Abstract

에스트로겐의 증식 효과는 남성에 있어서 양성 전립선 과증식 및(또는) 여성화 유방을 유발하는 직접적인 위험 요인과 연관된다[C. Knabbe, "Endocrine Therapy of Prostate Disease", in B. Allolio, H.M. Schulte (eds.), Practical Endocrinology, Urban & Schwarzenberg, Munchen, 645-651, 1996]. 이러한 이유로 남성에 있어서의 에스트로겐 대체는 알려진 이들 증후에도 불구하고 심각하게 고려된 적이 없다.
신경변성 질병을 치료하기 위한, 전신적인 작용(즉 신체의 모든 기관 및 계에서의 작용)을 갖는 천연 및 합성 에스트로겐들의 용도는 미국특허 제4,897,389호, 동 제5,554,601호, 국제특허공개 제 WO 95/12402호 및 동 제 WO 97/03661호 및 독일특허 제 DE 43 38 314 C1호에 특허청구되어 있다.
미국특허 제4,897,389호는 노년성 치매, 파킨슨씨 병, 대뇌 위축, 알츠하이머 병, 소뇌 위축, 노년성 또는 특이성 떨림증의 치료를 위하여 에스트라디올, 에스트론 및 에스트리올을 단독으로 또는 성선 자극 호르몬, 안드로겐, 아나볼릭 안드로겐 또는 인간 성장 호르몬과의 조합으로 사용하는 용도를 청구하고 있다.
미국특허 제5,554,601호 및 국제특허공개 제 WO 95/12402호는 진행성 손상 및 세포 괴사로부터 신경 세포들을 보호하고 신경변성 질병의 치료를 위한 약간의 "성적인 활성"을 갖는 에스트로겐 물질의 용도를 청구하고 있다. 에스트로겐, 즉 17α-에스트라디올은 약간의 "성적인 활성" 및 신경보호 작용을 갖는 물질의 일 예로 거론된다.
국제특허공개 제 WO 97/03661호는 신경보호를 보장하는, 두 개 이상의 고리 구조를 가지며(그 중 하나 이상이 말단 페놀 고리임) 분자량이 1000 달톤 이하인 비-에스트로겐 물질의 용도를 청구하고 있다.
독일특허 제 DE 43 38 314 C1호는 그 라디칼 포획 및 산화방지 특성이 에스트로겐-유사 활성의 정도에 의존하지 않는, 페놀 A 고리 구조를 갖는 스테로이드를 기술하고 있다. 이 화합물들은 라디칼에 의해 유발되는 세포 손상의 예방 및 치료에 사용될 수 있다.
상기한 모든 특허들에 있어서, 얻어진 물질의 치료 및 신경보호 능력은 이하의 최종 효과들 중 하나 이상에 의존한다:
자연적인 뉴런 성장 인자들의 생합성을 자극함;
아세틸콜린 합성 효소의 활성 또는 아세틸콜린 합성 기질 섭취를 자극함;
영양소 물질 또는 성장 인자의 소실로부터 신경세포의 저항성을 증가시킴으로써 직접적으로 세포를 보호함; 및
외상성 또는 신경독성 반응때문에 방출되는 자유 라디칼 및 반응성 산소종에 대한 신경 세포들의 민감성을 감소시킴.
