図13Aおよび13Bに示すように、パッド部品10は、救助者に対向するその上面に、CPRを行なう間の救助者に対する取っ手195を与える特徴を取り入れてもよい。取っ手は、織物ループ(図13B)またはより硬質のポリマー部材(図13A)の形態を取ることができる。織物は、パッド10に、縫い付けることもできるし、または粘着剤もしくは超音波ボンディングによって接着することもできる(図13B)。またポリマー製の取っ手を、パッドに、粘着剤または超音波ボンディングによって結合することができる(図13A)。胸部圧迫の減圧段階中に胸に対して圧力を維持すると、胸部圧迫の有効性が著しく減少することが、研究によって示されている。取っ手195は、救助者に、減圧段階中に少なくともわずかに引き上げる気にさせる。電極パッドの粘着剤ゲル、または他の粘着剤は、圧縮中に救助者の手が置かれる領域の下で延びることができる。その結果、救助者が減圧段階中に取っ手を引く間の、皮膚に対するパッドの接着が得られる。減圧段階の間に胸を引き上げると、負の胸腔内圧を高め、静脈還流を増加させ、したがって胸部圧迫の間に血流を増加させることが、分かっている。
あるいは、非線形の回帰を行なって、以下のような乗法項を可能にしてもよい。
0.24STE+0.12PAL+0.54(STE*PAL)=RISK
乗法項によって、PALが存在する場合のSTEの重要性が高まる。
RCRR=(R−R前VPB−R−R後VPB)/R−R前VPB、および
(2)R−R間隔が、それの後VPBの減少を受けている間のR−R間隔の変化の勾配(SRR)。RCRRが負でなく、また勾配SRRの急峻さが−2ms/R−R間隔以下である場合には、患者は危険な状態にあると考えられる。あるいは、個々の計算をSTEおよびPALとともに含めて、積分された測定値ベクトルを形成してもよい。これについては、以前の段落で前述した通りである。他の信号解析アルゴリズムでは、周波数ドメイン、ウェーブレット・ドメインにおける心拍数の変動性の解析を取り入れてもよいし、または非線形力学をベースとした方法を用いてもよい。
Claims (21)
- 患者を蘇生するための蘇生システムであって、
患者が長時間着用するように構成された少なくとも2つの除細動電極と、
患者のECGをモニタリングするための回路と。
患者のECGを分析すること、および電気治療を前記電極に送出すべきかどうかを判定することを行うように構成される少なくとも一つのプロセッサと、
前記除細動電極に除細動パルスを供給する回路と、
蘇生の注意換気を提供する回路と
を備える蘇生システム。 - 蘇生の注意換気を提供する前記回路は、第1の電気ユニット内に設けられ、前記除細動電極に除細動パルスを供給する前記回路は、前記第1の電気ユニットとは別の第2の電気ユニット内に設けられ、システムは更に前記第1と第2の電気ユニットと間の少なくとも一つの電気的な接続を提供する回路を備える、請求項1に記載の蘇生システム。
- 前記少なくとも2つの電極および前記第1の電気ユニットが、除細動電極パッド部品内に組み込まれている、請求項2に記載の蘇生システム。
- 前記第1の電気ユニットは、前記第2の電気ユニットに電気的に接続されることなく機能しおよび蘇生の注意喚起を与えることが可能である、請求項2に記載の蘇生システム。
- 蘇生の注意喚起に、CPRの注意喚起が含まれる、請求項2に記載の蘇生システム。
- 前記第1の電気ユニットと前記第2のユニットとの間の少なくとも一つの電気的な接続を提供する回路は、少なくとも一つのケーブルまたは少なくとも一つの無線接続を備える、請求項2に記載の蘇生システム。
- 前記第2の電気ユニットは、前記除細動電極に、電極に除細動パルスを伝える一つまたは複数のケーブルによって直接接続されている、請求項2に記載の蘇生システム。
- 前記第1の電気ユニットと前記第2の電気ユニットとの間の少なくとも一つの電気的な接続を提供する回路は、前記第1の電気ユニットに除細動パルスを送出するための少なくとも一つのケーブルを備え、除細動パルスは、前記第1の電気ユニットから前記電極へ送出される、請求項2に記載の蘇生システム。
- ECG信号は、前記除細動電極を用いて患者から検出される、請求項2に記載の蘇生システム。
- 前記第1の電気ユニットは、蘇生の注意喚起を与えるためのスピーカを備える、請求項2に記載の蘇生システム。
- 蘇生の注意喚起は、音声および視覚の注意喚起を含む、請求項1に記載の蘇生システム。
- システムを用いる間に記録される音を記憶するためのマイクロフォンおよび回路を備える請求項1に記載の蘇生システム。
- 前記除細動電極を支持する電極パッド部品と、該電極パッド部品に取り付けられ、胸部上への持ち上げ力を与えるための取っ手とを備える請求項1に記載の蘇生システム。
- 患者が着用するように構成され、かつ患者の現在の活動状態を推定することが可能な出力を提供することが可能な活動状態センサを備え、前記少なくとも一つのプロセッサは、該活動状態センサの出力を分析すること、患者のECGを分析すること、および電気治療を電極に送出すべきかどうかを判定することによって、患者の現在の活動状態を推定するように構成されている、請求項1に記載の蘇生システム。
- 前記少なくとも一つのプロセッサは、患者が動いているかどうかを推定するように構成されている、請求項14に記載の蘇生システム。
- 前記活動状態センサは、加速度計を備え、前記少なくとも一つのプロセッサは、前記加速度計の出力を積分して速度および変位のうちの少なくとも一つの推定値を与えるように構成されている、請求項15に記載の蘇生システム。
- 前記少なくとも一つのプロセッサは、前記活動状態センサの出力を処理すること、および処理の結果を用いて患者の生理的な状態を決定するために使用される技術において、少なくとも一つの閾値を変更することを行なうように構成されている、請求項14に記載の蘇生システム。
- 生理的な状態には、切迫した心臓発作または心拍停止の危険度を決定することが含まれる、請求項17に記載の蘇生システム。
- 前記蘇生システムは、音声による注意喚起を患者に発するためのスピーカを含み、前記少なくとも一つのプロセッサは、患者の推定される現在の活動状態に基づいて音声による注意喚起の種類を決定する、請求項14に記載の蘇生システム。
- 患者の現在の活動状態には、患者の向きを推定することが含まれる、請求項14に記載の蘇生システム。
- 患者の向きを推定することには、患者が仰向けに寝ているかどうかを判定することが含まれる、請求項20に記載の蘇生システム。
