Abstract
Description
従って一態様において、本発明は、MDSの治療及び/又は予防のための、IL-1β結合抗体又はその機能性断片(簡潔にする理由で、用語「IL-1β結合抗体又はその機能性断片」は本願では時に「本発明の薬物(DRUG of the invention)」と称され、これらは同じ用語と理解されなければならない)、好適にはカナキヌマブ又はその機能性断片(「本発明の薬物」に含まれる)、ゲボキズマブ又はその機能性断片(「本発明の薬物」に含まれる)の使用を提供する。
本明細書で使用されるとき、IL-1β阻害薬としては、カナキヌマブ又はその機能性断片、ゲボキズマブ又はその機能性断片、アナキンラ、ジアセレイン、リロナセプト、IL-1 Affibody(SOBI 006、Z-FC(Swedish Orphan Biovitrum/Affibody))及びルチキズマブ(ABT-981)(Abbott)、CDP-484(Celltech)、LY-2189102(Lilly)が挙げられるが、これらに限定されない。
(1)アミノ酸配列RASQSIGSSLH(配列番号1)、ASQSFS(配列番号2)、及びHQSSSLP(配列番号3)を有する3つのVL CDRと、アミノ酸配列VYGMN(配列番号5)、IIWYDGDNQYYADSVKG(配列番号6)、及びDLRTGP(配列番号7)を有する3つのVH CDRとを含む抗体;
(2)アミノ酸配列RASQDISNYLS(配列番号9)、YTSKLHS(配列番号10)、及びLQGKMLPWT(配列番号11)を有する3つのVL CDRと、アミノ酸配列TSGMGVG(配列番号13)、HIWWDGDESYNPSLK(配列番号14)、及びNRYDPPWFVD(配列番号15)を有する3つのVH CDRとを含む抗体;及び
(3)(1)又は(2)のいずれかに記載されるとおりの6つのCDRを含む抗体であって、CDR配列のうちの1つ以上、好ましくはCDRのうちの多くても2つ、好ましくはCDRのうちの1つのみが、それぞれ(1)又は(2)のいずれかに記載される対応する配列と1アミノ酸だけ異なる抗体
が含まれる。

(1)配列番号4に指定されるアミノ酸配列を有するVLを含み、且つ配列番号8に指定されるアミノ酸配列を有するVHを含む抗体;
(2)配列番号12に指定されるアミノ酸を有するVLを含み、且つ配列番号16に指定されるアミノ酸配列を有するVHを含む抗体;及び
(3)(1)又は(2)のいずれかに記載される抗体であって、重鎖の定常領域、軽鎖の定常領域又は両方が、カナキヌマブ又はゲボキズマブと比較したとき異なるアイソタイプに変更されている抗体

IL-1β抗体又はその機能性断片などのIL-1β阻害薬が、少なくとも部分的炎症基盤のある癌を有する患者のhsCRPレベルを有効に低下させることのできる用量範囲で投与された場合には、前記癌の治療効果が実現し得る可能性がある。特定のIL-1β阻害薬、好ましくはIL-1β抗体又はその機能性断片がhsCRPレベルを有効に低下させることのできる用量範囲は公知であり、又は臨床セッティングで試験することができる。
一態様において、本発明は、正常より高いC反応性タンパク質(hsCRP)レベルを有する患者のMDSの治療におけるIL-1β結合抗体又はその機能性断片、好適にはカナキヌマブ又はゲボキズマブの使用を提供する。
一態様において、本発明は、それを必要としている患者のMDSの治療における使用のためのIL-1β結合抗体又はその機能性断片、好適にはカナキヌマブ又はゲボキズマブを提供し、ここでは前記患者の血管新生を阻害するため治療量が投与される。理論によって拘束されることを望むものではないが、IL-1β経路の阻害は、腫瘍成長及び腫瘍転移の鍵となるイベントである血管新生の阻害又は低減につながり得ると仮定される。臨床セッティングでは、血管新生の阻害又は低減は、腫瘍縮小、腫瘍成長がないこと(病勢安定)、転移の予防又は転移の遅延によって測定することができる。
理論によって拘束されることを望むものではないが、IL-1β経路の阻害は腫瘍転移の阻害又は低減につながり得ると仮定される。これまでのところ、カナキヌマブが転移に及ぼす効果に関する報告はない。実施例1に提供されるデータは、IL-1βが転移部位と比較して原発部位で異なる転移促進機構を活性化する:乳癌細胞による内因性IL-1β産生が上皮間葉転換(EMT)、浸潤、遊走及び臓器特異的ホーミングを促進することを実証している。腫瘍細胞が骨環境に到達すると、腫瘍細胞と骨芽細胞又は骨髄細胞との間の接触によって3つ全ての細胞型からのIL-1β分泌が増加する。これらの高いIL-1β濃度は、播種性腫瘍細胞から顕性転移への成長を刺激することにより骨転移性ニッチの増殖を引き起こす。カナキヌマブ又はゲボキズマブなどの抗IL-1β治療の投与により、こうした転移促進過程が阻害される。
一態様において、本発明は、患者の癌、例えば少なくとも部分的炎症基盤がある癌の予防におけるIL-1β結合抗体又はその機能性断片、好適にはカナキヌマブ又はゲボキズマブの使用を提供する。用語「予防する」、「予防すること」又は「予防」は、本明細書で使用されるとき、本来癌の発症リスクが高い対象における癌の発生の予防又は遅延を意味する。用語「予防する」、「予防すること」又は「予防」はまた、本明細書で使用されるとき、先行するMDSを有した対象における続発性急性骨髄性白血病(AML)の発生の予防又は遅延も意味する。MDSは高頻度で続発性AMLに進行する。
用語のネオアジュバント治療は、通常、術前の放射線療法又は化学療法と理解される。ネオアジュバント療法の目的は、通常、腫瘍の切除をより容易にする又はより完全にするため腫瘍サイズを低下させることである。MDSは液性腫瘍であるためMDSでは外科的腫瘍切除が不可能であることに伴い、古典的な意味ではMDSにはネオアジュバント治療は適用できない。しかしながら、別の種類の手術がMDSの治療に用いられ、それは造血細胞移植である。この意味では、造血細胞移植前にMDSにネオアジュバント治療を適用することができる。特に、患者は多くの場合に好適なドナーを待つ必要があるため、その待ち時間の間にネオアジュバント治療を用いることができる。
