Abstract
Description
Invenţia se referă la medicină, în special la oftalmologie. The invention relates to medicine, in particular to ophthalmology.
Este cunoscut dispozitivul pentru tratamentul cataractei senile, care include o seringă cu ac rectiliniu cu o lungime de 12 mm [1]. The device for the treatment of senile cataract is known, which includes a syringe with a straight needle with a length of 12 mm [1].
Este cunoscută metoda extracţiei extracapsulare a cataractei senile, care include următoarele etape tradiţionale: anestezie topică şi aseptizare obişnuită a câmpului operator, anestezie regională, aplicarea blefarostatului, fixarea muşchiului rect superior, incizia conjunctivei de la limb (orele 10°°…14°°), deschiderea camerei anterioare la ora 11°°, efectuarea capsulorexisului anterior prin introducerea separată a aerului steril şi a colorantului, formarea inciziei limbale (orele 10°°…14°°), luxarea nucleului în camera anterioară, înlăturarea acestuia în întregime, lavajul maselor cristaliniene, introducerea viscoprotectorului în camera anterioară, implantarea cristalinului artificial. Suturarea în surget a inciziei limbale. Suturarea separată a conjunctivei la orele 10°° şi 14°°. Subconjunctival se administrează un preparat steroid şi un antibiotic. Ulterior se aplică un pansament steril aseptic [2]. Subsequently, an aseptic sterile dressing is applied [2].
Dezavantajele constau în faptul că dispozitivul şi metoda cunoscută necesită efectuarea unei incizii penetrante la nivelul limbului de 2…3 mm, prin care uneori se scurge conţinutul camerei anterioare cu inclavarea în plagă a irisului. Problema pe care o rezolvă invenţia solicitată constă în elaborarea unui dispozitiv şi a unei metode de tratament al cataractei senile mai eficiente, care ar permite păstrarea conţinutului camerei anterioare cu poziţionarea corectă a irisului în timpul puncţiei limbale şi ar asigura efectuarea marcajului capsulei anterioare cristaliniene într-o singură etapă. The problem solved by the claimed invention consists in developing a more efficient device and method for treating senile cataract, which would allow preserving the contents of the anterior chamber with the correct positioning of the iris during limbal puncture and would ensure marking of the anterior lens capsule in a single step.
Esenţa invenţiei constă în aceea că dispozitivul pentru tratamentul cataractei senile include o seringă cu ac executat din două segmente unite sub un unghi de 90º, segmentul proximal are lungimea de 5 mm, iar cel distal - de 7 mm. Metoda de tratament al cataractei senile constă în aceea că se efectuează anestezia locală şi regională, se fixează muşchiul rect superior, se efectuează o incizie a conjuctivei la nivelul limbului, ce corespunde orelor 1000…1400, se efectuează puncţia camerei anterioare la nivelul limbului, ce corespunde orei 1100, cu ajutorul dispozitivului menţionat şi se introduc 0,3 ml de aer steril sub capsula cristalinului, apoi se marchează capsula anterioară a cristalinului prin introducerea a 0,2 ml de albastru de metilen, se efectuează o incizie circulară la nivelul limbului, ce corespunde orelor 1000…1300, se înlătură nucleul cristalinului, se efectuează lavajul maselor cristaliniene, se implantează cristalinul artificial, se suturează ţesuturile pe straturi, iar în spaţiul subconjunctival se introduce un antibiotic şi un remediu corticosteroid. Avantajele invenţiei propuse constau în aceea că utilizarea dispozitivului menţionat, încărcat în prealabil cu colorant - albastru de metilen şi aer steril, permite perforarea camerei anterioare, fără a efectua în prealabil o incizie limbală, fapt care deseori contribuie la evacuarea conţinutului camerei anterioare cu inclavarea în plagă a irisului, şi păstrarea profunzimii camerei anterioare folosind concomitent combinaţia aer steril şi colorant, de asemenea se efectuează cu uşurinţă marcajul capsulei anterioare cristaliniene. Rezultatul tehnic al invenţiei constă în abordul miniinvaziv, operativ şi eficace, păstrarea conţinutului camerei anterioare cu poziţionarea corectă a irisului în timpul puncţiei limbale cu dispozitivul menţionat, ceea ce permite într-o singură etapă (cu ajutorul combinaţiei de aer steril şi colorant) de a efectua şi marcajul capsulei anterioare cristaliniene. Prin metoda dată au fost supuşi intervenţiei microchirurgicale 37 de pacienţi cu cataractă senilă, acuitatea vizuală fiind de la 0,3 până la 1,0 (cu corecţie la externare). Dispozitivul şi metoda s-au elaborat în cadrul Catedrei şi Secţiei de Oftalmologie ale Spitalului Clinic Republican.
