Abstract
Description

皮膚機能、例えば、皮膚バリア機能は、体を物理的損傷及び環境因子から保護すること、皮膚水和を調節すること、体温を調節すること、病原体の侵入に対する防御を提供すること、並びに外観にとって極めて重要である。皮膚が損傷を受けると、効果的なバリアとして機能するその能力が損なわれ、そのため、外因性刺激物及び潜在的病原性生物が対象に侵入することが可能になる。さらに、損傷を受けた皮膚は、体内に存在する水分が、それが蒸発する皮膚の表面に直接移動することができる場合、経表皮水分喪失の増大を可能にし、皮膚水和の減少(例えば、乾燥し、炎症を起こした皮膚)、及び皮膚の弾力性の喪失をもたらすことがある。
特許文献1は、皮膚科的障害などの皮膚バリア機能障害の状態の治療及びレーザー、光線、又は化学物質治療後の管理のための組成物を開示している。しかしながら、そのような組成物は、依然として、所望するほどの耐久性はなく、1日に複数回の適用を必要とする。
したがって、すぐに形成し、かつ薄く、耐久性があり、非侵襲性で、使い勝手がよく、かつ皮膚のような性質を有する、皮膚の機能を修飾するための組成物、装置、及び方法が、依然として必要である。
本発明は、少なくとも一部は、耐久性があり、自然な外見の、非侵襲性の組成物、及びそのような組成物を皮膚バリア機能障害の状態の治療において使用する方法の発見に基づいている。本発明は、1以上の皮膚バリア機能の増強及び/又は再建を可能にするカバー、層、フィルム、装置、又は人工皮膚を形成することができる安全な組成物を提供する。
本明細書に開示されるのは、皮膚上でカバー、層、フィルム、装置、及び/又は人工皮膚を形成することができる組成物であって、低い粘着性を有し、素早く形成され、結果として、装着可能で、快適で(正常で健康な皮膚と同様の温度及び湿度を維持する)、通気性があり、薄く、光学的に不可視で、美容的にエレガントで、可撓性があり、延伸性があり、弾性があり、かつ体の動きに同調し、しかも、長持ちするカバー、層、フィルム、装置、及び/又は人工皮膚を有し、皮膚水和並びに他の治療的、審美的、及び/又は美容的利益を提供するように、快適に装着されることができる、組成物である。
Technology 2000, 6(4):239-244に記載されている手順に準拠して、コルネオメーターを用いて測定することができる。ある実施態様において、該組成物の適用後の皮膚水和は、コルネオメーターの約20任意単位(規格化された水和値)超である。好ましい実施態様において、該組成物の適用後の皮膚水和は、コルネオメーターの約40任意単位超である。他の好ましい実施態様において、該組成物の適用後の皮膚水和は、コルネオメーターの約60任意単位超又はコルネオメーターの約80任意単位超である。
以下の試験法を用いて、流体材料の動的粘度(Pa・s)を、20mmパラレルプレートジオメトリを取り付けたBohlin CVO100レオメーター(Malvern Instruments)を用いて、0.5s-1で決定することができる。同様のレオメーターを粘度測定に使用することができる。試験された各々の材料について、少なくとも3つの試料を測定し、平均粘度及び測定の標準偏差を記録する。
(試験組成物の適用)
年齢、人種、又は性別に関係なく、健康な対象(少なくとも3人)を選択する。試験を室温及び約50%相対湿度で実施する。標準テンプレートをガイドとして用いて、選択された前腕掌側部分に4×4cm2の正方形の輪郭を描く。天秤を用いて、適量(例えば、約0.1g〜約0.3g)の試験組成物(又は二部分組成物の場合、約0.1gの第一の部分及び約0.15gの第二の部分)を秤量舟皿に量り分ける(二部分組成物の場合、混合しない)。試験組成物を、指先を用いて、好ましくは、指サックを着用して、前腕の4×4cm2の正方形に均一に適用する。正方形の全ての部分が組成物によってカバーされていることを確認する。二部分組成物の場合、きれいな指先又は新しい指サックを用いて、第二の部分を第一の部分の上に優しく展延し、第一の部分全体をカバーすべきである。
組成物を該部分に約15分間触れさせないでおく。その後、対象は、日常動作を再開することを認められる。対象は、定型的日常動作のみか、又は労力を要する動作、例えば、運動、水泳、蒸し風呂室、サウナなどを伴う定型的日常動作かのいずれかを行うことを許可される。各々の労力を要する動作の種類及び長さを記録する。試験組成物によって形成される層を、約24時間以上、皮膚の上にそのまま置く。組成物の適用後のある時点で、各々0.5×0.5cm2の8×8の正方格子(合計64個の正方形)を用いて、皮膚上の無傷な部分のパーセンテージを測定することにより、層の耐久性を評価する。4×4cm2の正方形部分の外側の余分な層は評価において考慮されない。各々の正方形は、層の目に見える欠陥、例えば、しわ、落屑、亀裂、及び/又は剥離がない場合にのみ、耐久性があるとみなされる。観察結果を記録する。
この方法は、機械式記録器を用いた有機コーティングのフィルム形成時の乾燥又は硬化の評価法であるASTM D5895-03から改変された。材料及び選択された対象に対する試験組成物の適用は、皮膚上でのフィルム耐久性試験に記載されているのと同じである。本試験は、ヒトの皮膚の代わりに、他の基板に対して、例えば、天然色の牛革ツーリング皮革、ポリウレタン、又はポリプロピレン基板に対して、同等の結果を伴って実施することもできる。試験される各々の組成物について、少なくとも3つの試料を試験し、平均指触乾燥時間、平均不粘着時間、及び測定の標準偏差を記録する。

