Abstract

Description
Sistema de filtración para vaso.
El presente invento se refiere en general a
dispositivos y métodos usados en la captura de material embólico en
los vasos sanguíneos. Más específicamente, los dispositivos y
métodos proveen un sistema de filtración de vaso para su despliegue
provisional en arterias como las arterias carótidas y la aorta, y
venas como la vena subclavia y la vena cava superior. El sistema
incluye también un alambre-guía para dirigir
dispositivos endovasculares, por ejemplo, catéteres de aterectomía,
catéteres de despliegue de endoprótesis, o catéteres de
angioplastia, a una región de interés y un catéter de guiado con
capacidad de lavado a presión con fluido para ayudar a la
filtración.
Recientemente se ha demostrado que el
tratamiento de las lesiones trombóticas y ateroescleróticas en los
vasos sanguíneos usando el concepto endovascular es una alternativa
eficaz y fiable a la intervención quirúrgica en pacientes
seleccionados. Por ejemplo, la aterectomía direccional y la
angioplastia coronaria transluminal percutánea (en adelante ACTP)
con o sin despliegue de endoprótesis son útiles en el tratamiento de
pacientes con oclusión coronaria. La aterectomía elimina físicamente
placas mediante corte, pulverización, o raspado en arterias
ateroescleróticas usando un dispositivo de endoarteriectomía
descargable por catéter. La angioplastia ensancha el diámetro
lumenal de un vaso estenótico ejerciendo una fuerza mecánica sobre
las paredes vasculares. Además de usar la angioplastia, las
endoprótesis, y/o la aterectomía en la vasculatura coronaria, estas
técnicas endovasculares han demostrado también que son útiles en el
tratamiento de otras lesiones vasculares, por ejemplo, en estenosis
de la arteria carótida, oclusión arterial periférica, (especialmente
la aorta, la arteria iliaca y la arteria femoral), la estenosis de
arteria renal causada por ateroesclerosis o por enfermedad
fibromuscular, síndrome de la vena cava superior, y trombosis por
oclusión de la vena iliaca resistente a la trombólisis.
Es un hecho bien reconocido que una de las
complicaciones relacionadas con las técnicas endovasculares es el
desprendimiento de materiales embólicos generados durante la
manipulación del vaso, causando de ese modo la oclusión de los vasos
más estrechos aguas abajo y la isquemia o el infarto del órgano que
alimenta el vaso. En 1995,
Waksman y colaboradores descubrieron que la embolización distal es común después de una aterectomía direccional en injertos en arterias coronarias y vena safena. Véase Waksman y colaboradores, American Heart Journal 129 (3): 430-5 (1995). Este estudio averiguó que la embolización distal ocurre en el 28% (31 de 111) de los pacientes que sufren aterectomía. En enero de 1999, Jordan, Jr, y colaboradores descubrieron que el tratamiento de la estenosis carótida usando angioplastia percutánea con procedimiento de endoprótesis está relacionado con más de ocho veces la tasa de microembolias vistas usando endarteriectomía de carótida. Existe una serie de dispositivos diseñados para
proporcionar la filtración de sangre para la compresión de émbolos
vasculares. La inmensa mayoría de estos dispositivos se ha diseñado
para su instalación permanente en venas con el fin de prevenir la
embolia pulmonar. En la patente de EE.UU. Nº 5.053.008 expedida a
Bajaj se describe un filtro venoso provisional. El dispositivo de
Bajaj es un catéter intracardíaco para instalación provisional en el
tronco pulmonar de un paciente predispuesto a la embolia pulmonar
debida, por ejemplo, a cirugía de cadera, traumatismo grave, cirugía
mayor abdominal o pélvica, o inmovilización. El dispositivo de Bajaj
incluye una sombrilla construida de malla que atrapa los émbolos
venosos antes de que lleguen a los pulmones. Este dispositivo se ha
diseñado para filtración venosa y no es apropiado para uso arterial,
debido a las diferencias hemodinámicas entre las arterias y las
venas.
Hay muy pocos dispositivos intravasculares
diseñados para uso arterial. Las arterias son mucho más flexibles y
elásticas que las venas y, en las arterias, la circulación de la
sangre es pulsátil con grandes variaciones de presión entre el flujo
sistólico y diastólico. Estas variaciones de presión dan lugar a que
las paredes de las arterias se dilaten y contraigan. Los caudales
de la sangre en las arterias varían desde aproximadamente 1 hasta
alrededor de 5 L/minuto. Ginsburg, autor de la patente de EE.UU. Nº
4.873.978, describe un sistema de filtración arterial, que incluye
un catéter con un dispositivo de filtro en su extremo distal. Este
dispositivo se introduce en el vaso aguas abajo del lugar de
tratamiento y, después del tratamiento, el filtro se aplasta
alrededor de los émbolos comprimidos y se retira del cuerpo. El
dispositivo de Ginsburg no podría soportar caudales de 5L/minuto. Se
ha diseñado solamente para arterias pequeñas y por tanto no podría
capturar émbolos destinados para todas las partes del cuerpo. El
ingeniero Walter Hengst de GMBH &Co, autor de la patente alemana
DE 34 17 738, describe también otro filtro arterial que tiene un
sistema de articulación plegable que convierte al filtro del estado
aplastado al estado dilatado.
