La presente invención se refiere a un

Más específicamente, la presente invención se refiere a los
dispositivos para reparar, reforzar y/o tratar el hueso fracturado
y/o enfermo empleando diversos dispositivos, incluyendo dispositivos
que forman cavidades.
El hueso sano normal está compuesto de un marco
formado por proteínas, colágeno y sales de calcio. Por lo general,
el hueso sano es lo suficientemente fuerte para resistir las
diversas tensiones a que es sometido un individuo durante sus
actividades diarias normales y por lo general puede resistir mucha
más tensión durante períodos de tiempo variables antes de
quebrarse. Sin embargo, "la osteoporosis" o un sinnúmero de

osteoporosis", el cáncer y otros trastornos relacionados con el
hueso, no son comúnmente curables en nuestra etapa actual de
desarrollo médico.
Para muchos individuos en nuestra población
mundial que está envejeciendo, las pérdidas de la fuerza ósea no
diagnosticadas y/o no tratadas ya han debilitado los huesos de estos
individuos a un punto tal que incluso las actividades diarias
normales plantean una amenaza significativa de fractura. Por
ejemplo, cuando los huesos de la espina dorsal están
suficientemente debilitados, las fuerzas compresivas en la espina
dorsal a menudo pueden causar fractura y/o deformación de los
cuerpos vertebrales. Para el hueso suficientemente debilitado,
incluso las actividades diarias normales como bajar escaleras o
llevar los productos alimenticios pueden causar un colapso de uno o
más huesos espinales, muy parecido a un pedazo de tiza que se
desbarata bajo el peso compresivo de un pie humano. Una fractura
del cuerpo vertebral de esta manera se denomina típicamente fractura
por "compresión" vertebral. Los investigadores calculan que al
menos 25% de todas las mujeres y un porcentaje algo menor de los
hombres, mayores de 50 años padecerán una o más fracturas "por
compresión" vertebral debido solo a la "osteoporosis". En
los Estados Unidos, se calcula que más de 700.000 fracturas "por
compresión" vertebral ocurren cada año, de las cuales más de
200.000 requieren alguna forma de hospitalización. Otras fracturas
que ocurren comúnmente como resultado de huesos debilitados pueden
incluir fracturas de la cadera, la muñeca, la rodilla y el tobillo,
por solo nombrar algunas.
Las fracturas como las fracturas "por
compresión" vertebral a menudo dan lugar a episodios de dolor que
son crónicos e intensos. Además del dolor causado por la propia
fractura, la participación de la columna espinal puede dar lugar a
nervios pellizcados y/o lesionados, causando parálisis, la pérdida
de función y dolor intenso que radia en todo el cuerpo del paciente.
Sin embargo, incluso en los casos en que los nervios no son
afectados, el dolor intenso asociado a todos los tipos de fracturas
es debilitante, dan lugar a mucho estrés, al deterioro de movilidad
y a otras consecuencias a largo plazo. Por ejemplo, las fracturas
espinales progresivas pueden, con el tiempo, causar grave
deformación de la espina dorsal ("cifosis"), dando a un
individuo una apariencia encorvada y también pueden dar lugar a una
significativa reducción de la capacidad pulmonar y a un aumento de
la mortalidad.
Hasta hace poco, las opciones de tratamiento
para las fracturas "por compresión" vertebral, así como otras
fracturas graves y/o pérdidas en la fuerza ósea, eran sumamente
limitadas - - principalmente el tratamiento del dolor con
medicamentos fuertes orales o intravenosos, la reducción de la
actividad, la terapia fortificante y/o radioterapia, todos con
resultados mediocres. Dado que los pacientes con estos problemas son
típicamente mayores y a menudo sufren de otras complicaciones
significativas de salud, muchos de estos individuos no pueden
tolerar una cirugía invasiva. Además, para frenar la pérdida
adicional de la fuerza ósea, a muchos pacientes se les suministra
hormonas y/o vitaminas/suplementos minerales una vez más con
resultados mediocres y a menudo con efectos colaterales
importantes.
