Abstract

Campo de la invención
La presente invención se refiere a la distracción y fusión de cuerpos vertebrales. Más específicamente, la presente invención se refiere a dispositivos para la distracción y fusión de cuerpos vertebrales que utilizan mecanismos de manguito de engranaje helicoidal coaxial.
Antecedentes de la invención
El concepto de fusión intervertebral para la columna cervical y lumbar después de una discectomía se introdujo generalmente en la década de 1960. Implicaba extraer un injerto óseo de la cadera e implantar el injerto en el espacio discal. El espacio del disco se preparó ampliando el espacio para que coincidiera con el implante. Las ventajas de este concepto fueron que proporcionaba una gran superficie de contacto hueso a hueso y colocaba el injerto bajo fuerzas de carga que permitían la osteoconducción y la inducción de la fusión ósea. Sin embargo, la técnica rara vez se practica hoy en día debido a numerosas desventajas, incluido el tiempo de operación prolongado, la destrucción de una gran parte del espacio discal, el alto riesgo de lesión nerviosa y el dolor de cadera después de la extracción del injerto óseo.
Actualmente, al menos dos dispositivos se usan comúnmente para realizar la porción intervertebral de una fusión de cuerpo intervertebral: el primero es el dispositivo de distracción y el segundo es el dispositivo de fusión del cuerpo intervertebral, a menudo denominado como una jaula. El concepto es introducir un implante que distraiga un disco colapsado y descomprima la raíz nerviosa, permite que la carga se comparta para mejorar la formación de hueso e implantar un dispositivo lo suficientemente pequeño como para permitir la implantación con una retracción mínima y tirando de los nervios.
En un procedimiento típico de fusión del cuerpo intervertebral, una porción del disco intervertebral se elimina primero de entre los cuerpos vertebrales. Esto puede hacerse mediante un enfoque abierto directo o un enfoque mínimamente invasivo. Se pueden utilizar afeitadoras de disco, pinzas gubias de hipófisis, curetas y/o raspadores de disco para extraer el núcleo y una porción del anillo anterior o posterior para permitir la implantación y el acceso al espacio interno del disco. El dispositivo de distracción se inserta en el espacio despejado para agrandar el espacio del disco y los cuerpos vertebrales se separan accionando el dispositivo de distracción. La ampliación del espacio discal es importante porque también abre el foramen donde existe la raíz nerviosa. Es importante que durante el proceso de distracción no se distraiga demasiado las articulaciones facetarias. A continuación se inserta un dispositivo de fusión intervertebral en el espacio distraído y se puede insertar factor de crecimiento óseo, como autoinjerto, una esponja de colágeno con proteína morfogenética ósea u otra sustancia potenciadora ósea, antes o después de la inserción del dispositivo en el espacio discal, en el espacio dentro del dispositivo de fusión intervertebral para promover la fusión de los cuerpos vertebrales.
La fusión y la distracción intervertebrales se pueden realizar a través de abordajes anterior, posterior, oblicuo y lateral. Cada enfoque tiene sus propios desafíos anatómicos, pero el concepto general es fusionar la vértebra adyacente en la columna torácica o lumbar cervical. Los dispositivos se han fabricado con diversos materiales. Dichos materiales incluyen hueso esponjoso cadavérico, fibra de carbono, titanio y polieteretercetona (PEEK). Los dispositivos también se han fabricado en diferentes formas, como una forma de frijol, forma de balón fútbol, forma de plátano, forma de cuña y una jaula cilíndrica roscada.
Como ejemplo de un dispositivo espaciador, US2007/0255415 de Edie et al describe un espaciador intervertebral expansible que puede insertarse entre los miembros vertebrales y expandirse. El documento US2007/0250171 de Bonin, J.R. describe otro dispositivo intervertebral expansible, diseñado para reemplazar uno o más miembros vertebrales. El documento WO2009/064787 da a conocer un implante intervertebral de altura ajustable para la sustitución total, parcial o nuclear de un espacio discal intervertebral.
Es importante que un dispositivo que se utiliza tanto para la fusión del cuerpo intervertebral como para la distracción sea lo suficientemente pequeño como para facilitar la inserción en el espacio intervertebral y de altura suficiente para mantener la altura normal del espacio discal. El uso de un dispositivo de tamaño insuficiente que no puede expandirse a una altura suficiente puede provocar una fusión inadecuada entre las vértebras adyacentes y provocar

complicaciones adicionales para el paciente, como la migración del dispositivo dentro o la extrusión fuera del espacio discal. Solucionar estos problemas puede requerir el uso de varios dispositivos de diferentes tamaños para ser utilizados en serie para expandir el espacio del disco en la cantidad adecuada, lo que aumenta el tiempo requerido para llevar a cabo el procedimiento, aumentando el costo y el riesgo asociados con el procedimiento.
En consecuencia, existe una necesidad en la técnica de un dispositivo de suficiente resistencia que pueda distraer desde un tamaño inicial lo suficientemente pequeño como para ajustarse inicialmente en el espacio discal a una altura suficiente para restablecer y mantener la altura normal del espacio discal.
Resumen de la invención
Los aparatos mejorados para la distracción y fusión del cuerpo vertebral de acuerdo con diversas realizaciones de la presente invención emplean uno o más mecanismos de manguito de engranaje helicoidal coaxial.
