【発明の詳細な説明】
HIV−感染による疾病の治療
発明の背景
本発明はヒト免疫不全ウィルス(HIV)感染による疾病に悩む患者の治療に用
いられる医薬に関するものである。
現在、多くの疾病に対し、治療法がない。多くの場合これらの疾病に間しては、
進行中の苦痛とか、無力化を緩和するような各種の一時的症状緩和剤を用いての
治療が行われている。この病気の原因であるレトロウィルスの働きが宿主に対し
て致命的でない間は、このウィルスは免疫系を弱体化させ、何らかの機会さえあ
れば患者を様々の病気にかかりやすくする。従って大部分の罹患者はカボジ肉腫
、肺炎、あるいはあらゆる機会に何かの原因による疾病で死亡する。病状が数年
にもわたって進行する間に、患者は非常に衰弱し、働いたり簡単な家事すらもで
きないようになる。ドローゲ等はバイオロジカル ケミストリー、ホツベーザイ
ラー(Biol、Chem、、Hoppe−8eyler)、369巻。
143−148頁、(1988)にエイズ患者の血清中のシスチンおよびメチオ
ニン濃度の低下、アルギニンおよびグルタメートの濃度増加を報告している。又
、同報告によるとエイズ患者血清中での有糸分裂は外部より与えたグルタメート
によって阻害され、システィンによって促進される。このような有糸分裂の阻害
と培養リンパ球中の抗体産生の促進は、生理的濃度を越えた高濃度のシスチン、
あるいはシスティンによってもたらされることは前に報告されている。

7)は白ねずみのリンパ細胞がシスチンの弱い輸送能力を持ち、システィンにつ
いては強力な輸送能力を持っていると報告している。
ミラーとルマックは腫瘍学セミナーの補遺、−巻、76〜85頁(1983)で
、アセタミノフェンを過剰投与した時のアセチルシスティンの影響について述べ
ている。N−アセチルシスティンはムコリティックとして使用できることが多く
の国々で認められている。
ヒト免疫不全ウィルス感染による影響の機構解明は目下、行われているところで
ある。一般的には有効な薬は未だ報告されていないし、対称治療薬も、抗ウィル
ス剤も未だ見つかっていない。
エイズと他のウィルス性疾患に関して、少なくとも治療、あるいは病状を軽減し
たり、疾病の勢いを弱め、一時的にせよ延命効果を得ることは、大変興味深いも
のがある。
それに加えて、HIVの複製を抑えることは、血清検査で反応が陽性に出ている
患者の不都合な症状の増悪を防ぐ、あるいは遅らせることになるであろう。
発明の目的
本発明の目的の1つは、血清反応が陽性を示している患者の体内で、HIV感染
により起こっている症状を軽減し、HIVの転写と増殖を阻害するような作用を
持つ薬を提供することにある。
もう1つの目的は、症状を軽減し、HIVの転写と増殖を抑えるための薬物を、
疾病の発病に影響を与えるとされている他の薬物と組み合わせて、HIV感染の
治療に用いられるような薬物を提供することである。
他の目的は、以下に展開する詳細な説明と例を注意深く検討してもらえれば明ら
かになるであろう。
発明の要約
HIV感染症で苦しんでおり、又、いくつかの場合にはかなり衰弱している患者
の治療のために、いくつかの薬剤が投与される。特にN−アセチル システィン
(NAC)とか、N−(自〜4−アシル)システィンかその塩が患者の衰弱状態
を改善し、そして/あるいはHIVの転写と複製を阻害して、強力に患者の延命
をはかるに充分な量を投与するために調整された。
NACかその関連化合物、あるいはそれらの塩(以後、これらをひとまとめにし
てNAC化合物”と表現する)は、そのままであるいは疾病の回復、治療に適当
であるとされる薬物と組み合わせて投与される。
詳細な説明
HIVウィルス感染そして/あるいはHIVにより誘発される疾病、例えばLS
(リンパ節疾患症候群)、ARC(エイズ起因性神経症)やエイズ、等の病気の
