Description
一方、わが国ではこれまでこうした症例を器質的な所見に関わりなく“慢性胃炎に伴う上腹部消化管愁訴”とし、臨床の場では慣例的に“胃炎”又は“慢性胃炎”と診断されてきた経緯が存在する。現在、FDのサブタイプとして、潰瘍症状型、消化管運動不全型、非特異型に分類されており、従来、胃アトニー、神経性消化不良、胃神経症と言われたものもこれらに含まれる。
一方、器質的病変(逆流性食道炎、消化性潰瘍、急性胃炎、消化器がん、膵・胆道疾患等)を明らかに伴う場合においても、腹部痛や不快感、食後の胃もたれ、悪心・嘔吐等が認められており、これらの不快感覚改善は患者のQOL向上にとって急務とされている。NUDは便秘に伴う排便困難感、残便感、腹痛、腹部膨満感等の下腹部消化管不定愁訴をあわせると、日本国内において総人口の約30%〜50%が何かしらの消化管の不定愁訴を経験していると推測されている。腹部不定愁訴の発症には性別、加齢、ストレス、また欧米型の食生活による肥満が影響していると考えられており、腹部不定愁訴は生活習慣病と並ぶ現代社会を代表する病といえる。これほど重大な疾患でありながら、消化管不定愁訴の原因としては、種々の疾患(慢性胃炎、糖尿病、肥満、便秘等)との関連が示唆されているのみであり、その発生機序としては消化管運動機能の低下が示唆されているに留まっている。
また、パーキンソン氏病、ハンチントン舞踏病、オリーブ橋小脳萎縮症等の進行性脳変性疾患や脳卒中患者等の多くは消化管運動機能障害を併発しており、消化管運動機能の改善によるQOL向上が必要であるとされている。これらの患者の中には言語障害、意識障害等の理由から自ら不定愁訴を訴えることのできない患者が多く存在すると考えられ、器質的な機能障害に対するケアと同時に、不定愁訴等の感覚障害を取り除くケアを実施することが真のQOL向上につながる。
FDの治療にはこれまで、5−HT4受容体作動薬等が用いられてきた。例えば、シサプリドやメトクロプラミドは、胃腸管の運動亢進作用を有し、慢性胃炎、腹部膨満感、逆流性食道炎、腹部不定愁訴及び偽性腸閉塞の症状等の治療に使用されている。しかし、メトクロプラミドは中枢のドーパミンD2受容体への作用に対する錐体外路症状の副作用が認められ、また、シサプリドにおいてもパーキンソン症状が現れることが明らかにされている。さらに、モサプリド等も使用されているが、効果が十分でない場合があり、また腹部膨満感等の副作用が出現する。逆流症状型(GERD)の治療にはH2拮抗剤やプロトンポンプ阻害剤が使用されているが、長期的に投与する場合、その安全性が未確定であるため定期的に検査する必要がある。したがって、これら既存の薬剤に十分な安全性が確保された状態で治療効果を求めることは困難な状況にある。
また、これらFD及び消化管器質性障害を伴った消化管不定愁訴の治療薬としては、部分的な5−HT3受容体作動薬、ニトログリセリンや硝酸塩等の一酸化窒素(以下NOとする)遊離薬等が知られている。しかしながら、当然これらの薬剤のターゲットである5−HT受容体やNO作動性の蛋白質は消化管粘膜だけに留まらず、脳を含む体中の臓器に分布しているため様々な生理作用を発揮する。すなわち、5−HT3拮抗薬を制吐薬として使用する場合には、特に非特異的な5−HT3受容体の作動薬は悪心、嘔吐を誘発する。また、体循環系において非特異的なNO遊離は血圧低下を引き起こすことが知られている。そのため、消化管臓器に限定してこれらのターゲットに作用させることのできる安全で有効性の高い薬剤の開発が必要である。
一方、グルタミンが副作用を発現することなく潰瘍等の器質性消化管疾患の改善作用を有することが報告されている(Elia M,Lunn PG.,Nutrition.1997 Jul−Aug;13(7−8):743−7)。しかし、グルタミンは溶解度が低く、水溶液中で非常に不安定であり、利便性に欠ける。同じく器質性疾患の1つである萎縮性胃炎の実験モデルにおいて、グルタミン酸ナトリウム単独、及びグルタミン酸とイノシン酸ナトリウムの両方を食事に添加することによって、胃液分泌を亢進させることができるとする報告がある(Vasilevskaia LS,et al.,Vopr Pitan.1993 May−Jun;(3):29−33,Rymshina MV & Vasilevskaia LS.,Vopr Pitan.1996;(1):9−11)。また、グルタミン酸ナトリウムについては萎縮性胃炎患者における胃液分泌の亢進を確認しており、さらにグルタミン酸とイノシン酸ナトリウムが萎縮性胃炎患者における消化促進剤としての可能性を示している(Kochetkov AM,Vopr Pitan.1992 Sep−Dec;(5−6):19−22,Shlygin GK,Klin Med(Mosk).1991 Aug;69(8):66−70)。しかしながら、NUDや胃腸障害に伴う感覚異常(不定愁訴)に関してグルタミン酸とイノシン酸ナトリウムの治療効果を示唆するには至っていない。