Se han propuesto filtros fijados al extremo
distal de alambres-guía para la filtración
intravascular de sangre. Una mayor parte de estos dispositivos
incluyen un filtro que está fijado a un alambre-guía
y se acciona mecánicamente por medio de puntales o por un cesto
preconformado que se despliega en el vaso. Estos filtros son
típicamente "paracaídas" de malla que se fijan al eje del
alambre en el extremo distal y a unos puntales del alambre que se
extienden hacia fuera en una dirección radial sobre el extremo
proximal. Los puntales radiales abren el extremo proximal del filtro
a la pared del vaso. La sangre que circula a través del vaso es
forzada a atravesar la malla, capturando de ese modo en el filtro el
material embólico. Un inconveniente importante relacionado con estos
dispositivos de filtro es que los filtros generalmente se basan en
la circulación vascular de la sangre para impulsar los residuos en
los filtros. Si se llega a restringir la circulación de la sangre en
el vaso, el material embólico fijado holgadamente podría no estar
sometido a una circulación turbulenta normal de la sangre. Las
partículas embólicas podrían permanecer en el vaso próximas al
filtro hasta que se restableciese la circulación normal de mayor
intensidad (es decir, cuando se retirase el filtro), reduciendo de
ese modo el rendimiento de los dispositivos de filtro.
Otros medios de extraer material embólico
utilizan dispositivos de oclusión provisional, tales como catéteres
de oclusión con globo y pinzas vasculares, para aislar una sección
del vaso. Una vez que se ha aislado la circulación de sangre en el
vaso, se aspira el fluido o la sangre contenidos en el vaso para
eliminar los residuos embólicos. Una de las ventajas relacionadas
con los dispositivos de oclusión es que requieren cesar o reducir
provisionalmente la perfusión distal que podría afectar a la
oxigenación de órganos distales. Se podrían colocar derivaciones en
puntos distales a los dispositivos para mantener la perfusión a
órganos distales. Sin embargo, la inserción de las derivaciones crea
traumartismos adicionales al vaso y podría generar material embólico
adicional.
Lo que se necesitan son dispositivos de
filtración sencillos y seguros que se puedan instalar
provisionalmente en las arterias y venas y que se puedan usar con
instrumentos endovasculares para impedir de un modo efectivo la
embolización distal. Los dispositivos actuales son inadecuados para
este fin.
El documento
US-A-5910154, que representa la
técnica anterior más aproximada al asunto de las reivindicaciones,
describe un alambre-guía de filtro sobre el que se
puede hacer avanzar un catéter con despliegue de endoprótesis.
Los documentos WO 97/42879 y WO 99/30766
describen oclusores aórticos que tienen filtros asociados en los
mismos.
Las placas fijas o móviles presentes en la aorta
pueden desprenderse y causar infarto renal o isquemia a otros
órganos. La acumulación de placas en las arterias carótidas plantea
también un riesgo de apoplejía isquémica por embolización y presenta
una amenaza adicional de reducción de la circulación de la sangre
por oclusión de la luz del vaso. Las placas presentes en las
arterias iliaca y femoral podrían causar isquemia de las
extremidades inferiores, bien por embolización distal de material
ateromatoso o bien por estenosis in situ del vaso sanguíneo
enfermo, es decir, por estrechamiento del diámetro lumenal. La
aterectomía o la angioplastia con o sin despliegue de endoprótesis
en estos vasos impiden que se produzca la enfermedad anterior, pero
también puede crear estas condiciones, a no ser que el dispositivo
esté diseñado especialmente para capturar material embólico
desprendido durante el procedimiento.

instalación provisional de posibilidades de filtración de sangre en
una arteria o vena durante procedimientos endovasculares. Más
específicamente, el invento provee un sistema de filtración de
vasos que acomoda la inserción de catéteres endovasculares para
extraer placas ateroescleróticas y/o trombos y para ensanchar el
diámetro lumenal de un vaso estenótico según se define en la
reivindicación 1. El sistema de filtro proporciona también medios
para mantener perfusión distal durante el aislamiento del flujo de
sangre, y para empujar los residuos embólicos al filtro durante un
estado de poca circulación, aumentando de ese modo las posibilidades
de filtración.
