosystems, Boston, USA. Membranmatrikser er også egnede, f.eks. "Sartobind" solgt av Sartorius, Tyskland, og "MemSep" solgt av Millipore, USA. Matriksen er fortrinnsvis en agarosematriks. Egnede agarosematrikser ved foreliggende oppfinnelse er, bortsett fra "Sepharose", "Minileak" solgt av Kem-En-Tec A/S, København, Danmark, og Bio-Gel A solgt av Bio-Rad, Brussel, Belgia. Matriksen er fortrinnsvis kryssbundet, hvilket tillater en hurtig gjennomstrømning (FF) og derved høy produksjonskapasitet. Mer foretrukket utføres kromatografien ved foreliggende oppfinnelse på en "Q Sepharose" FF-gel. Resiner sammensatt av kopolymerer av oligoetylenglykol, glysidylmetakrylat og pentaerytroldimetakrylat, slik som "Fractogel" (markedsført av Merck, Tyskland), cellulose og porøst glass, kan dessuten også anvendes med fordel. Konjugatene av et polypeptid og en biokompatibel polymer som kan erholdes ved hjelp av den foreliggende fremgangsmåte er nye. Med den foreliggende fremgangsmåte kan polypeptidaktiviteten etter konjugasjonen bibeholdes på minst 30 % av aktiviteten før konjugasjonen, passende minst 50 % og fortrinnsvis minst 70 % av aktiviteten før konjugasjonen. Konjugatene av et polypeptid og en biokompatibel polymer som kan oppnås ved hjelp av den foreliggende fremgangsmåte kan anvendes som medikamenter. Konjugater av faktor VIII, von Willebrand faktor eller en kombinasjon derav, eller faktor IX og en biokompatibel polymer produsert i overensstemmelse med foreliggende fremgangsmåte, kan spesielt anvendes som et medikament. Det er spesielt fordelaktig å anvende et konjugat av faktor VIII og en biokompatibel polymer, hvor graden av konjugasjon ligger i området fra 1 og opp til 15 monoalkyloverdekkende PEG/faktor Vlll-molekyl, passende i området fra 2 opp til 10 monoalkyloverdekkende PEG/faktor Vlll-molekyl, fortrinnsvis i området fra 3 og opp til 7 monoalkyloverdekkende PEG/faktor Vlll-molekyl. I denne sammenheng blir den konjugerte faktor VIII passende produsert ved hjelp av rekombinant DNA-teknikk, fortrinnsvis et rekombinant delesjonsderivat av uforkortet VIII med koagulasjonsaktivitet, og mer foretrukket delesjonsderivat av rekombinant faktor VIII
SQ (r-VIII SQ).
De konjugerte polypeptider ifølge foreliggende oppfinnelse anvendes passende for subkutan, intramuskulær, intradermal eller intravenøs administrering. Konjugert faktor VIII kan spesielt anvendes for fremstilling av et medikament for behandling av hemofili A, og spesielt for subkutan, intramuskulær, intradermal eller intravenøs administrering av et slikt medikament. Konjugert vWf kan dessuten anvendes ved fremstilling av et medikament for behandling av von Willebrands sykdom, og spesielt for subkutan, intramuskulær, intradermal eller intravenøs administrering av et slikt medikament. Faktor VIII og von Willebrands faktor, hvilke begge er blitt konjugert i overensstemmelse med den foreliggende fremgangsmåte, kan også anvendes i kombinasjon. Konjugert faktor IX kan også anvendes for fremstilling av et medikament for behandling av hemofili B, og spesielt for subkutan, intramuskulær, intradermal eller intravenøs administrering av et slikt medikament.
Foreliggende oppfinnelse angår dessuten en fremgangsmåte for behandling av hemofili A ved subkutan, intramuskulær, intradermal eller intravenøs administrering av et konjugat av faktor VIII og en biokompatibel polymer produsert i overensstemmelse med den foreliggende fremgangsmåte.
