Description
〔1〕CD19xCD3二重特異性単鎖抗体構築物を含む、小児急性リンパ芽球性白血病(ALL)患者における小児ALLの治療、改善または排除のための医薬組成物、
〔2〕前記小児急性リンパ芽球性白血病(ALL)が、小児B細胞系急性リンパ芽球性白血病(ALL)、好ましくは小児B前駆細胞性急性リンパ芽球性白血病ALL、より好ましくは小児プロB ALL、プレB ALLまたはcommon型ALL(cALL)である、〔1〕記載の医薬組成物、
〔3〕前記急性リンパ芽球性白血病(ALL)が、抗療性および/または再発ALLである、〔1〕または〔2〕記載の医薬組成物、
〔4〕小児ALL患者における微小残存病変(MRD)の治療、改善または排除のためのものである、〔1〕〜〔3〕いずれか記載の医薬組成物、
〔5〕前記小児ALL患者が血液学的完全寛解においてMRD陽性である、〔4〕記載の医薬組成物、
〔6〕前記CD19xCD3二重特異性単鎖抗体構築物が、投与期間にわたって一定の用量で投与される、〔1〕〜〔5〕いずれか記載の医薬組成物、
〔7〕注入期間の最初の1日または複数日の間、低用量のCD19xCD3二重特異性単鎖抗体構築物が投与され、続いて、残りの注入期間の間、高用量のCD19xCD3二重特異性単鎖抗体構築物が投与される、〔1〕〜〔6〕いずれか記載の医薬組成物、
〔8〕注入期間の最初の1日または複数日に、患者の体表面積1平方メートルあたり5μgのCD19xCD3二重特異性単鎖抗体構築物が投与され、続いて、残りの期間の1日用量として、患者の体表面積1平方メートルあたり15μgのCD19xCD3二重特異性単鎖抗体構築物が投与される、〔7〕記載の医薬組成物、
〔9〕注入期間の最初の1日または複数日に、患者の体表面積1平方メートルあたり5μgのCD19xCD3二重特異性単鎖抗体構築物が投与され、続いて、残りの期間の1日用量として、患者の体表面積1平方メートルあたり30μgまたは45μgのCD19xCD3二重特異性単鎖抗体構築物が投与される、〔7〕記載の医薬組成物、
〔10〕2種類の初期用量が、2、3、4、5、6もしくは7日間またはさらに長期間投与される、〔7〕〜〔9〕いずれか記載の医薬組成物、
〔11〕前記CD19xCD3二重特異性単鎖抗体構築物が、連続的に投与される、〔1〕〜〔6〕いずれか記載の医薬組成物、
〔12〕連続注入がポンプシステムによって実現される、〔11〕記載の医薬組成物、
〔13〕投与が、静脈内投与もしくは皮下投与、非経口投与、経皮投与、腹腔内投与、筋内投与または肺的投与である、〔11〕記載の医薬組成物
に関する。
1. 以下の実施例に示すように、CD19xCD3二重特異性単鎖抗体の投与は、再発および/または抗療性の小児急性リンパ芽球性白血病(ALL)の治療に使用することができる。CD19xCD3二重特異性単鎖抗体は、同種幹細胞移植に適さない小児患者における従来の小児急性リンパ芽球性白血病(ALL)治療(例えば化学療法)に取って代わり得るだけではない。それは、前記移植に適する小児ALL患者を、MRD陰性状態に変換するためにも使用することができる。本発明のこの局面は、MRD陰性患者がMRD陽性患者よりも低い移植後の再発のリスクを有することにおいて重要である。最良の場合の筋書きにおいて、CD19xCD3二重特異性単鎖抗体を用いた小児ALL治療により、化学療法および/またはHSCTは余分なものになる。

B細胞ALL治療について、蛍光細胞分析分離装置(FACS)、骨髄穿刺および種々の白血病特異的臨床化学パラメータならびに当該技術分野で公知の他の確立された標準的な方法を使用してもよい。微小残存病変(MRD)状態の決定のための方法および手段を、本明細書に記載している。
