含んでいる。細胞非含有(セルフリー)は、ここでは当
該技術分野におけるものと同様に用いられ、さらに細胞
分裂し広がり、又は生物学的に活性であり得る無傷の細
胞を実質的に含まない物質を意味する。好ましい実施態
様において、細胞非含有物質は無傷の有核細胞を一切含
まない。一実施態様において、本方法は、半透性コラー
ゲンマトリックスを含む細胞非含有被覆パッチの使用を
包んでいる。本方法の好ましい実施態様の1つにおいて
は、細胞非含有被覆パッチの多孔性表面が移植片物質に
向けられている。

作により最初に準備されている。一実施態様において

明の方法によって教示される通りに彫刻するように設計

gicel(R)(Ethicon Ltd.、UK)
であり、これは7〜14日後に吸収され得る。本発明の
方法におけるこの物質の使用は、Ethicon Lt
d.の包装挿入物に記述されるSurgicel(R)
のような止血装置の標準的な使用に反するものである。

細胞又は異種軟骨細胞の使用をも考慮する。

移植された軟骨細胞を適所に保持するけれども依然とし
て栄養分は得られるように修復部位全体に蓋をする細胞
非含有半透性障壁を具備するキットをも提供する。

トロシステムを用いて例証されている。このイン・ビト
ロ試験は、軟骨の欠損内に軟骨細胞を自己移植するプロ

出する場合にイン・ビボで起こるような、血管新生を抑
制する特定の試験物質の能力を予測する。
線維芽細胞等の成長又は発達する軟骨への浸潤を抑制す
る能力を有していることを特徴とする。適切な止血性物
質は、本発明の方法の目的を達成するものであり、軟骨
の形成を最適化するために、発達する軟骨への血管及び
細胞の浸潤が阻止されなければならず、関節の軟骨にお
ける完全厚のあらゆる欠損の修復が達成される。理想的
には、止血障壁は、完全な軟骨の修復を可能にするのに
十分な長期間安定であり、その後には再吸収され、又は
他の方法で自然に分解する。適切であると認められる物
質の1つは、Surgicel(R)W1912(酸化
再生無菌セルロースを含む吸収性止血剤:ロットGG3
DH.Ethicon Ltd.UK)と呼ばれるもの
である。適切な物質の他の例は、BioGide(R)

血管新生そのものの発達を阻止するためには、もっと長
い時間が必要であるため、この期間は短すぎるであろ
う。換言すると、血管新生の十分な抑制は、より長い時
間、例えば1ヶ月の間必要である。したがって、この生
成物は、この期間よりも著しく早く吸収されるべきでは
ない。他方、最終的な再吸収は必要である。したがっ
て、抑制障壁として用いられる有機物質は、これらの要
請に応えうるものであるべきであり、上記のように処理
されたSurgicel(R)が、その機能を提供する

た。

り合う適切なサイズに切断した。この場合、約4cmの
サイズの円であり(しかし、いかなる可能なサイズのも
のであってもよい)、無菌条件下で細胞研究作業用のN
UNCLONTMデルタ6−ウェル無菌使い捨てプレー
ト(NUNC、InterMed、ロスキルド、デンマ
ーク)のウェルの底部に置いた。ウェルの底部に置く止
血障壁は、実施例1に記述したように予備処理をしたも
のである。この処理は、Surgicelの吸収を十分
に遅らせる。次に、この止血障壁を蒸留水で数回洗浄
し、引き続き未反応グルタルアルデヒドが洗い流される
まで数回洗浄した。血清を含む少量の細胞培養培地を塗
布してこの止血障壁に吸収させ、同時にこの止血障壁を
ウェルの底部で湿った状態に保った。

