Изобретение относитс к медицинской технике, а именно к кардиохирургическим инструментам.
известен фильтр-ловушка, который содержит трубку-проводник со стерж- 5 нем, капсулу, а также округлой форкы пластмассовую пластинку с отверсти ми , в которую запрессован металлический каркас., придающий фильтру форму зонтика. По окружности фильтра вы- 10 полнены острые зубцы, вл квдиес продолжением каркаса, которые предназначены дл фиксации фильтра к стенкам нижней полой вены. Заключенный в еложенном виде в небольшую металличес- 15 кую капсулу фильтр под контролем рентгеновского экрайа ввод с помощью проводника-трубки через ремную вену в. нижнюю полую вену и фиксируют в ней путем внедрени острых его зуб-20 цов в стенку нижней полой вены .
Недостатками данного фильтра-ловушки вл ютс предельно узкие возможности его применени , св занные, прежде всего, с особенностью лишь од-25 постороннего его воздействи , а именио , открытый и установленный внутри полой вены фильтр конструктивно невозможно оп ть сложить и убрать в капсулу . зовании не исключаютс случаи т желой травмы больного, вызванные повреждением стенок нижней полой вены, окружающих тканей и органов и др. Кроме того, оставл емый в просвете нижней полой вены фильтр сам по себе вл етс местом оседани тромбов, что,в конечном итоге, может привести к полной закупорке нижней полой вены,.
Цель изобретени - обеспечение .атравматичного введени , фиксации :и извлечени фильтра при кардкохирургических вмешательствах.
Поставленна цель достигаетс тем, что в фильтр-ловушке капсула укреп- . лена на стержне между каркасом и фильтрующим элементом, причем наружный край каркаса снабжен упругим кольцом .
На фиг. 1 изображен фильтр-ловушка , в рабочем положении; на фиг. 2 то же, в сложенном состо нии на фиг. 3 - механизм управлени , в рабочем положении; на фиг. 4 - то же, при сложенном (фильтре; на фиг. 5 и 6 - Моменты использовани фильтра-ло-. вушки при операции.
В предлагаемом фильтре-ловушке фильтрующий злемент 1 с упругим кольцом 2 закреплен снаружи по кргио каркаса , выполненного из рычаго 3, шарнирно св зайных с трубкой-проводником 4, внутри которой размещен стержень 5. Один конец стержн 5 снабжён капсулой 6, установленной открытым торцом к трубке-проводнику 4 в верЬшне фильтрунвдего элемента 1. Другой конец стержн 5 св зан со штоком 7 механизма управлени , корпус 8 которого жестко соединен с трубкой-проводником 4. На корпусе 8 закреплен фиксатор. 9 и установлена возвратна пружина 10. .Стержень 5 имеет направл ющий конус 11, атрубка-проводник 4 - обратный конус 12.
Фильтр-ловушка работает следующим образом.
Во врем кардйохирургичёского вмешательства , например митральной комиссуротомии ,фильтр-ловушку в сложенном состо нии провод т через, отверстие в стенке левого желудочка в восход щую .часть аорты сразу за ее клапаны. Нажатием на шток 7 механизма управлени вызывают, перемещение стержн 5 с капсулой 6 относительно трубки-проводника 4 до освобождени рычагов 3 каркаса. Дальнейшее перемещение стержн 5 с капсулой б приводит к нат жению фильтрующего элемен-т .а 1 и расхозкдению рычагов 3 каркаса до упора в сТенку аорты, которое конролируетс снаружи рукой хирурга. Плотное прилегание кра фильтрующего элемента 1 обеспечено нат жением упругото кольа 2, при этом происходит сжатие пружины 10. . В уста;нб:вЛённом положении закрепл ют шток 7 относи ,тельно корпуса 8 фиксатором 9, затем вскрывают ушко левого предсерди и далее по обычцой методике производ т митральную комиссуротомйю.
Тромбы, имевшиес в полост х левой половины сердца и проникшие во BpesMi комиссуротомии, в аорту, собираютс в фильтре-ловушке, который в ,то же врем не преп тствует свободному кровотоку по аорте. После осуществлени комиссуротомии освобождают фиксатор 9 и под усилием возвратной пружины 10 стержень 5 с. капсулой 6 перемещаетс в сторону 15ычагов 3 каркаса. Рычаги 3 складываютс и вход т в капсулу б, а фильтрующий элемент 1 вместе, с тромбами 13 облегает
капсулу 6 снаружи. Затем извлекают фильтр-ловушку в сложенном состо нии Направл ющий конус 11 и обратный конус 12 облегчают проведение фильтраловушки через отверстие в стенке левого желудочка сердца и створках аортального клапан-а.
Таким образом, фильтр-ловушка дает возможность; значительно уменьшить опасность тромбоэмболии магистральных сосудов при оперативном вмешателстве , например во врем широко распространенной операции митральной и а:ортапьной комиссуротомии, расшир ет показани к оперативному вмешательству , повышает ее эффективность, облегчает технику операции. Кроме того воз1 1Ьжно дозированное раскрытие фильра-ловушки в зависимости от-диаметра аорты больного, что избавл ет от необходимости создани набора подобных инструментов разного калибра.
Применение фильтра-ловушки не требует дополнительного оперативного вмшательства и Явл етс безопасным. Инструмент может быть использован . как при закрытой,так и при открытой (с применением искусственного кровообращени ) методике операций на сердце, поскольку даже самое тщательное удаление тромбов .из полостей левой половины сердца при операци х в услови х искусственного кровообращени не гарантирует от попадани мелких тромбов и :капьцинатов в сосуды головного мозга.

- Формула изобретениФильтр-ловушка, содержащий трубку-проводник со стержнем; каркас сфильтрующим элементом и капсулу, о т л и ч а и щ и.й с тем, что, с целью атравматичного введени , фиксации и извлечени фильтра при кардиохирургических вмешательствах, капсула укреплена на стержне между каркасом и фильтрующим элементом, причем наружный край каркаса снабжен упругим Ельцом. . .Источники информации, прин тые во внимание, при экспертизе 1.
