本発明は、対象のがんの予防、軽減または治療方法を提供する。
(化合物I)、またはその製薬学的に許容可能な塩、遊離塩基、水和物、錯体もしくはキレート(鉄、亜鉛などのような金属のキレートを含む)を治療上有効な量、その対象に投与することを含み、その対象が、DNA修復遺伝子に変異を有する方法を提供する。一実施形態では、その対象は、ヒトである。別の実施形態では、その対象は、すでにがんである。
一実施形態では、本開示の方法は、化合物I、またはその製薬学的に許容可能な塩、遊離塩基、水和物、錯体もしくはキレートを治療上有効な量、対象に投与することであって、化合物Iを治療上有効な量投与されなかった対象と比べて、該対象における骨髄抑制の発生を予防または低減することに対するものである。特定の実施形態では、化合物Iを治療上有効な量投与されなかった対象は、がんの治療または予防のために、化合物Iではない化学療法剤を投与されたことがある。したがって、一実施形態では、本開示の方法は、化合物I、またはその製薬学的に許容可能な塩、遊離塩基、水和物、錯体もしくはキレート(鉄、亜鉛などのような金属のキレートを含む)を治療上有効な量、対象に投与することであって、化合物Iではない化学療法剤を投与されたことがある対象と比べて、該対象における骨髄抑制の発生を予防または低減することに対するものである。本明細書で使用する場合、骨髄抑制とは概して、造血の1つ以上の構成要素(例えば骨髄活性)の抑制を指し、造血プロセスの産物である種類の細胞のうちの1つ以上が異常なレベルとなることで顕在化する。造血の1つ以上の構成要素(例えば骨髄活性)の抑制とは、例えば、白血球数及び/または血小板数の抑制を指してよい。したがって、一実施形態では、対象のがんを予防、軽減または治療するための本開示の方法であって、化合物I、またはその製薬学的に許容可能な塩、遊離塩基、水和物、錯体もしくはキレート(鉄、亜鉛などのような金属のキレートを含む)を治療上有効な量、その対象に投与することを含み、その対象が、DNA修復遺伝子に変異を有し、その対象において、骨髄活性の低下が、化合物Iを治療上有効な量投与されなかった対象と比べて90%未満となる方法を提供する。例えば、その対象は、骨髄活性の低下が、化合物Iを治療上有効な量投与されなかった対象と比べて90%未満、85%未満、80%未満、75%未満、70%未満、65%未満、60%未満、55%未満、50%未満、45%未満、40%未満、35%未満、30%未満、25%未満、24%未満、23%未満、22%未満、21%未満、20%未満、19%未満、18%未満、17%未満、16%未満、15%未満、14%未満、13%未満、12%未満、11%未満、10%未満、9%未満、8%未満、7%未満、6%未満、5%未満、4%未満、3%未満、2%未満、1%未満、0.75%未満、0.5%未満、0.25%未満となる。一実施形態では、化合物Iを治療上有効な量投与された対象は、骨髄活性の低下が、化合物Iを治療上有効な量投与されなかった対象と比べて10%未満となる。一実施形態では、化合物Iを治療上有効な量投与された対象は、骨髄活性が、化合物Iを治療上有効な量投与されなかった対象と比べて低下しない。

一実施形態では、本発明は、化合物Iを少なくとも1つの追加の治療有効剤とともに含む併用療法剤を提供する。

薬物及び試薬
化合物I及び重水素化化合物I(化合物I−d6)は、APTOSE Biosciences(San Diego,CA)から得た。洗浄剤適合タンパク質アッセイキットDC(商標)Protein Assayは、BioRad Laboratories,Inc.(Hercules,CA)から購入した。CellTiter96(登録商標)Aqueous One Solution Cell Proliferation Assay(MTS)は、Promega(Madison,WI)から購入した。PARP、MCL−1、BAD、BIK、Na+/K+ ATPase抗体は、Cell Signaling Technology,Inc.(Danvers,MA)から得た。pSer139 H2AX及びATM抗体は、Abcam(Cambridge,UK)から購入した。ABCG2抗体は、KAMIYA Biomedical(Tukwila,WA)から得た。Ko143及びpSer1981−ATM抗体は、Millipore Sigma(St.Louis,MO)から得た。オラパリブは、Selleckchem(Houston,TX)から購入した。カルボプラチン及びトポテカンは、UCSD Moores Cancer Center Pharmacyから入手した。
ヒトバーキットリンパ腫細胞株Rajiは、American Type Tissue Culture Collectionから入手し、10%ウシ胎児血清(ATCC)を添加したRPMI1640培地(ATCC)中で、37℃、5%CO2で培養した。化合物I耐性Raji(Raji/化合物IR)細胞株は、6カ月の期間にわたって、漸増濃度の化合物Iに暴露させることによって作製した。CAOV3細胞は、ATCCから入手し、10%ウシ胎児血清を添加した完全DMEM中で培養した。MCF7ベクターコントロール及びBRCA1 shRNAサブクローンは、Dr.Simon Powell(Memorial Sloan−Kettering Cancer Center)から入手し、10%ウシ胎児血清を含むEMBM中で培養した。MCF10A及びhTERT−IMECクローンは、Dr.Ben Ho Park(Johns Hopkins University)から入手した。HCT116 BRCA2+/+細胞及びBRCA2−/−細胞は、Dr.Samuel Aparicio(British Columbia Cancer Research Centre)から入手した。PEO1及びPEO4細胞は、Dr.Sharon Cantor(University of Massachusetts)から入手し、これらの細胞株は、以前に公開されたのと同じ条件で培養した。