그러나, 상기 특허들 중 어느 것도 선택적인 에스트로겐-유사 신경친화성 전사 효과, 즉 생체내 투여량에 있어서 생식계에 어떠한 중요한 생물학적 작용도 미치지 않고 에스트로겐과 유사한 방식으로 중추 신경계 중의 에스트로겐-의존 유전자의 전사에 영향을 미치는 스테로이드에 관해 기술하고 있지 않다. 미국특허 제5,554,601호 및 국제특허공개 제 WO 95/12402호에 기재된 17α-에스트라디올, 즉 생식기관 내에서 감소된 발정원성(estrogenicity)을 갖는 물질의 작용[J.H. Clark, et al, "Effects of Estradiol 17α on Nuclear Occupancy of the Estrogen Receptor, Stimulation of Nuclear Type II Sites, and Uterine Growth", J. Steroid. Biochem. 16, 323-328, 1982]은 단지 영양성 매질의 소실에 의해 유발된 세포 괴사 이전의 배양된 신경 세포들의 보호에만 연관되고, 여기에서 17α-에스트라디올의 상대적인 효능을 17β-에스트라디올과 비교하거나 함께 평가하지 않았던 점은 특히 강조되어야 한다. 더욱이 17β-에스트라디올이 중추 신경계 선택성을 갖고 있지 않는 것으로 알려져있고 따라서 전신 작용성 에스트로겐으로 분류되는 반면, 현재의 실험 및 특허 문헌들로부터는 17α-에스트라디올 또는 그 유도체들의 선택적인 신경친화성 작용이 암시되어 있지 않다는 결론을 내리게된다. 국제특허공개 제 WO 97/03661호 및 독일특허 제 DE 43 38 314 C1호는 에스트로겐의 세포보호 작용을 그 말단 페놀 A-고리의 라디칼 포획 특성의 결과로 해석한다. 에스트로겐-민감성 유전자들의 전사에 대한 영향에 기초한 17α-에스트라디올의 분리된 신경친화성 작용은 상기 특허들에 개시된 연구에서 시도되거나 상기 문헌들에 보고된 적이 없었다.
본 발명은 중추 신경계의 에스트로겐 결핍을 선택적으로 보충하기 위한 제약 제제의 제조를 위한 선택된 스테로이드의 용도에 관한 것이다.
17α-에스트라디올을 포함하는 전신 작용성 천연 및 합성 에스트로겐과는 대조적으로 이 스테로이드들은 선택적인 신경 친화성의, 에스트로겐-유사 전사 작용을 갖는 특징이 있다. 이 스테로이드들은 하기 화학식 (I)의 화합물들이다.

본 발명의 목적 중 하나는 기타 기관 또는 계에 영향을 미치지 않으면서 중추 신경계(CNS)에서의 에스트로겐 결핍을 선택적으로 보충하기 위한 제약 제제를 제공하는 것이다.
상기 목적은 본 발명에 따라 기타 기관 또는 계에 영향을 미치지 않으면서 중추 신경계(CNS)에서의 에스트로겐 결핍 보충을 보장할 수 있는 제약 제제의 제조를 위해, 선택된 스테로이드를 사용함으로써 해결된다.
이 스테로이드들은 17α-에스트라디올을 포함하는 천연 및 합성 에스트로겐의 전신성 작용과 대조적으로, 선택적인 신경친화성 에스트로겐-유사 전사 작용을 갖는 것을 특징으로 한다.
상기한 본 발명에 따른 선택된 스테로이드들은,
CNS 중에서 에스트로겐-의존성 유전자 전사에 선택적으로 영향을 미치고 적절한 생리적 파라미터의 변화를 유발하며,
생식계의 조직 중에서 아무런 생물학적 영향없이 사용된 투여량에서 CNS-특이성 전사 효과들을 가지고,
17β-에스트라디올 또는 17α-에스트라디올 모두 아무런 작용을 갖지 못하는 투여량에서 CNS 특이성 전사 효과들을 가지며,
CNS 중의 에스트로겐-의존성 유전자의 전사는 2차적으로 형성된 17β-에스트라디올의 작용에 의해 영향받지 않는다는 것을 발견한 것은 놀라운 일이었다.
본 발명의 스테로이드들은 하기 화학식 (I)의 화합물들이다.

본 발명의 유리한 일 태양은 인식 능력의 연령-의존성 감소, 연령에 의해 조건지워지며 폐경기 부근에 나타나는 불쾌감, 월경기 전 증후군, 노이로제 및 신경쇠약, 불안 상태, 불안 노이로제, 에스트로겐 상실(폐경, 성선 적출, GnRH 유사체로의 처리에 기인) 이후의 일과성 전신 열감 및 성적 행동의 심인성 억제의 예방 및 치료용 제약 조성물 제조를 위한, 화학식 (I)의 화합물의 본 발명에 따른 용도이다. 제어되지 않은 증식 및 발암 현상의 의미에서 생식계(자궁내막, 자궁근층, 전립선, 유방)의 호르몬 민감성 조직에 대한의 손상의 위험 또는 손상은 크게 방지될 수 있다.
