Description
Den foreliggende oppfinnelse vedrører topiske farmasøytiske preparater, som kan foreligge som oppløsninger, geler, flytende geler, emulsjonsgeler og lotioner inneholdende en allylamin-forbindelse som det farmakologisk aktive middel.
Den foreliggende oppfinnelse tilveiebringer et topisk farma-søytisk preparat, som er kjennetegnet ved at det omfatter som aktivt middel en forbindelse med formel I, som kan foreligge i fri baseform eller syreaddisjonssaltform,
et ikke-ionisk overflateaktivt middel, en C1-C4alkanol og vann, og eventuelt en oljefase.
Den foreliggende oppfinnelse tilveiebringer videre et topisk farmasøytisk preparat i form av en emulsjonsgel eller lotion, som er kjennetegnet ved at det omfatter som aktivt middel en forbindelse med formel I som angitt ovenfor, en oljefase, en C1-C4alkanol og et carbomer-fortykningsmiddel.
Slike preparater kan f.eks. være ikke-fete oppløsninger, emulsjonsgeler eller lotion, som samtlige er flytende eller viskøse.
Eksempler på topiske løsninger omhandlet ved den foreliggende oppfinnelse er oppløsninger i seg selv eller er forstøvnings-væsker, geler eller flytende geler.
Vann er foretrukket tilstede i preparatene i samsvar med oppfinnelsen i konsentrasjoner fra 50 til 85 vekt%.
Forbindelsen med formel I kan f.eks. være i fri baseform eller i syreaddisjonssaltform. En syreaddisjonssaltform kan fremstilles fra den fri baseform på konvensjonell måte og vise versa. Eksempler på passende syreaddisjonssaltformer er hydrokloridet, laktatet og ascorbatet.
Forbindelsen med formel I er kjent fra f.eks. BE-PS-853 97 6 og EP-A-24587. Den hører til klassen av allylamin-antimykotiske midler. Den har det generiske navn terbinafin og er kommer-sielt tilgjengelig under handelsnavnet LAMISIL. Mens terbinafin er meget aktiv både ved topisk og oral tilførsel er det funnet at den eneste topiske sammensetning for omfattende bruk er en krem hvori det aktive middel holdes oppløst i den organiske fase, bare delvis oppfyller pasientenes behov, ettersom den bare er en passende sammensetning for utvalgte sykdomstilstander og hudtyper. En topisk sammensetning i form av et preparat i samsvar med oppfinnelsen er meget ønskelig da den tilbyr flere fordeler, f.eks. med hensyn til påføringsegenskaper, i sammenligning med klassiske sammensetninger som kremer, f.eks.
hurtigere og mer. fullstendig frigivelse av medikamentet
fra bæreren til huden og derfor høyere effektivitet; fravær av fettaktig karakter og ingen rest ved påføring
og derfor økt egnethet for påføring på håret hud; forbedret spredbarhet på huden og således bedre egnethet
for påføring på større nuddeler;
kjølevirkning på huden og derfor bedre egnethet for
påføring på seborroisk hud.
Fremstillingen av topiske preparater som frembyr de ovennevnte egenskaper er imidlertid vanskelig med forbindelsen med formel I på grunn av den lave oppløselighet av medikamentet i vandige systemer, tendensen til forbindelsen når den anvendes i syreaddisjonssaltform eller som en oljebasert emulsjon, en gel eller lotion, til å separere fra mediet som den fri base, som dråper eller endog i krystallinsk form, etter som det er funnet at den fri base er mindre oppløselig i vann enn de tilsvarende syreaddisjons-salter og på grunn av tendensen til medikamentet til å innvirke med anioniske hjelpestoffer. Anvendelse av syre- addisjonssaltformen er ønskelig og er også den foretrukne ut-førelsesform av denne oppfinnelse, på grunn av at oppløseligheten av medikamentet i vandige systemer kan forbedres. Separering av den fri base fra likevekten tilstede i preparater i samsvar med den foreliggende oppfinnelse frembyr imidlertid et alvorlig problem med hensyn til reproduserbarheten av den farmakologiske virkning, og lagringsstabiliteten av et slikt produkt.