Sakai et al.,Functional restoration of BRCA2 protein by secondary BRCA2 mutations in BRCA2−mutated ovarian carcinoma,Cancer Res.2009;69(16):6381−6、Konishi et al.,Mutation of a single allele of the cancer susceptibility gene BRCA1 leads to genomic instability in human breast epithelial cells,Proc.Natl.Acad.Sci.2011;108(43):17773−8、Xu et al.,CX−5461 is a DNA G−quadruplex stabilizer with selective lethality in BRCA1/2 deficient tumours,Nature Communications 2017;8:14432(いずれも、参照により、本明細書に援用される)。
細胞を播種し、示されている薬物によって、96ウェルプレート中で、5日間処理した。細胞の生存能は、Promegaから購入したCellTiter96 AQueous One Solution(MTS)Cell Proliferation Assayを用いて測定し、IC50値は、GraphPad Prism6というソフトウェアを用いて算出した。
ABCG2の発現を定量するために、細胞の表面をEZ−LINKスルホ−NHS−SS−ビオチン(Thermo Scientific,Pittsburg,PA)でビオチン化し、以前に報告されたように、ウエスタンブロット解析を行い、ウエスタンブロット解析を行った。Tsai CY,Liebig JK,Tsigelny IF,Howell SB,The copper transporter 1(CTR1)is required to maintain the stability of copper transporter 2(CTR2).Metallomics 2015;7:1477−87(参照により、本明細書に援用される)。
RNeasy Mini Kit(QIAGEN,Valencia,CA)を用いて、実験ごとに、3つの独立した試料から全細胞RNAを単離した。ライブラリーの作製、及びAgilent Bioanalyzerを用いた検証のために、RNA−seq試料をIGM Genomics Center,University of California,San Diego,La Jolla,CA(http://igm.ucsd.edu/genomics/)に提出した。シーケンシングをIllumina Sequencer HiSeq4000で行った。バイオインフォマティクス解析をOHSUによって行った。ABCG2の過剰発現の確認の際に用いたフォワードプライマーは、5’−TTA−GGA−TTG−AAG−CCA−AAG−G−3’、リバースプライマーは、5’−TAG−GCA−ATT−GTG−AGG−AAA−ATA−3’であった。
標準物質の重水素化化合物Iを5ng含むアセトニトリル中で、化合物IまたはFe(化合物I)3に暴露した細胞をホモジナイズした。UCSD Molecular Mass Spectrometry Facilityで、イオン源として、正イオンモードのエレクトロスプレーイオン化法(ESI)を用いた質量分析計Thermo LCQdecaと連結した液体クロマトグラフ(LC)システムAgilent1260を用いて、試料を解析した。そのESIのイオン源電圧は5kVに設定し、シースガス流量は80単位、補助ガス流量は20単位、キャピラリー温度は250℃とした。LCによる分離では、Phenomenex Kinetex Biphenylカラム(内径2.1mm×長さ50mm、粒径2.6μm)を使用し、0.1%ギ酸を含む水を移動相Aとして、0.1%ギ酸を含むアセトニトリルを移動相Bとして用いた。LC流量は、0.30mL/分に設定した。LCグラジエントは、10分で移動相Bを5%から移動相Bを95%まで増加させ、Bを95%で2分間保ち、1分でBを5%まで戻してから、Bを5%で6分間保った。正イオンモードのESI−MS/MS解析では、化合物Iは、正規化衝突エネルギー45%で、m/z368における分子イオンピークから、主要フラグメントピークがm/z353で観察され([M+H]+)、化合物I−d6は、正規化衝突エネルギー45%で、m/z374での分子イオンピークから、主要フラグメントピークがm/z359で観察された([M+H]+)。選択反応モニタリング(SRM)モードを用いて、m/z353及びm/z359のフラグメントピークを得た。試料における化合物I及びFe(化合物I)3の定量には、化合物I−d6の添加量に対するSRMピーク面積比(化合物I/化合物I−d6)を用いた。Fe(化合物I)3の検出でも、同じカラム、グラジエント及び流速を使用し、Agilent1100 HPLC、及びThermo IonMax ESIのインターフェースを用いた質量分析計Orbitrap XL(Thermo)を用いて検出した。Fe(化合物I)3は、これらの条件によって、11.5分前後で溶出された。0.3mL/分の溶出流速に対して、10:1の流量スプリット比を用いたので、スプリット後、約0.030mL/分をESIに導入した。イオン源MSパラメーターは、キャピラリー温度250℃、シースガス流量20単位、正の極性、イオン源電圧5.0kV、キャピラリー電圧22V及びチューブレンズ80Vであった。フーリエ変換MS(Orbitrap)パラメーターは、FTMS AGC 1e6、FTMS平均マイクロスキャン数2及びFTMSフルスキャン最大イオン時間500ミリ秒であった。15,000という分解能パラメーター(m/z400におけるピークm/zを、半値全幅として与えられたピーク幅で除した値)を使用した。MS−MS CIDのスペクトルでは、45%の正規化衝突エネルギーを使用した。