Según la presente invención, se proporciona un dispositivo de fusión del cuerpo intervertebral distractivo adaptado para implantación en un espacio discal intervertebral de un paciente que tiene una primera superficie de apoyo configurada para interconectarse con una placa de extremo de una vértebra superior del espacio discal intervertebral y una segunda superficie de apoyo configurada para interconectarse con una placa extrema de una vértebra inferior del espacio discal intervertebral, caracterizado además porque el dispositivo incluye al menos un mecanismo de manguito de engranaje helicoidal coaxial dispuesto entre la primera superficie de apoyo y la segunda superficie de apoyo, el mecanismo de manguito de engranaje helicoidal coaxial que incluye un poste que tiene una superficie exterior roscada en una parte del poste que sobresale hacia dentro desde una de la primera superficie de apoyo y la segunda superficie de apoyo; y un manguito correspondiente configurado para rodear el poste y que tiene: una superficie interior roscada en una parte del manguito que sobresale hacia dentro desde la otra de la primera superficie de apoyo y la segunda superficie de apoyo y configurada para interconectarse con la superficie exterior roscada del poste; y una superficie exterior de engranaje en una porción del manguito; la superficie exterior de engranaje definida en una rosca que se extiende en un patrón helicoidal a lo largo de una altura del manguito; y un mecanismo de accionamiento que tiene una superficie configurada para interactuar con la superficie exterior de engranaje del manguito y conducirla, de modo que la operación selectiva del mecanismo de accionamiento provoca una distracción de la primera superficie de apoyo y la segunda superficie de apoyo con respecto a la vértebra superior y la vértebra inferior del espacio discal intervertebral debido a una expansión telescópica que resulta del manguito que se traslada con relación a una de la primera superficie de apoyo y la segunda superficie de apoyo simultáneamente con la postraducción con respecto al manguito.
En diversas realizaciones, los mecanismos de manguito de engranaje helicoidal coaxial incluyen un poste con una superficie exterior roscada y un manguito correspondiente configurado para rodear el poste, el manguito correspondiente tiene una superficie interior roscada configurada para interconectarse con la superficie exterior roscada del poste y una superficie exterior de engranaje. Se puede configurar un mecanismo de accionamiento para interactuar con la superficie exterior de engranaje del manguito, haciendo que el dispositivo distraiga.
En una realización, se usa un dispositivo para la distracción intervertebral y la fusión de un espacio discal intervertebral. El dispositivo puede incluir una primera superficie de apoyo y una segunda superficie de apoyo con al menos un mecanismo de manguito de engranaje helicoidal coaxial dispuesto entre ellas. El mecanismo de manguito de engranaje helicoidal coaxial incluye un poste con una superficie exterior roscada que se proyecta hacia dentro desde una de las superficies de apoyo y un manguito correspondiente configurado para rodear el poste. El dispositivo se inserta de manera que una primera superficie de apoyo se conecta con una placa extrema de una vértebra superior del espacio discal intervertebral y una segunda superficie de apoyo se conecta con una placa extrema de una vértebra inferior del espacio discal. Al menos un mecanismo de manguito de engranaje helicoidal coaxial está dispuesto entre las superficies de apoyo e incluye un poste roscado, un manguito correspondiente que tiene una rosca interior que se acopla con el poste roscado y un engranaje exterior que se acopla con un mecanismo de accionamiento. Al girar el hexágono 144 en el sentido contrario a las agujas del reloj, y por lo tanto el tornillo 140 sin fin, en el sentido contrario a las agujas del reloj, la primera sección 142 roscada del tornillo 140 sin fin tira de los dientes 134 del engranaje del manguito 130 de engranaje roscado hacia la cabeza 144 hexagonal. Mecanismos de accionamiento alternativos a la unidad de tornillo sin fin incluyen accionadores piezoeléctricos y cualquier impulso que imparte mecanismo de colisión o configuración. Además, un mecanismo de accionamiento, como un tornillo sin fin, podría ser una parte integrada de un sistema de entrega. A medida que se gira el tornillo sin fin, la posición axial del tornillo sin fin estaría restringida por el sistema de suministro, en lugar de un pasador, lo que provocaría una distracción del dispositivo. Una vez que el dispositivo alcanza la altura deseada, el tornillo sin fin se puede atornillar fuera de la abertura del tornillo sin fin y el dispositivo se puede bloquear en su lugar enroscando un tornillo sin fin roscado. El tornillo sin fin podría tener una característica adicional de enroscado o ajuste que permita su fijación permanente o de manera desmontable al dispositivo. El tornillo sin fin de bloqueo podría estar hecho de un material radio transparente como PEEK, lo que permitiría la formación de imágenes a través del tornillo sin fin. El tornillo sin fin de bloqueo solo necesitaría ser lo suficientemente fuerte como para impedir que los manguitos con engranajes roscados entren o salgan del dispositivo, y no necesitaría ser lo suficientemente fuerte como para causar distracción. Se podría formar una ventana transparente de radio más grande retirando una parte de los lados del miembro inferior a cada lado de la abertura en el miembro inferior a lo largo de la longitud del dispositivo, siempre que el dispositivo retenga una cantidad necesaria de rigidez.

