Description
Oppfinnelsen angår anvendelse av selegilin (= L-N-(l-fenyl-isopropyl)N-metyl-N-propinylamin) eller dens farmasøytisk anvendbare salter til fremstilling av et farmasøytisk preparat med antiepileptisk virkning.
Det er kjent at selegilin er en blokkerer av monoaminoksydase av type B. Dette enzymet bryter ned monoaminerge neurotransmittere i gliacellene (fremfor alt dopamin), og dette fører til en avslutning av virkningen til dopamin. Ved blokkade av enzymet blir dopamin-speilet hevet i hjernen. På grunn av denne virkningen blir selegilin anvendt som tilleggsbehandling sammen med levodopa i terapien ved Parkinson-sykdom (f.eks. i handel under varebetegnelsen deprenyl).
I litteraturen er det videre beskrevet virkning av N-(l-fenyl-isopropyl)N-metyl-N-propinylaminer hhv. L-formen (selegilin).
I WO 92/17 169 Al blir det beskrevet en neurobeskyttende virkning med til
nå ikke definert virkningsmekanisme. Den blir her foreslått som neurobeskyt-telse (forhindring av nervecelletap ved sykdommer i ZNS, særlig Parkinson-sykdom, hjerne trauma og trauma i ryggmarg).
Videre er følgende virkninger av selegilin med eller uten kombinasjonspartner patentert, men det er ikke kjent noen entydig virkningsmekanisme:
Terapi av
a) Immunsystem-dysfunksjoner; b) Cushing-syndrom; c) psykiske abstinenssymptomer etter kokain, alkohol og opiat-derivater; d) aldersendringer; e) Alzheimers-sykdom; f) psykiske symptomer ved tobakk-abstinens; g) schizofreni; h) psykiske symptomer ved pre-menstruelt syndrom;
i) reisesyke (Kinetose);
j) høyt blodtrykk;
k) depresjon i kombinasjon med andre preparater

Som ytterligere anvendelse er markering av glioser ved degenerative sykdommer gjennom markert selegilin som markør av gliaceller patentert (US 7 052 921, US 6 853 119). I denne indikasjonen skal det også nevnes diagnosen av gliotiske sentre før kirurgisk fjerning av disse arrdannelsene fra hjernen til epilepsipasien-ter. Dette er imidlertid utelukkende en diagnostisk anvendelse. Forfatterne fast-slår at det finnes ingen henvisning til en patogenetisk vurdering av monoaminoksydase B ved epilepsi, selegilin egner seg bare til markering av gliaceller (Kumlien E. u.a., Epilepsia, Vol. 33, No. 4 1992. 610 ff).
Til behandling av epilepsi er det på markedet en rekke antikonvulsiver, Fenytoin og karbamazepin kan nevnes som viktige representanter. De antikonvulsive midlene som blir anvendt i terapi fører til undertrykkelse av krampene, men har ingen virkning på utvikling av det egentlige epileptiske utgangspunkt.
Oppgaven til foreliggende oppfinnelse er således å tilveiebringe mulighet for fremstilling av et legemiddel med god antikonvulsiv og antiepileptisk virkning.
Det er overraskende funnet at selegilin viser en sterk antikonvulsiv og antiepileptogen virkning. Terskelen for utløsning av fokaler og generaliserte kramper ble hevet tilsvarende sterkt som ved de antikonvulsive standardmidlene karbamazepin og fenytoin.
Foreliggende oppfinnelse omfatter følgelig anvendelse av selegilin eller dens farmasøytisk anvendbare salter til fremstilling av et farmasøytisk preparat til behandling av epileptiske tilstander.
I en foretrukket form av oppfinnelsen gjelder behandlingen særlig undertrykking av kramper og hemming av sykdommens fremdrift.
Krampeterskelen gjelder som det viktigste trekket i en antikonvulsiv virkning. Epilepsi er et terskelfenomen, og en krampe blir hos mennesker utløst gjennom overskridelse av terskelen ved endogen eller eksogen påvirkning. Medikamenter av første valg til behandling av epilepsi forhøyer terskelen, medikamenter av andre valg påvirker ofte ikke terskelen, men reduserer bare krampegraden i det enkelte anfall.
Også i en klassisk epilepsimodell, det maksimale elektrosjokk, kan en virkning av selegilin på terskelen påvises.
På grunn av de farmakologiske undersøkelsene viser selegilin en lignende virkning som medikamenter i første valg.
Hos epileptiske pasienter skjer det over tid ofte en forverring av sykdommen, anfallene blir sterkere og behandlingen har dårligere virkning på pasienten. Av spesielle interesse er det derfor at selegilin også er i den situasjon at den kan forsinke utvikling av epilepsi-utgangspunktet og dermed bremse sykdomsforløpet.
Den antiepileptiske virkningen til N-(l-fenyl-isopropyl)N-metyl-N-propinylamin) er strengt stereospesifikk. Bare L-formen (selegilin) virker antiepileptisk. D-formen er uvirksom. Den utløser i doser, der L-formen er uten bivirkning, sterke amfetaminlignende bivirkninger.
Farmakologiske undersøkelser
Utgangspunkt for undersøkelsene av selegilin i Amygdala-kindling som modell for fokal epilepsi og som modell for epileptogenese var kunnskap om den neurobeskyttende virkningen til selegilin.
I Amygdala-kindling blir det indusert et epileptisk sentrum ved gjentatt meget svak stimulering av et hjerneområde hos rotten via dypelektroder. Dette sentrum er permanent og dyrene, som er fullstendig kindlet ("gekindelt"), kan betegnes som epileptiske. I denne modell kan antikonvulsive midler undersøkes. Blir de gitt fullstendig kindlede dyr og blir det utløst en krampe, da kan terskelen for utløsning av krampe bli forhøyet (den må bli sterkt stimulert for å utløse en krampe) og den utløste krampen kan være svakere.
I denne modellen kan også medikamenter undersøkes, som uavhengig av deres virkning på den utløste krampen, kan undertykke utvikling av det epileptiske sentrum. Den antikonvulsive virkningen er uavhengig av virkningen mot utvikling av epilepsi-sentrum.