アルデヒドで予備処理した止血障壁の表面全体にわたっ
て分散させた。次に、このプレートをCO2インキュベ
ータ内、37℃で60分間インキュベートした。5〜

を有するウェルに、この培地放出の際にピペットの先端
をウェルの側面に対して接線方向に保持するようにして
細胞が跳ねるのを回避しながら、注意深く添加した。培
地のpHは低すぎる(pH〜6.8)ように思われた。
次いで、pHを7.4〜7.5に調整した。翌日、幾つ
かの軟骨細胞が止血障壁上で成長し始め、クラスター状
に配列した。細胞の幾らかは、pHを調整する前の低p
Hへの露出により死んでいた。このプレートを、第3日
に培地を交換しながら、3〜7日間インキュベートし
た。

らないためである。Tisseel(R)を、DUPL
OJECT(R)システムを用いてSurgicel
(R)の両面に塗布し、それにより、Surgicel
(R)の両面と用いようとするパッチをフィブリン接着
剤で被覆した。このグルーを、無菌条件下で少なくとも
3〜5分間放置して乾燥させる。この“被覆”止血障壁
を、細胞研究作業用のNUNCLONTMデルタ6−ウ
ェル無菌使い捨てプレートのウェルの底部に置いた。血
清を含む少量の組織培養培地を塗布して止血障壁に吸収
させた。血清を含む組織培養培地1ml中約106個の
細胞をこの止血剤の頂部に直接置き、止血障壁の表面全
体に分散させた。次に、このプレートをCO2インキュ
ベータ内、37℃で60分間インキュベートした。5〜
7.5%の血清を含む組織培養培地2〜5mlを、細胞
を有するウェルに、この培地放出の際にピペットの先端
をウェルの側面に対して接線方向に保持することにより
細胞が跳ねるのを回避しながら、注意深く添加した。3
〜6日後、顕微鏡検査により、細胞がSurgicel
(R)に満足のいくように付着し、かつそこで成長して
いることが判明した。これは、Surgicel(R)
が軟骨細胞に対する毒性を示さず、軟骨細胞が満足のい
く様式でSurgicel(R)に接着したことを示唆
する。

(R)は、(E.D.Geistlich Sohne
AG、CH−6110ボルフーゼン(Wolhuse
n)による)標準化され制御された製造プロセスによっ
て得られる純粋なコラーゲン膜である。このコラーゲン
は、獣医学的に認定されたブタから抽出され、抗原反応
を回避するように注意深く精製され、二重ブリスター内
でγ線照射により殺菌される。このブリスター膜は、多
孔性表面及び高密度表面を有する。この膜は、I型及び
III型コラーゲンから、さらなる架橋もしくは化学処
理なしに作製される。このコラーゲンは、24週以内に
再吸収される。この膜は、その構造的統合性を湿潤した
ときでさえ保持し、縫合糸又は爪で固定することができ
る。また、この膜は、縫合糸の代わりに、又は縫合糸と
共にTisseel(R)のようなフィブリン接着剤を
用いて隣接する軟骨又は組織に“接着”させることもで
きる。

軟骨細胞を入れた2日後に、細胞をNikon倒立顕微
鏡で検査した。幾つかの軟骨細胞がBio−Gide
(R)の縁に付着していることが認められた。もちろ
ん、この顕微鏡を用いて、Bio−Gide(R)それ
自体から通して見ることは不可能であった。

(R)の高密度表面上で培養した軟骨細胞は、Bio−
Gide(R)のコラーゲン構造内に成長することはな
いが、多孔性表面上で培養した細胞は、実際にコラーゲ
ン構造内に成長することが示され、さらにプロテオグリ
カンの存在が示されたが、線維芽細胞構造の徴候は示さ
れなかった。この結果は、コラーゲンパッチ、例えばB
io−Gide(R)パッチが軟骨の欠損を被覆するパ
ッチとして縫合される場合には、培養軟骨細胞を注入し
ようとする欠損に向けて多孔性表面を下向きにすべきで
あることを示す。それにより、それらがコラーゲンに浸
透することが可能となり、無傷の表面に沿って滑らかな
軟骨表面が生じて、この領域にプロテオグリカンの滑ら
かな層が積層する。これに対し、コラーゲンの高密度表
面が欠損に向けられている場合、移植しようとする軟骨
細胞がコラーゲンと一体化せず、細胞が上述のものと同
じ滑らかな表面を生成することはない。