この接着力試験法は、エラストマージョイントシーラントの剥離接着試験法であるASTM C794に準拠して開発された。延長グリップジオメトリが取り付けられた100Nロードセル(Instron #2519-103)を備えたInstron 3342単一カラム引張/圧縮試験系(Instron, Norwood, MA)を、試験基板としての1/32”の厚さのポリプロピレンシートとともに使用することができる。他の同様の器具及び他の柔らかい可撓性のある試験基板を用いて、剥離力を測定することもできる。材料及び選択された基板に対する試験組成物の適用は、次の通りである: 50-ミクロンのスペーサー(例えば、1層の3Mマジックスコッチテープ)をシートサイズ4.5”×1.5”の基板の上に置き、3.75”×0.75”の開放矩形を形成させ、基板表面を露出させる。試験組成物を基板の上に適用し、その後、スライドガラスをスペーサーの縁に沿って前後に滑らせ、滑らかでかつ均一な試験組成物の層を堆積させる。試験組成物を、室温及び周囲湿度で、3時間、該部分に触れさせないでおく。その後、0.75”の幅のシリコーン接着テープ(Mepitac)をフィルムの上に置いて、ポリプロピレン基板上のフィルム表面を完全にカバーし、測定の準備を整える。試験される各々の材料について、少なくとも3つの試料を測定し、平均剥離力及び測定の標準偏差を記録する。
シリコーンテープでカバーされた試験標本の一方の端を手で一部剥離して、機器の延長グリップジオメトリマウントによる効果的なグリップのために十分量のシリコーンテープでカバーされたフィルムをポリプロピレン基板から引き離す。各々の剥離する側をそれ自体の機器グリップの中に固定する。ストリップがジオメトリと実質的に平行にクランプされていることを確認する。2つの剥離ストリップが互いに完全に分離するまで、1mm/sの速度で伸長試験を実施する。剥離力と時間データを記録する。試料の幅(0.75”又は0.019m)によって規格化された実験の間に機器によって測定される瞬発力(N)を平均化することにより、試料の平均剥離力(N/m)を計算する。
皮膚又はゴムバンド又はパラフィルムなどの基板への試験品の堆積は、体積の減少(歪み)が原因でフィルム内部に残留圧縮応力を生じさせ、これが次に、下部の基板に対する引張応力に変わる。基板の上に堆積したフィルムの合わせた結果を観察し、フィルムの堆積後の基板の表面曲率のレベルに基づいて定量することができる。
Vernier Caliperを用いて、上方に湾曲した試験標本の幅側の端から端までの距離を測定する。端から端までの距離は、不完全な上向きの円を形成する弦長を指し、この場合、該円の対応する半径のその後の計算はコンピュータ計算される。半径値を報告し、その逆数を「曲率」値として報告する。曲率値を用いて、基板上に発生した表面張力を計算する。皮膚などの固有の表面張力を有するもともと湾曲している表面の場合、堆積した最外層によって発生した表面張力の変化は、それに応じて、固有の表面張力を修飾することになる。
これらのサイクル引張残留歪み(即時残留歪み)、サイクル引張ヒステリシス損失エネルギー、引張(ヤング)係数、剪断係数、引張強度/最大応力、破壊歪み、及び破壊靱性の試験方法は、ASTM D638、ASTM D412、ASTM D1876試験ガイドラインに従って、本明細書に開示される標本により適するように開発された。延長グリップジオメトリが取り付けられた100Nロードセル(Instron #2519-103)を備えたInstron 3342単一カラム引張/圧縮試験系(Instron, Norwood, MA)を使用することができる。他の同様の器具を用いて、本明細書で試験される特性を測定することもできる。試験される各々の材料について、少なくとも3つの試料を測定し、平均結果及び測定の標準偏差を記録する。
サイクル引張残留歪み(即時残留歪み)を測定することにより被験材料の弾性を決定するように、サイクル試験を設計する。一般に、材料の弾性が大きいほど、変形後のそのもとの形状への戻りが速くなる。より小さいサイクル引張残留歪みスコアは、より良好な弾性を示す。完全弾性材料では、サイクル引張残留歪み及びサイクル試験の領域はゼロに近づくはずである。
伸長引張試験を用いて、それぞれ、引張係数/ヤング係数及び破壊歪みを測定することにより、材料の剛性及び延伸性/可撓性を決定した。
蒸発水分喪失測定は、皮膚バリア機能の機器による評価を提供する。TEWLプローブによる蒸発測定法は、Groveらの文献、蒸発計及びDermaLab(登録商標) TEWLプローブの比較計量学(Comparative metrology of the evaporimeter and the DermaLab(登録商標) TEWL probe)、Skin Res. & Tech. 1999, 5:1-8及びGroveらの文献、DermaLab(登録商標) TEWLプローブを用いるコンピュータ制御された蒸発測定法(Computerized evaporimetry using the DermaLab(登録商標) TEWL probe)、Skin Res. & Tech. 1999, 5:9-13に完全に記載されている。Pinnagodaによって記載されているServo Med蒸発計の使用のために確立されたガイドライン(Pinnagodaらの文献、経表皮水分喪失(TEWL)測定のガイドライン(Guidelines for transepidermal water loss (TEWL) measurement)、Contact Dermatitis 1990, 22:164-178)は、DermaLab(登録商標) TEWLプローブにも適している。
