Description
Foreliggende oppfinnelse er rettet mot peptidanaloger av glukagon-lignende peptid-1, farmasøytisk-aseptable salter derav og farmasøytiske sammensetninger nyttige for dette omfattende nevnte analoger.
Glukagon-lignende peptid-1 (7-36) amid (GLP-1) blir syntetisert i tarm L-celler ved vevs-spesifikk post-translasjonell prosessering av glukagonforløperen preproglukagon (Vamdell, J.M., et al., J. Histochem Cytochem, 1985:33:1080-6) og blir frigjort sirkulasjon etter et måltid. Plasmakonsentrasjon av GLP-1 øker fra et nivå ved fasting på omtrent 15 pml/L til et topp postpranidalnivå på 40 pmol/L. Det er blitt demonstrert at for en gitt økning i plasmaglukosekonsentrasjon er økningen i plasma insulin omtrent tre ganger høyere når glukose blir administrert oralt sammenlignet med intravenøst (Kreymann, B., et al., Lancet 1987:2, 1300-4). Denne ernæringsmessige økningen av insulinfrigjøring, kjent som inkretineffekt, er hovedsakelig humoral, og GLP-1 antas nå å være det mest potente fysiologisk inkretin i mennesker. I 1994 ble det terapeutiske potentiale til GLP-1 foreslått etter observasjon om at en enkelt subkutan (s/C) dose av GLP-1 kunne fullstendig normalisere postprandialglukosenivåer i pasienter med ikke-insulin-avhengig diabetes mellitus (NIDDM) (Gutniak, M.K., et al., Diabetes Care 1994:17:1039-44). Denne effekten var antatt å være mediert både av øket insulinfrigjøring og av en reduksjon i glukagonsekresjon. En intravenøs infusjon av GLP-1 er blitt vist å forsinke postprandial mavetømming i pasienter med NIDDM (Williams, B., et al., J. Clin Endo Metab 1996:81:327-32). Til forskjell fra sulfonylureaer er insulinotrof virkning av GLP-1 avhengig av plasmaglukosekonsentrasjonen (Holz, G.G., 4* et al., Nature 1993:361:362-5). Videre er det også effektivt i pasienter med diabetes (Guniak, M., N, Engl J. Med 226:1316-1322,1992; Nathan, D.M. et al., Diabetes Care 15:270-276,1992), normalisering av gludglukosenivåer i individer med type 2 diabetes (Nauck, M.A., et al., Diagbetlogia 36:741-744, 1993) og forbedring av glykemisk kontroll i type 1 pasienter (Creutzfeldt, W.O., et al., Diabetes Care 19:580-586, 1996), og frembringe muligheten av anvendelse derav som et terapeutisk middel. ;GLP-1 er derimot metabolsk ustabil og har en plasma halveringstid (ti/2) på kun 1-2 min in vivo. Eksogent administrert GLP-1 blir også degradert (Deacon, C.F., et al., Diabetes 44:1126-1131,1995). Denne metabolske manglende stabiliteten begrenser det terapeutiske potensialet til nativ GLP-1. Det er følgelig et behov for GLP-1 analoger som er mer aktive og som er mer metabolsk stabile enn native GLP-1. ;Et aspekt av foreliggende oppfinnelse er rettet mot en forbindelse kjennetegnet ved at nevnte forbindelse er (Aib<8,35>)hGLP-1 (7-36)NH2 eller et farmasøytisk akseptabelt salt derav. ;Foreliggende oppfinnelse vedrører videre anvendelse av en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt derav for fremstilling av et farmasøytisk preparat for å utløse en agonisteffekt fra en GLP-1 reseptor i et individ som trenger det grunnet begynnelse av Type I diabetes, Type II diabetes, fedme, glukagomer, sekretoriske forstyrrelser i luft-veiene, metabolske forstyrrelser, artritt, osteoporose, sentralnervesystemsykdom, restenose og neurodegenerativ sykdom. ;Det er videre beskrevet anvendelse av en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt derav for fremstilling av et farmasøytisk preparat for å behandle en sykdom valgt fra gruppen bestående av Type I diabetes, Type II diabetes, fedme, glukanomer, sekretoriske forstyrrelser i luftveiene, metabolsk forstyrrelse, artritt, osteoporose, sentralnervesystemsykdom, restenose og neurodegenerativ sykdom, i et individ som trenger det. ;I et annet aspekt tilveiebringer foreliggende oppfinnelse en farmasøytisk sammensetning kjennetegnet ved at den omfatter