또한 본 발명의 요지는 경구 및 비경구 투여, 특히 국부, 직장내, 피하, 정맥내, 근육내, 복막내, 비강내, 질내, 구강내 또는 설하 투여에 적합한 제약 제제를 포함하며 이는 통상적인 담체 및 희석 물질(예컨대 물 또는 젤라틴)외에 유효 성분으로서 상기 화학식 (I)의 화합물을 함유한다.
이하에 본 발명의 제약 제제의 실시 태양들을 나타내었다.
- 본 발명의 유효 성분을 0.1 내지 2 mg 포함하는 일일 경구 투여용 정제 또는 환제,
- 본 발명의 유효 성분을 0.1 내지 2 mg 포함하는 일일 피하주사용 앰플,
- 매일 본 발명의 유효 성분 0.05 내지 2 mg을 피부를 통해 방출하기 위한 고약(plasters),
- 매일 본 발명의 유효 성분 0.05 내지 2 mg을 피부를 통해 방출하기 위한 겔 및 크림, 및
- 매일 본 발명의 유효 성분 0.1 내지 1 mg을 방출하기 위한 구강 투여 시스템.
본 발명에 따른 의약은 통상적인 제약 보조 물질 및 통상적인 고체 또는 액체 담체 또는 희석제들과 함께 적절한 투여량으로 제공된다.
이하의 실시예들에서 본 발명의 화합물, 즉 도면에는 원형 물질이라 표시된 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(R1= R2= R3= R4= R5= R6= R7= H이고, R8= α-OH, β-H이며, R9= CH3이고, Z = -C=C-임)의 선택적인 에스트로겐-유사 작용을 시험하고 17β-에스트라디올 및 17α-에스트라디올과 비교한다.
실시예 1
생체 내에서 원형 물질의 장기간 피하 투여의 자궁 중량에 대한 영향
성숙한 암컷(생후 3개월, 중량 250 ± 30 g)인 위스타(Wistar) 흰 쥐(독일 Tierzucht Schonwalde GmbH 사 제품)를 케타민(ketamine) 혼수상태 하에서 난소를 절제하였다. 14일 후 상기 동물에 7일간 매일 0.01, 0.1, 0.3, 1, 3, 30 또는 100 ㎍의 시험 물질(15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올, 17β-에스트라디올 및 17α-에스트라디올)을 방출하는 삼투압 미니펌프(미국 Alzett사 제품)를 이식하였다. 대조 동물에는 해당 부피의 부형제(프로필렌 글리콜)를 투여하였다. 7번째 처리일에 상기 동물을 치사시켜 자궁의 습한 상태의 중량(체중 100 g에 대한)을 측정하였다.
도 1은 자궁을 절제한 쥐에서의 17β-에스트라디올(직사각형 기호), 17α-에스트라디올(원형 기호) 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(삼각형 기호)의 투여량에 따른 자궁향성 작용(uterotropic action)을 예시한다. 각 점들은 실험 동물 7 내지 10 마리의 평균값 ± 표준 편차(X ± SEM)를 나타낸다. 음영 처리 부분은 플라시보(placebo)로 처리한 동물들(OVX)에서의 이들 파라미터의 분산 폭을 나타낸다.
17β-에스트라디올을 투여하는 경우 0.03 내지 0.1 ㎍의 일일 투여량에서 자궁에서의 현저한 증가가 이미 나타나는 것이 명백하다. 17α-에스트라디올을 투여하는 경우 대등한 효과를 얻기 위해서는 100 ㎍의 일일 투여량이 필요하다. 원형 물질을 투여하는 경우에는 대등한 효과를 얻기 위해서는 30 ㎍ 일일 투여량의 원형 물질 즉, 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올이 필요하다.