一実施形態において、本発明は、MDSのファーストライン治療としての使用のためのIL-1β抗体又はその機能性断片、好適にはカナキヌマブ又はゲボキズマブを提供する。用語「ファーストライン治療」は、患者が1つ以上の他の療法剤による初期治療に抵抗性を生じる前に前記患者にIL-1β抗体又はその機能性断片を与えることを意味する。好ましくは1つ以上の他の療法剤は、白金系単剤療法若しくは組み合わせ療法、チロシン阻害薬療法などのターゲット療法、チェックポイント阻害薬療法又はこれらの任意の組み合わせである。ファーストライン治療として、カナキヌマブ又はゲボキズマブなどのIL-1β抗体又はその機能性断片は、単剤療法として、又は好ましくは、チェックポイント阻害薬、特にPD-1又はPD-L1阻害薬、好ましくはペンブロリズマブなどの1つ以上の療法剤との組み合わせで、1つ以上の小分子化学療法剤を伴い又は伴わず患者に投与することができる。一実施形態において、ファーストライン治療として、カナキヌマブ又はゲボキズマブなどのIL-1β抗体又はその機能性断片は、MDSに対する標準ケア療法との組み合わせで患者に投与することができる。好ましくはカナキヌマブ又はゲボキズマブは、疾患進行時までファーストライン治療として投与される。
一実施形態において、本発明は、MDSのセカンド又はサードライン治療としての使用のためのIL-1β抗体又はその機能性断片、好適にはカナキヌマブ又はゲボキズマブを提供する。用語「セカンド又はサードライン治療」は、1つ以上の他の療法剤の最中又はその後の癌進行、特に当該の癌に対するFDAが承認したファーストライン療法の最中又はその後の癌進行を有する患者にIL-1β抗体又はその機能性断片が投与されることを意味する。好ましくは1つ以上の他の療法剤は、白金系単剤療法剤又は組み合わせ療法剤などの化学療法剤、チロシン阻害薬療法剤などのターゲット療法剤、チェックポイント阻害薬又はこれらの任意の組み合わせである。セカンド又はサードライン治療として、IL-1β抗体又はその機能性断片は、単剤療法として、又は好ましくは、同じ1つ以上の療法剤による先行治療の継続を含めた1つ以上の療法剤との組み合わせで患者に投与することができる。好ましくはカナキヌマブ又はゲボキズマブは、疾患進行時までセカンド/サードライン治療として投与される。
一態様において、本発明はまた、MDSの治療における使用のためのIL-1β結合抗体又はその機能性断片、好適にはゲボキズマブ又はカナキヌマブも提供し、ここでIL-1β結合抗体又はその機能性断片は2ライン以上の治療において患者に投与される。
一態様において、本発明は、それを必要としている患者のMDSの治療における使用のための、放射線療法との組み合わせでの、又は1つ以上の療法剤、例えば化学療法剤又は例えばチェックポイント阻害薬との組み合わせでの、又は放射線療法及び1つ以上の療法剤の両方との組み合わせでのIL-1β結合抗体又はその機能性断片、好適にはカナキヌマブ又はゲボキズマブを提供する。

本発明の一態様において、IL-1β阻害薬又はその機能性断片はPD-L1阻害薬と共に投与される。一部の実施形態において、PD-L1阻害薬は、FAZ053(Novartis)、アテゾリズマブ(Genentech/Roche)、アベルマブ(Merck Serono及びPfizer)、デュルバルマブ(MedImmune/AstraZeneca)、又はBMS-936559(Bristol-Myers Squibb)から選択される。
本発明の一態様において、IL-1β阻害薬又はその機能性断片はLAG-3阻害薬と共に投与される。一部の実施形態において、LAG-3阻害薬は、LAG525(Novartis)、BMS-986016(Bristol-Myers Squibb)、TSR-033(Tesaro)、IMP731又はGSK2831781及びIMP761(Prima BioMed)から選択される。
自然免疫及び適応免疫の両方におけるTIM-3の免疫調節的役割、並びにAML及びMDSにおける白血病幹細胞上でのその発現を所与とすれば、TIM-3阻害薬は抗腫瘍免疫応答の回復を助け得るのみならず、加えてMDS幹細胞を直接標的化し得る。結果として、TIM-3阻害薬は低リスクMDSにおいて直接的及び間接的な疾患修飾活性を有することができ、これを炎症促進経路に向けられる療法であるIL-1β遮断によって増強し得る可能性がある。
特定の実施形態において、本明細書に記載される組み合わせは抗TIM3抗体分子を含む。一実施形態において、抗TIM-3抗体分子については、2015年8月6日に公開された「Antibody Molecules to TIM-3 and Uses Thereof」と題される米国特許出願公開第2015/0218274号明細書(全体として参照により援用される)に開示されている。
一実施形態において、抗TIM-3抗体分子はTSR-022(AnaptysBio/Tesaro)である。一実施形態において、抗TIM-3抗体分子は、TSR-022のCDR配列のうちの1つ以上(又はまとめて全てのCDR配列)、重鎖又は軽鎖可変領域配列、又は重鎖又は軽鎖配列を含む。一実施形態において、抗TIM-3抗体分子は、例えば表6に開示されるとおりの、APE5137又はAPE5121のCDR配列のうちの1つ以上(又はまとめて全てのCDR配列)、重鎖又は軽鎖可変領域配列、又は重鎖又は軽鎖配列を含む。APE5137、APE5121、及び他の抗TIM-3抗体については、国際公開第2016/161270号パンフレット(全体として参照により援用される)に開示されている。

