Description
Foreliggende oppfinnelse angår en analogifremgangsmåte ved fremstilling av nye, terapeutisk aktive spiro-(1,3-dioxolan-4,3<1>)-kinuclidiner med den generelle formel:
hvor R er hydrogen, alkyl med 1-4 C-atomer eller fenyl og R2 er alkyl med 1-4 C-atomer eller fenyl, og isomere og fysiologisk godtagbare salter av disse.
Forbindelsene med formel I kan fremstilles ved en ringslut-ningsreaksjon. To synteseveier for fremstilling av slike forbindelser er: a. Fremstilling først av 3-hydroxymethyl-3-kinuclidinol ved omsetning av 3-carbomethoxy-3-kinuclidinol med et reduksjonsmiddel, som metallhydrider (f.eks. lithiumaluminiumhydrid) i et passende reaksjonsmedium, som en høy ether (f.eks. i tetrahydrofuran), under tilbakeløp; reaksjonsblandingen behandles med et lavere alkylacetat (som methylacetat eller ethylacetat) i en inert atmos-fære, fulgt av behandling med en vandig base (som natriumhydroxyd), fulgt av vasking med vann for å få den ønskede 3-hydroxymethyl-3-kinuclidinol.
b. Fremstilling først av kinuclidin-3-epoxyd ved omsetning av dimethylsulfoxonium-methylid med kinuclidin-3-on.
Produktet fra det ene eller det annet av de ovenstående re-aksjonstrinn (a) og (b) omsettes så med et aldehyd (R^ -CHO) eller med et keton (R-^ -CO-R2) , hvor R^ og R_ er som ovenfor angitt, for å få den ønskede forbindelse med formel I. Sluttelig kan en erholdt isomerblanding eventuelt spaltes i sine isomere, og/eller en erholdt forbindelse av formel (I) isoleres i den frie form eller i form av et fysiologisk godtagbart salt.
De nye forbindelser med formel I kjennetegnes ved inter-essante og verdifulle farmakologiske egenskaper. De kan anvendes som aktive bestanddeler av farmasøytiske preparater for anvendelse i human- bg veterinærmedisin.
Eksempelvis er forbindelsen 2-methy1-spiro-(1,3-dioxolan-4,3')-kinuclidin en spesifikk og sterkt cholinerg stimulant, og den har en høy grad av spesifisitet mot autonom ganglion- og sentralmuscarine reseptorer beliggende i de perifere sympatiske ganglia eller i sentralnervesystemet. På grunn av sin høye grad av spesifisitet for disse områder kan den anvendes for å akti-vere disse reseptorer, under tilstander hvor acetylcholin mangler i områdene.
Blant tilstander i hvilke denne forbindelse kan anvendes, kan nevnes dem som krever behandling med acetylcholin-lignende droger som Huntingtons chorea, tardiv dyskinesia eller hyper-kinesia og også i mentale forstyrrelser på grunn av mangel
på sentral-acetylcholin som fører til mentale forstyrrelser. Slike mangler kan vær|e spontane eller de kan være drogeinduserte. Ovenstående forbindelse er også nyttig ved behandling av lidelser som krever anvendelse av et lengevarende cholinergt middel med mild lokal aktivitet. Et slikt middel er nødvendig ved lidelser som glaucoma da forbindelsen ikke ødelegges av
i
enzymet som desaktiveirer acetylcholin.
Forbindelsen 2,2-difenyl-spiro-(1, 3-dioxolan-4,3')-kinuclidin er et sterkt anticholinergt middel, og det kan anvendes ved behandling av lidelser som skyldes et overskudd av acetylcholin, enten dette er spontant eller drogeindusert. Den er nyttig ved behandling<!>av forskjellige sykdommer som Parkinsons sykdom, ved mental depresjon, og den kan også anvendes som hjelpemiddel i kirurgi istedenfor atropin, scopolamin, etc.
Den kan også anvendes!i ofthalmologi når langvarig mydriasis kreves for diagnostiske eller for terapeutiske formål.
Fremstillingen av de nye forbindelser illustreres i de følgende eksempler.
Eksempel 1
Fremstilling av 2- methyl- spiro-( 1, 3- dioxolan- 4, 3')- kinuclidin
Til en omrørt suspensjon av 17,5 9 lithiumaluminiumhydrid i lOO ml tørt tetrahydrofuran tilsettes i løpet av 1 time en oppløsning av 50 g 3-carbomethoxy-3-kinuclidinol i 100 ml tetrahydrofuran. Denne ble fremstilt i henhold til Grob. Heiv.
