Abstract

Description

単純ヘルペスウイルス2型(HSV−2)は、性器ヘルペスの主な原因である。HSV−2は、性的接触によって伝染される場合が最も多く、ウイルスの感染は、典型的には、生殖器および肛門周辺部における病変の再発性の発症をもたらすと同時に、ウイルス粒子を生殖管内に排出する。ウイルス排出は、病変または他の症状が存在しなくても起こり得る。HSV−2はまた、知覚神経節で潜伏を確立する。HSV−2感染は、感染患者に身体的不快感を与え、性的罹患を引き起こし、さらなる健康上のリスクをもたらす。具体的には、HSV−2に感染した患者は、HIVに感染するリスクが上昇し、HSV−2に感染した妊婦は、最終的にHSV−2を胎児に感染させ得る。免疫不全個体または新生児では、HSV−2感染は、致命的であり得る。現在、HSV−2感染の治癒法が存在しない。

A.免疫原性HSV−2ポリペプチド
表1および/または表2に示されている免疫原性ポリペプチドまたはポリヌクレオチドは、医薬組成物に使用することができる。本発明は、免疫原性ポリペプチドまたはこれらの免疫原性ポリペプチドをコードするポリヌクレオチドと共に医薬キャリアを含む医薬組成物を提供する。HSV−2に対する抗原は、HSV−2に感染した患者からの免疫細胞をスクリーニングすることによって同定することができる。簡単に述べると、HSV−2抗原のライブラリーは、細菌によって発現され、抗原提示細胞(APC)と混合された。次いで、APCが、HSV−2由来ポリペプチドをプロセシングして、HSV−2に感染したヒト患者から単離されたリンパ球に提示した。患者は、いくつかの集団:(1)HSV−2に曝露されたが感染に対して血清陰性、(2)HSV−2に感染したが無症状、(3)HSV−2に感染し、低頻度で発症する、(4)HSV−2に感染し、頻繁に発症する、(5)未感作、(6)HSV−2(HSV−2−)に対して血清陰性であるが、HSV−1(HSV−1+)に対しては血清反応陽性、に属した。各集団のリンパ球応答を、HSV−2由来ポリペプチドに対する反応性について比較し、スクリーニングにより、他の患者集団と比較してある患者集団でより頻繁に反応性リンパ球を誘導した抗原を検出した。感染したが無症状である患者および曝露されたが血清陰性である患者は、頻繁に発症する患者が活性化できない防御免疫応答を活性化させることができ;特に、曝露されたが血清陰性である患者は、HSV−2感染に対する殺菌免疫を開始すると推測される。ポリペプチドの固有のセットが、これらの患者集団のリンパ球を活性化すると考えられる。したがって、本発明は、感染したが無症状である患者または曝露されたが血清陰性である患者のリンパ球を活性化する特定のHSV−2ポリペプチドの組成物、またはHSV−2による感染を抑制または妨げるこれらのポリペプチドの組み合わせも企図する。

4.US6によってコードされる糖タンパク質D−2(配列番号118)およびUS6ΔTMRによってコードされる内部欠失糖タンパク質D−2(配列番号4)

本明細書に記載されるワクチンは、HSV−1および特にHSV−2を含むヘルペスの予防および/または治療処置に使用することができる。ワクチン接種を受ける被験体は、男性でも女性でも良く、子供でも成人でも良い。一部の実施形態では、処置される被験体はヒトである。他の実施形態では、被験体は、非ヒト動物である。
予防の実施形態では、HSV−2ワクチンを、HSV−2の確立の防止に役立ち得る免疫応答を誘導するために被験体に投与する。