この試験は、当技術分野で一般に知られている、製造業者による説明書に従って、Minolta CR-400 Chromaメーターを使用する。その後、L*(D65)、a*(D65)、及びb*(D65)の3連の測定値を、試験品の6カ所以上の異なる位置で収集する。
ウイルス透過に基づくバリア保護試験を実施して、血行性病原体の危険から保護することが意図される保護材料のバリア性能を評価する。試験品を、21±5℃及び60±10%相対湿度(%RH)で、最低24時間順化させ、その後、φX174バクテリオファージ懸濁液を用いて、ウイルス透過について試験した。試験の最後に、試験品の観察面を滅菌培地ですすぎ、φX174バクテリオファージの存在についてアッセイした。ウイルス透過法はISO 16604に適合する。3連の読取値を各試験品から取得する。
ニッケルと接触するとピンク色に変わる1%ジメチルグリオキシム及び10%水酸化アンモニウム溶液を含む簡単なスポット試験を用いて、ニッケルをppmレベルで検出することができる。硫酸ニッケル(II)六水和物溶液の0.2M溶液を基板に添加し、両方を試験品によってカバーする。その後、スポット試験溶液を試験物に適用する。ピンク色への変色は、ニッケルが試験品を透過し、色溶液と接触していること、又は色溶液が試験品を透過し、ニッケルと接触していることを示している。対照的に、変色の欠如は、試験品が透過されていないこと及びそのバリア機能が無傷であることを示している。
試験品の存在は、特に、該試験品が、二酸化チタン、酸化亜鉛、アボベンゾン、オクチノキセート、オクトクリレン、ホモサレート、又はオキシベンゾンなどのSPF活性成分を含有するとき、紫外線光の皮膚吸収の低下を助けることができた。

皮膚上での試験組成物P1-030/P2-021(フルカバレッジ着色フィルム)及びP1-028/P2-004(透明着色フィルム)の摩擦に対するインビボフィルム耐性を図5で視覚的に示した。
臨床試験(S16-01)は、正常な皮膚を有する25人の健康なボランティアを基にした。試験組成物を、技術者の指示の下、4つの試験部位(前腕、肘の内側、脚、膝の後ろ)に自分で適用した。各試験部位は、4cm×4cm(〜0.1%BSA)の部分をカバーした。耐久性を、24時間後に残存する試験組成物部分のパーセンテージと12時間で自分で撮影した写真とにより、6時間及び24時間後に視覚的に評価した。さらに、耐久性を、写真を用いて、24時間後の腕での色素排除により追加で評価し、この写真もフィルムのバリア特性を視覚的に示した。結果は図6にも示されている。
P1-017/P2-004〜P1023/P2-004を比較して、インビトロ機械特性を行い、第一の部分における相対的なヒドリド対ビニルのモル比に基づいて順位付けた。

皮膚表面調節のインビボ評価を視覚的に評価し、眼下部分の近く及び笑いじわ部分の近くの顔輪郭の調節に関して図8に示した。写真は、女性試験対象(上)でのP1-031/P2-022(右、眼下及び笑いじわ)及びP1-032/P2-022(左、眼下及び笑いじわ)による並びに男性試験対象でのP1-033/P2-023(右、眼下及び笑いじわ)及びP1-032/P2-023(左、眼下及び笑いじわ)による試験組成物の適用の15分前、15分後、及び6時間後に撮影された。試験組成物は全て、顔輪郭の外観の扁平化を示した。

液体水透過に対するインビボバリア評価を、ワセリン及び対照皮膚と比較した、試験組成物P1-016/P2-004の防水性の実証によって行った。まず、水溶性色素を、試験組成物の局所適用前に、皮膚の3つの部位全てに沈着させた。試験組成物の局所適用後、各皮膚部位を水浴に沈め、その場合、対照皮膚部位の水溶性色素が皮膚から洗い落とされることが観察された。その後、残りの2つの部位を石鹸で洗い、手で徹底的に擦り、その場合、擦った後に、ワセリン部位の水溶性色素も皮膚から洗い落とされることが観察された。皮膚上で水溶性色素が唯一残った部位は、試験組成物P1-016/P2-004により保護された皮膚部位であった。結果を図11に示した。

ニッケル接触に対するインビトロバリア評価を、P1-016/P2-004(右側)と対照(左側、試験組成物なし)とを比較して行った。対照(左)はピンク色への変色を示し、ニッケルとの直接的な接触を示した。P1-016/P2-004を含む試験組成物(右)は変色を示さず、ニッケル接触などの化学物質に対する試験品のバリア保護を示している。結果は図12にも示されている。
P1-026/P2-004と対照ブランク及び一般用医薬品SPF 50スプレー(Banana Boat)とを比較して、UV照射に対するインビトロバリア評価を行った。P1-026/P2-004は、UV照射に対するバリア保護を示したが、SPF 50スプレーほど良好ではなかった。結果は図13にも示されている。
水蒸気透過試験を、有機コーティングフィルムの水蒸気透過についての標準試験法に関するASTM D1653のガイドラインに従って、ただし、10%RH、37Cの条件下で行った。本試験は、どれくらいの水蒸気がフィルムを通過することができるかを測定することにより、10%RH、37Cの設定条件下での、カップの内部のリザーバーからの水分喪失に反映される水分閉塞バリアのレベルを比較するためのものである。結果は図14にも示されている。