Debe observarse que aunque las roscas representadas en las figuras son todas roscas de tomillo en forma de nervaduras helicoidales salientes, la "rosca" para los fines de la presente invención también puede referirse a cualquier otro mecanismo que traduzca la fuerza rotacional en movimiento traslacional o longitudinal. Por ejemplo, en algunas realizaciones, las roscas pueden estar compuestas por una disposición de cojinetes en recirculación o en espiral o cualquier otra disposición de baja fricción, tal como imanes cooperantes.
En una realización, la altura del dispositivo 100 entre las superficies 102, 104 de soporte en la configuración completamente comprimida es de 6,5 milímetros y la altura máxima completamente distraída es de 12 milímetros, proporcionando así una gran cantidad de distracción en relación con la altura inicial del dispositivo. La altura máxima se define por la mayor altura a la que el dispositivo puede cumplir los requisitos dinámicos de compresión, cizalladura y torsión para dispositivos de fusión del cuerpo intervertebral implantables. Las variables que determinan esta altura incluyen el ancho de los manguitos de engranaje roscados, que está limitado por el ancho deseado del dispositivo y el material del que está hecho el dispositivo. Con respecto al material para el dispositivo, los materiales con mayor rendimiento de fatiga permiten que la altura máxima del dispositivo sea más alta incluso con un ancho más estrecho. En una realización, el dispositivo está hecho de titanio. El dispositivo también puede estar hecho de cobalto cromo, MP35N o PEEK, para aumentar las características de resistencia o aumentar las características radiotransparentes, dependiendo del material. La transparencia de rayos X es una propiedad deseable, ya que permite obtener imágenes del hueso de fusión a través del dispositivo. En una realización, el dispositivo puede diseñarse de manera que en la configuración comprimida los manguitos de engranaje roscados se proyecten a través de la superficie de soporte del segundo miembro con el fin de proporcionar una cantidad aún mayor de distracción. Para acomodar el dispositivo en la implantación, las aberturas configuradas para contener las partes sobresalientes de los manguitos se pueden cortar en la placa terminal vertebral adyacente.
Una vez distraído, el dispositivo 100 no requiere un mecanismo de bloqueo para mantener la altura deseada dentro del cuerpo. Esto se debe a que, cuando se lo empuja hacia atrás, el dispositivo exhibe una relación de transmisión muy alta que provoca que incluso la menor fricción en el sistema abrume cualquier cantidad de compresión, torsión o carga de cizallamiento que pueda aplicarse al dispositivo. En las pruebas dinámicas de cizalladura, torsión y compresión, la cantidad máxima en que se modificó la altura del dispositivo fue de aproximadamente 0.01 milímetros. El dispositivo 100, porque la altura puede mantenerse en cualquier punto a lo largo de los manguitos de engranaje roscados, por lo tanto también exhibe un control de altura de resolución muy alta, del orden de 1 micrómetro.
En una realización, las roscas 121, 131 externas y los dientes 124, 134 de engranaje en los manguitos 120, 130 de engranaje roscados pueden ser de forma sustancialmente trapezoidal. En una realización, la rosca es una rosca métrica trapezoidal de 8 milímetros por 1,5 milímetros. Un diseño trapezoidal permite un tamaño de diente de engranaje relativamente grande y, en consecuencia, un área más grande sobre la que se distribuye la carga de distracción. Adicionalmente, con fabricación precisa, se pueden acoplar múltiples dientes 124, 134 de engranaje en los manguitos 120, 130 de engranaje roscados mediante el tornillo 140 sin fin al mismo tiempo a lo largo del ángulo de presión ANG, como se muestra en las Figuras 4A y 4B. La distribución de la carga de distracción sobre múltiples dientes de los manguitos 120, 130 y el tornillo 140 sin fin es crítica para lograr el tamaño mínimo del dispositivo a la vez que proporciona una cantidad máxima de distracción y capacidad de carga.
En las figuras 5A (configuración comprimida) se representa un sistema 200 de suministro para implantar un dispositivo de fusión del cuerpo intervertebral que se puede separar de acuerdo con una realización de la presente invención, 5B (configuración parcialmente distraída, y 5C (configuración totalmente distraída). El sistema 200 de suministro también incluye una herramienta de actuación 300 para activar la distracción.
