Description

様々な種類の外科用ステープル器具及び関連構成要素が作製され使用されてきたが、本発明者ら以前には、付属の請求項に記載されている本発明を誰も作製又は使用したことがないものと考えられる。
本明細書に組み込まれると共に本明細書の一部をなす添付の図面は、本発明の実施形態を示すものであり、上記の本発明の一般的説明、及び以下の実施形態の詳細な説明と共に、本発明の原理を説明するのに役立つものである。

Ｉ．例示の外科用ステープラー
図１〜６は、外科手術を行うために、アーティキュレーション操作されていない図１Ａに示したような状態において、トロカールのカニューレ通路を通じて患者の手術部位に挿入される寸法を有する、例示の外科用ステープル及び切断器具（１０）を図示している。外科手術、ステープル留め、及び切断する器具（１０）は、実施部分（２２）に接続されたハンドル部分（２０）を含み、実施部分は、アーティキュレーション機構（１１）において遠位に終端するシャフト（２３）と、遠位に取り付けられたエンドエフェクタ（１２）と、を更に備える。アーティキュレーション機構（１１）及び遠位のエンドエフェクタ（１２）がトロカールのカニューレ通路を通じて挿入された後、図１Ｂに図示されているように、アーティキュレーションコントロール（１３）によってアーティキュレーション機構（１１）を遠隔的にアーティキュレーション操作することができる。そのようにして、エンドエフェクタ（１２）は、所望の角度から又は他の理由のために、臓器の背後に到達する若しくは組織に近づくことができる。本明細書では、「近位」及び「遠位」といった用語は、器具（１０）のハンドル部分（２０）を握っている臨床医を基準として使用されていることを理解されたい。したがって、エンドエフェクタ（１２）は、より近位のハンドル部分（２０）に対して遠位である。便宜上、また説明を明確にするため、本願では「垂直」及び「水平」といった空間的な用語を図面に対して使用する点も更に認識されるであろう。しかしながら、手術器具は、多くの配向及び配置において使用され、これらの用語は、制限的及び絶対的であることが意図されない。
この例のエンドエフェクタ（１２）は下部つかみ具（１６）及び枢動可能なアンビル（１８）を含む。ハンドル（２０）はピストル把持部（２４）を含み、臨床医がピストル把持部（２４）に向かって閉鎖トリガー（２６）を枢動して引くことにより、エンドエフェクタ（１２）の下部つかみ具（１６）に向かってアンビル（１８）のクランプすなわち閉鎖が引き起こされる。そのようなアンビル（１８）の閉鎖は、ピストル把持部（２４）に対する閉鎖トリガー（２６）の枢動に応じてハンドル部分（２０）に対して長手方向に移動する最も外側の閉鎖スリーブ（３２）を通じてもたらされる。閉鎖スリーブ（３２）の遠位の閉鎖リング（３３）は、実施部分（２２）の枠（３４）によって間接的に支持される。アーティキュレーション機構（１１）にて、閉鎖スリープ（３２）の近位側の閉鎖管（３５）は遠位部分（閉鎖リング）（３３）と連通している。枠（３４）は、アーティキュレーション機構（１１）を介して下部つかみ具（１６）に偏向可能に取り付けられて、単一の平面でのアーティキュレーションを可能にする。枠（３４）はまた、発射トリガー（２８）からの発射運動を発射バー（１４）に伝えるためにシャフト（２３）を通じて延在する発射駆動部材（図示せず）を長手方向に摺動可能に支持する。発射トリガー（２８）は、閉鎖トリガー（２６）より更に外側寄りにあり、以下に詳細に説明するように、これが臨床医によって枢動式に引かれると、エンドエフェクタ（１２）にクランプされている組織にステープルが打ち込まれ、組織が切断される。その後、開放ボタン（３０）を押してエンドエフェクタ（１２）から組織を開放する。