De ovennevnte vanskeligheter forsterkes når man forsøker å fremstille enfase-sammensetninger som flytende geler og geler inneholdende forbindelsen med formel I i fri baseform eller salt-form i den samme fase i kombinasjon med de klassiske carbomer-fortykningsmidler (polyakrylsyrederivater) som er et typisk hjelpestoff for geler. Det er iakttatt at der er en innvirkning mellom komponentene som resulterer i et uopp-løselig kompleks eller i krystallisasjon av basen. Der er således et behov for forbedrede topiske former, f.eks. flytende geler og geler.
Som skissert i det foregående kan dette problem ved et aspekt av oppfinnelsen omgås ved å sammensette et emulsjonssystem hvor den fri baseform er oppløst i den oljeaktige fase. Disse systemer kan imidlertid være uegnet for visse hudtyper og sykdomstilstander ettersom det i mange situasjoner foretrekkes en sammensetning som er hovedsakelig fri for fettaktige materialer slik som fettsyrer, fettsyreestere og fettalkoholer som f.eks. polyetylenstearater eller -palmitater, cetylstearater eller -palmitater, stearyl- eller cetylalkoholer. En klar oppløsning er også meget ønskelig som et forbedret topisk preparat. Der er således også et behov for slike forbedrede former som er basert på vann og ikke inneholder oljer eller fettstoffer og hvori den fri base tilstede ved likevekt forblir i oppløsning.
Det er nå funnet at separeringen av den fri base i vannbaserte løsninger kan unngås f.eks. for de topiske oppløsninger ved tilsetning av passende oppløseliggjørende midler i sammensetningene, som er ikke-ioniske, f.eks. at det ikke er noe anionisk overflateaktivt middel tilstede. Disse midler bør foretrukket være overflate-aktive midler som er vannoppløselige eller vannblandbare og forlikelige med den aktive substans og hvilke som helst ytterligere hjelpestoffer tilstede i sammensetningen, og de bør tåles godt på huden.
Et ønskelig trekk ved de angjeldende oppløseliggjørende midler er videre en penetrasjonsøkende virkning for den aktive substans uten å bevirke noen irritasjon på huden.
Eksempler på passende oppløseliggjørende midler for de topiske oppløsninger er: a) Reaksjonsprodukter av en naturlig eller hydrogenert ricinusolje og etylenoksyd. Slike produkter kan oppnås på kjent måte, f.eks. ved omsetning mellom en naturlig eller hydrogenert ricinusolje med etylenoksyd, f.eks. i et molart forhold på fra omtrent 1:35 til omtrent 1:60, med eventuell fjernelse av polyetylenglykolkomponenten fra produktet, f.eks. i samsvar med metodene omhandlet i DOS 1 182 388 og 1 518 819. Særlig egnet er de forskjellige tensider som kan fås under handelsnavnet Cremophor. Særlig egnet er produktene: Cremophor RH 40 med forsåpningstall omtrent 50-60,
syretall < 1, jodtall < 1, vanninnhold (Fischer) < 2%, en
nD<60->verdi omtrent 1,453-1,457 og en HLB på omtrent 14-16; Cremophor RH 60 med forsåpningstall omtrent 40-50,
syretall < 1, jodtall < 1, vanninnhold (Fischer) 4,5-5,5%, en nD<25->verdi på omtrent 1,453-1,457 og en HLB på
omtrent 15-17, og
Cremophor EL med molekylvekt (ved damp-osmometri) omtrent 1630, et forsåpningstall omtrent 65-70, syretall omtrent 2, jodtall omtrent 28-32 og en nD<25->verdi på omtrent 1,471.