濃縮水ストック中のFeSO4として二価鉄イオンを5モル当量、エタノール中の化合物Iに加えたところ、深赤色の沈殿物が得られ、その後、その沈殿物をDMSOに溶解させ、HPLC及び質量分析によって特徴付けた。Fe(化合物I)3は、純度が95%超であったとともに、完全RPMI−1640培地中で少なくとも5日間安定していた。
コメットアッセイキットをTrevigen(Gaithersburg,MD)から購入し、中性コメットアッセイを製造元の指示に従って行った。Keyence Fluorescent Microscope(Keyence America,Itasca,IL)によって画像を取得し、OpenCometというソフトウェアを用いて定量した。
細胞を回収し、PBSで2回洗浄し、Z固定液(緩衝亜鉛ホルマリン固定剤、Anatech,Inc,Creek,MI)で固定し、5%ウシ血清アルブミンを含むPBS中の0.3%Triton X−100で浸透化処理及びブロックした。続いて、その細胞をγH2AX抗体(1%ウシ血清アルブミンを含むPBS中の0.3%Triton X−100での1:250希釈液)とともに一晩インキュベートしてから、3回洗浄した。細胞を1時間、蛍光コンジュゲート二次抗体(1:1000希釈液)とともにインキュベートしてから、3回洗浄した。4’,6−ジアミジノ−2−フェニルインドール(DAPI)とともに、非退色試薬ProLong Goldを用いてスライドに封入して、細胞核を染色した(分子プローブ)。100倍の対物倍率を用いて、Keyence Fluorescent Microscopeで蛍光を観察し、Image Jというソフトウェア(National Institutes of Health)で定量した。
いずれの2群比較においても、不等分散と仮定したスチューデントt検定を用いた。データは、少なくとも3回の独立した実験の平均±標準誤差として示されている。
化合物Iが強力な細胞毒性を示すタイプの細胞の中でも、リンパ腫は注目されている。この疾患の治療で用いる標準的な化学療法剤の大半が、用量を制限する骨髄抑制を起こすからである。このため、化合物Iの細胞薬理試験用には、Rajiバーキットリンパ腫細胞を選択した。Raji細胞における化合物Iの細胞内蓄積を液体クロマトグラフィータンデム質量分析(LC−MS/MS)によって定量した。化合物I及びその内部標準である化合物I−d6は、鋭いピークプロファイルで、LCカラムから約6.9分で溶出された。Raji細胞には、化合物Iは比較的ゆっくり蓄積され、6時間までに、含有量は定常状態に近づいた(図6A)。
化合物Iの構造は、DNAの四重鎖構造に結合して鎖を切断させる薬物と似ていることから、化合物IがDNAを損傷させるか調べた。親Raji細胞を0.5μMの化合物Iで長期間処理し、ウエスタンブロット解析によって測定される、ATMのリン酸化形態及びγH2AXの蓄積によって、DNA損傷の誘導を評価した。図2Aには、Raji細胞において、化合物Iによって、6時間の時点から、リン酸化ATM及びγH2AXが明らかに増加したこと、ならびに、その量は、薬物暴露時間(最長で24時間)とともに増加したことが示されている。8時間の時点から、PARPの切断(アポトーシスの誘導を示す)が検出された。Raji細胞は、核が非常に小さいため、γH2AXのフォーカス形成を定量するのは難しいので、この目的では、ヒト卵巣癌細胞株CAOV3を使用した。図2Bには、DMSOまたは1μMの化合物Iに24時間暴露させたCAOV3細胞におけるγH2AXのフォーカス形成の代表的な画像が示されている。図2Cには、1時間の時点に、フォーカス数の増加が検出可能となり、8時間後に、フォーカス数の増加がさらに顕著になったことが示されている。主にDNA二本鎖切断を検出する中性コメットアッセイの結果によって、DNA損傷の証拠がさらに強化された(図2D)。細胞を0.5μMの化合物Iで6時間処理したところ、DMSOで処理した場合と比べて、テールDNAの増加は見られなかったが、細胞を化合物Iで6時間処理してから、無薬剤培地で18時間インキュベートしたところ(パルスチェイス)、コメットテール内のDNAが著しく増加した。これらの結果から、化合物IがDNAを損傷させ、アポトーシスを誘導できるDNA鎖切断を蓄積させる強力な証拠が示されている。
化合物IがDNAを損傷させるという知見から、相同組み換えを欠損した細胞が、化合物Iに対する過敏性を有するかを調べることにした。化合物IとBRCA1の欠損には合成致死性があるのではないかという仮説について、BRCA1非欠損ヒト細胞株及びBRCA1欠損ヒト細胞株の同種同系対を用いて試験を行った。2つの独立したMCF10Aサブクローン(それぞれ、BRCA1の2bpが欠失されたヘテロ接合体ノックインを含み、その結果、中途終止コドンが生じる)(BRCA1−het#1及びBRCA1−#2)は、ゲノム内でターゲティングベクターのランダムインテグレーションを起こしたクローン(コントロール)よりも、オラパリブに対する感受性が高いことが明らかとなり、その2つのノックインクローンで、BRCA1機能が喪失していることが確認された(図3A、左図)。これらの2つのノックインクローンは、化合物Iに対する方が、過敏性がさらに高かった(図3A、右図)。hTERT−IMEC細胞株に由来する、同じ2bpのノックインを含むクローンで、BRCA1の機能不全の作用を確認した場合にも、オラパリブ及び化合物Iの両方に対する過敏性を有することが明らかになった(図3B)。shRNAiの発現により、BRCA1の発現が安定的にノックダウンされるMCF7 E7細胞で得られた結果によって、BRCA1欠損細胞が、化合物Iに対する過敏性を有するという結論が、さらに裏付けられた。図3Cに示されているように、E7クローンも、オラパリブ及び化合物Iに対する過敏性が同程度である。