Abstract
Description
Invenţia se referă la medicină, în special la oftalmologie. The invention relates to medicine, in particular to ophthalmology.
Este cunoscut dispozitivul pentru tratamentul cataractei senile, care include o seringă cu ac executat din două segmente, unul proximal cu lungimea de 9 mm este îndoit sub un unghi de 135° faţă de axa longitudinală a seringii, iar unul distal cu lungimea de 3 mm este perpendicular pe axa menţionată, totodată metoda de tratament al cataractei senile constă în aceea că se efectuează o incizie la nivelul limbului, se deschide camera anterioară şi se introduce aer steril cu ajutorul dispozitivului menţionat mai sus, apoi cu vârful acului dispozitivului se efectuează capsulotomia anterioară circulară, se introduc 0,3 ml de aer steril sub capsula restantă a cristalinului şi cu vârful acului dispozitivului se divizează nucleul în două fragmente, după care se implantează cristalinul artificial [1]. Dezavantajul dispozitivului cunoscut constă în imposibilitatea mobilizării nucleului cristalinului, în cazul când acesta este fixat de capsula posterioară, fapt ce poate duce la pierdere de corp vitros în timpul intervenţiei, contribuind la declanşarea unui glaucom secundar sau la o iridociclită în perioada postoperatorie, totodată utilizarea acestui dispozitiv exercită presiuni asupra structurilor intraoculare. Metoda de tratament al cataractei senile hipermature constă în aceea că se efectuează anestezia locală şi regională, se fixează muşchiul rect superior, se efectuează o incizie a conjunctivei la nivelul limbului, ce corespunde orelor 10°°…14°° şi a corneei ce corespunde orelor 11°°…13°°, se deschide camera anterioară şi se efectuează capsulorexusul anterior la nivelul orei 11°°, cu ajutorul dispozitivului conform invenţiei se mobilizează nucleul, după care el se fixează şi se înlătură prin mişcări de rotaţie în direcţia acelor de ceasornic, apoi se implantează cristalinul artificial, se suturează corneea şi conjunctiva, iar în spaţiul subconjunctival se introduce un antibiotic şi un remediu steroid. Rezultatul constă în simplitatea intervenţiei chirurgicale de extracţie a nucleului cristalin, ceea ce permite efectuarea unei incizii mai mici a limbului. Dispozitivul pentru tratamentul cataractei senile hipermature este simplu în executare şi constă dintr-o seringă cu ac bont, îndoit sub un unghi de 135° faţă de axa longitudinală a seringii, lungimea acului fiind de 10 mm. Metoda revendicată la fel este simplă în efectuare şi nu este costisitoare. Metoda de tratament al cataractei senile se realizează în modul următor: se efectuează anestezia locală şi regională, se fixează muşchiul rect superior, se efectuează o incizie a conjunctivei la nivelul limbului, ce corespunde orelor 10°°…14°° şi a corneei ce corespunde orelor 11°°…13°°, se deschide camera anterioară şi se efectuează capsulorexusul anterior la nivelul orei 11°°. Cu ajutorul dispozitivului conform invenţiei se mobilizează nucleul, după care el se fixează şi se înlătură prin mişcări de rotaţie în direcţia acelor de ceasornic, apoi se implantează cristalinul artificial, se suturează corneea şi conjunctiva, iar în spaţiul subconjunctival se introduce un antibiotic şi un remediu steroid. Pacientul A., în vârstă de 79 de ani, cu diagnosticul clinic de cataractă senilă hipermatură la ochiul drept (OD). Visus OD = 1/ωpr L certa. Patient A., 79 years old, with a clinical diagnosis of hypermature senile cataract in the right eye (OD). Visus OD = 1/ωpr L certa.
A fost efectuată o intervenţie chirurgicală de înlăturare a cataractei senile hipermature conform metodei solicitate şi cu ajutorul dispozitivului revendicat, după care s-a obţinut o detaşare uşoară a nucleului cristalinian şi o poziţionare corectă a cristalinului artificial de cameră posterioară, acuitatea vizuală (OD) la externare fiind de 0,7 (fără corecţie). Acuitatea vizuală a fost aceeaşi la un an după intervenţia microchirurgicală. Pacientul B., în vârstă de 85 de ani. Diagnosticul clinic: Cataractă senilă hipermatură la ochiul stâng (OS). Visus OD = 1/ωpr L certa. Patient B., 85 years old. Clinical diagnosis: Hypermature senile cataract in the left eye (OS). Visus OD = 1/ωpr L certa.
A fost efectuată o intervenţie chirurgicală de înlăturare a cataractei senile hipermature conform metodei solicitate şi cu ajutorul dispozitivului revendicat, după care s-a obţinut o detaşare uşoară a nucleului cristalinian de capsula cristaliniană posterioară şi o poziţionare corectă a cristalinului artificial de cameră posterioară, acuitatea vizuală (OS) la externare fiind de 0,7 (fără corecţie). Acuitatea vizuală a fost aceeaşi la un an după intervenţia microchirurgicală. Metoda dată a fost aplicată la 30 de pacienţi cu cataractă senilă hipermatură, operaţi în Clinica de Oftalmologie Universitară a Universităţii de Stat de Medicină şi Farmacie N. Testemiţanu a ISMP SCR. Complicaţii primare şi tardive nu s-au evidenţiat. This method was applied to 30 patients with hypermature senile cataract, operated on in the University Ophthalmology Clinic of the N. Testemiţanu State University of Medicine and Pharmacy of ISMP SCR. No primary and late complications were noted.