図２〜５は、数多くの機能を実行するために光線発射バー（１４）を取り入れているエンドエフェクタ（１２）を図示する。図３Ａ〜３Ｂに最もわかりやすく図示されているように、発射バー（１４）は横断方向に配向された上方のピン（３８）と、発射バーキャップ（４４）と、横断方向に配向された中間ピン（４６）と、遠位側に提供された切断エッジ（４８）と、を含む。上方のピン（３８）は、アンビル（１８）のアンビルポケット（４０）の内部に位置づけられており、移動可能である。発射バーキャップ（４４）は、下部つかみ具（１６）を通じて形成されたチャネルスロット（４５）（図３Ｂに図示）を通じて延在する発射バー（１４）を有することによって、下部つかみ具（１６）の下側表面と摺動可能に係合する。中間ピン（４６）は、発射バーキャップ（４４）と協力動作する下部つかみ具（１６）の上面と摺動可能に係合する。そのようにして、発射バー（１４）は、発射中にエンドエフェクタ（１２）の間隔を肯定的に取り、クランプされる組織の量が最低限の状態でのアンビル（１８）と下部つかみ具（１６）の間に生じる場合のあるピンチングを克服し、クランプされた組織の量が多過ぎる状態でのステープルの不良形成を克服する。
図２は、未使用のステープルカートリッジ（３７）が下部つかみ具（１６）のチャネル内に取り外し可能に設置され得るように、近位側に位置づけられた発射バー（１４）及び枢動されて開位置になっているアンビル（１８）を示す。図４〜５に最もわかりやすく示されているように、この例のステープルカートリッジ（３７）はカートリッジ本体（７０）を含み、カートリッジ本体は上部デッキ（７２）を提供し、下方のカートリッジトレー（７４）と連結されている。図２に最もわかりやすく示されているように、ステープルカートリッジ（３７）の部分を通じて縦スロット（４９）が形成されている。また、図２には、縦スロット（４９）の一方の側に上部デッキ（７０）を通じて形成された３列のステープルアパーチャ（５１）及び、縦スロット（４９）のもう一方の側に上部デッキ（７０）を通じて形成された３列のステープルアパーチャ（５１）が図示されている。再び図３〜５を参照すると、楔形スレッド（４１）及び複数のステープルドライバ（４３）がカートリッジ本体（７０）とトレー（７４）の間に捉えられており、ステープルドライバ（４３）に対して近位に楔形スレッド（４１）が位置づけられている。楔形スレッド（４１）はステープルカートリッジ（３７）の内部で長手方向に移動可能であり、一方、ステープルドライバ（４３）はステープルカートリッジ（３７）の内部で垂直方向に移動可能である。ステープル（４７）もまたカートリッジ本体（７０）の内部に、対応するステープルドライバ（４３）の上に位置づけられる。具体的には、それぞれのステープル（４７）はステープルドライバ（４３）によってカートリッジ本体（７０）の内部で垂直方向に駆動されて、関連付けられたステープルアパーチャ（５１）を通してステープル（４７）を外へと駆動する。図３Ａ〜３Ｂ及び図５に最もよく図示されているように、楔形スレッド（４１）は、楔形スレッド（４１）がステープルカートリッジ（３７）を通じて遠位方向に駆動されるにつれてステープルドライバ（４３）を上方に促すように傾斜したカム表面を呈する。
図３Ａのようにエンドエフェクタ（１２）が閉じられている状態で、上方のピン（３８）を長手方向のアンビルスロット（４２）に入れることによって、発射バー（１４）がアンビル（１８）と係合して前進する。押しブロック（８０）は、発射バー（１４）の遠位端に配置され、発射バー（１４）がステープルカートリッジ（３７）を通じて遠位側に前進するにつれて押しブロック（８０）が楔形スレッド（４１）を遠位側に押すように、楔形スレッド（４１）と係合するように構成される。そのような発射中に、発射バー（１４）の切断エッジ（４８）がステープルカートリッジ（３７）の縦スロット（４９）に入り、ステープルカートリッジ（３７）とアンビル（１８）の間にクランプされている組織を切断する。図３Ａ〜３Ｂに示すように、中間ピン（４６）と押しブロック（８０）とがともにステープルカートリッジ（３７）内の発射スロット内に入ることによってステープルカートリッジ（３７）を作動し、楔形スレッド（４１）をステープルドライバ（４３）との上向きのカム接触へと駆動すると、ステープル（４７）は、ステープルアパーチャ（５１）を通して外へと駆動され、アンビル（１８）の内面のステープル形成ポケット（５３）との形成接触へと駆動される。図３Ｂは、組織を完全に切断及びステープルした後に遠位方向に完全に移動された発射バー（１４）を図示する。