실시예 2
생체 밖에서의 α-에스트로겐 수용체-의존성 리포터 유전자 전사의 활성화
에스트로겐-수용체(Era)의 α-이성질 형태를 발현하는 유방암 세포 MCF-7/2A를 수용체 플라스미드 EREwtcLUC로 꾸준히 전이시켰다. 리포터(reporter)는 비텔로게닌(Vitellogenin) 유래의 에스트로겐 반응 요소(ERE), 티미딘 키나제(thymidine kinase) 프로모터 및 포티누스 피랄리스(Photinus pyralis)의 루시페라제-코딩된 유전자를 포함한다. 세포 배양조직을 실험 시작에 앞서 7일 동안 스테로이드가 없는 매질 내에서 배양하였고 이어서 4가지 상이한 농도(10-11, 10-10, 10-9, 10-8M)의 17β-에스트라디올, 17α-에스트라디올 또는 원형 물질인 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올과 함께 48시간 동안 인큐베이션시켰다. 세포들을 세포용해성으로 만들고 루시페라제 리포터 유전자의 전사를 측정하여 특정 시험 물질(Serva/Promega, 독일)과의 루시페라제 활성을 결정하였다.
도 2는 상이한 투여량의 17β-에스트라디올(직사각형 기호), 17α-에스트라디올(원형 기호) 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(삼각형 기호)로 48시간 처리 후의 에스트로겐 수용체 유방암 세포 MCF-7 중의 전이된 안정한 리포터 유전자(루시페라제) 전사의 유도를 나타낸다. 이는 두 독립적인 실험들의 평균값을 나타낸다.
시험 화합물이 투여량에 의존하는 방식으로 리포터의 전사를 자극한다는 것이 명백하다. 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올 및 17α-에스트라디올의 효율이 17β-에스트라디올보다 한 차수(즉 10배) 적다. 이러한 결과는 원형 물질인 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올이 유방암 조직에서 몇 배 약한 에스트로겐 작용을 갖는다는 것을 보여준다.
실시예 3
자궁에 효과가 없는 투여량으로 생체 내에서 장기간 처리한 후의 뇌 중의 옥시토신 유전자 전사의 자극
성숙한 암컷(3개월, 중량 250 ± 30 g)인 위스타 흰 쥐(독일 Tierzucht Schonwalde GmbH 사)를 케타민 혼수상태 하에서 난소를 절제하였다. 14일 후 상기 동물에 7일간 매일 0.01, 0.1 또는 1 ㎍의 시험 물질(15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올, 17β-에스트라디올 및 17α-에스트라디올)을 방출하는 삼투압 미니펌프(미국 Alzett사 제품)를 이식하였다. 대조 동물에는 해당 부피의 부형제(프로필렌 글리콜)를 투여하였다. 7번째 처리일에 상기 동물을 치사시켜 자궁의 습한 상태의 중량(체중 100 g에 대한)을 측정하였다. 옥시토신의 생합성을 코딩하는 메신저 리보핵산(mRNA)을 시상 하부의 실방핵(hypothalmic Nucleus paraventricularis; PVN)에 대해 확립된 방법에 따라 특정 방사성 물질로 표시된 올리고데옥시뉴클레오티드 프로브로 제자리-혼성화(in-situ-hybridization)에 의해 제조하였다[D. Fischer, et al, "Lactation as a Model of Naturally Reversible Hypercorticalism: Plasticity in the Mechanism Governing Hypothalamo-pituitary-adrenal Activity in the Rat", J. Clin. Invest. 96, 1208-1215, 1995]. 옥시토신 유전자 전사에 있어서 처리에 의해 조건지워지는 변화를 명확한 해부 조직내에서의 특정 혼성화 신호를 농도계로 측정하여 정량분석하였다.
도 3은 상이한 3 가지 투여량에서 17β-에스트라디올(원형 기호), 17α-에스트라디올(직사각형 기호) 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(삼각형 기호)로 장기간 피하 처리한 후의 시상하부 실방핵 난소 절제된 쥐에서의 옥시토신-코딩된 전사(OT mRNA, 상부 도표)의 유도를 나타낸다. 하부 도표는 자궁 중량에 대한 시험 화합물의 효과를 나타낸다. 각 점들은 5 내지 7 개의 각각의 측정치들의 X ± SEM을 나타낸다. 음영처리된 부분은 부형제로 처리한 쥐들에서의 대응 파라미터들의 분산 폭을 나타낸다. 별표 기호는 대조군(OVX)과 비교하여 현저한 차이(p < 0.05)를 나타낸다.