また、消化管に器質的な障害を有する患者あるいはそれによる機能の低下した患者に、粘膜保護や下部消化管の運動改善を目的としてグルタミン酸含有組成物を経腸的に投与することで、消化管機能を改善し、ひいては栄養補給効率の向上を目指した幾つかの試みが報告されている(独国特許第4133366号明細書、米国特許出願公開第2003/138476号明細書、欧州特許第0318446号明細書、特開昭56−57385号公報、加国特許第2404005号明細書、特開昭48−30583号公報)。しかしながら、これらの場合には、アミノ酸や蛋白質等の栄養組成物にグルタミン酸を含有させることで、粘膜保護による消化管バリアの維持、下部消化管の運動改善等を目的としたものであり、FDに対する治療効果を示唆するものではない。
したがって、消化管不定愁訴は消化管からの警告と捉えることができ、消化管器質性病変を有する場合はもちろん、病理的には判明し難い軽度の非器質的な消化管機能異常によって消化管不定愁訴が引き起こされることは容易に想像できる。そのため、従来からセロトニンやNOの遊離を促進する5−HT作動薬やNO遊離薬が使用されているが、これらの薬剤は、前述のとおり全身性の副作用や安全性の点で問題が多い。したがって、消化管内で特異的にセロトニンやNOの遊離を促進し、FD等に伴う諸症状を改善する薬剤の開発が望まれている。
本発明はこのような実情に鑑みなされたものであり、その課題とするところは、機能性消化管障害、特に機能性胃腸症、胃食道逆流症等の上部消化管機能障害の改善、消化管運動機能亢進、嚥下障害予防・改善可能な薬剤及び食品を提供することにある。また、本発明は消化管内で特異的にセロトニン及び/又はNOの遊離を促進するための薬剤及び食品を提供することである。
本発明者らは、上記課題を解決するため鋭意研究を重ねた結果、グルタミン酸、5’−ヌクレオチド及びそれらの塩のうちの少なくとも1種を投与対象に投与すると、消化管内でのみNO及びセロトニンの濃度が上昇して消化管運動機能が亢進されるため、全身性の副作用を誘発することなく機能性消化管障害、嚥下障害が改善されることを見出し、本発明を完成するに至った。
ここで、グルタミン酸ナトリウムや5’−ヌクレオチドの吸収性について、例えば、グルタミン酸ナトリウム、イノシン酸ナトリウムを経口摂取した場合、摂取したグルタミン酸の90%以上は消化管粘膜上で完全酸化され二酸化炭素と水になり、残り10%弱がアラニンと乳酸に変換され、グルタミン酸の血中移行量は非常に少ないと報告されている(PJ.Reeds,DG Burin,B Stoll,Jahoor,J.Nutrition 130:978S(2000))。更に、新島らの研究によれば、グルタミン酸ナトリウムの胃及び十二指腸内投与は迷走神経求心路を活性化するが、グルタミン酸ナトリウムの静脈内及び門脈内投与は迷走神経を活性化させないとの知見が得られている(Niijima A.,et al.,Physiol Behav.1991 May;49(5):1025−8、Niijima A.,et al.,J Nutr.2000 Apr;130(4S Suppl):971S−3S)。よって、これらの知見から、仮にグルタミン酸の一部が吸収されて体循環へ移行したとしても、体内においてグルタミン酸が直接的に迷走神経求心路を活性化することはないと考えられる。
かかる知見から、本発明の薬剤による機能性消化管障害改善の作用機序は、以下のように推察される。すなわち、本発明の薬剤を投与対象に投与すると、胃粘膜中においてのみNO及び/又はセロトニンの遊離が促進される。これにより、かかる局所的な環境のみにおいてNO及び/又はセロトニンの濃度が上昇し、消化管運動機能が亢進される。よって、安全で有効にFD等の機能性消化管障害に伴う不定愁訴が改善される。また、有効成分の血中移行量が非常に少ないので、全身性の副作用を誘発することが殆どない。
本発明は、以下の内容を包含する。
(1)グルタミン酸、5’−ヌクレオチド及びそれらの塩からなる群より選ばれる少なくとも1種を有効成分として含有する、機能性消化管障害予防・改善剤。
