Abstract
Description

良性前立腺肥大症(BPH)および前立腺癌は、高齢男性の集団によく見られる疾患である。BPHを有する人々にとって、前立腺肥大は、尿道を圧迫し、排尿困難をもたらす尿経路の閉塞を引き起こす可能性がある。前立腺肥大は、また、尿道結石、炎症、感染症、および場合によっては腎不全を引き起こし得る。

図1は、電源1を含む前立腺治療システム20ならびに細長の支持部材2と、支持部材2に沿って、もしくは支持部材の内部に位置付けられる光伝達装置6とを有する経尿道的治療装置21を図示する。経尿道的治療装置20は、さらに、バルーン3または細長の支持部材2によって運ばれる位置決め要素の他の種類を含み得る。支持部材2は、ルーメン4を有するカテーテルであり得、または支持部材2は、ルーメン4がない閉ざされた本体であり得る。実施形態によれば、支持部材2は、400〜450mmの全長を有し、3.327mmの外径を有す。支持部材2の遠位端のバルーン3は、10〜30mlの容積を有し、前立腺等の治療部位の近位に光伝達装置6を位置付け、固定するために使用される。

前記支持部材に沿った光発生器と治療部位の少なくとも近位で尿道内に位置付けられるように構成される光発光領域とを有し、前記光発生器が光活性組成物の活性化波長帯の範囲で事前に選択された波長または波長帯で光を発生するように構成される光伝達装置と、
尿道内に前記細長の支持部材を配置するように構成され、前記細長の支持部材によって運ばれる位置決め要素と、

- 温度センサに電気的に接続される温度モニタをさらに含む、請求項1に記載の経尿道的光活性化薬物療法装置。
- 前記温度モニタが、温度が事前に選択された値より上昇したときに作動する視覚的表示器、聴覚的表示器および/または自動切断を備える、請求項9に記載の経尿道的光活性化薬物療法装置。
- 前記支持部材が、その内部に前記光伝達装置が位置付けられることが可能である第1のルーメンと、患者の体内の部位と前記患者の体外の部位との間で液体を伝送するのに適している第2のルーメンとを有するカテーテルを含む、請求項1に記載の経尿道的光活性化薬物療法装置。
- 前記光伝達装置についての位置情報を提供するために所定の部位で前記光伝達装置の上にまたは固定関係でエコー源性の物質をさらに含む、請求項1に記載の経尿道的光活性化薬物療法装置。
- 前記温度センサが、制御ループに電気的に接続される熱電対であり、前記温度センサが、治療の間治療部位で温度を測定する、請求項1に記載の経尿道的光活性化薬物療法装置。
- 前記光伝達装置が、前記支持部材に固定されている、請求項1に記載の経尿道的光活性化薬物療法装置。
- 前立腺疾患の治療のための経尿道的光活性化薬物療法装置であって、
近位端、遠位端、および第1のルーメンを有し、前記遠位端が患者の尿道に挿入される寸法に作られる経尿道的カテーテルと、
前記カテーテルの前記遠位端にあって、収縮した形態で患者の膀胱に挿入される寸法であるバルーンと、
前記カテーテルの前記遠位端にあって、前記第1のルーメンに対して開いた流出開口部と、
前記カテーテルによって運ばれる前記発光ダイオード(LED)または固体レーザダイオード(LD)のアレイであって、前記LEDまたはLDの光源のアレイが、光活性組成物の活性化波長帯の範囲で事前に選択した波長または波長帯で光を生成する、LEDまたはLDのアレイと、
を含む、経尿道的光活性化薬物療法装置。 - 前記温度センサに電気的に接続されるモニタリング装置をさらに含む、請求項16に記載の経尿道的光活性化薬物療法装置。
- 前記モニタリング装置が、さらに出力装置を備え、前記出力装置が、前記治療部位の温度に対する視覚的表示器、聴覚的表示器、または他の温度表示器を提供する、請求項16に記載の経尿道的光活性化薬物療法装置。
- 前記カテーテルが、前記遠位端から光を発する場所を制御するための選択的コーティングを有する、請求項16に記載の経尿道的光活性化薬物療法装置。
- LEDまたはLDの前記アレイを内部に含み、かつ前記アレイが前記カテーテルの内部を軸方向に移動できる第2のルーメンをさらに含む、請求項16に記載の経尿道的光活性化薬物療法装置。
- LEDまたはLDの前記アレイが、治療の間、前記カテーテルの内部の選択された部位に固定される、請求項16に記載の経尿道的光活性化薬物療法装置。

- 前記カテーテルの前記近位端の上に位置決めの指標をさらに含む、請求項16に記載の経尿道的光活性化薬物療法装置。
- 良性前立腺肥大症の光活性化薬物療法の方法であって、
経尿道的治療装置の光発光領域が、治療部位の少なくとも近位になるように、尿道内で経尿道的治療装置の支持部材を位置付けることと、
前記治療部位に光反応剤を供給することと、
前記経尿道的治療装置の前記支持部材に沿って光発生器から光を生成して、前記光発光領域を通って前記光を通過させることと、
を含み、ここで、光は、前記光反応剤の吸収の波長帯と実質的に同等である1つまたは複数の波長もしくは波長帯を有する、方法。 - 前記治療部位の温度を温度センサでモニタすることを含む、請求項27に記載の方法。
- 前記治療部位の前記温度に基づいて光療法の投与を修正することを含む、請求項27に記載の方法。
- 前記光療法を投与する前に、前記支持部材を解放可能なように固定することをさらに含む、請求項27に記載の方法。