治療への適用では、本発明のポリペプチドまたは核酸を含むワクチンを、HSV−2に罹患している患者に患者の処置に十分な量投与することができる。患者の処置は、この場合、感染した個体におけるHSV−2の症状を遅延させる、または軽減することが該当し得る。一部の実施形態では、患者の処置は、病変の期間を低減すること、病変の数を低減すること、1回の発症における症状の期間を低減すること、および/またはそれら以外の、1回の発症における症状の強さを低減することが該当する。ある実施形態では、ワクチンは、軽度の症状の期間または重症度を低減し;ある実施形態では、ワクチンは、重度の症状の期間または重症度を低減する。一部の実施形態では、ワクチンは、ウイルス排出を低減し、これによりワクチン接種された患者からのHSV−2の伝染性を低減する。ある実施形態では、上記の低減は、少なくとも25%、30%、40%、50%、60%、70%、80%、または実に90%である。ある実施形態では、上記の低減は、症状、ウイルス排出、および/または将来の発症の完全な阻止を含む(例えば、知覚神経節での潜伏を確立するウイルスの能力を阻害することによって)。
本明細書に開示されるワクチンでのワクチン接種の有効性を多数の方式で決定することができる。
1.HSV感染に対する防御
本開示の医薬組成物は、HSV−2に対して免疫応答を惹起するようにデザインされている。本明細書に記載される組成物は、これらの組成物が投与された被験体において、先天性免疫応答、抗体応答、細胞性免疫応答、またはこれらの応答の組み合わせを刺激し得る。一部の実施形態では、本組成物は、好中球、マクロファージ、およびNK細胞などの免疫細胞を、感染の末梢部位または知覚神経節で刺激する。組成物は、マクロファージによる浸潤;好中球による一酸化窒素、TNF−α、インターフェロン(IFN)、およびインターロイキン12(IL−12)などを含む抗ウイルス化合物の産生;および/またはNK細胞の刺激によるIFN−γの産生を刺激することができる。IL−2、IFN−α、およびIFN−βの産生は、本組成物のポリペプチドによっても開始させることでき、感染の制御に役立つと考えられる。
本出願は、患者におけるHSV−2の検出のための、とりわけ迅速かつ安価で精度の高い特殊な方法を提供する。これに関連して、病院および医師が、HSV−2に感染している、または感染のリスクのある患者を検査し処置することが有用なはずである。本明細書で使用される場合、「患者」は、HSV−2に感染しているか、またはHSV−2に感染する可能性のある個体(例えば、ヒト)を指す。
本質的にあらゆる個体は、HSV−2に対してある感染リスクがある。しかしながら、ある亜母集団は、感染リスクが高い。一部の実施形態では、HSV−2に対する組成物が投与される患者は、免疫不全である。
1.投与量およびタイミング
各ワクチン投与量における抗原の量は、単回投与または複数回投与で、上記のように予防または治療応答を誘導する有効量として選択される。好ましくは、この投与量は、典型的なワクチン接種で有意な有害副作用がない。このような量は、どの特異的抗原が利用されるかによって異なる。一般に、投与量は、1〜1,000μg、場合によっては、2〜100μg、例えば、4〜40μgのタンパク質を含むと思われる。一定のワクチンの最適な量は、抗体力価、T細胞の活性化レベル、および被験体における他の応答の観察を含む標準的な研究によって確定することができる。一部の実施形態では、送達される抗原の適切な量は、被験体の年齢、体重、および健康状態(例えば、免疫不全状態など)によって異なる。存在する場合は、アジュバントは、典型的には、1回の投与量に付き1μg〜250μg、例えば、50〜150μg、75〜125μg、または100μgの量で存在する。
本明細書に記載されるワクチン製剤および医薬組成物は、個体への投与、典型的には、全身投与(例えば、静脈内、腹腔内、筋肉内、皮内、皮下、経皮、真皮下、頭蓋内、鼻腔内、粘膜、肛門、膣、経口、舌下、頬経路、または吸引され得る)によって送達することができ、または局所適用によって投与することができる。
