Cementos de fosfato de
amonio-magnesio, su producción y utilización.
La invención se refiere a una preparación de
cemento de fosfato de amonio-magnesio, a su
utilización para producir un cemento de fosfato de
amonio-magnesio, así como a su utilización para
otros fines.
Esta invención se refiere, en particular, a un
cemento biodegradable que, después de su fraguado, se compone en
su fase principal de fosfatos de amonio-magnesio
y de nanoapatitas y que al mismo tiempo presenta una gran
resistencia. El material puede utilizarse como material sustitivo
óseo, para aumentar la materia ósea y para la regeneración de los
huesos. Puede servir como material portador para principios activos
farmacéuticos o biológicos.
Los componentes minerales más importantes del
hueso humano y del tejido duro dental son el calcio y fosfatos. Sin
embargo, también existen considerables cantidades de sodio, magnesio
y carbonatos.
Gracias a estudios de precipitación de sistemas
sintéticos se sabe que los iones sodio y carbonato pueden
incorporarse muy fácilmente en precipitados de
calcio-fosfato lo que resulta en una estructura
molecular similar a la apatita. Sin embargo, el magnesio tiene una
fuerte tendencia a precipitar en una estructura diferente no del
tipo apatita.
Los fosfatos cálcicos precipitados
fisiológicamente como huesos y dentina es nanocristalino. En el
difractograma de rayos X no se puede reconocer si se trata de una
estructura de tipo apatita o de otra estructura debido al
ensanchamiento de líneas. Algunos científicos son de la opinión que
en los huesos y la dentina existe tanto magnesio que toda su
cantidad no puede incluirse en la estructura apatita. Por esta
razón, se supone que se trata aquí de una forma de mezcla mineral de
nanoapatita y nanodolomita o bien nanoestruvita. Los fosfatos
cálcicos no solamente son biocompatibles sino que también son
reconocidos por las células vivas como parte del cuerpo. Por esta
razón existen muchos materiales biológicos y productos médicos que
se componen parcialmente de fosfato cálcico.
Desde 1970 existen materiales cerámicos de
fosfato cálcico en el mercado, parte en forma de bloques
prefabricados o bien en forma de granulado. El implante de estos
materiales en estructuras óseas ha tenido gran éxito. La mayor
desventaja de estos sistemas es que los bloques han de prefabricarse
y que los granulados se alejan (debido al lavado) del lado del
implante, lo que con frecuencia conduce a que tales implantes
fracasen.
Los materiales de fosfato cálcico cerámico
tienen mucho éxito si están compuestos de hidroxilapatita (HA) o de
fosfatos cálcicos beta-terciarios
(\beta-TCP, una estructura del tipo whitloctita) o
si los materiales cerámicos de fosfato cálcico se componen tanto de
HA como de \beta-TCP en proporciones variables.
Como implante óseo, la HA prácticamente no se reabsorbe, mientras
que el \beta-TCP se reabsorbe lentamente y es
reemplazado por nuevos huesos. Por tanto, es posible actuar sobre el
grado de reabsorción del material cerámico de fosfato cálcico
variando la proporción \beta-TCP/HA. También es
posible añadir a la mezcla otras sustancias reabsorbibles como son
monetita CaHPO_{4}, brushita CaHPO_{4} \cdot 2H_{2}O,
calcita CaCO_{3} y dolomita
CaMg(CO_{3})_{2}.
Desde 1985 se ha intentado desarrollar cementos
de fosfato cálcico para evitar las desventajas de los materiales
cerámicos de fosfato cálcico prefabricados o en forma de granulado
(W.E. Brown y L.C. Chow, "A new calcium phosphate,
watter-setting cement", Cem. Res. Prog. 1986,
352-379 (1987)).
Entre éstos se encuentra un cemento de brushita,
que todavía no se encuentra en el mercado, con una relación molar
Ca/P de la fase precipitada de 1,00. Esta fase no es nanocristalina
sino microcristalina.
Todos los demás cementos de fosfato cálcico
desarrollados hasta la fecha forman una estructura de precipitación
nanocristalina y tienen una relación molar Ca/P \geq 1,5, que
todavía se puede aumentar mediante la adición de carbonato. Estos
materiales son conocidos de U.S. 5.605.713, EP 0 835 668 A1, WO
96/14265 y algunos de los mismos ya se encuentran en el mercado.
