本開示は、一部の局面では、NK細胞およびT細胞を含む、養子療法用の操作された細胞に関する。一部の局面において、さらに、本開示は、前記細胞を操作および生成するための方法および組成物、前記細胞を含有する組成物、ならびにこれらを対象に投与するための方法に関する。一部の局面において、前記細胞および前記方法の特徴は特異性および/または効力を提供する。一部の態様において、前記細胞は、抗原に特異的に結合する遺伝子操作された抗原受容体、例えば、キメラ抗原受容体(CAR)および共刺激受容体を含有する。一部の態様において、前記細胞は、複数種の抗原を標的化する受容体を含む。一部の態様において、前記細胞は、例えば、遺伝子産物をコードする遺伝子の破壊による、1種類または複数種の遺伝子産物の抑制を含む。一部の態様において、操作された抗原受容体が認識する抗原をコードする遺伝子が破壊されて、操作された細胞が標的化される可能性が小さくなる。一部の態様において、操作された抗原受容体は、標的抗原、例えば、疾患特異的(例えば、腫瘍特異的または腫瘍関連)抗原などの、ある抗原を認識してもよく、例えば、ある抗原に特異的に結合してもよく、さらに、別の抗原にも特異的に結合するか、または別の抗原も認識してもよく、別の抗原は、場合によっては、処置されている疾患もしくは状態または標的細胞もしくは標的組織もしくは病原体において発現していないか、あるいは処置されている疾患もしくは状態または標的細胞もしくは標的組織もしくは病原体と関連しないか、あるいは処置されている疾患もしくは状態または標的細胞もしくは標的組織もしくは病原体の表面で、またはその中で特異的に発現も過剰発現もアップレギュレートもされていない。しかしながら、一部のこのような局面では、別の抗原は、疾患特異的抗原、例えば、腫瘍特異的抗原などの第1の抗原のファミリーメンバーでもよく、第1の抗原と構造類似性および/もしくは配列類似性を共有してもよく、ならびに/または第1の抗原とエピトープを共有してもよく、ならびに/あるいは第1の抗原に関連してもよい。従って、一部の態様において、前記受容体は、疾患特異的抗原である第1の抗原に特異的に結合し、かつ処置されている疾患もしくは状態または細胞もしくは組織もしくは生物の疾患特異的抗原でない第2の抗原にも特異的に結合する受容体である。
養子療法用の操作された細胞を生成および投与するために様々な戦略を利用することができる。例えば、遺伝子操作された抗原受容体、例えば、CARを発現する免疫細胞を作製するための戦略、および前記細胞において遺伝子発現を抑止または抑制するための戦略を利用することができる。例えば、このような細胞を用いて処置することができる、もっと広い範囲の標的抗原および疾患を提供するために、前記細胞の特異性または選択性を改善するために、例えば、オフターゲット効果を回避するために、ならびに前記細胞の効力を改善するために、例えば、エフェクター機能の抑止を回避することによって、ならびに対象に投与した際に前記細胞の活性および/または生存を改善することによって前記細胞の効力を改善するために、改善した戦略が必要とされる。このようなニーズを満たす方法、細胞、組成物、キット、およびシステムが提供される。
操作された細胞、例えば、操作された免疫細胞または免疫賦活細胞を含む細胞、ならびに前記細胞を、例えば、養子療法において生成および使用するための方法、ならびに前記細胞を含有する組成物、例えば、薬学的組成物が提供される。前記細胞の中には、単独もしくは集団で安全性、特異性、選択性、および/もしくは効力を改善する、ならびに/または養子細胞療法による、もっと広い範囲の抗原もしくは疾患の標的化を可能にする1つまたは複数の特徴、例えば、二重抗原標的化特徴および/または遺伝子破壊をもつ細胞がある。
I.養子細胞療法において特異性および/または効力をもたらす組成物および方法