BRCA1機能正常細胞とBRCA1機能不全細胞との3つの独立した同種同系対におけるこれらの結果から、化合物Iによって生じるDNA損傷の修復は部分的に、BRCA1が機能する相同組み換え経路及び/またはその他のDNA修復経路に依存することが示されている。BRCA2非欠損卵巣癌細胞株PEO4及びBRCA2欠損卵巣癌細胞株PEO1を用いて、BRCA2欠損細胞の方が、化合物Iに対して高い感受性を有するかについて、試験を行った。PEO1は、BRCA2を欠損しており、シスプラチン及びポリ(ADPリボース)ポリメラーゼ阻害剤AG14361に対して感受性を有する。PEO4は、再発時の腹水から抽出したものであり、シスプラチン耐性を有し、BRCA2機能を回復させる二次変異を含む。BRCA2機能の回復により、オラパリブ(図3D、左図)及び化合物I(図3D、右図)の両方に対する耐性が向上した。BRCA2を欠損していないHCT116細胞、ならびに2つのBRCA2−/−サブクローン(B18及びB46)を用いた場合も、同様の結果が得られた(図3E)。したがって、悪性細胞は、BRCA1またはBRCA2の機能のいずれが喪失しても、化合物Iに対する過敏性を有するようになる。
化合物Iの作用のうち、化合物Iに対する感受性と最も密接に関係する作用を明らかにするために、6カ月間にわたって、漸増濃度の化合物Iに繰り返し暴露させた結果、耐性を獲得したRajiバーキットリンパ腫細胞株のサブライン(Raji/化合物IR)を確立した。耐性は、選択プロセスのいずれの時点においても、急激に変化することなく、ゆっくりかつ段階的に獲得された。薬物に120時間暴露させた際に、成長速度を定量するアッセイを用いて試験したところ、親Raji細胞に対する化合物IのIC50は、91.9±22.3nMであった。この値は、新鮮単離AML芽球細胞及びCLL細胞で報告された値と同じ範囲内である。Zhang et al.,“Inhibition of c−Myc by ATPO−COMPOUND I as an Innovative Therapeutic Approach to Induce Cell Cycle Arrest and Apoptosis in Acute Myeloid Leukemia [abstract],”Blood 2016;128:1716、Kurtz et al.,“Broad Activity of COMPOUND I in AML and other Hematologic Malignancies Correlates with KFL4 Expression Level [abstract],”Blood 2015;126:1358(いずれも、参照により、本明細書に援用される)。Raji/化合物IR細胞は、化合物Iに対する耐性が16.7±3.9倍であった(IC50:1387.7±98.5nM)。耐性のレベルは、無薬剤培地での培養中、少なくとも3カ月安定した状態を保った(図4A)。Raji/化合物IR細胞は、親細胞よりもわずかに成長が速かったが、統計的に有意な差は見られなかった。Raji/化合物IR細胞においては、感受性Raji細胞でアポトーシスが誘導された濃度では、化合物Iによってアポトーシスは誘導されなかった。その感受性細胞を0.5μMの化合物Iで24時間処理したところ、コントロールのDMSOと比べて、プロアポトーシスタンパク質のBIKが47.5±16.8%及びBADが2.1±0.25倍増加し(P<0.05、n=3)、抗アポトーシスタンパク質のMCL−1は、38.1±2.3%減少した(p<0.001、n=3)。同じ暴露を行ったRaji/化合物IR細胞では、これらの変化は検出されなかった(図4B)。
Raji/化合物IR細胞における耐性は、薬物の流入もしくは排出の変化、細胞内解毒または一次標的の変化によるものと思われる。ネイティブ化合物Iまたは錯体Fe(化合物I)3のいずれかとともにインキュベートしたRaji細胞及びRaji/化合物IR細胞におけるネイティブ化合物I及びFe(化合物I)3の両方の細胞内蓄積をモニタリングした。化合物Iに暴露させたRaji/化合物IR細胞では、両方の形態の薬物の蓄積速度が大きく低下した(図6A及び表1)。
細胞を錯体Fe(化合物I)3とともにインキュベートした場合も、程度は低いものの、同様の結果が得られた(図6B)。対照的に、最初の2時間において、ATPO−化合物IまたはFe(化合物I)3の排出は、細胞にいずれの形態の薬物を負荷した後でも、明らかな差は見られなかった。これらの結果から、Raji細胞における化合物Iに対する耐性が、両方の形態の薬物の蓄積低下と関連することが示されている。6時間時点における薬物蓄積をさらに詳細に測定したところ、Raji/化合物IR細胞を化合物Iとともにインキュベートした場合に、両方の形態の薬物の蓄積は著しく低下したが、錯体Fe(化合物I)3のレベルは依然として、その原薬のレベルよりも高かったことが確認された(図4C)。Raji/化合物IR細胞を少なくとも3倍多い化合物Iで処理した場合のみ、Fe(化合物I)3の細胞内含有量が最終的に、感受性細胞におけるレベルと同程度のレベルに達した(図4D)。Raji/化合物IR細胞を0.5μMの化合物Iで24時間処理したところ、リン酸化ATMまたはリン酸化γH2AXは増加せず、PARPの切断は検出不能であったこと(図7)は、その耐性細胞において、細胞内の化合物I及びFe(化合物I)3が実質的に少ないことと整合している。
Raji/化合物IRにおいて、いくつかの他の多剤耐性ABCトランスポーターもアップレギュレートしたが、ABCG2転写産物の増加が最も顕著であった(表3)。Raji/化合物IR細胞におけるABCG2の顕著なアップレギュレーションは、qRT−PCR及びウエスタンブロット解析によって確認した(図5A及びB)。