en effektiv mengde av en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt derav og en farmasøytisk akseptabel bærer eller et fortynnings-middel. ;diabetes, fedme, glukagonomer, sekrektoriske forstyrrelser i luftveien, metabolske forstyrrelser, artritt, osteoporose, sentralnervesykdom, restenose, neurodegenerativ sykdom, nyresvikt, kongestiv hjertesvikt, nefrotisk syndrom, sirrose, lungeødem, hypertensjon og forstyrrelser hvor redksjon av matinntak er ønsket. ;En forbindelse ifølge foreliggende oppfinnelse kan bli testet for aktivitet som en GLP-1 bindende forbindelse ifølge nedenfor angitte prosedyre. ;Celledyrking: ;RIN 5F rotteinsulinomceller (ATCC-# CRL-2058, American Type Culture Collection, Manassas, VA), som uttrykket GLP-1 reseptoren, ble dykret i Dulbecco's modifiserte Eagle's medium (DMEM) inneholdende 10% føtalt kalveserum og opprettholdt ved omtrent 37°C i 1 fuktatmosfære bestående av 5% COz/95% luft. Radioligandbinding: Membraner ble dannet for radioligandbindingsstudier ved homogenisering av RIN celler i 20 ml is-kald 50 mM Tris-HCI med en Brinkman Polytron (Westbury, NY) ;(innstilling 6,15 sek). Homogenatene ble vasket to ganger ved sentrifugering (39,000 g/10 min), og de endelige pelletene ble resuspendert i 50 mM Tris-HCI ;inneholdende 2,5 mM MgCI2, 0,1 mg/ml basitrasin (Sigma Chemical, St. Louis, MO), og 0,1% BSA. For analysering ble alikvoter (0,4 ml) inkubert med 0,05 mM (<125>)GLP-1(7-36) (-2200 Ci/mmol, New England Nucklear Boston, Mm A), med og uten 0,05 ml umerket kompereende testpeptider. Etter 100 min. inkubasjon (25°C) ble bundet ;(<125>)GLP-1 (7-36) separert fra fri ved hurtig filtrering gjennom GF/C filtere (Brandel, Gaithersburg, MD), som på forhånd var blitt nedsenket i 0,5% polyetylenimin. Filtrene ble deretter vasket tre ganger med 5 ml alikvoter is-kald 50 mM Tris-HCI, og bundet radioaktivitet innesluttet på filtrene ble opptelt ved gamma spektrometri (Wallac LKG, Gaithersburg, MD). Sepseifikk binding var definert som total (125)GLP-1 (7-36) bundet minus det som er bundet i nærvær av 1000 mM GLP1(7-36) (Bachem. Torrence, ;CA). ;Forbindelsene ifølge oppfinnelsen kan bli tilveiebrakt i form av farmasøytisk ;akseptable salter. Eksempler på slike salter innbefatter, men er ikke begrenset til, de som blir dannet med organiske syrer (f.eks. eddiksyre, melkesyre, maleinsyre, sitron-syre, malinsyre, askorbinsyre, ravsyre, benzosyre, metansulfonsyre, toluensulfonsyre eller pamoinsyre), uorganiske syrer (f.eks. saltsyre, svovelsyre eller fosfosyre) og ;polymersyrer (f.eks. tanninsyre, karboksymetylcellulose, polymelkesyre, polygiykolsyre eller kopolymerer av polymelke-glykolsyrer). En typisk metode for dannelse av et salt av en forbindelse ifølge foreliggende oppfinnelsen er velkjent innenfor fagområdet, og kan bli oppnådd ved standardmetoder for saltutveksling. Følgelig kan TFA saltet av en forbindelse ifølge foreliggende oppfinnelsen (TFA saltet er et resultat av rensing av forbindelsen ved anvendelse av preparativ HPLC, eluering med TFA inneholdende bufferløsninger) kan bli omdannet til et annet salt, så som et acetatsalt ved oppløsning av peptidet i en liten mengde 0,25 N eddiksyre vandig løsning. Den resulterende løsningen blir applisert på en semi-prep HPLC kolonne (Zorbax, 300 SB, C-8). Kolonnen blir eluert med (1) 0,1 N ammoniumacetat vandig løsning i 0,51., (2) 0.25N eddiksyre vandig løsning i 0,51. og (3) en lineær gradient (20% til 100% av løsning B over 30 min.) ved en strømningsrate på 4 ml/min. (løsning A er 0.25N eddiksyre vandig løsning; løsning B er 0,25 N eddiksyre i acetonitril/vann, 80:20). Fraksjonen inneholdende peptidet blir samlet og lyofilisert til tørrhet. ;Som kjent for fagfolk innenfor dette området, blir kjente og potensielle anvendelser av GLP-1 variert og mangfoldige (se, Todd, J.F., et al., Clinical Science, 1998, 95 s. 