En cuanto a la posibilidad de reabsorción de
estos materiales después del implante en huesos y en tejidos blandos
existen informes contradictorios.
En cada caso se distingue entre cementos de
fosfato cálcico basados en hidroxilapatita (HA), que son no
reabsorbibles (cerámicos HA, véase más arriba), y cementos de
fosfato cálcico basados en hidroxilapatitas de calcio deficientes
(CDHA, Calcium deficient Hydroxyapatite), que tienen una buena
osteotransductividad.
En este último caso, esto significa que pueden
ser reabsorbidos por los osteoclastos y ser sustituidos por
osteoplastos en los nuevos tejidos óseos. En la reabsorción de estos
cementos influye decisivamente el mecanismo de reestructuración
local del hueso.
Actualmente, la mayoría de los cirujanos
desearían un cemento de fosfato cálcico sobre el que existiera, en
primer lugar, un efecto de apoyo mecánico, donde, sin embargo, la
reabsorción final tuviera lugar independientemente del mecanismo de
reestructuración local del hueso; es decir, que el material se
desintegrara por completo. Además, en el campo de la ortopedia es
bien conocido que el hueso vivo solamente permanece donde es
necesario desde el punto de vista biomecánico. Esto se conoce como
la Ley de Wolff. Si, por lo tanto, un cemento de fosfato cálcico
introducido en un defecto óseo tiene una mayor resistencia a la
presión que el hueso que lo rodea y si esta resistencia a la presión
se mantiene inalterada, esto conduce a una desintegración del tejido
óseo situado alrededor del implante (aquí el cemento de fosfato
cálcico).
Para cumplir este requisito, aunque sea
solamente en parte, algunos productores han mezclado con sus
cementos de CDHA, similares a la nanoapatita, sustancias que son
reabsorbidas pasivamente por los líquidos corporales debido a
los
gradientes de concentración, por ejemplo monetita (CaHPO_{4}) o calcita (CaCO_{3}), como es sabido de la EP 0 543 765.

Sin embargo, de esta forma se soluciona el
problema tan sólo en parte. Sigue siendo necesario un cemento que se
pueda reabsorber pasivamente por completo y en el que el frente de
reabsorción y el frente de sedimentación estén en contacto
directo.
El yeso, por ejemplo, no cumple este requisito.
El yeso se reabsorbe con tanta velocidad que siempre queda un
intersticio entre el frente de reabsorción y el frente de
sedimentación y estos materiales no tienen una función de apoyo
suficiente debido a su baja estabilidad a la presión. Tales
materiales han sido revelados, por ejemplo, en la U.S.
5.281.265.
Por estas razones es deseable proporcionar un
material de adición ósea que de momento se encargue, presentando una
alta estabilidad a la presión, de la función perdida de apoyo al
hueso, sin embargo reduciendo después sucesivamente su estabilidad a
la presión gracias a lo cual se estimulan los procesos propios del
cuerpo de remodelación ósea (remodelling) y, por tanto, estimulan
una osteoneogénesis más rápida y con ello también iniciando una
reabsorción activa del material de adición ósea. Esto también se
puede conseguir, por ejemplo, incorporando una sustancia de fácil
disolución en una pasta de cemento endurecida. Ya que el hueso crece
bien en estructuras macroporosas es ventajoso mezclar con la pasta
de cemento sustancias solubilizantes en forma de granulado o
pastillas compuestas de, por ejemplo, azúcares, sales (por ejemplo
NaCl) o yeso (CaSO_{4}). Éstas se disuelven entonces muy
rápidamente en el cuerpo desde la estructura endurecida del cemento
y queda únicamente una estructura porosa esponjosa. También se puede
pensar en producir los cementos porosos (acabados) fuera del
cuerpo.
Para la utilización de un cemento en
aplicaciones dentales, por ejemplo como relleno y cierre de pequeños
canales de dentina después de una extirpación viva, para la
utilización de un cemento de este tipo como material de relleno
radicular en endodoncias, los materiales de este tipo no pueden
encogerse para evitar el paso de bacterias. Incluso sería deseable
un material con características de expansión gradual reducida.
