Abstract
L'invention concerne une prothèse de pied et une vessie utilisable dans une telle prothèse. Cette prothèse de pied est caractérisée en ce qu'elle comprend, en combinaison, des moyens de fixation (26,28) pour faciliter le contact opérationnel de la prothèse de pied avec un moignon d'un porteur, une pluralité d'organes structuraux courbes (20,22,24), stockant de l'énergie, attachés opérationnellement aux moyens de fixation (26,28) et/ou les uns aux autres, et des moyens à vessie (14) adjacents aux organes structuraux (20,22,24), ces moyens à vessie (14) coopérant avec les organes structuraux (20,22,24) pour la détermination de la caractéristique de stockage d'énergie de la prothèse de pied.
Description
La présente invention concerne d'une manière générale des prothèses et
plus particulièrement l'utilisation d'une vessie réglable dans une prothèse telle qu'une prothèse de pied, cette vessie étant utilisable pour modifier ou ajuster d'une manière fine les caractéristiques de performance de la prothèse En particulier la vessie peut être juxtaposée à divers organes structuraux de la prothèse afin d'amortir, de limiter ou modifier autrement les mouvements de cette prothèse. Bien que de nombreuses prothèses ont essayé de simuler la marche d'un pied normal, pratiquement toutes ne sont pas aisément ajustables en ce qui concerne leurs caractéristiques de performance Cet ajustement est particulièrement désirable dans le cas de personnes amputées qui participent à une variété d'activités physiques exigeant différents niveaux de
possibilités de stockage et de libération d'énergie.
Il est certain que certains des dispositifs suivant la technique antérieure sont plus proches que d'autres de l'obtention de la facilité de réglage désirée Par exemple on
peut se reporter aux brevets US-A-4 547 913 et 4 822 363.
Egalement les demandes de brevets US 07/337 374, 07/585 920 et 07/293 824 décrivent des prothèses de pied avec des matériaux et des procédés de fabrication préférés similaires et avec les
avantages correspondants qui en résultent.
Chacune des inventions faisant l'objet des brevets précités est caractérisée par des structures légères et allongées comportant une imprégnation par un polymère de couches de renfort superposées, maintenues à la configuration désirée De telles configurations et constructions fournissent les caractéristiques désirables de résistance et de flexibilité de la prothèse et permettent d'obtenir une simulation des performances du pied naturel, ce qui n'a pu être obtenu antérieurement De telles prothèses peuvent être fournies sous la forme d'ensembles modulaires si bien que les caractéristiques de performance particulières d'une prothèse donnée peuvent être adaptées et ajustées aisément afin de répondre aux besoins et au niveau d'activité du patient individuel. Indépendamment de la contribution et des caractéristiques appréciables des solutions faisant l'objet des brevets précités, et notamment de la modularité des prothèses, l'ajustement de ces prothèses est relativement un problème comparativement à celui des prothèses suivant la présente invention Tout ajustement des caractéristiques de performance de ces prothèses exige le démontage d'un capot de couverture (s'il est présent) et d'organes structuraux les uns à partir des autres (par exemple par l'intermédiaire de combinaisons de vis, écrous et rondelles) Après qu'une nouvelle combinaison d'organes structuraux a été choisie, ces

D'autres prothèses de pied sont moins aisément ajustables Parmi ces prothèses on peut citer celle décrite dans le brevet US 3 335 428 qui vise à reproduire la structure du squelette et de la peau d'un pied humain naturel, celle décrite dans le brevet US 2 075 583 qui comporte une forme en caoutchouc montée en relation opérationnelle avec un noyau métallique rigide, et celle décrite dans le brevet US 4 645 509 qui a trait à une prothèse de pied comportant une quille ou une poutre monolithique de proportions relativement massives destinées à réagir à la charge du corps d'un amputé pendant la marche, la course, le saut et des activités similaires et pour libérer l'énergie accumulée résultante, afin de créer une élévation et une poussée du pied complétant
le mouvement naturel de l'amputé.
En outre la performance dynamique de la plupart des dispositifs suivant la technique antérieure est relativement raide et immédiate et elle ne permet pas de s'approcher de l'amortissement progressif que l'on peut obtenir avec l'utilisation des vessies suivant la présente invention, en
combinaison avec les organes structuraux.

de cette prothèse.
En outre la forme d'exécution préférée de la vessie suivant l'invention peut être fabriquée suivant une grande variété de configurations, y compris celles présentant des chambres multiples et des sections effilées De telles configurations permettent à la vessie ou aux vessies d'être mises en place dans des emplacements inhabituels mais utiles dans la prothèse et dans les organes structuraux de celle-ci, et elles procurent des avantages additionnels telles qu'une moindre sensation d'un "fort centrage", comme il a été décrit
précédemment.
