のイミダゾールピペラジンである。
固体分散体カプセル製剤のバイオアベイラビリティは、従来型錠剤(Coartem(登録商標)に類似する組成/プロセス)及びマイクロエマルジョンと一緒に、雄性ビーグル犬において評価した。

2実施例2の組成物に対応する。
上記の実施例1及び2に記載した2種のSDカプセル変異体(どちらも非晶質形にあるルメファントリンを含有する)の薬物動態は、健常志願者における無作為化オープンラベル順次2部式試験において評価した。第1部では、2種のSDF変異体の相対的バイオアベイラビリティを3群の並行群において絶食条件下で480mgの単回投与後に従来型製剤(120mgの従来型錠剤)と比較した。
全血液試料は、直接静脈穿刺又は前腕静脈内に挿入された留置カニューラのいずれかにより採血した。PK試料は、全用量レベルで全被験者において入手して評価した。ルメファントリンのPK解析のための血液試料は、投与前並びに投与1、2、4、5、6、8、10、12、24、36、48、72、120、168、216及び264時間後に採血した。
サンプルサイズ:第1部:被験者36例(1群当たり12例)は、それに対して少なくとも1.5倍の変化の適正な検出を許容するであろう各群からの完全なデータを有していた。1.5〜5.0倍の変化から観察された比率に対して、一次薬物動態パラメーター比率(対数変換に基づく、AUCinf、AUClast及びCmax)についての推定90%信頼区間は:変動性に関する歴史的データを使用して1.5(1.08、2.09)、2.0(1.44、2.78)、3.0(2.15、4.18)、4.0(2.87、5.57)、5.0(3.59、6.96)である。第2部では、試料サイズを計算する際に、公式の統計的計算を考慮に入れなかった。
従来型錠剤、SDF変異体1及びSDF変異体2として絶食条件下での480mgの用量の単回投与後のルメファントリンの濃度時間プロファイルは、図1に示す。図1に対応する薬物動態パラメーターの要約は、下記の表に示す:
試験デザインは、下記の表に記載する。胃液分泌を刺激するために、各動物(イヌ)は、試験物質投与のおよそ60分前にペンタガストリンの6μg/kgの単回筋肉内注射を受けた。各治療間のウォッシュアウトは、およそ7〜8日間であった。
KAF156についての1母集団PKモデル及びLUM−SDF変異体1についての1母集団PKモデルは、SAEMアルゴリズムを使用して、Monolix Suite 2016R1(Lixoft,Paris,France)の一部としてMonolix 4.4.0において実行された。R−3.2.3におけるmlxR 3.1.0パッケージのsimulx関数を使用してシミュレーションを実施した。最終モデルを使用して、様々な投与レジメンについてのシミュレーションを実施した。様々な併用レジメン下で500時間までのKAF156及びLUM−SDF変異体1の個別PKプロファイルは、最終モデルを使用してシミュレーションした。第6日及び第7日の濃度、第7日にLUM−SDF変異体1について200ng/mLを超える濃度を有した患者の比率、第6日及び第7日にKAF156、Cmax、AUCinfについて58ng/mLを超える濃度を有する患者についての比率についての要約統計量を計算した。所定のLUM−SDF変異体1の用量レベルの成功の可能性は、200ng/mLを超えるルメファントリンの第7日の血漿中濃度を有した、この所定用量を摂取した患者のパーセンテージとして計算した;所定のKAF156用量レベルの成功の可能性は、KAF156の58ng/mLを超える第6日の血漿濃度を有する、この所定用量を摂取した患者のパーセンテージとして計算した。併用治療の成功の可能性は、任意の相乗作用を含まないその成分の独立作用であると仮定して計算した。
本試験は、2段階逐次デザインで構成されている。
およそ325例の男性及び女性の成人/青少年患者(≧12歳及び≧35.0kg)が本試験の第A部に登録されることになる。
第1群:1日間の400mgのKAF156及び960mgのLUM−SDFの1日1回(QD)
第2群:1日間の800mgのKAF156及び960mgのLUM−SDFのQD
第3群:2日間の400mgのKAF156及び960mgのLUM−SDFのQD
第4群:3日間の200mgのKAF156及び480mgのLUM−SDFのQD
第5群:3日間の400mgのKAF156及び480mgのLUM−SDFのQD
第6群:3日間の400mgのKAF156及び960mgのLUM−SDFのQD
第7群:3日間のCoartem(登録商標)、1日2回(BID)(製品ラベルにしたがって投与)。
合併症を有していない熱帯熱マラリア原虫(P.falciparum)マラリアを有するおよそ175例までの小児(2〜12歳未満及び体重10.0kg以上)が2:1の比率で3群のKAF156及びLUM−SDFの用量組み合わせ群並びにコントロール群(各KAF156及びLUM−SDFの用量組み合わせ群については患者2例並びにコントロール群については患者1例)に無作為割り付けされる。
本試験に含めるために適格な患者は、下記の基準全部を満たさなければならない:
第A部:12歳以上で35.0kg以上の体重を有する男性及び女性患者。
第B部:青少年及び成人患者において有効/忍容用量及びレジメンを決定した後に、2歳以上〜12歳未満で10.0kg以上の体重を有する男性及び女性患者が含められる。
ギムザ染色した厚膜及び薄膜による熱帯熱マラリア原虫(P.falciparum)の顕微鏡検査による確証。
スクリーニング時点の寄生虫1000匹超〜150,000匹/μL未満の熱帯熱マラリア原虫(P.falciparum)寄生虫血症。
37.5℃以上の腋窩温若しくは38.3℃以上の口腔/鼓膜/直腸温度;又は以前の24時間中の類似の発熱歴(発熱歴は文書で記録されなければならない)。
出産可能性を有する女性(WOCBP)については陰性の妊娠検査結果。
下記の基準のいずれかを満たす患者は、本試験への登録には適格ではない。
1.混合型マラリア原虫(Plasmodium)感染症。
2.高寄生虫血症だけを特徴とするのでなければ、WHO2015基準による重症マラリアの徴候及び症状。
3.結核を含む活動性感染症。
4.同時熱性疾患(例えば、腸チフス)を有する患者。
5.過去30日間中に5日間以上の各日の機会に5回以上の飲酒として規定されたアルコール乱用/依存症歴又は現在のアルコール乱用/依存症。
6.公知の関連する肝臓疾患、例えば、代償性又は非代償性の慢性肝炎、肝硬変、B型肝炎又はC型肝炎の病歴、過去3カ月間中のB型又はA型肝炎のワクチン接種、公知の胆嚢又は胆管疾患、急性又は慢性膵炎。
7.ヒト免疫不全症ウイルス(HIV)感染症を含む、任意の確証若しくは疑似免疫抑制又は免疫不全状態。
8.重症栄養失調(12歳以上の患者については16.0未満のボディ・マス・インデックス(BMI)、及び12歳未満の小児については70%未満のメジアン正規化WHO参照体重)。
9.本試験に登録される前24時間中に3回超と規定された重度の嘔吐又は1日当たり3回超の水様便であると規定された重症下痢。
10.妊娠中又は養育中(授乳中)の女性。
11.効果的な避妊法を実行する意志のない性的に活発的な患者。
12.投与中及び試験期間中に極めて効果的な避妊法を使用しているのでなければ、生理学的に妊娠可能な全ての女性と規定された、出産が可能である女性。
13.活動性十二指腸潰瘍、潰瘍性大腸炎、クローン病、非ステロイド性抗炎症薬(NSAID)の慢性的(すなわち、2週間超の)使用。
14.修正を必要とする、臨床的に重要な電解質平衡の異常、例えば低カリウム血症、低カルシウム血症又は低マグネシウム血症。
15.貧血(8g/dL未満のヘモグロビンレベル)。
16.薬物の吸収、分布、代謝若しくは排泄を有意に変化させる可能性がある、又は患者が本試験へ参加した場合に患者の安全を脅かす可能性のある任意の外科的若しくは医学的状態。治験責任医師は、患者の病歴及び/又は下記のいずれかの臨床的若しくは検査室証拠:
総ビリルビンのレベルとは無関係に、AST/ALT>2×正常範囲の上限(ULN)
AST/ALT>1.5及び≦2×ULN並びに総ビリルビンが>ULN
AST/ALTのレベルとは無関係に、総ビリルビン>2×ULN
を検討する際に、この決定を下さなければならない。
17.スクリーニング時の静止時QTcF>450ms(男性)、QTcF>460ms(女性)。
18.脱水の非存在下でのクレアチニン>2×ULN。脱水の症例では、クレアチニンは、経口/非経口再水和後に<2×ULNでなければならない。
19.局所的再発若しくは転位の証拠があるかどうかとは無関係に、過去5年間以内に、治療された、又は未治療の、(皮膚局所性基底細胞癌若しくは子宮頸上皮内癌以外の)任意の器官系の悪性腫瘍の病歴。
20.例えば、鎌状赤血球病及び重症の心臓、腎臓又は肝臓障害などの公知の慢性基礎疾患。
21.公知の活動性又は管理されていない甲状腺疾患。
22.経口薬(錠剤形及び/又は液剤形)妨害物質を忍容する能力がないこと。
23.スクリーニングの2カ月間以内に事前に抗マラリア薬療法を受けた患者。
24.スクリーニングの4週間以内に抗マラリア活性を備える任意の抗生物質の使用。
25.登録の5半減期以内、又は30日間以内、又は予測薬物動態作用がベースラインに戻るまでのいずれか長い方の間の、他の試験対象薬の使用。
26.プロトコルによって禁止された医薬品を摂取している患者。
27.任意のマラリアワクチン試験への以前の参加又は任意の他の状況においてマラリアワクチンを摂取していた患者。
Claims (33)
- マラリアを予防する、治療する、又はマラリアに関連する症状を遅延させる、若しくは状態を改善する際に使用するための、2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩であって、前記化合物が、1〜5日間、例えば1〜3日間、約200mg〜約350mgの用量で連日投与される化合物。
- 前記化合物は、1〜3日間、例えば1日又は2日間、約200mgの用量で連日投与される、請求項1に記載の化合物。
- 前記化合物は、1〜3日間、例えば1日又は2日間、約400mgの用量で連日投与される、請求項1に記載の化合物。