En una realización, el sistema de filtración de
vasos incluye un filtro expansible, por ejemplo, un paracaídas, un
cesto, o una espiral, fijados en un extremo distal de un
alambre-guía, y un catéter de gran diámetro (tal
como un catéter de guiado, un catéter angiográfico, vainas
introductoras, o vasodilatadores) que tiene una luz que comunica con
un extremo proximal y un orificio en su extremo distal. El extremo
distal del catéter está destinado a la inserción en una arteria o
vena. El extremo proximal del catéter podría incluir una válvula
hemostática. Un oclusor expansible, que podría comprender un globo
elastómero, está dispuesto alrededor del extremo distal del catéter
de guiado y comunica con una luz de insuflación para proveer
aislamiento del flujo de sangre en el vaso. La luz del catéter está
destinada a recibir el alambre-guía, que atraviesa
el orificio distal. Un extremo proximal del
alambre-guía se podría accionar típicamente desde el
exterior del extremo proximal del catéter para la manipulación del
alambre-guía independientemente del catéter. La luz
del catéter está destinada también a recibir un dispositivo
endovascular, por ejemplo, un catéter de angioplastia, un catéter
con despliegue de endoprótesis, o un catéter de aterectomía.
Los catéteres endovasculares típicamente
incluyen un extremo proximal, un extremo distal y una luz que recibe
el alambre-guía. Un miembro de escisión, por
ejemplo, una cuchilla, un miembro abrasivo, una cortadora de
alambre, mordazas, garras, pinzas, trampas, etc., se incluye en la
región distal de un catéter de aterectomía. Un globo expansible se
incluye en una región distal de un catéter de angioplastia. Una
endoprótesis expansible se fija en una región distal de un catéter
de despliegue de endoprótesis. El catéter de aterectomía podría
opcionalmente incluir además medios para radiografías
intravasculares, por ejemplo, un transductor ultrasónico. En ciertas
realizaciones, los catéteres de angioplastia incluyen una
endoprótesis provista alrededor del globo en su región distal. Los
dispositivos de radiografías intravasculares y las endoprótesis se
describen por completo en la técnica y no se describirán
adicionalmente en la presente memoria.
En otra realización, el filtro expansible
comprende un bastidor de expansión y una malla dispuesta sobre el
bastidor. En ciertas realizaciones, el bastidor comprende una
pluralidad de puntales unidos al alambre-guía en un
primer extremo, y los puntales se expansionan radialmente hacia
fuera en un segundo extremo. El bastidor podría comprender un cierre
hermético para insuflación para proporcionar un contacto mejor con
las paredes vasculares. La construcción y el uso de medios de
expansión y malla de filtro asociada se han descrito minuciosamente
en solicitudes anteriores de patentes que incluyen la de Barbut y
colaboradores, solicitud de patente de EE.UU. Nº de serie

Todavía en otra realización, el catéter de
guiado incluye un orificio de infusón proximal al globo de oclusión.
El orificio comunica con una luz de infusión del catéter y está
destinado a la infusión de fluido o de productos farmacéuticos.
Usando el orificio de infusión, se puede reducir la dosificación de
producto farmacéutico requerida para obtener efecto local comparada
con la administración por vía general. Se pueden minimizar también
los efectos colaterales, por ejemplo, la hemorragia asociada con la
administración general de activadores de plasminógeno de tejido (en
adelante t-PA). En ciertas realizaciones, el catéter
de angioplastia podría incluir un orificio de infusión proximal al
globo de angioplastia y un orificio de perfusión distal al globo.
Los orificios de infusión y perfusión comunican, respectivamente,
con una luz de infusión y perfusión incluida en el catéter de
angioplastia. El orificio de infusión está destinado a la aspiración
de fluido, sangre, aire, o residuos vasculares.
El sistema de filtración de vasos se podría usar
en métodos para proteger a un paciente de la embolización durante un
procedimiento endovascular con el fin de extraer placas y/o trombos
de la arteria coronaria, aorta, arteria carótida primitiva, arterias
carótidas externa e interna, tronco braquiocefálico, arteria media
cerebral, arteria basilar, arteria subclavia, arteria humeral,
arteria axilar, arteria iliaca, arteria renal, arteria femoral,
arteria poplítea, tronco celíaco, arteria mesentérica superior,
arteria mesentérica inferior, arteria tibial anterior, arteria
tibial posterior, y todas las demás arterias que transportan sangre
oxigenada. Los métodos incluyen también la prevención de la
embolización distal durante un procedimiento endovascular para
eliminar trombos y/o cuerpos extraños en la circulación venosa,
incluyendo la vena cava superior, vena cava inferior, venas
yugulares externa e interna, vena braquiocefálica, arteria pulmonar,
vena subclavia, vena braquial, vena axilar, vena iliaca, vena renal,
vena femoral, vena femoral profunda, vena safena interna, vena
porta, vena esplénica, vena hepática, y vena ácigos.