325-329; og Todd, J.F. et al., European Journal of Clinical Investigation, 1997, 27, s, 533-536). Administrering av forbindelsene ifølge oppfinnelsen for å utløse en agonisteffekt kan ha samme virkninger og anvendelser som GLP-1 selv. GLP-1 analoger ifølge foreliggende oppfinnelse som utløser en antagonist-effekt fra et individ, kan bli anvendt for behandling av følgende: hypoglycemi og feilabsorpsjonssyndrom assosiert med gastroektomi eller liten tarmresekskjon. ;Foreliggende oppfinnelse innbefatter følgelig farmasøytiske sammensetninger omfattende, som et aktivt ingrediens, minst en av forbindelsene med formel (I) sammen med en farmasøytisk akseptabelt bærer. ;Dosering av det aktive ingredienset i sammensetningene ifølge oppfinnelsen kan bli variert, men det er derimot nødvendig at mengden av aktivt ingrediens er slik at en egnet doseringsform blir oppnådd. Valgt dosering avhenger av ønsket terapeutisk effekt, administreringsvei og på varighet av behandling. Generelt er en effektiv dosering for aktiviteten i området 1x10"<7> til 200 mg/kg/dag, fortrinnsvis 1x10"<* >til 100 mg/kg/dag, som kan bli administrert som en enkelt dose eller delt inn i flere doser.
Forbindelsene ifølge oppfinnelsen kan bli administrert ved orale, parenterale (f.eks. intramuskulære, intrepeirtoneale, intravenøse eller subkutane injeksoner, eller implantat), nasale, vaginale, rektale, sublingualet eller topiske administreirngsveier og kan bli formulert med farmasøytisk akseptable bærere for å tilveiebringe doserings-former hensiktsmessige for hver administreringsvei.
Faste doseringsformer for oral administrering innbefatter kapsler, tabletter, piller, pulvere og granuler. I slike faste doseringsformer blir den aktive forbindelsen blandet sammen med minst en inert farmasøytisk akspetabel bærer så som sukrose, laktose eller stivelse. Slike doseringsformer kan også omfatte, som normal praksis, ytterligere forbindelser forskjellige fra slike inerte fortynningsmidler, f.eks. smøremidler så som magnesiumstearat. Når det gjelder kapsler, tabletter og piller, kan doseringsformene også omfatte buf ringsmidler. Tabletter og piller kan i tillegg bli fremstilt med enteriske belegg.
Flytende doseringsformer for oral administrering innbefatter farmasøytisk akseptable emulsjoner, løsninger, suspensjoner, syruper, eliksirer inneholdende inerte fortynningsmidler som vanligvis blir anvendt innenfor fagområdet, så som vann. Bortsett fra slike interte fortynningsmidler kan sammensetningene også innbefatte adjuvants, så som fuktmidler, emulgeringsmidler og suspenderingsmidler, og søtningsmidler, smaks-stoffer og parfymeringsmidler.
Preparater ifølge denne oppfinnelsen for parenteral administrering innbefatter sterile vandige eller ikke-vandige løsninger, suspensjoner eller emulsjoner. Eksempler på ikke-vandige lønsingsmidler eller bærere eller propylenglykol, polyetylenglykol, vegetabilske oljer, så som olivenolje og maisolje, getalin og injiserbare organiske estere så som etyloleat. Slike doseringsformer kan også inneholde adjuvants så som konserveringsmidler, fuktmidler, emuleringsmidler og dispergeringsmidler. De kan for eksempel bli sterilisert ved filtrering gjennom et bakterie-tilbakeholdende filter, ved inkorporering av steriliseirngsmidler inn i sammensetningene, ved bestrålning av sammensetningene, eller ved oppvarming av sammensetningene. De kan også bli fremstilt i form av sterile faste sammensetninger som kan bli oppløst i sterilt vann, eller et annet sterilt injiserbart medium rett før bruk.
Sammensetninger for rektal eller vaginal admtnstrering er fortrinnsvis suppositorier som kan inneholde, i tillegg til den aktive forbindelsen, eksipienter så som kakaosmør eller en suppositorievoks.
Sammensetninger for nasal eller sublingual administrering blir også fremstilt med standard eksipienter velkjente innenfor fagområdet.