실험 결과들은 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올이 투여량에 의존하는 방식으로 PVN에서 옥시토신 유전자의 전사를 자극한다는 것을 보여준다. 17β-에스트라디올의 자극 효과는 매우 유사하다. 일반적으로 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올의 신경향성의 전사 효과는 자궁 확대없이 17β-에스트라디올과 상당히 다르다. 사용된 투여량에서 17α-에스트라디올은 시상 하부의 PVN에서 옥시토신-mRNA의 농도에 대해 아무런 영향을 미치지 않는다.
이러한 결과들은 흰 쥐의 뇌에서 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올의 선택적인 에스트로겐-유사 작용을 증명한다.
실시예 4
자궁향성 작용을 갖지 않는 투여량으로 생체 내에서 장기간 처리한 후의 해마 중의 항세포자멸사(antiapoptotic) 유전자 bcl-2 전사의 자극
동물들로부터 유래된 실험 물질을 실시예 3에서와 같이 처리하였다. 유전자 bcl-2는 세포 증식의 연쇄적 증폭(cascade)과 연관된 단백질의 합성을 코딩하며 세포자멸사(apoptosis)의 프로그래밍에 대항하는 작용을 한다[D.E. Merry, S.J. Korsmeyer, "Bcl-2 Gene Family in the Nervous System", Ann. Rev. Neurosci. 20, 245-267, 1997]. 상기 유전자의 전사는 에스트로겐에 의해 자극받는다[M. Kandouz, et al, "Antagonism between Estradiol and Progestin on Bcl-2 Expression in Breast Cancer Cells", Int. J. Cancer 68, 120-125, 1996]. 자이러스 덴타투스(Gyrus Dentatus)는 쥐에 있어서(심지어 늙어가는 쥐에 있어서도) 신경 발생이 지속되는 해마 구조의 한 구성요소이며[E. Gould, et al, "Proliferation of Granule Cell Precursors in the Dentate Gyrus of Adult Monkeys is Diminished by Stress" Proc. Natl. Acad. Sci. USA 96, 3168-3170, 1998] bcl-2가 발현된다. Bcl-2 전사를 특정 올리고뉴클레오티드 프로브로 제자리-혼성화(in-situ-hybridization)로 제조하였고[R.S.B. Clark, et al, "Apoptosis-suppressor Gene Bcl-2 Expression after Traumatic Brain Injury in Rats", J. Neurosci. 17, 1997, pp. 9172-9182], 실시예 3에 기재된 농도계 방법에 따라 정량화하였다.
도 4는 난소 절제된 쥐 해마의 자이러스 덴타투스에서의 bcl-2 발현에 대한 상이한 3 가지 투여량의 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(삼각형 기호), 17β-에스트라디올(원형 기호) 및 17α-에스트라디올(직사각형 기호)의 효과를 도시한다. 참조 문자 및 약자들은 도 3에서와 같다.
15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올 물질로 처리하면 투여량에 의존하는 자이러스 덴타투스에서의 bcl-2 발현의 자극을 야기한다. 상기 효과는 같은 투여량의 17β-에스트라디올에 의해 생성되는 것과 같다. 도 4로부터 명백한 바와 같이 다른 투여량에서 17α-에스트라디올 물질은 bcl-2 전사에 대해 아무런 영향을 주지 못하였다. 이러한 결과들은 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올이 사용된 투여량에서 자궁에 작용하지 않으면서 에스트로겐과 유사한 방식으로 중추 신경계에서의 항세포자멸사 유전자 bcl-2의 전사에 영향을 미침을 보여준다.
실시예 5
뇌 및 자궁근층에서의 옥시토신-수용체의 분리적인 유도
펩티드 호르몬 옥시토신에 대한 동일한 생화학적 특성을 갖는 결합 부위들은 자궁근층 및 중추 신경계 중에 존재한다. 양 기관들에 있어서 급성 또는 만성적인 에스트로겐 처리는 옥시토신 수용체의 수(밀도) 증가를 초래한다. 이들 파라미터가 에스트로겐에 의해 특히 선택적으로 영향을 받는 뇌 구조는 선말단간질핵(Nucleus interstitialis striae terminalis), 복내측핵(Nucleus ventromedialis) 및 유편도핵 복합체(amygdaloid Nuclear complex)이다. 이들 구조에서 에스트로겐에 의해 조건지워지는 옥시토신 수용체의 유도는 일련의 전사회적인(prosocial) 행동 패턴, 또한 성적 행동의 발현을 야기한다[T.R. Insel, "Oxytocin, a Neuropeptide for Affiliation: Evidence from Behavioral, Receptor Autoradiographic, and Comparative Studies", Psychoneuro-endocrinology 17, 3-35, 1992].