本発明の一態様において、IL-1β阻害薬又はその機能性断片はGITRアゴニストと共に投与される。一部の実施形態において、GITRアゴニストは、GWN323(NVS)、BMS-986156、MK-4166又はMK-1248(Merck)、TRX518(Leap Therapeutics)、INCAGN1876(Incyte/Agenus)、AMG 228(Amgen)又はINBRX-110(Inhibrx)である。
本発明の一態様において、IL-1β阻害薬又はその機能性断片はIL-15/IL-15Ra複合体と共に投与される。一部の実施形態において、IL-15/IL-15Ra複合体は、NIZ985(Novartis)、ATL-803(Altor)又はCYP0150(Cytune)から選択される。
本発明の一態様において、IL-1β阻害薬又はその機能性断片はCTLA-4の阻害薬と共に投与される。一部の実施形態において、CTLA-4阻害薬は抗CTLA-4抗体又はその断片である。例示的抗CTLA-4抗体としては、トレメリムマブ(旧称チシリムマブ、CP-675,206);及びイピリムマブ(MDX-010、Yervoy(登録商標))が挙げられる。
カナキヌマブは、静脈内投与、又は好ましくは皮下投与することができる。投与経路が指定される実施形態でない限り、両方の投与経路とも、本願に開示されるあらゆるカナキヌマブ関連実施形態に適用可能である。
その良好な安全性プロファイルにより、カナキヌマブ又はゲボキズマブは患者に長期間投与することができ、IL-1β媒介性炎症を抑制する利益を付与し及び維持し得る。更に、その抗癌効果により、単剤療法で使用されても、又は1つ以上の療法剤との組み合わせで使用されても、本発明の治療がない場合と比べて、限定はされないが、DFS、PFS、OSの持続期間の延長、ハザードリスク低下を含め、患者の寿命を延ばすことができる。用語「本発明の治療」は、本願で使用されるとき、本願に教示されるとおりの投与レジメンに従い投与される本発明の薬物、好適にはカナキヌマブ又はゲボキズマブを指す。好ましくは、臨床的有効性は、約3週間毎に又は約毎月、好ましくは少なくとも約6ヵ月間、好ましくは少なくとも約12ヵ月間、好ましくは少なくとも約24ヵ月間、好ましくは最長約2年間、好ましくは最長約3年間にわたって投与される約200mgカナキヌマブの用量で実現する。好ましくは、それらの結果は、約3週間毎に又は約毎月、好ましくは少なくとも約6ヵ月間、好ましくは少なくとも約12ヵ月間、好ましくは少なくとも約24ヵ月間、好ましくは最長約2年間、好ましくは最長約3年間にわたって投与される約30mg~120mgゲボキズマブの用量で実現する。一実施形態において、本発明の治療は単一の治療である。一実施形態において、本発明の治療は適応癌種に対するSoC治療に重ねて追加される。SoC治療は時間とともに展開するが、ここで使用されるとおりのSoC治療に本発明の薬物は含まれないものと理解されなければならない。
一態様において、本発明は、癌、例えば少なくとも部分的炎症基盤のある癌の治療における使用のための、単独での、又は1つ以上の療法剤との組み合わせでのIL-1β結合抗体又はその機能性断片、好適にはゲボキズマブ又は好適にはカナキヌマブを提供し、ここで前記癌には骨髄異形成症候群(MDS)、好適には低リスクMDSが含まれ、又は前記癌には、慢性骨髄単球性白血病(CMML)、骨髄増殖性新生物(MPN)、及び多発性骨髄腫(MM)などの他の骨髄性新生物が含まれる。
腫瘍由来IL-1βは転移における差次的腫瘍促進機構を誘導する
材料及び方法
細胞培養
ヒト乳癌MDA-MB-231-Luc2-TdTomato(Calliper Life Sciences、Manchester UK)、MDA-MB-231(親)MCF7、T47D(European Collection of Authenticated Cell Cultures(ECACC))、MDA-MB-231-IV(Nutter et al.,2014)並びに骨髄HS5(ECACC)及びヒト初代骨芽細胞OB1をDMEM+10%FCS(Gibco、Invitrogen、Paisley、UK)において培養した。細胞株は全て、加湿インキュベーターにおいて5%C02下で培養し、20継代を超える低継代数で使用した。
C末端GFPタグ(OriGene Technologies Inc.Rockville MD)を有するヒトIL1B又はIL1R1(それぞれ、受託番号NM_000576及びNM_0008777.2)を含有するORFプラスミドで形質導入したコンピテント大腸菌(E.Coli)から精製したプラスミドDNAを使用してヒトMDA-MB-231、MCF7及びT47D細胞を安定にトランスフェクトすることにより、遺伝子IL1B又はIL1R1を過剰発現させた。プラスミドDNA精製はPureLink(商標)HiPureプラスミドミニプレップキット(ThemoFisher)を使用して実施し、DNAを紫外分光法により定量化した後にLipofectamine II(ThermoFisher)の助けを借りてヒト細胞に導入した。対照細胞には、IL-1B又はIL-1R1コード配列を含まない同じプラスミドから単離したDNAをトランスフェクトした。
0~5ng/ml組換えIL-1β(R&D systems、Wiesbaden、Germany)±50μM IL-1Ra(Amgen、Cambridge、UK)を加えて及び加えずにインビトロ研究を行った。