El objetivo de la invención consiste en
proporcionar un preparado de cemento con el que se pueden evitar las
desventajas del estado de la técnica.
Este objetivo se alcanza gracias a un preparado
de cemento de fosfato de amonio-magnesio según las
reivindicaciones 1, 13 y 15, mediante un procedimiento para la
producción de un cemento de fosfato de
amonio-magnesio utilizando el preparado según la
reivindicación 23 y gracias a la utilización según las
reivindicaciones 26 a 28. En las subreivindicaciones se indican
desarrollos de la invención.
Los problemas con el estado de la técnica se
solucionan con la invención presente, en especial debido a que
variando la adición de sales de estroncio se puede ajustar la
capacidad de expansión de la pasta de cemento endurecida. En los
ensayos, tal como se muestra en los ejemplos, se ha mostrado que la
capacidad de expansión de la mezcla de cemento, cuya fase principal
representa, en estado endurecido, el fosfato de
amonio-magnesio, se reduce a medida que aumenta la
proporción de sales de estroncio en la mezcla de polvo total. Por
tanto, se puede proporcionar con esta invención un material para la
endodontología el cual, además de una estabilidad sólida mecánica
suficiente, también presenta características de expansión.
Un objeto de esta invención es proporcionar un
material como sustitutivo del hueso para el aumento y la
regeneración óseos, material que puede reabsorberse en un tiempo
limitado y cuya estabilidad frente a la presión puede reducirse de
forma adaptada a las exigencias de regeneración del cuerpo.
Otro objeto de esta invención es proporcionar un
material que se puede realizar, preparar y modelar bajo condiciones
normales de temperatura, preferentemente a la temperatura corporal;
esto es, un cemento.
Una característica del material puesto a
disposición es que, además, se puede ajustar su tiempo de
procesamiento, especialmente a temperatura ambiente, debido a la
fuerza del grado de sinterización del
Mg_{3}(PO_{4})_{2}, gracias a lo cual se
controla la velocidad de disolución en la superficie de estas
partículas según el grado de sinterización y la densidad resultante
del Mg_{3}(PO_{4})_{2} utilizado, de manera que
se puede controlar la precipitación necesaria para la solidificación
del compuesto Ca/Mg/fosfato precipitado.
Otro objeto de esta invención es proporcionar un
cemento de fosfato de solubilidad parcial, preferentemente gracias a
la solubilidad lenta del cemento de fosfato de
amonio-magnesio de estructura apatita.
Otro objeto de la presente invención es
describir un proceso de reacción en el que a partir de una cantidad
de componentes individuales se forma un cemento de fosfato de
amonio-magnesio y que endurece en un tiempo
clínicamente aceptable a la temperatura ambiente y/o corporal.
Otro objeto de la presente invención consiste en
proporcionar un material que, en un tiempo clínicamente aceptable,
endurece y queda lo suficientemente sólido y que tiene una gran
capacidad de adhesión sobre superficies mineralizadas.
Otro objeto de la invención consiste en que el
material revelado según la técnica de la invención se destaca por
una gran capacidad de adhesión sobre superficies metálicas.
Otro objeto de la presente invención consiste en
proporcionar un cemento reabsorbible que se puede inyectar en la
etapa de pasta mezclada.
Un aspecto de esta invención consiste en que el
producto final se compone de una mezcla de polvo con una relación
molar Ca/P en el intervalo de 1,00 a 1,50 (P significa
ortofosfato).
Además, es esencial que la relación molar Mg/P
de esta mezcla de polvo oscile en el rango de 0 a 1,00.
Para mezclar y formar una pasta de cemento que
se endurece en un tiempo aceptable, estas mezclas de polvo han de
ser lo suficientemente reactivas. Para conseguir esto, otro aspecto
de esta invención consiste en mezclar con las mezclas de polvo
cantidades adecuadas de soluciones acuosas ligeramente alcalinas
(7<pH<12) de ingredientes iónicos solubles como
Na_{3}PO_{4}, K_{2}CO_{3} y/o Na_{2}CO_{3} en
combinación con (NH_{4})_{2}HPO_{4}.
Otra característica preferente de esta invención
consiste en que con la pasta de cemento endurecida se mezclan
sólidos granulados pero fácilmente solubles en los líquidos
corporales, de manera que, después de su disolución, se produce un
sistema micro- a macroporoso.
