Description
糸を含み、
前記血管閉塞用ワイヤは可撓性チューブからなるカテーテルから血管へ挿入できる構成であり、
前記血管閉塞用ワイヤは前記カテーテル内で前進かつ後退するために弛緩した捻回螺旋状態(relaxed convoluted condition)から伸張直線状態(extended linear condition)へ伸張自在であり、かつ
前記糸は、前記血管閉塞用ワイヤに取り付けられたプッシャが前記カテーテル内でプッシャをその基端方向へ引っ張ることにより移動するときに、前記血管閉塞用ワイヤが前記カテーテル内で後退しかつ前記プッシャの移動が前記血管閉塞用ワイヤの伸張により影響されないように構成されている、ことを特徴とする。
本発明による血管閉塞用カテーテル装置の使用方法は、血管内の所定部位にカテーテルの先端を案内し、捻回螺旋状となり得る弛緩状態から伸張した直線状態へ自在に伸張する血管閉塞用ワイヤをカテーテルへ進行させ、前記ワイヤの進行の際に、このワイヤは、該ワイヤの所定部がカテーテルの先端を越えて前進するまでコイルをカテーテルの内部で軸の両方向へ移動できる軸方向把持状態で維持し、前記カテーテル先端を越えた地点で前記ワイヤを把持状態から解放する。
上記及びその他の本発明の目的と利点は、添付の図面を参照して以下の詳細な説明を読めば更に明瞭となろう。
図1は本発明に使用されるタイプの血管閉塞用カテーテル装置10の代表例を示す。
血管系の所定部位に血管閉塞ワイヤを設置するためのカテーテル装置の使用法を図8と図9を参照して説明する。普通の場合、血管閉塞部位には多くのカーブや曲り角、及び一つ以上の分岐部を含む屈曲した小血管を経由してアクセスが可能となる。カテーテルはガイドワイヤを用いてこうした部位へ案内される。この場合、ガイドワイヤは経路に沿って方向を決めるために使用され、カテーテルはこのガイドワイヤに沿って周期的に進行する。血管閉塞部位は遮断の必要な血管瘤や閉鎖が望まれる小血管内の領域等である。
図9(A)〜(D)は、図8(A)と(B)とに示された装置が、血管116内の血管閉塞部位114にワイヤを設置した後に血管閉塞用ワイヤ106の位置と向きを如何にして調整することができるか、又は如何にしてこの部位からワイヤを回収できるかを示す。図9(A)では、ワイヤは弛緩した捻回螺旋状態にあって血管の両側壁の間に挟まれているが、血管の下部はこのワイヤによって実質的には閉塞されていない。操作の第1段階として、図9(B)に示すように、あご部を開いた状態でカテーテルとプッシャを進行させ、あご部をワイヤの一部分上に被せる。
Bで述べた手順において、カテーテルは先ずカテーテルを屈曲した血管経路へ案内するガイドワイヤを使用して血管内の所定部位に位置決めされた。この項で述べる本発明の実施例においては、血管閉塞用ワイヤ自体がカテーテルを所定部位へ案内するのに使用できる構成になっている。これは、上述の装置の把持構造体によって、伸張した血管閉塞用ワイヤをカテーテルの内部で先端方向と基端方向の両方に進行させることができるように構成することによって、達成できることは理解されるであろう。この試案の利点は、先ずカテーテルをガイドワイヤと共に位置決めし、次にガイドワイヤを取り外し、そしてガイドワイヤをプッシャワイヤアセンブリに置き換える工程を単一工程に結合した点にある。
最後に、C項で述べたように、この装置のワイヤは屈曲した血管経路から小径のカテーテルを案内するの使用されると共に、一旦所定の血管閉塞部位に到達すると血管閉塞用ワイヤとしても使用可能である。このワイヤは、血管経路に沿って移動する流れ指向型又はワイヤ指向型のいずれの運動用にも構成することができる。
本発明は特定の実施例と方法に基づいて説明されたが、本発明から逸脱せずに多くの変更改変を行うことが可能なことは言うまでもない。

Claims (1)
- 基端および先端を有する血管閉塞用ワイヤ、ならびに
前記血管閉塞用ワイヤの基端および先端間に延在しかつ前記血管閉塞用ワイヤのコイル巻き部内中央に位置する実質的非弾性の糸を含む血管閉塞用カテーテル装置であって、
前記血管閉塞用ワイヤは可撓性チューブからなるカテーテルから血管へ挿入できる構成であり、
前記血管閉塞用ワイヤは前記カテーテル内で前進かつ後退するために弛緩した捻回螺旋状態から伸張直線状態へ伸張自在であり、かつ
前記糸は前記カテーテル装置に連結されていて、前記カテーテル内で基端部に設置されたプッシャを引っ張ることにより、前記血管閉塞用ワイヤを所定の伸張直線状態以上に実質的に伸長させることなく、前記カテーテル内で前記血管閉塞用ワイヤを後退させる構成である、ことを特徴とする血管閉塞用カテーテル装置。