Også egnet for bruk innen denne kategori er de forskjellige tensider som fås under handelsnavnet Nikkol, f.eks. Nikkol HCO-60. Nikkol HCO-60 er et reaksjonsprodukt mellom hydro genert ricinusolje og etylenoksyd med følgende egenskaper: syretall 0,3; Forsåpningstall 47,4; hydroksyverdi 42,5; pH (5%) 4,6; farge APHA = 40; Smp =36,0°C; frysepunkt = 32,4°C; H20-innhold (%, KF) = 0,03;
b) polyoksyetylen-sorbitan-fettsyreestere eller poly-sorbater, f.eks. av den type som er kjent og som fås i
handelen under hande ls ena vriene Tween (Fiedler 2., s. 1300-1304) og Armotan (Fiedler _1, s.172) inkluderende produktene Tween 20 [polyoksyetylen(20)sorbitanmonolaurat],
Tween 40 [polyoksyetylen(20)sorbitanmonopalmitat],
Tween 60 [polyoksyetylen(20)sorbitanmonostearat],
Tween 65 [polyoksyetylen(20)sorbitantristearat],
Tween 80 [polyoksyetylen(20)sorbitanmonooleat],
Tween 85 [polyoksyetylen(20)sorbitantrioleat],
Tween 21 [polyoksyetylen(4)sorbitanmonolaurat] ,
Tween 61 [polyoksyetylen(4)sorbitanmonostearat], og

c) Polyoksyetylenfettsyreestere, f.eks. polyoksyetylen-stearinsyreestere av den type som er kjent og fås i handelen
under handels ena vnet Myrj (Fiedler 2_, s. 834-835) såvel som polyoksyetylenfettsyreestere som er kjent og fås i handelen under handelsnavnet Cetiol HE (Fiedler 1, s. 283-284). d) Polyoksyetylen-polyoksypropylen-kopolymerer f.eks. av typen som er kjent og fås i handelen under handelsnavnet
Pluronic og Emkalyx (Fiedler 2.' s. 956-958).
e) Polyoksyetylenfettalkoholetere, f.eks. polyoksyetylenste-aryleter, oleyleter eller cetyleter, f.eks. av den typen som
er kjent under handelsnavnet Brij (Fiedler 1, s. 222-224) f.eks. Brij 78 og 96, og Cetomacrogol 1000 (Fiedler 1, s. 284) .
Foretrukne oppløseliggjørende midler er midlene under a), b), c) og e), særlig Cetomacrogol 1000, Cremophor RH40 og Tween 20. Særlig foretrukket er Cetomacrogol 1000.
Forbindelsen med formel I og det oppløseliggjørende middel er foretrukket tilstede i den topiske oppløsningssammensetning i mengdeforhold fra 1:0,5 til 1:15, foretrukket fra 1:1 til 1:10 på en vekt/vektbasis. Forbindelsen med formel I utgjør foretrukket fra 0,1% til 5%, og foretrukket fra 0,5% til 2% av det totale preparat på en vektbasis.
I en form omfatter de topiske oppløsninger i samsvar med oppfinnelsen den aktive substans, det ikke-ioniske overflate-aktive middel, en C1-C4alkanol og vann. C1-C4alkanoler i oppfinnelsens sammenheng er fysiologisk holdbare C1-C4alkoholer, f.eks. isopropanol eller foretrukket etanol. Vektkonsentrasjonen av alkanol i preparatet kan f.eks. være fra 5% til 90%, f.eks. fra 5% til 35%. En typisk konsen-trasjon er fra 25% til 45% for et flytende preparat, f.eks. fra 25 til 35%, og for et viskøst preparat, f.eks. en gel, fra 5 til 15%, f.eks. 10%.
Oppløsningene kan fylles i konvensjonelle glassflasker med dråpeinnretning eller i mer kompliserte innretninger, f.eks. plastflasker eller plastflasker med forstøvningsinnretning.
På grunn av lettere påføring kan en fortykket oppløsning være ønskelig, f.eks. en flytende gel eller en transparent gel. Dette kan oppnås ved å tilsette konvensjonelle fortykningsmidler til de ovenfor beskrevne oppløsninger.Egnede komponenter inkluderer f.eks.: polymetylakrylatharpikser, f.eks. av den type som er
kjent og fås i handelen under handelsnavnet Eudispert (Fiedler 1, s. 485-486);
cellulosederivater inkluderende f.eks. etyl-, propyl-,
metyl- og hydroksypropylmetyl-cellulose;
polyvinylharpikser, f.eks. inkluderende polyvinyl-alkoholer og polyvinylpyrrolidoner, såvel som andre polymere materialer inkluderende gelatin, alginater,
pektiner, tragantgummi, gummi arabicum og xantangummi; materialer som silikagel, bentonitt og magnesium-aluminiumsilikat.