図６は、１回のストロークによって組織（９０）を通じて作動されたエンドエフェクタ（１２）を示す。図では、切断エッジ（４８）が組織（９０）を貫通して切断しており、一方、ステープルドライバ（４３）は、切断エッジ（４８）が作り出している切断線のそれぞれの側で、組織（９０）を通じてステープル（４７）の３本の交互の列を駆動している。この例では、全てのステープル（４７）が切断線とほぼ平行に配向されているが、ステープル（４７）は任意の好適な配向で位置づけられ得ることを理解されたい。この例では、最初のストロークが完了した後にエンドエフェクタ（１２）をトロカールから後退し、使用済みのステープルカートリッジ（３７）を新しいステープルカートリッジと交換し、その後に、エンドエフェクタ（１２）を再びトロカールを通して挿入して、ステープル留めする部位に到達し、更なる切断及びステープルを行う。所望の量の切断及びステープル（４７）が提供されるまでこのプロセスを繰り返すことができる。トロカールを通じた挿入及び後退を可能にするためにアンビル（１８）を閉じる必要がある場合があり、ステープルカートリッジ（３７）の交換を可能にするためにアンビル（１８）を開く必要がある場合がある。

図１０Ｂに示した楔形スレッド（４１）、及びステープルドライバ（１３０）は、カートリッジ本体（１０２）内に配置され、楔形スレッド（４１）が、最初はステープルドライバ（１３０）の近位に位置づけられる。楔形スレッド（４１）はステープルカートリッジ（１００）の内部で長手方向に移動可能であり、一方、ステープルドライバ（１３０）はステープルカートリッジ（１００）の内部で垂直方向に移動可能である。本実施例のステープルドライバ（１３０）は、ドライバ本体（１３２）と、一体式の容器突出部（１３４）とを備える。ステープル（１４０）はカートリッジ本体（１０２）内に含まれており、ステープルドライバ（１３０）の対応するドライバ本体（１３２）の上にある。各ステープル（１４０）は、関連付けられたドライバ本体（１０２）によってカートリッジ本体（１３２）の内部で垂直方向に駆動されて、対応するステープルアパーチャ（１１２）を通じてステープル（１４０）を外へと駆動する。この例の容器突出部（１３４）は、例えば、容器（１２０）と実質的に垂直整合であるように構成され、したがって、ステープルドライバ（１３０）が楔形スレッド（４１）の長手方向の移動により上方にカム作用を受けると、容器突出部（１３４）が接触して容器（１２０）を圧縮し、オリフィス（１１４）を介して流体（１２２）を排出する。当業者には理解されるように、容器（１２０）が容器突出部（１３４）と最初に接触した場合は、流体（１２２）は容器突出部（１３４）の初期の垂直方向への移動の際に放出され得る。代替の構成では、容器（１２０）は、容器突出部（１３４）と最初は接触していなくてもよく、容器（１２０）と容器突出部（１３４）との間の垂直変位は、ステープルドライバ（１３０）の初期の垂直移動から流体（１２２）の排出までのタイミング遅延をもたらすことができる。
具体的に図１０Ａを参照すると、ステープル（１４０）及び、流体（１２２）を有する容器（１２０）が、まだ配備されていない状態で示されている。本例では、カートリッジ（１００）が下部つかみ具（１６）に挿入されており、アンビル（１８）が枢動されて組織（９０）を閉じている。ステープル（１４０）は、ステープル開口部（１１２）の下及びステープルドライバ（１３０）の上に配置されている。容器（１２０）は、上述のように、オリフィス（１１４）から下方に延びており、流体（１２２）を包含している。ステープル本体（１３２）及び容器突出部（１３４）を有するステープルドライバ（１３０）は、容器本体（１０２）の底部の上に載せられてもよく、又はステープルドライバ（１３０）は、楔形スレッド（４１）が垂直にステープルドライバ（１３０）にカム作用するのを助けるために垂直に支えられてもよい。１つの構成では、ステープルドライバ（１３０）は楔形スレッド（４１）と相補的な楔形を有する底面を備えることができ、従って、ステープルドライバ（１３０）の上面は、ステープルドライバ（１３０）が楔形スレッド（４１）によって垂直にカム作用を受ける際に、上部デッキ（１１０）と平行になることができる。