ワクチン製剤は、ヒト患者に投与するのに適し得、ワクチン調製物は、USFDAのガイドラインに適合し得る。一部の実施形態では、ワクチン製剤は、非ヒト動物に投与するのに適している。一部の実施形態では、ワクチンは、エンドトキシンもエキソトキシンも実質的に含まない。エンドトキシンは、リポ多糖(LPS)分子などの発熱物質を含む。ワクチンは、不活性なタンパク質断片も実質的に含み得ない。一部の実施形態では、ワクチンは、工業用水、水道水、または蒸留水よりも低いレベルの発熱物質を有する。他のワクチン成分を、当技術分野で周知の方法、例えば、イオン交換クロマトグラフィー、限外ろ過、または蒸留などで精製することができる。他の実施形態では、発熱物質は、患者に投与する前に不活化または破壊することができる。ワクチンの原料、例えば、水、緩衝剤、塩、および他の化学物質を、スクリーニングして脱発熱化することもできる。ワクチン中のすべての物質を滅菌して、ワクチンの各ロットを滅菌性について試験することができる。したがって、ある実施形態では、ワクチン中のエンドトキシンのレベルが、USFDAによって規定されたレベル、例えば、くも膜下注入組成物の場合は、製品1kg当たり0.2エンドトキシン(EU)、非くも膜下注入組成物の場合は、製品1kg当たり5EU、そして滅菌水が1mL当たり0.25〜0.5EUよりも低くなる。
本明細書に記載されるHSV−2ワクチンは、様々な技術を用いて生産することができる。例えば、ポリペプチドを、組換えDNA技術を用いて適切な宿主細胞で産生させることができる。適切な宿主細胞は、細菌、酵母、哺乳動物、または他の種類の細胞とすることができる。宿主細胞は、関連ポリペプチド遺伝子の1つの外来性コピーを発現するように改変することができる。典型的には、遺伝子は、強いプロモーターおよびポリアデニル化配列などの適切な制御配列に機能的に連結される。一部の実施形態では、プロモーターは、誘導性または抑制性である。他の制御配列は、ポリペプチドをどのように精製したいかによって、細胞質または膜における目的のポリペプチドの分泌もしくは排出または目的のポリペプチドの保持を行うことができる。遺伝子は、染色体外プラスミドに存在しても良いし、または宿主ゲノムに組み込まれても良い。当業者であれば、天然配列と100%同一の核酸を使用する必要はないことを理解できよう。むしろ、これらの配列に対するある程度の改変は許容され、望ましい場合もある。例えば、核酸を変更して、コードされたポリペプチドが同じままであるように遺伝子コードの縮重を利用することができる。一部の実施形態では、遺伝子は、特定の宿主における発現を改善するためにコドンが最適化される。核酸は、例えば、PCRまたは化学合成によって作製することができる。
実施例1、HSV−2抗原の同定
HSV−2抗原のライブラリー(HSV−2 Strain G、ロット番号7C0013、Advanced Biotechnologies Inc、Maryland)を用意し、ヒトドナーからの末梢血単核細胞(PBMC)でスクリーニングした。簡単に述べると、HSV抗原のライブラリーを細菌によって発現させ、抗原提示細胞(APC)と混合した。次いで、APCが、HSV−2に感染したヒト患者から単離されたリンパ球にHSV由来ペプチドを提示した。患者は、後述されるように、いくつかの集団に属した。各集団からのリンパ球の応答を、発現された各タンパク質に対する反応性について比較し、スクリーニングにより、他の患者集団と比較してある患者集団でより頻繁に反応性リンパ球を誘導した抗原を検出した。感染したが無症状の患者および曝露されても血清陰性の患者は、頻繁に発症した患者が活性化しない防御免疫応答を活性化することができる;特に、曝露されたが血清陰性の患者は、HSV−2感染に対して殺菌免疫を開始したと推測される。ポリペプチドの固有のセットが、これらの患者集団からのリンパ球を活性化させると考えられる。
リンパ球を、いくつかの集団:無症状(n=40)、曝露(n=40)、1年に4回以上発症した高頻度再発者(n=43)、1年に4回未満しか発症しない低頻度再発者(n=19)、未感作(n=10)、およびHSV−2−/HSV−1+(n=10)に属する患者から単離した。表3は、1年に2回以上の再発者と比較した、曝露された患者コホートにおけるUL10、UL19、UL40、US4、US6、RS1(RS1.1、RS1.2、RS1.3)、UL36(UL36.3、UL36.4、UL36.5)、UL32、およびRL2によってコードされる13のHSV−2抗原についての頻度分析を示す。