Un autre but de la présente invention est de fournir une vessie utilisable dans des prothèses telles qu'une prothèse de pied du type précité Dans une forme d'exécution préférée la prothèse comporte, comme il a été indiqué précédemment, des moyens pour ajuster la pression interne et la sensation et les caractéristiques de performance correspondantes de la vessie et de la prothèse dans laquelle
la vessie est incorporée.
La prothèse suivant l'invention, comportant les organes structuraux et les vessies, est de préférence d'un type modulaire, ce qui se traduit par une facilité de montage et de réglage, comme dans le cas des inventions précitées Les divers composants de la prothèse de pied suivant l'invention peuvent être de préférence échangés aisément avec des composants de structure correspondante ou bien encore avec des composants ayant une structure différente, afin de permettre des réglages des dimensions et des performances et de donner la possibilité d'une adaptation de la prothèse à la dimension du pied de l'amputé ou à son pas, sa hauteur, son poids et son niveau d'activité Par conséquent une plage de combinaisons de la constante élastique, de possibilités de réglage et de dimensions peut être fournie à l'amputé, permettant de donner un pas naturel et une souplesse de la démarche, ce qui n'1 a pu être obtenu totalement avec les prothèses suivant la technique

Par conséquent la prothèse de pied suivant l'invention est caractérisée par un poids très léger, une réponse instantanée aux charges imposées et une libération instantanée correspondante de l'énergie accumulée lorsque la démarche du porteur de la prothèse indique que cette énergie accumulée doit être libérée Le porteur de la prothèse de pied peut S'engager dans une grande variété d'activités et il peut ajuster aisément et avec précision la raideur et la caractéristique élastique en fonction de l'activité
particulière considérée.
On décrira ci-après, à titre d'exemples non limitatifs, diverses formes d'exécution de la présente invention, en référence au dessin annexé sur lequel: La figure 1 est une vue en élévation, partiellement en coupe verticale, d'une prothèse suivant l'invention comportant
une vessie unique.
La figure 2 est une vue en élévation, partiellement en coupe verticale, d'une variante d'exécution de la prothèse suivant l'invention, semblable à celle de la figure 1 mais comportant une vessie additionnelle et des moyens pour ajuster
la pression interne des vessies.
La figure 3 est une vue en coupe horizontale faite
suivant la ligne 3-3 de la figure 1.
La figure 4 est une vue en perspective d'une vessie
réglable suivant l'invention.
La figure 5 est une vue en coupe faite suivant la ligne -5 de la figure 4. La figure 6 est une vue en perspective d'une variante
d'exécution d'une vessie réglable suivant l'invention.
La figure 7 est une vue en coupe faite suivant la ligne
7-7 de la figure 6.
Si on se réfère aux dessins et en particulier à la figure 1, on voit que cette figure représente une prothèse de pied 10 suivant l'invention, laquelle comporte des organes structuraux 12 et un dispositif 14 à vessie comprenant une vessie proprement dite 16 assemblée opérationnellement avec les organes structuraux Une chaussure 18, d'un type pouvant
être porté avec la prothèse, est représentée en trait mixte.
Ainsi qu'il a été indiqué précédemment, les organes structuraux 12 peuvent être fabriqués, combinés et assemblés suivant une grande variété de formes, de dimensions et de caractéristiques de performance, et ils sont réalisés de préférence à partir de matériaux et par des procédés décrits dans les brevets précités A titre illustratif la figure 1 représente des organes structuraux comportant une portion de talon 20, une portion d'avant-pied 22 et un organe auxiliaire 24.
Dans la forme d'exécution préférée, la portion d'avant-
pied 22 et l'organe auxiliaire 24 sont attachés opérationnellement à un pylône 26, au moyen de combinaisons 28 du type vis-écrou ou d'un dispositif similaire, ou bien ils sont adaptés d'une autre façon pour pouvoir être montés sur un moignon d'un porteur La portion de talon 20 est reliée opérationnellement (de préférence d'une manière démontable) à la portion d'avant-pied 22, au moyen de combinaisons
d'éléments du type vis-écrou 30 ou de dispositifs similaires.
Le dispositif à vessie 14, comportant la vessie proprement dite 16, est fabriqué de préférence en une matière appropriée robuste, flexible, étanche et légère, telle que l'uréthane ou similaire Ainsi qu'il est représenté à titre d'exemple sur les figures 4 et 5, la vessie peut être formée par thermoscellement, l'une à l'autre, de pièces de matière 32 et 34 de forme et dimension appropriées Des épaisseurs d'uréthane appropriées ont été trouvées être comprises entre 0,25 millimètre et 0,6 millimètre mais une large gamme d'épaisseurs et de matières peut être utilisée avec efficacité Des pressions de vessie allant jusqu'à 0,55 M Pa
ont été utilisées avec efficacité.
