本発明は、関節置換(articular resurfacing)を行う方法、システム、装具(devices)に関する。本発明は、関節インプラントの据付のための準備工程において適切な入刀(cut)面を得るための外科用型(surgical mold)を含む。
関節部の外科治療に際し外科医を補助する装具は多種多様である。膝に関してはMcDanielの米国特許第4501266号(1985年2月26日発行)が知られている。この特許は、膝の関節形成をスムースに行う異常膝治療器を開示している。同装具によれば、調節力を調節する目盛機構を採用している。これにより本装具は、膝の両側夫々に当てる靭帯伸張力を選択することができる。Poggieらの米国特許第5002547号(1991年3月26日発行)は、モジュラー性人工全体膝のインプラント移植のための骨面の準備に使うモジュラー装置を開示している。同装置は、引離器及びモジュール性を供するクランプ装置に加えて、入刀ガイド、テンプレート、アラインメント装置を備えて骨の切断と人工インプラント移植を進める。Poggieらの米国特許第5250050号(1993年10月5日発行)もモジュラー性人工全体膝の骨面の準備に使用するモジュラー装置に関するものである。Thornhillらの米国特許第5387216号(1995年2月7日発行)は、膝の改良外科に用いる装具を開示している。ベアリング・スリーブが損傷を受けたカナルに挿入されて増量分を取りさる。ロッドがスリーブを通り、骨の自然カナルに接するように位置付けられる。次に、ロッドはベアリング・スリーブにより一定位置に保持される。入刀ガイドはロッドに搭載されて骨の切断を行い、インプラントのための設置場所を確保する。Engらの米国特許第6056756号(2000年5月2日発行)は、人工インプラントのための大腿骨先端部を用意する装具を開示している。同装具は、人工置換のための骨の切断を想定しており、対向する骨を軸支するジャッキを含んでいる。同ジャッキは、同対向する屈曲した骨を所定の人工骨用の空間まで持ち上げる。Techieraの米国特許第6106529号(2000年8月22日発行)は、人工関節置換のための骨端部を用意するため、切断に使用するドリルガイドを導く上顆軸を開示している。Williamsonの米国特許第6296646号(2001年10月2日発行)の開示のシステムは、外科医は移植工程の終了時に向けて患者の脚を揃えて位置させ、脚が揃って固定している間に大腿骨と脛骨の両方を切断するというものである。Lombardiらの米国特許第6620168号(2003年9月16日発行)は、脛骨コンポーネントの中間的修正のための装具を開示している。
本発明の一つの様相では、インプラントを受け入れる関節部を用意するための外科用装具、例えば、関節面の形状(例えば、膝関節部の大腿部関節顆及び/又は脛骨プラトー)に少なくとも一部が合致する一つ以上の面又は部材を含む装具が説明される。ある実施形態では、本装具は、ルーサイト(登録商標)・シラスティック(登録商標)及び/又はその他のポリマー、或いは適切な材料を含む。本装具は、再使用型のものでも単回使用型のものでもよい。本装具は単一の構成体からなってもよいし、複数の構成体からなってもよい。ある実施形態では、本装具は、調節可能な、間隔の密なるピンアレイを含む。

- 患者の膝蓋骨の関節面に係合するための患者固有のドリルガイドであって、
第一の面を有する型を含み、
前記第一の面は、前記患者の膝蓋骨の前記関節面に実質的に適合する形状に形成され、或いは前記患者の膝蓋骨の前記関節面に厳密に一致し、
前記型が前記患者の膝蓋骨の前記関節面の構造に正確に合致する、患者固有のドリルガイド。 - 前記型は、透孔を有する請求項1に記載のドリルガイド。
- 前記透孔は、ドリルガイドの略中央に位置する請求項2に記載のドリルガイド。
- 前記透孔は、ドリルガイドの中央を外して位置する請求項2に記載のドリルガイド。
- 前記型は、複数の透孔を有する請求項1に記載のドリルガイド。