B.UL1、UL49.5、およびUL54によってコードされる抗原の同定
リンパ球を、いくつかの集団:無症状(n=40)、曝露(n=40)、1年に4回以上発症した高頻度再発者(n=43)、1年に4回未満しか発症しない低頻度再発者(n=19)、未感作(n=10)、およびHSV−2−/HSV−1+(n=10)に属する患者から単離した。
C.RL1、UI1、およびUL11によってコードされる抗原の同定
リンパ球を、いくつかの集団:無症状(n=40)、曝露(n=40)、1年に4回以上発症した高頻度再発者(n=43)、1年に4回未満しか発症しない低頻度再発者(n=19)、未感作(n=10)、およびHSV−2−/HSV−1+(n=10)に属する患者から単離した。
実施例2.in vivoでのデータ
A.[プロトコルA]モルモット治療ワクチン接種プロトコル
雌ハートレイ系モルモットに、生殖器感染を確立するために5×105pfuでHSV−2株MSを経膣的に曝露した。動物を、感染後第1日目に膣用スワブによって感染を監視し、感染後第3日目〜第14日目に急性疾患を観察した。第14日目に、原発性疾患の回復後、動物を12の群に無作為に分け、抗原(15μgの用量のHSV−2ポリペプチド)とアジュバント(50μgの用量のISCOMマトリックスとQuillajaサポニン画分AとCが91:9の混合物)を用いて皮下免疫した。各群に、感染後第14日目、第21日目、および第34日目に合計3回のワクチン接種を行った。生殖器用スワブをワクチン接種期間中に回収して、ウイルス排出を監視し、観察結果を毎日記録した。症状を、重症度に基づいた0〜4のスコア:0=無症状、1=赤みまたは腫れ、2=少数の小水疱、3=いくつかの大きな小水泡、4=浸軟を伴ういくつかの大きな小水泡、でスコアを付けた。加えて、症状が上記のスコア間の中間の病変をもつ動物には、0.5、1.5、2.5、または3.5のスコアを付けた。
研究の結果が、以下の表6〜表10に示されている。IgG力価を、各群の12匹の動物のうちの4匹の平均を用いて、感染後第41日目および第3の免疫化の7日後に決定した。平均再発病変スコアおよび平均病変日数をそれぞれ、感染後第15日目〜第63日目に決定した。病変スコアは、第15日目〜第60日目の各群に対する合計病変数を表し、平均を計算した。平均病変日数は、免疫化または非免疫化動物にヘルペス性病変が現れるまでの感染してからの平均日数である。膣用スワブサンプルを、感染後第20日目〜第59日目の間の12日間にすべての動物から回収し、量的リアルタイムPCRによるウイルス排出力価についてのアッセイまで−80℃で保存した。
B.[プロトコルB]マウスの予防ワクチン接種のプロトコル
第0日目および第9日目に、6〜8週齢の雌C57BL/6マウスを、抗原(HSV−2ポリペプチド)とアジュバント(12μgの用量のISCOMマトリックスとQuillajaサポニン画分AとCが82:18の混合物)を用いて皮下免疫した。第11日目に、発情周期をデポプロベラと同期させ、次いで第16日目に、マウスに、麻酔下でLD50値の10倍量のHSV−2株333を膣内沈着により曝露した。すべての動物を、罹患率(臨床スコア)、死亡率、および体重について監視し、膣用スワブを感染後第17日目〜第28日目の間に回収した。臨床スコアを、次のスコア:0=無症状、1=膣の紅斑、2=膣の紅斑および浮腫、3=膣のヘルペス性病変、4=片側性麻痺または重度の陰部潰瘍、および5=両側性麻痺または死亡、を用いて記録した。
実験群では、第0日目および第9日目に、マウスを、5μgまたは10μgの抗原とアジュバント(12μgの用量のISCOMマトリックスとQuillajaサポニン画分AとCが82:18の混合物)を用いて皮下免疫した。コントロール動物に、リン酸緩衝生理食塩水(PBS)のみまたはアジュバントのみを投与した。