(2)前記5’−ヌクレオチドが5’−イノシン酸、5’−グアニル酸、5’−アデニル酸、5’−シチジル酸、5’−ウリジル酸及び5’−キサンチル酸からなる群より選ばれる、(1)記載の機能性消化管障害予防・改善剤。
(3)前記5’−ヌクレオチドが5’−イノシン酸及び5’−グアニル酸から選ばれる(1)記載の機能性消化管障害予防・改善剤。
(4)前記塩が塩基性アミノ酸との塩である(1)〜(3)のいずれか1つに記載の機能性消化管障害予防・改善剤。
(5)塩基性アミノ酸がアルギニン、リジン及びオルニチンからなる群より選ばれる(4)記載の機能性消化管障害予防・改善剤。
(6)塩基性アミノ酸がアルギニンである(4)記載の機能性消化管障害予防・改善剤。
(7)前記有効成分がグルタミン酸のアルギニン塩である(1)記載の機能性消化管障害予防・改善剤。
(8)前記機能性消化管障害が上部消化管機能障害である、(1)〜(7)のいずれか1つに記載の機能性消化管障害予防・改善剤。

本発明の機能性消化管障害とは、消化性潰瘍やガン症状のような器質的疾患が認められず、消化管、特に胃の内容物の停滞に基づく腹部の膨満感、悪心・嘔吐、上腹部痛、食欲不振あるいは便通異常等の上腹部不定愁訴の続く病態を言い、消化管の器質的疾患が見られなくても、患者のQOLを低下させる再現性のある消化器症状が認められる症状をいう。ここで、本発明における「消化管」とは、口腔から肛門までの一連の消化に携わる管腔臓器をいい、例えば、咽頭、食道、胃、小腸(十二指腸、空腸、回腸)、大腸が挙げられる。さらに「上部消化管」とは、咽頭、食道、胃、十二指腸をいう。「機能性胃腸症」とは、消化性潰瘍やガン症状のような器質的疾患が認められず、胃の内容物の停滞に基づく腹部の膨満感、悪心・嘔吐、上腹部痛、食欲不振あるいは便通異常等の上腹部不定愁訴の続く病態を言い、消化管の器質的疾患が見られなくても、患者のQOLを低下させる再現性のある消化器症状が認められる症状をいう。当該胃腸症はこれまで慢性胃炎や胃炎として診断されてきた疾患であり、腹部痛、胃もたれ、胸やけ等の症状を呈することを特徴とする。近年、開業医外来の患者の4〜6割が機能性胃腸症といわれており、ピロリ菌の除去療法により増加傾向にある。
また、「胃食道逆流症」は逆流性食道炎も含み、胃酸が逆流することで発症し、胸やけ・胃酸が口まで上がってくる等の特有の症状がある。さらに、「嚥下」とは、水や食物を飲み込むことをいうが、食道に嚥下した食物塊などが詰まることで誤嚥や嘔吐を引き起こすなど、口腔、咽頭だけでなく、食道など消化管の運動も密接に関係している。
機能性消化管障害予防・改善剤、消化管運動機能亢進剤及び嚥下障害予防・改善剤は、患者のQOLを低下させる再現性のある機能性消化管障害、特に機能性胃腸症、胃食道逆流症等の上部消化管機能障害を改善するための予防・改善剤である。
以下、本発明の機能性消化管障害予防・改善剤、消化管内における特異的一酸化窒素及び/又はセロトニン遊離促進剤、消化管運動機能亢進剤及び嚥下障害予防・改善剤を、単に「予防・改善剤」ということもある。
本発明の予防・改善剤は、グルタミン酸、5’−ヌクレオチド及びそれらの塩から選ばれる少なくとも1種を有効成分として含有するものである。5’−ヌクレオチドとしては、例えば、5’−イノシン酸、5’−グアニル酸、5’−アデニル酸、5’−シチジル酸、5’−ウリジル酸、5’−キサンチル酸が挙げられ、これらのなかでは5’−イノシン酸、5’−グアニル酸が好適である。
「塩」としては、薬理的に許容される、グルタミン酸又は5’−ヌクレオチドの塩が好ましい。このような塩としては、無機塩基との塩、無機酸との塩、有機酸との塩及び有機塩基との塩等が挙げられる。無機塩基との塩としては、ナトリウム、カリウム、リチウム等のアルカリ金属塩、カルシウム、マグネシウム等のアルカリ土類金属塩、アンモニウム塩等が挙げられる。無機酸との塩としては、ハロゲン化水素酸(塩酸、臭化水素酸、ヨウ化水素酸等)、硫酸、硝酸、リン酸等との塩が挙げられる。有機酸との塩としては、ギ酸、酢酸、プロピオン酸、シュウ酸、コハク酸、マレイン酸、フマル酸、クエン酸、グルタミン酸、アスパラギン酸、ヒスチジン等との塩が挙げられる。有機塩基との塩としては塩基性アミノ酸(アルギニン、リジン、オルニチンなど)、ヌクレオチド(プリン誘導体、ピリミジン誘導体など)、アルカロイドなどがある。これらのなかでは、アルギニンなど塩基性アミノ酸との塩や、ナトリウム塩等のアルカリ金属塩、カルシウム塩が好適であり、イノシン酸やヒスチジン等の有機酸との塩も有用である。