Chim. Acta, 37, l689 (1954)- Reaksjonsblandingen ble kokt under tilbakeløp i 4,5 timer, behandlet med 20 ml ethylacetat under nitrogen, derpå med 17 ml vann, derpå med 17 ml 15%-ig vandig natriumhydroxyd, og igjen med 51 ml vann. Den dannede blanding ble filtrert, og filtratet ble inndampet under nedsatt trykk, hvilket ga et residuum i form av en sirup inneholdende 3_ hydroxymethyl-3-kinuclidinol. En mengde på 5 9 av denne sirup ble behandlet med 20 ml acetaldehyd og 20 ml methylenklorid.
Den dannede emulsjon ble avkjølt til 0°C og behandlet under om-røring med 20 ml bortrifluorid-etherat i 1 time. Den dannede blanding ble så helt i vann inneholdende et overskudd av kaliumhydroxyd og omrørt inntil en fullstendig spaltning av bortrifluoridet hadde funnet sted. Blandingen ble ekstrahert med ether, og ekstraktet ble underkastet inndampning under nedsatt trykk. Residuet besto av rått 2-methyl-spiro-(l,3-dioxolan-4,3')-kinuclidin, kokepunkt 68 - 72°C ved 0,2 mmHg,
R^. på nøytralt aluminiumoxyd (ethylacetat) = 0,6; M<+> -183-Dette produkt besto av en blanding av de to isomerer hvor methylgruppen er i ett tilfelle i cis-stilling (70%), i det annet i trans-stilling (30%) i forhold til nitrogenet i ringen. Blandingen ble spaltet i de to isomerer ved kolonnekromatografi på nøytralt aluminiumoxyd med ethylacetat. Hydrokloridene av disse ble fremstilt ved å behandle de to forbindelser i tørt a'ceton med gassformig hydrogenklorid, smeltepunkt for cis-isomeren 233,8°C (spaltn.), og for blandingen: 234,2°C (spaltn.).
Eksempel 2
2, 2- dimethyl- spiro-( 1, 3- dioxolan- 4, 3•)- kinuclidin
Dimethylsulfoxonium-methylid ble fremstilt ved metoden ifølge Corey et al., Organic Synth. 49, 78 (1969) fra 17,2 g natriumhydrid, 88 9 trimethylsulfoxoniumjodid i 455 ml dimethylsulfoxyd i en 1 liters reaksjonskolbe forsynt med mekanisk rører, tilbakeløpskjøler og gassinnløpsrør, beskyttet mot fuk-tighet. Efter avslutning av reaksjonen ble gassinnløpsrøret erstattet med en trykkompensert dråpetrakt inneholdende 29 g kinuclidin-3-on i 129 'ml tørt dimethylsulfoxyd. Dette ble til-satt til dimethylsulfdxonium-methylidet i løpet av 5 minutter fulgt av omrøring i ytterligere 15 minutter ved den omgivende temperatur, fulgt av oppvarmning til 55 - 60°C i 2 timer på vannbad. Reaksjonsblandingen ble helt i 200 ml koldt vann og ekstrahert med 5 x 200 ml benzen. Ekstraktene ble forenet, vasket med 100 ml vann, med ICO ml mettet vandig natriumklorid, tørret over vannfritt magnesiumsulfat og inndampet, hvilket ga rått kinuclidin-3-epoxyd, kokepunkt 55-60°C ved 0,5 mmHg. Utbyttet var 21 g, dvs. et utbytte på 49%. R^ på nøytralt aluminiumoxyd (ethylacetat) 0,3; M = 139; hydrokloridsaltet: 200,7 - 202°C.
3,5 g av epoxydejt ble behandlet med 50 ml aceton og 30 ml bortrifluorid-etherat ved 0°C under omrøring. Omrøringen ble fortsatt i 48 timer ved værelsetemperatur, og derefter ble overskudd bortrifluorid spaltet ved å helle reaksjonsblandingen i koldt vann inneholdende overskudd av kaliumhydroxyd. Den organiske fase ble ekstrahert med 100 ml ether, fraskilt og tørret over vannfritt magnesiumsulfat. Efter fordampning av oppløsningsmidlet fikk man et klart sirupsaktig stoff, 5 g, 2,2-dimethyl-spiro-(1,3~dioxolan-4,3')-kinuclidin, kokepunkt: 6o - 70°C ved 1 mmHg, utbytte 4 g (80%); hydrokloridsaltet: smeltepunkt: 245 - 245,5°C.