最新の機序研究によって、化合物Iは、そのプロモーターにおけるアセチル化H3K27を減少させること、さらには、c−MYC mRNA不安定化することによって、c−MYCを転写レベルで調節するをことが示された。加えて、RNA−seqの示唆的遺伝子発現解析及び逆相タンパク質アレイ(RPPA)データによって、化合物Iの機序におけるc−MYCの役割が明らかにされた(GOターム:c−MYCによってダウンレギュレート(p値6E−26)、c−MYCが結合する遺伝子プロモーター(p値4.2E−10)、c−MYCのChIP標的(p値3.3E−8))。さらに、RPPAデータから、p−CHK1、p−CHK2、γH2AX、ならびに全p53及びE2F1の増加が観察され、これらのいずれも、DNA損傷応答経路の活性化を示すものである。この増加には、細胞ストレス応答シグナル伝達を示すXBP1、GRP78及びp−p38のレベルの上昇(GOターム:細胞ストレスの調節、p値1.89E−8)が伴っていた。
細胞及び化合物
EOL−1、GRANTA−519、Jeko−1、Jurakat、Molm−13、NOMO−1、SKM−1及びSU−DHL−6は、Leibniz−Institut DSMZから入手した。HL−60、KG−1、Mino、MV4−11、Raji及びTHP−1は、ATCCから入手した。HEL92.1.7は、European Collection of Authenticated Cell Culturesから入手し、Ramos細胞は、Dr.M.Andreeff(MD Anderson Cancer Center,Houston,TX)から寄付されたものである。すべての細胞は、製造元の指示に従って、完全培地中で培養した。製造元から受領してから1カ月以内に、初期継代細胞を回収し、凍結した。すべての実験は、初期継代細胞で、解凍から6週間以内に行った。6カ月ごとに、MycoScope Mycoplasma Detection Kit(Genlantisカタログ番号MY01050)を用いて、コンタミネーションの可能性についてスクリーニングした。Ficoll−Paque PLUS(GE Healthcare、カタログ番号17−1440−02)を用いて、新鮮な健常ドナー血液から末梢血単核球(PBMC)を単離した。化合物Iの遊離塩基の合成では、10−フェナントロリン−5,6−ジオン(1.2当量)、酢酸(22倍量)、2−メチル−5−フルオロインドール−3−カルボキシアルデヒド(1.0当量)及び酢酸アンモニウム(15当量)を培地攪拌下で、95±5℃で加熱しながら、3〜7時間反応させた。その反応物を20℃〜25℃まで冷却し、濾過し、酢酸及びエタノールでリンスし、N2パージで乾燥してから、65℃の2:1のエタノール:水で4時間洗浄し、20℃〜25℃まで冷却し、濾過し、2:1のエタノール:水及びEtOAcでリンスしてから、N2パージで乾燥した。HPLCによる純度は99.5%であり、構造的な同定をFT−IR、1HNMR、13C NMR及びLC/MSによって確認した。Fe(化合物I)3の合成では、水中の5モル当量の二価鉄イオンFeSO4を、エタノールに溶解させた化合物Iに加えた。得られた深赤色の沈殿物Fe(化合物I)3を回収し、DMSOに溶解させ、HPLC及び質量分析によって、純度95%超として特徴付けられた。CX−5461(7)は、MedChem Expressから購入した(カタログ番号HY−13323)。
細胞を播種し、ビヒクルのDMSOまたは化合物I(10個の濃度)によって、96ウェルプレートにおいて5日間、37℃及び5%CO2で処理した。CellTiter96 AQueous One Solution(MTS)Cell Proliferation Assay(Promega、カタログ番号G3581)を用いて、細胞の生存能を測定し、GraphPad Prism7というソフトウェアを用いて、IC50値を算出した。
標準物質の重水素化化合物Iを5ng含むアセトニトリル中で、化合物Iに暴露させた細胞をホモジナイズした。UCSD Molecular Mass Spectrometry Facilityで、イオン源として、正イオンモードのエレクトロスプレーイオン化法を用いた質量分析計Thermo LCQdecaと連結した液体クロマトグラフ(LC)システムAgilent1260を用いて、試料を解析した。
細胞をビヒクルまたは化合物Iによって、様々な濃度で24時間、または単一の濃度で1時間、3時間、6時間、12時間及び24時間処理してから、回収した。QiaShredderカラム(QIAGEN、カタログ番号79656)によって細胞を溶解させ、QIAGEN RNeasy Plus Mini Kit(カタログ番号74134)を用いて、全RNAを単離し、Transcriptor Universal cDNA master mix(Roche、カタログ番号05893151001)を用いて、cDNAを合成してから、FastStart essential DNA probes master mix(Roche、カタログ番号06402682001)及びRoche LightCycler96を用いたqRT−PCR解析で、そのcDNAを使用した。プライマープローブ対は、IDTから購入した(表5)。発現量は、GAPDHに対して正規化した後、ビヒクルで処理した試料に対する倍率変化として計算した(2ΔΔC t)。
細胞を上記のようにして処理した。全細胞溶解液を調製し、SDS−PAGEによって分離し、ニトロセルロース膜に転写した。使用した検出抗体は、表6に列挙されている。ImageJまたはImage Studio Liteバージョン5.2というソフトウェアを用いて、デンシトメトリーを行い、GAPDHの密度に対して正規化した。
細胞を上記のようにして処理した。アポトーシスを測定するために、細胞をFITC−アネキシンV及びプロピジウムアイオダイド(PI、BD Pharmingen、カタログ番号556570)で染色してから、フローサイトメーターBD Accuri C6で解析した。DNA合成及び細胞周期相を測定するために、処理した細胞を5−エチニル−2′−デオキシウリジン(Edu)Alexa Fluor488(Thermo Fisher Scientific、カタログ番号C10425)及びPI(PI/RNase A染色溶液、BD Biosciences、カタログ番号550825)で染色した。Live/Dead Fixable Far Red Dead Cell Stain Kit(Thermo Fisher Scientific、カタログ番号L34973)を用いることによって、死細胞を解析から除外した。染色は、製造元の指示に従って行った。