病気の結果として、血清反応陽性、精神的ストレス、生理的な変調をきたしてい
る患者を治療するための薬として、NAC化合物が単独、あるいは他の抗ウィル
ス剤と組み合わせて使用される。
H’I Vウィルス感染して衰弱した患者にNAC化合物を投与すると、急速に
、そして持続的に、体重増加、体力増強など患者の生理的状態の改善がみられる
ことが解った6
そのような患者においては、NAC投与後、CD8”T#Il胞の増加がみられ
、他方CD4” T細胞の増加はそれ程でもなかった。CD4” T細胞は一般
的には、主としてHIV惑染の作用を弱める作用を持つとされており、又、エイ
ズ患者のCD4” T細胞の減少が何らかの機会さえあれば病気にかかり易くす
る原因と関連があると思われているため、この結果は驚くべきことであった。
血清反応陽性の患者においては、HIVにたまたま感染して体内でウィルスが発
育、増殖し、例えば進行性のカポジ肉腫のように体が衰弱してくるような症状が
出る以前の段階でも、システィン欠乏と、それに付随して起こる細胞内グルタチ
オン濃度の減少が見られる。
細胞内グルタチオン濃度の減少は腫瘍の壊死因子(TNF−α)の活発な生産を
伴い、そのことは呼吸を大変激しくさせ、細胞内酸化剤の生産を高め、結果とし
て遺伝子発現と蛋白生合成を誘発する、ある種の因子の活性化を起こさせる。
HIVの誘導によってつくられるDNAの上にある調節遺伝子に結合する蛋白を
活性化するのに関係があるとされているHIV遺伝子の発現は、TNF−α濃度
と相関がある。
その詳細な機構はさておいて、NAC化合物の投与は細胞内グルタチオノ濃度を
上げ、HIVの転写そして複製、増殖を押さえる。その結果、転写による遺伝情
報の発現、そして翻訳によるウィルス蛋白の合成がNAC化合物により押さえら
れる。
衰弱も又、グルタチオン濃度の減少と、TNF−α濃度の増加に関係しており、
NAC化合物はこのエイズの症状を改善する。NAC化合物はHIVによりひき
起こされる種々の病気にも有効である。
患者の治療はNACあるいはC1〜C4のN−アシル システィン類やその塩な
どのうち生理学的に投与可能な化合物を用いて行われる。
その塩とは、アルカリあるいはアルカリ土類金属、特にナトリウム、カリウム、
カルシウム塩など薬理学的に許容される陽イオンであり、あるいはアンモニアの
ような簡単なアミンのうち投与可能な塩基、そして特にリジン、アルギニンのよ
うな塩基性アミン酸、なども用いられる。
NAC化合物は様々な方法で調整される。これらは粉末、顆粒、錠剤、カプセル
、糖衣錠のような固型薬剤を含むが、このようなものに限ったわけではなく、滅
菌注射液や経口投与のための液体、あるいは懸濁液、座薬、吸入剤などで、局所
にて徐放される、あるいは注射剤にできる徐放性の薬剤などがある。
これらの処方には、一般的な添加剤、例えば香料、賦形剤、安定化剤、発泡剤、
抗酸化剤などが含まれる。これらの添加剤は通常、賦形薬を除いた全重量のl0
%以内である。
徐放性粒子のために、ポリラクテート、ポリグリコレート、ポリアルデヒド、ポ
リアンハイドライドのような様々な生理学的に投薬可能で生物学的に分解可能な
ポリマーが用いられる。
リポソームも又、担体として利用され、リポソームの膜内腔にNAC化合物を入
れ込んで利用する。リポソームは常法に従って調整するが、これは文献に記載さ
れているのでここに記載する必要はないであろう。
リポソーム膜内のNAC化合物の濃度は通常50μg/m1〜5mg/m1の範
囲である。リポソーム粒子のサイズは通常1へ500 )tである。

ウィルスが宿主の特異的な細胞に結合し、そこで特異的なターゲットであるリポ
ソームと結合することにより感染が起こるが、この治療のための薬の輸送のさら
なる改善がなされた。
例えば、抗体はリポソームと共有結合あるいは非共有結合をしているかもしれず
