Description
本出願は、2015年6月3日出願の米国仮特許出願62/170,579に基づく優先権を主張し、その内容は引用によりここに明確に包含させる。
グルココルチコイド誘導性TNF受容体(GITR)は、活性化エフェクターT(Teff)細胞によりおよび低レベルでB細胞、好酸球、好塩基球およびマクロファージにより発現されるT細胞共刺激I型細胞表面膜貫通糖タンパク質である(Nocentini et al., Adv Exp Med Biol 2009;647:156-73)。GITRは制御性T(Treg)細胞で構成的に発現される。GITRを介するシグナル伝達は、T細胞のT細胞受容体活性化を増強し、Teff細胞をTreg介在抑制に抵抗性とする(Stephens et al., J Immunol 2004;173:5008-20)。腫瘍組織におけるGITR陽性の決定は、抗GITR免疫療法から利益を受ける(すなわち、応答性である)可能性が最も高い患者を標的とするならびにGITR陽性腫瘍の状態および抗GITR免疫療法の有効性のモニタリングをする機会を提供し得る。
診断用抗GITR抗体、および腫瘍組織サンプルのような生体サンプルにおけるGITRタンパク質発現を検出する方法がここに提供される。このような方法は、例えば、抗GITR免疫療法に応答する可能性がある癌患者の同定およびGITR陽性腫瘍の状態および抗GITR免疫療法の有効性のモニタリングに有用であり得る。
(a)患者からの腫瘍サンプルと配列番号5〜7に各々示すアミノ酸配列を含む重鎖可変領域CDR1、CDR2およびCDR3ならびに配列番号8〜10に各々示すアミノ酸配列を含む軽鎖可変領域CDR1、CDR2およびCDR3を含む抗体またはその抗原結合部分を接触させ、ここで、抗体またはその抗原結合フラグメントはヒトGITRに特異的に結合するものであり、
(b)サンプル中のGITRへの抗体の結合を検出し、そして
(c)サンプル中のGITRタンパク質発現レベルを決定することを含み、ここで、閾値レベルを超えるGITRタンパク質レベルは、該腫瘍がGITR陽性であることを示すものである、
方法である。

(b)腫瘍がGITR陽性であると決定されたら、患者にGITRシグナル伝達を調節する薬剤を投与する

(e)第一の時点と第二の時点で決定されたGITRタンパク質発現のレベルを比較することを含み、ここで、第二の時点に比して第一の時点のレベルが高いのは腫瘍退縮の指標であり、第二の時点に比して第一の時点のスコアが低いのは腫瘍進行の指標であり、そして第一の時点と第二の時点のレベルが相対的に変わらないのは腫瘍安定の指標であるものである、

(c)工程(a)と同じ抗体を使用して抗GITR免疫療法開始後の第二の時点でGITR陽性腫瘍におけるGITRタンパク質発現を検出し、
(d)第二の時点での腫瘍におけるGITRタンパク質発現レベルを決定し;
(e)第一の時点と第二の時点で決定されたGITRタンパク質発現のレベルを比較することを含み、ここで第二の時点に比して第一の時点のレベルが高いのは有効な抗GITR免疫療法の指標であり、第二の時点に比して第一の時点のスコアが低いのは抗GITR免疫療法が無効である指標であり、そして第一の時点と第二の時点のスコアが変わらないのは抗GITR免疫療法が安定的であるとの指標であるものである、

診断用抗GITR抗体を使用する、生体サンプル、例えば、腫瘍検体におけるGITR発現の検出ための種々の方法がここに開示される。このような抗体は、例えば、対象におけるGITRタンパク質レベルのモニタリング、腫瘍組織のような特定の組織におけるGITR陽性である特定の細胞の同定(例えば、Treg細胞および/または腫瘍細胞におけるGITRの存在の同定)、癌の早期検出、既存の癌のモニタリングおよび同定ならびに治療的抗GITR免疫療法の有効性の評価に有用であり得る。
本明細書の記載がより容易に理解されるために、まずいくつかの用語を定義する。さらなる定義は、詳細な記載をとおして示される。
ここに記載する種々の態様を次の小節にさらに詳細に記載する。
ここに提供されるのは、ヒト組織、例えば、腫瘍組織におけるGITRタンパク質発現の検出に有用な診断用抗GITR抗体である。このような抗体は、例えば、抗GITR免疫療法での処置により利益を得る(すなわち、応答する)可能性のある対象の同定/選択、GITR陽性腫瘍の進行のモニタリングおよび/または抗GITR免疫療法の有効性のモニタリングに有用である。
ここに記載する診断用抗GITR抗体を、当分野で認められている技術を使用して製造できる。ある実施態様において、抗体を、抗体(例えば、6G10抗体)を産生するハイブリドーマから精製できる。例えば、抗体を産生するハイブリドーマを、2リットルスピナーフラスコでモノクローナル抗体精製用に増殖できる。上清を濾過し、濃縮し、その後プロテインA−セファロース(Pharmacia, Piscataway, N.J.)を用いる親和性クロマトグラフィーに付し得る。溶出IgGをゲル電気泳動および高速液体クロマトグラフィーで調べて、純度を確認できる。緩衝液をPBSに交換し、濃度を、1.43吸光係数を使用するOD280により決定できる。