ヒト骨移植片を使用した実験は10週齢雌NOD SCIDマウスで行った。IL-1β/IL-1R1過剰発現骨ホーミング実験では、6~8週齢雌BALB/cヌードマウスを使用した。IL-1βが骨微小環境に及ぼす効果を調べるため、10週齢雌C57BL/6マウス(CharlesRiver、Kent、UK)又はIL-1R1-/-マウス(Abdulaal et al.,2016)を使用した。マウスは飼料及び水を自由摂取として12時間:12時間明暗サイクルに維持した。実験は、プロジェクトライセンス40/3531,University of Sheffield、UKに基づく英国内務省の承認を得て行われた。
全ての患者が本試験への参加前に書面によるインフォームドコンセントを提出した。HTAライセンス12182、シェフィールド筋骨格バイオバンク(Sheffield Musculoskeletal Biobank)、University of Sheffield、UKに基づきヒト骨試料を収集した。人工股関節置換術を受けた女性患者の大腿骨頭から、Precisionダイヤモンドウエハブレード(Buehler)を備えたIsomat 4000 Precisionノコギリ(Buehler)を使用して骨梁コアを調製した。続いて骨用トレフィンを使用して5mm直径のディスクを切断した後、滅菌PBS中に周囲温度で保存した。
ヒト骨移植片へのヒト乳癌転移のモデルを作成するため、10週齢雌NOD SCIDマウス(n=10匹/群)に2つのヒト骨ディスクをイソフルラン(isofluorane)麻酔下で皮下移植した。マウスは0.003mgベテルゲシク(vetergesic)の注射を受け、骨移植後1週間にわたって飲用水にセプトリン(Septrin)を加えた。マウスを4週間静置した後、20%Martigel/79%PBS/1%トルエンブルー中の1×105個のMDA-MB-231 Luc2-TdTomato、MCF7 Luc2又はT47D Luc2細胞を2つの後部乳房脂肪体に注射した。毎週、30mg/ml D-ルシフェリン(Invitrogen)の皮下注射後にIVIS(ルミノール)システム(Caliper Life Sciences)を使用して原発腫瘍の成長及び転移の発生をモニタした。実験終了と同時に乳房腫瘍、循環腫瘍細胞、血清及び骨転移を摘出した。以前に記載されているとおり(Nutter et al.,2014;Ottewell et al.,2014a)、リアルタイムPCRによる下流分析用にRNAを処理し、タンパク質分析用に細胞ライセートを、及び組織学用に全組織を取った。
全血を10,000gで5分間遠心し、ELISAアッセイ用に血清を除去した。細胞ペレットを5mlのFSM溶解溶液(Sigma-Aldrich、Pool、UK)に再懸濁して赤血球を溶解させた。残りの細胞を再びペレット化し、PBSで3回洗浄し、PBS/10%FCS溶液に再懸濁した。各群10匹のマウスからの試料をプールした後、Coherent I-90C tenableアルゴンイオン(Coherent、Santa Clara、CA)からの470nMレーザー線を備えたMoFlow高性能セルソーター(Beckman Coulter、Cambridge UK)を使用してTdTomato陽性腫瘍細胞を単離した。555LPダイクロイックロングパス及び580/30nmバンドパスフィルタによりTdTomato蛍光を検出した。細胞の捕捉及び分析はSummit 4.3ソフトウェアを用いて実施した。選別後、細胞を直ちにRNA保護細胞試薬(Ambion、Paisley、Renfrew、UK)に入れ、RNA抽出時まで-80℃で保存した。循環腫瘍細胞数のカウントについては、561nmレーザー及びYL1-Aフィルタ(585/16発光フィルタ)を使用してTdTomato蛍光を検出した。細胞の捕捉及び分析はAttune NxTソフトウェアを用いて実施した。
X線管(電圧、49kV;電流、200uA)及び0.5mmアルミニウムフィルタを備えたSkyscan 1172 X線コンピュータμCTスキャナ(Skyscan、Aartselar、Belgium)を使用してマイクロコンピュータ断層撮影(μCT)分析を行った。以前に記載されているとおり(Ottewell et al.,2008a;Ottewell et al.,2008b)、画素サイズは5.86μmに設定し、走査は近位脛骨の上端から開始した。
Leica RMRB正立顕微鏡及びOsteomeasureソフトウェア(Osteometrics,Inc.Decauter、USA)並びに以前に記載されているとおりの(Ottewell et al.,2008a)コンピュータ画像解析システムを使用して、各マウスにつき3つの非連続H&E染色5μm脱灰脛骨組織切片で骨腫瘍面積を測定した。
哺乳類細胞溶解キット(Sigma-Aldrich、Poole、UK)を使用してタンパク質を抽出した。30μgのタンパク質を4~15%プレキャストポリアクリルアミドゲル(BioRad、Watford、UK)に流し、Immobilonニトロセルロース膜(Millipore)に転写した。非特異的結合を1%カゼイン(Vector Laboratories)でブロッキングした後、1:1000希釈のヒトN-カドヘリン(D4R1H)、1:500希釈のE-カドヘリン(24E10)又は1:500希釈のγ-カテニン(2303)に対するウサギモノクローナル抗体(Cell シグナル伝達)又は1:1000希釈のマウスモノクローナルGAPDH(ab8245)(AbCam、Cambridge UK)と共に4℃で16時間インキュベートした。二次抗体は抗ウサギ又は抗マウス西洋ワサビペルオキシダーゼ(HRP;1:15,000)であり、HRPをSupersignal化学発光検出キット(Pierce)で検出した。Quantity Onceソフトウェア(BioRad)を使用してバンド定量化を行い、GAPDHで正規化した。
