Description
Claims (32)
- 1.体の血管内の所定部位で血管閉塞を行うのに使用される装置であって、 基端と先端並びにその両端の間に延在する中空部を有し、その先端を所定部位に 設置するように構成したカテーテルと、捻回螺旋状態となり得る弛緩状態から前 記中空部にわたって進行し得る伸張・直線状態へ伸張自在である血管閉塞用ワイ ヤと、前記ワイヤの端部と対面する端部との接触によって、前記ワイヤを前記カ テーテルへ伸張状態で進行させるための、前記カテーテルの基端から制御できる プッシャと、 前記プッシャが前記ワイヤを前記カテーテルの中空部へ進行させるために使用さ れるときに、前記ワイヤの前記カテーテル中空部への進行に応じてワイヤとプッ シャとを把持係合状態に保持するための前記プッシャおよび前記ワイヤのそれぞ れの対面端部と連携する把持手段と、 前記ワイヤの所定部が前記カテーテルの先端を越えて進行したときに、前記ワイ ヤと前記プッシャとの把持係合状態を解除するための、前記カテーテルの先端に 隣接して前記カテーテル内に設けられた解除手段とを具えたことを特徴とする装 置。
- 2.前記把持手段は前記対面端部の一方に担持された半径方向拡大部と、前記対 面端部の他方と連携する拡開可能なあご部とを具備し、前記あご部は、前記あご 部が前記半径方向拡大部を把持状態で保持することにより前記あご部と前記カテ ーテル内壁との接触によって形成される閉鎖状態と、前記あご部が拡開して前記 半径方向拡大部を解放することにより前記あご部と前記解除手段との相互作用に よって形成される開放状態との間を運動する請求項1に記載の装置。
- 3.前記半径方向拡大部が前記ワイヤに担持され、前記あご部が前記プッシャと 連携する請求項2に記載の装置。
- 4.前記把持手段は前記対面端部の一方に担持された半径方向拡大部と、前記対 面端部の他方と連携する拡開可能なコイルとを具備し、前記コイルは、前記コイ ルが前記半径方向拡大部を把持係合状態で保持することにより前記コイルと前記 カテーテル内壁との接触によって形成される、曲率の小さい閉鎖螺旋状態と、前 記コイルが拡大して前記半径方向拡大部を軸方向に解放することにより前記あご 部と前記解除手段との相互作用によって形成される、曲率の大きい開放螺旋状態 との間で拡開自在である請求項1に記載の装置。
- 5.前記半径方向拡大部が前記ワイヤに担持され、前記コイルが前記プッシャと 連携する請求項4に記載の装置。
- 6.前記半径方向拡大部が前記プッシャに担持され、前記コイルが前記ワイヤと 連携する請求項4に記載の装置。
- 7.前記解除手段は前記カテーテルの先端によって形成されたリムを含む請求項 1に記載の装置。
- 8.前記解除手段は前記カテーテルの中空部内に前記カテーテルの先端に隣接す る半径方向拡大セグメントを含む請求項1に記載の装置。
- 9.前記ワイヤは両端が実質的に非弾性的な糸で連結された細長いコイルを含む 請求項1に記載の装置。
- 10.前記カテーテルの先端の流れ指向運動によって分岐血管経路ヘカテーテル を案内するために、前記ワイヤは、前記ワイヤが前記カテーテルの先端を越えて 前進した場合に開いて血管内の血流に接触する面積を増加させるように構成され た可撓性の先端構造体を具えている請求項1に記載の装置。
- 11.前記ワイヤは両端が実質的に非弾性的な糸で連結された細長いコイルを具 えている請求項10に記載の装置。
- 12.前記ワイヤはトルク伝達可能であって前記カテーテルの先端を越えて前進 した場合に湾曲形態をとる先端領域を有し、かつ前記把持手段は前記ワイヤの先 端領域と前記カテーテルの中空部内の前記ワイヤとの間のトルク伝達を行うこと ができる、前記カテーテルの先端のワイヤ指向運動によって分岐血管経路ヘカテ ーテルを案内するために使用される請求項1に記載の装置。
- 13.前記ワイヤはその両端がトルク伝達可能な実質的に非弾性的なバンドで連 結された細長いコイルを含む請求項12に記載の装置。
- 14.捻回螺旋形状となり得る弛緩状態とカテーテルの中空部内を進行可能な伸 張・直線状態との間を自在に伸張する血管閉塞用ワイヤと、 前記ワイヤの端部と対面する端部との接触によってカテーテルへ伸張状態で前記 ワイヤを軸方向に進行させる、前記カテーテルの基端からの操作が可能なプッシ ャと、 前記対面端部の一方に担持された半径方向拡大部と、前記対面端部の他方と連携 する拡開可能な構造体であって、そのあご部と前記カテーテル内壁との接触によ って形成される閉鎖状態と構造体の拡開によって形成される開放状態との間を移 動でき、前記閉鎖状態では前記半径方向拡大部を把持し、前記開放状態では前記 拡大部を軸方向へ解放させる構造体とを具えた、ことを特徴とする基端と先端及 び該両端の間に延在する中空部を有し、血管の所定部位に設置されるように構成 されたカテーテルと共に使用されるプッシャ・ワイヤ・アセンブリ。
- 15.前記拡開可能な構造体は弛緩した開放状態、およびカテーテル内壁との接 触によって形成される閉鎖状態との間を移動することのできる拡開可能なあご部 を具えている請求項14に記載のアセンブリ。
- 16.前記半径方向拡大部が前記ワイヤに担持され、前記あご部が前記プッシャ と連携する請求項15に記載のアセンブリ。