、CD3.−4.−8.Mac−1あるいはそれらを組み合わせた抗原に特異的
であるかもしれない。
NAC化合物を投与するのに都合のいい方法はどんなものでも試してみた。注射
、経口投与錠剤、粉末液剤、他の適当な方法により、経口、非経口、局所的投与
、等がなされる。中でも経口投与が好ましい。
投薬は一日一回、二回、数回、あるいは他の都合のいい間隔で行われ、投薬量は
、臨床医の間では常法となっているが、NACが作用持続型、あるいは類似の剤
型であるかどうかなど、その投薬の方法に依って決まる。
薬物はHIVの増殖を抑え、エイズを妨害し衰弱させるために、最大許容量が投
与される。
経口投与の場合は、100mg〜1gあるいはより多量のNACを含有する錠剤
、あるいはカプセル(被覆したもの、腸溶錠あるいはカプセルを含む)、顆粒剤
、散剤、あるいは発泡錠が用いられる。
錠剤やカプセルは普通に服用させるが、顆粒剤、散剤あるいは発泡錠は水に浮遊
させて服用させる。
NACの患者への投与量は普通、200mg〜20gの範囲であり、600mg
〜5gが好ましく、1〜4gが最適である。これ位の投与量は一般的にはlI[
著な副作用もなく、血清反応陽性患者、非自覚症状患者、HIV感染症状に苦し
む患者の細胞内グルタチオン濃度を正常レベルに回復させる。
食餌療法は、治療に好都合な方法で個々の患者の状態、反応、治療内容に合わさ
ねばならず、患者の状態の変化そして/あるいは他の臨床的症状に対応する必要
がある。
NACと関連化合物は、HIVにより誘発される病気の発病に影響を与える他の
薬物と結合させて用いてもよい1例えばよく用いられる薬としては逆転写酵素に
影響を与える様々な薬がある。
興味をひく薬物としては、ジドヴジン(AZT) 、ジデオキシシトシン、ジデ
オキシイノシン、等のようなヌクレオタイド類似化合物、アシクロヴイル、リバ
ヴイリン、アスコルビン酸、LL−1,2,3あるいは4のようなサイトカイン
類、成長因子、γ−インターフェロンのようなインターフェロン類等がある。
これらの薬物の投与方法については、フルブロウン エイズ、特別の薬物の使用
、投与回数、等、個々の病状に応じて異なる。通常これらの薬物は5μg〜10
0mg/kg/日投与される。
エイズの治療には、他の抗ウィルス剤を投与しつつ、連続的にNACを投与する
と満足すべき結果が得られるが、この治療法はウィルスの増殖に統一性を欠く影
響を与えるので副作用がある。
このようにして、NACとその関連化合物は体調の保守のために用いられ、他の
抗ウィルス剤は慢性の症状の治療のために投与される。
HIV感染で苦しんでいる患者の治療のためには、先に述べたような方法を適用
するために、N(C1〜4−アシル)システィン、あるいはその塩で薬理学的に
投与可能なものを100mg〜10gと、HIV感染の発病を抑えるための少な
くとももう一種の薬を調剤する。
14名のエイズ起因性神経症、あるいはエイズ患者に投薬して観察してみると、
疲労度とCD8“の数は改善される傾向にあり、CD4“の数は変わらなかった
。体重増加は14名中9名にみられ、2名については体重が著しく減少し、さら
にリンパ球の減少も観察された。薬の作用が逆に出た例が少し認められた。
白ネズミを用いてエイズのモデルを観察してみると、ジドヴジンは免疫機能障害
の発病を遅らせ、生存期間を延ばすことが示され、NACも又生存期間を延ばす
ことが分かった。そしてジドヴジンとNACを一緒に投与するとさらに延命され
るという結果であった。
HIVの調節遺伝子部位を持っているDNAを観察してみると、NACはHIV
道伝子の発現を押さえ、腫瘍壊死因子や化学的に類似の因子の作用を妨害するこ
のようにNACはLS、ARClそしてエイズ患者の症状tl和し、彼等の生活
内容を向上させる。NACはス、潜在的HIV感染患者あるいは初期ABC患者