RNeasyキット(Qiagen)を使用して全RNAを抽出し、Superscript III(Invitrogen AB)を使用してcDNAに逆転写した。IL-1B(Hs02786624)、IL-1R1(Hs00174097)、CASP(カスパーゼ1)(Hs00354836)、IL1RN(Hs00893626)、JUP(ジャンクションプラコグロビン/γ-カテニン)(Hs00984034)、N-カドヘリン(Hs01566408)及びE-カドヘリン(Hs1013933)の相対的なmRNA発現をハウスキーピング遺伝子グリセルアルデヒド-3-リン酸デヒドロゲナーゼ(GAPDH;Hs02786624)と比較し、ABI 7900 PCRシステム(Perkin Elmer、Foster City、CA)及びTaqmanユニバーサルマスターミックス(Thermofisher、UK)を使用して評価した。Data Assist V3.01ソフトウェア(Applied Biosystems)にCT値を挿入することにより治療群間での遺伝子発現の変化倍数を分析し、CT値が25以下の遺伝子についてのみ遺伝子発現の変化を分析した。
臨床試験AZURE(Coleman et al.2011)に含まれた1,300例の患者から採取した原発性乳房腫瘍コアを含む組織マイクロアレイ(TMA)でIL-1β及びIL-1R1発現を評価した。試料は、転移のエビデンスのないステージII及びIII乳癌患者から治療前に採取された。続いて患者は、10年間にわたるゾレドロン酸を追加した又は追加しない標準アジュバント療法に無作為化された(Coleman et al 2011)。TMAをIL-1β(ab2105、1:200希釈、Abcam)及びIL-1R1(ab59995、1:25希釈、Abcam)に関して染色し、腫瘍細胞又は関連する間質のIL-1β/IL-1R1に関して組織病理学者の手引きに従い盲検的にスコア化した。次に腫瘍又は間質IL-1β又はIL-1R1を疾患再発(任意の部位)又は特に骨(±他の部位)における疾患再発と関連付けた。
ヒト骨移植片へのヒト乳癌自然転移のマウスモデルを利用して、様々な転移段階を通じてIL-1β経路がどのように変化するかを調べた。このモデルを使用すると、トリプルネガティブ(MDA-MB-231)及びエストロゲン受容体陽性(ER+ve)(T47D)の両方の乳癌細胞でIL-1β経路に関連する遺伝子の発現レベルが転移過程の各段階で段階的に増加した:IL-1βシグナル伝達経路に関連する遺伝子(IL-1B、IL-1R1、CASP(カスパーゼ1)及びIL-1Ra)の発現レベルは、インビトロで成長させたMDA-MB-231及びT47Dの両方の細胞で極めて低く、インビボで転移しなかった同じ細胞からの初代乳房腫瘍では、これらの遺伝子の発現は変化しなかった(図1a)。
腫瘍細胞接着及び上皮間葉転換(EMT)に関連する遺伝子の発現レベルは、骨に転移した原発腫瘍では、転移しなかった腫瘍と比較して有意に変化した(図1c)。IL-1β過剰発現細胞を作成して(MDA-MB-231-IL-1B+、T47D-IL-1B+及びMCF7-IL-1B+)、腫瘍由来IL-1βがEMT及び骨への転移の誘導に関与するかどうかを調べた。IL-1β+細胞株は全て、上皮表現型から間葉表現型への形態学的変化を呈するEMTの増加(図3a)並びにE-カドヘリン、及びJUP(ジャンクションプラコグロビン/γ-カテニン)の発現低下及びN-カドヘリン遺伝子及びタンパク質の発現増加(図3b)を実証した。IL-1βシグナル伝達が増加した腫瘍細胞では、そのそれぞれの対照と比較して、創傷閉鎖(MDA-MB-231-IL-1β+においてp<0.0001(図3d);p<0.001 MCF7-IL-1β+及びT47D-IL-1β+)並びにマトリゲルを通って骨芽細胞に向かう遊走及び浸潤が増加した(MDA-MB-231-IL-1β+(図3c)p<0.0001;MCF7-IL-1β+及びT47D-IL-1β+ p<0.001)。ヒト骨移植片に自然転移したER陽性及びER陰性乳癌細胞では、非転移乳癌細胞と比較してIL-1β産生の増加が見られた(図1)。10年の期間のうちに癌再発を経験したAZURE試験(Coleman et al.,2011)に登録したステージII及びIII乳癌患者の原発腫瘍試料では、IL-1βと転移との間に同じ関連性が成り立った。AZURE患者の原発腫瘍におけるIL-1β発現は、骨における再燃及び任意の部位での再燃の両方と相関したことから、このサイトカインの存在が一般に転移において役割を果たしているものと思われることが指摘される。これと一致して、人工的にIL-1βを過剰発現するように乳癌細胞を遺伝子操作すると、インビトロでの乳癌細胞の遊走及び浸潤能力が増加した(図3)。
腫瘍由来のIL-1βがEMTの誘導を通じて転移の発生を促進しているように見えたことに伴い、IL-1Ra(アナキンラ)又はヒト抗IL-1β結合抗体(カナキヌマブ)によるIL-1βシグナル伝達の阻害がヒト骨移植片への自然転移に及ぼす効果を調べた:IL-1Ra及びカナキヌマブは両方とも、ヒト骨への転移を低下させた:10匹中7匹の対照マウスでヒト骨移植片に転移が検出されたが、IL-1Raで治療したマウスは10匹中4匹、及びカナキヌマブで治療したマウスは10匹中1匹で検出されたに過ぎなかった。IL-1Ra及びカナキヌマブ治療群の骨転移はまた、対照群で検出されたものと比べて小さかった(図4a)。カナキヌマブ又はIL-1Raで治療したマウスの循環中に検出された細胞の数は、プラセボ治療群に検出された数と比べて有意に少なかった:カナキヌマブ及びアナキンラで治療したマウスの全血では、プラセボ治療マウスの血中でカウントされた108個の腫瘍細胞/mlと比較して、それぞれ3個の腫瘍細胞/mlがカウントされたに過ぎなかったことから(図4b)、IL-1シグナル伝達を阻害すると原発部位から循環中への腫瘍細胞のシェディングが防止されることが示唆される。従って、抗IL-1β抗体カナキヌマブによるIL-1βシグナル伝達の阻害又はIL-1R1の阻害により、循環中にシェディングされる乳癌細胞の数が減少し、及びヒト骨移植片における転移が減少した(図4)。