En un primer método de usar el sistema de
filtración de vasos, el extremo distal del
alambre-guía, que tiene el filtro en un estado
aplastado, se inserta a través de una arteria o vena. El filtro y la
región distal del alambre-guía se sitúan en el vaso
más allá de una región de interés, operación a la que sigue la
expansión del filtro. El catéter de guiado se inserta sobre el
alambre de guiado, y se sitúa el globo de oclusión en un lugar
proximal a la región de interés. La región distal de un catéter de
aterectomía, de despliegue de endoprótesis, o de angioplastia se
inserta sobre el alambre-guía, donde el
alambre-guía es transportado dentro de la luz del
catéter de guiado, y se avanza a la región de interés. Luego se
expansiona el globo de oclusión para aislar el flujo de sangre en
el vaso mientras el catéter endovascular extrae o trata de otro modo
la lesión estenótica en la luz vascular. El catéter se podría luego
retirar o dejar en su sitio, e infundirse fluido o sangre a través
de la luz del catéter de guiado para lavar a presión los residuos
embólicos en el filtro expansionado. En ciertas realizaciones, el
fluido se dirige como un chorro hacia el ateroma con el fin de
despegar el ateroma de la pared del vaso y a continuación
impulsarlo al filtro. Se podrían repetir las etapas de insertar el
catéter endovascular y de infundir fluido para lavar a presión los
residuos embólicos hasta que se establezca un diámetro lumenal
adecuado. Después se aplasta el filtro y se retira del vaso, junto
con los residuos embólicos capturados, mediante la extracción del
alambre-guía. El catéter de guiado se extrae después
de haber desinflado el globo de oclusión.
En otro método, después de que el filtro
expansionado y el catéter de guiado se han situado, respectivamente,
en un vaso distal a y proximal de la región de interés, se infla el
globo de angioplastia transportado por el catéter de angioplastia
para dilatar la luz vascular estenótica. Se podrían aspirar sangre,
fluido, aire, y/o residuos embólicos presentes entre el globo de
oclusión y el globo de angioplastia. Alternativamente, el globo de
oclusión se podría desinflar durante la insuflación del globo de
angioplastia para permitir que la sangre se aspire del orificio
proximal y pase al orifico de perfusión distal al globo de
angioplastia, manteniendo de ese modo la perfusión a los órganos
distales durante la angioplastia.
Se entenderá que existen varias ventajas en el
uso de los dispositivos y métodos descritos en la presente memoria
para capturar y extraer residuos embólicos durante procedimientos
endovasculares. Por ejemplo, el sistema de lavado a presión del
filtro, (1) es particularmente adecuado para la filtración
provisional de sangre en cualquier vaso con el fin de atrapar
residuos embólicos, minimizando de ese modo las complicaciones
neurológicas, cognitivas, y cardíacas relacionadas con la
embolización distal,. (2) puede soportar un flujo intenso de sangre
arterial durante un tiempo prolongado, (3) incluye una malla que es
suficientemente porosa para permitir que la sangre adecuada circule
en un vaso sanguíneo mientras captura émbolos móviles, (4) está
destinado a alojar un catéter endovascular con o sin dispositivo
para radiografías, (5) podría extraer las placas móviles contenidas
en un vaso mediante un lavado a presión a través del catéter de
guiado, (6) provee medios para mantener la perfusión a órganos
distales durante los procedimientos endovasculares, (7) proporciona
medios para administrar productos farmacéuticos, por ejemplo,
activador plasminógeno de tejido o nitroglicerina, localmente a la
región de interés, minimizando de ese modo los efectos asociados con
la administración por vía general, y (8) se puede usar en pacientes
adultos y pediátricos.