Una ventaja de esta invención es que estos
cementos alcanzan su solidez máxima en el transcurso de unas pocas
horas.
Otra característica preferente de esta invención
consiste en la capacidad de expansión del cemento durante el
fraguado. La capacidad de expansión se determina, o bien se ajusta,
según la cantidad relativa añadida de una sal de estroncio.
Otra característica preferente de esta invención
consiste en que el cemento endurecido se compone de fosfato de
amonio-magnesio microcristalino.
Otra característica preferente de esta invención
consiste en que el tiempo inicial de endurecimiento del cemento
puede ajustarse a 1 hasta 40 minutos y el tiempo final de
endurecimiento a 2,5 hasta 60 minutos (según ASTM
C266-89).
Otra característica preferente de esta invención
consiste en que el cemento puede alcanzar una resistencia máxima a
la compresión superior a 50 MPa.
Otra característica preferente de esta invención
consiste en que la pasta de cemento puede inyectarse antes de
alcanzarse el tiempo de endurecimiento inicial.
Otra característica preferente de esta invención
consiste en que la pasta de cemento puede servir como material
portador para otras sustancias que no sean Ca, Mg y/o fosfato, por
ejemplo ZnO, para principios activos farmacéuticos (antibióticos,
citostáticos, factores de crecimiento) o para otras sustancias
bioactivas.
Otras características y funcionalidades de la
invención resultan de la descripción de los ejemplos de
realización.
En los ejemplos se utilizan los siguientes

Claims (28)
1. Preparación de cemento de fosfato de
amonio-magnesio que comprende:
- -
- una mezcla de polvo con cantidades molares de los componentes calcio (Ca), magnesio (Mg) y ortofosfato (P) en la mezcla en los intervalos 1,00 < Ca/P < 1,50 y 0 < Mg/P < 0,50,
- -
- una sal de amonio y
- -
- en caso dado agua y/o una solución acuosa.
2. Preparación según la reivindicación 1,
caracterizada porque la solución acuosa es una solución
acuosa de una sal de amonio con un pH en el intervalo 7 < pH <
12.
3. Preparación según la reivindicación 1,
caracterizada porque la sal de amonio está incluida en la
mezcla de polvo y porque las cantidades molares de los componentes
de calcio (Ca), magnesio (Mg), ortofosfato (P) y amonio (NH_{4})
oscilan en el intervalo 1,00 < Ca/P < 1,50 y 0 < Mg/P <
0,50 y 0 < NH_{4} < 0,50.
4. Preparación según una de las reivindicaciones
1 a 3, caracterizada porque la mezcla de polvo comprende un
fosfato cálcico \alpha/\beta-terciario
(\alpha/\beta-TCP) y, de preferencia,
MgHPO_{4}·3H_{2}O.
5. Preparación según una de las reivindicaciones
1 a 3, caracterizada porque la mezcla de polvo contiene
Mg_{3}(PO_{4})_{2} , además del
\alpha/\beta-TCP y MgHPO_{4}·3H_{2}O.
6. Preparación según una de las reivindicaciones
1 a 5, caracterizada porque la solución acuosa comprende una
solución acuosa de (NH_{4})_{2}SO_{4}.
7. Preparación según una de las reivindicaciones
1 a 6, caracterizada porque la mezcla de polvo comprende
(NH_{4})_{2}SO_{4}.
8. Preparación según una de las reivindicaciones
1 a 7, caracterizada porque el líquido de mezcla se compone
de una solución acuosa de (NH_{4})_{2}HPO_{4}.
9. Preparación según una de las reivindicaciones
1 a 8, caracterizada porque la mezcla de polvo comprende
además KH_{2}PO_{4}.
10. Preparación según una de las
reivindicaciones 1 a 9, caracterizada porque la mezcla de
polvo comprende además Na_{2}HPO_{4}.
11. Preparación según una de las
reivindicaciones 1 a 10 caracterizada porque comprende además
SrCO_{3}.
12. Preparación según la reivindicación 11,
caracterizada porque el contenido de SrCO_{3} es del 0,01
al 10% en peso, preferentemente del 0,1 al 5% en peso, con respecto
al peso total de la mezcla.