isseel(R)に標準的に見出されるものよりもか
なり高い含量のさらなるアプロチニンを含むTisse
el(R)のような有機グルーと共に用いることもでき
る。アプロチニンの含量を25,000KIU/mlに
高めることにより、この物質の再吸収が、数日という通
常の期間に代わり数週間遅れる。
Tisseel(R)をNUNCLONTMのウェル底
部に塗布し、不完全に固化させる。次に、Bio−Gi
de (R)のようなパッチをTisseel(R)上に
貼り、ウェルの底部に接着する。このBio−Gide
(R)とTisseel(R)との組み合わせは、軟骨
移植領域への血管の発達又は浸潤を抑制又は阻止する止
血障壁となるように設計される。この混成パッチは、病
変の底部での止血障壁(修復しようとする表面に対して
最も近位)としてだけではなく、軟骨形成の支持体とし
ても用いることができる。これは、末端(遠位)表面が
コラーゲンパッチの多孔性側であって、軟骨細胞及び軟
骨マトリックスの浸潤を促進することがあり得るためで
ある。したがって、この混成コラーゲンパッチは、移植
した軟骨細胞及びその頂部を形成する障壁に向けてその
多孔性表面を下向きにして、移植物の頂部を被覆するの
に用いることもできる。また、アプロチニン成分が増加
しているこのコラーゲンパッチは、Tisseel
(R)のようないかなる有機グルーをも必要とせずに用
いることができ、欠損内に直接配置して天然の力で付着
させることができる。したがって、このコラーゲンパッ
チは、その多孔性表面を移植した軟骨細胞/軟骨に向け
て、止血障壁及び修復/移植部位の細胞非含有被覆の両
者として用いることができる。他の変更例では、II型
コラーゲンからなるコラーゲンパッチ(すなわち、Ge
istlich Sohne AG、CH−6110ボ
ルフーゼンのもの)を用いることができる。
グルーを用いる、アプロチニン成分の濃度が好ましくは
約25,000KIU/mlに上昇しているコラーゲン
マトリックスである混成コラーゲンパッチであって、コ
ラーゲン成分がBio−Gide(R)再吸収性二層材
料又はII型コラーゲンに類似し、かつ有機グルーがT
isseel(R)材料に類似する混成コラーゲンパッ
チを提供する。別の実施態様においては、この混成パッ
チを修復部位に付着させるのに、いかなる有機グルーを
も用いない。
移植部位に移植される培養自己軟骨細胞の付着が抑制さ
れ、それにより新たに移植された軟骨/軟骨細胞とそれ
を取り巻く確立された軟骨との間に辺縁帯(境界帯)が
生じることがある。この辺縁帯は、直線状に切断された
真っ直ぐの滑らかな壁を創出することにより移植物のた
めの移植部位を調製する場合に、最も顕著である。(図
3Aに示されているような)このような辺縁帯を横切る
剪断及び圧縮力は、移植部位が直線状に切断される場合
に、その移植物を取り除こうとする大きな力を発揮す
る。この辺縁帯、及びこの帯域に沿う物質の差動は、移
植された物質とそれを取り巻く物質との間の集密的な治
癒を抑制する。この辺縁帯剪断は、隣接する物質の硬度
が異なる場合に悪化する。多くの場合、移植物質はそれ
を取り巻く物質よりも柔らかいが、骨関節炎疾患の中に
は、周囲の軟骨が実際には移植された軟骨細胞/軟骨よ
り柔らかいことがある。
発明の方法は、移植部位の壁を非直線的になるように彫