En una primera realización, se ha provisto un
sistema de filtro para instalación provisional en un vaso, ya sea
una arteria o una vena, según se ha representado en las Figuras 1A y
1B. El sistema de filtro incluye un alambre-guía 10
que tiene un extremo proximal, un extremo distal 11, y un filtro
expansible 20 fijado en el extremo distal. El filtro comprende un
bastidor 22 de sombrilla y una malla 25 que se ha soldado con
decapado sónico o unido con adhesivo a unos brazos 26 del bastidor
de sombrilla. Se podrían aplicar a la malla 25 anticoagulantes,
tales como heparina y heparinoides, para reducir la formación de
trombos sobre la malla. El filtro se puede aplastar como se muestra
en la Figura 1A para facilitar su inserción en un vaso, y después
expansionarse como se ha mostrado en la Figura 1B. En la patente de
EE.UU. Nº 5.910.154, expedida a Tsugita y colaboradores, se
describen una variedad de alambres - guía adecuados para filtro.
El sistema de filtro incluye también un catéter
de guiado 30 que tiene una luz 33 que comunica con un extremo
proximal y con un extremo distal 31, y un globo 40 de oclusión
provisto alrededor del extremo distal. El globo 40 comunica con la
luz 41 de insuflación, que está destinada a recibir fluido o aire
para la expansión del globo. La luz 33 del catéter comunica con el
orificio distal 35 y está destinada a recibir al
alambre-guía 10 y a otros dispositivos
endovasculares, tales como catéteres de aterectomía, dispositivos
para radiografías endovasculares, catéteres para despliegue de
endoprótesis, catéteres de angioplastia, monitores de presión,
catéteres para electrofisiología, y aspiradores.
En uso, según se ha representado en las Figuras
2A, 2B y 2C, el filtro 20 (en la condición aplastada) y el extremo
distal 11 del alambre-guía se insertan por vía
subcutánea a través de una arteria o vena periféricas típicamente en
la dirección de la circulación de la sangre (sin embargo, se
contempla que el alambre-guía se podría insertar en
una dirección contraria a la circulación de la sangre). El filtro 20
se avanza en un punto distal hasta la placa ateromatosa 100 y se
expansiona para establecer contacto con las paredes 101 del vaso
como se ha representado en la Figura 2A. Luego se inserta el catéter
de guiado 30 sobre el alambre-guía 10 hasta que el
orificio distal 35 está situado proximal a la placa 100. El catéter
de aterectomía 40, que tiene el dispositivo 42 de aterectomía fijado
en una región distal, se inserta dentro de la luz 33 del catéter y
sobre el alambre-guía 10. Se hace avanzar al
dispositivo 42 de aterectomía distal al orificio 35 del catéter
hasta una posición adyacente a la placa 100. Se infla el globo de
oclusión 40 para aislar la circulación de sangre en el vaso, al
mismo tiempo que el dispositivo 42 de aterectomía extrae la placa
100 según se ha representado en la Figura 2B.
Una vez que se ha obtenido por aterectomía un
diámetro lumenal adecuado, se desinfla el globo 40 de oclusión,
permitiendo que se restablezca la circulación de la sangre en la
región de interés para empujar al material embólico generado durante
el procedimiento hacia el filtro 20 y para prefundir los órganos
distales. El globo 40 de oclusión se puede volver a inflar para
aislar la circulación de sangre con el fin de repetir el
procedimiento de aterectomía y otros procedimientos endovasculares,
por ejemplo la aspiración. Una vez finalizado el procedimiento, se
retira del catéter de guiado 30 el catéter de aterectomía según se
ha representado en laa Figura 2C. Se infunde fluido o sangre a
través de la luz 33 del catéter de guiado para lavar a presión el
material embólico que no se haya eliminado como consecuencia de un
estado de poca circulación hacia el filtro 20. Esta operación podría
hacerse con o sin el globo 40 desplegado. El lavado a presión con
fluido se podría dirigir también en la pared del vaso para soplar el
material ateromatoso que se haya retenido holgadamente que
permanezca después de la aterectomía. En ciertos casos, el filtro
podría llegar a cargarse con cantidades excesivas de material.
Entonces, podría ser conveniente desplegar un catéter separado de
aspiración a través del catéter de guiado para extenderlo
distalmente hasta el filtro, y de ese modo trabajar para aspirar
material embólico capturado en la malla. Se retira del vaso el
catéter de guiado 30. El filtro 20 con los residuos embólicos
capturados se aplasta y retira del vaso mediante la extracción del
alambre-guía 10. Alternativamente, el
alambre-guía se podría extraer en el interior del
catéter de guiado 30, y a continuación extraerse simultáneamente
ambos dispositivos. En otra realización, el
alambre-guía podría tener una vaina de captura
separada para ayudar en su colocación y retirada tirando del
alambre-guía en el interior de la vaina antes de su
colocación o retirada.