(i)第9染色体と第22染色体間での転座、フィラデルフィア染色体(Ph+)は成人ALL症例の約20%および小児ALL症例の5%で起こること、
(ii)第4染色体と第11染色体間の転座は症例の約4%で起こり、12ヶ月未満の乳児で最も一般的であること
が挙げられる。

を含む。さらに、前記成分は、バイアルもしくはボトル内に個々に、または容器もしくは多容器ユニット内に組合せでパッケージされ得る。
1. CD19xCD3二重特異性単鎖抗体
CD19xCD3二重特異性単鎖抗体の生成、発現および細胞傷害活性はWO99/54440に記載されている。CD19xCD3二重特異性単鎖抗体の対応するアミノ酸および核酸配列を、それぞれ、配列番号1および2に示す。CD19xCD3二重特異性単鎖抗体のCD3結合ドメインのVHおよびVL領域を、それぞれ、配列番号7〜10に示し、CD19xCD3二重特異性単鎖抗体のCD19結合ドメインのVHおよびVL領域を、それぞれ配列番号3〜6に示す。
リンパ球の表現型分類解析およびキメラ現象解析のため、CD19xCD3二重特異性単鎖抗体での治療前、治療中および治療後に、EDTA含有収集チューブを用いて患者の血液試料を収集した。Adviaでの識別的血液分析によって血液試料中のリンパ球の絶対数を測定した。CD3、CD4、CD8、CD19およびCD56(すべて、Becton-Dickinson、Heidelberg、Germanyから入手)に対する蛍光標識抗体を用いてリンパ球を染色した。標識細胞の解析およびデータ収集は、FACSCalibur(Becton-Dickinson)を用いて行なった。
MRDの検出のため、ポリメラーゼ連鎖反応(PCR)系アッセイ(上記引用のBader et al.)またはFACS解析のいずれかを使用した。簡単には、DNAを、DNeasy Blood&Tissue Kit(Qiagen GmbH、Hilden、Germany)によって単離した。PCR系標的としての免疫グロブリンおよびT細胞受容体遺伝子再配列は、最近、幹細胞移植後の急性リンパ芽球性白血病におけるMRDをモニタリングするための安定なマーカーであることが示された(Kreyenberg、Leukemia、2009)。
4.1 症例報告 患者1
この7歳の患者は、2004年に、高リスクCD10+ common型ALL(CD19-、CD34陽性;CD45減少;TCR再配列;CNS陰性)と診断された。治療後、患者は、2006年6月に骨髄再発を経験し、S3部門のALL-REZ BFM試験に従って治療された。2回の化学療法サイクル後、患者は持続的な疾患を有し、3回のクロファラビン過程後、完全寛解が達成された。2007年、患者は、全身照射およびエトポシドでのコンディショニング後、10名のHLA-対立遺伝子適合の血縁関係のないドナーのうち9名から同種異系HSCTを受けた。HSCTの1年後、患者は、さらなる骨髄再発を経験し、その後、1サイクルのクロファラビン/シクロホスファミド/VP16(Nobuko)、2サイクルのアムサクリン、VP16、プレドニゾン(Hamburg)および1サイクルのメルファラン/シタラビンを含む化学療法を受けた;図2参照。この集中的な化学療法にもかかわらず、患者は、図2の高いMRDレベルから明白なように、治療中、持続的な形態学的疾患を有した。次いで、患者は、5週間の15μg/m2のCD19xCD3二重特異性単鎖抗体(ブリナツモマブ;配列番号1)の連続注入による特別な配慮により認められた(compassionate)使用下で治療された;図3参照。記載の抗体での治療前(第0日)および治療中のリンパ球集団の連続解析により、CD8+ Tリンパ球の印象的な拡大が示されたが、CD4+ T細胞およびCD56+ NK細胞では変化はみられなかった。