肺癌及び診断現状
2010年には、米国で新たに発症した肺癌の症例は20万件以上であり、これは全ての新たな癌症例の15%を占めている。同時期の肺癌死亡者の推定数は約16万人で、全癌関連死亡者の約28%を占めている。残念ながら、限られた治療選択肢のために、肺癌は癌関連死の最も一般的な原因である。この病気が初期段階で診断されれば、腫瘍の完全な外科的切除が治癒の好機を提供する。従って、この疾患の早期発見は過去数十年に亘って多くの試みの焦点になってきた。胸部X線検査、胸部コンピュータ断層撮影(CT)、陽電子放出断層撮影(PET)スキャンを含む放射線撮影技術を使用した幾つかの臨床試験では、結果がまちまちであり、臨床上のメリットが不明確で且つ非常に高いコストを伴うことが示されてきた。上記のように、胸部CTスキャンを含む肺癌の早期発見のための放射線撮影方法は高線量の放射線を必要とし、それ自体が、頻繁に使用されると二次悪性腫瘍を発症する高いリスクを課す。その結果、胸部CTスキャンを肺癌のスクリーニングに頻繁に使用することは、おそらく安全でも経済的でもない。
正常細胞の完全なエピジェネティック平衡は、細胞が形質転換されると実質的に変化する。得られたDNAレベルのエピジェネティック変化は、次の二つのカテゴリーに分類される:(i)CpG島と称する遺伝子の豊富なゲノム領域の遺伝子プロモーターにおけるCpGの遺伝子特異的な過剰メチル化、及び(ii)ゲノム規模の低メチル化であり、その多くは反復DNA要素において生じる。異常なメチル化パターンは、幾つかの癌のタイプに関連している。ゲノム規模での低メチル化は、悪性度に密接に類似しており、普遍的な所見である。DNA低メチル化の分析は、殆ど未開拓のまま残されている。研究モデルとして広く使用されている癌細胞株は、ゲノム全体の脱メチル化において大きな変動を示し、これは組織特異性を反映するものであり、確率過程から生じる可能性は低い。悪性細胞は、正常な対応物よりも20〜60%少ないゲノムメチルシトシンを含み得る。メチル基の消失は主に、ヒトゲノムの90%超を占める反復DNA配列の低メチル化によって達成され、これには、多くは進化を通してレトロウイルスとして獲得された、短鎖及び長鎖の散在した核要素(SINES及びLINES等の移入可能な要素(ゲノムの約48%)を含む)が含まれる。グローバルなメチル化はまた、多くの癌タイプにおける臨床的結果とグローバルなメチル化レベルの間の関連により示されるように、臨床的に関連性がある。前癌病変が進行するにつれてグローバルなメチル化の損失がより顕著になる傾向があるため、グローバルな低メチル化は癌の進行と関連しているように考えられる。今日まで、差分DNAメチル化分析は、主に電気泳動技術、クロマトグラフィー技術、PCRベースの技術、アレイベースの技術及び配列決定技術を含む分子アプローチによって、定量的に評価されてきた。これら方法の特異性、感度、及び固有の単一塩基分解能の大幅な改善にもかかわらず、それらは単一細胞の高スループット分析においては技術的に困難なままである。これには、PCRに基づく手法の多重化における限界、並びに全ゲノム配列決定、特に反復要素のインターロゲーションについての困難な感度及びコストの問題が含まれる。或いは、DNAメチル化の不均衡、特に反復要素の低メチル化の広がり及び負荷を考慮すると、グローバル核5mCパターンのイメージングに基づく評価は、基礎をなす分子的プロセスが光学顕微鏡で見える大規模のクロマチン再編成を含んでいるので、多数の細胞を同時に分析及び特徴付けるための強力なツールを提供する。
本明細書で実証されるように、定量的DNAメチル化イメージング(3D−qDMI)の方法が開発され、肺癌に適用されている。この非破壊的方法は、細胞及び組織を特徴付けるために、5−メチルシトシン負荷及び空間核分布の並列的な定量的測定を必要とする(次の文献を参照されたい。Tajbakhsh J,et al. 本明細書に記載の特定の実施形態で実施される3D−qDMIソフトウェアは、画像フレーム内において、(集団解析と結果出力ではなく)個々の細胞の洗練された3次元画像解析を実行するように設計され、従って、可変統計のための細胞のフレキシブルな除去及び組み合わせを可能にする。幾つかの実施形態において、5mC/DAPI共局在パターンの結果は、散布図として表すことができる(図2参照)。しかしながら、当業者は、この種類のデータを表す他の多くの方法があることを容易に理解するであろう。