A título de ejemplo, cuando el sistema de filtro
que se ha descrito en la presente memoria está destinado a usarlo en
la aorta, el área de la malla requerida para el dispositivo se
calcula a partir de la ecuación de Bernouilly según se describe en
nuestras solicitudes anteriores que incluyen la de Barbut y
colaboradores, Solicitud de patente de EE.UU. Nº de serie
08/553.137, presentada el 7 de noviembre de 1995, la de Barbut y
colaboradores, Solicitud de patente de EE.UU. Nº de serie

Una vez que se han determinado las
características físicas apropiadas, la malla adecuada se puede
encontrar entre las mallas estándar conocidas en la técnica. Por
ejemplo, se podrían usar mallas de poliéster, tales como las
fabricadas por Saati Corporation and Tetko Inc. Estas mallas se
venden en forma de lámina y se pueden cortar y conformar fácilmente
a la forma deseada. En una realización preferida, la malla se suelda
con decapado sónico en una forma de cono. Son también adecuadas
otras mallas conocidas en la técnica, que tengan las
características físicas previstas.
Se pueden aplicar a la malla anticoagulantes
como heparina y heparinoides para reducir las posibilidades de que
la sangre se coagule en la malla. Se podrían usar también
anticoagulantes distintos a los heparinoides, por ejemplo,
anticuerpos monoclonales tales como los de ReoPro (Centocor). El
anticoagulante se podría aplicar a la malla por pintado o
pulverización. Se podría usar también un baño químico que contenga
el anticoagulante. Se podrían usar otros métodos conocidos en la
técnica para la aplicación de productos químicos.
Las Figuras 3A, 3B y 3C representan otra
realización del sistema de lavado a presión de filtro para su
instalación provisional en un vaso. La malla 25 del filtro 20 se une
operativamente al cierre hermético 27 de insuflación en un primer
borde y a una placa extrema 28 en un segundo borde. El cierre
hermético de insuflación es expansible entre una condición
contraída y una condición ensanchada. En uso, en una condición
contraída, el cierre hermético 27 de insuflación y la malla 25 se
pueden insertar a través de un vaso periférico en una región de
interés, típicamente distal a la placa ateromatosa 100, según se ha
representado en la Figura 3A. El cierre hermético de insuflación se
expansiona mediante la inyección de fluido o gas para establecer
contacto con la pared interior del vaso 101. Se inserta el catéter
de guiado 30 sobre el alambre-guía 10, y se sitúa el
orificio distal 35 proximal a la placa 100.
La angioplastia translumenal percutánea ha
resultado satisfactoria en el tratamiento de estenosis arteriales
así como de la trombosis oclusiva de vena resistente a la
trombolisis Véase American Heart Journal 125 (2 Pt 1):
362-6 (1993). En la Figura 3B, un catéter de
angioplastia 50, que tiene un globo 52 de angioplastia fijado en una
región distal, se ha insertado a través de la luz 33 del catéter de
guiado sobre el alambre-guía 10. En un estado
desinflado, se avanza el globo de angioplastia a través del orificio
35 hasta una posición adyacente a la placa 100. La placa ateromatosa
se comprime mediante la insuflación del globo 52, dilatando de ese
modo la estenosis en el vaso.
En ciertas realizaciones, el catéter de
angioplastia incluye un orificio 54 de infusión proximal y un
orificio 55 de perfusión proximal y distal al globo 52,
respectivamente. El orificio 54 de infusión se podría usar para
administrar productos farmacéuticos, por ejemplo,
t-PA o nitroglicerina y para aspirar aire, trombos,
placas, y/o residuos de tejido. Los globos 40 se podrían inflar o
desinflar durante la angioplastia. Se podría infundir un fluido
oxigenado o sangre oxigenada a través del orificio 55 para
proporcionar perfusión a los órganos distales durante la
angioplastia y para facilitar el lavado a presión de material
embólico presente en el filtro.
En ciertas realizaciones, una prótesis, tal como
una endoprótesis, está estrechamente relacionada con el globo de
angioplastia. La endoprótesis típicamente se fija por presión sobre
el globo y es capaz de realizar una expansión radial controlada en
una región de interés tras la aplicación de una fuerza radial que se
extienda hacia fuera desde el interior de la endoprótesis. La
construcción de un sistema de catéter que lleva una endoprótesis se
describe detalladamente en la patente de EE.UU. Nº 5.749.848,
concedida a Jang y colaboradores.
Luego se podría extraer del vaso el catéter de
angioplastia a través de la luz 33 del catéter de guiado después de
terminar la angioplastia, según se ha representado en la Figura 3C.
Se desinfla el globo 40 de oclusión para restablecer la circulación
de la sangre. Antes o después de desinflar el globo 40, se puede
infundir fluido o sangre a través de la luz 33 y del orificio 35
para lavar a presión el material embólico presente en el filtro 20.