のエイズ発病を防いだり、遅らせたりする。
又、ジドヴジンはHIV惑染患者、エイズ発病患者、重症ARC患者の治療の補
助として用いると有効である。
以下、実施例にて症例を示すが、症例はここに示されたものに限られてはいない
。
実施例 1
男性、HIV−1型抗体陽性のエイズ患者で危篤状態の者、他の症状に加えて著
しい体重減少がみられる。4ケ月の間、毎日200mgのN−アセチルシスティ
ンを5〜8回投薬。NACは市販の顆粒剤を水に入れて服用させる。この4ケ月
の間に患者のCD8“T!胞の数はファクター4にぢで増加したがCD4“T細
胞には著しい変化が見られなかった。
2−3週間の間に患者の症状は改善され、体重は急激に増加した。NAC治療を
始めて6ケ月後、患者は勤務にもどることが出来た0体力の回復が得られた。
8ケ月を過ぎても患者は元気に勤務している。
実施例 2
男性。LS症より、HIV感染症と診断された。患者は進行性虚弱症が進んでお
り、T−4ヘルパー細胞の漸次減少が見られ、6ケ月の間に疲労感の強まりがあ
った。
NAC治療が開始され、1日5〜8回、200mgが投与された。5週間以内に
患者の自覚症状は劇的に改善され、T−4ヘルパー細胞の数がほぼ発症前のレベ
ルにまで回復するに共ない、疲労感が著しく減少した。この状態は数ケ月続いて
いる。
実施例 3
46オの男性、HIV陽性と診断され、2年後にARCの診断が下った。臨床的
には鵞口癒を共なう粘膜と皮膚のカンジダ症が観察された。17ケ月後に血小板
が38.000を示し、突発性血小板減少性紫斑病と診断された。
その時点で、ジドヴジン治療を開始し、4時間毎に200mgが投与されたが、
副作用のなめ6時間おきに100mg投与へと減量を余儀なくされた。
ジドヴジン治療開始6ケ月後、ヘルパーT細胞は159に、ヘルパー細胞/サプ
レッサー細胞の値は0.29を示した。疲労感と精神的抑圧は続き、職を退くに
至った。翌月よりNAC治療を開始、1日1〜2gの投薬を行った。患者自身も
、又医者の所見も、体力の増強と精神的抑圧の解消を認め、フルタイムで職場復
帰を果たした。
最後の検査より4ケ月後のヘルパーT細胞は70に、ヘルパー細胞/サプレッサ
ー細胞は0.3となった。
以上の結果より、ジドヴジン化合物の投与はエイズの衰弱症状を軽減するのに用
いることが出来る。NAC化合物はHIVの増殖を抑え、血清反応陽性患者のエ
イズ発病を遅らせるか、あるいは阻害する。
先にも記したように、NAC化合物はNACと全く同じ、あるいは一部、同様の
作用を持ち得る。
本明細書記載のすべての刊行物と特許出願は、引用するために個々の刊行物なり
特許出願が特異的に、そして個々に示されているが、これらは引用することによ
り統一される。
請求項に記載した本発明の精神あるいは適用範囲からは、はづれることなしに、
ある変更や修正が近い将来になされることは明らかであろう。

Claims (24)
- 1.N−(C1〜4−アシル)システイン、あるいはそれらの塩で投与可能なも のを、HIV感染で苦しんでいる人患者内でのHIV転写、増殖を抑え、症状を 回復させるための薬として使用すること。
- 2.N−(C1〜4−アシル)システイン、あるいはそれらの塩で投与可能なも のを、HIV感染で苦しんでいる人患者内でのHIV転写、増殖を抑え症状を回 復させるために、HIV感染による発病を抑えるのに有効な少なくとももう一つ の薬と併用して使用すること。
- 3.請求項2に記載のある薬物として、逆転写酵素の阻害剤、ヌクレオタイド類 似化合物、ビタミン、サイトキン、あるいは成長因子を用いるもの。
- 4.請求項3に記載の薬物として、ジドヴジンあるいはアシクロヴィルを用いる もの。
- 5.請求項1〜4に記載の疾病のうち、LS(リンパ節疾患症候群)、ARC( エイズ起因性神経症)あるいはエイズに適用するもの。