- 17.前記把持構造体は、拡開自在のコイル具備し、前記コイルは前記カテーテ ルの内壁との接触によって形成する曲率の小さい閉鎖螺旋状態と、拡大して前記 半径方向拡大部を解放する曲率の大きい開放螺旋状態との間を拡関するように構 成されている請求項15に記載のアセンブリ。
- 18.前記半径方向拡大部が前記ワイヤに担持され、前記コイルが前記プッシャ と連携する請求項17に記載のアセンブリ。
- 19.前記半径方向拡大部が前記プッシャに担持され、前記コイルが前記ワイヤ と連携する請求項17に記載のアセンブリ。
- 20.前記ワイヤが細長いコイルを具え、その両端が実質的に非弾性的な糸で連 結されている請求項14に記載のアセンブリ。
- 21.前記ワイヤはカテーテルの先端を越えて進行したときに開いて血管内の血 流に接触する面積を増加させるように構成された可撓性の先端構造体を具えてい る、カテーテルの先端の流れ指向運動によって分岐血管経路ヘカテーテルを案内 するために使用される請求項14に記載のアセンブリ。
- 22.前記ワイヤは、その両端が実質的に非弾性的な糸で連結されている細長い コイルを具えている請求項21に記載のアセンブリ。
- 23.前記ワイヤはトルク伝達可能であって前記カテーテルの先端を越えて進行 したときに湾曲形態をとる先端領域を具え、かつ前記把持手段は前記カテーテル の中空部内で前記プッシャと前記ワイヤとの間にトルクを効果的に伝達する、カ テーテルの先端のワイヤ指向運動によって分岐血管経路ヘカテーテルを案内する ために使用される請求項14に記載のアセンブリ。
- 24.前記ワイヤは、その両端がトルク伝達が可能な実質的に非弾性的なバンド で連結された細長いコイルを含む請求項23に記載のアセンブリ。
- 25.基端と先端並びに該両端間に延在する中空部を有しかつ血管の所定部位に 設置されるように構成されたカテーテルと、前記カテーテルの中空部から解放位 置へ半径方向の運動して該解放位置でその先端に担持されたあご部が閉鎖位置か ら開放位置まで拡開するように構成された前記カテーテルの基端から操作できる プッシャと共に使用される血管閉塞用ワイヤであって、弛緩した捻回螺旋状態か ら前記カテーテルの中空部を進行するときの伸張・直線状態へ自在に伸張するワ イヤ部分と、前記プッシャのあご部が閉鎖位置のときに前記あご部に把持されか つ開放位置のときにそこから解除される、前記ワイヤ 部の一端に担持された半径方向拡大部とを具えたことを特徴とする、血管閉塞用 ワイヤ。
- 26.前記伸張可能なワイヤ部は、両端が実質的に非弾性的な部材で連結された 細長いセグメントを具えている請求項25に記載のワイヤ。
- 27.前記部材が前記半径方向拡大部から前記伸張可能部を通じてトルクを伝達 するように構成されたトルク伝達可能なバンドである請求項26に記載のワイヤ 。
- 28.前記ワイヤは前記カテーテルの先端を越えて進行したときに開いて血管内 の血流に接触する面積を増加させるように構成された可撓性の先端構造体を具え ている、カテーテルの先端の流れ指向運動によって分岐血管経路ヘカテーテルを 案内するために使用される請求項25に記載のワイヤ。
- 29.前記ワイヤはトルク伝達可能であってカテーテルの先端を越えて進行した ときに湾曲形態をとる先端領域を具備している、カテーテルの先端のワイヤ指向 運動によって分岐血管経路ヘカテーテルを案内するために使用される請求項25 に記載の装置。
- 30.カテーテルの先端を血管の所定部位まで案内し、弛緩した捻回螺旋状態か ら伸張・直線状態へ自在に伸張する血管閉塞用ワイヤを前記カテーテルの中空部 へ進行させ、かつその進行の際に、前記ワイヤの所定部が前記カテーテルの先端 を越えて前進するまで前記カテーテル内を軸に沿って両方向に移動するように前 記ワイヤを軸方向に把持した状態で維持し、前記カテーテル先端を越えた点で前 記ワイヤを把持状態から解放する、ことを特徴とする血管閉塞用ワイヤを血管の 所定部位に設置する方法。
- 31.前記ワイヤはカテーテルの先端を越えて進行したときに開いて血管内の血 流に接触する面積を増加させるように構成された可撓性の先端構造体を具え、前 記カテーテルの先端の流れ指向運動によって分岐血管経路ヘカテーテルを案内す る方法であって、(a)血管分岐点において流量の多い方の分岐血管内に前記カ テーテルを案内したい場合には、前記ワイヤを前進させて前記ワイヤ部をカテー テルの先端から突出させ、 (b)血管分岐点において流量の多い方の分岐血管から遠ざけるように前記カテ ーテルを案内したい場合には、前記ワイヤを後退させて前記ワイヤ部を前記カテ ーテルの先端内へ引き込む工程を更に含む請求項30に記載の方法。
- 32.前記ワイヤはその先端領域が前記カテーテルの先端を越えて進行すると曲 がった形態となる先端セグメントを有すると共に、前記カテーテル内を進行する 時に遠くからトルクを伝達できる構造を有する装置を使用して、前記カテーテル の先端のワイヤ指向運動によって分岐血管経路ヘカテーテルを案内する方法であ って、(a)血管分岐点において二つの分岐血管の一方に前記カテーテルを案内 したい場合には、前記ワイヤを進行させて前記セグメントを前記カテーテルの先 端を越えて前進させ、(b)前記ワイヤを捩じって前記ワイヤセグメントを分岐 点の当該一方の血管の方に向け、かつ (c)前記ワイヤを前記カテーテル先端方向に進行させて前記カテーテルを前記 一方の血管内に案内する工程を更に含む請求項30に記載の方法。
