Abstract

The invention relates to medicine, in particular to ophthalmology and can be used for treating senile cataract.The device for treating senile cataract comprises a syringe with blunt-pointed needle made of two segments bent at an angle of 90°, the proximal segment has a length of 9 mm and the distal one - of 2 mm.The method for treating senile cataract consists in that it is carried out the local and regional anesthesia, is applied the blepharostat, is fixed the superior rectus muscle, is performed an incision of conjunctiva at the limbus level, which corresponds to 10.00…13.00 o'clock, is performed the incision of cornea at the level of 10.00…13.00 o'clock, is performed the anterior capsulorhexis, is removed the nucleus and lens masses by means of said device, is performed the circular dilatation of pupil up to the periphery, are aspirated the residual lens masses, is implanted the artificial lens behind the posterior chamber and is sutured the cornea and conjunctiva.

Invenţia se referă la medicină, în particular la oftalmologie şi poate fi aplicată pentru tratamentul cataractei senile. The invention relates to medicine, in particular to ophthalmology and can be applied for the treatment of senile cataract.

Este cunoscut un dispozitiv pentru tratamentul cataractei senile, care include o seringă cu ac executat în părţile proximală şi distală în două trepte perpendiculare pe axa longitudinală a seringii, treapta proximală fiind de 2 mm, iar cea distală de 1 mm [1]. Este cunoscută o metodă de extracţie extracapsulară utilizată în tratamentul cataractei senile, care constă din următoarele etape tradiţionale: anestezia topică şi aseptizarea câmpului operator, anestezia regională, aplicarea blefarostatului, fixarea muşchiului rect superior, incizia conjunctivei de la limb la nivelul orelor 10.00…13.00, incizia corneei la nivelul orelor 10.00…13.00, deschiderea camerei anterioare, efectuarea capsulorexisului anterior, înlăturarea nucleului cristalinului, lavajul maselor cristaliniene, implantarea cristalinului artificial de camera posterioară, suturarea definitivă a corneei şi conjunctivei. Apoi se introduce în spaţiul subconjunctival un antibiotic şi un preparat corticosteroid, se eliberează muşchiul rect superior şi se aplică un pansament monocular aseptic [2]. Dezavantajele dispozitivului şi metodei de tratament al cataractei senile constau în imposibilitatea controlului maselor cristaliniene restante şi aspiraţia ulterioară a acestora. The disadvantages of the device and method of treating senile cataracts consist in the impossibility of controlling the remaining crystalline masses and their subsequent aspiration.

Problema pe care o rezolvă invenţia este elaborarea unui dispozitiv şi a unei metode noi pentru tratamentul cataractei senile, care să permită controlul maselor cristaliniene restante cu aspiraţia ulterioară a acestora. Esenţa invenţiei constă în aceea că dispozitivul pentru tratamentul cataractei senile include o seringă cu ac bont executat din două segmente unite sub un unghi de 90º, segmentul proximal are lungimea de 9 mm, iar cel distal - de 2 mm, iar metoda de tratament al cataractei senile constă în aceea că se efectuează anestezia locală şi regională, se aplică blefarostatul, se fixează muşchiul rect superior, se efectuează o incizie a conjunctivei la nivelul limbului, ce corespunde orelor 10.00…13.00, se efectuează incizia corneei la nivelul orelor 10.00…13.00, se efectuează capsulorexisul anterior, se înlătură nucleul şi masele cristaliniene cu ajutorul dispozitivului menţionat, se dilată pupila circular până la periferie, se aspiră masele cristaliniene restante, se implantează cristalinul artificial după camera posterioară şi se suturează corneea şi conjunctiva. Avantajele metodei propuse constau în aceea că se poate efectua dilatarea pupilei şi depistarea maselor cristaliniene restante cu aspiraţia ulterioară a acestora. The advantages of the proposed method are that it can dilate the pupil and detect residual lens masses with their subsequent aspiration.

Rezultatul constă în faptul că prin utilizarea dispozitivului şi metodei propuse se asigură realizarea operaţiei într-un mod miniinvaziv, rapid, efectiv şi necostisitor. Pacientul X., în vârstă de 62 ani. Diagnosticul clinic: OD - Cataractă legată de vârsta matură. Patient X., 62 years old. Clinical diagnosis: OD - Age-related cataract.

Acuitatea vizuală (Visus OD = 1pr.L. certa). Visual acuity (Visus OD = 1pr.L. certa).

După efectuarea intervenţiei microchirurgicale conform metodei propuse şi implantarea cristalinului artificial, acuitatea vizuală (OD) la externare a fost de 0,6 (cu corecţie). La un an după operaţie acuitatea vizuală a fost de 0,9 (cu corecţie). După efectuarea intervenţiei microchirurgicale conform metodei propuse şi implantarea cristalinului artificial, acuitatea vizuală (OS) la externare a fost de 0,7 (cu corecţie). La un an după operaţie acuitatea vizuală a fost de 1,0 (cu corecţie). Prin metoda dată au fost supuşi intervenţiei microchirurgicale 39 de pacienţi cu cataractă senilă, acuitatea vizuală a constituit 0,2…1,0 (cu corecţie la externare). Dispozitivul şi metoda s-au realizat în cadrul Catedrei şi Secţiei de Oftalmologie a IMSP SCR.