Eksempler
Eksempel 1
Fremstilling av rekombinant faktor VIII
Fremstillingen av rekombinant faktor VIII SQ (r-VIII SQ) ble vesentlig utført som beskrevet i WO-A-9109122, eksempel 1-3. En DHFR-manglende CHO-cellelinje (DG44N.Y.) ble underkastet elektroporering med en ekspresjonsvektor inneholdende r-VIII SQ-genet og en ekspresjonsvektor inneholdende dihydrofolatreduktasegenet. Etter seleksjon på selektivt medium ble overlevende kolonier amplifisert ved dyrking i trinnvis økende mengder metotrexat. Supernatant fra de resulterende kolonier ble individuelt screenet for faktor Vlll-aktivitet. En produksjonsklon be utvalgt og denne ble deretter tilpasset til serumfri suspensjonsdyrking i et definert medium, og til sist ble det utviklet en dyrkingsprosess i stor skala. Supernatant innsamlet etter bestemte tidsperioder og renses ytterligere som beskrevet nedenfor.
Det kondisjonerte medium ble klaret ved filtrering, pH ble justert og filtratet ble deretter applisert på en "Sepharose" FF-kolonne (kolonnevolum 3 liter). Etter vask ble faktor VIII eluert med en saltbuffer inneholdende 5 mM CaCl2 og 0,02 % "Triton" X-100. Dette kationebytterkromatografitrinn ble utført ved 2-8 °C. Eluatet fra "S Sepharose" FF-trinnet (S-eluat) ble nedfrosset inntil ytterligere rensing.
700 ml av S-eluatet ble tinet og temperaturen ble justert til romtemperatur. Virusinaktivering ble utført ved inkubasjon i 30 minutter med tri-n-butylfosfat (TNBP) og "Triton" X-100, ved en sluttkonsentrasjon på hhv. 0,3 % (volum/volum) og 1,0 % (volum/volum). Et monoklonalt antistoff (mAb)-immunaffinitetskolonne med et volum på 260 ml ble ekvilibrert med en S-eluatbuffer inneholdende de tilsvarende mengder virusinaktiveringskjemikalier. Faktor Vlll-oppløsningen ble deretter applisert på mAb-kolonnen som deretter ble vasket. Elueringen ble utført med en buffer inneholdende 50 % etylenglykol.
En "Q Sepharose"-kolonne ble preekvilibrert ved en høy konsentrasjon av natriumklorid, og deretter ekvilibrert med en buffer med den samme sammensetning som immunaffinitetskolonnen ble eluert med. mAb-eluatet ble applisert og kolonnen ble deretter vasket med ekvilibreringsbuffer, etterfulgt av en vaskebuffer med fysiologisk ionestyrke. Kolonnen ble eluert ved å øke natriumkloridkonsentrasjonen til 0,6 M. Det ble ikke anvendt detergent for vaskingen og elueringen av Q-kolonnen. En "Butyl Sepharose" 4 FF-kolonne ble ekvilibrert med buffer inneholdende 50 mM histidin, 1,4 M NH4AC, 50 mM CaCl2 og 0,02 % "Tween" 80, pH 6,8. NH4Ac ble tilsatt til Q-eluatet til en sluttkonsentrasjon på 1,0 M og "Tween" 80 til 0,02 %. Denne oppløsning ble applisert på butylgelkolonnen ved en lineær gjennomstrømningshastighet på 60 cm/time. Kolonnen ble deretter vasket med 5 kolonnevolumer ekvilibreringsbuffer og deretter eluert ved en lineær gjennomstrømningshastighet på 35 cm/time, med en buffer inneholdende 50 mM histidin, 0,5 M NH4Ac, 50 mM CaCl2 og 0,02 % "Tween" 80, pH 6,8. Renheten av dette preparat var meget høyt og ga en spesifikk aktivitet på 12 000-17 000 IU/mg protein.