グルタミン酸、5’−ヌクレオチド及びそれらの塩は、動物や植物に由来する天然のもの、あるいは化学合成法、発酵法、遺伝子組換法によって得られるもののいずれを使用してもよい。また、グルタミン酸においては、L体、D体、これらの混合物(例えば、ラセミ体)のいずれでもよいが、L体が好適に使用される。なお、グルタミン酸と同様のアミノ酸である、アスパラギン酸、トリコロミン酸、イボテン酸又はそれらの塩においても、機能性消化管障害を改善する作用があると推測される。
好適な有効成分としては、グルタミン酸、L−グルタミン酸ナトリウム、D−グルタミン酸ナトリウム、5’−グアニル酸、5’−グアニル酸ナトリウム、5’−キサンチル酸ナトリウム、5’−アデニル酸ナトリウム、デオキシ−5’−アデニル酸ナトリウム、5’−イノシン酸ナトリウム、2−メチルチオ−5’−イノシン酸ナトリウム、N’−メチル−2−メチルチオ−5’−イノシン酸ナトリウム等のグルタミン酸、5’−ヌクレオチド及びそれらの塩が挙げられる。これらのなかでも、グルタミン酸、L−グルタミン酸ナトリウム、5’−イノシン酸ナトリウムが好ましい。また、有効成分は、1種又は2種以上を混合して使用することができる。
本発明において、機能性消化管障害における、改善可能な不定愁訴の具体的な症状としては、悪心、嘔吐、吐き気、胸焼け、膨満感、胃もたれ、ゲップ、胸中苦悶感、胸痛、胃部不快感、食欲不振、嚥下障害等の代表的な上部消化管不定愁訴、腹痛、便秘、下痢等の下部消化管不定愁訴及び関連した愁訴、例えば息切れ、息苦しさ、意欲低下、喉頭閉塞・異物感(漢方でいう「梅核気」)、易疲労感、肩こり、緊張、口のかわき(口渇・口乾)、呼吸促迫、四肢熱感・冷感、集中困難、焦燥感、睡眠障害、頭痛、全身倦怠感、動悸、寝汗、不安感、ふらつき感、めまい感、熱感、のぼせ、発汗、腹痛、便秘、抑鬱感等が挙げられる。
上述のように本発明においては、上記有効成分の有効量を投与対象に投与することで機能性消化管障等が改善されるが、その場合、有効成分をそのままあるいは医薬担体と混合して、投与されるべき有効成分の量、及び投与対象(例えば、患者)の状態等を考慮した上で、錠剤(糖衣錠、フィルムコーティング錠を含む)、丸剤、カプセル剤、アンプル剤、分包剤、エリキシル剤、懸濁剤、シロップ剤、ガム製剤、ドロップス製剤、散剤、注射剤、坐薬、徐放剤等の医薬製剤として、経口的、経腸的又は非経口的に投与することができる。投与方法としては経口投与が好ましく、さらに薬物徐放形態であるものが好ましい。徐放形態としては、ゲル被覆製剤、多重被覆製剤等の通常の徐放剤の他、ガム製剤、ドロップス製剤、定位放出剤(幽門部破裂製剤)等が挙げられる。
ここで、「投与対象」としては機能性消化管障害等を罹患した個体(例えば、ヒトのほかウシ、ウマ、ブタ、ヒツジ、イヌ、トリ等の家畜や家禽、及びマウス、ラット等の実験動物。以下、同様。)、該機能性消化管障害等を罹患する可能性のある個体等が挙げられる。また、「有効量」とは所望の改善効果を与えるのに十分な量をいう。有効成分の投与量は、投与対象の性別、年齢、体重、食餌、投与の形態、FD等の症状、FD等を誘発するリスクの程度、消化管の器質性疾患の症状等によって異なるが、例えば、成人(体重60kgとして)に対する有効成分の1日当たりの投与量は、0.01〜20gが好ましく、0.01〜10gがより好ましく、0.1〜10gが更に好ましい。このような投与量を1回又は数回に分けて投与することができる。
上述した「医薬担体」とは、薬学的に許容されるものであって、体内において薬学的作用を呈することが極力少ないものをいう。経口投与される場合の医薬担体としては、トラガントガム、アラビアゴム、コーンスターチ、ゼラチン等の結合剤;リン酸二カルシウム等の賦形剤;コーンスターチ、馬鈴薯澱粉、アルギン酸等の崩壊剤;ステアリン酸マグネシウム等の滑沢剤;シュークロース等の甘味剤;色素;オレンジフレーバー等の香料;水、エタノール、グリセロール等の溶剤;蛋白質、アミノ酸、ビタミン、脂質、グルコース等の栄養素等を適宜使用することができる。さらに、医薬担体として、システイン、グルタチオン、アスコルビン酸、メタ亜硫酸ナトリウム、重亜硫酸ナトリウム等の薬学的に許容される抗酸化剤、炭酸カルシウム、水酸化アルミニウムゲル、ケイ酸アルミニウム等の酸中和剤が挙げられる。
なお、上述した医薬製剤の剤形や医薬担体は、当業者によく知られたものであり、例えば、Reimington’s Pharmaceutical Science,ed.16(1980),Mack Publishing Companyに記載されるような剤形とし、また医薬担体を使用することができる。