Después que el material embólico se ha capturado y retenido en el
filtro, se retira del vaso el catéter 30 de guiado. Luego se contrae
el filtro desinflando el cierre hermético 27 de insuflación y, con
el material embólico capturado, se extrae del vaso y se retira del
cuerpo del paciente.
En las Figuras 4A, 4B y 4C se ha representado
otra realización del sistema de lavado a presión de filtro. El
filtro 20 está en la forma de un cesto preconformado, que tiene una
malla 25 unida operativamente al cierre hermético 27 de insuflación.
El cierre hermético de insuflación se puede contraer y dilatar
mediante la infusión de un gas o de un fluido. En uso, en una
condición contraída, el cierre hermético 27 de insuflación y la
malla 25 se insertan a través de un vaso periférico distal a la
placa ateromatosa 100 según se ha representado en la Figura 4A. El
cierre hermético de insuflación se dilata para establecer contacto
con la pared interior del vaso 101. Se inserta el catéter 30 de
guiado sobre el alambre-guía 10, habiendo situado el
orificio distal 35 proximal a la placa 100.
La instalación de endoprótesis primarias para
placas ateroescleróticas complejas ha sido eficaz en el tratamiento
de estenosis aórtica e iliaca que se pueden tratar con angioplastia
de globo. Véase Onal y colaboradores, Cardiovascular Interventional
Radiology 21 (5): 386-92 (1998). La intervención con
catéter al sistema venoso usando endoprótesis metálicas expansibles
ha sido satisfactoria en el tratamiento del síndrome de la vena cava
superior que principalmente está asociado con tumores malignos y es

65, que está dotado de una endoprótesis autoexpansible 60 fijada en
una región distal, se inserta a través de la luz 33 del catéter de
guiado sobre el alambre-guía 10 como se ha
representado en la Figura 4B. La endoprótesis podría estar
constituida por un metal que conserve la forma, tal como el nitinol.
El catéter 65 incluye medios para retener la endoprótesis
autoexpansible. Los medios de retención podrían ser de la forma de
una vaina dispuesta alrededor de la región distal del catéter, de
tal manera que la endoprótesis esté relacionada con la vaina y
contenida por ella, y cuando se retira la vaina, la endoprótesis se
libera y automáticamente se ensancha hasta alcanzar el diámetro
dilatado. En las Figuras 4B y 4C se muestra la placa 100 reducida
por la endoprótesis 60 autoexpansible y térmicamente activable. La
construcción y el despliegue de la endoprótesis autoexpansible se
describen en la patente de EE.UU. Nº 5.224.953, concedida a
Morgentaler.
En la Figura 4C, después de que la endoprótesis
60 se ha desplegado sobre la placa 100, se retira el catéter
endovascular 65, dejando a la luz 33 del catéter 30 de guiado
disponible para la infusión de un medio fluido. Se podría encontrar
una placa móvil, que se refiere a depósitos vasculares que
comprenden una base solidificada y un saliente colgante fijado a la
base que se puede desprender mediante un flujo normal pulsátil de la
sangre, que sobresalga a través de la malla de la endoprótesis 60.
Mediante un lavado a presión con fluido a través de la endoprótesis
60, se podría desprender la placa móvil y capturarse por el filtro
20. Una vez que el material embólico ha sido capturado y retenido en
el filtro, se retira del vaso el catéter 30 de guiado. Se contrae el
filtro 20, antes o después de la retirada del catéter de guiado,
desinflando el cierre hermético 27 de insuflación, y con el material
embólico capturado, se extrae del vaso el filtro y se retira del
cuerpo del paciente.
La Figura 5 representa otra realización del
sistema de filtración de vaso que tiene un orificio 70 de infusión,
que está situado proximal al globo 40 de oclusión y comunica con la
luz 33 del catéter 30 de guiado. El orificio 70 permite que la
admisión de fluido y la sangre circulen desde el lado proximal del
globo de oclusión y salgan por el orificio distal 35 del catéter
para proveer perfusión a los órganos distales durante un
procedimiento endovascular. En ciertas realizaciones, el catéter de
guiado podría incluir una pluralidad de orificios de infusión
proximales al globo de oclusión, y opcionalmente una válvula
unidireccional en el orificio de infusión para eliminar la
circulación retrógrada de la sangre. En ciertas realizaciones, el
catéter de guiado podría incluir un orificio (o unos orificios) de
aspiración distales al globo de oclusión para aspirar residuos
vasculares generados durante el procedimiento endovascular.