Le dispositif à vessie 14 est de préférence enveloppé dans un moyen de couverture 36 fabriqué en matière connue sous le nom de "KEVLAR" ou en une matière résistante similaire, afin d'empêcher que le dispositif à vessie 14 n'explose sous des pressions élevées et de contribuer à définir la forme gonflée finale du dispositif à vessie 14 Dans les formes d'exécution préférées représentées sur les figures 2 et 4 à 7, le moyen de couverture 36 comporte des sections 38 et 40 qui peuvent être cousues ensemble (ainsi qu'il est indiqué en 42), à l'endroit du périmètre de la vessie 16, avant ou après que les sections 38 et 40 ont été coupées à leurs formes finales désirées Les hommes du métier comprendront qu'une grande variété de matériaux de couverture et de procédés de fabrication et d'assemblage peuvent être utilisés avec
efficacité, sans sortir du cadre de l'invention.
Le dispositif à vessie 14 renferme de préférence de l'air, du dioxyde de carbone ou une substance gazeuse similaire, mais, suivant une variante, il pourrait renfermer des liquides ou des gels, tels que l'eau, un silicone ou similaire Un tel ensemble peut fournir la déformabilité
désirée et par conséquent l'effet d'amortissement, c'est-à-
dire de stockage, d'absorption et de libération de l'énergie.
Pour permettre une adaptation aisée de la constante élastique et des performances de la prothèse à son porteur, le dispositif à vessie 14 comporte de préférence un moyen de réglage 44, tel qu'un tube 46 en chlorure de polyvinyle ou uréthane, qui est collé par un solvant ou attaché d'une autre façon appropriée à la vessie 16, (par exemple à l'endroit 48) de manière à établir une communication pour un gaz avec
l'intérieur de la vessie.
Le moyen de réglage 44 comporte en outre, de préférence, une valve appropriée 50 branchée à l'extrémité opposée du tube 46 Dans la forme d'exécution préférée la valve 50 est adaptée de manière à pouvoir recevoir une aiguille provenant d'une pompe à air (non représentée) ou provenant d'une cartouche de C 02 (non représentée), ainsi qu'il sera décrit plus complètement plus loin, et elle peut être disposée sur un support approprié 52 (figures 2 et 3) pour permettre un accès aisé pour le porteur La valve peut être reliée opérationnellement au tube 46 en dilatant l'extrémité du tube et en insérant la valve dans cette extrémité, ainsi qu'il est indiqué en 54, ou en prévoyant un
adaptateur 56.
Des passages tels que des trous 58 (figure 2), peuvent être prévus dans les organes structuraux 12 pour laisser passer le tube 46 De préférence les passages 58 ont une dimension et sont placés de façon à faciliter le montage et la
marche de la prothèse sans toucher ou endommager le tube 46.
Une variante d'exécution 60 de la vessie du dispositif à vessie 14 est illustrée sur les figures 6 et 7 et cette variante d'exécution est fabriquée et fonctionne de la même manière que celle décrite précédemment Elle est toutefois
subdivisée de manière à comporter deux chambres 62 et 64.
Cette subdivision en deux chambres peut être aisément réalisée en thermoscellant une bande 66 s'étendant en travers de la totalité (ce qui n'est pas représenté) ou d'une partie de la vessie 60, ou bien encore elle peut être réalisée de nombreuses façons différentes De telles constructions peuvent comporter une large variété de formes (telle que la forme en U modifiée de la figure 6) et n'importe quel nombre avantageux de chambres, ces chambres pouvant être ou non en communication, pour un gaz ou un fluide, les unes avec les
autres, suivant les performances désirées de la vessie.
La prévision de la bande thermoscellée 66 dans la vessie 60 procure un certain nombre d'avantages pour le porteur de la prothèse Par exemple la bande 66 peut être utilisée pour produire un effilement ou une réduction de la section transversale suivant la longueur du dispositif à vessie 14, comme on peut mieux le voir sur la figure 7 Une vessie effilée peut être disposée plus aisément, en certains emplacements à l'intérieur de la prothèse 10, qu'une vessie non effilée, comme dans le cas de la vessie placée sous un organe structural de talon 68 comme il est représenté sur la figure 2 Une telle vessie peut également donner des effets d'amortissement plus désirables ou plus progressifs qu'une
vessie non effilée.