本発明は他の薬剤を併用してもよく、かかる薬剤としては、例えばH2受容体拮抗薬、プロトンポンプ阻害薬等の酸分泌抑制剤、5−HT受容体作用剤、D2拮抗剤等の運動機能改善剤、ムスカリン受容体拮抗薬、抗ガストリン薬、抗コリン薬等の制酸剤、テプレノン、プラウノトール、オルノプロスチル、エンプロスチル、ミソプロストール、レバミピド、スクラルファート、ポラプレジンク、アズレン、エグアレンナトリウム、グルタミン、アルジオキサ、ゲファルナート、エカベトナトリウム等の粘膜保護剤、スルファサラジン、5−ASA製剤、ステロイド、レミケード等の炎症性大腸炎治療剤を含有することができる。これらは1種又は2種以上を含有することができる。
次に、本発明の食品について説明する。本発明の食品は、グルタミン酸、5’−ヌクレオチド及びそれらの塩から選ばれる少なくとも1種の化合物を含有し、消化管内における特異的な一酸化窒素及び/又はセロトニンの遊離促進、機能性消化管障害の改善、消化管運動機能の亢進、嚥下障害の改善といった特定の目的のために摂取するものである。また、本発明の食品は、いわゆる健康食品を含む一般食品としてもよい。さらに、本発明の食品は、厚生労働省の保健機能食品制度に規定される、保健機能食品、特定保健用食品、栄養機能食品、さらにダイエタリーサプリメント(栄養補助食品)とすることができる。この場合、グルタミン酸、5’−ヌクレオチド又はそれらの塩を1種又は2種以上混合して使用することができる。
本発明の食品は、上述した化合物をそのまま摂取してもよいが、より摂取しやすくするために上記化合物に通常の食品素材、調味料、香味料等を添加しドリンク剤、ガム、粉末、錠剤、顆粒、ゼリー等の形態に加工して摂取してよい。この場合、例えば、上記化合物と崩壊剤とからなる錠剤、上記化合物と増量剤(蛋白加水分解物、澱粉、カゼイン、グルコース等)との混合物、口腔内で徐放可能な上記化合物と粘剤(ガム、舌下錠、トローチ剤)との混合物、上記化合物とこれを溶解している溶媒(例えば食用油脂、エタノール又は水)との溶液、上記化合物を含むW/O若しくはO/W乳化物、上記化合物と栄養素(例えば、蛋白質、アミノ酸、ビタミン、脂質、グルコース等)との混合物とすることができる。また、本発明の機能性消化管障害予防・改善用、消化管運動亢進用、嚥下障害予防・改善用、及び消化管内における特異的一酸化窒素及び/又はセロトニン遊離促進用のグルタミン酸、5’−ヌクレオチド及びそれらの塩から選ばれる少なくとも1種は、食事を摂取する際に添加して食事と共に摂取することも可能である。例えば、ドリンク、清涼飲料水、ヨーグルト、ゼリー、乳飲料等の既存の食品に添加して摂取してもよい。
本発明の食品を上述した特定の目的に使用する場合、成人1日当たりのグルタミン酸、5’−ヌクレオチド及びそれらの塩の摂取量は、0.01〜20gが好ましく、0.01〜10gがより好ましく、0.1〜10gが更に好ましい。一方、本発明の食品中の上記化合物の含有量は、通常0.001〜20重量%であり、好ましくは0.001〜10重量%、より好ましくは0.01〜10重量%、さらにより好ましくは0.1〜10重量%である。上記一般食品中の上記化合物の含有量を上記範囲とすることにより、消化管機能改善という効果を顕著に奏することができる。
本発明の組成物は、上述の説明からわかるように、当業者には明らかな、いくつかの変更があり得る。このような本発明の趣旨を逸脱しない変更も、本発明の範囲に含まれる。
(実施例1)
グルタミン酸ナトリウム又はグアニル酸ナトリウムの水溶液で惹起される迷走神経胃枝求心活動に対する5−HT3拮抗薬の効果について検討するため、以下の(A)〜(C)の実験を行った。
SD(IGS)系ラット(雄、8−10週齢:日本チャールズリバー)を実験に用いた。ラットを15−17時間絶食後、ウレタン麻酔(1g/kg,i.p.)下に開腹し腹側迷走神経胃枝を露出させ、新島らの方法(Niijima A.,et al.,Physiol Behav.1991 May;49(5):1025−8.)に従って、求心性神経活動を記録した。グルタミン酸ナトリウム(MSG;150mM)又はグアニル酸ナトリウム(GMP;10,30,60mM)の水溶液を胃内に留置したカテーテルより2mL/ratの割合で投与した。5−HT3拮抗薬(グラニセトロン;granisetron)は大腿静脈に留置したカテーテルから0.1−10μg/kg/ratの量で投与を行った。