処理したMV4−11細胞において、(qRT−PCR解析におけるように、)全RNA抽出を行い、UCSDのGenomics Core Facilityでシーケンシングした。Illumina TruSeqを用いて、RNAを処理し、Illumina HiSeq4000で、50bpのリードに対してシングルエンドシーケンシングを行った。Oregon Health and Science University(Portland,OR)のMcWeenyのラボで、データを解析した。FASTQC(http://www.bioinformatics.babraham.ac.uk/projects/FASTQc/)を用いて、FASTQファイルをリードの塩基分布及び配列の表現について評価した。SubRead v1.5.0−plを用いて、リードをHG37にアラインメントし、ユニークにマッピングされたリードを保持した。(4つのすべの試料にわたって、)50リード未満の差次的発現遺伝子を除外した。生のデータ及び処理したファイルは、GEO(https://www.ncbi.nlm.nih.gov/geo/query/acc.cgi?acc=GSE111949)アクセッション番号GSE111949で入手可能である。
RNAシーケンシング(RNA−seq)解析におけるように、MV4−11細胞を処理し、全細胞抽出物をウエスタンブロッティング用に調製した。試料をMD Anderson Cancer CenterのReverse−Phase Protein Array(RPPA)Core Facilityで処理した(詳細はhttps://www.mdanderson.org/research/research−resources/core−facilities/functional−proteomics−RPPA−core/RPPA−process.html)。タンパク質発現レベルを3回反復分について平均し、GraphPad Prism7を用いて、ヒートマップを作成した。
MV4−11細胞をビヒクルのDMSOまたは500nmol/Lの化合物Iで2時間、6時間または24時間処理してから、1%ホルムアルデヒドを用いて架橋した。超音波処理によってクロマチンを抽出してから、H3K27ac抗体(Active Motif、#39133)とともに一晩インキュベートした。プロテインGビーズ(Invitrogen Dynabeadsカタログ番号10004D)を用いて、その抗体:DNA複合体を単離し、MYCプロモーターに特異的なプライマー(表7)を用いて、qPCRによって解析した。H3K27acの濃縮は、インプットDNAコントロールに対する倍率として計算した。
細胞を3時間、ビヒクルのDMSOまたは500nmol/Lの化合物Iで処理してから、1μmol/LのアクチノマイシンDで処理した。qRT−PCR解析におけるように、RNAの抽出及びcDNAの合成を行うために、アクチノマイシンDの添加前及び添加後10分おきに、細胞のアリコートを取り出した。特異的プライマー(表8)を用いて、MYC及び28s rRNAのレベルを解析し、MYC RNAの発現を28s rRNAに対して正規化した[2^(28sのCt値−MYCのCt値)]。
以前に説明されたようにして、FRETアッセイ及びデータ解析を行い、二重標識(5′FAM−3′BHQ1)一本鎖オリゴを用いることによって修飾した。Roche LightCycler96[37℃で300秒後、温度を3℃ずつ91℃まで上昇させ(25段階)、各温度における総インキュベーション時間を300秒とした]を用いて、ビヒクルのDMSO、または漸増濃度の化合物I、Fe(化合物I)3、CX−5461もしくはTMPyP4の存在下で、各オリゴの融解温度を評価した。薬物及びオリゴ反応ミックスを直ちに解析するか、または6時間、室温でインキュベートしてから、解析をするかした。プライマー情報は、表9に示されている。インキュベーション時間が長いほど、Fe(化合物I)3、TMPyP4またはCX−5461の活性は影響を受けなかったが、化合物IのG4結合能は向上した。化合物Iのデータは、6時間の時点について示されている。
化合物Iは、AML細胞株及び様々な型のリンパ腫細胞株において増殖を阻害し、IC50値は、57nmol/L〜1.75μmol/Lの範囲であった(図8、表10)。この薬物は、MV4−11細胞において、暴露期間の関数としての効能の変動が中程度に過ぎず、IC50値は、48時間の暴露では0.47μmol/L、72時間では0.40μmol/L、120時間では0.24μmol/Lであった。以前の研究によって、腫瘍におけるAPT0−化合物I活性の考え得る機序として、KLF4及びCDKN1Aの発現のアップレギュレーションが報告された。化合物Iは、試験した6個のAML細胞株のうちの4個で、KLF4の発現をアップレギュレートするが(図15A)、すべてのAML細胞株で、濃度依存的に、CDKN1A(p21)の発現が誘導された(図15B及び15C)。CDKN1A mRNAのアップレギュレーションは、試験したすべてのAML細胞株において、暴露期間とともに増大した(図15D及び15E)。その後、CDKN1Aの発現は減少し始めたが、その理由は、細胞死の増加である可能性が高い。CDKN1Aの発現の誘導は典型的には、その後にG0−G1細胞周期が停止することと関連し、その停止は、固形腫瘍株の化合物Iによる処理後に観察された。これと整合して、試験したすべてのAML細胞株において、G0−G1期の細胞の用量依存的な増加が観察され、S期及びG2−M期の細胞画分が同時に減少した(図9A〜C、上図)。MV4−11の場合には、すべての生細胞は、1μmol/LのAPT0−化合物Iに24時間暴露後、G0−G1が停止した。CCND3(サイクリンD3)及びCDK4は、G1細胞周期の進行を促進することが知られているが、CDKNIAは、このプロセスを負に調節する働きをする。化合物Iで処理したAML細胞のウエスタンブロット解析によって、3つのAML株のそれぞれにおいて、程度は異なるものの、CDK4及びCCND3の両方が、用量依存的に阻害されたことが明らかになった(図9A〜C、下図、図16A)。