Eksempel 1
(Alb<8,35>)hGLP-1(7-36)NH2
Tittelpeptider ble syntetisert på et Applied Biosystems (Foster City, CA) modell 430 A peptidsyntetisator som ble modifisert for å utføre akselert Boc-kjemi fastfase peptidsyntese. Se Schnolzer, et al., Int. J. Peptide Protein Res., 90:180 (1992). 4-metyl-benzhydrylamin (MBHA) harpiks (Peninsula, Belmon, CA) med substitusjon av 0,91 mmol/g ble anvendt. Boe aminosyrene (Bachem, CA. Torrance, CA; Nova Biochem., Ljolla, CA) ble anvendt med følgende sidekjedebeskyttelse: Boc-Ala-OH, Boc-Arg(Tos)-OH, Boc-Asp(OcTex)-OH, Boc-Tyr)2BrZ-OH, Boc-His(DNP)-OH, Boc-Val-OH, Boc-Leu-OH, Boc-Gly-OH, Boc-Gin-OH, Boc-lle-OH, Boc-Lys(OcTex)-OH og Boc-Trp(Fm)-OH. Syntesen ble utført i 0,20 mmol skala. Boe gruppene ble fjernet ved behandling med 100% TFA i 2 x 1 min. Boe aminosyrene (2,5 mmol) ble preaktivert med HBTU (2,0 mmol) og DIEA (1,0 ml) i 4 ml DMF, og ble koblet uten på forhånd nøytralisering av peptid-harpiks TFA saltet. Koblingstider var 5 min. med unntakelse av Boc-Aib-OH residier og følgende residier, Boc-Lys(2CIZ)-OH og Boc/His(DNP)-OH hvori koblings-tidene var 2 timer.
Etter endt oppstilling av peptidkjeden ble harpiksen behandlet med en løsning av 20% merkaptoetanol/10% DIEA i DMF i 2 x 30 min. for å fjerne DNP gruppen på His sidekjeden. N-terminal Boe gruppe ble deretter fjernet ved behandling med 100% TFA i 2 x 2 min. Etter nøytralisering av peptid-harpiksen med 10% DIEA i DMF (1 z 1 min.), ble formylgruppen på sidekjeden av Trp fjernet ved behandling med en løsning av 15% etanolamin/15% vann/70% DMF i 2 x 30 min. Peptid-harpiksen ble vasket
med DMF og DCM og tørket under redusert trykk. Den endelige spaltningen ble utført med omrøring av peptid-harpiksen i 10 ml HF inneholdende 1 ml anisol og
ditiotreitol (24 mg) ved 0°C i 75 min. HF ble fjernet med en nitrogenstrøm. Resten ble vasket med eter (6x10 ml) og ekstrahert med 4N HOAc (6x10 ml).
Peptidblandingen i det vandige ekstraktet ble renset på revers-fase preparativ høytrykksvæskekromatografi (HPLC) ved anvendelse av en reversfase VYDA® Cia kolonne (Nest Group, Southborough, MA). Kolonnen ble eluert med en lineær gradient (20% til 50% av løsning B over 105 min.) ved en strømningsrate på 10 ml/min (løsning A = vann inneholdene 0,1% TFA; løsning B = acetonitril inneholdende 0,1% TFA). Fraksjoner ble samlet og undersøkt på analytisk HPLC. Oe som inneholdt rent produkt, ble kombinert og lyofilisert til tørrhet. 135 mg av et hvitt faststoff ble oppnådd. Renheten var 98,6% basert på analytisk HPLC analyse. Elektro-spray massespektrometer (MS(ES))S analyse ga molekylvekt 3339,7 (i samsvar med beregnet molekylvekt på 3339,7).
Eksempel 2
((Na-HEPES-His)<7>, Aib8,35)hGLP-1 (7-36)NH2
Tittelforbindelsen (HEPES er (4-(2-hydroksyetyl)-1-piperazinetansulfonsyre)) kan bli syntetisert som følger: etter oppstilling av peptidet (Aib<8,3S>)hGLP-1 (7-36)NH2 på MBHA harpisk (0,20 mmol) ifølge prosedyren i eksempel 1, ble peptid-harpiksen behandlet med 100% TFA (2x2 min.) og vasket med DMF og DCM. Harpiksen blir deretter nøytralisert med 10% DIEA i DMF i 2 min. Etter vasking med DMF og DCM blir harpiksen behandlet med 0,23 mmol 2-klor-1 -etansulfonylklorid og 0,7 mmol DIEA i DMF i omtrent 1 time. Harpiksen blir vasket med DMA og DCM og behandlet med 1,2 mmol 2-hydroksyetylpiperazin i omtrent 2 timer. Harpiksen blir vasket med DMF og DCM og behandlet med forskjellige reagenser ((1) 20% merkaptoetanol / 10% DIEA i DMF og (2) 15% etanolamin /15% vann i 70% DMF) for å fjerne DNP gruppen på His sidekjeden og formylgruppen på Trp sidekjeden som beskrevet ovenfor, før den endelige HF spaltningen av peptidet fra harpiksen.