本発明者らは、健康な個体(癌歴のない非喫煙者)の痰由来のヒト上気道細胞、並びに肺癌患者(喫煙者)由来の痰細胞及び一致する組織標本、並びに3つの人細胞株における3D核5mCパターンを調査した。細胞株には、不死化正常ヒト上皮細胞株(BEAS−2B)、及びNSCLC株A549(肺胞基底上皮細胞)、及びH157(高度浸潤性肺癌細胞)が含められた。図4は、新たに診断され、外科的に切除された肺癌由来の、蛍光標識された切片の正常な柔組織及び腫瘍領域を示す。本発明者らは、著しくグローバルに低メチル化された癌患者の癌性の細胞、及び異常な痰細胞の5mC/DAPIパターンとは有意に異なる健康な細胞の間で、共通のグローバルなDNAメチル化パターン(図5)を観察した。3つの異なる細胞株及び痰由来細胞並びに正常組織の全ての集団は、その5mC/DAPI共分布に関して、細胞集団レベル及び個々の代表的な核種についての散布図として表示された、高度の均一性(KLカテゴリーマップによって可視化された)を示した。
方法
細胞検体の調製
痰の誘導は、高張食塩水(3〜5%NaCl)の吸入によって行うことができる。ネブライザーを使用して、1.5mL/分の出力でエアロゾルを生成することができる。対象は、生理食塩水のエアロゾルを最大20分間吸入する。対象は、5分毎に水道水で口を濯いだ後に痰を吐き出すことが推奨される。剥離した上部呼吸器細胞を単離し、スライド/カバースリップまたはマイクロプレートウェル上に固定する。プラスチック容器内に採取したサンプルを、処理するまで4℃に維持する。10%のスプトリシン溶液として一般に知られる0.1%ジチオスレイトール(DTT)含有のリン酸緩衝生理食塩水(PBS)溶液でサンプルを希釈し、20分間インキュベートしてから、細胞及び液体(粘液)相を分離するために、300〜1500×gにおいて室温で5〜10分間遠心分離する。このプロセスは、細胞懸濁液が均質かつ透明に見えるようになるまで繰り返される。次いで、細胞ペレットをPBS中に再懸濁させ、細胞を40〜100μmのナイロンメッシュ(細胞ストレーナー)で濾過して、残留粘液及び残屑を除去する。次いで、細胞を300〜1500×gで5〜10分間遠心分離する。その細胞ペレット(全ての収穫された細胞を含む)を、1〜2マイクロリットルの上皮細胞培地に再懸濁させ、顕微鏡ガラスカバースリップ上に移し、37℃及び5%CO2で16〜48時間培養して、細胞を該カバースリップに付着させる。幾つかの実施形態では、遠心分離されたサンプル(サイトスピン)に対して細胞計数を行い、細胞サンプルを顕微鏡スライド/カバースリップまたはマイクロプレートウェル上に広げる。続いて、細胞を4%パラホルムアルデヒド中で15分〜45分間固定し、4℃でPBS中に維持する。次に、本明細書に記載の3D定量的DNAメチル化イメージング(3D−qDMI)により、固定細胞の特徴付けが達成される。

以下の非限定的なプロトコールは、18mmの丸いガラスカバースリップ(No.1)上に捕捉され、12ウェルのマイクロプレートにおいて処理される痰由来細胞の簡便な処理のためのものである。試薬の量は、他の細胞支持体及び反応チャンバについて調節する必要がある。
(a)組織切片の固定
1)痰由来細胞を、4%パラホルムアルデヒド(PFA)/PBS中において室温で30〜45分間固定し、次いで室温で3〜5分間PBSで3回洗浄する。直ちに処理しない細胞は、2〜8℃において、0.002%NaN3/PBS中で更に保存すべきである。
2)細胞をPBS(2mL)中で5分間洗浄する。
3)室温で20分間、0.5%サポニン/0.5%TritonX−100/PBS(5mL)で細胞を透過性処理し、室温において3〜5分間PBS(2mL)で3回洗浄する。
4)細胞を100μg/mLのRNアーゼA/PBS(0.2mL)を用いて37℃で30分間処理し、室温においてPBS(2mL)で3〜5分間3回洗浄する。
5)37℃で30分間、3%ウシ血清アルブミン(BSA)/PBS(1mL)を用いて組織をブロックする(一次抗体を適用する前に)。
6)組織を、細胞表現型を決定するための一次抗体または適合性抗体のカクテル(例えば、ウサギ抗CK19ポリクローナル抗体、Abcamカタログ番号ab15463、1:1000希釈液;ヒツジ抗CD34ポリクローナル抗体、R&Dシステムズ、カタログ番号AF7227、1μg/mLの濃度で;ニワトリ抗CD45、GeneTexカタログ#GTX82139、1:500希釈)と共に、3%BSA/PBS(0.7mL)中において2〜8℃で一晩インキュベートする。