臨床試験(S15-28)は、正常な皮膚を有する8人の健康なボランティアを基にした。試験組成物は、技術者によって、アームガードを6時間着用した掌側前腕部位に適用された。P1-016/P2-024を正常皮膚と(乾燥剃処理した)損傷皮膚という2つの異なる試験部位でワセリンと比較した。各試験部位は、4cm×4cm(〜0.1%BSA)の部分をカバーした。TEWLを、試験品の適用の前、並びに2、6、及び24時間後に、蒸発計によって測定した。各TEWL測定の前に、対象を45分間平衡化させた。全ての無傷の皮膚部位についての初期のTEWL報告は、乾燥剃処理した全ての皮膚部位について、平均して16.92g/m2/時であったのとは対照的に、平均して4.85g/m2/時であった。結果は図15に示されている。
死体皮膚に対するフランツ拡散セルによるエクスビボ試験は、フランツ拡散セルシステムを用いて、無傷のヒト死体皮膚に入り、そこを通過する3つの異なる製剤組合せからのステロイド薬「トリアムシノロンアセトニド」の皮膚透過の割合及び程度を決定するためのものであった。
・死体皮膚を冷凍庫から取り出し、バイオセーフティフード内で30分間解凍させておいた。包装を開ける前に、皮膚を完全に解凍した。
・死体皮膚を包装から取り出し、角質層側を上にして、バイオセーフティフードのカウンターに置いた。皮膚をキムワイプで軽く叩いて乾燥させ、その後、新しいPBSを噴霧し、再び軽く叩いて乾燥させた。このプロセスをもう3回繰り返して、皮膚上に存在する残留物を除去した。
・その後、レセプターウェルに、脱気したレセプター液を充填した。テフロンコーティングした撹拌棒を各レセプターウェルに加えた。
・解凍した死体皮膚を調べ、均一な厚さを有し、かつ表面に目に見える損傷のない部分のみを使用した。
・皮膚を〜2cm×2cm角に切った。
・皮膚片を、ドナーセルの中央に、角質層(SC)側を上にして置いた。
・皮膚を再び中央に置き、縁を完全に平らにした。その後、ドナーウェル及びレセプターウェルを並べ、ピンチクランプで一緒にクランプした。
・必要な場合、追加のレセプター液を添加した。存在する気泡は、セルを転倒させることにより除去し、空気を試料ポートに沿って逃がした。
・その後、拡散セルを撹拌式乾燥ブロックヒーターに入れ、レセプター液から20分間再水和させておいた。ブロックヒーターを、連続撹拌しながら、実験全体を通して、32℃±0.5℃で維持した。
・20分後、皮膚の表面を調べた。皮膚が湿っているか又は「発汗」の兆候を示した場合、SCを損傷しているとみなし、破棄した。
・150μlのトリチウム水のアリコート(25μCi水/10ml水をスパイクしたもの)を各FDCドナーウェルに添加した。
・5分後、ドナーウェル由来のトリチウム水を除去し、皮膚を、キムワイプを用いて軽く叩いて乾燥させた。
・トリチウム化されたドナー液を除去した後、レセプターウェルをさらに1時間撹拌した。
・1時間撹拌した後、300μlのアリコート試料を各レセプターウェルから採取した。残りのレセプター液を廃棄し、新しいPBSと交換した(膜完全性試験は、レセプター液中のPBSしか使用しない)。
・600μlのシンチレーションカクテル(Ultima Gold XR)を各試料アリコートに添加した。
・その後、レセプターウェルアリコートのトリチウム含有量を、液体シンチレーションカウンター(LSC)を用いて測定した。
・LSC分析が終了した後、結果を分析した。異常に高い水フラックスを示す任意のFDCを廃棄した。
・その後、FDCを3H水フラックスに従って順位付けした。その後、各製剤がほぼ同等の平均トリチウム水フラックス値を有するFDCに割り当てられるように、FDCを分配した。
・膜完全性点検試験が終了したら、レセプターチャンバーの容積全体をレセプター液と交換した。

フィルム層が存在するため、ステロイドの効力の増強におけるインビボ閉塞利益を評価するための臨床試験は、従来の血管収縮1点アッセイから改良したものであった。本試験は、正常な皮膚を有する37人の健康なボランティア(23人の女性及び14人の男性)を基にした。P1-016/P2-024フィルム閉塞:トリアムシノロンアセトニド(TA)ローション、0.1%−クラス5、プロピオン酸フルチカゾン(FP)ローション、0.05%−クラス5、ヒドロコルチゾン(HC) 2.5%溶液−クラス7の有無によるステロイドの血管収縮転帰を比較して、効力が増加する3つのステロイドを試験するために、試験品は、技術者によって、対象1人当たり6つの試験部位で、掌側前腕部位に適用された。血管収縮の読取りを、16時間での洗い落としなしで、18時間で報告した。
・診断及び主な組入れ基準:血管収縮をすぐに評価することが可能であり、かつ局所コルチコステロイドに対する陽性の皮膚白化応答の既往又は証拠を有する前腕の皮膚を有する18〜65歳の健康な男性又は女性対象。

・安全性:
試験中に報告される有害事象(AE)は全て、経過、重症度、及び転帰を記録して、掲載されることになっていた。
データは、Excelを用いるダブルエントリー法を用いて入力した。統計処理は全て、SAS(登録商標)、バージョン9.4を用いて行った。これは対象内デザインであったので、性別及び人種などの人口学的特性を度数分布としてまとめ、一方、年齢は、平均及び標準偏差としてまとめた。