マウスの尾静脈に乳癌細胞を注射すると、通常、腫瘍細胞が肺毛細血管に捕捉されることに起因して肺転移が生じる。静脈内注射後に骨微小環境に優先的にホーミングする乳癌細胞はIL-1βを高度に発現することが以前示されており、このサイトカインが乳癌細胞の骨への組織特異的ホーミングに関与し得ることが示唆される。本研究では、MDA-MB-231-IL-1β+細胞をBALB/cヌードマウスに静脈内注射すると、骨転移を生じる動物の数(75%)が対照細胞(12%)(p<0.001)の細胞と比較して有意に増加した(図5a)。MDA-MB-231-IL-1β+腫瘍は、対照細胞と比較してマウス骨に有意に広い溶骨性病変の発生を引き起こし(p=0.03;図5b)、MDA-MB-231-IL-1β+細胞を注射したマウスでは対照細胞と比較して肺転移が少なくなる傾向があった(p=0.16;図5c)。これらのデータは、内因性IL-1βが骨環境への腫瘍細胞ホーミング及びこの部位における転移の発生を促進し得ることを示唆している。
ヒト骨移植片へのヒト乳癌転移のマウスモデルからの遺伝子分析データからは、原発部位又は循環中の転移細胞と比較して乳癌細胞が骨環境で成長しているときIL-1β経路が更に増加することが示唆された(図1a)。従って、腫瘍細胞が骨細胞と接触したときIL-1β産生がどのように変化するか、及びIL-1βが骨微小環境をどのように変えて腫瘍成長に影響を及ぼすかを調べた(図6)。ヒト乳癌細胞を全ヒト骨片中に入れて48時間培養すると、培地中へのIL-1βの分泌が増加した(MDA-MB-231及びT47D細胞についてp<0.0001;図6a)。ヒトHS5骨髄細胞との共培養により、癌細胞(p<0.001)及び骨髄細胞(p<0.001)の両方に由来するIL-1β濃度の増加が明らかとなり、共培養後に腫瘍細胞からのIL-1βは約1000倍に増加し、HS5細胞からのIL-1Bは約100倍に増加した(図6b)。
臨床セッティングでの知見の意味を確立するため、患者試料におけるIL-1βとその受容体IL-1R1との間の相関を調べた。転移のエビデンスがないステージII/III乳癌患者からの約1300例の原発腫瘍試料(AZURE試験(Coleman et al.,2011)から)をIL-1R1又は活性(17kD)形態のIL-1βに関して染色し、腫瘍細胞及び腫瘍関連間質におけるこれらの分子の発現に関して生検を個別にスコア化した。患者は生検後10年間フォローアップし、骨におけるIL-1β/IL-1R1発現と遠隔再発又は再燃との間の相関について、多変量Coxモデルを用いて評価した。腫瘍細胞のIL-1βは、任意の部位での遠隔再発(p=0.0016)、骨における再発のみ(p=0.017)又は任意の時点での骨における再発(p=0.0387)(図9)と強い相関があった。その腫瘍細胞にIL-1βを有し且つ腫瘍関連間質にIL-1R1を有した患者は、その腫瘍細胞にIL-1βを有しない患者と比較して将来遠隔部位で再燃を経験する可能性がより高かったことから(p=0.042)、腫瘍由来のIL-1βが転移を直接促進し得るのみならず、間質のIL-1R1と相互作用してこのプロセスを促進し得ることが指摘される。従って、IL-1βは、乳癌再燃リスクの予測に使用することのできる新規バイオマーカーである。

PDR001+カナキヌマブ治療は結腸直腸腫瘍におけるエフェクター好中球を増加させる。
RNAシーケンシングを用いて癌におけるカナキヌマブ(ACZ885)の作用機序に関する洞察を得た。CPDR001X2102及びCPDR001X2103臨床試験は、追加的な療法と組み合わせたスパルタリズマブ(PDR001)の安全性、忍容性及び薬力学を判定する。各患者について、治療前、並びに治療3サイクル目に腫瘍生検を採取した。端的には、試料をRNA抽出、リボソームRNA枯渇、ライブラリ構築及びシーケンシングによって処理した。シーケンスリードをSTARによってhg19参照ゲノム及びRefseq参照トランスクリプトームとアラインメントし、遺伝子レベルのカウントをHTSeqによってコンパイルし、M値のトリム平均値を使用した試料レベルの正規化をedgeRによって実施した。
癌の治療におけるスパルタリズマブ(PDR001)との組み合わせでのカナキヌマブ(ACZ885)の有効性。
患者5002-004は、2012年6月に診断され、前レジメンで治療されたステージIICのマイクロサテライト安定性中分化型上行結腸腺癌(MSS-CRC)を当初有した56歳男性である。

癌患者に対するゲボキズマブの用量を選択するための計算。
少なくとも部分的炎症基盤のある癌の治療におけるゲボキズマブの用量選択は、以下のことを考慮に入れつつ、ゲボキズマブの利用可能なPKデータと組み合わせて、CANTOS試験により明らかになった臨床上有効な用量設定に基づく。ゲボキズマブ(約2~5pMのIC50)はカナキヌマブ(約42±3.4pMのIC50)と比較して約10倍高いインビトロ(in virto)効力を示す。0.3mg/kg(約20mg)Q4Wのゲボキズマブ最高用量はhsCRPの低下を示し、2型糖尿病患者においてhsCRPが最大45%低下する可能性がある(図12aを参照のこと)。
抗IL-1β治療の効果に関する前臨床データ。
抗IL-1βヒトIgG1抗体のカナキヌマブは、それがマウスIL-1βと交差反応しないという事実のため、マウス癌モデルで直接判定することができない。マウスサロゲート抗IL-1β抗体が開発されており、マウス癌モデルにおけるIL-1β遮断の効果の判定に用いられている。サロゲート抗体のこのアイソタイプはIgG2aであり、これはヒトIgG1と密接に関係している。