養子細胞療法、例えば、養子免疫治療のための細胞が提供される。前記細胞は、T細胞およびNK細胞などの免疫細胞を含み、一般的に、遺伝子操作された抗原受容体、例えば、操作されたTCRおよび/またはキメラ抗原受容体(CAR)を発現する。例えば、多発性骨髄腫(MM)を含むがんを処置する際の養子療法における、前記細胞の方法および使用も提供される。前記細胞を操作、調製、および生成するための方法、前記細胞を含有する組成物、ならびに前記細胞を含むキットおよび装置、ならびに前記細胞を使用、生成、および投与するためのキットおよび装置も提供される。一部の局面において、提供される態様は、抗原特異的養子細胞療法の改善した選択性、特異性、および/もしくは効力を提供する、ならびに/または養子細胞療法を介して標的化される可能性のある疾患、状態、および/もしくは標的抗原の範囲を拡大する。一部の局面において、提供される態様は、投与されている細胞への曝露を改善する。
一部の態様において、前記細胞、例えば、操作された細胞は、真核細胞、例えば、哺乳動物細胞、例えば、ヒト細胞である。一部の態様において、前記細胞は血液、骨髄、リンパ、またはリンパ系臓器に由来し、免疫系の細胞、例えば、自然免疫もしくは適応免疫の細胞、例えば、リンパ球、典型的にはT細胞および/またはNK細胞を含む骨髄系細胞またはリンパ系細胞である。他の例示的な細胞には、幹細胞、例えば、人工多能性幹細胞(iPSC)を含む多分化能性幹細胞および多能性幹細胞が含まれる。一部の局面において、前記細胞はヒト細胞である。前記細胞は、典型的には、初代細胞、例えば、対象から直接単離された、および/または対象から単離され、凍結された細胞である。一部の態様において、前記細胞には、T細胞または他の細胞タイプの1つまたは複数のサブセット、例えば、全T細胞集団、CD4+細胞、CD8+細胞、およびその部分母集団、例えば、機能、活性化状態、成熟度、分化能、増殖、再循環、局在化、および/もしくは持続の能力、抗原特異性、抗原受容体のタイプ、特定の臓器もしくは区画における存在、マーカーもしくはサイトカイン分泌プロファイル、ならびに/または分化の程度によって規定されるT細胞または他の細胞タイプの1つまたは複数のサブセットが含まれる。処置しようとする対象に関して、前記細胞は同種異系および/または自己由来でもよい。前記方法の中には既製の方法が含まれる。一部の局面において、例えば、既製の技術の場合、前記細胞は多能性および/または多分化能性であり、例えば、幹細胞、例えば、人工多能性幹細胞(iPSC)である。一部の態様において、前記方法は、本明細書に記載のように、対象から細胞を単離する工程、細胞を調製、処理、培養、および/または操作する工程、ならびに凍結保存前または凍結保存後に細胞を同じ患者に再導入する工程を含む。
操作用の細胞および細胞を含有する組成物は、典型的には、試料、例えば、生物学的試料、例えば、対象から入手した生物学的試料、または対象から得られた生物学的試料から単離される。一部の態様において、細胞が単離される対象は、特定の疾患もしくは状態があるか、または細胞療法を必要とするか、または細胞療法が投与される対象である。対象は、一部の態様では、哺乳動物、例えば、ヒト、例えば、特定の治療介入、例えば、細胞が単離、処理、および/または操作される養子細胞療法を必要とする対象である。
一部の態様において、前記細胞の単離には、1つまたは複数の調製工程および/または親和性に基づかない細胞分離工程が含まれる。一部の例では、例えば、望ましくない成分を除去するために、望ましい成分を濃縮するために、細胞を溶解するか、または特定の試薬に対して感受性のある細胞を除去するために、1種類または複数種の試薬の存在下で細胞が洗浄、遠心分離、および/またはインキュベートされる。一部の例では、細胞は、密度、粘着性、サイズ、特定の成分に対する感受性および/または耐性などの1つまたは複数の特性に基づいて分離される。
一部の態様において、単離方法には、細胞における、1種類または複数種の特定の分子、例えば、表面マーカー、例えば、表面タンパク質、細胞内マーカー、または核酸の発現または存在に基づく様々な細胞タイプの分離が含まれる。一部の態様において、このようなマーカーに基づいて分離するための任意の公知の方法を使用することができる。一部の態様において、分離は親和性または免疫親和性に基づく分離である。例えば、単離は、一部の局面では、例えば、このようなマーカーに特異的に結合する抗体または結合パートナーとインキュベートし、その後に、一般的に洗浄工程を行い、抗体または結合パートナーに結合している細胞を、抗体または結合パートナーに結合していない細胞から分離することによって、1種類または複数種のマーカー、典型的には細胞表面マーカーの細胞発現または発現レベルに基づいて細胞および細胞集団を分離することを含む。