Para distraer al dispositivo 100, un hexágono 143 o 144 del dispositivo se conecta primero al sistema 200 de suministro a través de un controlador de socket en un extremo 201 del eje 203 de suministro. Con el fin de unir de manera más segura el dispositivo 100 y el sistema 200 de suministro, un extremo 202 roscado del eje 204 de suministro se puede enroscar en uno de los orificios 156A o 156B roscados en el segundo miembro 150 del dispositivo 100. El dispositivo 100 puede entonces insertarse en el cuerpo mediante un procedimiento de fusión intercorporal lumbar transforaminal estándar (TLIF) o fusión intersomática lumbar posterior (PLIF) usando el sistema de suministro 200. En una realización, una palanca para aplicar un par de torsión al eje 204 puede fijarse al hexágono 215 en el extremo del eje 204. La herramienta 300 de accionamiento puede incluir un rebaje o bucle 304 que le permite al usuario girar la herramienta 300 de accionamiento con un solo dedo y/o grandes superficies 301 de agarre que el usuario puede agarrar para girar la herramienta 300 de accionamiento. En una realización, el bucle puede estar revestido con una superficie resbaladiza o de apoyo para permitir que el bucle gire fácilmente alrededor de los dedos enguantados del usuario. La herramienta 300 de accionamiento también puede incluir una amplia superficie 303 diseñada para recibir el impacto de un martillo para la implantación. Los rebajes 302 también se pueden incluir en la herramienta 300 de accionamiento para proporcionar al usuario una vista mejorada del dispositivo 100 mientras se está implantando. En otras realizaciones, la herramienta de actuación, manual o automática, emplea sensores en el dispositivo para transmitir datos con respecto a los parámetros de implantación y al entorno, tales como la carga del dispositivo y la tensión muscular, a un operador o sistema operativo para mejorar el rendimiento del procedimiento quirúrgico y el resultado. El sistema 200 de suministro podría usar medidores de tensión pequeños ubicados en el dispositivo 100 y/o células de carga unidas a los ejes 203, 204 de entrega y herramienta de actuación para medir las cargas presentes durante el proceso de implantación y distracción. En una realización, el dispositivo 100 puede tener un segundo miembro 150 reforzado como se muestra en las Figuras 6A y 6B. Esto permite que una rosca 152B interna completa enganche el manguito 130 de engranaje roscado en el lado opuesto a la rosca 152A interna. Al capturar el manguito de engranaje roscado con una rosca completa en ambos lados, cuando el dispositivo está cargado con cizalladura y torsión, una cantidad máxima de material resiste la carga, lo que minimiza la tensión resultante y aumenta la vida de fatiga del dispositivo 100.
Las figuras 7A y 7B representan otra realización de la presente invención en la que los postes 111, 112 roscados emplean un rosca 113A, 115A de refuerzo (compárense las roscas 113A en la figura 7B con las roscas 113, 115 en la Figura 1D). Una configuración de rosca de contrafuerte hace que la cara de rosca que soporta la carga sea perpendicular al eje del tornillo del poste 111, 112, lo que aumenta la resistencia axial del dispositivo. Las figuras 8A y 8B representan una realización adicional que utiliza una rosca 113B, 115B estándar de 60 grados en los postes 111, 112 roscados. Las roscas de 60 grados se consideran estándar de la industria y, por lo tanto, se pueden crear con prácticas de mecanizado comunes. Esto puede dar como resultado un dispositivo que se puede producir más rápida y económicamente.
Haciendo referencia ahora a las figuras 9A-9D, otra realización de un dispositivo 400 de fusión del cuerpo intervertebral distractible incluye un único par de postes 423 de engranaje roscados que se extienden entre el primer miembro 410 y el segundo miembro 450 en lugar de los manguitos 120, 130 de engranaje roscados separados y los postes 111, 112 roscados descritos anteriormente. Los postes 423 de engranaje roscados incluyen cada uno una porción 421 de engranaje roscado y una porción 411 de poste. Las figuras 10A-10D representan una realización adicional de un dispositivo 500 de fusión del cuerpo intervertebral distractible que permite el ajuste diferencial de los manguitos 520 de engranaje roscados. Los postes 511 roscados pueden incluir cada uno una parte 515 arqueada que corresponde a un rebaje 517 arqueado en el primer miembro 510. La interfaz arqueada entre los postes 511 roscados y el primer miembro 510 creado por las partes arqueadas 515 correspondientes y los rebajes 517 arqueados permiten que el primer miembro 510 gire y se acople en relación al segundo miembro 550. Se puede usar una unión de pasador que utiliza un pasador 572 de pivote para mantener fija una interfaz entre el primer miembro 510 y un poste 511 roscado, mientras que la otra interfaz puede deslizarse debido a las superficies arqueadas. El tornillo 540 sin fin puede ser un tornillo sin fin de dos partes que incluye una primera porción 546 que tiene una primera sección 543 roscada y una segunda porción 548 que tiene una segunda sección 544 roscada que se ajusta a un poste 547 de la primera porción 546. Por lo tanto, las dos partes 546, 548 pueden girarse independientemente una de otra, con cada una accionando un manguito 520 de engranaje separado. Debido a que cada manguito 520 de engranaje roscado se puede acoplar por separado, se pueden distraer diferentes cantidades, dando como resultado un primer miembro 510 en ángulo, como se muestra más claramente en la figura 10D. Tal configuración acomoda la geometría lordótica o cifótica. Opcionalmente, los rebajes 517 arqueados en el primer miembro 550 y las superficies 515 arqueadas de los postes 511 podrían reemplazarse por juntas de flexión o por rótula o junta de cilindro y casquillo.