癌の治療における抗PD-1(ペンブロリズマブ)との組み合わせでのカナキヌマブの有効性に関する前臨床データ。
単剤療法としての、又は抗PD-1(ペンブロリズマブ)との組み合わせでのカナキヌマブが腫瘍成長及び腫瘍微小環境に与える影響を評価するためパイロットスタディを設計した。ヒト肺癌細胞株H358(KRAS突然変異体)をBLTマウス異種移植モデルに皮下注射することにより、ヒトNSCLCの異種移植モデルを作成した。
癌の治療におけるドセタキセルとの組み合わせでのカナキヌマブの有効性に関する前臨床データ。
高悪性度肺モデル(LL2)におけるドセタキセルと組み合わせた抗IL-1βの研究では、抗IL-1βで、並びにドセタキセル単独でも、中程度の有効性が認められた。この有効性は、いずれかの群単独又は対照と比較して、組み合わせで亢進した(図15A)。抗IL-1β単独又は組み合わせで、初回投与後5日のPD時点においてIL-1β阻害後に免疫抑制細胞、特に調節性T細胞及び腫瘍中の好中球、単球及びMDSCを含めた抑制性マウス骨髄性細胞の減少が認められた(図15B~図15E)。これらのデータは、提案されるIL-1β阻害作用機序をインビボで実証し得ることを裏付けており、また抗IL-1β単剤療法の何らかの有効性も認められた。
01BSUR及びドセタキセルによる4T1腫瘍の治療は腫瘍微小環境の改変につながる。
4T1腫瘍を右側腹部に皮下(s.c.)移植した雌Balb/cマウスの治療をアイソタイプ抗体、ドセタキセル、01BSUR、又はドセタキセルと01BSURとの組み合わせにより、腫瘍移植後8及び15日で腫瘍が約100mm3に達したところで開始した。カナキヌマブはマウスIL-1βと交差反応しないため、01BSURはマウスサロゲート抗体である。01BSURは、マウスIgG2aサブクラスに属し、これは、カナキヌマブが属するヒトIgG1サブクラスに対応する。初回投与後5日で腫瘍を摘出し、浸潤免疫細胞集団の変化について分析した。これを研究終了時、2回目の投与後4日で再び行った。
01BSUR抗IL-1β単独治療群では、媒体/アイソタイプ対照と比較して腫瘍成長の僅かな鈍化が見られた。この遅延は単剤ドセタキセル群で亢進した。組み合わせ群はドセタキセル単独群と同様の成長の鈍化を示した(図16)。
ドセタキセル単独での、又は01BSURとの組み合わせでの単回治療の後、4T1腫瘍に好中球の減少があった。組み合わせ群は、ドセタキセル単剤群と比べて好中球細胞数のより大幅な減少を示した。単剤01BSURは、4T1腫瘍における好中球の僅かな増加につながったが、これは対照群と比較して有意な変化ではなかった。治療の各々が、媒体/アイソタイプ群と比較して単球の減少につながった。単剤01BSUR治療は、ドセタキセル単独群と比べて単球のより大幅な減少につながった。更に、組み合わせは、対照群と比較して単球の更に大幅な減少を示した(P=0.0481)(図17)。顆粒球及び単球と同様の傾向が顆粒球性及び単球性骨髄由来サプレッサー細胞(MDSC)の間で見られた。ドセタキセル単独及び01BSURとの組み合わせは顆粒球性MDSCの減少につながった。全ての治療が単球性MDSCの減少につながり、組み合わせは単剤のいずれと比べても、より大幅な減少につながった(図18)。
ドセタキセル及び01BSURの2回目の投与から4日後に、4T1腫瘍を免疫細胞浸潤に関して分析した。TIM-3を発現するCD4+及びCD8+T細胞の両方の割合を決定した。ドセタキセル単独は対照群と比較してTIM-3発現細胞の変化につながらなかったが、一方、01BSUR単独での、又はドセタキセルとの組み合わせでの治療後にはTIM-3発現細胞の減少があった。組み合わせ群は、対照と比較したCD4+T細胞について、単剤01BSUR群よりもやや大きいTIM-3発現細胞の減少を示しているように見える(P=0.0063)(図19)。細胞のTregサブセットにも同様の傾向が見られ、組み合わせ群は対照と比較してTIM-3発現細胞の最も大きい減少レベルを示した(P=0.0064)(図20)。
IL-1βの遮断は、自己免疫疾患における炎症性微小環境を変化させる強力な方法であることが示されている。ACZ885(カナキヌマブ)は、CAPS(クリオピリン関連周期性症候群)などの一部の炎症性自己免疫疾患の治療に極めて有効となっている。多くの腫瘍が炎症性微小環境を有するため、IL-1βの遮断については、それが単独で、及びPD-1/PD-L1軸を遮断する働きをするであろう薬剤又はドセタキセルなどの標準ケア化学療法剤との組み合わせで腫瘍微小環境に及ぼし得る影響を決定しようと研究が進められているところである。前臨床実験及びCANTOS試験を通じて、IL-1βの遮断が腫瘍の成長及び発育に影響を及ぼし得ることが示されている。しかしながら、アテローム性動脈硬化症試験であるCANTOS試験では、これが予防的セッティングにおいて、登録時に既知の又は検出可能な癌を有しない患者で判定された。樹立された腫瘍又は転移を有する患者では、IL-1β遮断に対する応答レベルが異なり得る。
IL-1β抗体に対する免疫原性/アレルゲン性の決定
CANTOS試験の期間中、ベースライン時、12、24ヵ月目及び研究終了来院時に免疫原性評価のため血液試料が採取された。免疫原性はブリッジング免疫原性電気化学発光イムノアッセイ(ECLIA)を用いて分析した。試料を酢酸で前処理し、標識薬物(ビオチン化ACZ885及びスルホ-TAG(ルテニウム)標識ACZ885)を含有する緩衝液で中和した。抗カナキヌマブ抗体(抗薬物抗体)をビオチン化及びスルホ-TAG標識形態のACZ885の組み合わせにより捕捉した。続いて複合体形成を電気化学発光により、Mesoscale Discoveryストレプトアビジン(MSD)プレート上への複合体の捕捉によって検出した。