- 6.N−C1〜4−アシル)システインがN−アセチルシステインである請求項 1〜5記載の用途。
- 7.N−アセチルシステインの投与量が1日量にして約200mg〜10gであ る請求項6記載の用途。
- 8.上記の量が1日当たり約1g〜4gである請求項7記載の用途。
- 9.HIV感染に苦しんでいる人患者のHIVの転写と増殖を押さえ、症状を軽 減するのに有効な方法を示した指示書を付した、人患者の症状を軽減するのに有 効な量のN−(C1〜4−アシル)システインあるいはそれらの塩のうちで薬理 学上許容しうるものを投与するための、先に述べた人患者の症状を軽減するのに 有効な量のN−(C1〜4−アシル)システインあるいはそれらの塩のうちで薬 理学上許容しうるものからなる薬物。
- 10.HIV感染による発病に有効な薬物を、治療上有効な量、少なくとも一種 類加えた請求項9記載の薬物。
- 11.HIV感染に悩む人患者のHIVによりひき起こされた疾病の回復と、H IVめ転写、増殖を押さえるために、治療上有効な量のN−(C1〜4−アシル )システイン、あるいはそれらの塩のうちで薬理学上許容しうるものを含んだ薬 物の製法。
- 12.HIV感染による発病に有効な薬物を少なくとも一種類加えた、請求項1 1記載の方法。
- 13.HIV感染に苦しんでいる人患者のHIV起因の疾病の回復の目的に用い られる請求項11記載の方法。
- 14.HIV感染に苦しんでいる人患者のHIV転写と増殖を押さえる目的に用 いられる請求項11記載の方法。
- 15.HIV感染に苦しむ人患者内でのHIVの転写、増殖を押さえ、症状を改 善するために用いられるキットで、治療上有効な量のN−(C1〜4−アシル) システイン、あるいはそれらの塩のうちで薬理学上許容しうるものを患者に投与 するためのものであり、HIV感染に苦しんでいる人患者のHIVの転写と増殖 を押さえ、症状を軽減するのに有効なN−(C1〜4−アシル)システイン、あ るいはそれらの塩のうちで薬理学上、許容しうるものの有効量よりなるキット。
- 16.HIV感染の治療に有効な薬物を、治療上有効な量、少なくとも一種加え た請求項15記載のキット。
- 17.HIV感染に苦しんでいる人患者のHIV転写、増殖を押さえるのに必要 な量を含んだ請求項15記載のキット。
- 18.HIV感染に苦しんでいる人患者のHIV起因性疾病に有効である量を含 んだ請求項15記載のキット。
- 19.以下の組成物の薬物。 a)N−(C1〜4−アシル)システイン、あるいはそれらの塩のうちで薬理学 上、許容しうるもの。 b)HIV感染により起こる疾病に有効な薬物を少なくとももう一種加え、そし て薬理学的に許容し得るキャリアを含むもの。
- 20.N−(C1〜4−アシル)システインがN−アセチルシステインである請 求項19記載の薬物。
- 21.もう一種加える薬物がジドヴジンである請求項19記載の薬物。
- 22.HIV起因の疾病の諸症状に苦しんでいる患者、あるいはHIV感染の結 果、HIV抗体陽性の患者に有効量のN−(C1〜4−アシル)システイン、あ るいはそれらの塩のうちで薬理学上、許容しうるものを投与する方法により成る やり方で、HIV感染に苦しんでいる人患者のHIV起因の疾病の症状緩和、H IV転写と増殖の抑止のために用いられる方法。
- 23.LS(腸内膜リンパ節症)、ARC(エイズ起因性神経症)、あるいはエ イズに苦しむ人患者、あるいはHIV感染の結果HIV抗体反応陽性の人患者の 治療法で、人患者にN−(C1〜4−アシル)システイン、あるいは薬理学上許 容しうる塩を1日に約200mg〜最大許容量と、HIV感染に有効な薬を少な くとも一種加える方法。
- 24.N−(C1〜4−アシル)システインがN−アセチルシステインである請 求項22あるいは23の方法。