一部の態様において、調製方法は、単離、インキュベーション、および/または操作の前または後に細胞を凍結、例えば、凍結保存するための工程を含む。一部の態様において、凍結工程およびその後の解凍工程によって、細胞集団の中にある顆粒球が取り除かれ、単球がある程度まで取り除かれる。一部の態様において、例えば、血漿および血小板を取り除く洗浄工程の後に、前記細胞は凍結溶液に懸濁される。様々な任意の公知の凍結溶液およびパラメータを一部の局面で使用することができる。一例は、20%DMSOおよび8%ヒト血清アルブミン(HSA)を含有するPBS、または他の適切な細胞凍結培地を使用することを伴う。次いで、DMSOおよびHSAの最終濃度がそれぞれ10%および4%になるように、これを培地で1:1に希釈する。次いで、細胞は1分につき1°の速度で-80℃まで凍結され、液体窒素貯蔵タンクの気相の中で保管される。
一部の態様において、前記細胞は遺伝子操作の前に、または遺伝子操作に関連してインキュベートおよび/または培養される。インキュベーション工程は、培養、発育、刺激、活性化、および/または増殖を含んでもよい。一部の態様において、前記組成物または細胞は刺激条件または刺激薬剤の存在下でインキュベートされる。このような条件は、遺伝子操作のために、例えば、遺伝子操作された抗原受容体を導入するために、集団内の細胞の増殖(proliferation)、増殖(expansion)、活性化、および/もしくは生存を誘導するように、抗原曝露を模倣するように、ならびに/または細胞を初回刺激(prime)するように設計された条件を含む。
一部の態様において、前記細胞は、1種類または複数種の抗原受容体をコードする、遺伝子操作を介して導入された1つまたは複数の核酸、およびこのような核酸の遺伝子操作された産物を含む。一部の態様において、このような核酸は異種である、すなわち、細胞または細胞から得られた試料に通常は存在せず、例えば、別の生物または細胞から得られたもの、例えば、操作されている細胞、および/またはこのような細胞が得られた生物に普通は見られないものである。一部の態様において、前記核酸は非天然であり、例えば、複数の異なる細胞タイプに由来する様々なドメインをコードする核酸のキメラ組み合わせを含む核酸を含む、天然に見られない核酸である。
一部の態様において、操作された抗原受容体には、活性化CARもしくは刺激CAR、共刺激CAR(WO2014/055668を参照されたい)、および/または阻害CAR(iCAR、Fedorov et al., Sci. Transl. Medicine, 5(215)(December, 2013)を参照されたい)を含むキメラ抗原受容体(CAR)が含まれる。CARは、一般的に、一部の局面ではリンカーおよび/または膜貫通ドメインを介して、1つまたは複数の細胞内シグナル伝達成分と連結した細胞外抗原(またはリガンド)結合ドメインを含む。このような分子は、典型的には、天然抗原受容体を介したシグナル、このような受容体と共刺激受容体の組み合わせを介したシグナル、および/または共刺激受容体だけを介したシグナルを模倣するか、またはこれに似せる。
一部の態様において、遺伝子操作された抗原受容体には、組換えT細胞受容体(TCR)および/または天然T細胞からクローニングしたTCR、および/または天然T細胞からクローニングしたTCRの鎖の対が含まれる。
遺伝子操作された抗原受容体によって標的化される抗原の中には、養子細胞療法を介して標的化される疾患、状態、もしくは細胞タイプの状況において発現している抗原(例えば、養子細胞療法を介して標的化される疾患、状態、もしくは細胞タイプの表面で、またはその中で特異的に発現しているか、養子細胞療法を介して標的化される疾患、状態、もしくは細胞タイプの表面で、またはその中で過剰発現しているか、あるいは養子細胞療法を介して標的化される疾患、状態、もしくは細胞タイプと関連する抗原)がある。一部の態様において、このような抗原は、疾患特異的抗原、疾患特異的標的、および/または疾患特異的標的抗原と呼ばれる。一部の態様において、疾患特異的標的抗原は、非疾患特異的標的抗原とは、エピトープまたは比較的小さな変化の点だけ、例えば、疾患もしくは状態にしか露出しないか、または疾患もしくは状態にしか存在しないエピトープの点だけ異なる。疾患および状態の中には、血液がん、免疫系のがん、例えば、リンパ腫、白血病、および/または骨髄腫、例えば、B、T、および骨髄性の白血病、リンパ腫、ならびに多発性骨髄腫を含むがんおよび腫瘍を含む、増殖性、新生物性、および悪性の疾患および障害がある。