C.[プロトコルC]モルモットの予防ワクチン接種のプロトコル
250〜350g(体重)の雌ハートレイ系モルモットに、第0日目および第14日目〜第21日目に、15μgの抗原とアジュバント(50μgの用量のISCOMマトリックスとQuillajaサポニン画分AとCが91:9の混合物)を用いて皮下免疫した。ブーストワクチン接種の2〜3週間後に足の爪クリップで血清を採取し、次いでモルモットに、膣沈着により5×105PFUのHSV−2株MSを曝露した。膣用スワブサンプルを、第30日目および第32日目にすべての動物から回収し、量的リアルタイムPCRによるウイルス力価についてのアッセイまで−80℃で保存した。モルモットは、毎日(第1日目〜第14日目)評価し、原発性性器皮膚疾患を、1〜4の病変重症度スコアで表した。数字スコアを、次の通り特定の疾患症状に割り当てた:0、無疾患;1、赤みまたは腫れ;2、少数の小水疱;3、いくつかの大きな水泡;4、浸軟を伴ういくつかの大きな水泡。研究の最後で、モルモットを安楽死させ、後根神経節(DRG)を採取し、量的リアルタイムPCR分析のための処理まで−80℃で保存した。
D.[プロトコルD]免疫原性アッセイI(標準)
マウスを、生理食塩水に溶解した5μgの抗原とアジュバント(12μgの用量のISCOMマトリックスとQuillajaサポニン画分AとCが82:18の混合物)を100μl、首根に注射して皮下免疫した。マウスに、1回または2回(7日間空けて)注射した。注射の免疫原性の分析を、最後の注射から7日後に行った。
免疫原性アッセイIは、ICP4.2を用いた1回および2回の注射療法の両方で強い免疫原性応答を示した。1回の注射療法の場合は、200,000のT細胞当たりのIFN−γスポットの数は、CD4+およびCD8+細胞のそれぞれで8および101であった。2回の注射療法の場合は、スポットの数がそれぞれ50および70であった。対照的に、15未満のスポットが、培地またはアジュバントのみで、CD4+またはCD8+細胞で観察された。
免疫原性アッセイIの結果が、図1Aおよび図1Bに示されている。強いCD4+およびCD8+T細胞応答が、完全長gD2およびgD2ΔTMRの両方で得られた。対照的に、膜貫通ドメイン(図1に306tとして示されている)のすぐ上流で切断されたgD2抗原は、有意に低い応答を示した。
HSV−2 orfeomeのGenocea所有のライブラリーからの組換え大腸菌を、gL2またはICP4タンパク質の断片(ICP4.2、ならびにRS1.1、RS1.3.1、およびRS1.3.2によってコードされるポリペプチド)を発現するように作製した。タンパク質は、細菌細胞内で維持される。次いで、細菌をPFAで固定し、PBSで広範囲に洗浄し、免疫化に使用するまで−80℃で保存した。
F.[プロトコルF]免疫原性アッセイIII(重複ペプチドのプール)
マウスを、gL2、ICP4、ならびにRS1.3.1およびRS1.3.2によってコードされるICP4断片の全配列に跨る重複ペプチドのプール(OLP)を2μg/マウスで免疫化した。OLPは、1マウス当たり100μlの総量にTiterMaxアジュバント(Alexis Biochemical)と配合し、アジュバントは、皮下投与量の1/3を占めた。マウスを、第0日目に免疫化し、第6日目に追加免疫し、脾臓および血液を第11日目に採取した。1つの細胞懸濁液を脾臓から調製し、赤血球を溶解した。次いで、脾細胞懸濁液を半分に分けた。初めの半分を、抗原提示細胞、CD4+T細胞、およびCD8+T細胞に分け;IFN−γELISPOTプレートの1ウェルに付き200,000のT細胞を播種し、100,000のAPCと、免疫化、無関係のペプチド、陽性コントロール、および陰性コントロールに対応するOLPプールとで刺激した。細胞を、プレートで一晩インキュベートしてから、プレートを展開し、ウェルごとにスポットをカウントした。各脾細胞懸濁液の次の半分を、分けられていない脾細胞(400,000/ウェル)として行い、ペプチドでパルス処理し、上記のようにアッセイした。