抗体治療を行なうと、MRD陰性が達成され得た。同時のキメラ現象解析により、100%ドナーキメラ現象が示された。図4に示されるように、10日目のCD19xCD3二重特異性単鎖抗体治療の骨髄解析により、骨髄芽細胞(MRD < 10-4)の完全な除去が示された。CD19xCD3二重特異性単鎖抗体治療の終了時の別のBM解析により、再度、骨髄由来の白血病性芽細胞の完全な非存在(MRD <10-4)が示された。前記抗体での治療前、治療中および治療後のMRDの連続解析を図5に示す。抗体治療の前は、小児患者は非常に不良な予後を有し、臨床症状が悪いため、HSCTに適格でなかった。抗体治療を行なうと、MRD陰性が達成され得た。失調の一過性の孤立性の徴候以外は、主な副作用は観察されず、ドナー由来CD8+ Tリンパ球の印象的な拡大にもかかわらず、GvHDの徴候は見られなかった。2008年10月、CD19xCD3二重特異性単鎖抗体での治療終了の2週間後(すなわち、図3〜5の第0日)、患者は、クロファラビン、チオテパおよびメルファラン(Lang)からなる非骨髄除去的予備計画を用いて、ハプロタイプ一致の自身の母親から2回目のHSCTを受けた。2009年11月時点で、患者はMRD陰性完全寛解のままであった。
この15歳の患者は、2001年4月にフィラデルフィア染色体-およびCD19-陽性B前駆細胞性ALLと診断された。化学療法後、患者は、2001年10月、12GyのTBIおよびエトポシドでのコンディショニング後、HLA一致の兄弟姉妹からのHSCTを受けた。2002年、再発骨髄が診断され、マチニブおよび化学療法により新たに寛解が達成された。2004年10月、次いで、患者は、HLA一致の血縁関係のないドナーから2回目のHSCTを受けた。2008年3月、2回目の再発が診断され、患者は、イマチニブに対する抵抗性のため、低用量化学療法およびダサチニブで治療した。クロファラビンおよびシトシン/アラビノシドでのさらなる化学療法後、患者は、分子寛解が達成され、ダサチニブでの移植後治療により、6つのうちの3つのHLA-対立遺伝子不適合ハプロタイプ一致の自身の父親から3回目の同種異系HSCTを受けた。胃腸管出血および拡張性心筋症のため、ダサチニブは、移植の5ヶ月後に中止した。2009年4月、CNS中に7×109/Lの芽細胞および骨髄中に3%の芽細胞を有する複合型中枢神経系(CNS)再発が診断された。次いで、患者をニロチニブ、髄腔内化学療法および18GyでのCNSの分別照射で治療した。この治療の3ヶ月、患者の骨髄は、1.1×10-3のレベルでMRD陽性のままであったが、CNSには芽細胞がなかった。末梢血のキメラ現象解析により、ハプロタイプ一致の自身の父親による完全なドナー由来造血が示された。
ALLの治療に利用可能なこれまでの薬物は、特異性が低く、種々の他の細胞に影響を及ぼして、免疫抑制、骨髄形成不全、粘液分泌抑制(mucositis)、神経障害、心臓毒性、および脱毛などの重度副作用をもたらす。したがって、副作用がより少ない、より良好な標的化アプローチが緊急に必要とされている。さらに、一部の患者では、ALLクローン、特に、HSCT後に生じたクローンが、従来の化学療法に完全抵抗性を発現するため、異なる作用様式を有する薬物が望ましい。化学療法と組合せたNK細胞のFc受容体および顆粒球による抗体依存性細胞傷害性の有効性を、モノクローナルキメラ抗体リツキシマブを用いて小児ALLにおいて示すことができた。現在、ブリナツモマブは、B細胞非ホジキンリンパ腫およびリンパ性白血病においてCD19を標的化するための細胞傷害性T細胞動員を可能にする臨床試験中の唯一の抗体である。T細胞は、従来のモノクローナル抗体によって動員される免疫細胞よりも高い細胞傷害潜在性を有するとみなされる。