(A)150mM MSG応答に対するグラニセトロンの抑制効果について検討した。その結果を図1Aに示す。図1Aにおいて、縦軸は5秒間の平均神経線維発火数を示し、横軸は時間(分)を示す。
(B)150mM MSG応答に対するグラニセトロンの抑制効果における用量依存性について検討した。その結果を図1Bに示す。なお、各データは4例の平均値±標準誤差を示す。
(C)GMP応答の用量依存性及びグラニセトロンの拮抗作用について検討した。その結果を図1Cに示す。なお、図1Cにおいて、縦軸は5秒間の平均神経線維発火数を示し、横軸は時間(分)を示す。
図1Aの結果から、MSG水溶液の胃内投与により引き起こされる迷走神経胃枝求心性神経活動は、5−HT3受容体拮抗薬であるグラニセトロン(10μg/kg)の静脈内投与によりほぼ完全に阻害されることが確認された。また、図1Bの結果から、MSG応答の半分を阻害するグラニセトロンの投与量(ED50値)は約0.3μg/kg/ratであることが分かった。さらに、図1Cの結果から、GMP(10,30,60mM)水溶液の胃内投与は用量依存的に迷走神経胃枝を活性化し、また、GMPより引き起こされる迷走神経胃枝求心性活動もグラニセトロン(10μg/kg)の静脈内投与によりMSGと同様に阻害されることが確認された。
以上の結果から、グルタミン酸ナトリウム又はグアニル酸ナトリウムを摂取すると、胃粘膜中にセロトニンを遊離させること、及び迷走神経胃枝終末の5−HT3受容体を介して迷走神経を活性化させることが明らかとなった。
(実施例2)
粘膜セロトニンを枯渇又はNO合成を阻害させた場合のグルタミン酸ナトリウムの胃内投与による迷走神経の活性化について検討するため、下記の(A)及び(B)の実験を行った。
SD(IGS)系ラット(雄、8−10週齢:日本チャールズリバー)を実験に用いた。ラットを15−17時間絶食後、ウレタン麻酔(1g/kg,i.p.)下に開腹し腹側迷走神経胃枝を露出させ、新島らの方法(Niijima A.,et al.,Physiol Behav.1991 May;49(5):1025−8.)に従って、求心性神経活動を記録した。
(A)P−chlorophenylalanine(PCPA)を5%CMC溶液に溶解した後、200mg/kg/ratで1日2回、2日間投与した(腹腔内投与)。PCPAの最終投与は、MSG水溶液(150mM;胃内投与)を投与する15分前に行った。その結果を図2Aに示す。
(B)NG−nitro−L−arginine methyl ester(L−NAME)を生理食塩水に溶解した後、10mg/kg/ratでMSG水溶液を投与する15分前に大腿静脈に留置したカニューレから投与を行った。その結果を図2Bに示す。
なお、図2A及びBにおいて、縦軸は10秒間の平均神経線維発火数を示し、横軸は時間(1bin=10秒)を示す。また、各データポイントは4例の平均値±標準誤差を示す。
図2Aの結果から、セロトニン合成酵素阻害剤であるPCPAの前処置によって、MSGの胃内投与後の持続性の神経活動亢進が消失することを確認した。また、図2Bの結果から、(A)と同様の現象が、NO合成酵素阻害剤L−NAMEの前処置によっても確認された。これらの結果から、MSG水溶液の胃内投与時の迷走神経活性化には粘膜セロトニン及びNOの産生が必須であることが判明した。
(実施例3)
グルタミン酸の胃内投与による粘膜中でのNOの放出について検討するため、以下の実験を行った。
SDラットをウレタン麻酔下に開腹し、前胃と小腸に小切開を加え、直径2mmのポリエチレンチューブを挿入し、ペリスタポンプを使用して潅流液の導入と排出を行った。潅流液は38℃に加温し流速は1mL/minで行った。NO電極を温度センサーともに粘膜筋板下に保持し、2−3時間生理食塩水で胃内潅流して、遊離NO値を安定させた後、14:00−18:00の時間帯に等張のMSG溶液(150mM)を潅流した。
20例中3例で等張(2.5%)MSGにより潜時15分程度の遊離NOの増大が観察された。代表的な例について図3に示した。図3において、縦軸は遊離NO濃度nMを示し、横軸は時間secを示す。胃粘膜にMSG2.5%を灌流投与すると胃粘膜表面に留置したNO電極によりNOを検出することができる。これにより、MSG摂取させることで胃粘膜中のNO濃度が上昇することが確認された。
(実施例4)
グルタミン酸の胃内投与による門脈血中のセロトニンの漏出について検討するため、以下の実験を行った。