Disse komponenter, når de er tilstede, er passende tilstede i en mengde på opptil 20%, mer foretrukket opptil 10%, basert på den totale vekt av preparatet. Mest passende er de tilstede i en mengde på fra 0,5% til 15%, f.eks. fra 1,0% til 3,0% basert på den totale vekt av preparatet.
De topiske oppløsningepreparater i samsvar med oppfinnelsen kan oppnås ved hjelp av en metode omfattende at forbindelsen med formel I i fri baseform eller i syreaddisjonssaltform sammen med det ikke-ioniske overflateaktive middel oppløses i en passende bærer og ytterligere hjelpestoffer tilsettes etter behov. Hvis en fortykket oppløsning eller en gel er ønsket tilsettes fortykningsmiddelet på konvensjonell måte til systemet. Fremstillingen kan gjennomføres på konvensjonell måte.
Ved et ytterligere aspekt av oppfinnelsen tilveiebringes stabile emulsjonsgeler og lotioner. Når en emulsjonsgel eller en lotion basert på et carbomer-fortykningsmiddel er nødvendig for å tilføre forbindelsen med formel I på huden, er de problemer som er forbundet med den gjensidige innvirkningen mellom den fri base og den anioniske polymer i prinsippet de samme som med de ovennevnte vandige løsninger. Det er nå funnet at disse gjensidige innvirkninger kan unngås og stabile emulsjonsgeler og lotioner kan oppnås når en oljeaktig fase, som f.eks. isopropylmyristat, tilsettes til forbindelsen med formel I, foretrukket i fri baseform, og carbomer. Dette resulterer i stabile emulsjonsgeler og lotioner som har alle de gunstige egenskaper av en gel og unngår den gjensidige innvirkning av det aktive middel med carbomer.
I emulsjonsgeler og lotioner i samsvar med oppfinnelsen kan forbindelsen med formel I og en oljefase være tilstede i emulsjonsgelen og lotioner i mengdeforhold fra 1:5 til 1:40, foretrukket fra 1:7 til 1:20 på en vekt/vektbasis. Oljefasen er foretrukket isopropylmyristat. Den er foretrukket tilstede i en vektkonsentrasjon på 10%.
Forbindelsen med formel I utgjør foretrukket fra 0,1% til 5%, mer foretrukket fra 0,5% til 3%, av det totale preparat på en vektbasis. Foretrukket er mengden av C1_C4-alkanol, vann og oljefase, når denne er tilstede, fra 83 til 9 6 vekt% av preparatet. Konvensjonelle hjelpestoffer er spesielt fortykningsmidler som carbomerer (polyakrylsyrederivater) som er kjent og fås i handelen under handelsnavnet Carbopol (Fiedler 1, s. 206-207), f.eks. Carbopol 934 P eller Carbopol 1342. Ytterligere hjelpestoffer er f.eks. emulgeringsmidler som sorbitan monolaurat (Span 20) og polysorbat 20 (Tween 20). Det er imidlertid iakttatt at med forbindelsen med formel I stabiliserer carbomerene emulsjonen i den grad at bare lite eller ikke noe emulgeringsmiddel overhodet er nødvendig for å oppnå stabile emulsjonsgeler og lotioner, særlig når carbomeren er Carbopol 1342.
De resulterende emulsjonsgeler og lotioner har forbedrede kosmetiske egenskaper, som lettet utspredning på huden og fravær av fettkarakter. På grunn av den reduserte mengde eller fravær av konvensjonelle emulgeringsmidler har de også forbedrede farmakologiske egenskaper, særlig bedre tålbarhet når de påføres på infisert og ofte inflammert hud. En ytterligere fordel ved den foreliggende oppfinnelse er at utseende og konsistens av det endelige preparat fritt kan reguleres ved å variere mengden av fortykningsmiddel (carbomer) i preparatet.
Emulsjonsgelene og 'lotionene i samsvar med oppfinnelsen kan oppnås ved hjelp av en metode omfattende at en forbindelse med formel I i fri baseform eller i syreaddisjonssaltform og ytterligere hjelpestoffer etter behov, oppløses f.eks. i en passende oljefase som isopropylmyristat.