7)細胞を、室温において0.1%BSA/01%Tween20/PBS(2mL)で5分間、4回洗浄する。
8)組織を、3%BSA/PBS(0.7mL)中の各々5μg/mL濃度の2次抗体(例えば、ロバ抗ヤギIgG(H+L)−Alexa 568、Invitrogen、A11057)と共に、37℃で1時間インキュベートする。
9)組織を、0.1%BSA/0.1%Tween20/PBS(2mL)で4回、室温において3〜5分間洗浄し、また0.1%BSA/PBS(2mL)で1回洗浄する。
10)室温で15分間、4%PFA/PBS(1mL)中で組織を固定する。細胞を、PBSで3〜5分間、3回洗浄する。
11)2N HCl(1mL)を用いて、細胞を室温で正確に40分間脱プリン化し、室温で3〜5分間PBS(2ml)で3回洗浄する。
12)37℃で30分間、3%BSA/PBS(1mL)で細胞をブロックする(一次抗体を適用する前)。
13)3%BSA/PBS(0.7mL)中の1〜2μg/mL濃度の一次抗体(例えばマウス抗MeC、クローン33D3mAb、Aviva Systems Biology、カタログ番号AMM99021)と共に、2〜8℃で細胞を一晩インキュベートする。
14)細胞を、室温において5分間、0.1%BSA/01%Tween20/PBS(2mL)で4回、0.1%BSA/PBS(2mL)で1回洗浄する。
15)細胞を、2次抗体(例えば、ロバ抗マウスAlexa488 IgG(H+L)、Invitrogen A21202、及びニワトリ抗ウサギIgG(H+L)−Alexa 647、Invitrogen A21443)と共に、両者共3%BSA/PBS(0.2mL)中5μg/mLの濃度で、37℃において2時間インキュベートする。
16)組織を、室温において、0.1%BSA/0.1%Tween 20/PBS(5mL)で4回5分間、0.1%BSA/PBSで1回5分間洗浄する。
17)組織を、室温において20分間、DAPI/PBS溶液(室温に温める)5mL中でインキュベートし、非特異的DAPI染色をリンスためにPBS中で約30秒間リンスする。
18)マイクロプレートからカバースリップを取り出し、室温または37℃のオーブン(10〜30分)中において、暗所で完全に乾燥させる。
19)清潔で乾燥したガラススライド区画に7〜10μLのマウント溶液(例:Prolong−Gold、Invitrogen)を移し、該マウント液滴上にカバースリップ(スライドガラスに面した細胞を含む)を置く(厳格に気泡を避ける)。
分子的確認方法は、画像サイトメトリー(3D−qDMI)5mCの特徴的結果を確認するために、単離された細胞(選択された痰サンプルから)から抽出したDNAに対して並行して行うことができる:(i)5mC負荷及び(ii)反復DNA要素類の低メチル化(Alu/LINE−1/Satα/Sat2)−the major causative of global DNA hypomethylation−両者共にRepeat−Sequence MethyLightによって評価することができる(Weisenberger DJ et al.、Analysis of repetitive element DNA methylation by MethyLight.Nucleic Acids Res.2005 Dec 2;33(21):6823−36を参照のこと、その全体を本明細書の一部として援用する)。この方法は、グローバル5mCの含量測定に従来使用されていた、5mC負荷の測定における高速液体クロマトグラフィー(HPLC)または高性能キャピラリー電気泳動(HPCE)の正確な代用品であることが証明されている。Repeat−Seq MethyLight及び3D−qDMIによって比較的に行われた5mCの含量測定値は、非常に高い相関(0.86−0.96)を示した(Gertych A,et al.3−D DNA methylation phenotypes correlate with cytotoxicity levels in prostate and liver cancer cell models.BMC Pharmacol Toxicol.2013 Feb 11;14:11に記載されており、その全体を本明細書の一部として援用する)。