実施例1.C57BL/6マウスにおけるgD2ΔTMRおよびICP4またはICP4.2の免疫原性
精製タンパク質をアジュバントと混合し、未感作マウスを免疫化して、CD4+およびCD8+T細胞をタンパク質抗原に対して応答させる能力を評価した。簡単に述べると、抗原のみ(gD2ΔTMR(5μg))または抗原の組み合わせ(gD2ΔTMRおよびICP4.2(10μg))をアジュバント(12μgの用量のISCOMマトリックスとQuillajaサポニン画分AとCが82:18の混合物)と混合し、9日間の間隔で2回、マウスに皮下投与した。第2の免疫化から7日後に、マウスを安楽死させ、ex vivoでのIFNγELISPOTアッセイのために脾臓を回収した。CD4+およびCD8+T細胞を、抗体被覆磁気ビーズを用いて脾細胞集団から選別し、次いで、特異的または非特異的抗原(上記)でパルス処理されてX線照射器でX線照射された未感作脾細胞と共に、96ウェルプレート内のIFNγ特異的抗体被覆膜上で共培養した。インキュベーションから18時間後に、捕捉したIFNγをビオチン化二次IFNγ特異的抗体で検出し、西洋わさびペルオキシダーゼおよび3−アミノ−9−エチルカルバゾール基質で可視化した。データは、各群に付き3匹のマウスの、2×105のT細胞当たりのIFN−γスポット形成単位数±標準偏差として記録した。図3は、各群に付き3匹のマウスの、2×105のCD4+またはCD8+T細胞当たりのIFN−γスポット形成単位数±標準偏差を示す。図3Aおよび図3Bに示されているように、CD4+T細胞またはCD8+T細胞当たりのIFN−γスポット形成単位数は、gD2ΔTMR抗原のみと比較すると、ICP4.2と組み合わせたgD2ΔTMR抗原で免疫化されたマウスで増加した。
gD2と組み合わせたICP4.2タンパク質とアジュバントでの免疫化後に防御免疫を引き起こす能力を、致死HSV−2曝露マウスモデルで評価した。簡単に述べると、1群に付き8匹のC57BL/6マウスを、gD2(2μg)またはICP4.2(10μg)とアジュバントで個々に、または両方の抗原を互いに混合したものとアジュバントで免疫化した。製剤を、9日の間隔で2回、首根に皮下投与した。ウイルス感染の5日前に発情周期をデポプロベラと同期させ、二次免疫化の7日後に動物に、LD50値の20倍量のHSV−2株333を経膣的に曝露した。疾患の症状を、感染後にスコアを付け、生存動物を監視した。疾患の重症度のスコアは、次の通りとした:0=無症状、1=赤み、2=赤みおよび腫れ、3=ヘルペス性病変、4=重度の潰瘍または片側性麻痺、および5=両側性麻痺または死亡。
実施例3.gD2ΔTMRとICP4.2の組み合わせとアジュバントの免疫化によるマウスにおける疾患の症状および死亡率の低減
gD2ΔTMRとICP4.2抗原の組み合わせでのマウスの免疫化は、個々の抗原とアジュバントが曝露された動物と比較して、ウイルス曝露から10日後の平均疾患スコアが低いことを示した。
実施例4.gD2とICP4.2の組み合わせとアジュバントでの免疫化による、HSV−2感染モルモットに治療として投与したときの再発病変の重症度の軽減
抗原とアジュバントでの治療免疫後の感染モルモットでHSV−2を再活性化する能力を評価した。簡単に述べると、モルモットを、5×105pfuのHSV−2株MSで経膣的に感染させ、14日間にわたって原発性疾患を監視し、次いで無作為に免疫化群(n=15)に分けた。動物を、抗原(15μg)とアジュバント(50μg)またはアジュバントのみ、または賦形剤コントロールでの感染後第14日目、第21日目、および第35日目の3回、皮下免疫し、局所疾患の重症度について毎日スコアを付けた。スコア付けの方法は、次の通りとした:0=無症状、1=赤み、2=1つの病変、3=大きい病変または融合病変、4=重度の潰瘍または片側性麻痺、および5=両側性麻痺または死亡。