방사성 물질로 표시된 옥시토신-수용체 길항체들 d(CH2)5-Try(Me)2, Thr4, Orn8-[125I]Tyr9-바소토신(125I-OVTA)의 결합에 의한 방사선 사진 표시법은 한정된 해부 구조에서의 옥시토신 수용체의 밀도를 결정하기 위해 선택한 방법이다[P. Kremarik, et al, "Histoautoradiographic Detection of Oxytocin- and Vasopressin-binding sites in the Telencephalon of the Rat", J. Comp. Neurol. 333, 343-359, 1993]. 7일 동안 매일 1 ㎍의 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올, 17β-에스트라디올 또는 17α-에스트라디올을 피하 투여한(실시예 3과 비교) 난소 절제된 쥐의 뇌 및 자궁의 냉동 단편들을 50 pM의 농도로125I-OVTA(독일 NEW DuPont 사)와 함께 인큐베이션 시켰다. 이어서 필름 방사선 사진을 제작하여 이를 확립된 방법에 따라 옥시토신 결합 부위들의 농도계 측정에 사용하였다[V.K. Patchev, et al, "Oxytocin Binding Sites in Rat Limbic and Hypothalamic Structures: Site-specific Modulation by Adrenal and Gonadal Steroids", Neuroscience 57, 537-543, 1993].
도 5는 1 ㎍의 일일 투여량으로 17β-에스트라디올(흑색 컬럼), 17α-에스트라디올(빗금표시 컬럼) 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(교차빗금 표시 컬럼)로 7일간 처리한 후 자궁근층 및 두 에스트로겐-민감성 뇌 구조 즉, 시상하부 복측 정중핵(hypothalamic ventromedial Nucleus(VMN)) 및 선말단간질핵(Nucleus interstitialis striae terminalis(BNST))에서의 옥시토신 수용체의125I 표시된 리간드(125I-OVTA)의 특이 결합을 나타낸다. 별표 기호는 플라시보로 처리한 난소 절제된 쥐(OVX)와 비교하여 현저한 차이(p < 0.05)를 나타낸다. 우측 그래프는 자궁내막의 증식에 대한 시험 물질의 효과를 나타낸다. 각 컬럼은 4 내지 5회의 개별 측정치들의 X ± SEM을 나타낸다.
이 실험들의 결과는 도 5에 유사하게 나타난다. 17β-에스트라디올 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올로 처리하면 모든 처리된 뇌 구조에서 옥시토신 수용체의 밀도가 현저히 증가한다. 그러나 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올은 VMN에서 17β-에스트라디올보다 약한 효과를 나타낸다. 17α-에스트라디올은 사용된 투여량에서 어떠한 뇌 구조에도 영향을 미치지 못하였다. 자궁근층에서 17β-에스트라디올은 옥시토신 결합 부위들의 강한 유도를 야기한 반면, 17α-에스트라디올 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올은 현저히 작은 효과를 나타냈다. 컴퓨터를 이용한 자궁 표본에서의 자궁내막의 농도 측정치는 1 ㎍의 일일 투여량이 현저한 자궁내막 증식을 야기한 반면, 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올은 동등한 투여량에서 자궁내막의 농도에 영향을 미치지 못함을 나타낸다. 요약하면, 실시예 5의 결과들은 중추 신경계에서 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올이 생화학적 파라미터의 하나이며 생식계(자궁내막) 및 중추 신경계의 특징을 나타내는 옥시토신 수용체에 현저한 영향을 미치며, 그 선택적인 신경향성 작용때문에 천연 에스트로겐인 17α-에스트로겐 및 17β-에스트로겐과는 질적으로 상이함을 보여준다.
