Abstract

The invention relates to medicine, in particular to ophthalmology and can be used for treating patients with senile cataract.According to the invention, the method consists in that it is carried out the local anesthesia, is treated the operative field, is used the blepharostat, is carried out a non-invasive corneal incision which corresponds to 1000 and 1300 o'clock, is opened the anterior chamber at the level of 1100 o'clock, is introduced sterile air into the anterior chamber, is carried out the marking of the anterior capsule, is introduced a viscoelastic agent in the anterior chamber, is carried out the anterior capsulorhexis, is removed the nucleus, is performed the lavage of lens masses, is implanted the intraocular lens to the posterior chamber, is sutured the cornea, is restored the anterior chamber and is sutured the conjunctiva, and subconjunctivally is introduced an antibiotic and a corticosteroid preparation, at the same time upon implantation of the intraocular lens the upper leg of the lens is positioned on the iris, and the lower leg and the optical part are positioned under the iris.

Invenţia se referă la medicină, în special la o metodă utilizată în oftalmologie pentru tratamentul pacienţilor cu cataractă senilă. The invention relates to medicine, in particular to a method used in ophthalmology for the treatment of patients with senile cataract.

Este cunoscută metoda de tratament al cataractei, care constă în extracţia cataractei cu implantarea unui cristalin artificial. În cadrul acesteia, după anestezia topică şi aseptizarea obişnuită a câmpului operator, se aplică blefarostatul, se efectuează o incizie a corneei la nivelul orei 10ºº pană la nivelul orei 13ºº. Camera anterioară se deschide la nivelul orei 11ºº şi se introduce aer steril. Se efectuează marcajul capsulei anterioare, se introduce în ea un remediu vâscoelastic şi se efectuează capsulorexisul anterior, se înlătură nucleul cristalin, se efectuează lavajul maselor cristaliniene, se implantează cristalinul artificial. Incizia corneană se suturează cu fir 10-0 în surget. Se suturează conjunctiva cu fir 8-0 la nivelul orelor 10ºº şi 13ºº, subconjunctival se introduce un antibiotic şi un preparat corticosteroid şi se aplică un pansament monocular aseptic [1]. The cataract treatment method is known, which consists of cataract extraction with implantation of an artificial lens. Dezavantajele acestei metode constau în faptul că în cazul prezenţei unei breşe a capsulei posterioare la implantarea cristalinului artificial, acesta se poate subluxa sau luxa în corpul vitros cu o posibilă declanşare postoperator a unui glaucom secundar, a uveitei postoperatorii sau a keratopatiei edemato-buloase. Problema se rezolvă prin aceea că se efectuează anestezia locală, se prelucrează câmpul operator, se aplică blefarostatul, se efectuează o incizie nepenetrantă a corneei ce corespunde orelor 10ºº şi 13ºº, se deschide camera anterioară la nivelul orei 11ºº, se introduce aer steril în camera anterioară, se efectuează marcajul capsulei anterioare, se introduce un remediu vâscoelastic în camera anterioară, se efectuează capsulorexisul anterior, se înlătură nucleul, se efectuează lavajul maselor cristaliniene, se implantează cristalinul artificial de camera posterioară, se suturează corneea, se reface camera anterioară şi se suturează conjunctiva, iar subconjunctival se introduce un antibiotic şi un remediu corticosteroid, totodată la implantarea cristalinului artificial picioruşul superior al cristalinului se poziţionează pe iris, iar picioruşul inferior şi partea optică se plasează sub iris. Avantajele metodei constau în faptul că este uşor de realizat datorită plasării picioruşului superior pe iris, iar poziţionarea părţii optice sub iris asigură o stabilitate a cristalinului artificial implantat, ceea ce evită declanşarea unor complicaţii, cum ar fi: glaucomul secundar, uveita postoperatorie şi keratopatia edemato-buloasă. Se efectuează anestezia locală, se prelucrează câmpul operator, se aplică blefarostatul, se efectuează o incizie nepenetrantă a corneei ce corespunde orelor 10ºº şi 13ºº, se deschide camera anterioară la nivelul orelor 11ºº, se introduce aer steril în camera anterioară, se efectuează marcajul capsulei anterioare, se introduce un remediu vâscoelastic în camera anterioară, se efectuează capsulorexisul anterior, se înlătură nucleul, se efectuează lavajul maselor cristaliniene, se implantează cristalinul artificial de camera posterioară, se suturează corneea, se reface camera anterioară şi se suturează conjunctiva, iar subconjunctival se introduce un antibiotic şi un remediu corticosteroid, totodată picioruşul superior al cristalinului artificial se poziţionează pe iris, iar partea optică şi picioruşul inferior se plasează sub iris. Pacientul X., în vârstă de 71 ani. Diagnosticul clinic: ochiul drept (OD) - cataractă senilă hipermatură. Ochiul stâng (OS) - cataractă senilă incipientă. Patient X., 71 years old. Clinical diagnosis: right eye (RO) - hypermature senile cataract. Left eye (LO) - incipient senile cataract.

La fel nu s-au depistat semne ale unui glaucom secundar, ale unei uveite postoperatorii sau ale keratopatiei edemato-buloase. La un an de la intervenţia microchirurgicală la OS, în cadrul biomicroscopiei, semne de decentrare a cristalinului artificial implantat nu s-au depistat. De asemenea nu s-au depistat semne ale unui glaucom secundar, ale unei uveite postoperatorii sau ale keratopatiei edemato-buloase. Metoda dată a fost aplicată la 13 pacienţi, în cadrul catedrei Oftalmologie, IMSP SCR, care au fost inspectaţi la un an de la intervenţia microchirurgicală şi nu s-a depistat decentrarea cristalinului artificial implantat sau semne ale unui glaucom secundar, ale unei uveite postoperatorii sau ale keratopatiei edemato-buloase. Metodă de tratament al cataractei senile care constă în aceea că se efectuează anestezia locală, se prelucrează câmpul operator, se aplică blefarostatul, se efectuează o incizie nepenetrantă a corneei ce corespunde orelor 10ºº şi 13ºº, se deschide camera anterioară la nivelul orei 11ºº, se introduce aer steril în camera anterioară, se efectuează marcajul capsulei anterioare, se introduce un remediu vâscoelastic în camera anterioară, se efectuează capsulorexisul anterior, se înlătură nucleul, se efectuează lavajul maselor cristaliniene, se implantează cristalinul artificial de camera posterioară, se suturează corneea, se reface camera anterioară şi se suturează conjunctiva, iar subconjunctival se introduce un antibiotic şi un remediu corticosteroid, caracterizată prin aceea că implantarea cristalinului artificial se efectuează în aşa mod ca picioruşul superior al cristalinului să fie poziţionat pe iris, iar picioruşul inferior şi partea optică să fie plasat sub iris.Method of treatment of senile cataract which consists in performing local anesthesia, processing the operating field, applying the blepharostat, making a non-penetrating incision of the cornea corresponding to 10 o'clock and 13 o'clock, opening the anterior chamber at 11 o'clock, introducing sterile air into the anterior chamber, marking the anterior capsule, introducing a viscoelastic remedy into the anterior chamber, performing anterior capsulorhexis, removing the nucleus, washing the crystalline masses, implanting the artificial lens in the posterior chamber, suturing the cornea, restoring the anterior chamber and suturing the conjunctiva, and introducing an antibiotic and a corticosteroid remedy subconjunctivally, characterized in that the implantation of the artificial lens is performed in such a way that the upper pedicle of the lens is positioned on the iris, and the lower pedicle and the optical part are placed under the iris.