De plus la prévision de chambres multiples, telles que les chambres 62 et 64, permet de réduire et même de supprimer le problème précité de la sensation de "fort centrage" Au lieu d'assurer uniquement un emplacement de contact central entre la vessie 60 et l'organe structural de talon 68 la surmontant (figure 2), les chambres 62 et 64 produisent des surfaces de contact multiples et elles établissent de telles zones de contact latéralement par rapport au centre de
l'organe structural de talon 68.
Une autre modification avantageuse de l'invention est la prévision d'un moyen de restriction ou de retenue tel qu'une bande 70 (figure 1) La bande 70 est fabriquée en une matière flexible, de résistance appropriée, telle qu'une toile imprégnée ou similaire, et elle est conformée et assemblée avec les organes structuraux 12 et/ou le dispositif à vessie il 14 afin de donner à la prothèse les performances désirées en
ce qui concerne le stockage et la libération de l'énergie.
Par exemple la bande 70 peut être attachée opérationnellement à la portion d'avant-pied 22, au moyen de l'ensemble précité 30 du type vis-écrou, et elle peut être attachée d'une manière séparable autour de l'organe structural auxiliaire 24 au moyen d'éléments de fixation du type connu
sous le nom "VELCRO" ou d'un dispositif similaire.
La bande 70 fournit un certain nombre d'avantages Par exemple, si elle est juxtaposée à une vessie 16, la bande peut être serrée d'une manière appropriée afin d'aplatir la vessie, ce qui augmente les aires de contact entre les organes structuraux et la vessie et ce qui entraîne éventuellement une réduction du problème de la sensation de "fort centrage" dont
il a été question précédemment.
En outre un moyen de restriction, tel que la bande 70, peut être incorporé en divers emplacements sur la prothèse 10, afin de limiter la distance dont des organes structuraux associés 12 peuvent se déplacer l'un par rapport à l'autre Le moyen de restriction ou de retenue peut être utilisé pour empêcher une charge et une contrainte excessives et indésirables d'un organe structural particulier 12 et pour combiner les caractéristiques de réponse ressort-contrainte des organes structuraux associés 12 dans certaines conditions de charge Par exemple la bande 70, telle qu'assemblée comme représenté sur la figure 1, permet à l'organe structural auxiliaire 24 de contribuer à élever le bout du pied de la prothèse 10 et à stocker et libérer de l'énergie élastique lorsque la portion de talon 20 vient frapper le sol, en avant
du porteur.
En fonctionnement les vessies peuvent être totalement dégonflées si le porteur désire obtenir les caractéristiques de performance des seuls organes structuraux Par exemple dans l'ensemble illustré sur la figure 3 le dispositif à vessie antérieur 14 serait inactif, en étant dégonflé, et la réponse ressort-contrainte de l'organe structural auxiliaire 24 serait combinée avec celle de la portion d'avant-pied 22 uniquement après que cette portion d'avant-pied a fléchi suffisamment
pour venir en contact avec l'organe auxiliaire 24.
Cependant, lorsque la vessie est gonflée afin d'augmenter en pression et en volume, la combinaison des réponses ressort-contrainte de la portion d'avant-pied 22 et de l'organe auxiliaire 24 se produit effectivement après une flexion moindre correspondante de la portion d'avant-pied Si la vessie est suffisamment gonflée, la portion d'avant-pied 22 et l'organe auxiliaire 24 agissent et réagissent toujours à l'unisson. Additionnellement l'effet d'amortissement précité du dispositif à vessie 14 peut dépendre du degré de gonflage de la vessie Bien que, de préférence, la vessie conserve toujours un certain degré de compressibilité et d'effet d'amortissement, la compressibilité pourrait s'approcher d'une valeur négligeable si la vessie était suffisamment gonflée, ce qui éliminerait virtuellement tout effet d'amortissement
pendant la marche de la prothèse 10.
Les hommes du métier comprendront que la vessie suivant l'invention peut être utilisée efficacement dans une grande variété de prothèses, comportant des prothèses de pied ayant des configurations notablement différentes de celles
représentées sur les dessins.
Il sera également évident pour les hommes du métier qu'une très large variété de caractéristiques de performance peuvent être obtenues par suite de l'incorporation de la vessie suivant l'invention dans n'importe quel ensemble donné d'organes structuraux d'une prothèse En outre la sélection d'un ensemble désiré de caractéristiques de performance et le réglage à ces valeurs peuvent être aisément effectués par le porteur de la prothèse sans qu'il soit nécessaire de démonter et de remonter divers composants de cette prothèse Le porteur peut ajuster d'une manière répétée la caractéristique de réponse ressort-contrainte de la prothèse de pied dans un temps très court et au prix d'un effort très faible La valve comporte une fonction de soutirage afin de supprimer la pression dans la vessie et, comme il a été exposé précédemment, elle permet un gonflage aisé de la vessie en
utilisant une pompe à main, une cartouche de C 02 ou similaire.