雄性SDラット8週令を用いた。7:00−19:00明期の明暗周期制御室において飼育した。夜間絶食した後、ウレタン(1.25g/kg i.p.)麻酔下にて開腹し、門脈に血液採取用のカテーテルを挿入した。カテーテルには10単位/mLのヘパリンを含有する生理食塩水で満たし、カテーテル内の血液の凝固を防止した。生理食塩水、MSG450mM溶液はそれぞれ2mL経口ゾンデを使用して経口投与した。投与10分前より投与後60分まで10分ごとに血液を採取した。血小板の不要な活性化を防ぐため、採血操作には十分留意した。採取した血液は定法により速やかに摂氏4度下、3000rpm、15分間遠心分離し、血漿を分離した。分離した血漿は速やかに等量のpH2に調整した酢酸・塩酸混合液を添加し除蛋白を行った。上清に含まれるセロトニン(5−HT)量及び5HIAA量は電気化学検出器(ECD−100、エイコム社製)を用いて分析した。なお、分析条件は以下の通りである。
分析カラム EICOMPAK SC50DS、
移動相 83%0.1Mクエン酸・0.1M酢酸ナトリウムpH3.9、
17%メタノール、
140mg/L 1−オクタンスルホン酸ソーダ(SOS)、
5mg/L EDTA・2Na、
流速 0.23mL/min、
分析温度 摂氏25度、
検出器設定加電圧 +700mV、
作用電極 グラファイト電極。
上述した分析法を用いることにより、同一サンプルよりセロトニン及びその安定な代謝産物の5HIAAを定量した。血漿5HTの基礎値は29.4±15.7nM(mean±S.D)であった。また、代謝産物5HIAAの基礎値は149±22.8nMであった。それぞれ投与直後0分の値を100%として正規化した。なお、2mL胃内投与直後の血液採取時間を0分とした。実験結果を図4に示す。図4において、縦軸は血漿5HT又は5HIAAの相対濃度を示し、横軸は時間minを示す。図4Aの結果では、「●(黒丸)」で示される450mM MSG投与、及び「○(白丸)」で示される生理食塩水投与には差が認められなかった。一方、図4Bの結果から、代謝産物5HIAAはMSG450mMを投与したところ、生食投与群を上回る、5HIAAの上昇が認められた。これより、摂取されたMSGにより胃粘膜中で産生されたセロトニンは胃粘膜中で解毒され門脈中に代謝物5HIAAとして放出されていることが判明した。即ち、MSGは胃粘膜部位に限局してセロトニン濃度を上昇させることが確認された。
(実施例5)
グルタミン酸の胃内投与による胃排出促進について検討するため、以下の実験を行った。
7−9週齢の雄性SDラットを1晩絶食させた後、実験に使用した。被験食を10mL/kgの容量で経口投与し、その1時間後に直径1mmのガラスビーズを約80個経口投与した。30分後に剖検し、胃、小腸に存在したビーズの個数を計測した。なお、小腸は4分割し、胃に近い方からB1−B4とした。実験結果を図5に示す。図5において、横軸のGは胃を示し、白抜きカラムはコントロール群を示す。被験食は、コントロール群は5%のカゼインを蒸留水に溶解したもの、グルタミン酸投与群は5%のカゼインに1%のグルタミン酸ナトリウムを溶解したものを用いた(グルタミン投与量100mg/kg)。データは胃、小腸に存在したビーズの総数を100とし、当該部位に存在するビーズの割合をパーセント表示した。
実験結果から、グルタミン酸投与群(N=5)は、コントロール群(N=4)と比較して胃に存在したビーズの個数が少なく、小腸に存在したビーズの個数が多い傾向が観察された。このことから、グルタミン酸は胃排出の促進作用を有することが確認された。
(実施例6)
グルタミン酸ナトリウム及びそのほかの塩の胃排出促進について検討するため、以下の実験を行った。
雄性ICRマウスを使用した。0.05%フェノールレッド及び試験薬を含む5%カゼイン流動食を0.5mL経口投与し、その30分後に開胸し、胃を摘出した。0.1N水酸化ナトリウム(14ml)に入れ、ホモジナイズし、1時間室温に放置した。5mlの上清に20%トリクロロ酢酸0.5ml加え、遠心分離(3000回転,20分)した。上清に0.5N水酸化ナトリウム4mlを加え、吸光光度計(560nm)にて吸光度を測定した。なお、胃排出率は以下の算出式によって求めた。
胃排出率(%)=(1−試験サンプルの吸光度/標準サンプルの吸光度)×100
なお、標準サンプルの吸光度は0.05%フェノールレッド溶液0.5mlを投与直後に胃を摘出したものを用いた。