En las figuras 11A-11D se representa un dispositivo 600 de fusión del cuerpo intervertebral que se puede separar de acuerdo con otra realización de la presente invención. El dispositivo 600 utiliza tres mecanismos de manguito de engranaje helicoidal coaxial, teniendo cada uno un manguito 620 de engranaje roscado y un poste 621 roscado, entre el primer miembro 610 y el segundo miembro 650. Como se ve en las figuras 11C y 11d, para distraer el dispositivo, el accionamiento de tornillo 640 sin fin gira y se acopla con uno de los manguitos 620 de engranaje roscados, haciendo que gire. Las figuras 12A y 12B representan un dispositivo 700 de fusión del cuerpo intervertebral distractible que emplea solo un único mecanismo de engranaje de tornillo coaxial que tiene un manguito 720 de engranaje roscado y un poste 721 roscado para distraer el primer miembro 710 con respecto al segundo miembro 750 con el tornillo 740 sin fin. El dispositivo 700 también puede incluir primeros 774 y segundos 776 elementos de soporte telescópicos. Los elementos 774, 776 de soporte telescópicos sirven para mantener la posición giratoria relativa del primer miembro 710 con respecto al segundo miembro 750, permitir que el manguito 720 de engranaje roscado gire con respecto tanto al primer miembro 710 como al segundo miembro 750 para distraer al dispositivo 700. Las figuras 13A y 13B representan una variación adicional del dispositivo 700 que utiliza una pluralidad de espigas 778 que se extienden desde el primer miembro 710 y el segundo miembro 750 para restringir rotacionalmente el primer miembro 710 y el segundo miembro 750. En una realización, los dispositivos de fusión del cuerpo intervertebrales que se pueden separar como se describe en la presente memoria pueden estar hechos de titanio y el sistema de suministro puede estar hecho principalmente de acero inoxidable. Los componentes de cada mecanismo que se deslizan uno contra el otro pueden estar hechos de diferentes tipos del material general. Por ejemplo, el primer miembro puede estar hecho de Ti 6A1 4V titanio estándar, que tiene un alto rendimiento de fatiga suave, mientras que los manguitos de engranaje roscados pueden fabricarse a partir de Ti 6A1 4V ELI, que tiene un alto rendimiento de fatiga en muescas. En diversas realizaciones, el dispositivo está conformado para ser ergonómico. El dispositivo puede tener diversas formas, como, por ejemplo, rectangular, riñón o balón de fútbol. Un dispositivo con forma de riñón o de fútbol maximiza el contacto entre el dispositivo y los cuerpos vertebrales porque las placas terminales de las vértebras tienden a ser ligeramente cóncavas. Uno o ambos extremos del dispositivo también pueden estrecharse para facilitar la inserción. Esto minimiza la cantidad de fuerza necesaria para insertar inicialmente el dispositivo y separar los cuerpos vertebrales. Además, el dispositivo puede ser convexo a lo largo de su longitud y su ancho, o bi-convexo. El dispositivo se puede fabricar de diversas maneras con, en algunas realizaciones, diferentes componentes del dispositivo que se pueden fabricar de diferentes maneras. En una realización, el fresado de rosca puede implementarse para fabricar las diversas roscas en el dispositivo. El electro erosionado por rosca se puede utilizar para fabricar algunos o todos los orificios y aberturas en el dispositivo. Las plantillas de ensamblaje y los pasos posteriores al procesamiento también se pueden utilizar para permitir que el dispositivo se fabrique con los estándares más exigentes.
En algunas realizaciones, después de la distracción del dispositivo, un estimulante de crecimiento óseo, tal como autoinjerto, proteína morfogénica ósea, o material potenciador de hueso, puede administrarse en el dispositivo. En una realización, el estimulante del crecimiento óseo se suministra a través de una cámara hueca en la herramienta de inserción antes de que la herramienta de inserción se desacople del dispositivo. El dispositivo admite cargas in vivo durante el tiempo en que se produce la fusión entre los cuerpos vertebrales.
En una realización, la superficie del dispositivo puede tratarse para minimizar la rugosidad superficial o para reducir las picaduras del material dentro del cuerpo. Una superficie rugosa o picaduras pueden aumentar la tensión en el dispositivo, lo que puede ocasionar acortamiento de la vida de fatiga y/o reducir la resistencia a la fatiga. En una realización, la superficie puede tratarse con electro pulido, eliminando rebabas de los bordes del dispositivo y terminando la superficie. En otra realización, la superficie puede dejarse sin tratar porque una superficie rugosa en las placas extremas ayuda a evitar la extrusión accidental del dispositivo. En una realización, el dispositivo también se puede revestir con un material impermeable altamente elástico para prolongar su vida de fatiga. Específicamente, el material impermeable evitaría que las propiedades corrosivas de la sangre degraden el dispositivo. En otra realización, el dispositivo puede estar compuesto por un material biocompatible, de modo que no es necesario ningún recubrimiento. En una realización adicional, el dispositivo puede estar hecho de un material biodegradable diseñado para degradarse en el cuerpo en una etapa seleccionada del proceso de cicatrización, tal como después de la fusión ósea.