上述のように発射トリガー（２８）が引かれると、図１０Ｂに示すように、発射バー（１４）及び楔形スレッド（４１）がステープルカートリッジ（１００）を通って遠位に移動する。具体的には、発射バー（１４）が前進するにつれて、中間ピン（４６）が楔形スレッド（４１）と係合して、楔形スレッド（４１）を摺動して遠位に移動する。楔形スレッド（４１）がステープルドライバ（１３０）に行き当たると、楔形スレッド（４１）がドライバ本体（１３２）と係合し、ステープルドライバ（１３０）に垂直方向にカム作用し始める。ステープルドライバ（１３０）が垂直方向にカム作用するにつれて、容器突出部（１３４）が容器（１２０）と接触して容器（１２０）を垂直方向に圧縮し始めることにより、流体（１２２）を強制してオリフィス（１１４）を通じて排出する。ステープルドライバ（１３０）が上方にカム作用し続けるにつれて、ステープル（１４０）が組織（９０）を貫通し、ステープル形成ポケット（５３）と係合し、ステープル形成ポケット（５３）に従って変形する。容器突出部（１３４）は、ステープルドライバ（１３０）が楔形スレッド（４１）の頂上に来るまで、引き続き垂直方向に容器（１２０）を圧縮することができる。当業者は理解できるように、本構成において流体（１２２）は、ステープル（１４０）が組織（９０）をステープル留めし、発射バー（１４）の切断エッジ（４８）が組織（９０）を切断する際に、オリフィス（１１４）から排出される。止血液が流体（１２２）として使用される場合、流体（１２２）の排泄は、組織を切断及びステープル留めするときの血液の損失を低減又は制限することができる。更に、薬効成分が流体（１２２）に含まれている場合は、組織（９０）の切断及びステープル留めをするときに炎症又は他の組織の状態を治療することができる。ステープルドライバ（１３０）及び容器（１２０）の１つの例示に過ぎない構成を説明してきたが、ステープルドライバ（１３０）、容器突出部（１３４）、及び／又は容器（１２０）の他の構成は、本明細書の教示を鑑みれば当業者に明らかであろう。
図１１に示す、１つのそのような代替形態は固体の容器であってもよい容器（２２０）を有しており、オリフィス（１１４）から下方に延び容器（２２０）の部分に沿って垂直に延びるチャネル（２２６）を有する。この例において、容器（２２０）は円筒状の容器として示されているが、他の形状、大きさ、及び容器（２２０）の構成が使用されてもよいことが理解されるべきである。実際に、代替形態において、容器（２２０）はカートリッジ本体（１０２）及び／又は上部デッキ（１１２）内に形成された空洞であってもよい。更に、そのような生体吸収性の栓のような１つ以上の栓は、オリフィス内（１１４）及び／又は容器の一部（２２０）に挿入することができ、又は代替的に、フィルムがカートリッジ（１００）の上部デッキ（１１０）の１つ以上のオリフィス（１１４）の上に置かれてもよい。この例のチャネル（２２６）は、ＲＴＶ、シリコーン、又は他の、半固体、可撓性、又は穿刺可能なシーラントのような穿刺可能シール材（２２８）で充填することができる。シーラント（２２８）は、実質的に流体的にチャネル（２２６）をシールし、したがって、流体（２２２）は実質的にチャネル（２２６）を通って漏出することなく容器（２２０）を充填することができる。この例の流体（２２２）は、下記に説明するように、容器の突出部（２３４）を挿入する前にオリフィス（１１４）を通じて又は容器（２２０）の底から容器（２２０）内に注入され得る。
