Abstract
Description
項1. 被験体の腫瘍部位に存在するプロトポルフィリン類の蛍光を分光検出する腫瘍部位の識別装置であって、前記プロトポルフィリン類がプロトポルフィリンIX(PpIX)とフォト−プロトポルフィリン(PPp)であり、前記識別装置がPpIXの一部をPPpに変換する光照射部と、PpIXの蛍光とPPpの蛍光を分光する分光部と、PpIXの蛍光とPPpの蛍光の相対蛍光強度を検出する分光検出部と、PpIXとPPpの相対蛍光強度に基づいて腫瘍部位と非腫瘍部位の判別を行う腫瘍判別部とを有する腫瘍部位の識別装置。
項2. 前記光照射部が、光源と光源からの励起光を被験体に導光照射する光源用光ファイバーを備える、項1記載の識別装置。
項3. 分光検出部が、PpIXに由来する635nm付近の蛍光を検出する手段とPPpに由来する675nm付近の蛍光を検出する手段を備える、項1又は2に記載の識別装置。
項4. PpIXの蛍光とPPpの蛍光を分光部に導く分光用光ファイバーを備える、項1〜3のいずれかに記載の識別装置。
項5. 前記腫瘍判別部による腫瘍判別結果の情報を、その腫瘍判別された蛍光を発生する被験体の位置に対応した画像情報として表示する表示装置をさらに備える、項1〜4のいずれかに記載の識別装置。
項6. 被験体の腫瘍部位の識別方法であって、
被験体の腫瘍部位に蓄積されたプロトポルフィリンIX(PpIX)に光照射してPpIXの一部をフォト−プロトポルフィリン(PPp)に変換する工程、
PpIXとPPpに対する励起光を照射する工程、
前記励起光によって励起されたPpIXとPPpが発する蛍光を分光部で分光する工程、
PpIXの蛍光とPPpの蛍光の相対蛍光強度を検出する工程、
PpIXとPPpの相対蛍光強度に基づいて腫瘍部位と非腫瘍部位の判別を行う工程、
を含む、腫瘍部位の識別方法。
項7. 腫瘍が、センチネルリンパ節に転移した腫瘍である、項6に記載の方法。
・術中迅速診断の一助となり、より正確で迅速な診断につながる。
・正確な術中診断により、縮小手術や遺残のない手術につながる。
PpIX溶液:0.1mMの濃度となるようにジメチルスルホキシド(DMSO)に溶解した。
使用した細胞株:MKN-45(ヒト低分化型胃癌から樹立)
(1)5-ALAの投与
培養ディッシュに1mMの5-ALAを加え、30分間培養した。その後培地を交換し、さらに3時間インキュベーションした。トリプシン処理を行い、1×107個/mlの濃度の細胞浮遊液を回収し、実験に使用した。
(2)スペクトル分析
以下の装置を用いてスペクトル分析を行った。
intensified multichannel spectrophotometer
(MCPD-7000, Otsuka Electronics, Osaka)
水銀ランプ(U-LH100HG, Olympus)
励起・蛍光フィルター (D436 20x - E455LPV2)
(Ex. 436±20nm, Em. >455nm, Chroma Technology Corp.)
(3)分光画像の取得
以下の装置を用いて分光画像の取得を行った。
12ビットモノクロCCDカメラ (ORCA-ER, Hamamatsu Photonics)
液晶チューナブルフィルタ(Varispec VIS-20-HC-20, CRi Inc.)
蛍光ミラーユニット(U-MNBV2, Olympus)
まず画像取得前にバックグラウンドノイズを撮影 (IN 635nm ,IN 675nm)。
(4)Ratio imageの作成
画像処理は、image-Jを用いて行った。
得られたratio imageを下の式によって除算を行った。
胃癌細胞株(MKN-45)に5-ALAを投与して3時間培養した後、436nmの励起光を照射し続け、一定時間ごとにスペクトルを測定した。PpIX溶液と変化率は異なるが、同様の傾向(635nmのピークの低下と675nmのピークの上昇)が、培養細胞においても観察された(図10)。また、レシオ(I675/I635)は照射量依存的に上昇することが確認された(図11)。但し、その変化率はPpIX溶液を用いた場合よりも低かった。
スペクトル変化を画像的に捉えるため、レシオイメージングを行った。すなわち、436nmの励起光の元、一定の光照射ごとに635nm及び675nmの分光画像を取得し、それらのレシオイメージを作成し、照射前後でのレシオイメージの変化を観察した。結果を図12に示す。スペクトル測定の結果と同様に、光照射に伴って、レシオイメージの輝度が徐々に上昇することが示された。
実施例2と同様にして5-ALAを投与した癌細胞の細胞浮遊液とコラーゲンファイバーとを同時に観察し、レシオイメージングを行った。
対象患者は術前にリンパ節転移が疑われる胃癌および大腸癌の患者のうち、あらかじめ書面により臨床試験への参加の同意を得られた患者である。ただし、ポルフィリン症、アレルギー歴のある者、肝・腎機能障害を有する者、消化管通過障害のある者は除外した。
手術の2時間前に15mg/kgの5-ALAを50%ブドウ糖溶液に溶かして内服させる。
転移が疑われる領域のリンパ節を1つずつ半割し、その割面の蛍光観察と分光画像の取得を上述の方法に従い行った。
本実施例では以下の材料を用いた。
(1)使用した細胞株
MKN-45(ヒト低分化型胃癌から樹立)
(2)5-ALAの投与
MKN-45細胞を培養したディッシュに1mMの5-ALAを加え、30分間培養した。その後培地を交換し、さらに3時間インキュベーションした。トリプシン処理を行い、1×107個/mlの濃度の細胞浮遊液を回収し、実験に使用した。
(3)スペクトル分析
以下の装置を用いてスペクトル分析を行った。