Par exemple, si un individu pratique des sports ou d'autres activités qui soumettent la prothèse 10 à des charges plus grandes que celles apparaissant lors de son port normal journalier, les vessies 16 peuvent être gonflées d'une manière appropriée avant les périodes de plus grande activité, pour
être ensuite dégonflées.
Claims (12)
1 Prothèse de pied caractérisée en ce qu'elle comprend, en combinaison, des moyens de fixation ( 26,28) pour faciliter le contact opérationnel de la prothèse de pied avec un moignon d'un porteur, une pluralité d'organes structuraux courbes ( 20,22,24), stockant de l'énergie, attachés opérationnellement aux moyens de fixation ( 26, 28) et/ou les uns aux autres, et des moyens à vessie ( 14) adjacents aux organes structuraux ( 20,22,24), ces moyens à vessie ( 14) coopérant avec les organes structuraux ( 20,22,24) pour la détermination de la caractéristique de stockage d'énergie de
la prothèse de pied.
2 Prothèse de pied suivant la revendication 1 caractérisée en ce que l'un au moins des moyens à vessie ( 60)
est pourvu d'une pluralité de chambres ( 62,64).
3 Prothèse de pied suivant l'une quelconque des
revendications précédentes caractérisée en ce que l'un au
moins des moyens à vessie est réalisé avec une forme en U modifiée. 4 Prothèse de pied suivant l'une quelconque des
revendications précédentes caractérisée en ce que l'un au
moins des moyens à vessie ( 60) comporte une subdivision partielle en deux branches qui divisent le moyen à vessie ( 60) en une pluralité de sections tout en maintenant une
communication pour un fluide entre ces sections.
Prothèse de pied suivant l'une quelconque des
revendications précédentes caractérisée en ce que les moyens à
vessie ( 14) comprennent une vessie de talon ( 60) disposée opérationnellement en dessous d'un organe structural inférieur
( 20), disposé vers l'arrière.
6 Prothèse de pied suivant l'une quelconque des
revendications précédentes caractérisée en ce que les moyens à
vessie sont recouverts d'une matière protectrice ( 38,40).
7 Prothèse de pied suivant l'une quelconque des
revendications 1 à 6 caractérisée en ce qu'elle comporte un
moyen de retenue ( 70) juxtaposé à une pluralité d'organes structuraux ( 22,24), afin de limiter le mouvement de ces
organes structuraux ( 22,24) les uns par rapport aux autres.
8 Prothèse de pied suivant l'une quelconque des
revendications 1 à 6 caractérisée en ce qu'elle comporte un
moyen de réglage ( 44) associé opérationnellement aux moyens à vessie ( 14), afin de faciliter le réglage de la pression ou du
volume de ces moyens à vessie ( 14).
9 Prothèse de pied suivant l'une quelconque des
revendications 1 à 6 caractérisée en ce que les moyens à
vessie ( 14) contiennent un gaz tel que l'air.
Vessie réglable destinée à être utilisée dans des prothèses, caractérisée en ce qu'elle comporte une vessie proprement dite ( 16, 60) et une valve ( 50) reliée opérationnellement à la vessie proprement dite ( 16,60), afin de permettre d'obtenir le réglage désiré de la vessie proprement dite ( 16,60), si bien que cette vessie ( 16,60) peut être placée opérationnellement dans une prothèse afin d'influer sur la caractéristique de stockage d'énergie de
celle-ci.
11 Vessie suivant la revendication 10 caractérisée en
ce que la vessie ( 60) comporte des chambres multiples ( 62,64).
12 Vessie suivant l'une quelconque des revendications
et 11 caractérisée en ce que la vessie ( 14) est fabriquée
avec une forme en U modifiée.
13 Vessie suivant l'une quelconque des revendications
à 12 caractérisée en ce que la vessie ( 60) a une section transversale allant en décroissant suivant la longueur de cette vessie, afin de faciliter la juxtaposition de celle-ci à la prothèse.
14 Vessie suivant l'une quelconque des revendications
à 13 caractérisée en ce que la vessie proprement dite
( 14,60) est fabriquée en uréthane.
Vessie suivant l'une quelconque des revendications
10 à 14 caractérisée en ce que la vessie proprement dite est
recouverte d'une matière protectrice.