실시예 6
장기간 처리 후의 인식 기능에 대한 효과
에스트로겐 농도의 감소는 학습 및 사고 능력의 감소와 관련된다는 사실이 알려져있다[H. Kopera, "Aging and Estrogens", Front Hormone Res. 2, 118-133, 1973]. 혈청 에스트로겐 농도 및 인식 능력 간의 연관성 또한 동물을 모델로 한 연구에서 증명되었다[Y. Kondo, K. Suzuki, Y. Sakuma, "Estrogen Alleviates Cognitive Dysfunction Following Transient Brain Ischemia in Ovariectomized Gerbils" Neurosci. Lett. 238, 45-48, 1997]. 인식 능력에 대한 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올, 17β-에스트라디올 및 17α-에스트라디올의 효과를 비교하기 위하여 이하의 실험을 수행하였다.
성숙한 암컷 위스타 흰 쥐(중량 240 ± 20 g)를 넴뷰탈(nembutal)로 마취시킨 상태에서 난소를 절제하였다. 일주일 후 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올 30 ㎍, 17β-에스트라디올 1 ㎍, 및 17α-에스트라디올 100 ㎍의 일일 투여량으로 매일 피하 투여하기 시작했다. 전체 처리 기간은 14일 이었다. 확립된 방법에 따라 5일 및 6일째 처리일에 조절된 탈출 행동 학습 훈련을 수행하였다[G. Diaz-Veliz, et al, "Influence of the Estrous Cycle, Ovariectomy and Estradiol Replacement upon the Acquisition of Conditioned Avoidance Responses in Rats", Physiol. Behav. 46, 397-401, 1989]. 각 동물을 한 세션(session) 당 조절되지 않은 자극 1회(전기 쇼크) 및 2회의 조절된 자극(빛과 소리 신호)에 노출시켰다. 7일째 처리일에 학습 행동 패턴의 보유를 시험하였다. 학습 세션의 6일 중단 후, 14일째 처리일에는 학습된 조절된 행동이 사라진 것으로 측정되었다. 5회 연속적인 노출로부터의 올바른 행동 반응(조절 신호를 제공한 다음 3초 이내에 장치의 "안전한" 구역으로 탈출)의 수를 학습 행동의 보유 또는 소멸의 평가 기준으로 사용하였다.
도 6은 난소 절제된 쥐들(백색 컬럼, OVX)에서의 새로운 행동 패턴의 획득 및 보유에 대한 17β-에스트라디올(흑색 컬럼), 17α-에스트라디올(빗금 컬럼) 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(교차 빗금 컬럼)의 영향을 나타낸다. 별표 기호는 해당 시험일에 플라시보로 처리한 동물들(OVX)과 비교한 현저한 차이를 나타낸다. 14일간 처리 후 자궁 중량은 OVX의 경우는 53 ± 2, 17β-에스트라디올은 187 ± 9, 17α-에스트라디올은 100 ± 5 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올의 경우는 108 ± 4(X ± SEM, n = 처리군 당 8 내지 10, mg/100 g KG)로 측정되었다.
15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올이 사용된 투여량에서 학습 행동 패턴의 보유에 대하여 에스트로겐과 유사한 자극 효과를 가지는 반면, 1 ㎍의 일일 투여량에서 17β-에스트라디올보다 자궁향성 작용이 현저히 작다는 것이 명백하다. 이러한 결과는 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올이 에스트로겐과 같이 인식 능력에 영향을 미치지만 생식계에서 미약한 증식 작용만을 유발함을 나타낸다.
실시예 7
17β-에스트라디올에 대한 17α-히드록시-14,15α-메틸렌-에스트라-3,5,(10)8-테트라엔-3-올 및 17α-에스트라디올의 형질 전환
난소를 절제한 쥐들에게 매일 100 ㎍의 17α-에스트라디올, 30 ㎍의 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올 또는 1 ㎍의 17β-에스트라디올을 피하 투여하였다(실시예 6 참조). 17β-에스트라디올의 혈청 농도를 마지막 처리일에 3개의 실험군에서 측정하고 부형제로 처리한 대조군과 비교하였다.