En diversas realizaciones, los dispositivos como se describen en este documento se pueden usar con diversos estimulantes del crecimiento óseo. En una realización, puede transportarse un soporte de andamiaje de proteína de fibroína de seda premineralizado en 3D sobre la superficie del dispositivo o dentro del mismo para administrar una proteína morfogenética ósea (BMP), que puede combinarse opcionalmente con células estromales de médula ósea modificadas (bMSC) para mejorar la fusión. En otras realizaciones, se puede usar un andamio de malla de fibra 3D de quitosano compuesto o un armazón de gelatina. El dispositivo también puede utilizar el factor de crecimiento endotelial vascular (VEGF) depositando VEGF inmovilizado sobre sustratos de aleación de titanio recubiertos con película de polidopamina adherente delgada para aumentar la unión, viabilidad y proliferación de células dérmicas humanas para promover el desarrollo del suministro de sangre al hueso fusionado a través de la revascularización alrededor del implante. En algunas realizaciones, podrían usarse ciertos polímeros tales como PLGA biodegradable para hacer un armazón para VEGF para potenciar la neovascularización y la regeneración ósea. En algunas realizaciones, VEGF puede usarse junto con BMP para inhibir la función de las BMP de promover la osteogénesis para permitir que el dispositivo se ajuste continuamente a lo largo del tiempo. En diversas realizaciones, los andamios en o alrededor del dispositivo podrían sembrarse con células madre derivadas de médula ósea, células madre derivadas de pulpa dental y células madre derivadas de tejido adiposo. Los andamios también pueden estar compuestos de diversos materiales incluyendo poliéster (por ejemplo, ácido poliláctico-co-ácido glicólico o poli-3- hidroxibutuerato-co-3-hydorxivalerato), seda (por ejemplo, biomimético, biomaterial poroso revestido de apatito basado en andamios de fibroína de seda), hidrogeles tales como policaprolactona, poliepsilon- caprolactona/colágeno (mPCL/Col) cospun con PEO o gelatina, mallas mPCL/Col con fibras del tamaño de micras, y microfibras mPCL/Col cosificadas con Heprasil, y titanio poroso y aleaciones de titanio (tales como una aleación de titanio-niobio-zirconio) funcionalizadas por una variedad de tratamientos superficiales, tales como un recubrimiento de VEGF o fosfato de calcio.
En algunas realizaciones, el dispositivo puede incluir una estructura adaptada para retener hueso dentro de un interior o adyacente al implante. Dicha estructura puede incluir una matriz o andamios de micronivel o ranuras, divots u otras características similares en el cuerpo del dispositivo. El hueso también puede retenerse mediante el uso de un material poroso tal que el hueso se retiene en los espacios intersticiales del material. También se pueden implementar funciones más extensas y extensibles. Tales características, tales como una cubierta circunferencial, también podrían tener la función adicional de endurecer el dispositivo en torsión.
En algunas realizaciones, se puede implantar más de un dispositivo de fusión del cuerpo intervertebral distractible según la presente invención en el espacio discal. Como se muestra en la figura 15, en una realización, se puede implantar un par de dispositivos 100 de forma tal que la superficie 104 externa del segundo miembro 150 de uno de los dispositivos 100 interactúe directamente con la superficie 102 exterior del primer miembro 110 del otro dispositivo 100. Tal configuración puede permitir el uso de un canal de acceso más pequeño para implantar los dispositivos. En una realización, las superficies 102, 104 cooperantes de los dos dispositivos son planas. Los dispositivos 100 pueden activarse simultáneamente o por separado. Los dispositivos también podrían voltearse uno con respecto al otro para tener ambos mecanismos de accionamiento centralmente ubicados. Además, los dispositivos podrían estar configurados para girar o flexionarse uno con respecto al otro para permitir que las superficies de soporte de los dispositivos ajusten su posición para acoplarse cómodamente con las placas terminales de los cuerpos vertebrales, o para preservar el movimiento de la columna vertebral.
En una realización, se puede usar una varilla y tornillos con el dispositivo como parte de un conjunto fijado al cuerpo vertebral. En otra realización, los miembros del dispositivo pueden extenderse y, de hecho, doblada sobre los lados de los cuerpos vertebrales adyacentes de modo que el dispositivo pueda fijarse a los cuerpos vertebrales con tornillos colocados a través de las extensiones de los miembros del dispositivo sustancialmente paralelas al plano formado por las placas terminales de los cuerpos vertebrales. En otra realización, el dispositivo de fusión del cuerpo intervertebral que se puede distraer puede comprender una placa terminal reforzada con flexiones para poder inclinarse de adelante hacia atrás y/o de lado a lado. Adicionalmente, los mecanismos de manguito de engranaje helicoidal coaxial que utilizan al menos en parte un material flexible pueden orientarse alrededor de la periferia del dispositivo para permitir la inclinación en una variedad de ejes. Un dispositivo de acuerdo con las diversas realizaciones puede usarse para una variedad de aplicaciones de fusión intervertebral, que incluyen, por ejemplo, cervical, torácica anterior lumbar, transforaminal lumbar, extremo lateral lumbar y posterior lumbar. Varias realizaciones de los procedimientos de implantación para estas aplicaciones pueden ser las siguientes:
Cervical: el dispositivo se implanta mediante un abordaje anterior en los niveles C3 a C7 utilizando autoinjerto. El dispositivo se usa con fijación de placa anterior suplementaria

El dispositivo se usa con una fijación de varilla posterior suplementaria.
Lumbar anterior: el dispositivo se implanta mediante un abordaje anterior desde los niveles L3 a S1 usando autoinjerto. El dispositivo se usa con fijación de placas anterior complementaria de fijación de varilla posterior.
Extrema lumbar lateral: el dispositivo se implanta mediante un abordaje lateral desde los niveles T12 a L4 utilizando autoinjerto. El dispositivo se usa con una fijación de varilla posterior suplementaria.
En otra realización, el dispositivo puede usarse en reemplazo de cuerpo vertebral. Después de la resección de un cuerpo vertebral o múltiples vértebras debido a una fractura o tumor, el dispositivo se puede distraer para unir dos vértebras separadas. El dispositivo distraído une y apoya el vacío que queda después de la resección. El dispositivo se puede construir en diferentes tamaños para acomodar la diferencia de tamaño de las vértebras cervicales, torácicas y lumbares.
