Description
本発明は、重篤な気管支肺疾患を予防または治療するための薬剤の製造を目的とした、治療用量の2種以上の活性薬物成分を肺に供給できる加圧式定量噴霧吸入器(pMDI)用の溶液製剤の使用であって、前記吸入器の作動時に放出される液体粒子の実質的にすべてが、前記薬剤における2種以上の前記活性薬物成分の前記所定比と実質的に同じ比で2種以上の活性薬物成分を含有し、しかも2種以上の該活性薬物成分は実質的に同じ粒度分布で送達される使用に関する。
喘息およびCOPDのような気管支肺疾患の吸入エアロゾル薬を用いる治療は、肺への直接的なターゲッティングのため、発現の迅速化および副作用の低下を含む全身的治療にまさる利点を提供する。
本発明の1つの目的は、重篤な気管支肺疾患を予防および/または治療するための薬剤の製造を目的とした、HFA噴射剤および共溶媒としてのエタノール中に溶解した所定比の2種以上の活性薬物成分を含有する、定量噴霧吸入器用の加圧溶液製剤の使用であって、前記吸入器の作動時に放出される液体粒子の実質的にすべてが、前記薬剤における2種以上の前記活性薬物成分の前記所定比と実質的に同じ比で2種以上の活性薬物成分を含有し、前記吸入器の作動時に放出される液体粒子の実質的にすべてが、前記薬剤における2種以上の前記活性薬物成分の前記所定比と実質的に同じ比で2種以上の活性薬物成分を含有し、しかも2種以上の該活性薬物成分は実質的に同じ粒度分布で送達される使用を提供することである。
本明細書中で使用される場合、「前記吸入器の作動時に放出される液体粒子の実質的にすべてが、前記薬剤における2種以上の前記活性薬物成分の前記所定比と実質的に同じ比で2種以上の活性薬物成分を含有し」という表現は、アンダーセンカスケードインパクター(ACI)の各ステージに沿って測定された薬物量の割合の相対標準偏差(RSD)が、活性成分の少なくとも一つが4と16μgの間を含む用量で送達される場合5.0%未満、好ましくは3.5%、より好ましくは2.0%未満であり、活性成分の少なくとも一つが0.5μgを超えるが4μg未満の用量で送達される場合、15%未満、好ましくは12%未満であることを意味する。
図1は、実施例1で測定されたサンプルについて篩いを通過したBDPおよびFF累積質量堆積(%)を示す図である。
本発明の薬剤は、2種以上の薬物成分の溶液製剤を提供することにより得られ、加圧式定量噴霧吸入器(pMDI)を用いて使用され、吸入器の作動により薬剤中のこの活性薬物成分の所定比と実質的に同じ比で活性薬物成分を送達し、この活性薬物成分は、ステージに実質的に同じ粒度分布で送達される。実際にこれは組合せ活性薬物成分の肺内における共沈着の達成を可能にし、個別に投与される薬物の応答の合計を超える肺機能の臨床応答を生じる。同じ肺領域における共沈着は確かに患者の肺機能における大きな改善を伴う相乗効果を最大にする。
(a)1つまたは複数の共溶媒中の1つまたは複数の活性成分の溶液を調製する工程;
(b)デバイスをこの溶液で満たす工程;
(c)場合によって所定の量の強力な鉱酸を添加する工程;
(d)ヒドロフルオロアルカン(HFA)を含む噴射剤を添加する工程;および
(e)バルブをクリンピングし、ガス処理する工程。
実施例1.アンダーセンカスケードインパクター(ACI)によるエタノールベースの溶液HFA pMDI内のフマル酸ホルモテロールおよびジプロピオン酸ベクロメタゾンの粒度分布
溶液製剤は、共溶媒として12%w/wエタノールおよび安定剤として0.024%w/w塩酸(1M)を有するHFA 134a噴射剤媒体50μl中にBDP(100μg/用量)およびFF(6μg/用量)を含んでいた。本製剤を50μlバルブを備えたカンにパッケージし、0.30mmアクチュエータを使用して噴霧した。空気動力学的粒度の評価は、2バッチ各々からカンの使用有効期間の始めと終わりでUSPインダクションポートを備えるアンダーセンカスケードインパクターを使用して実施した。各々の測定を28.3l/分のサンプリング流速で15回の連続した用量をサンプリングして得た。試験された各々のカンについて、送達される用量を、カンの使用有効期間の始め、中間および終わりでDUSA法(用量単位スプレー装置(Dose Unit Spray Apparatus))を使用して測定した。試験サンプル中のBDPおよびFFの定量は、HPLC法により実施した。各々のインパクター測定に関して計量用量、送達用量、微粒子量、微粒子分画、空気動力学的中央粒子径(MMAD)および幾何学的標準偏差(GSD)を計算した。
LABA TA2005は、組合せ中に1回作動あたり強度1μg/用量で存在する一方、ブデソニドは、HFA 134aを用いて加圧噴霧される酸性エタノール溶液内に1回作動あたり100μg/用量で存在する。微粒子の空気動力学的評価は、各pMDIからACIへの10回連続投与をサンプリングすることにより実施した。このインパクターはUSPインダクションポートを備え、28.3l/分のサンプリング流速で実施した。3つのpMDIを試験し(バッチの始めから、中間、および終わり)、有効期間の始めおよび終わりで試験した。インパクター内の薬物堆積は、HPLCアッセイを使用して定量した。微粒子量(FPD)は、S3とフィルターの間のACIステージで採取された薬物の合計により決定した。
Claims (5)
- 重篤な持続型喘息、重篤および非常に重篤である慢性閉塞性肺疾患(COPD)から選択される重篤な気管支肺疾患を予防および/または治療するための、HFA134a噴射剤および共溶媒として12%w/wのエタノールを含む混合物に溶解した、活性成分としてフマル酸ホルモテロールとジプロピオン酸ベクロメタゾンの組合せを含有する定量噴霧吸入器用の加圧溶液製剤であって、1回の吸入器の作動時に該フマル酸ホルモテロールが6μgの量及び該ジプロピオン酸ベクロメタゾンが100μgの量を送達する定量噴霧吸入器用の加圧溶液製剤。
- 見かけのpHを3.0から5.0に調節するために塩酸をさらに含むことを特徴とする、請求項1に記載の溶液製剤。
- 前記吸入器がアルミニウム容器であることを特徴とする、請求項1または2に記載の溶液製剤。
- 前記気管支肺疾患が重篤な持続型喘息であることを特徴とする、、請求項1から3のいずれかに記載の溶液製剤。
- 前記気管支肺疾患が重篤または非常に重篤である慢性閉塞性肺疾患(COPD)であることを特徴とする、、請求項1から3のいずれかに記載の溶液製剤。