- 前記化合物は、2〜5日間、例えば2日又は3日間、約800mgの用量で連日投与される、請求項1に記載の化合物。
- マラリアを予防する、治療する、又はマラリアに関連する症状を遅延させる、若しくは状態を改善する際に使用するための、2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩であって、前記化合物が、第2抗マラリア薬とともに投与される化合物。
- 前記化合物は、1〜5日間、例えば1〜3日間、約200mg〜約1000mgの用量で連日投与される、請求項5に記載の化合物。
- 前記第2抗マラリア薬は、ルメファントリンである、請求項5又は請求項6に記載の化合物。
- ルメファントリンは、固体分散体又はマイクロエマルジョンとして、例えば、固体分散体として調製される、請求項7に記載の化合物。
- 前記ルメファントリンは、約400mg〜1000mgの用量で連日投与される、請求項6〜8のいずれか一項に記載の化合物。
- 前記ルメファントリンは、約400mg〜約500mg、例えば約480mgの用量で連日投与される、請求項6〜8のいずれか一項に記載の化合物。
- 前記ルメファントリンは、約900mg〜約1000mg、例えば約960mgの用量で連日投与される、請求項6〜8のいずれか一項に記載の化合物。
- それを必要とする被験者において、マラリアを予防若しくは治療する、又はマラリアに関連する症状を遅延させる、若しくは状態を改善する方法であって、前記被験者に、1〜5日間、1日1回送達される約200mg〜350mgの2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩を投与する工程を含む方法。
- 前記化合物は、約200mgの用量で投与される、請求項11に記載の方法。
- 前記化合物は、約400mgの用量で連日投与される、請求項11に記載の方法。
- 前記化合物は、約800mgの用量で1日1回投与される、請求項11に記載の方法。
- 前記化合物は、1〜3日間、例えば1日又は2日間、連日投与される、請求項11〜14のいずれか一項に記載の方法。
- それを必要とする被験者において、マラリアを予防若しくは治療する、又はマラリアに関連する症状を遅延させる、若しくは状態を改善する方法であって、前記被験者に、第2抗マラリア薬と併用して、約200mg〜1000mgの2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩を投与する工程を含む方法。
- 前記第2抗マラリア薬は、ルメファントリンである、請求項17に記載の方法。
- 前記ルメファントリンは、約400mg〜1000mgの用量で連日投与される、請求項18に記載の方法。
- 前記ルメファントリンは、約400mg〜約500mg、例えば約480mgの用量で連日投与される、請求項18又は19に記載の方法。
- 前記ルメファントリンは、約900mg〜約1000mg、例えば約960mgの用量で連日投与される、請求項18又は19に記載の方法。
- ルメファントリンは、固体分散体又はマイクロエマルジョン、例えば、固体分散体の形態にある、請求項18〜21のいずれか一項に記載の方法。
- ルメファントリン及び約200mg〜約1000mgの用量の2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩を含む医薬組み合わせ。
- 約200mgの2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩を含む、請求項23に記載の医薬組み合わせ。
- 約400mgの2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩を含む、請求項23に記載の医薬組み合わせ。
- 約800mgの2−アミノ−l−(2−(4−フルオロフェニル)−3−(4−フルオロフェニルアミノ)−8,8−ジメチル−5,6−ジヒドロイミダゾ[1,2−a]ピラジン−7(8H)−イル)エタノン又はその医薬上許容される塩を含む、請求項23に記載の医薬組み合わせ。
- 約400mg〜1000mgのルメファントリンを含む、請求項23〜26のいずれか一項に記載の医薬組み合わせ。
- 約400mg〜約500mgのルメファントリン、例えば約480mgのルメファントリンを含む、請求項23〜26のいずれか一項に記載の医薬組み合わせ。
- 約900mg〜約1000mgのルメファントリン、例えば約960mgのルメファントリンを含む、請求項23〜26のいずれか一項に記載の医薬組み合わせ。
- 前記ルメファントリンは、マイクロエマルジョン又は固体分散体、例えば、固体分散体の形態にある、請求項23〜29のいずれか一項に記載の医薬組み合わせ。
- 連日投与されるべき、請求項23〜30のいずれか一項に記載の医薬組み合わせ。
- 固定用量組み合わせである、請求項23〜31のいずれか一項に記載の医薬組み合わせ。
- マラリアを予防又は治療する際に使用するための、請求項22〜32のいずれか一項に記載の医薬組み合わせ。