・試験集団:
無作為に割り付けられ、かつ少なくとも1つの試験品が適用された、本試験に登録された全ての対象を安全性及び有効性の分析に含めた。これは、治療意図(ITT)集団であった。対象が顕著なプロトコル逸脱をしないで試験を終えた場合、彼らをプロトコル適合(PP)集団の有効性分析に含めた。
・有効性分析:
閉塞あり又はなしでの、各試験品についての皮膚白化スコアの合計及び平均を計算した。統計的検定は全て、5%の有意水準(両側)で行った。
一次分析は、視覚的に評価される治療白化スコア平均が互いに等しかったという帰無仮説を検定するものである。これは対象内デザインであったので、視覚的な皮膚白化データを、対象をブロック変数とする無作為化ブロック分散分析(ANOVA)を用いて、治療間の平均差について分析した。
この分析では、実験毎の第I種過誤率を完全帰無仮説の下で5%に制御するライアン-エイノット-ガブリエル-ウェルシュの多重範囲検定(REGWQ)を用いて、平均視覚的評価スコアの対比較を行った。帰無仮説は、治療白化スコア平均が互いに等しいということを述べるものである。
・安全性分析:
試験中に報告されるAEは全て、経過、重症度、及び試験品との関係、並びに転帰を記録して、掲載されることになっていた。報告されるAEは全て、可能であれば治療別に、AEを報告する対象の数、器官別大分類(SOC)、基本語(PT)、重症度、及び試験品との関係別にまとめることになっていた。
結果のまとめ: 37人の対象を登録し、試験で治療した。1人の登録対象(対象01-106)を除く全員が試験を終えた(N=36)。試験に登録したのは、女性24人(64.9%)及び男性13人(35.1%)であった。対象の約86%(32/37)が白人、8.1%(3/37)がアジア人、残り2人の対象がアメリカ先住民/アラスカ先住民(1/37、2.7%)及び白人と黒人の混血(White and Black)/アフリカ系アメリカ人(1/37、2.7%)であった。対象の約3分の2は、ヒスパニック起源でもラテンアメリカ起源でもなかった(25/37、67.6%)。平均年齢は34.2歳であり、対象の年齢は19〜62歳の範囲であった。1人の対象(対象01-106)を除く全員が指定の時間域内で試験部位を洗浄し、フィルムを除去し、血管収縮評価を指定の時間域内で行っていた。対象01-106は、ITT集団に含まれたが、PP集団から除外された。この対象は、血管収縮評価を有さず;したがって、血管収縮評価のまとめに含まれなかった。
・有効性の結果:閉塞ありのプロピオン酸フルチカゾンローション、0.05%(クラス5)と閉塞ありのトリアムシノロンアセトニド、0.1%ローション(クラス5)は、互いに統計的に有意な差がなかったが、閉塞なしのトリアムシノロン及びフルチカゾン、閉塞あり及びなしのヒドロコルチゾン(クラス7)と統計的に有意な差があった。閉塞なしのトリアムシノロンは、閉塞なしのヒドロコルチゾンと同様、他の全ての製品と統計的に有意な差があった。閉塞ありのヒドロコルチゾン及び閉塞なしのフルチカゾンは、互いに統計的に有意な差がなかった。
・安全性の結果: AEを経験した対象はおらず、安全性の理由のために、試験を中断した対象はいなかった。
表78 血管収縮アッセイによるステロイド効力の増強におけるインビボ閉塞利益の臨床評価
ITT集団の対象1人が評価を受けなかったので、ITT及びPP集団についての血管収縮評価の結果は本質的に同じであった。P1-016/P2-004による閉塞ありのクラス5のフルチカゾン及びトリアムシノロン参照ローション製品は、互いに統計的に有意な差がなく、かつ他の全ての参照製品(P1-016/P2-004による閉塞あり又は閉塞なし)と統計的に有意な差があった(より効力が高かった)。閉塞なしのクラス5のトリアムシノロン参照製品は、クラス7の(ヒドロコルチゾン)参照製品(P1-016/P2-004による閉塞あり又は閉塞なし)及び閉塞なしのクラス5の参照製品フルチカゾンと統計的に有意な差があった(より効力が高かった)。この結果は、同一であるフルチカゾンローション製品とトリアムシノロンローション製品の公表されている効力等級(すなわち、クラス5の効力)と若干異なる。しかしながら、そのようなばらつきは、時に、VCAで想定外のものではない。P1-016/P2-004による閉塞ありのクラス7のヒドロコルチゾン参照製品及び閉塞なしのクラス5のフルチカゾン参照製品は、互いに統計的に差がなく、閉塞なしのクラス7の参照製品と統計的に有意な差があった(より効力が高かった)。クラス7のヒドロコルチゾン参照製品は、他の全ての参照製品と統計的に有意な差があった(より効力が低かった)。
本明細書に開示される組成物の適用による特定の皮膚バリア機能障害の状態の管理についての臨床効率の評価が以下に示されている。