刻し、それにより辺縁帯剪断を減少させる波打つような
(ゆるやかに起伏する)表面を提供し、かつ移植された
物質の足場を提供する手術器具の使用を教示する。ま
た、骨表面に対して近位の部位の直径が骨に対して遠位
の開口部よりも大きな寸法となるように、及び軟骨の表
面に“逆漏斗”効果が存在するように移植部位を成形す
ることも可能である。表面が狭まった開口部は、辺縁帯
剪断及び移植部位からの移植物質の排除を減少させる助
けとなる。好ましい実施態様では、移植部位の壁を(図
3Bに示されるように)ボルト又はネジを受容するため
のネジが切られた開口部に類似する方式で彫刻し、それ
により、圧縮に対する機械的な抵抗力、及び/又は、
“オス”及び“メス”ネジとして説明することができる
移植部位からの移植物質の排出、が提供されることが記
述される。
使い捨ての、もしくは複数回使用の再利用可能な手術器
具を作製するのに適した金属及び/又はプラスチックか
ら製造することができる。軟骨は比較的柔らかい物質で
あるため、骨に損傷を与えることなく軟骨を彫刻するこ
とが可能である硬化プラスチックのカッティングエッジ
を製造することが有利であり得る。このようなカッティ
ング器具は、流体の導入、切削屑及び流体の吸引除去、
並びに欠損部位を照射及び可視化するための光ファイバ
繊維のための開口部を組み込むように製造することがで
きる。本発明の特定の実施態様においては、この器具の
底部は、その器具を移植部位に導いてそこに配置する手
助けをする突出点もしくはピン様構造を有していてもよ
い。もちろん、このようなピンは下層をなす骨に与える
損傷を最小にするように設計される。
であっても複数の歯であってもよく、又は金属部材にネ
ジ切り穴を作製するのに用いられる金属タップにおける
もののようなネジ様パターンを描いていてもよいが、こ
のカッティング器具に要求される特徴は、生じる移植部
位の彫刻された側面がゆるやかに起伏しており、非直線
的であることである。例えば、特定の実施態様におい
て、この器具のカッティングエッジは、図4A又は図4
Bに示されるものに類似する形状であり得る。カッティ
ングエッジは、平坦であっても、そのカッティング器具
のほぼ直径を覆う円形であってもよい。界面を創出し、
その界面が移植された物質とそれを取り巻く物質とに加
わる圧縮及び剪断力に対する異なる反応に対する機械的
抵抗力を提供する、という本発明の方法の目的を達成す
るため、多くの他の形状を設計することができる。
仰向けにした。このブタを、アリゾナ州フェニックスの
ハリントン関節炎研究センター(Harrington
Arthritis Reaseach Cente
r)の手術室で洗浄し、布で覆った。全ての手術手順は
無菌的に行った。左後肢並びに隣接する腹部及び鼠径部
領域をヨウ素で清浄化した。膝関節を探り出し、膝蓋を
探り出した。医学的切開を膝蓋の後部から約3cm行
い、内側大腿関節丘を扱うために幾らかの皮下組織、筋
肉層及び靱帯を大まかに切断した。円形カッターを用い
て、内側関節丘の内側部分の白色軟骨に、その関節丘の
後部内側部分を覆う軟骨の縁まで0.5〜1cmを余し
て病変を作製した(左関節丘、図6A)。内側関節丘の
後部重量支持部分に0.5〜1cmの欠損を配置した。
全ての手術手順は、左大腿に止血帯を用いることなく行
った。異なる層及び皮膚を適切に縫合した。