Tittelforbindelsen ble syntetisert vesentlig ifølge prosedyren beskrevet for eksempel 1 ved anvendelse av hensiktsmessig beskyttet aminosyrer. MS (ES) ga molekylvekt på 3325,7, beregnet MW = 3325,8, renhet = 99%, utbytte = 85 mg.
Syntese av andre forbindelser ifølge oppfinnelsen kan bli oppnådd vesentlig på samme måte som prosedyren beskrevet for syntese av (Aib<8,35>)hGLP-1 (7-36)NH2 i eksempel 1 ovenfor, men ved anvendelse av hensiktsmessig beskyttete aminosyrer avhengig av ønsket peptid.
Eksempel 5
(Aib8,3<5>, Arg28,34, Lys<M>(Nrtetradel<ano<y>l))hGLP-1(7-36)NH2
Boe aminosyrer som ble anvendt, var de samme som de ved syntese av (Aib<8>,<35>)hGLP-1(7-36)NH2 beskrevet i eksempel 1 med unntakelse av at Fmoc-Lys(Boc)-OH ble anvendt i dette eksemplet. Første aminosyreresten ble koblet til harpiksen manuelt på en risteanordning. 2,5 mmol Fmoc-Lys(Boc)-OH ble oppløst i 4 ml 0,5N HBTU i DMF. Til løsningen ble det tilsatt 1 ml DIEA. Blandingen ble ristet i omtrent 2 min. Til løsningen ble det deretter tilsatt 0,2 mmol MBHA harpiks (substitusjon = 0,91 mmol/g). Blandingen ble ristet i omtrent 1 t. Harpiksen ble vasket med DMF og behandlet med 100% TFA i 2x2 min. for å fjerne Boe beskyttelses-gruppen. Harpiksen ble vasket med DMF. Muristinsyre (2,5 mmol) ble pre-aktivert med HBTU (2,0 mmol) og DIEA (1,0 ml) i 4 ml DMF i 2 min., og ble koblet til Fmoc-Lys-harpiksen. Koblingstiden var omtrent 11. Harpiksen ble vasket med DMF og behandlet med 25% pierpidin i DMF i 2,20 min. for å fjerne Fmoc beskyttelsesgruppen. Renheten var 98% basert på analytisk HPLC analyse. Elektra-spray massespektrometeranalyser ga molekylvekt på 3577,7 i samsvar med beregnet molekylvekt 3578,7.

Dersom Asp(1-alkylamino) eller Glu(1-alkylamino) residiet ikke er ved den C-etemrinale enden, blir peptidfragmentet rett før Asp{1 - alkylamino) eller Glu(l-alkyl-amino) residiet oppstilt på harpiksen på peptidsynteseinnretningen først.

Boe aminosyrer som blir anvendt, er de samme som de ved syntese av (Aib835, Arg2634, Lys^Ne-tetradekanoyOJhGLP-l^-SejNHz (eksempel 5). 270 mg Boc-p-Ala-PAM harpiks (Novabiochem, San Diego, California, substitusjon = 0,74 mmol/g) ble anvendt. Claims (5)
1. Forbindelse, karakterisert ved at nevnte forbindelse er (Aib<8>,<35>)hGLP-1(7-36)NH2 eller et farmasøytisk akseptabelt salt derav.
2. Farmasøytisk sammensetning, karakterisert ved at den omfatter en effektiv mengde av en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt derav og en farmasøytisk akseptabel bærer eller et fortynnings-middel.
3. Anvendelse av en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt derav for fremstilling av et farmasøytisk preparat for å utløse en agonisteffekt fra en GLP-1 reseptor i et individ som trenger det grunnet begynnelse av Type I diabetes, Type II diabetes, fedme, glukagomer, sekretoriske forstyrrelser i luft-veiene, metabolske forstyrrelser, artritt, osteoporose, sentralnervesystemsykdom, restenose og neurodegenerativ sykdom.
4. Anvendelse av en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt derav for fremstilling av et farmasøytisk preparat for å behandle en sykdom valgt fra gruppen bestående av Type I diabetes, Type II diabetes, fedme, glukanomer, sekretoriske forstyrrelser i luftveiene, metabolsk forstyrrelse, artritt, osteoporose, sentralnervesystemsykdom, restenose og neurodegenerativ sykdom, i et individ som trenger det.
5. Anvendelse ifølge krav 4, ved at nevnte sykdom er Type I diabetes eller Type II diabetes.