特定の皮膚バリア機能障害の状態(例えば、アトピー性皮膚炎、乾癬、湿疹、尋常性魚鱗癬、乾皮症、酒さ)を有する、統計分析に好適な多くの対象(例えば、24〜64人)を試験のために選択する。例えば、アトピー性皮膚炎又は湿疹を有する対象を、Hanifin及びRajkaの文献、アトピー性皮膚炎の診断上の特徴(Diagnostic features of atopic dermatitis)、Acta. Derm Venereol Suppl(Stockh) 1980; 92: 44-47によって提唱された広く受け入れられている基準に基づいて選択する。尋常性魚鱗癬を有する対象を、Williamsらの文献、英国の専門調査委員会のアトピー性皮膚炎の診断基準.III.独立した院内検証(The U.K. Working Party’s Diagnostic Criteria for Atopic Dermatitis. III. Independent hospital validation)、Br J Dermatol 1994; 131(3):406-416に記載されている広く受け入れられている基準に基づいて選択する。傷跡、瘢痕、擦過傷、又は任意の皮膚疾患を有する者は誰であれ、除外されない。
1.あらゆる年齢層の男女(例えば、アトピー性皮膚炎の場合)、6〜70歳の男女(例えば、湿疹、尋常性魚鱗癬の場合)、又は18〜70歳の男女(例えば、乾癬の場合);
2.試験当日のその予約の前の2時間の間、運動すること及び/又は熱いもしくはカフェイン入りの飲料を飲むこと(これは測定に影響を及ぼし得る)を控えることに同意する;
3.全ての試験要件及び制限に従う意志がありかつそれができる;並びに
4.同意書を読み、それを理解し、かつそれに署名することができる。
1.インタビューにより決定されるときに、妊娠しているか、授乳しているか、又は妊娠を計画している;
2.現在、閉経期にある(すなわち、顔面紅潮を経験している);
3.喫煙者である;
4.治験責任医師又はその適法に選定された代表者が考える任意の他の条件又は因子が皮膚応答又は試験結果の解釈に影響を及ぼし得る。
標準的なテンプレートをガイドとして用いて、特定の皮膚疾患(「皮膚病変」)を有する2以上の部分及び正常な外見の皮膚を有する1以上の部分を含む、対象の皮膚に、標準的なテンプレートを用いて2〜6カ所の5cm×5cmの試験部位の輪郭を描く。
全ての試験部分を洗浄して、臨床的測定の前に試験組成物を除去する。除去剤を使用前によく振って均一にする。除去剤(25cm2当たり1.5mL)を綿ラウンドパッドに注ぎ、その後、湿ったパッドを試験部分の上に置いて、試験組成物を除去する。
臨床的測定は、以下の態様のうちの1つ又は複数において実施される。
・疾患の重症度:
o SCORAD又はOSCORAD(アトピー性皮膚炎の客観的スコア、アトピー性皮膚炎に関する欧州作業部会、アトピー性皮膚炎の重症度のスコアリング: SCORAD指数(Severity scoring of atopic dermatitis: the SCORAD index)、Dermatology 1993, 186:23-31)は、スコア範囲0〜103の、アトピー性皮膚炎疾患強度(紅斑/色素沈着、浮腫/集団(population)、湿潤/瘡蓋、表皮剥離、苔蘚化/痒疹/掻痒症、及び乾燥)の6つの臨床的特徴を含む、9の規則を利用している。
o PASI(乾癬領域及び重症度指数、Fredriksson及びPetterssonらの文献、重症乾癬−新しいレチノイドによる経口療法(Severe psoriasis - oral therapy with a new retinoid)、Dermatologica 1978;157:238-44)は、病変のある皮膚表面積と組み合わせた、0±4の尺度での、乾癬性病変:紅斑(赤み)、浸潤(肥厚)、及び落屑(desquamation)(落屑(scaling))という3つの典型的な兆候の定量的評価に基づいている。PASIスコアの基礎は、4つの別々の身体部分:頭、胴体、並びに上肢及び下肢の評価である。これらを、病変のある皮膚表面の程度を確定した後に、紅斑、浸潤、及び落屑について別々にスコアリングするのは、時間のかかることであり、経験豊富な人でも10±15分はかかる場合がある。スコアリング形式の例を図17に示す。PASIスコアは、次のように計算される:

o Revicometer RVM 600: A Courage-Khazaka Reviscometer RVM600(CKelectronic GmbH, Koln, Germany)による乾癬性病変でのCRTT(皮膚共鳴実行時間、Songらの文献、治癒したハンセン病対象における皮膚共鳴実行時間の減少(Decreased cutaneous resonance running time in cured leprosy subjects)、Skin Pharmacol Physiol 2009, 22:218-224及びXinらの文献、皮膚共鳴実行時間は、通常の中国人集団の年齢、身体部位、及び性別によって変わる(Cutaneous resonance running time varies with age, body site and gender ina normal Chinese population)、Skin Res Technol 2010, 16: 413-421)は、前腕の伸筋及び対照としての役割を果たす反対側の非病変部位の乾癬性病変におけるCRRTを測定するために使用される。このプローブでの測定部分は8mmである。そして、音響衝撃波走行距離は、1.77 lJのエネルギーで2mmである。測定は12時の位置で開始され、この位置は、以前に記載されているように、テーブルに置かれた右前腕を用いて決定される。その後、測定を1時間毎の間隔で又は30°毎に時計回りに行う。これらの測定から、0〜6時、1〜7時、2〜8時などの方向にCRRTが提供される。左前腕における1〜7時、2〜8時、3〜9時、4〜10時、及び5〜11時の方向での読取りを、右前腕における、ぞれぞれ、5〜11、4〜10、3〜9、2〜8、1〜7時の方向での読取りと比較する。対象は全員、測定を行う前に、20〜24℃、約50〜55%の相対湿度で、30分間休息を取った。