Analyse
I eksempel 1-8 ble faktor Vlll-aktiviteten analysert ved hjelp av en kromogenanalyse (Chromogenix AB, Molndal, Sverige) dersom ikke annet er angitt. Mengden av faktor VIII ble bestemt spektrofotometrisk ved A280 og ved aminosyre-analyse. Mengden av mPEG kobles til polypeptidet ble målt ved hjelp av en proton NMR-metode (Dreborg, S. og Åkerblom, E. B., Crit. Rew. Therap. Drug Carr. Sys. 6(4), 315-365 (1990)).
Eksempel 2 ( sammenligningseksempel)
Faktor VIII konjugert med et blandet anhydrid av mPEG- succinat
Buffersammensetningen i HIC-eluatet, preparater i overensstemmelse med eksempel 1, ble utbyttet ved hjelp av gelpermeasjonskromatografi utført på en Superose 12-kolonne (markedsført av Pharmacia Biotech, Uppsala, Sverige), som tidligere var ekvilibrert med en buffer med den følgende sammensetning: 0,25 M hepes, 4 mM CaCl2, pH 7,8. Til aliquoter av r-VIII SQ-løsningen ble det tilsatt 35,60 og 100 gangers molart overskudd (basert på r-VIII) av et blandet anhydrid av mPEG-succinat (molekylvekt 3 000). Blandingene ble tillatt å reagere i 1,5 time på et rotasjonsapparat ved romtemperatur. For å fjerne overskuddet av mPEG-ravsyre og for å separere den heterogene populasjon av konjugert r-VIII etter størrelse, ble det utført gelpermeasjonskromatografi, nå ved anvendelse av en Superose 6-kolonne (markedsført av Pharmacia Biotech, Uppsala, Sverige). Fra hver koblingsreaksjon ble fraksjonene tilsvarende den første og andre halvpart av proteintoppen analysert separat (betegnet pool 1 og pool 2 i tabell I). Tabell I viser resultatene fra konjugasjon av r-VIII SQ med det blandede anhydrid av mPEG-succinat.
Eksempel 3
Faktor VIII adsorbert på en " O Sepharose" FF- gel under kobling med et blandet anhydrid av mPEG- succinat
Reaktive lysiner omgitt av negative ladninger på faktor Vlll-molekylet ble beskyttet mot mPEG-konjugasjon ved adsorpsjon av faktor VIII på en anionebyttergel. En kolonne pakket med "Q Sepharose" FF (markedsført av Pharmacia Biotech, Uppsala, Sverige), ekvilibrert med en buffer med den følgende sammensetning: 50 mM L-histidin, 0,15 M NaCl, 4 mM CaCl2, pH 6,5, ble anvendt for å adsorbere faktor VIII. Et HIC-eluat preparert i overensstemmelse med eksempel 1 ble applisert på kolonnen. For å oppnå egnede betingelser for mPEG-kobling ble kolonnen vasket med en buffer inneholdende 0,25 M hepes, 4 mM CaCl2, pH 7,8, før tilsetningen av mPEG. Gelen ble overført til en reaksjonsbeholder, et blandet anhydrid av mPEG-succinat (molekylvekt 3 000) ble tilsatt til oppslemmingen i en mengde som tilsvarer det molare overskudd i forhold til faktor VIII, som vist i tabell II. Reaksjonsblandingen ble inkubert under rotasjon i 1,5 time. Kolonnen ble deretter pakket på nytt og det konjugerte r-VIII ble eluert med en buffer bestående av 50 mM L-histidin, 0,6 M NaCl, 4 mM CaCl2, pH 6,8. Det ble laget fire preparater med en konstant mengde r-VIII SQ applisert pr. mengde gel. Alle trinn ble utført ved romtemperatur. Tabell II viser resultatene fra den konjugerte r-VIII SQ adsorbert på en "Q Sepharose" FF-gel.
Eksempel 4
FVIII adsorbert på en " Q Sepharose" FF- gel under kobling med mPEG- tresvlat
I dette eksempel ble alle trinn med adsorpsjon, kobling og eluering av r-VIII SQ på "Q Sepharose" FF-gel, utført som i eksempel 3. mPEG-tresylat (molekylvekt 5 000) ble anvendt for å konjugere faktor VIII. Tabell III viser resultatene fra konjugert r-VIII SQ adsorbert på en "Q Sepharose" FF-gel med mPEG-tresylat, i overensstemmelse med foreliggende oppfinnelse.