画像取得は、高分解能共焦点走査顕微鏡法を利用して行うことができる。幾つかの非限定的な実施形態では、Leicaの市販されているTCS SP5 X Supercontinuum顕微鏡(Leica Microsystems)が利用される。このシステムは、470〜670nmの連続範囲内において1nm刻みで励起及び発光に完全な自由度及び適応性を提供する。顕微鏡は、DAPI蛍光の励起のために、405nmのダイオードレーザラインと結合させることができる。シリアル光学セクションは、Plan−Apo 63X 1.4オイル浸漬レンズ及びピンホールサイズ1.0のエアリーユニットを使用して、200〜300nm単位で収集することができる。表面滲みを回避するために、各チャネルの画像化を順次取得することができる。非限定的な例として、典型的な画像サイズは、約116nm×116nm×230.5nm(x、y、及びz軸)のそれぞれのボクセルサイズ、及び全てのチャネルにおいて画素当たり8〜16ビットの分解能で、1024×1024〜2048×2048の範囲に亘ることができる。出力ファイルフォーマットは、3D画像解析に利用できる一連のTIFF画像であることができる。

Claims (22)
- 細胞が癌性または前癌性であるかどうかの評価を補助するための方法であって、
前記細胞の核内におけるグローバル5−メチルシトシン(5mC)含量並びに5mC及びグローバルDNA(gDNA)の空間的核共分布を決定するステップを含み、前記細胞の核内におけるグローバル5mC含量並びに5mC及びgDNAの空間的核共分布が、非癌性もしくは非前癌性の参照細胞及び/または非癌性もしくは非前癌性の参照細胞集団とは有意に異なる場合に、前記細胞が癌性または前癌性であると示されるか、或いは、グローバル核5mC含量並びに5mC及びgDNAの空間的核共分布が、非癌性もしくは非前癌性の参照細胞及び/または非癌性もしくは非前癌性の参照細胞集団のものとは有意に異ならない場合に、前記細胞が癌性でもなく前癌性でもないと示される、
方法。 - 前記グローバル5mC含量が、非癌性もしくは非前癌性の参照細胞及び/または非癌性もしくは非前癌性の参照細胞集団よりも有意に低い場合に、前記細胞が癌性または前癌性であると示される、請求項1に記載の方法。

- 前記生物学的サンプルが、喫煙歴のない対象から得られる、請求項5に記載の方法。
- 前記生物学的サンプルが、肺癌を有しており且つ肺癌について治療されたことのない対象から得られる、請求項5に記載の方法。
- 前記生物学的サンプルが、放射線療法、化学療法、外科手術、及びそれらの組み合わせからなる群から選択される肺癌治療を受けたことのある対象から得られる、請求項5に記載の方法。
- グローバル5mC及びgDNAの含量が、前記細胞を(a)5mCに特異的な抗体を用いた免疫蛍光染色に、及び(b)4’,6−ジアミジノ−2−フェニルインドール(DAPI)を用いた対比染色に供した後に、顕微鏡を用いて決定される、請求項1に記載の方法。
- 5mC及びgDNAの空間的核共分布が、前記細胞を(a)5mCに特異的な抗体を用いた免疫蛍光染色に、及び(b)4’,6−ジアミジノ−2−フェニルインドール(DAPI)を用いた対比染色に供した後に、顕微鏡を用いて決定される、請求項1に記載の方法。
- 高張食塩水を対象の気道に投与するステップと、
前記高張食塩水を吸入した結果として前記対象から吐き出されたある量の痰を回収するステップと
を含む方法により、痰サンプルが前記対象から得られた、請求項5に記載の方法。 - 前記高張食塩水がネブライザーを介して投与される、請求項13に記載の方法。
- 前記高張食塩水が3〜5%のNaClである、請求項13に記載の方法。
- 前記顕微鏡が、500ナノメートルまたはそれ未満の分解能を有する共焦点走査顕微鏡である、請求項11または12に記載の方法。
- 癌を発症する高いリスクを対象が有するかどうかの評価を補助するための方法であって、
前記対象の生物学的サンプルから得られた細胞の核内におけるグローバル5−メチルシトシン(5mC)含量並びに5mC及びグローバルDNA(gDNA)の空間的核共分布を決定するステップを含み、前記細胞の核内におけるグローバル5mC含量並びに5mC及びgDNAの空間的核共分布が、非癌性もしくは非前癌性の参照細胞及び/または非癌性もしくは非前癌性の細胞集団とは有意に異なる場合に、前記細胞が癌性または前癌性であると示され、前記細胞が癌性または前癌性であると示された場合に、臨床的に検証可能な癌を発症する高いリスクを前記対象が有すると示される、