・角質層の完全性及び密着:テープストリッピングは、TEWLを1時間に20g/m2ずつ増大させるのに必要とされる連続的なD-squameテープストリッピングの数の定量として使用される。
・角質層の厚さ:角質層の厚さは、(低周波数サセプタンスの平方根)/(高周波数アドミッタンス2)として、コルネオメーターにより、低周波数サセプタンス及び高周波数アドミッタンスから計算される。また、角質層の厚さは、従来の免疫組織学的染色によって可視化される。また、角質層の厚さは、様々な皮膚層の厚さの深層(200uM)測定値を測定するための、共焦点タンデム走査顕微鏡(TSM)などの、光学顕微鏡法を用いて測定される。
・皮膚生検/免疫組織化学的染色:パラフィン包埋切片の免疫ペルオキシダーゼ染色は、角質層及び表皮構造、表皮の厚さ、並びに細胞外の層状膜を可視化するために、ChemMate Peroxidase/DABシステム(Dako Cytomation, Hamburg, Germany)を用いて実施される。

・臨床検査:末梢血EOS数(1ml当たりの数100;正常40〜440)、血清LDH(IU l-1;正常119〜229)、全血清IgE(IU ml-1;正常0.0〜400.0)、及びアレルゲン特異的IgE(SRL社, Tokyo, Japan)を測定する。アレルゲン特異的IgEは、ハウスダスト、ダニアレルゲン、草花粉(タンジー(Tancy))、スギ花粉、真菌アレルゲン(カンジダ)、動物の鱗屑、及び食品についての蛍光酵素免疫アッセイによって推定した。特異的IgEの検出感度に関して、100ルミカウント(lumicount)及び100ルミカウント以上の値を陽性(+)とみなす。
oさらに、血清カテリシジン(LL-37)濃度を、酵素免疫アッセイ(Bachem, San Carlos, CA, USA, Leungらの文献、循環LL-37は、小児における湿疹重症度のバイオマーカーである(Circulating LL-37 is a biomarker for eczema severity in children)、J Eur Acad Dermatol Venereol. 2012;26:518-522に記載されている)を用いて測定する。測定前に、試料を90倍希釈する。このアッセイの感度は1ng/mLであった。
・統計分析:単純回帰分析を用いて、角質層の水和、厚さ、又はTEWLとSCORAD又はPASIとの有意な関連も同定する。0.05未満のP-値を有するデータを有意と評価し、0.005未満のP-値を極めて有意と評価する。ウィルコクソンの順位和検定及び単純回帰分析を行って、IgE、LDH、EOS、及びSCORAD又はPASIを含む、様々な生物学的マーカー間の関連又は相関を評価する。
表79:臨床的エンドポイント及びバイオマーカー
本明細書に開示される組成物の適用による湿疹の管理についての臨床効率の評価を以下に記載する。本試験は、単一施設非盲検試験であった。
アトピー性皮膚炎を含む軽度から重度の湿疹を有する18歳以上の10人の対象(女性7人及び男性3人)(グレード2〜4の治験責任医師包括的評価[IGA]疾患重症度)
1.対象は、同意の時点で18歳以上の男性又は非妊娠女性である。
2.妊娠の可能性のある女性(WOCBP)は、資格を取得するために、1回目の来院/ベースライン時に尿妊娠検査(UPT)が陰性でなければならない;閉経後であるか、過去の産科的手術が原因で妊娠することができないか、又は効果的な避妊方法を用いている女性対象。
3.対象は、書面によるインフォームドコンセントを提出する意志があり、かつそれができる。
4.対象は、「湿疹」の臨床的診断を有し、これは、Hanifin及びRajkaの基準に基づくアトピー性皮膚炎(AD)又は治験責任医師の意見による他の形態の湿疹様皮膚炎(例えば、貨幣状湿疹など)を含むものとする。
5.対象は、指定の処置部分に、治験責任医師包括的評価(IGA)によって決定されるような、安定な[3カ月以内の]軽度から重度の(グレード2〜5の)湿疹の臨床的診断を有する。この処置部分は、最低0.5%BSAの活動性疾患を含む。
6.対象は、試験品を指示通りに適用し、試験に関する指示を順守し、試験期間中の全ての再診を約束する意志があり、かつそれができる。
7.治験責任医師の意見によれば、対象は、全般的に健康であり、試験参加により対象を許容できないリスクに曝すか又は試験に参加することにより対象もしくは試験品の評価を損なう疾患状態又は身体的状態が全くない。
1.対象は、試験中に妊娠しているか、授乳期にあるか、又は妊娠を計画している。
2.治験責任医師の意見によれば、対象は、試験品の評価を妨げ得る皮膚病変もしくは皮膚状態を有するか、又は妨害する局所もしくは全身療法の使用を試験中に必要とする。
3.対象は、以下の局所調製物のいずれかを処置部分に対して使用したことがある:
a.限定されないが、1回目の来院/ベースラインの1週間以内の、コルチコステロイド、免疫調節物質(タクロリムス、ピメクロリムスなど)、タール、カルシポトリエンもしくは他のビタミンD調製物、抗ヒスタミン薬(ドキセピン、ジフェンヒドラミンなど)、又は抗生物質を含む、局所(OTC製品を含む)治療。注意:アレルギー性鼻炎の治療用の経口もしくは鼻腔内抗ヒスタミン薬、気管支喘息の治療用の吸入もしくは鼻腔内コルチコステロイド、又は座瘡の治療用の抗生物質の安定(>30日)投与は許容されるが、記録されなければならない。
