Claims (20)
- 医療処置を実行するための装置であって、

第1または第2の接触面のうちの一方にあるドップラーセンサと、
ステープル留めされた組織に熱エネルギーを供給するために、前記ドップラーセンサーとほぼ対向するように第1または第2のジョーのうちの他方に配置された熱要素と、
前記シャフトの近位端にあるハンドルであって、開位置と閉位置との間でジョーを選択的に動かすための第1のアクチュエータと、第1のジョーから第1および第2の接触面間の組織内に第2のジョーに向けて1または複数のステープルを展開して1または複数のステープルを変形させるための第2のアクチュエータと、組織内の血流を検出するために前記ドップラーセンサを作動させるための第3のアクチュエータとを有するハンドルと
を備えることを特徴とする装置。 - 請求項1に記載の装置において、
前記第1および第2のジョーが、前記シャフトの遠位端に取り外し可能に結合されたエンドエフェクタに保持されていることを特徴とする装置。 - 請求項1に記載の装置において、
前記第1のアクチュエータが、前記第2のジョーを開位置から閉位置へと誘導して前記第1および第2の接触面の間で組織を固定するためのトリガーを含み、前記第2のアクチュエータが、前記第1のジョーから組織を通って前記第2のジョーの方に前記1または複数のステープルを駆動して組織をステープル留めするためのウェッジ機構を含むことを特徴とする装置。 - 請求項1に記載の装置において、
前記ドップラーセンサに結合され、前記ドップラーセンサに接触している組織内の血流を識別するために前記ドップラーセンサから信号を受信するプロセッサと、前記プロセッサに結合され、組織内に血液が流れているか否かを示す出力を提供する出力デバイスとをさらに備えることを特徴とする装置。 - 請求項1~4のいずれか一項に記載の装置において、

前記ステープルの第1および第2のセットが、長手方向軸と整列した列に配置され、前記切断要素が、長手方向軸と整列した後退位置から前進位置まで前進可能であることを特徴とする装置。 - 請求項5に記載の装置において、
前記熱要素は、前記ドップラーセンサーと対向するように前記第1の接触面または前記第2の接触面の他方に沿って延びる細長い焼灼電極であり、前記焼灼電極および前記ドップラーセンサーは、前記第1の接触面と前記第2の接触面との間で捕捉された組織の両側に配置されることを特徴とする装置。 - 請求項5に記載の装置において、
前記ドップラーセンサーおよび前記熱要素は、前記切断要素の第1の側をそれぞれ前記第1および第2の接触面に沿って延びており、前記複数のステープルは、前記熱要素を備える前記第1の側における前記第1のジョーに複数の軸方向のレセプタクルの列を含み、前記熱要素および前記ドップラーセンサーは、前記複数の軸方向のレセプタクルの列と前記切断要素との間に配置され、単一の軸方向のレセプタクルの列が、前記切断要素の前記第1の側とは反対側の第2の側において前記第1のジョーに配置されていることを特徴とする装置。 - 請求項1~5のいずれか一項に記載の装置において、
前記熱要素が、前記第1のジョーの第1の接触面に沿って軸方向に延びる細長い電極を含み、電気エネルギー源が前記熱要素に結合され、前記ハンドルが、電気エネルギーを前記電極に供給して熱エネルギーをステープル留めされた組織に送達するためにエネルギー源を作動させるためのアクチュエータを含むことを特徴とする装置。 - 請求項1~5のいずれか一項に記載の装置において、
前記第1および第2のジョーが導入される患者の体内の領域の撮像を容易にするために、遠位端にカメラおよび光源の一方または両方をさらに含むことを特徴とする装置。 - ハンドルを含む近位端と、患者の体内に導入するためのサイズの遠位端と、近位端と遠位端との間に延びる長手方向軸とを含むシャフトを含むステープラ装置用のエンドエフェクタであって、
前記エンドエフェクタを前記シャフトの遠位端に取り外し可能に接続するための1または複数のコネクタと、
第1および第2のジョーであって、前記ハンドル上の第1のアクチュエータを用いて開位置と閉位置との間で互いに対して相対的に移動可能であり、それによって第1のジョーの第1の接触面と第2のジョーの第2の接触面とがそれぞれ互いに離れる方向および互いに近づく方向に移動し、第1および第2の接触面の間で組織を係合し、前記第1のジョーが、前記シャフトに対して固定され、第1の接触面上に組織を配置することが可能な開位置と、組織を第1および第2の接触面の間に固定する閉位置との間で、前記第2のジョーが前記第1のジョーに対して回動可能である、第1および第2のジョーと、
前記第1のジョーによって保持される1または複数のステープルであって、前記ハンドル上の第2のアクチュエータの作動により、前記1または複数のステープルが前記第1のジョーから第1および第2の接触面間の組織内に前記第2のジョーに向けて展開されて変形する、1または複数のステープルと、
組織内の血流を検出するために第1および第2の接触面のうちの一方にあるドップラーセンサと、
ステープル留めされた組織に熱エネルギーを送達するために、前記ドップラーセンサーとほぼ対向するように第1および第2のジョーの他方に配置された熱要素と、
を備えることを特徴とするエンドエフェクタ。 - 請求項1~5のいずれか一項に記載の装置において、