Eksempel 3
2, 2- difenyl- spiro-( 1, 3- dioxolan- 4, 3')- kinuclidin
Denne forbindelse ble fremstilt ved begge av veiene i eksempel 1 og eksempel 2 under anvendelse av benzofenon. Smeltepunktet var 105,5 - 105,6°C. Utbyttet var IO - 20%. Hydrokloridsaltet smeltet ved 204 - 206°C. R, ved tynnskikts-kromatografi på aluminiumoxyd med kloroform: 0,4 (fri base).
Forskjellige andre forbindelser innen formel I ble fremstilt på disse måter, enten via 3-hydroxymethyl-3-kinuclidinol eller kinuclidin-3-epoxyd ved omsetning med et aldehyd R-^-GHO eller med et keton R^CO-R^ hvor R og R2 er som angitt for formel I.
Forbindelsene med formel I hvor R1 er forskjellig fra R2 , foreligger som to stereoisomerer, nemlig en cis- og en trans - isomer. Disse kan spaltes ved konvensjonelle midler, som f.eks. ved kolonnekromatografi. Forbindelsen hvor R1 er methyl og R2 er hydrogen, ble spaltet, og det viste seg at blandingen besto av ca. 30 vekt% trans- og ca. 70% cis-isomer. Cis-isomeren har større biologisk aktivitet, og i de følgende tabeller er aktiviteten av den i det vesentlige rene cis-isomer av denne forbindelse angitt.
Fremgangsmåteforbindelsene kan opparbeides til farmasøy-tiske preparater ved konvensjonelle metoder og ved å anvende konvensjonelle fortynningsmidler, hjelpestoffer og lignende.
De kan administreres pr. os, ved injeksjon, ved. inf us jon, etc . Dosen av forbindelsen med formel I hvor R^ er methyl, R^ er hydrogen, er ca. 0,2 mg til 3,0 mg/kg pr. dag for voksne, hvis gitt ved injeksjon, og den orale dose må være ca. 2 til 3 ganger så stor. Difenylforbindelsen skal ved parenteral administrasjon gies i doser på fra ca. 1 til 5 mg pr. dag for voksne. Difenylforbindelsen kan effektivt anvendes i form av ofthalmologiske preparater, sammen med passende hjelpestoffer, puffere eller lignende, og preparater med 0,5 til ca. 2 vekt% gir tilfreds-stillende resultater.
Det er klart at dosen og administrasjonsmåten må avpasses i henhold til sykdommen eller lidelsen som skal behandles, og i henhold til alvorligheten av denne.
De følgende tabeller indikerer noen av anvendelsene av de nye forbindelser og av farmasøytiske preparater som inneholder disse, og de er sammenlignet med noen konvensjonelle forbindelser anvendt for lignende anvendelser.
Claims (1)
- Analogifremgangsmåte ved fremstilling av terapeutisk aktive spiro-(1,3-dioxolan-4,3<1>)-kinuclidiner med den generelle formel:hvor er hydrogen, alkyl med 1-4 C-atomer eller fenyl, og R2 er alkyl med 1-4 C-atomer eller fenyl,og isomere og fysiologisk godtagbare salter av disse, karakterisert ved at 3-carbomethoxy-3-kinuclidinol omsettes med lithiumaluminiumhydrid, den dannede forbindelse behandles med ethylacetat, med vann, med en vandig alkalisk oppløsning, og med vann for å få 3-hydroxymethyl-3-kinuclidinol, som omsettes med et aldehyd R^-CHO eller med et keton R^-CO-R2, hvor R^ og R2 har de ovenfor angitte betydninger, eller dimethylsulfoxonium-methylid omsettes med kinuclidin-3-on til kinuclidin-3-epoxyd, som omsettes med et aldehyd R^-CHO eller med et keton R^-CO-R2, hvor R^ og R2 har de ovenfor angitte betydninger,og at en erholdt isomerblanding eventuelt spaltes i sine isomere, og/eller en erholdt forbindelse av formel (I) isoleres i den frie form eller i form av et fysiologisk godtagbart salt.