(e)第一の時点と第二の時点で決定されたGITRタンパク質発現のレベルを比較することを含み、ここで第二の時点に比して第一の時点のレベルが高いのは有効な抗GITR免疫療法の指標である可能性があり、第二の時点に比して第一の時点のスコアが低いのは抗GITR免疫療法が無効である指標であり、そして第一の時点と第二の時点のスコアが変わらないのは抗GITR免疫療法が安定的であるとの指標である可能性があるものである、

ここに記載する診断用抗GITR抗体および使用のための指示を含むキットがここに提供される。従って、ある実施態様において、キットは、配列番号5〜7および8〜10に各々示す重鎖および軽鎖可変領域CDR配列、配列番号11および12に各々示す重鎖および軽鎖可変領域配列または配列番号13および14に各々示す抗体6G10の完全長重鎖および軽鎖配列を含む抗体を含み、ここで、抗体またはその抗原結合フラグメントはヒトGITRに特異的に結合する。ある実施態様において、重鎖はC末端リシンを欠く。ある実施態様において、抗GITR抗体は非ヒト定常領域(例えば、マウス定常領域)以外抗体6G10の可変領域を含むキメラ抗体、例えば、配列番号15および16に各々示す重鎖および軽鎖配列を含むキメラ抗体である。ある実施態様において、キメラ抗体の重鎖はC末端リシンを欠く。ある実施態様において、抗体は検出可能標識、例えば、抗体−標識コンジュゲートを含む。このようなキットは、少なくとも一つのさらなる試薬を含み得る。例えば、ある実施態様において、キットは緩衝液、安定化剤、基質、免疫検出剤(例えば、免疫組織化学において使用する二次抗体)および/またはアッセイに必要な補助因子を含む。ある実施態様において、抗体および、場合により試薬は、適切には等分される。ある実施態様において、キットは、癌患者から生体サンプルを得るための手段を含む。このような手段は、例えば、癌患者から液体または組織サンプルを得るのに使用できる試薬を含む。
ヒト抗GITR抗体6G10を、HuMAb(登録商標)トランスジェニックマウス(“HuMAb”はMedarex, Inc., Princeton, New Jerseyの商標である)およびKMマウス(KMマウス(登録商標)系は、PCT公開WO02/43478号に記載のとおりSC20導入染色体を含む)の免疫化の過程で産生し、GITRに結合する抗体への融合についてスクリーニングした(同時出願のPCT出願PCT/US15/33991号参照)。cDNA配列決定は、6G10抗体について1重鎖および1軽鎖を同定した。6G10抗体の可変領域アミノ酸配列およびアイソタイプを図1〜4および表2に示す。
6G10抗体の可溶性GITRへの結合をBiacoreにより決定した。6G10抗体をヒトカッパ被覆チップ(約5KRU;Southernbiotech)に捕捉し、組み換えヒトGITR(rHGITR/Fc:R&D systems)を500nM、250nM、125nM、62nMおよび31nM濃度でチップを通して流した。抗体/体積の捕捉濃度は、2〜40μg/mL(10μL/分で5μL)であった。抗原結合時間は、15μL/分で5分であり、抗原解離時間は6分であり、再生を50mM HCl/50mM NaOH(100μL/分で各12μL)で実施した。6G10は、可溶性ヒトGITRへの結合について1.87×10−9MのKDを示し、3.83×105M−1s−1のkaおよび7.15×10−4s−1のkdであった。
2種の免疫組織化学(IHC)技術を、組織切片におけるGITR検出に使用した。一つのIHC方法をホルマリン固定パラフィン包埋(FFPE)組織切片で行い、他方を凍結組織切片で実施した。
数抗GITR抗体がこれらのIHC方法で試験されており、6G10が一貫して良好であった。
ヒト組織におけるGITR発現の検出のために、蛍光標識形態の6G10抗体を調製し、6G10−FITCと名づけた。利用者がおよび臨床で扱いやすい方法をさらに開発するために、6G10抗体をマウスキメラIgG2a形式に操作し、Leica BondRXプラットホームを使用する自動化免疫組織化学のために最適化した。キメラ6G10抗体の重鎖および軽鎖配列を各々図5および6および表2に示す。重鎖はC末端リシンを含んでも含まなくてもよい。