(excitation 436±20nm, emission >455nm)
(4)照射およびスペクトル取得条件
405nmまたは436nmの光を照射し、0.25J/cm2ずつ蛍光スペクトルを取得した。
<Case1>照射光、励起光ともに405nmを選択した場合
光照射に伴い、635nmのピーク(PpIX)の減弱を認める一方、675nmに新たなピーク(PPp)の出現を認めた(図17(a)、(b))。

<Case2>照射光、励起光ともに436nmを選択した場合
光照射に伴い、635nmのピーク(PpIX)の緩やかな減弱を認め、675nmに新たなピーク(PPp)の出現を認めた(図18(a)、(b))。

<Case3>照射光に405nm、励起光に436nmを選択した場合
405nmの光照射に伴い、635nmのピーク(PpIX)の減弱を認め、675nmに新たなピーク(PPp)の出現を認めた(図19(a)、(b))。

<Case4>上記3つの組み合わせの比較(図20)
いずれの組み合わせでも、照射前と1J/cm2照射した後ではI675/I635の比(ratio)が上昇していることが確認できるが、照射光に405nmを、励起光に436nmを選択した組み合わせ(Case3)が最も大きな変化を示している。これは、405nmの波長がPpIXをより効率よくPPpに変化させるとともに、436nmの励起光がPPpの励起に最も適しているためである。
実施例6:コラーゲンとFADのスペクトル変化についての検討
本実施例では、代表的な生体内在性蛍光物質のスペクトル変化について検討するため、以下の材料を用いた。
FAD:90μMに希釈したFAD溶液
スペクトル分析は上述の装置で行った。照射光は405nm、励起光は436nmを選択した。
<Case1>コラーゲンのスペクトル変化
光照射に従って全体に蛍光ピークの減弱を認めるが、スペクトル変化は認めない(図21)。
<Case2>FADのスペクトル変化
光照射に従って全体に蛍光ピークの減弱を認めるが、スペクトル変化は認めない(図22)。
<Case3>光照射に伴うI675/I635 比(Ratio)の変化
コラーゲン、FAD共に、光照射を行ってもRatioの値はほぼ一定を示し(図23)、PpIXの変化とは異なることからPpIXと区別が可能である。
Claims (7)
- 被験体の腫瘍部位に存在するプロトポルフィリン類の蛍光を分光検出する腫瘍部位の識別装置であって、前記プロトポルフィリン類がプロトポルフィリンIX(PpIX)とフォト−プロトポルフィリン(PPp)であり、前記識別装置がPpIXの一部をPPpに変換する光照射部と、PpIXの蛍光とPPpの蛍光を分光する分光部と、PpIXの蛍光とPPpの蛍光の相対蛍光強度を検出する分光検出部と、PpIXとPPpの相対蛍光強度に基づいて腫瘍部位と非腫瘍部位の判別を行う腫瘍判別部とを有する腫瘍部位の識別装置。
- 前記光照射部が、光源と光源からの励起光を被験体に導光照射する光源用光ファイバーを備える、請求項1記載の識別装置。
- 分光検出部が、PpIXに由来する635nm付近の蛍光を検出する手段とPPpに由来する675nm付近の蛍光を検出する手段を備える、請求項1又は2に記載の識別装置。
- PpIXの蛍光とPPpの蛍光を分光部に導く分光用光ファイバーを備える、請求項1〜3のいずれかに記載の識別装置。
- 前記腫瘍判別部による腫瘍判別結果の情報を、その腫瘍判別された蛍光を発生する被験体の位置に対応した画像情報として表示する表示装置をさらに備える、請求項1〜4のいずれかに記載の識別装置。
- 被験体の腫瘍部位の識別方法であって、
被験体の腫瘍部位に蓄積されたプロトポルフィリンIX(PpIX)に光照射してPpIXの一部をフォト−プロトポルフィリン(PPp)に変換する工程、
PpIXとPPpに対する励起光を照射する工程、