13. Preparación de cemento de fosfato de
amonio-magnesio que comprende:
- -
- una mezcla de polvo compuesta de MgHPO_{4}·3H_{2}O, KH_{2}PO_{4}, Na_{2}HPO_{4} y \alpha-TCP y \beta-TCP ó \alpha/\beta-TCP y, en caso dado, Mg_{3} (PO_{4})_{2},
- -
- iones amonio y
- -
- en caso dado, agua y/o una solución acuosa.
14. Preparación según la reivindicación 13,
caracterizada porque comprende, como iones amonio, una
solución acuosa de una sal de amonio en forma de un líquido de
mezcla.
15. Preparación de cemento de fostato de
amonio-magnesio que comprende:
- -
- una mezcla de polvo compuesta de \alpha-TCP, \beta-TCP, KH_{2}PO_{4}, Na_{2}HPO_{4} y o bien (NH_{4})_{2}SO_{4} o bien fosfato de amonio primario o secundario,
- -
- una sal de magnesio, de preferencia MgHPO_{4}·3H_{2}O; y
- -
- en caso dado, agua y/o una solución acuosa.
16. Preparación según la reivindicación 15,
caracterizada porque comprende, como líquido de mezcla, una
solución acuosa de una sal de magnesio.
17. Preparación según una de las
reivindicaciones 13 a 16, caracterizada porque comprende
además SrCO_{3} en la mezcla de polvo, preferentemente en una
cantidad del 0,01 al 10% en peso, con respecto al peso total de la
mezcla.
18. Preparación según una de las
reivindicaciones 1 a 17, caracterizada porque comprende, como
líquido de mezcla, una solución acuosa de una sal de estroncio.
19. Preparación según una de las
reivindicaciones 1 a 18, caracterizada porque comprende, como
componente adicional, ZnO en la mezcla de polvo y/o en el líquido de
mezcla.
20. Preparación según una de las
reivindicaciones 1 a 19, caracterizada porque la mezcla de
polvo contiene además Ca_{2}NaK(PO_{4})_{2} y/o
CaKPO_{4}.
21. Preparación según una de las
reivindicaciones 1 a 20, caracterizada porque comprende, como
componente adicional, iones fluoruro en la mezcla de polvo y/o en el
líquido de mezcla.
22. Preparación según una de las
reivindicaciones 1 a 21, caracterizada porque comprende, como
componentes adicionales, principios farmacéuticos y/o bioactivos en
la mezcla de polvo y/o en el líquido de mezcla, preferentemente
antibióticos, citostáticos, analgésicos, desinfectantes, factores de
crecimiento, proteínas o inhibidores de elastina, en dosis
terapéuticas.
23. Procedimiento para la preparación de un
cemento de fosfato de amonio-magnesio utilizando una
preparación de cemento de magnesio-amonio según una
de las reivindicaciones 1 a 22, caracterizado porque la
mezcla de polvo se mezcla con el líquido de mezcla consiguiendo una
distribución uniforme del líquido en la mezcla de polvo y porque la
pasta así obtenida se aplica o introduce en la zona de destino y
porque se deja endurecer con la formación de un cemento que contiene
fosfato de amonio-magnesio microcristalino.
24. Procedimiento según la reivindicación 23,
caracterizado porque a la mezcla de polvo se añaden
partículas granuladas fácilmente solubles en líquidos acuosos, de
entre aproximadamente 10 \mum y aproximadamente 300 \mum,
preferentemente de entre 50 \mum y 200 \mum.
25. Procedimiento según la reivindicación 24,
caracterizado porque las partículas granuladas se componen,
de preferencia, de NaCl, azúcares, CaSO_{4},
\beta-TCP, poliláctidos, poliglicólidos o bien
copolímeros poliláctido/poliglicólido, CaCO_{3}, CaHPO_{4}.
26. Utilización de una preparación según una de
las reivindicaciones 1 a 22 para la producción de un medio para
fines médicos.
27. Utilización de una preparación según una de
las reivindicaciones 1 a 22 para la producción de un cemento
dental.
28. Utilización de una preparación según una de
las reivindicaciones 1 a 22 para la producción de un sustitutivo
óseo o de un material de relleno óseo o de un cemento óseo o de un
adhesivo óseo.