도 7은 표시한 투여량에서 17β-에스트라디올(흑색 컬럼), 17α-에스트라디올(빗금 컬럼) 및 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올(교차 빗금 컬럼)으로 7일간 난소 절제된 쥐들을 피하 처리한 후의 17β-에스트라디올의 혈청 농도를 나타낸다. 별표 기호는 플라시보로 처리한 동물들에 대해 측정한 값과 비교했을때 현저한 차이를 나타내고, 플라시보로 처리한 동물들의 경우는 방법의 오차한계 내에 있으며, 각 처리군은 7마리의 동물로 구성된다.
상기한 투여량으로 17α-에스트라디올 및 17β-에스트라디올을 투여한 후 17β-에스트라디올이 측정 가능한 농도로 혈청내에 존재함이 명백하다. 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올로 장기간 피하 처리하면 내생적인 17β-에스트라디올 농도의 증가를 야기하지 못하였다. 이러한 결과는 15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올을 투여한 후 관찰된 약리학적 효과들에는 17β-에스트라디올로의 형질 전환이 포함되지 않음을 보여준다.
Claims (3)
- 중추 신경계 중의 에스트로겐 결핍을 선택적으로 보충하기 위한 제약 제제를 제조하기 위한 하기 화학식 (I)의 스테로이드 화합물의 용도(I)상기 식에서,R1은 수소 원자, 히드록시기 또는 탄소 원자수가 1 내지 5인 알콕시기이고,R2는 수소 원자, 탄소 원자수가 1 내지 5인 알킬기, 탄소 원자수가 1 내지 5인 아실기 또는 일반식 SO2NR10R11인 기(여기에서 R10및 R11은 서로 독립적으로 수소 원자, 탄소 원자수가 1 내지 5인 알킬기이거나 또는 질소 원자와 함께 피롤리디노기, 피페리디노기 또는 모르폴리노기 임)이며,R3는 수소 원자 또는 히드록시기이고,R4는 수소 원자, 히드록시기 또는 탄소 원자수가 5 이하인 알킬기이며,R5및 R6는 각각 서로 독립적으로 수소 원자 또는 할로겐 원자이고,R7은 수소 또는 메틸기이며,R8은 수소 원자 및 히드록시기, 옥소기 또는 일반식 CR12R13의 기(여기에서 R12및 R13은 서로 독립적으로 수소 원자 또는 할로겐 원자임)이고,R9는 메틸 또는 에틸기이며,Z는 탄소-탄소 이중 결합이거나 치환 또는 비치환된 시클로프로판 고리이고,>CR5R6기는 자체가 α-상태인 기 또는 β-상태인 기이도록 배열되며,R7은 >CR5R6가 α기인 경우 β기이고 >CR5R6가 β기인 경우에는 α기이다.
- 제1항에 있어서, 상기 스테로이드 화합물이15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올,15βH,3'H-시클로프로프[14,15]-18α-호모-에스트라-1,3,5(10),8-테트라엔-3,17α-디올,17α-히드록시-15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3-일-펜타노에이트,17-메틸렌-15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3-올,15βH,3'H-3',3'-디플루오로-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올,17-메틸렌-15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3-일-술파메이트,17-디플루오로메틸렌-15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3-올,3-메톡시-15β-메틸-3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3-올,15α-메틸-3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3,17α-디올,17-디플루오로메틸렌-15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-테트라엔-3-일-(테트라메틸렌이미노)술포네이트 및17-메틸렌-3'H-시클로프로프[8,9]-15βH,3'H-시클로프로프[14,15]-에스트라-1,3,5(10),8-트리엔-3-올로 이루어지는 군으로부터 선택되는 용도.
- 제1항 또는 제2항에 있어서, 상기 제약 제제가 인식 능력의 연령-의존성 감소, 연령에 의해 조건지워지며 폐경기 부근에 나타나는 불쾌감, 월경기 전 증후군, 노이로제 및 신경쇠약, 불안 상태, 불안 노이로제, 에스트로겐 결핍(폐경, 성선 적출, GnRH 유사체로의 처리) 이후의 일과성 전신 열감 및 성적 행동의 심인성 억제의 예방 및 치료를 위한 것인 용도.