この例に示すように、ステープルドライバ（２３０）は、ステープルを垂直方向に移動するように構成されたステープル本体（２３２）と、ステープル本体（２３２）から延びている穿刺部分（２３６）と、オリフィス（１１４）から流体（２２２）を押し出すように容器内を垂直方向に摺動するように構成された容器突出部（２３４）とを備える。容器突出部（２３４）は、注射器のプランジャーとほぼ同様のやり方で構成されてよく、例えば、容器突出部（２３４）の周囲で流体（２２２）が漏出するのを防ぐように、容器突出部（２３４）の周囲に配置された１つ以上のシール（図示せず）を含むことができる。穿刺部分（２３６）は、ステープルドライバ（２３０）が垂直方向にカム作用を受ける際にシーラント（２２８）を穿刺するように構成されるカッター部分（２３８）を備える。このようにして、容器突出部（２３４）は流体（２２２）を押し出すように容器（２２０）内で垂直方向に移動することができ、その一方で、穿刺部分（２３６）が容器突出部（２３４）と連結して、ドライバ本体（２３２）及びカッター部分（２３８）がシーラント（２２６）を穿刺する。容器突出部（２３４）が垂直方向に移動するときの流体（２２２）の圧力は、前述の生体吸収性の栓を「ポンと押し出す」ために十分であることができる。したがって、この追加的な構成を介して流体（２２２）を組織（９０）及び／又はステープル（１４０）に送達することができる。容器突出部（２３４）はドライバ本体（２３２）と共に実質的に平面のものとして示されているが、ドライバ本体（２３２）の上又は下に容器突出部（２３４）がオフセットされてもよい。
図１２に示す別の構成では、ステープルドライバ（３３０）は、容器突出部（３３４）をドライバ本体（３３２）に連結している細長い垂直のＵ形の部分（３３６）を含む。Ｕ形部分（３３６）の垂直方向の寸法は、容器突出部（３３４）が固体容器（３２０）内で摺動式に移動して、Ｕ形部分（３３６）の底が容器（３２０）の下方の縁に接触することなくオリフィス（１１４）から流体（３２２）を排出することができるような寸法であり得る。あるいは、Ｕ形部分（３３６）の底は、ステープルドライバ（３３０）が頂点にあるときに容器（３２０）に接触してもよく、又は、Ｕ形部分（３３６）は、流体（３２２）を排出するとき及びステープルをステープルドライバ（３３０）で駆動するときに、既定の位置で離脱するように構成されてもよい。ステープルドライバの他の好適な構成は、本明細書の教示に鑑みれば、当業者にとって明らかであろう。

前述の装置の変形物は、ロボット支援された医療及び処置での用途だけでなく、医療専門家によって行われる従来の医療及び処置での用途を有することができる。

単に例として、本明細書で説明した形態は、手術の前及び／又は後に、滅菌してもよい。１つの滅菌技術では、装置は、プラスチック又はＴＹＶＥＫバッグなど、閉められかつ密閉された容器に入れられる。次いで、容器及び装置は、γ放射線、Ｘ線、又は高エネルギー電子など、容器を透過し得る放射線場に置かれてもよい。放射線は、装置上及び容器内の細菌を死滅させることができる。次に、滅菌された装置は、後の使用のために、滅菌した容器内に保管してもよい。装置はまた、限定されるものではないが、ベータ若しくはガンマ放射線、エチレンオキシド、又は水蒸気を含めて、当該技術分野で既知の任意の他の技術を使用して滅菌されてもよい。

Claims (17)

  1. 装置であって、
    （ａ）器具であって、
    ｉ．ハンドル部分と、
    ｉｉ．前記ハンドル部分から遠位に延在するエンドエフェクタと、
    を備える器具と、
    （ｂ）前記エンドエフェクタに挿入可能なステープルカートリッジであって、前記ステープルカートリッジが、
    ｉ．上部デッキを有するカートリッジ本体であって、前記上部デッキが、
    （１）前記上部デッキに形成され、前記上部デッキの近位端から長手方向に延在する縦スロットと、
    （２）複数のステープルアパーチャと、
    （３）複数のオリフィスと、
    を備える、カートリッジ本体と、
    ｉｉ．前記カートリッジ本体に対して垂直方向に移動可能な、複数のステープルドライバであって、
    （１）ドライバ本体と、
    （２）前記ドライバ本体に連結された容器突出部と、
    を備える、複数のステープルドライバと、
    ｉｉｉ．前記複数のステープルドライバの上に配置された複数のステープルであって、前記カートリッジ本体に対して垂直方向に移動可能な複数のステープルと、