前記1または複数のステープルが、前記第1のジョーによって保持され、切断要素の両側に配置されたステープルの第1および第2のセットであって、ステープルの少なくともいくつかが他のステープルとは異なるサイズである、ステープルの第1および第2のセットを備え、
前記第1のアクチュエータは、前記第1および第2のジョーの一方または両方を閉位置に誘導して、第1および第2の接触面の間に組織を固定するように構成され、
前記第2のアクチュエータは、前記第1のジョーから固定された組織を介して前記第2のジョーに向けてステープルを展開してステープルを変形させるように構成され、
前記ハンドルは、さらに、前記切断要素を前進させて隣接した組織からステープル留めされた組織を切断するための第4のアクチュエータを有することを特徴とする装置。 - 請求項12に記載の装置において、
前記ステープルの第1および第2のセットが、前記第1のジョーのキャビティ内に取り外し可能に受け入れられるカートリッジ内に保持されることを特徴とする装置。 - 医療処置を実行するための装置であって、
近位端と、患者の体内に導入するためのサイズの遠位端と、近位端と遠位端との間に延びる長手方向軸とを有するシャフトと、
前記シャフトの遠位端にある第1および第2のジョーであって、開位置と閉位置との間で互いに対して移動可能であり、それによって第1のジョーの第1の接触面と第2のジョーの第2の接触面とがそれぞれ互いに離れる方向および近づく方向に移動し、前記第1のジョーが、前記シャフトに対して固定され、前記第2のジョーが、第1の接触面上に組織を配置することが可能な開位置と、組織を第1および第2の接触面の間に固定する閉位置との間で、前記第1のジョーに対して回動可能である、第1および第2のジョーと、
組織内の血流を検出するために第1または第2の接触面のうちの一方にあるドップラーセンサと、
ステープル留めされた組織に熱エネルギーを送達するために、前記ドップラーセンサーとほぼ対向するように第1および第2のジョーの他方に配置された熱要素と、
前記第1のジョーによって保持され、切断要素の両側に配置されたステープルの第1および第2のセットを保持するカートリッジであって、ステープルの少なくともいくつかが他のステープルとは異なるサイズである、カートリッジと、
前記シャフトの近位端にあるハンドルであって、
前記第2のジョーを閉位置に誘導して、第1および第2の接触面の間に組織を固定するための第1のアクチュエータと、
前記第1のジョーから固定された組織を介して前記第2のジョーに向けてステープルを展開してステープルを変形させるための第2のアクチュエータと、
長手方向軸と整列した後退位置から前進位置まで前記切断要素を前進させてステープル留めされた組織を切断するための第3のアクチュエータとを有するハンドルと
を備えることを特徴とする装置。 - 請求項14に記載の装置において、
前記ステープルの第1および第2のセットが、長手方向軸と整列した列に配置されていることを特徴とする装置。 - 請求項15に記載の装置において、
前記第1のセットのステープルすべてが、前記第2のセットのステープルすべてと異なるサイズであり、又は、
前記ステープルの第1のセットまたは第2のセットが、ステープルの第1のサブセットおよびステープルの第2のサブセットを含み、前記ステープルの第2のサブセットが、前記ステープルの第1のサブセットと異なるサイズであることを特徴とする装置。 - 組織内へ送達するための、請求項1~5のいずれか一項に記載の装置で使用されるステープルであって、
第1および第2の端部を含むほぼ直線的なベース要素と、
前記第1の端部から前記ベースに対してほぼ垂直に延びる第1のタインであって、第1の長さを有し、かつ当該第1のタインの先端に隣接する1または複数のノッチを含む第1のタインと、
前記第2の端部から前記ベースに対してほぼ垂直に第2の先端まで延びる第2のタインであって、第2の長さを有し、この第2の長さが、前記第1の長さよりも長く、かつ当該第2のタインが前記ベースに隣接するように曲げられたときに、当該第2のタインの第2の先端が前記第1のタイン上に位置する前記1または複数のノッチと係合するように、前記直線的なベースの長さよりも長い、第2のタインとを備えることを特徴とするステープル。 - 請求項1~5のいずれか一項に記載の装置において、
前記シャフトの周りに配置された撮像スリーブであって、前記シャフトの近位端に隣接するハブと、前記シャフトの遠位端に隣接するイメージャとを含む撮像スリーブと、
前記ハブに取り付けられたディスプレイとを備えることを特徴とする装置。 - 請求項18に記載の装置において、
前記シャフトが、エンドエフェクタが前記イメージャの視野に対して相対的に回転され得るように、前記撮像スリーブに対して長手方向軸を中心に回転可能であり、及び/又は、
前記シャフトが、前記イメージャの視野を調整するために前記イメージャをエンドエフェクタに近づける方向および遠ざける方向に導くことができるように、前記撮像スリーブに対して長手方向軸に沿って軸方向に移動可能であることを特徴とする装置。 - 請求項18に記載の装置において、
前記イメージャが、
前記シャフトの遠位端に回転可能に取り付けられたハウジングと、
前記ハウジングから径方向に延びるアームと、
前記アームに保持された撮像要素とを備え、前記ハウジングの回転により前記撮像要素が軸の周りを移動することを特徴とする装置。