Faktor VIII adsorbert på en tentakkel- anionebvttergel under kobling med et blandet anhydrid med mPEG- succinat
En kolonne pakket med "Fractogel" EMD TMAE 650 (markedsført av Merck), ble anvendt for å adsorbere r-VIII SQ. Alle trinn ble utført som i eksempel 3. Tabell IV vise resultatene fra konjugasjon av r-VIII SQ adsorbert på en tentakkel-anionebyttergel med et blandet anhydrid av mPEG-succinat, i overensstemmelse med foreliggende oppfinnelse.
Eksempel 6
Trombinaktivering av mPEG- konjugert faktor VIII
Trombinaktivering av konjugert r-VIII SQ ble testet i en in vtfro-analyse som er velkjent for fagfolk. En prøve preparert i overensstemmelse méd eksempel 3 som ga en derivatiseringsgrad på 4 mPEG/r-VIII SQ, ble analysert. Det konjugerte r-VIII SQ kunne aktiveres ved hjelp av humant trombin på en doseavhengig måte. Inaktivering ble deretter utført. I figur 1 vises kurven for de oppnådde aktivitets-endringer når 1 NIH-enhet trombin pr. 1 enhet konjugert r-VIII SQ ble tilsatt. En ett-trinns koagulasjonsmetode, utført vesentlig i overensstemmelse med Mikaelsson et al., 1983, Blood 62, s. 1006, ble anvendt for umiddelbar analyse av prøver fra reaksjonsblanding. En 30 gangers aktivering ble oppnådd innen ett minutt, hvilket ble etterfulgt av inaktivering. De oppnådde aktiverings-inaktiveringsmønstere med trombin var overensstemmende med tidligere rapporterte undersøkelser av faktor Vlll-trombininteraksjon (Fulcher et al., 1983, Blood, 61, s. 807, Andersson et al., 1986, Proe. Nati. Acad. Sei., 83, s. 2979, Eaton et al., 1986, Biochemistry, 25, s. 505, Rotblat et al., Biochemistry, 24, s. 4294).
Eksempel. 7
Stabilitetstester av mPEGvlert faktor VIII i et ekstrakt av vev fra gris
For å evaluere påvirkningen av mPEG-konjugasjonen på stabiliteten av faktor VIII i en oppløsning som er representativ for det subkutane miljø, ble det utført en in W/ro-test ved anvendelse av ekstrakt av grisevev. To preparater av mPEG-konjugert faktor VIII ble produsert som beskrevet i eksempel 3 og inkubert i ekstraktet, og aliquoter ble analysert etter 0, 2, 4, 6 og 24 timer for koagulasjonsaktivitet ved anvendelse av en kromogenanalyse. Under de første 6 timer var konjugasjonsgraden overensstemmende med den resterende aktivitet.
Eksempel 8
Biotilgiengelighetsundersøkelser av mPEGvlert faktor VIII i cynomolgus- aper
For å evaluere påvirkningen av mPEG-konjugasjonen på in v/vo-stabiliteten av faktor VIII, ble det utført en farmakokinetisk undersøkelse. To preparater av mPEG-konjugert faktor VIII ble produsert som beskrevet i eksempel 3, og deres spesifikke aktiviteter ble bestemt, som angitt i tabell VI. For å oppnå høy homogenitet av det derivatiserte materiale produsert ved koblingsprosedyren, ble fraksjonering av hvert preparat utført to ganger ved hjelp av gelpermeasjonskromatografi. En Superdex 200 PG XK 16/60 kolonne (markedsført av Pharmacia Biotech, Uppsala, Sverige) ekvilibrert med 19 mM L-histidin, 0,31 M NaCl, 3,4 mM CaCl2, 0,03 % Tween 80, pH 7,0, ble anvendt. Volumreduksjon av faktor Vlll-løsningene før den andre gelpermeasjonskromatografi ble utført i autoklaverte Centriplus 30 konsentratorer; sentrifugale ultrafiltreringsapparater (sperre 30 kDa, markedsført av Amicon Inc. MA, USA). Løsningene ble konsentrert tre ganger ved 2 500 xg i 45 minutter. De sammenslåtte fraksjoner etter den andre gelpermeasjonskromatografi ble deretter fortynnet i en buffer tilpasset den endelige formulering; 19 mM L-histidin, 0,31 M NaCl, 3,4 mM CaCl2, 0,02 % Tween 80, 17,5 mM sukrose, pH 7,0. Proteinløsningene ble til sist filtrert ved anvendelse av 0,22 im Millex GV-filtere (markedsført av Millipore, MA, USA) og deretter fylt på autoklaverte flasker. Løsningene ble lagret ved -70 °C inntil bruk.