En otra realización, el dispositivo se puede usar como un dispositivo de distracción interespinoso como se muestra en las figuras 16A y 16B. El dispositivo 800 se puede colocar entre dos procesos 801a, 801b espinosos adyacentes a través de un sistema de acceso mínimo. El dispositivo se puede insertar en una configuración contraída para facilitar la colocación. Una vez en posición, el dispositivo puede ser accionado a una configuración expandida con mecanismos 804 de manguito de engranaje helicoidal coaxial para bloquear las vértebras en una posición distraída. Los mecanismos 804 de manguito de engranaje helicoidal coaxial se pueden configurar como se describió previamente en el presente documento. El dispositivo puede tener dientes 800 de agarre en el punto de contacto con las apófisis 801a, 801b espinosas para ayudar a fijarlo en su lugar.
En otra realización, el dispositivo se puede usar para la fusión interespinosa. El dispositivo se puede colocar entre dos procesos espinosos adyacentes a través de un sistema de acceso mínimo en una configuración colapsada. Una vez en posición, el dispositivo puede activarse para bloquear la vértebra en una posición distraída. El dispositivo puede tener un mecanismo de bloqueo de perno o una disposición de bloqueo similar para bloquear el dispositivo en la posición distraída y para bloquear las placas de bloqueo a través de las apófisis espinosas. El dispositivo también puede tener corrugaciones de agarre o características en el exterior para ayudar a mantenerlo en su lugar. El material de autoinjerto o de fusión ósea se puede colocar en el espacio abierto en el dispositivo.
En otra realización, el dispositivo se puede usar como un dispositivo reductor de fractura distractible para hueso osteoporótico. El dispositivo se puede insertar debajo de una fractura de placa terminal a través de un abordaje de pedículo mínimamente invasivo. El dispositivo luego se acciona con un actuador del sistema de entrega. Una vez que se reduce la fractura, el dispositivo se explanta y el vacío se llena con cemento acrílico u otro relleno óseo que fortalecerá el hueso.
En otra realización, el dispositivo puede usarse en reemplazo de articulación facetaria. Después de la resección de una articulación facetaria hipertrófica, el dispositivo puede ser activado. Cada miembro se puede fijar a las vértebras adyacentes con un tornillo pedicular. Esto permitirá un movimiento similar al de una articulación facetaria y evitará la inestabilidad. El dispositivo puede ser parte de un sistema de dispositivo de fusión suave y se puede usar en combinación con un dispositivo de reemplazo de disco intervertebral. El mecanismo de manguito de engranaje helicoidal coaxial o poste roscado también se puede usar para expandir los dispositivos de reemplazo de disco intervertebral.
En otra realización, el dispositivo se puede usar como una jaula de distracción programable con un dinamizador y estimulador óseo. Se puede implantar un dispositivo accionador de micro máquina programable dentro del dispositivo. El dispositivo se distrae durante la implantación y puede proporcionar lecturas de fuerza a través de un comunicador de radiofrecuencia después de la cirugía. La forma del dispositivo se puede alterar mientras se implanta al distraer los miembros con el dispositivo actuador, lo que puede provocar lordosis, cifosis, una mayor

distracción o menos distracción. En una realización, un dispositivo de batería alimenta el sistema y también puede formar un campo magnético que funciona como un estimulador óseo. La duración de la batería puede estar limitada a un corto período de tiempo, como una semana. Pequeños movimientos del dispositivo se pueden usar para generar energía eléctrica con piezoeléctricos o polímeros conductores que pueden usarse para recargar las baterías, condensadores u otros dispositivos de almacenamiento de energía. Alternativamente, el dispositivo puede ser alimentado a través de una disposición inductiva de RF o acoplada de forma capacitativa.
En otra realización, el dispositivo puede ser una jaula de distracción autoactiva. El dispositivo se puede insertar en el espacio del disco en un estado colapsado. Una vez que se libera el dispositivo, puede distraer lentamente a una altura preestablecida
En otra realización, el dispositivo se puede usar en cirugía maxilar facial como un dispositivo de alargamiento de la fractura para fracturas mandibulares. El dispositivo puede diseñarse con miembros estrechos que tienen placas perpendiculares con orificios que permiten la fijación de cada miembro a una fractura proximal o distal. El dispositivo puede ser accionado a una altura preestablecida. Esto permitirá el alargamiento del defecto en casos de pérdida ósea por fractura, displasia o hipoplasia.
En otra realización, el dispositivo se puede usar en aplicaciones ortopédicas como un clavo alargador para la distracción de fracturas de huesos largos. Después de que se produce una fractura ortopédica con pérdida ósea, se puede colocar un clavo alargado que se puede distender para alargar el hueso. El alargamiento se produce durante unos días con movimientos micrométricos. Esta aplicación implicará un dispositivo de distracción insertado entre la parte móvil de las uñas que ejerce fuerzas de contradisciplina, lo que proporcionará un alargamiento del hueso.
En otra realización, el dispositivo se puede usar para reemplazar las articulaciones falángicas en la mano, las articulaciones metatarsianas en el pie o las articulaciones calcáneo-astragalinas. Estas articulaciones pueden tener implantes que permitirán el movimiento de los huesos adyacentes y limitarán la hiperextensión o hiperflexión.