b. 1回目の来院/ベースラインの4週間以内のレチノイド(タザロテン、アダパレン、及びトレチノインを含む)。
c. 1回目の来院/ベースラインの4週間以内の光線治療(PUVA、UVB、エキシマレーザーなど)、マイクロダーマブレーション、又はケミカルピール。
d.治験責任医師の意見によれば、対象のアトピー性皮膚炎に著しく影響を及ぼし得る、他の局所療法。
4.対象は、以下の全身用薬物のいずれかを使用したことがある:
a. 1回目の来院/ベースラインの1週間以内のコルチコステロイド(筋肉内及び病変内注射を含む)。
b. 1回目の来院/ベースラインの1週間以内の免疫調節物質(ロイコトリエンを含む)又は代謝拮抗薬。
c.座瘡に対する安定投与(3カ月よりも長い使用)を除いて、1回目の来院/ベースラインの1週間以内の経口又は局所抗生物質(OTC又は処方薬)。
d.治験責任医師の意見によれば、対象のアトピー性皮膚炎に著しく影響を及ぼし得る、他の全身療法。
5.対象は、現在、臨床試験用薬物もしくは臨床試験用装置による治療を使用しているか、又は1回目の来院/ベースラインの30日内にそれを使用したことがある。
6.対象は現在、治験に登録されている。
7.対象は、試験品の成分のいずれかに対する感受性の既往を有する(プロトコルの第5.1節を参照)。
8.治験責任医師の意見によれば、対象は、(例えば、アルコール依存症、薬物依存症、精神的無能力のために)試験プロトコルの要件を順守しないことが知られているか又はそれ順守する可能性が低い。
9.対象は現在、皮膚感染を有する。
試験は、5日間で3回の来診並びに15日目(4回目の来院)及び30日目(5回目の来院)の2回の再診からなる。
1.罹患部分を抗微生物石鹸で洗浄し、完全に乾燥させる。
2.清潔かつ乾燥した指先を用いて、P1-016製剤の薄い層を分配し、処置部分に適用する。製品が均一な層となり、厚い部分がないようにする。約4分の1サイズの量のP1-016が1%BSAをカバーするはずである。
3.指先を清潔にする。
4.指先を用いて、P2-004製剤の薄い層を分配し、それを適用する。P2-004をゆっくりと展延して、P1-016部分を完全にカバーする。P1-016層に擦り込むことなく;それが上に均一に置かれるまで単に滑らせる。P1-016と比較して、1〜1.5倍の量のP2-004を使用することができる。約4分の1サイズの量のP2-004が1%BSAをカバーするはずである。
5.フィルムが落ち着く間の少なくとも2分間は、触れたり動かしたりしない。
有効性:有効性を毎回の診察時に処置部分において評価する。
(1)(2〜5回目の来院時、フィルム除去の前に)試験品が処置部分から剥離した:(a)全くない、(b)わずかな量、(c)中程度の量、又は(d)大量;
(2)(2〜5回目の来院時、フィルム除去の前に)試験品が処置部分から剥がれ落ちた:(a)全くない、(b)わずかな量、(c)中程度の量、又は(d)大量;
(3)(2〜5回目の来院時、フィルム除去の前に)試験品がベースライン診察時にフィルムでカバーされたもとの処置部分の(a)0〜25%、(b)25.1〜50%、(c)50.1〜75%、(d)75.1〜85%、又は(e)85.1〜100%を現在カバーしている(2〜5回目の来院、フィルム除去の前);
(4)(2〜5回目の来院時に)フィルム除去プロセスが処置部分の皮膚刺激をもたらした:(a)全くない、(b)わずかな量、(c)中程度の量、又は(d)大量;
(5)(2〜5回目の来院時に)試験品が、治療日の間、平均して:(a)12時間以下又は(b)12〜24時間持続した;注意:この質問は対象により回答されるべきである。
(6)(1〜5m回目の来院時、1回目の来院の適用又はフィルム除去の前に)過去24時間にわたる処置部分の痒みの程度をどのように評価したか: 0=なし、1=わずか、2=軽度; 3=中程度; 4=重度?、注意:この質問は対象により回答されるべきである。

(5)自身の経験に基づいて、ステロイドのような他の外用薬ではなく、この製品を用いて、自身の状態を治療することを検討しますか:はい/いいえ(無料テキスト説明は任意である)[5回目の来院、EOSのみ]。

10人の対象が、活動性疾患の個別の部分(0.5〜3%BSA)にP1-016/P2-004試験組成物を1日に最大2回適用した。女性:70%、男性:30%。平均年齢:31.6歳
・皮膚上での耐久性(%残存): P1-016/P2-004フィルムは平均して24〜48時間の耐久性があり、除去したときの刺激は皆無かそれに近かった。
・安全性:材料の安全上の問題はない
・有効性(治験責任医師包括的評価及びアトピー性皮膚炎の臨床的兆候による):30日間の治療にわたる全体的な疾患及び兆候/症状の著しい改善。臨床的兆候の改善はIGAの改善と同等である。結果は図18(A〜I)にも示されている。
・全体的満足:対象の大多数は治療に満足し、本試験製品を使用及び除去しやすいものと評価した。対象の88.9%が製品に適度に又はとても満足した(30日目)。対象の88.9%が製品を適度に又はとても適用しやすいと感じた(30日目)。対象の88.9%が他の薬剤ではなく、本製品を使用することを検討している(30日目)。