    ｉｖ．前記カートリッジ本体内に配置された複数の容器であって、前記複数の容器のそれぞれが、前記複数のオリフィスのうちの１つのオリフィスと流体連通する端部を有する、複数の容器と、
    ｖ．前記複数の容器のそれぞれの内部に配置された流体と、
    を備える、ステープルカートリッジと、
    を備えており、
    前記容器突出部が、前記容器を圧縮するように動作可能である、装置。
  2. 前記複数のステープルドライバのうちのそれぞれのステープルドライバが、前記複数のステープルアパーチャのうちの１つのステープルアパーチャを通じて、前記複数のステープルのうちの１つのステープルを駆動すると同時に、一方で、前記複数の容器のうちの１つの容器内の前記流体を前記複数のオリフィスのうちの１つのオリフィスを通じて排出するように構成されている、請求項１に記載の装置。
  3. 前記複数の容器のそれぞれが、垂直方向に圧縮可能な部分を備える、請求項１に記載の装置。
  4. それぞれのステープルドライバが、１つのステープルアパーチャ及び１つのオリフィスと実質的に垂直に整合される、請求項１に記載の装置。
  5. 前記複数の容器のうちのそれぞれの容器が、
    （１）対応するステープルアパーチャと前記容器との間に介在する垂直のチャネルと、
    （２）前記チャネル内に配置されたシーラントと、
    を備える、請求項１に記載の装置。
  6. 前記複数のステープルドライバのうちのそれぞれのステープルドライバが、前記ドライバ本体と前記容器突出部との間に配置されたそれぞれ対応する穿刺部分を備えており、前記穿刺部分が、前記複数の容器のうちの１つの関連する容器の前記チャネル内に配置された前記シーラントを穿刺するように動作可能である、請求項５に記載の装置。
  7. 前記容器突出部が、前記容器内を摺動式に移動するように動作可能である、請求項５に記載の装置。
  8. 前記複数の容器が前記カートリッジ本体内に形成される、請求項１に記載の装置。
  9. 前記複数の容器が互いに対して別個になっている、請求項１に記載の装置。
  10. 前記流体が止血剤を含む、請求項１に記載の装置。
  11. 前記流体がトロンビンを含む、請求項１に記載の装置。
  12. 前記流体がゲルを含む、請求項１に記載の装置。
  13. 前記流体が薬剤を含む、請求項１に記載の装置。
  14. 前記エンドエフェクタが、切断エッジを有する発射バーを備えており、前記上部デッキの前記縦スロットが、前記切断エッジを前記縦スロットに沿って長手方向に移動することを可能にするように構成される、請求項１に記載の装置。
  15. 体内手術に使用するための装置であって、
    （ａ）上部デッキを有する、カートリッジ本体であって、前記上部デッキが、
    ｉ．複数のステープルアパーチャと、
    ｉｉ．複数のオリフィスと、
    を備える、カートリッジ本体と、
    （ｂ）前記カートリッジ本体に対して垂直方向に移動可能なステープルドライバと、
    （ｃ）前記カートリッジ本体に対して垂直方向に移動可能な複数のステープルと、
    （ｄ）複数の容器であって、それぞれの容器が前記複数のオリフィスのうちのそれぞれ対応するオリフィスと流体連通する、複数の容器と、
    （ｅ）互いに対して別個になっている前記複数の容器のうちのそれぞれの容器の内部に配置された流体と、
    を備えており、
    前記ステープルドライバが容器突出部を備え、
    前記容器突出部が、前記容器を圧縮するように動作可能である、装置。
  16. 前記容器突出部が、前記容器の内部に摺動式に配置される、請求項１５に記載の装置。
  17. 前記ステープルドライバが、前記複数のステープルアパーチャのうちの１つのステープルアパーチャを通じて、前記複数のステープルのうちの１つのステープルを駆動する一方で、同時に、前記複数の容器のうちの１つの容器内の前記流体を前記複数のオリフィスのうちの１つのオリフィスを通じて押し出すように構成されている、請求項１５に記載の装置。