一部の態様において、前記細胞および方法は、例えば、一部の局面では、それぞれが異なる抗原を認識し、一部の局面では、それぞれが異なる細胞内シグナル伝達成分を含む、2種類以上の遺伝子操作された受容体を細胞の表面に発現させるなどの多標的化戦略を含む。前記受容体は、同じまたは異なるプロモーター下で、同じまたは異なるベクターによってコードされ、同じまたは異なる発現マーカーの1つまたは複数の発現と関連付けられてもよい。一部の局面において、多標的化アプローチは、それぞれが、異なる、このような操作された受容体、例えば、異なる抗原を認識する異なる受容体を発現する、複数の細胞を投与することを包含する。一部の局面において、前記細胞は同じ組成物の中にある。他の局面では、前記細胞は異なる組成物の中にあり、これらは同じ時間に、または連続して投与されてもよい。一部の態様において、投与される組成物は、2種類以上の受容体のうちの1つを発現する細胞と、2種類以上の受容体のうちの別の1つを発現する(が、第1の受容体を発現しない)他の細胞、および両受容体を発現する他の細胞を含む。
前記の遺伝子操作された細胞を生成するための方法、核酸、組成物、およびキットも提供される。一部の局面において、遺伝子操作は、遺伝子操作された成分をコードする核酸または細胞に導入するための他の成分、例えば、遺伝子を破壊するタンパク質または核酸をコードする成分の導入を伴う。
提供される免疫細胞の中には、遺伝子操作された抗原受容体の抗原をコードする1つまたは複数の遺伝子の発現、活性、および/または機能が細胞内で抑制された免疫細胞がある。場合によっては、関心対象の特定の抗原は、疾患または状態、例えば、腫瘍またはがんにおいて発現しているが、免疫細胞、例えば、T細胞または活性化T細胞でも発現していることがある抗原でもよい。一部の局面において、免疫細胞による抗原の発現は、例えば、免疫細胞を、その抗原に対する遺伝子操作された抗原受容体を用いて操作する状況では、例えば、養子免疫治療の状況では望ましくない場合がある。従って、一部の態様において、疾患または状態、例えば、腫瘍またはがんにある標的抗原に特異的な遺伝子操作された抗原受容体を含有し、細胞において、その抗原をコードする内因性遺伝子の破壊または抑制も含有する免疫細胞、例えば、T細胞が提供される。
一部の態様において、抑制/破壊される遺伝子は、操作された細胞に対して阻害効果を有するか、または阻害効果に寄与することができる産物、例えば、前記細胞による免疫応答を阻害することができる産物をコードする。例えば、場合によっては、抑制および/または破壊される遺伝子は、T細胞などの免疫細胞の表面で発現しているか、またはT細胞などの免疫細胞の表面で発現するように計画される、遺伝子操作された抗原受容体によって認識される抗原をコードする。
一部の態様において、前記遺伝子の発現、活性、および/または機能の抑制は遺伝子を破壊することによって行われる。一部の局面において、前記遺伝子は、その発現が、遺伝子破壊が無い、または破壊するために導入される成分が無い場合の発現と比較して少なくとも20%、30%、もしくは40%、または約20%、約30%、もしくは約40%、一般的には少なくとも50%、60%、70%、80%、90%、もしくは95%、または約50%、約60%、約70%、約80%、約90%、もしくは約95%低減するように破壊される。