Oljefasen kan emulgeres med en passende vannfase og deretter innlemmes i et forhåndsfremstilt gelkonsentrat inneholdende carbomeren og ytterligere hjelpestoffer etter behov. Denne fremstillingsmåte unngår den gjensidige innvirkning mellom forbindelsen med formel I og carbomeren under fremstillingen. Foretrukket nøytraliseres carbomeren før den blandes med oljefasen.
Preparatene kan inneholde ytterligere bestanddeler, f.eks. kompleksdannende midler, f.eks. etylendiamin-tetraacetat
(dinatriumsalt) ,
aromastoffer,
fargestoffer.
Preparatene i samsvar med oppfinnelsen kan også inneholde konvensjonelle tilsetningsmidler for å innstille pH-verdien til en passende verdi for hudbehandling. Ytterligere kan preparatene inneholde konserveringsmidler og/eller anti-oksydasjonsmidler, f.eks. i en mengde på 0,05 til 1 vekt% av den totale vekt av preparatet, f.eks. ascorbylpalmitat, natriumpyrosulfitt, butylhydroksyanisol (BHA), butyl-hydroksytoluen (BHT) , tokoferoler, f . eks. cc-tokoferol (vitamin E), benzylalkohol, propyl-eller metyl-p-hydroksybenzoater.
Preparatene i samsvar med oppfinnelsen er nyttige for de samme indikasjoner som er kjent for andre topiske preparater, f.eks. fungale infeksjoner og i de samme doseringer, f.eks. som bekreftet ved hjelp av standard kliniske forsøk. Typiske effektive doseringer oppnås når konsentrasjonen av det aktive middel i det behandlede hudvev er mellom 10 og 10.000 ng pr.cm<2>. Foretrukne hudvevkonsentrasjoner er mellom 500 og 2 000 ng pr.cm<2>, f.eks. 1000 ng pr.cm<2>, f.eks. som indikert ved standard farmakologiske tester. Høyere og lavere doseringer kan imidlertid være effektive og kan bestemmes ved hjelp av standardprøver. F.eks. kan de effektive konsentrasjoner i den behandlede hud oppnås ved påføring av f.eks. forbindelsen med formel I i form av f.eks. et 1% preparat i henhold til oppfinnelsen over det infiserte området, f.eks. 5 mg aktivt middel pr. dag på et hudområde på omtrent 100 cm<2.>De etterfølgende eksempler illustrerer oppfinnelsen. Alle temperaturer er i grader Celsius (r.t. = romtemperatur).

En farmasøytisk tålbar emulsjonsgel oppnås fra de ovennevnte bestanddeler når fremstillingsprosessen gjennomføres med de følgende trinn: I. A, B, C, D, E og F blandes sammen med forsiktig opp-varming inntil alle faste partikler er oppløst; II. i en passende beholder eller prosessor inneholdende et røreverk og en homogenisator oppvarmes omtrent halvparten av vannet til 60-70°C; III. I tilsettes sakte til II under omrøring og homogenisering inntil det oppnås en homogen emulsjon med passende dråpestørrelse. Den konsentrerte emulsjon avkjøles så til r.t.; IV. i en separat beholder fremstilles en basisk carbomergel ved å dispergere carbomeren i H og den andre halvdel av vannet og nøytralisering med I; V. den basiske emulsjon III tilsettes til den basiske gel og hele blandingen omrøres ved r.t. inntil en homogen emul-sjons gel er oppnådd.
Eksempel 5; Emulsjonsgel 1 %
En farmasøytisk tålbar emulsjonsgel oppnås fra de ovennevnte bestanddeler når fremstillingsprosessen gjennomføres med følgende trinn: I. A, B, C, D, E og F blandes sammen med forsiktig opp-varming inntil alle faste partikler er oppløst; II. i en passende beholder eller prosessor inneholdende et røreverk og en homogenisator oppvarmes omtrent halvparten av vannet til 60-70°C; III. I tilsettes sakte til II under omrøring og homogenisering inntil det oppnås en homogen emulsjon med passende dråpestørrelse. Den konsentrerte emulsjon avkjøles så til r.t.; IV. i en separat beholder fremstilles en basisk carbomergel ved å dispergere carbomeren i H og den andre halvdel av vannet og nøytralisering med I; V. den basiske emulsjon III tilsettes til den basiske gel og hele blandingen omrøres ved r.t. inntil en homogen emul-sjons gel er oppnådd.