た。左後肢、腹部及び鼠径部領域を上述の通りにイオン
化した。縫合糸を切断し、その領域を開いた。膝関節に
中程度の血腫が存在することが認められた。この血餅を
除去し、欠損を点検した。欠損内に血餅が存在していた
のでそれを除去した。病変の外周に相当する、又はそれ
よりも僅かに大きいサイズを有する、オスのネジ切りカ
ッティングエッジを備えるように設計された無菌の手術
器具を、欠損内に注意深くねじ込んだ。BioGide
(R)パッドを欠損の底部と等しいサイズに切断した。
Adhesive Protein(A−2707、S
igma Chemical、USA)と呼ばれる第1
のグルーを、切り込んだ止血障壁パッドの高密度側に塗
布し、そのパッドを高密度側を下にして病変の底部に配
置して、上述の通りに障壁として用いた。このグルーは
非常に迅速に乾燥するようではないことが見出された。
欠損の底部からの僅かな出血が直ちに停止した。第2の
BioGide(R)を病変より幾らか大きい外周で切
断し、上述の通り高密度側を上にして(したがって、多
孔性側を移植物に向けて下にして)配置した。
つを開けたまま腔の上部で縫合した。外殖しようとする
軟骨細胞は、この開けたままの縁から移植部位に注入す
ることができる。このパッドの縁を取り巻く部分を第2
のグルー、Dow Corning Medical
Adhesive B(カタログ#895−3、Dow
Corning、USA)で被覆した。この第2のグ
ルーは、非常に迅速に乾燥し、第1のグルーより効率的
であった。この特定の手順の間、第1のグルーが、この
蓋の縫合を試みる際に止血障壁を適所に保持するのに十
分なほど乾燥していないことが見出された。移植部位の
近位表面上に形成される主要障壁は、そのグルー自体に
よるものであった。
て、軟骨細胞懸濁液(約0.6ml)をシリンジの外筒
内に吸い上げた。16ゲージ針を23ゲージ短針に切り
替え、縫合した被覆パッチの下の移植部位内に細胞懸濁
液を注入した(約10×106細胞)。その後、針を取
り除く前に蓋の開口縁を接着し、針を注意深く引き抜い
た。細胞の漏出は見られなかった。傷口を縫合したとこ
ろ、上述のように止血帯を用いることはなかったが出血
は観察されなかった。最終皮膚層を縫合した。縫合の後
に皮膚の隆起が生じることはなく、これは、血腫が存在
しないことを示していた。術後の回復は順調であった。
試験ブタの膝関節の関節軟骨表面に作製した欠損を修復
するのに十分な軟骨マトリックスを生成した。図6A
は、ブタの膝のMRI画像であって、その膝(左関節
丘、内側関節丘)に作製した軟骨の欠損を示すものであ
り、図6Bは治療の3ヶ月後の同じブタの膝のMRI画
像であって、欠損の修復を示すものである。
外科的な設定における本発明の方法の簡便な実施が考慮
される。好ましい実施態様においては、本発明のキット
は、手術環境における簡便な使用に適する無菌の構成要
素を提供し、かつ適切な止血障壁、適切な被覆パッチ、
及び、必要であれば、有機グルーを提供する。また、本
発明のキットは、関節表面の欠損内に移植しようとする
自己軟骨細胞を支持するのに適する無菌の細胞非含有マ
トリックス物質をも提供する。一実施態様において、本
発明のキットは、Surgicel(R)止血障壁及び
Tisseel(R)有機グルーで適切に覆われている
Bio−Gide(R)被覆パッチを備え、このSur
gicel(R)及びBio−Gide(R)は、本発
明の教示に従って、再吸収までの時間が延びるように処
理されている。Tisseel(R)が予め被覆されて
いる場合には、一実施態様において、再吸収までの時間
が延びるように、このTisseel(R)にさらなる
アプロチニンが補充されている。
壁及び被覆パッチは両者とも、その再吸収までの時間が
延びるように処理されている半透性コラーゲンマトリッ
クスである。また、修復/移植手順毎に固有の可変性及
び独自の状況に遭遇するため、Tisseel(R)グ
ルーを、エンハンスされた形態で、必要に応じて塗布す
る分離成分として提供することもできる。
記実施例7で説明される手術器具又はそれらの適切な変
形を含む。
特定の実施態様に示される本発明に対し、記述された本
発明の精神及び範囲から逸脱することなく、多くの変形
及び/又は修正を行うことができることを理解するであ
ろう。