胃食道癌、結腸直腸癌及び膵癌を有するCANTOS試験患者からのバイオマーカー分析をGI群に群分けした。膀胱癌、腎細胞癌及び前立腺癌を有する患者をGU群に群分けした。この群内で、患者をそのベースラインIL-6又はCRPレベルに基づき中央値を上回る群と中央値を下回る群とに更に分割した。癌イベントが起こるまでの時間の平均値及び中央値は、以下の表に示すとおり計算された。
偶発性貧血におけるカナキヌマブ-CANTOS試験
無作為化した10,061例の参加者のうち、417例の女性及び899例の男性にベースライン貧血(ヘモグロビンが女性について12g/dL及び男性について13g/dL未満)があり、一方、4例の参加者はベースラインヘモグロビン測定値を有しなかった。従って、本分析には8,741例のCANTOS参加者を組み入れた。無作為化時及び試験中(ベースライン、無作為化後3、6、9、12、18、24、30、36、42、48、54、及び60ヵ月)にカナキヌマブ群及びプラセボ群の全ての試験参加者から血液試料を採取した。試料は全て、ヘモグロビン、ヘマトクリット、赤血球数、白血球分画、及び血小板数を含め、標準的な血液学的評価にかけた。フォローアップCBCにより、本試験への登録時のヘモグロビン正常値者に生じる男性について13g/dl未満及び女性について12g/dl未満のヘモグロビンと前向きに定義される偶発性貧血を評価できるようにした。
Claims (53)
- 患者の骨髄異形成症候群(MDS)の治療及び/又は予防における使用のためのIL-1β結合抗体又はその機能性断片。
- 患者の先行するMDSから起こる続発性急性骨髄性白血病(AML)の予防における使用のためのIL-1β結合抗体又はその機能性断片。
- MDSに少なくとも部分的炎症基盤がある、請求項1又は2に記載の使用のためのIL-1β結合抗体又はその機能性断片。
- 患者のMDSの治療における使用のためのIL-1β結合抗体又はその機能性断片であって、1回の治療当たり約30mg~約200mgの用量又は約30mg~約300mgの用量で投与されるIL-1β結合抗体又はその機能性断片。
- 前記患者が前記IL-1β結合抗体又はその機能性断片の初回投与前に約2mg/L以上の高感度C反応性タンパク質(hsCRP)を有する、請求項1~4のいずれか一項に記載の使用。
- 前記患者が前記IL-1β結合抗体又はその機能性断片の初回投与前に約4mg/L以上又は約10mg/L以上の高感度C反応性タンパク質(hsCRP)を有する、請求項1~5のいずれか一項に記載の使用。
- 前記IL-1β結合抗体又はその機能性断片の初回投与の少なくとも約3ヵ月後に評価した前記患者の高感度C反応性タンパク質(hsCRP)レベルが、約5mg/L、約3.5mg/L、約2.3mg/Lを下回るまで、好ましくは約2mg/Lを下回るまで、好ましくは約1.8mg/Lを下回るまで低下している、請求項1~6のいずれか一項に記載の使用。
- 前記IL-1β結合抗体又はその機能性断片の初回投与の少なくとも約3ヵ月後に評価した前記患者の高感度C反応性タンパク質(hsCRP)レベルがベースラインと比較して少なくとも約20%低下している、請求項1~7のいずれか一項に記載の使用。
- 前記IL-1β結合抗体又はその機能性断片の初回投与の少なくとも約3ヵ月後に評価した前記患者のインターロイキン-6(IL-6)レベルがベースラインと比較して少なくとも約20%低下している、請求項1~8のいずれか一項に記載の使用。
- 前記IL-1β結合抗体又はその機能性断片を約3週間毎又は約4週間毎(毎月)に投与することを含む、請求項1~9のいずれか一項に記載の使用。
- 前記IL-1β結合抗体がカナキヌマブである、請求項1~10のいずれか一項に記載の使用。
- 前記患者に1回の治療当たり約200mg、約250mg、又は約300mgのカナキヌマブを投与することを含む、請求項1~11のいずれか一項に記載の使用。
- カナキヌマブが約3週間毎に投与される、請求項11又は12に記載の使用。
- カナキヌマブが約4週間毎(毎月)に投与される、請求項11又は12に記載の使用。
- カナキヌマブが皮下投与される、請求項11~14のいずれか一項に記載の使用。
- それを必要としている患者のMDSの治療における使用のためのカナキヌマブであって、前記使用が、約200mgの用量のカナキヌマブを約3週間毎又は約4週間毎に皮下投与すること、又は約250mgの用量のカナキヌマブを約3週間毎又は約4週間毎に皮下投与することを含む、カナキヌマブ。
- 前記IL-1β結合抗体がゲボキズマブである、請求項1~10のいずれか一項に記載の使用。
- 前記患者に1回の治療当たり約30mg~約120mgのゲボキズマブを投与することを含む、請求項17に記載の使用。
- 前記患者に1回の治療当たり約30mgのゲボキズマブを投与することを含む、請求項17に記載の使用。

- 前記1つ以上の療法剤、例えば化学療法剤が、MDSの標準ケア薬剤である、請求項27に記載の使用。
- 前記1つ以上の療法剤が、エリスロポエチン、エポエチンアルファ、エポエチンベータ、エポエチンオメガ、エポエチンデルタ、エポエチンゼータ、エポエチンシータ、ダルベポエチンアルファ、メトキシポリエチレングリコール-エポエチンベータを含めた赤血球生成促進剤(ESA);顆粒球コロニー刺激因子(G-CSF);レナリドマイド;アザシチジン(AzaC);デシタビン;アルペリシブ;又はアバトロンボパグ、エルトロンボパグ、ルストロンボパグ、プロメガポエチン、ロミプロスチム、及びトロンボポエチンを含めたトロンボポエチン受容体作動薬(TPO)からなる群から選択される、請求項27又は28に記載の使用。
- 前記1つ以上の療法剤が、ニボルマブ、ペンブロリズマブ、アテゾリズマブ、デュルバルマブ、アベルマブ及びスパルタリズマブ(PDR-001)からなる群から好ましくは選択されるPD-1阻害薬又はPD-L1阻害薬である、請求項27又は28に記載の使用。