化合物Iが細胞死を引き起こす機序を調べるために、MV4−11、EOL−1及びKG−1というAML細胞を化合物Iで処理するか、または処理しないかし、フローサイトメトリー及びウエスタンブロッティングによって、アポトーシスマーカーの検出を行った。細胞をPI及びアネキシンVで染色して、生細胞(アネキシンV陰性かつPI陰性)、初期アポトーシス細胞(アネキシンV陽性かつPI陰性)、後期アポトーシス(アネキシンV陽性かつPI陽性)、及び死細胞(アネキシンV陰性かつPI陽性)を識別した。すべての細胞株において、24時間の時点に、アポトーシス細胞の濃度依存的な増加が観察された(図10A、図17A)。アネキシンV染色及びPI染色に基づくIC50値は、抗増殖性のIC50値と整合した。PARPの切断(c−PARP1)は、内因性経路及び外因性経路の両方の下流におけるアポトーシスの古典的シグナルである。化合物Iによって、濃度依存的かつ時間依存的にc−PARP1の蓄積が誘導され、この蓄積は、アネキシンV/PI染色によって測定した場合のアポトーシスの誘導と整合した(図10B〜C、図17B)。3つのすべてのAML細胞株において、化合物Iへの暴露から3〜6時間後に、アポトーシス細胞の増加が見られ(図10D)、その後、2時間の暴露時に、G0−G1細胞周期の停止が観察された。
化合物Iが細胞周期停止及びアポトーシスを引き起こすのに利用する経路について、さらなる知見を得るために、mRNAレベル及びタンパク質レベルの両方で、差次的発現解析を行った。MV4−11細胞をビヒクルまたは500nmol/Lの化合物Iのいずれかで、6時間処理してから、RNA−seqによって遺伝子発現を解析した。合計1,643個の遺伝子が、化合物Iによる処理によって、差次的に調節されたことが明らかになり(2倍超の変化、P<0.05)、416個がアップレギュレートされ、1,227個がダウンレギュレートされた(表11)。RNA−seq解析によって、KLF4が2倍増加し、CDKN1Aの発現が4.5倍増加したことが検出され、これは、MV4−11細胞を用いたqRT−PCRデータによって検証されている。差次的に調節された遺伝子は、Broad Molecular Signaturesのデータベース(http://software.broadinstitute.org/gsea/msigdb/index.jsp)を用いて、経路の濃縮またはGO(遺伝子オントロジー)ターム(図18A)について解析した。予想どおり、差次的に発現した遺伝子セットにおいて、アポトーシス経路及び細胞周期経路が濃縮されていた。予想外にも、化合物Iによる処理後の遺伝子発現プロファイルでは、DNA損傷応答(DDR)経路及び小胞体(ER)ストレス/折り畳み不全タンパク質応答経路も濃縮されていた。加えて、アップレギュレートした遺伝子では、MYCによってダウンレギュレートされるTP53経路及びが濃縮されていた。MV4−11細胞において検出された遺伝子発現変化により、化合物Iが、DNA損傷を誘導し、細胞ストレス経路を活性することによって、及び/またはMYCがん遺伝子の発現を阻害することによって、アポトーシスを引き起こし得る可能性が高まった。
MYCの発現は、白血病及びリンパ腫を含む広範ながんの発症に関与する。最近の研究によって、MYCの転写を阻害すると、血液由来のがん細胞においてアポトーシスが起こることが示され、MYCは、魅力的な治療標的となっている。我々のRNA−seqデータセットの考察によって、MV4−11細胞において、6時間の時点に、MYCが化合物Iによってダウンレギュレートされたことが明らかになった。化合物Iで処理したMV4−11細胞において、MYCによって負に調節される遺伝子の転写が増加したことも観察された。試験したすべてのAML細胞株において、化合物Iによって、MYC mRNA及びタンパク質のレベルの両方が濃度依存的に低下し、MYC阻害のIC50値は、抗増殖性のIC50値と整合していた(図11A及びB)。これらの変化は、MV4−11、EOL−1及びKG−1のAML細胞(図11C、図19A及び19B)において、最長24時間までの暴露時間の関数とした場合、増大した。MV4−11細胞におけるMYCタンパク質の抑制の経時的経過は、RNA−seqによって検出された、MYC遺伝子の発現レベルの阻害と整合していた。試験したすべてのAML細胞株は、健常ドナー由来のPBMCと比べて、MYCの基底発現が有意に高かった(図11D、図19C)。したがって、化合物Iは、調べたすべてのAML細胞株において、mRNAレベル及びタンパク質レベルで、MYCをダウンレギュレートする。