・2名の小児患者は特別な配慮により認められた使用に基づいて治療された。
・前記患者はMRD陰性状態が継続中
・大きな有害事象(AE)の観察はなし
・AEはすべて一過性、治療の中断の必要なし
[1]小児急性リンパ芽球性白血病(ALL)の治療、改善または排除のための方法であって、該治療、改善または排除の必要がある小児ALL患者にCD19xCD3二重特異性単鎖抗体構築物を含む医薬組成物を投与することを含む、方法。

[11]前記CD19xCD3二重特異性単鎖抗体構築物が、配列番号1に示すアミノ酸配列、または配列番号1と少なくとも90%、好ましくは95%同一のアミノ酸配列を含む、[10]記載の方法。
[12]CD19xCD3二重特異性単鎖抗体構築物を含む医薬組成物を連続注入によって少なくとも4週間投与した後、2週間、無治療期間にする、[1]〜[11]いずれか記載の方法。
[13]前記投与が、MRD陰性状態の決定後、10回のうち少なくとも2、3、4、5、6、7、8、9 回繰り返される、[12]記載の方法。
[14]MRD陽性ALLをMRD陰性状態に変換するために同種異系幹細胞移植(HSCT)前になされる、[12]または[13]記載の方法。
[15]同種異系造血幹細胞移植(HSCT)後になされる、[12]または[13]記載の方法。
[16]CD19xCD3二重特異性単鎖抗体構築物により対白血病移植片(GvL)効果が誘導される、[15]記載の方法。

Claims (11)
- CD19xCD3二重特異性単鎖抗体構築物を含む、小児急性リンパ芽球性白血病(ALL)患者における小児ALLの治療、改善または排除のための医薬組成物。
- 前記小児急性リンパ芽球性白血病(ALL)が、小児B細胞系急性リンパ芽球性白血病(ALL)、好ましくは小児B前駆細胞性急性リンパ芽球性白血病ALL、より好ましくは小児プロB ALL、プレB ALLまたはcommon型ALL(cALL)である、請求項1記載の医薬組成物。
- 前記急性リンパ芽球性白血病(ALL)が、抗療性および/または再発ALLである、請求項1または2記載の医薬組成物。
- 前記CD19xCD3二重特異性単鎖抗体構築物が、投与期間にわたって一定の用量で投与される、請求項1〜3いずれか記載の医薬組成物。
- 注入期間の最初の1日または複数日の間、低用量のCD19xCD3二重特異性単鎖抗体構築物が投与され、続いて、残りの注入期間の間、高用量のCD19xCD3二重特異性単鎖抗体構築物が投与される、請求項1〜4いずれか記載の医薬組成物。
- 注入期間の最初の1日または複数日に、患者の体表面積1平方メートルあたり5μgのCD19xCD3二重特異性単鎖抗体構築物が投与され、続いて、残りの期間の1日用量として、患者の体表面積1平方メートルあたり15μgのCD19xCD3二重特異性単鎖抗体構築物が投与される、請求項5記載の医薬組成物。
- 注入期間の最初の1日または複数日に、患者の体表面積1平方メートルあたり5μgのCD19xCD3二重特異性単鎖抗体構築物が投与され、続いて、残りの期間の1日用量として、患者の体表面積1平方メートルあたり30μgまたは45μgのCD19xCD3二重特異性単鎖抗体構築物が投与される、請求項5記載の医薬組成物。
- 2種類の初期用量が、2、3、4、5、6もしくは7日間またはさらに長期間投与される、請求項5〜7いずれか記載の医薬組成物。
- 前記CD19xCD3二重特異性単鎖抗体構築物が、連続的に投与される、請求項1〜4いずれか記載の医薬組成物。
- 連続注入がポンプシステムによって実現される、請求項9記載の医薬組成物。
- 投与が、静脈内投与もしくは皮下投与、非経口投与、経皮投与、腹腔内投与、筋内投与または肺的投与である、請求項9記載の医薬組成物。
