En algunas realizaciones, las propiedades del entorno de difusión polimérico mucoadhesivo se ven influidas por su pH. En algunas realizaciones, por ejemplo, cuando el entorno de difusión polimérico mucoadhesivo comprende buprenorfina, su pH es de entre aproximadamente 4,0 y aproximadamente 7,5. En una realización, el pH es de entre 4,0 y 6,0, más específicamente, entre 4,5 y 5,5, e incluso más específicamente entre 4,75 y 5,25. En una realización específica, el pH es de 4,75. Ha de entenderse que todos los valores e intervalos entre estos valores e intervalos pretenden estar abarcados por la presente invención.
El pH del entorno de difusión polimérico mucoadhesivo puede ajustarse y/o mantenerse mediante métodos que incluyen, pero no se limitan a, el uso de agentes de tamponamiento, o ajustando la composición del dispositivo de la presente invención.
Los agentes de tamponamiento adecuados para su uso con la presente invención incluyen, por ejemplo, fosfatos, tales como fosfato de sodio; fosfatos monobásicos, tales como dihidrogenofosfato de sodio y dihidrogenofosfato de potasio; fosfatos dibásicos, tales como hidrogenofosfato de disodio e hidrogenofosfato de dipotasio; fosfatos tribásicos, tales como fosfato de trisodio; citratos, tales como citrato de sodio (anhidro o deshidratado) y citrato de trietilo; bicarbonatos, tales como bicarbonato de sodio y bicarbonato de potasio; acetatos, tales como acetato de sodio, pueden usarse. En una realización, se usa un único agente de tamponamiento, por ejemplo, un agente de tamponamiento dibásico. En otra realización, se emplea una combinación de agentes de tamponamiento, por ejemplo, una combinación de un agente de tamponamiento tribásico y un agente de tamponamiento monobásico. En algunas realizaciones, la cantidad de agente de tamponamiento que está presente en una composición usada para preparar la capa mucoadhesiva es de aproximadamente el 1 al 20 % de la cantidad del fármaco agonista, por ejemplo, buprenorfina.
ii. Capa de soporte
El dispositivo comprende además al menos un entorno polimérico no adhesivo adicional, por ejemplo, una capa de soporte. Esta capa está dispuesta adyacente al entorno de difusión polimérico mucoadhesivo, por ejemplo, una capa de soporte, funciona facilitando la administración del agonista de opioideo, tal como buprenorfina, a la mucosa. Esta capa adicional puede comprender la misma o diferente combinación de polímeros que el entorno de difusión polimérico mucoadhesivo o el entorno de difusión polimérico no adhesivo.
En algunas realizaciones, la capa de soporte incluye un medicamento adicional, tal como un antagonista opioideo, para hacer que el dispositivo de la invención sea resistente al uso inadecuado. En algunas realizaciones, el antagonista opioideo es naloxona. La dosis de naloxona que puede incorporarse en el dispositivo de la presente invención depende de la dosificación de tratamiento deseada que va a administrarse y puede oscilar entre aproximadamente 100 µg y 5 mg de naloxona para el tratamiento de dependencia, y entre 60 µg y 1,5 mg de naloxona para la indicación de dolor. En una realización, la dosis es de 0,145 mg de naloxona. En otra realización, la dosis es de 290 µg de naloxona. En otra realización, la dosis es de 580 µg de naloxona. En aún otra realización, la dosis es de 870 µg de naloxona. En algunas realizaciones, la cantidad de buprenorfina y la cantidad de naloxona dispuestas en el dispositivo están presentes en una razón elegida de manera que la naloxona suprime el efecto de la buprenorfina si la mezcla se inyecta o se inhala. En tal realización, la buprenorfina y naloxona dispuestas en el dispositivo están presentes en una razón p/p que oscila entre 1:4 y 1:10. En una realización preferida, la razón p/p de buprenorfina con respecto a naloxona es de
1:4 a 1:6, en la que 1:6 es la realización más preferida.
En algunas realizaciones, la capa de soporte (es decir, la realización polimérica no adhesiva) funciona como barrera para facilitar el flujo unidireccional del medicamento, por ejemplo, buprenorfina, dispuesto en la capa mucoadhesiva. Tras la aplicación a una superficie mucosa, se crea un gradiente de difusión de un medicamento hacia la mucosa. En otra realización, la capa de soporte puede servir como polímero erosionable que facilita la absorción de buprenorfina en el tejido orofaríngeo. En algunas realizaciones, impide la difusión lejos de la superficie mucosa. En tales casos, la mayoría del medicamento, es decir, al menos el 50 % fluye hacia la mucosa. En otras realizaciones, tal como se representa en la figura 1, el entorno polimérico no adhesivo puede circunscribirse a los límites del entorno de difusión polimérico mucoadhesivo garantizando de ese modo que el medicamento fluye hacia la mucosa.