La longitud del catéter de guiado y del
alambre-guía estará generalmente comprendida entre
15 y 200 centímetros, con preferencia aproximadamente entre 50 y 150
centímetros El diámetro interior de la luz del catéter estará
comprendido en general entre 1,0 y 7 milímetros, con preferencia
aproximadamente entre 1,5 y 2,6 milímetros. El diámetro del globo
de oclusión dilatado estará en general comprendido entre 1, 5 y 50,0
milímetros, con preferencia aproximadamente entre 3,0 y 8,0
milímetros. Los intervalos anteriormente indicados se han dado
exclusivamente con el fin de ilustrar dimensiones típicas de los
dispositivos. Evidentemente, las dimensiones reales de un
dispositivo construido de acuerdo con los principios del presente
invento podrían variar saliéndose de los intervalos expuestos sin
apartarse de estos principios básicos.
Aunque el invento anterior se ha descrito con
cierto detalle, para mayor claridad y entendimiento, a título
ilustrativo y de ejemplo, será obvio que se podrían llevar a la
práctica ciertos cambios y modificaciones que todavía entrarían en
el alcance de las reivindicaciones incluidas como apéndice. Por
ejemplo, se entenderá que se puede usar cualquier tipo de bastidor
de expansión descrito en la presente memoria o en solicitudes
anteriores con cualquiera de las intervenciones terapéuticas con
catéteres, aún cuando cualquier cifra dada pudiera representar
solamente una combinación particular. Más aún, se pueden usar
dispositivos de oclusión distintos a los globos con cualquiera de
las realizaciones descritas en la presente memoria.
Claims (16)
1. Un sistema de filtración de vaso, que
comprende
un catéter (30) de guiado que tiene una región
de extremo proximal, una región de extremo distal (31), y una luz
interna (33);
un catéter terapéutico provisto dentro de la luz
interna (33) del catéter (30) de guiado, cuyo catéter terapéutico
define una luz de alambre-guía;
un alambre-guía (10) dispuesto
deslizablemente dentro de la luz de alambre-guía del
catéter terapéutico;
un filtro expansible (20) acoplado al
alambre-guía (10);
un globo (40) de oclusión acoplado a la región
de extremo distal (31) del catéter (30) de guiado; y
un elemento terapéutico (42, 52, 60) acoplado al
catéter terapéutico y situado distalmente del globo (40) de
oclusión.
2. El sistema de la reivindicación 1, en el que
el catéter terapéutico comprende un catéter de aterectomía, y en el
que el elemento terapéutico incluye un miembro de escisión (42).
3. El sistema de la reivindicación 1, en el que
el catéter terapéutico comprende un catéter de angioplastia, y en el
que el elemento terapéutico incluye un globo (52) de
angioplastia.
4. El sistema de la reivindicación 3, que
comprende además una endoprótesis (60) dispuesta alrededor del
catéter de angioplastia.
5. El sistema de la reivindicación 1, en el que
el catéter terapéutico comprende un catéter de descarga de
endoprótesis, y en el que el elemento terapéutico incluye una
endoprótesis autoexpansible (60).
6. El sistema de la reivindicación 1, en el que
el filtro expansible (20) comprende un bastidor (22) de expansión y
una malla (25) dispuesta sobre el bastidor (22).
7. El sistema de la reivindicación 6, en el
que la malla (25) está tejida.
8. El sistema de la reivindicación 6, en el que
la malla (25) comprende una película delgada que tiene agujeros para
permitir la circulación de fluido.
9. El sistema de la reivindicación 6, en el que
el bastidor (22) de expansión comprende una pluralidad de puntales
(26) unidos al alambre-guía (10) en un primer
extremo, y que se expande radialmente hacia fuera en un segundo
extremo.
10. El sistema de la reivindicación 6, en el que
el bastidor (22) de expansión comprende un cierre hermético (27) de
insuflación.
11. El sistema de la reivindicación 1, en el
que el catéter (30) de guiado incluye un orificio de aspiración
distal del globo de oclusión, cuyo orificio de aspiración comunica
con una luz de aspiración.
12. El sistema de la reivindicación 1, en el
que el globo (40) de oclusión comunica con una luz de
insuflación.
13. El sistema de la reivindicación 1, en el
que el catéter (30) de guiado incluye un orificio (70) de infusión
proximal al globo (40) de oclusión.
14. El sistema de la reivindicación 13, en el
que el orificio (70) de infusión comunica con una luz interna (33)
del catéter (30) de guiado.
15. El sistema de la reivindicación 3, en el
que el catéter terapéutico incluye un orificio (54) de infusión
proximal al elemento terapéutico (42, 52, 60).
16. El sistema de la reivindicación 15, en el
que el orificio (54) de infusión comunica con un orificio (55) de
perfusión distal del elemento terapéutico (42, 52, 60).