Claims (18)
- 【請求項1】 治療対象部表面に止血障壁及び被覆パッ
チを用いて適切なマトリックス中の軟骨細胞を移植する
ことにより関節表面軟骨を有効に治療するための方法で
あって、止血障壁を治療対象部表面の近位に配置し、適
切なマトリックス中の軟骨細胞を前記止血障壁に対して
遠位の治療部位表面上に配置し、かつ治療対象部表面を
被覆パッチで被覆することを包む方法。 - 【請求項2】 移植部位を被覆する軟骨の表面にまず前
記被覆パッチを付着し、次いで前記軟骨細胞を前記被覆
パッチの下の前記移植部位内に配置する請求項1に記載
の方法。 - 【請求項3】 前記被覆パッチが半透性コラーゲンマト
リックスを含む請求項1または2に記載の方法。 - 【請求項4】 前記被覆パッチが多孔性表面を有する半
透性コラーゲンマトリックスを含む請求項3に記載の方
法。 - 【請求項5】 前記多孔性表面が移植された軟骨細胞と
接触する請求項4に記載の方法。 - 【請求項6】 前記被覆パッチが無傷の細胞を実質的に
含まない半透性コラーゲンマトリックスを含む請求項
3、4または5に記載の方法。 - 【請求項7】 前記止血障壁がこの障壁を貫通する血管
の浸潤を抑制又は阻止する再吸収性半透性物質である請
求項1から6に記載の方法。 - 【請求項8】 前記止血障壁がコラーゲンを含む請求項
1から7に記載の方法。 - 【請求項9】 治療対象部表面に、移植部位、止血障壁
及び被覆パッチを用いて、適切なマトリックス中の軟骨
細胞を移植するための方法であって、まず移植部位を、
その移植部位の壁が非直線的になり、及び/又は、ゆる
やかに起伏するように彫刻し、彫刻された移植部位内の
治療対象部表面に対して近位に止血障壁を配置し、適切
なマトリックス中の軟骨細胞を移植部位内の前記止血障
壁上に配置し、そして治療対象部表面を被覆パッチで被
覆することを包む方法。 - 【請求項10】 軟骨細胞を配置する前に、前記被覆パ
ッチが移植部位内の止血障壁上に部分的に付着される請
求項9に記載の方法。 - 【請求項11】 前記止血障壁がこの障壁を貫通する血
管の浸潤を抑制又は阻止する再吸収性半透性物質である
請求項9または10に記載の方法。 - 【請求項12】 前記止血障壁がコラーゲンを含む請求
項9から11に記載の方法。 - 【請求項13】 前記被覆パッチが細胞非含有半透性コ
ラーゲンマトリックスである請求項9から12に記載の
方法。 - 【請求項14】 関節の表面上に軟骨細胞及び、任意に
軟骨を移植するためのキットであって、再吸収に抗する
ように予備処理された、前記表面上への配置に適合可能
な止血障壁、再吸収に抗するように予備処理された、前
記表面の被覆に適合可能な被覆パッチ、及び前記止血障
壁及び/又は前記被覆パッチの前記表面への付着に適合
可能な有機グルーを備えたキット。 - 【請求項15】 請求項1から13に記載の方法を実施
するためのキットであって、再吸収を抑制するように処
理された止血障壁、再吸収を抑制するように処理された
被覆パッチ、有機グルー、及び任意に、移植部位の壁を
彫刻するための手術器具、及び任意に、前記移植部位内
に配置される軟骨細胞を支持するのに適するマトリック
ス物質を備えたキット。 - 【請求項16】 関節骨軟骨表面内に壁及び底部を有す
る移植部位を彫刻するための手術器具であって、ロッド
又は柄に取り付けられた切刃であって、前記移植部位内
で円方向に回転又はスクライブされたときに前記移植部
位がその壁が非直線的であるか又は任意にゆるやかに起
伏するように彫刻されるようになっている切刃を備えた
手術器具。 - 【請求項17】 前記刃が図4A又は4Bに表されたよ
うな形状である請求項16に記載の手術器具。 - 【請求項18】 前記刃が、管状又は環状ロッドの表面
を螺旋状に上がっていく螺旋状の刃の形状である請求項
16に記載の手術器具。