La capa de soporte (por ejemplo, una capa de soporte no adhesiva erosionable en agua) puede incluir además al menos un polímero formador de película erosionable en agua. Esta capa puede incluir opcionalmente un fármaco. El polímero o polímeros pueden incluir poliéteres y polialcoholes así como polímeros celulósicos con enlaces de hidrógeno que tienen o bien sustitución con grupo hidroxialquilo o bien sustitución con grupo hidroxialquilo y grupo alquilo, preferiblemente con una razón de moderada a alta de grupo hidroxialquilo con respecto a alquilo. correctamente, puede no producir la reacción de aversión prevista experimentada por los que son dependientes de opioides agonistas completos. Por consiguiente, el objetivo del estudio era determinar la dosis eficaz mínima de naloxona que producirá una respuesta de abstinencia cuando se administra con una dosis de 0,75 mg de buprenorfina en sujetos dependientes de opioides. El objetivo secundario del estudio era determinar si la administración de una dosis de 0,75 mg de buprenorfina sin naloxona producirá una respuesta de abstinencia en sujetos dependientes de opioides.
Población de estudio
Se diseñó el estudio para incluir un total de 12 sujetos adultos que completaran el estudio de grupos cruzados de 4 periodos. Los sujetos eran elegibles para su inclusión en el estudio si cumplían todos de los siguientes criterios:
los sujetos experimentaban dolor no oncológico de moderado a intenso crónico que se habían tratado con > 100 mg de equivalentes de morfina al día durante al menos 3 meses antes del estudio;
los sujetos presentaban signos y síntomas de abstinencia (es decir, puntuación de COWS ≥ 5) tras la administración de naloxona durante la exposición a naloxona.
Descripción del medicamento en estudio
Durante la visita de tratamiento hospitalaria, los sujetos recibieron una única dosis en bolo i.v. de 3 ml de cada uno de los siguientes tratamientos separados por al menos 72 horas:
a) 0,75 mg de buprenorfina (B)
b) 0,75 mg de buprenorfina + 0,1 mg de naloxona (BN1)
c) 0,75 mg de buprenorfina + 0,2 mg de naloxona (BN2)
d) Placebo (solución glucosada al 5 %) (P)
Procedimientos de estudio
Los sujetos elegibles presentaban signos de abstinencia, por ejemplo, tenían una respuesta positiva (COWS ≥ 5) a la prueba de abstinencia de naloxona, tras la administración de hasta ocho dosis i.v. de 0,05 mg de naloxona.
Escala clínica de abstinencia de opioides
Se usó la escala clínica de abstinencia de opioides (COWS) para evaluar los síntomas de la abstinencia de opioides. La escala contiene 11 ítems para clasificar los signos y síntomas de la abstinencia de opioides, incluyendo frecuencia cardiaca, malestar gastrointestinal, sudoración, temblores, inquietud, bostezos, tamaño de la pupila, ansiedad o irritabilidad, dolores óseos o articulares, piel de gallina, goteo nasal, lagrimeo. Cada síntoma se clasifica en una escala única. Las puntuaciones totales para la escala oscilan entre 0 y 48, indicando las puntuaciones de 512 abstinencia leve; indicando las puntuaciones de 13-24 abstinencia moderada; indicando las puntuaciones de 2536 abstinencia moderadamente intensa; e indicando las puntuaciones > 36 abstinencia intensa.
Escala de agonistas opioideos
Se les pidió a los sujetos que evaluaran los siguientes ítems: asentir con la cabeza, sensación de pesadez o de pereza, boca seca, despreocupación, buen humor, energía, náuseas, picor cutáneo, sensación de relajación, vagancia, muy animado, mareo agradable, fuerza, estado de embriaguez, simpatía y nerviosismo usando una escala de Likert de 5 puntos (0 = en absoluto, 1 = un poco, 2 = moderadamente, 3 = bastante y 4 = extremadamente).
Análisis estadístico
Se usaron estadísticos descriptivos para resumir los resultados de los análisis de FD en cada punto temporal para cada grupo de tratamiento, sin pruebas estadísticas formales. Se ajustó un modelo de efectos mixtos a los datos para el cambio respecto al periodo basal en la puntuación total en donde el modelo incluía factores para: el cambio medio global; efectos fijos debidos a la secuencia; tratamiento, tiempo y periodo; y un efecto aleatorio para sujetos anidado dentro de la secuencia.
Se obtuvieron los cambios en la media de mínimos cuadrados y los errores estándar y se usaron para construir IC del 95 % para las diferencias entre los tratamientos de un modo por parejas. Se excluyeron los datos de sujetos recogidos después de la administración de tratamiento de rescate (es decir, el tiempo en el que los sujetos alcanzan una puntuación de COWS total de 13 o más) para minimizar el efecto de confusión del tratamiento de rescate sobre los análisis de FD.

Los resultados de este estudio doble ciego, controlado por placebo, en sujetos dependientes de opioides indican que dosis de buprenorfina intravenosas de 0,75 mg tienen poco potencial de uso indebido, y que la adición de naloxona aumenta la incidencia de abstinencia, así como evaluaciones farmacológicas negativas. Por tanto, la naloxona a dosis de 0,1 y 0,2 mg proporciona un efecto de disuasión del uso indebido a una dosis de 0,75 mg de buprenorfina cuando se administra por vía intravenosa a sujetos dependientes de opioides.