Eksempel 6; Lotion 1 %
En farmasøytisk tålbar lotion oppnås fra de ovennevnte bestanddeler når fremstillingsprosessen gjennomføres med følgende trinn: I. A oppløses i B ved romtemperatur; II. i en passende prosessor inneholdende et røreverk og en effektiv homogenisator oppløses C og D i E under opp-varming opp til 90°C. Oppløsningen avkjøles så til omtrent 30-40°C; III. F dispergeres i II. Den homogene dispersjon nøytraliseres så med G og dette resulterer i en
opalescerende fortykket løsning;
IV. den organiske løsning I blir så emulgert inn i den for-tykkede oppløsning III ved omrøring og homogenisering inntil det oppnås en lotion med passende dråpestørrelse
(2 til 2 0 fim) ;
V. til slutt tilsettes H til IV og hele blandingen omrøres
til det ferdige produkt er oppnådd.
Claims (17)
1. Topisk farmasøytisk preparat,
karakterisert ved at det omfatter som aktivt middel en forbindelse med formel I, som kan foreligge i fri baseform eller syreaddisjonssaltform,
et ikke-ionisk overflateaktivt middel, en C1 -C4 alkanol og vann, og eventuelt en oljefase.
2. Preparat som angitt i krav 1,
karakterisert ved at det inneholder fra 50 til 85 vekt% vann.
3. Preparat som angitt i ett eller flere av de foregående krav,
karakterisert ved at det inneholder fra 5 til 3 5 vekt% av alkanolen.
4. Preparat som angitt i krav 3,
karakterisert ved at alkanolen er etanol.
5. Preparat som angitt i ett eller flere av de foregående krav,
karakterisert ved at det har form av en opp-løsning, en lotion, en gel eller en flytende gel.
6. Preparat som angitt i ett eller flere av de foregående krav,
karakterisert ved at det ikke-ioniske overflateaktive middel er en polyoksyetylen-fettalkoholeter.
7. Preparat som angitt i ett eller flere av de foregående krav,
karakterisert ved at forholdet mellom forbindelsen med formel I og det ikke-ioniske overflateaktive middel er fra 1:0,5 til 1:15.
8. Preparat som angitt i ett eller flere av de foregående krav,
karakterisert ved at det inneholder isopropylmyristat som en oljefase.
9. Preparat som angitt i ett eller flere av de foregående krav,
karakterisert ved at forholdet mellom forbindelsen med formel I og oljefasen er fra 1:5 til 1:40 på en vekt/vektbasis.
10. Preparat som angitt i ett eller flere av de foregående krav,
karakterisert ved at forbindelsen med formel I er til stede i en mengde på 0,1% til 5% av den totale vekt av preparatet på en vekt/vektbasis.
11. Topisk farmasøytisk preparat i form av en emulsjonsgel eller lotion,
karakterisert ved at det omfatter som aktivt middel en forbindelse med formel I som angitt i krav 1, en oljefase, en C1 -C4 alkanol og et carbomer-f ortyknings-middel .
12. Preparat som angitt i krav 11, karakterisert ved at oljefasen er isopropylmyristat.
13. Preparat som angitt i krav 11 eller 12, karakterisert ved at forbindelsen med formel I og oljefasen er tilstede i et forhold fra 1:5 til 1:40 på en vekt/vektbasis.
14. Preparat som angitt i krav 11, 12 eller 13, karakterisert ved at forbindelsen med formel I er tilstede i en mengde fra 0,1 % til 5 % på en vektbasis.
15. Preparat som angitt i ett eller flere av kravene 11 - 14,
karakterisert ved at det ytterligere omfatter vann, hvori mengden av alkanol, vann og oljefase er fra 83 vekt% til 96 vekt% av preparatet.
16. Preparat som angitt i ett eller flere av kravene 11 - 15,
karakterisert ved at alkanolen er etanol.
17. Preparat som angitt i ett eller flere av kravene 11 - 16,