Las figuras 17A y 17B representan un dispositivo 900 distractible que incluye un manguito de engranaje helicoidal coaxial envolvente con cojinetes de recirculación de acuerdo con otra realización de la presente invención. El dispositivo 900 incluye un poste 910, un manguito 920 de engranaje helicoidal coaxial envolvente, un tornillo 930 sin fin y una carcasa 940. El poste 910 incluye una superficie 912 exteriores lisa y un canal 911 de rodadura helicoidal mecanizada para los cojinetes 913. También se mecaniza 8un canal de rodadura helicoidal (no mostrada) en la superficie interior del manguito 920 de engranaje helicoidal coaxial envolvente que es complementario al canal 911 de rodadura helicoidal para alojar los cojinetes 913. Esto provoca simultáneamente que el poste 910 se traduzca (pero no gire) fuera del manguito 920 de engranaje helicoidal coaxial y lejos de la carcasa 940. Los cojinetes 913, 914 permiten la rotación del manguito 920 de engranaje helicoidal coaxial envolvente con muy poca fricción, lo que permite que el dispositivo 900 presente una ventaja mecánica muy alta y un control de desplazamiento con una resolución muy alta. Con referencia ahora a las figuras 18A-18D, se puede ver otro dispositivo 1000 distractible que utiliza un manguito de engranaje helicoidal coaxial de acuerdo con una realización de la presente invención. El dispositivo 1000 incluye un engranaje 1010 helicoidal coaxial envolvente, un alojamiento 1020 y un tornillo 1030 sin fin. La superficie exterior del manguito 1010 de engranaje helicoidal coaxial envolvente incluye una ranura helicoidal que tiene una serie de dientes 1014 de engranaje helicoidal coaxial. La ranura helicoidal puede cooperar con una rosca 1021 interna en la superficie 1022 interior de la carcasa 1020 para permitir que el dispositivo 1000 transporte una carga axial. En otra realización, los dientes de engranaje 1014 se pueden mecanizar directamente en la superficie exterior del manguito 1010 de engranaje helicoidal coaxial envolvente. En una realización, la superficie exterior del manguito 1010 de engranaje helicoidal coaxial envolvente puede ser una superficie mecanizada lisa que actúa como una superficie de apoyo cuando está configurada con una superficie de apoyo lisa similar en la superficie 1022 interna de la carcasa 1020 para permitir que el dispositivo 1000 transporte una carga lateral.
Para expandir el dispositivo 1000, el tomillo 1030 sin fin se hace girar para enganchar los dientes 1014 del engranaje de helicoidal coaxial envolvente para girar y trasladar el manguito 1010 de engranaje helicoidal coaxial envolvente con respecto a la carcasa 1020. En una realización, la superficie 1010 interna y el agujero 1012 central se pueden configurar para contener un poste similar al poste 910 descrito en las figuras 17A y 17B para componer la distracción 5 del dispositivo. En una realización, no hay rosca 1021 presente en la superficie 1022 interior de la carcasa 1020, de modo que la ranura helicoidal y/o los dientes 1014 de engranaje del manguito 1010 de engranaje de tornillo coaxial envolvente hacen que el manguito 1010 se traslade con respecto a la carcasa 1030 a medida que gira el manguito 1010. - 3. Dispositivo de distracción según la reivindicación 1, en donde hay un par de mecanismos de manguito de engranaje helicoidal coaxial dispuestos entre la primera superficie (102) de apoyo y la segunda superficie (104) de apoyo que son accionadas por el mecanismo de accionamiento.

El dispositivo distraíble de la reivindicación 3, en donde la superficie interior roscada (122) y la superficie (124) exterior de engranaje del manguito (120) de uno de los mecanismos de manguito helicoidal coaxial tienen una orientación de paso de tornillo generalmente opuesta a la orientación de paso de tornillo de la superficie interior roscada (132) y superficie (134) exterior de engranaje del otro mecanismo de manguito de engranaje helicoidal coaxial.
- 5. Dispositivo distractible según la reivindicación 1, en donde el mecanismo de accionamiento es un accionamiento (140, 540, 640, 740, 930) de tornillo sin fin.

Dispositivo distractible según la reivindicación 3, en donde el mecanismo de accionamiento es un accionamiento (140, 540, 640, 740, 930) de un tornillo sin fin que tiene un par de secciones (141, 142) roscadas, cada sección roscada configurada para interconectarse con uno de los mecanismos de manguito de engranaje helicoidal coaxial.
- 7. Dispositivo distractible según la reivindicación 1 o 3, en donde el mecanismo de accionamiento está estacionario de manera translacional cuando el mecanismo de accionamiento acciona la superficie (124, 134, 922) exterior de engranaje del manguito (120, 130, 520, 620, 720, 920).

Dispositivo de distracción según la reivindicación 8, en donde el mecanismo de accionamiento se conecta con la superficie (124, 134, 922) exterior de engranaje del manguito (120, 130, 520, 620, 720, 920) a través de una abertura (157, 158) en la rosca (151, 152) interna de la abertura (161, 162) del manguito.5 10. Dispositivo distractible según la reivindicación 1, en donde una porción del manguito (120, 130, 520, 620, 720,920) también sobresale hacia fuera a través de la otra de la primera superficie (102) de apoyo y la segunda superficie (104) de apoyo.
