図1Aおよび3Fは、髄内釘109の一例を表し、その髄内釘109は、第一の、遠位部分または端110から、第二の、近位部分または端111に延在する略細長本体を備え得る。細長本体は、骨折した骨の髄内管内部で長手方向に延在するように構成された細長い管状ロッドの形状であり得る。細長ロッドは、その長さに沿って中空であってもよく、または中実であってもよい。細長本体は、釘109の長手方向軸に沿って略直線状であってもよく、または髄内管の解剖学的形状に一致するような一つ以上の湾曲または屈曲を含んでもよい。釘109の断面は、髄内釘109の中心長手方向軸に対して直角に取られており、円形、楕円形(oval、elliptical)、またはその他の任意の好適な断面形状であってもよい。近位部分111は、釘109の遠位部分110に対して拡大された直径または頭部分を有し得る。拡大された頭部分111は、大腿骨の大転子部位で受容されるようにサイズ決定および構成され得る。髄内釘109は、大脳延髄(cephalomedullary)固定のために大腿骨の近位端に位置付けられるように構成され得る。しかしながら、髄内釘109が、骨(例えば、大腿骨、脛骨)および骨折の種類によって、他の手法および部位(例えば、遠位端)を通って位置付けられるように構成され得ることを想定する。

図4A〜4Dは、髄内釘109に類似した、釘201を近位係止するための交差係止特徴が追加された髄内釘201の別の実施形態を表す。髄内釘201は、髄内釘109に関する上述の特徴のうちのいずれかを含み得る。髄内釘201は、例えば、股関節の骨の最も強い部分である大腿骨229の距部における骨の固定とともに、食いつきの収束および分散を提供することによって、二つの相互係止用近位固定装置203、205(例えば、骨アンカー、締結具、またはねじ)をさらに含み得る。よって、カットアウトおよび/または回転に起因する不良のリスクを低減させることができる。

- 第二孔が前記髄内釘の前記遠位部分内に形成され、前記第二孔が、前記第一孔から離間した、請求項1に記載のインプラント。
- 前記釘内で前記第二孔を通って配置可能な第二固定装置をさらに備え、前記第二固定装置がその長さにそってねじ山加工された、請求項9に記載のインプラント。

前記近位部分に形成された第一孔と、
前記遠位部分に形成された第二孔であって、前記第二孔が前記第一孔から離間した第二孔と、
遠位端と近位端とを有し、前記釘内の前記第一孔を通って配置可能な第一固定装置であって、外平面を有し、前記外平面が、当該外平面に溝を画定する第一および第二の角度付けられている端面によって画定されている第一固定装置と、
前記釘内の前記第二孔を通って配置可能な第二固定装置と、を有する髄内釘と、
上面および平面状の底面を有する止めねじであって、前記平面状の底面の一部分が前記第一固定装置の前記外平面に接触するように、前記チャネルの第一部分において螺合受容された止めねじと、を備え、
前記止めねじは、前記止めねじの前記上面から前記底面にかけて雄ねじ山が設けられており、
前記第一固定装置において、前記遠位端は雄ねじ山が設けられており、前記近位端は略円筒外面を有し、前記外平面は前記遠位端と前記近位端との間に配置され、
前記チャネルの前記第一部分が前記細長本体の前記近位端からねじ山が設けられた第一固定装置まで延在している、インプラント。 - 前記チャネルの第二部分が、前記第一孔から前記髄内釘の前記遠位端に延在し、前記第二部分が、内側が滑らかな部分である、請求項12に記載のインプラント。
- 前記第一固定装置が、長手方向軸に沿って近位端部分から遠位端部分に延在する、請求項12に記載のインプラント。
- 前記第一固定装置の前記外平面が、前記第一固定装置の前記長手方向軸に平行に延在する、細長く、凹みのある面である、請求項14に記載のインプラント。
- 前記第一固定装置が、ねじ山が設けられた遠位部分を有する、請求項14に記載のインプラント。
- 前記第一固定装置が、前記遠位端部分から延在するヘリカルブレードを有する、請求項14に記載のインプラント。
- 前記第一固定装置は、前記髄内釘を係止するためのカニューレ処置がなされる、請求項14に記載のインプラント。
- 前記雄ねじ山が設けられた止めねじは、前記チャネルに螺合挿入されるカニューレ処置がなされる、請求項12に記載のインプラント。