Seks cynomogulus hunnaper ble administrert enkle intravenøse doser av preparat 1 med 250 IU/kg og enkle subkutane doser av preparat 1 og preparat 2 med 1 500 IU/kg på forskjellige tidspunkter. Alle løsninger ble fortynnet 1:1 med vann for injeksjon før administrering. Injeksjonsstedet for subkutan administrering var i det venstre eller det høyre dorsale område på dyret, og for intravenøs administrering i hhv. venstre eller høyre sefalvene. Blodprøver ble oppsamlet fra alle dyr via lårvenepunktur og i rør inneholdende natriumsitrat som antikoagulasjonsmiddel (10 % av det totale volum) på de følgende tidspunkter etter injeksjon: Intravenøs administrering: 0 (predose), 0,25, 1, 2,4, 8,24, 30,48 h. Subkutan administrering: 0 (predose), 3, 6, 9, 12, 15,24, 30, 48 h.
En farmakokinetisk undersøkelse av ukonjugert faktor VIII ble utført i en separat undersøkelse i den samme art. Dosene, administreringsmåten og resultatene (gjennomsnitt (± standardavvik (SD))) er vist i tabell VI.
Som det fremgår fra tabell VI resulterte subkutan administrering av begge preparater av mPEG-konjugert faktor VIII i en markert høyere biotilgjengelighet sammenlignet med subkutan administrering av ukonjugert r-VIII SQ. Statistisk analyse ved anvendelse av student t-testen bekreftet at forskjellen var signifikant.
Claims (9)
1. Fremgangsmåte for forbedring av in vtvo-funksjonen av rekombinant faktor VIII SQ ved å skjerme utsatte mål på polypeptidet,
karakterisert veda) immobilisering av rekombinant faktor VIII SQ ved interaksjon med en gruppespesifikk adsorbent som inneholder anionebytterligander. b) aktivering av en biokompatibel polymer; c) konjugasjon av den aktiverte biokompatible polymer til eksterne seter på den immobiliserte, rekombinante faktor VIII SQ; etterfulgt av d) eluering av konjugatet fra adsorbenten.
2. Fremgangsmåte ifølge krav 1,
karakterisert ved at den spesifikke aktivitet av faktor VIII SQ er minst 2 000 IU/mg av totalt protein før immobilisering.
3. Fremgangsmåte ifølge krav 2,

karakterisert ved at aktiviteten av faktor VIII SQ ligger i området fra 5 000 til 50 000 IU/ml før immobilisering.
6. Fremgangsmåte ifølge ethvert av kravene 1-5,
karakterisert ved at den biokompatible polymer er utvalgt fra gruppen bestående av homopolymerer, kopolymerer eller blokk-kopolymerer av monoalkyl-overdekkende polyalkylenoksider.

karakterisert ved at det monoalkyl-overdekkende polyalkylenoksid er utvalgt fra gruppen bestående av polyetylenglykol-homopolymerer og polypropylenglykol-homopolymerer.

karakterisert ved at det monoalkyl-overdekkende polyalkylenoksid er en monometoksypolyetylenglykol(mPEG).