MYCに加えて、RNA及びタンパク質の差次的発現解析によって、化合物Iの作用機序に、TP53、DNA損傷及びERストレスが関与することが示された。MV4−11細胞において、化合物Iによる処理後、TP53タンパク質レベルの上昇を示したRPPAデータの検証を試みた。MV4−11細胞を500nmol/Lの化合物Iに暴露させたところ、早い時点(1時間、3時間及び6時間)に、TP53レベルが有意に上昇し、その後、12時間の時点に、ベースラインまで戻り、24時間の時点には、おそらくは、この時点における大規模な細胞死が原因で、さらに低下した(図12A)。リン酸化Ser15及びアセチル−K382の増加に付随して、全タンパク質の増加が見られた(図12B)。TP53は、DNA損傷に応答して、Ser15及びSer20でリン酸化され、それにより、MDM2の結合及びp53のプロテアソームによる分解が低減する。さらに、p53は、細胞ストレスに応答してアセチル化され、この修飾によって、TP53タンパク質レベルをさらに安定化して、結合活性を調節し得る。TP53の活性化は、BBC3(PUMA)、PMAIP1(NOXA)及びBAXのようなプロアポトーシス因子のアップレギュレーションを通じて、アポトーシスを誘導し得る。RNA−seqデータセットによって、化合物Iで処理したMV4−11細胞において、BBC3が3.95倍増加し、PMAIP1が1.38倍増加したことが示された。DNA損傷及び細胞ストレス経路の関与について、MV4−11細胞において、500nmol/Lの化合物Iによる処理後、早い時点においてさらに調べた。化合物Iを添加した1時間後に、リン酸化CHEK1の増加が検出され、ピークは約4時間の時点だったことから、DNA損傷が早期事象であったことが示唆された(図12C)。CHEK1のリン酸化後、6時間の時点までに、DDRマーカーγH2AXの著しい増加が見られた。試験したすべてのAML株において、γH2AXの濃度依存的な増加が検出されたことによって、化合物IがDDR経路を誘導するという概念がさらに裏付けられた(図12D)。加えて、4〜6時間の処理で、MAPK14リン酸化T180及びMAPK8リン酸化Thr183/pTyr185の両方が増加したことから、DDRまたはERストレス経路を通じたシグナル伝達が示された(図12C)。総合すると、それらのデータによって、化合物Iによって誘導されるDNA損傷が、化合物Iの機序における初期事象であることが示唆されている。
KG−1 AML細胞における化合物Iの取り込み及び排出の動態測定を質量分析によって行ったところ、定常状態に徐々に近づき、初期に急激に排出されるが、終末排出が非常に長く持続するがことが示された(図20A及び20B)。KG−1細胞を化合物Iに1時間または6時間のいずれかで暴露させてから、無薬剤培地に入れたところ、排出パターンは、最初の30分間で急速な相が見られた後、持続的な終末相が続くものからなり、その結果、24時間以上、有意な量の化合物IがKG−1細胞内に維持された。これらのデータと整合して、細胞内薬物動態試験によって、化合物Iが細胞内で、1原子のFe及び3分子の化合物Iを含む錯体[Fe(化合物I)3]に変換されることが明らかになった(図20C及び20D)。実際、事前に錯体化したFe(化合物I)3薬物は、細胞毒性アッセイにおいて、単量体の親化合物Iと同様の効能である(図13A)。さらに、錯体Fe(化合物I)3は、MV−4−11細胞において、c−PARPによって測定したところ、アポトーシス経路を誘導し、γH2AXによって測定したところ、DNA損傷経路を誘導した。Fe(化合物I)3は、用量依存的に、KLF4及びCDKN1Aの発現も誘導し、MYCを阻害した(図13B)。しかしながら、24時間アッセイ(細胞毒性アッセイにおける5日間の処理と比較)において、親化合物Iと同等の応答を惹起するには、親化合物よりも高い濃度のFe(化合物I)3が必要であった。この理由は、事前に錯体化したFe(化合物I)3で観察された流入速度の方が遅かったことである可能性が高い(図20E)。
親化合物I及びその細胞内形態Fe(化合物I)3は、その薬剤をG−四重鎖(G4)DNAリガンドとして機能させることができる金属配位フェナントロリン環及び平面構造のような特定の特徴を含む。G4は、グアニンリッチ領域に起因する動的なDNA二次構造であり、それらの領域は、折り畳まれて、平らなグアニン四分子を形成し、それらが上に積み重なっている。G4特異的な配列は、テロメア及び多くの重要ながん遺伝子のプロモーターに見られる。G4配列は、遺伝子発現の調節因子として機能し、G4四重鎖を安定化させる小分子リガンドは、KIT及びMYCのような重要ながん遺伝子をダウンレギュレートするのに活用されている。テロメアDNAにおけるG4モチーフの安定化は、テロメラーゼの阻害、テロメアの不安定性及び脱保護(これらはいずれも、DDR経路を誘導できる)を引き起こすことができる。さらに、DNA複製部位の開始点は、DNA G4配列と重複しており、このような部位でのG−四重鎖構造の安定化により、複製フォークの停止及び細胞周期停止が起きる。
化合物Iは、非臨床モデルにおけるその有効性、及び動物または固形腫瘍患者における初期の第I相試験で骨髄抑制を起こさなかったことから、現在、AMLの治療用として臨床開発段階にある。本明細書に報告されているデータによって、より精密な臨床応用及びバイオマーカーの開発への方向性を示すこの新規薬剤の作用機序に関する新たな知見が得られる。これらの試験によって、化合物Iが、AML細胞におけるG0−G1細胞周期の停止及びアポトーシスの強力な誘導剤であることが確認された。さらなる新たな所見には、化合物Iが、MYCのプロモーター及びmRNAの安定性の両方に対する作用を通じて、MYCを時間依存的かつ濃度依存的にダウンレギュレーションすること、化合物Iが、多くのAML細胞株において、マスター転写因子及び腫瘍抑制因子のKLF4を誘導すること、ならびに、化合物Iが、DNA損傷を誘導することが含まれる。加えて、事前に錯体化した鉄形態の化合物IであるFe(化合物I)3によって、アポトーシス、DNA損傷及びMYCの発現のダウンレギュレーションを含め、同等の細胞傷害性細胞作用が生じる。

の1つ以上の分子を治療上有効な量、1つ以上の金属原子との錯体で投与すること含み、前記対象が、DNA修復遺伝子に変異を有する前記方法。 - 前記1つ以上の金属原子が、鉄、亜鉛、アルミニウム、マグネシウム、白金、銀、金、クロム、ニッケル、チタン、銅、スカンジウム、ジルコニウム、バナジウム、モリブデン、マンガン、タングステン及びコバルトからなる群から選択される、請求項32に記載の方法。
- 前記1つ以上の金属原子が、鉄である、請求項33に記載の方法。
- 前記錯体が、下記の構造を有する、請求項34に記載の方法。