Når det oppstår et epileptisk sentrum i kindling fører det til en plastisk forandring i hjernen. For den fullstendige utpregingen av epilepsihjernen er epileptiske utladninger av en bestemt samlet varighet nødvendig.
Under behandling med selegilin (5 og 10 mg/kg, 1 x/dag) blir det brukt 35% og 53% mer stimulering, inntil første generaliserte krampe opptrer. I tillegg er signifikant lengre epileptisk utladning nødvendig for å etablere sentrum (52% og 117%). Dette viser at selegilin forsinker den nødvendige plastiske forandringen i hjernen for etablering av sentrum.
Etter avbryting av behandlingen er varigheten av den epileptiske utladningen, som kunne bli utløst ved fullstendig kindlede dyr, betydelig kortere enn i kontrolldyrene.
Dette viser at det epileptiske sentrum er mindre sterkt utpreget enn i kontrollgruppen.
Selegilin kan således bli anvendt som høyspesifikk virksomt legemiddel for behandling av epileptiske sykdommer. Ikke bare undertrykkes krampene, men i tillegg forsinkes fremdrift av sykdommen.
Forbindelsene ifølge oppfinnelsen og fremgangsmåte for deres fremstilling er kjent.
Forbindelsene kan på kjent måte overføres til vanlige formuleringer som tabletter, kapsler, drageer, piller, granulater, siruper, emulsjoner, suspensjoner og oppløsninger, under anvendelse av inerte, ikke-toksiske, farmasøytisk egnede bærestoffer eller oppløsningsmidler.
Dagdosen av selegilin ved oral eller parenteral anvendelse utgjør 5-20 mg.
Dersom det er nødvendig kan man avvike fra de nevnte mengdene og justere avhengig av kroppsvekt og den spesielle applikasjonsvei.

Anvendelse ifølge krav 1, hvor behandlingen særlig gjelder undertrykking av kramper og hemming av sykdommens fremdrift.