簡潔には、通常の組織学法に従いスライドを脱パラフィン処理および再水和し、Decloaking Chamber(BioCareMedical)を使用して10分、110℃でpH6標的回収緩衝液(Dako)と共に手動でHIER(熱誘導エピトープ回復)に付した。次いでスライドをLeica BondRX自動化免疫組織化学着色剤に載せた。組織切片をキメラ6G10抗体と0.5μg/mlで60分インキュベートし、続いてLeica Bond Polymer Refine Detection System(DS9800)で製造業者の指示に従い検出した。簡潔には、組織切片をウサギ抗マウスリンカー(Leica)と30分およびNovolinkMaxポリマー(Leica)と30分インキュベートした。最後に、スライドをDAB基質−色素原溶液と6分反応させた。次いでスライドを通常の組織学的方法に従いマイヤーのヘマトキシリンで対比染色し、脱水し、浄化し、パーマウントのカバーガラスをした。0.5%Hu−ガンマ−グロブリン(Sigma)添加Dakoタンパク質ブロックを、非特異的結合の遮断に使用し、0.5%ヒトガンマグロブリンおよび0.5%BSA添加PBSを、一次および二次抗体療法の希釈剤として使用した。
免疫染色後、スライドを光学顕微鏡下に試験した。GITRおよびTILの採点のために、Aperio(登録商標)ScanScopeおよびHALOTM画像解析ソフトウェアを使用する自動化および定量解析を、スライド全体で実施した。代表的画像をAperio ScanScopeまたはOlympus DP71デジタル・マイクロスコープ・カメラで撮った。
表1は、子宮頸癌(CC)、黒色腫(Mel)、肝細胞癌(HCC)、結腸直腸癌(CRC)および頭頸部扁平上皮細胞癌(HNSCC)におけるGITR陽性TILの分布を要約する。顕微鏡20×対物レンズ下の陽性細胞数の概算による13〜26ケースの手動スコア。最小、<1細胞/20×対物レンズ視野数;軽度、1〜<10細胞/20×対物レンズ視野数;中程度、10〜<50細胞/20×対物レンズ視野数;顕著、50〜<200細胞/20×対物レンズ視野数;強い、>200細胞/20×対物レンズ視野数。腫瘍切片全体の少なくとも1/3が上記基準に適合した。
GITR陽性染色は、NSCLC(非小細胞肺癌)、CC、HNSCCおよびHCCにおける腫瘍細胞でも観察された(図7)。NSCLCおよびCCにおいて、染色の特異性を、GITR融合タンパク質とのインキュベーションによる染色の完全退色により確認した。
種々の量のGITR+TILが3腫瘍タイプで観察された。FoxP3はTILの小サブセットに存在した。腫瘍細胞サブセットの抗GITR抗体での染色も観察され、染色の特性をGITR融合タンパク質とのインキュベーションによる染色の完全退色により確認した。GITR腫瘍細胞におけるGITR mRNAの存在も、凍結切片のRNAscopeで観察され、これはさらにIHC結果を確認した。
これらの結果は、抗GITR抗体6G10が固定および凍結組織サンプルにおけるヒトGITRタンパク質を効率的に検出できることを示す。
当業者は、ここに開示する特定の実施態様の多くの均等物を認識し、または日常的なものを超えない実験を使用して確認することができる。このような均等物は添付する特許請求の範囲に包含されることが意図される。
Claims (7)
- ヒト組織サンプルにおける、腫瘍浸潤性リンパ球(TIL)および/または腫瘍細胞のグルココルチコイド誘導性TNF受容体(GITR)発現レベルを決定する方法であって、
(a)患者からの組織サンプルと、配列番号15および16に各々示す重鎖および軽鎖配列を含み、ここで、重鎖は場合によりC末端リシン残基を欠く、抗体を接触させ、ここで、抗体はヒトGITRに結合するものであり、
(b)組織サンプルにおけるTILおよび腫瘍細胞のGITRへの抗体の結合を免疫組織化学により検出し、
(c)GITRへの抗体の結合と対照とを比較して、ヒトGITRに対する抗体の特異性を確かめ、そして場合により
(d)組織サンプルを、TILおよび/または腫瘍細胞のマーカーおよび/またはヘマトキシリンおよびエオシンで染色し、工程(b)においてGITR陽性として同定されたTILおよび/または腫瘍細胞を同定する
ことを含む、方法。 - TILが制御性T細胞である、請求項1に記載の方法。