検定は一元配置分散分析の後、ダネット多重比較を用いて行った。なお、*P<0.05,**P<0.01,***P<0.001を示す。
結果を図に示す。各群の例数は8−24例である。縦軸は胃排出率を示し(図7A、7B、7C)、図6A、6Bは対照群の胃排出率を100とした胃排出度を表した。グルタミン酸ナトリウム及びアルギニングルタミン酸塩は胃排出を促進した(図6A、6B)。また、その効果を比較すると、アルギニン塩のほうがナトリウム塩よりも低用量から胃排出を促進することが明らかとなった。また、リジングルタミン酸塩及びグルタミン酸カルシウムについても検討したところ、胃排出を促進することが明らかとなった(図7A、7B)。このことからグルタミン酸は他の塩形態においても胃排出を促進することが明らかとなった。さらに、イノシン酸についても検討したところ、胃排出を促進した(図7C)。
(実施例7)
グルタミン酸ナトリウムの食後感覚に対する影響について検討するため、以下の実験を行った。
45歳以上の健常男性(被験者)18名による、クロスオーバーでの二重盲検試験を行った。被験者はカゼイン蛋白流動食400mLを2分以内に飲み、その後15分おきに4時間、胃の張りに関して下記基準による0−10までのスコアを記録した。0が胃の張りがなく、10は胃の張りがかなり大きいことを示し、数値が大きいほど胃の張りが大きいことを示す。試験は2回実施し、被験者は、2回のうちどちらか1回は0.5%MSGを添加した試験食、もう1回は対照食を飲んだ。なお、カゼイン蛋白流動食の組成は以下の通りである
試験食:
MSG(味の素(株)) 2.1g
カゼインカルシウム(商品名EM9−N:DMV Japan) 57.96g
デキストリン(商品名TK16:松谷化学) 52.5g
アスパルテーム(味の素(株)) 0.097g
プラムフレーバー(GIV010790:ジボダンジャパン) 1.47g
蒸留水 400mL
対照食:上記組成のうちMSGを含まない。
結果を図8に示す。流動食を飲んだ時間を0分とした。試験食群では対照食群に比べて胃の張りが小さかった。このことから、グルタミン酸は食後感覚を改善することが示唆された。
以上、本発明の具体的な態様のいくつかを詳細に説明したが、当業者であれば示された特定の態様には、本発明の教示と利点から実質的に逸脱しない範囲で様々な修正と変更をなすことは可能である。従って、そのような修正及び変更も、すべて後記の請求の範囲で請求される本発明の精神と範囲内に含まれるものである。
本出願は、日本で出願された特願2004−271884(出願日:2004年9月17日)を基礎としており、その内容は本明細書に全て包含されるものである。
Claims (17)
- グルタミン酸の塩基性アミノ酸塩を有効成分として含有する、機能性消化管障害予防・改善剤。
- 塩基性アミノ酸がアルギニン、リジン及びオルニチンからなる群より選ばれる請求項1記載の機能性消化管障害予防・改善剤。
- 塩基性アミノ酸がアルギニンである請求項2記載の機能性消化管障害予防・改善剤。
- 前記機能性消化管障害が上部消化管機能障害である、請求項1〜3のいずれか1項に記載の機能性消化管障害予防・改善剤。
- 前記上部消化管機能障害が機能性胃腸症又は胃食道逆流症である、請求項4記載の機能性消化管障害予防・改善剤。
- 成人に対する前記有効成分の1日当たりの投与量が0.01〜20gである、請求項1〜5のいずれか1項に記載の機能性消化管障害予防・改善剤。
- グルタミン酸の塩基性アミノ酸塩を有効成分として含有する、消化管運動機能亢進剤。
- グルタミン酸の塩基性アミノ酸塩を有効成分として含有する、消化管に特異的なNO及び/又はセロトニン遊離促進剤。
- 前記消化管が、胃である請求項8記載の遊離促進剤。
- 機能性消化管障害予防・改善剤を製造するための、グルタミン酸の塩基性アミノ酸塩の使用。
- 塩基性アミノ酸がアルギニン、リジン及びオルニチンからなる群より選ばれる請求項10記載の使用。
- 塩基性アミノ酸がアルギニンである請求項11記載の使用。
- 前記機能性消化管障害が上部消化管機能障害である、請求項10〜12のいずれか1項に記載の使用。
- 前記上部消化管機能障害が機能性胃腸症又は胃食道逆流症である、請求項13記載の使用。
- 成人に対する前記有効成分の1日当たりの投与量が0.01〜20gである、請求項10〜14のいずれか1項に記載の使用。
- 消化管に特異的なNO及び/又はセロトニン遊離促進剤を製造するための、グルタミン酸の塩基性アミノ酸塩の使用。
- 前記消化管が胃である請求項16記載の使用。