一部の態様において、抑制は、特異的に遺伝子に結合またはハイブリダイズする、DNA標的化分子、例えば、DNA結合タンパク質もしくはDNA結合核酸、またはそれを含有する複合体、化合物、もしくは組成物を用いて成し遂げられる。一部の態様において、DNA標的化分子は、DNA結合ドメイン、例えば、ジンクフィンガータンパク質(ZFP)DNA結合ドメイン、転写アクチベーター様タンパク質(TAL)もしくはTALエフェクター(TALE)DNA結合ドメイン、クリスパーDNA結合ドメイン、またはメガヌクレアーゼに由来するDNA結合ドメインを含む。
一部の態様において、DNA標的化分子は、エフェクタータンパク質、例えば、エンドヌクレアーゼと融合したDNA結合タンパク質、例えば、1種類または複数種のジンクフィンガータンパク質(ZFP)または転写アクチベーター様タンパク質(TAL)を含む。例には、ZFN、TALE、およびTALENが含まれる。Lloyd et al., Fronteirs in Immunology, 4(221), 1-7(2013)を参照されたい。
一部の態様において、DNA標的化分子は、例えば、転写アクチベーター様タンパク質エフェクター(TALE)タンパク質の中にある、天然のまたは操作された(非天然の)転写アクチベーター様タンパク質(TAL)DNA結合ドメインを含む。例えば、その全体が参照により本明細書に組み入れられる、米国特許出願公開第2011030107号を参照されたい。
一部の態様において、抑制は、1種類または複数種のDNA結合核酸を用いて、例えば、RNAガイドエンドヌクレアーゼ(RGEN)を介した破壊、または別のRNAガイドエフェクター分子による他の形の抑制を介した破壊を用いて行われる。例えば、一部の態様において、抑制はクリスパーおよびクリスパー関連(Cas)タンパク質を用いて行われる。Sander and Joung, Nature Biotechnology, 32(4): 347-355を参照されたい。

本明細書で使用する、遺伝子発現の「抑制」とは、細胞内にある対象遺伝子によってコードされる1つまたは複数の遺伝子産物の発現が、抑制が無い場合の遺伝子産物発現レベルと比較して無くなること、または低減することを指す。例示的な遺伝子産物には、その遺伝子によってコードされるmRNAおよびタンパク質産物が含まれる。抑制は場合によっては一過性または可逆的であり、他の場合では恒久的である。切断型産物または非機能的産物が生成される可能性があるという事実にもかかわらず、抑制は、場合によっては、機能的な、または完全長のタンパク質またはmRNAの抑制である。本明細書における一部の態様において、発現に対立するものとして、遺伝子の活性または機能が抑制される。遺伝子抑制は、一般的に、人為的な方法によって、すなわち、化合物、分子、複合体、もしくは組成物を添加もしくは導入することによって、および/または遺伝子の核酸もしくは遺伝子に関連する核酸を、例えば、DNAレベルで破壊することによって誘導される。遺伝子抑制のための例示的な方法には、遺伝子サイレンシング、ノックダウン、ノックアウト、および/または遺伝子破壊法、例えば、遺伝子編集が含まれる。例には、一般的に発現を一過性に低減するアンチセンス技術、例えば、RNAi、siRNA、shRNA、および/またはリボザイム、ならびに例えば、切断および/または相同組換えを誘導することによって、標的化された遺伝子不活化または破壊をもたらす遺伝子編集法が含まれる。

- (c)免疫細胞において第1の抗原および/または第2の抗原の発現の抑制を行う工程をさらに含む、請求項74〜83のいずれか一項記載の方法。
- 抑制を行う工程が、特異的に遺伝子に結合またはハイブリダイズするDNA結合タンパク質またはDNA結合核酸を免疫細胞に導入することを含む、請求項84記載の方法。

- 前記抑制が、操作された免疫細胞における標的抗原の発現を、抑制が無い方法によって生成された操作された細胞と比較して少なくとも50%、60%、70%、80%、90%、または95%低下させる、請求項84〜86のいずれか一項記載の方法。
- 請求項74〜87のいずれか一項記載の方法によって生成された、細胞。
- (a)第1の抗原に特異的に結合しかつ細胞に対して活性化シグナルを誘導することができる、第1の遺伝子操作された抗原受容体;および
(b)第2の抗原に特異的に結合しかつ細胞に対して共刺激シグナルを誘導することができるキメラ共刺激受容体である、第2の遺伝子操作された抗原受容体
