Description
血管形成術(angioplasty)又はアテレクト
ミ(atherectomy)の後、動脈内腔の開存を
維持するために用いられる動脈内ステントに関する。
0年代の中頃から動脈内ステントは、バルーン血管形成
術又はアテレクトミの後の再狭窄を防ぐための治療に広
く使用されてきている。この再発の問題は、バルーン拡
張された又はアテレクトミによるプラーク切除の個所に
おける過度の組織成長(内膜の増殖)が動脈の再狭窄を
起こすことにある。この問題を解決できる1つの方法
は、血小板と線維素の沈着とを減らすように、抗血栓形
成の表面材でステントを覆うことである。このことは、
ロバート イー フィシェル及びティム エイ フィシ
ェルによる、「動脈狭窄拡張及び再狭窄防止用の血管内
ステント及び経皮挿入カテーテルシステム」なる名称の
1988年9月に発行された米国特許第4,768,5
07号に記載されている。坑血栓形成材のコーティング
によれば、血栓による動脈閉鎖症を防ぎかつ抗凝固剤治
療の必要性を減らすことができるが、内膜の増殖に基づ
く再狭窄を減らすことは、依然、緊急な必要性を有して
いる。
おける癌細胞の急速な成長による増殖を低減できること
は良く知られている。本発明の動脈内ステントは、バル
ーン血管形成術又はアテレクトミによって生じる動脈壁
の外傷を原因とする急速な組織成長を低減するために、
ステントの挿入位置に近接した組織に照射可能であり、
動脈内ステントと一体化したラジオアイソトープを使用
するものである。

許第4,768,507号の図5及び図6には、このよ
うなスプリングワイヤ及びヘリカルコイルステントの断
面が示されている。

属又は合金から製造されたヘリカルコイルスプリングス
テントの2ターンの断面10を示している。これら2タ
ーンは、動脈壁AW内でプラークP内に埋められている
ように示されている。断面10から外方向に向いた矢印
12は、ステントを構成するワイヤからの粒子の全方向
への放出を表している。この放射線の目的は、外傷を受
けた動脈壁AWの増殖性細胞の成長(この成長は「内膜
の増殖」と称される)速度を減少させることである。こ
れにより、ステント挿入後に頻繁に生じる再狭窄がかな
り低減されるであろうと期待される。
トープは、アルファ、ベータ又はガンマ線の放射線源で
ある。その半減期は、理想的には10時間から100日
の間である。最適な放射線源は、16日の半減期を有し
かつその放射エネルギの8%だけがガンマ放射線からの
ものであるバナジウム48のようなベータ線放射アイソ
トープである。ベータ線放射源の理想的な属性は、放射
線が人体の組織内をさほど遠くまで伝わらないことであ
る。これにより、ラジオアイソトープステントに近接す
る組織だけが影響されるであろう。さらに、非常に高い
レベルが非増殖の組織に対して損傷を与えることが知ら
れているため、放射線は中程度のレベルとすることが望
ましい。

の一時的配置、例えば、規定時間後に取り出すことがで
きる放射線チップ備えた薄いワイヤ、も想定できる。

付の特許請求の範囲の範囲内であれば、以上述べた実施
例以外にも本発明が実施可能であることは理解されるべ
きである。

Claims (10)
- 【請求項1】 外側表面が動脈壁に係合するようになさ
れておりその全長にわたって開存しているほぼ管状の構
造体を備えた動脈内ステントであって、 前記ステントは、人体の動脈内に挿入されるように適合
されており、前記ステントの少なくとも一部は、当該ス
テントに近接した動脈壁における細胞の増殖を低減する
ことができる放射線を放出する放射性物質から形成され
ていることを特徴とする動脈内ステント。 - 【請求項2】 放射線を放出する前記放射性物質が、ラ
ジオアイソトープである請求項1に記載のステント。 - 【請求項3】 前記ラジオアイソトープが、当該ステン
トの前記ほぼ管状の構造体のコアの位置に設けられてい
る請求項2に記載のステント。 - 【請求項4】 前記ラジオアイソトープが、当該ステン
トの前記ほぼ管状の構造体上にメッキされている請求項
2に記載のステント。 - 【請求項5】 抗血栓剤材料による外側コーティングを
含む請求項2に記載のステント。 - 【請求項6】 前記ラジオアイソトープが、ベータ粒子
を放出するラジオアイソトープである請求項2に記載の
ステント。 - 【請求項7】 人体の動脈壁に対して放射外方向に広が
るようになされたほぼ管状で薄い壁の構造体を備えた動
脈内ステントであって、 前記ステントの少なくとも一部は、当該ステントに近接
した細胞の増殖を低減することができる放射線を放出す
るラジオアイソトープ材料から形成されていることを特
徴とする動脈内ステント。 - 【請求項8】 前記ラジオアイソトープが、100日よ
り少ない半減期を有している請求項7に記載のステン
ト。 - 【請求項9】 狭窄位置における細胞成長の増殖を低減
すべく、動脈内の該狭窄位置に隣接して一時的に配置す
るために、人体の動脈内に挿入される放射性チップを有
する細長のワイヤを備えたことを特徴とする動脈再狭窄
防止装置。 - 【請求項10】 動脈外傷後の動脈の再狭窄防止に使用
する薄いワイヤ状の構造体であって、動脈内の外傷位置
に一時的に挿入可能であり該外傷位置で動脈の再狭窄を
防止するための放射線チップの形態の放射線源を備えて
おり、該放射線チップは、ベータ粒子放射源のラジオア
イソトープを含んでおり、前記動脈内にベータ粒子を放
出するものであることを特徴とする薄いワイヤ状の構造
体。