Durante el último decenio, se introdujo en
Estados Unidos una técnica llamada vertebroplastia. La
vertebroplastia incluye la inyección de un material de refuerzo
fluible, generalmente el polimetacrilato de metilo (PMMA -
comúnmente conocido como cemento óseo), en un cuerpo vertebral
fracturado, debilitado o enfermo. Poco después de la inyección, el
material de relleno líquido se endurece o polimeriza, liberablemente
soportando el cuerpo vertebral internamente, aliviando el dolor y
evitando que el cuerpo vertebral inyectado continúe
colapsándose.
Aunque la vertebroplastia ha demostrado que
alivia algunos dolores asociados a fracturas por compresión
vertebral, este procedimiento tiene ciertos inconvenientes
inherentes. El peligro más significativo asociado a la
vertebroplastia es la incapacidad del profesional de controlar el
flujo del cemento óseo líquido durante la inyección en un cuerpo
vertebral. Aunque la ubicación y los modelos de flujo del cemento
pueden ser controlados por el escaneo CT o por la fluoroscopía de
rayo x, una vez que el cemento líquido sale de la aguja de
inyección, sigue de manera natural el recorrido de menor resistencia
dentro del hueso, que es a menudo a través de las grietas y/o las
fisuras en el hueso canceloso y/o cortical. Además, debido a que el
hueso canceloso rechaza la inyección del cemento óseo y al pequeño
diámetro de las agujas que se empleaban normalmente en los
procedimientos de vertebroplastia, son necesarias presiones muy
elevadas para forzar el cemento óseo a través de la aguja y hacia el
interior del cuerpo vertebral. El cemento óseo, que es viscoso, es
difícil de inyectar con agujas de pequeño diámetro, y por
consiguiente muchos profesionales deciden "aligerar" la mezcla
de cemento para facilitar la inyección de cemento, lo cual, en
última instancia agrava los problemas de fuga. En un reciente
estudio en el que 37 pacientes con metástasis óseas o mieloma
múltiple fueron tratados con vertebroplastia, 72,5% de los
procedimientos dieron como resultado la fuga del cemento hacia el
exterior del cuerpo vertebral. Cortet B. et al.,
Percutaneous Vertebroplasty in Patients With Osteolytic Metastases
or Multiple Myeloma (1998). Además, cuando el profesional intenta
"aligerar" el cemento agregando un monómero líquido adicional a
la mezcla de cemento, aumenta la cantidad de monómeros no
polimerizados o "libres", lo cual en última instancia puede ser
tóxico para el paciente.
Otro inconveniente de la vertebroplastia se debe
a la imposibilidad de visualizar (empleando el escaneo CT o la
fluoroscopía de rayos x) las diferentes estructuras venosas y otros
tejidos blandos existentes dentro de la vértebra. Aunque
normalmente se visualiza la posición de la aguja dentro del cuerpo
vertebral, no se ve la ubicación de las estructuras venosas dentro
del cuerpo vertebral. En correspondencia, una aguja de diámetro
pequeño de verteroplastia puede colocarse con facilidad y de manera
accidental dentro de una vena en el cuerpo vertebral, y bombearse
el cemento líquido directamente al sistema venoso, donde el cemento
pasa fácilmente a las paredes anterior y/o posterior de las
vértebras a través del plexo venoso externo anterior o la vena
basivertebral.
Otro inconveniente significativo inherente a la
vertebroplastia es la incapacidad de este procedimiento de restaurar
el cuerpo vertebral una condición previa a la fractura anterior a la
inyección del material de refuerzo. Debido a que el hueso se
fracturó y/o deformó y no fue recolocado antes de la inyección del
cemento, la vertebroplastia esencialmente "congela" el hueso
en su condición fracturada. Es poco probable que un procedimiento de
vertebroplastia tradicional pudiera restaurar la anatomía
significativa anterior a la fractura - debido a los flujos de
cemento óseo hacia la vía de menor resistencia, cualquier
movimiento en bloque del hueso cortical podría crear fisuras en el
interior y/o las paredes del cuerpo vertebral a través de las cuales
el cemento óseo podría fluir de inmediato.
Un procedimiento desarrollado más recientemente
para tratar fracturas tales como las fracturas por compresión
vertebral y otros trastornos relacionados con el hueso se conoce
como Cifoplastia^{TM}. Vea, por ejemplo, las Patentes
Norteamericanas Nr. 4.969.888 y 5.108.404. En la Cifoplastia, un
cuerpo expansible se inserta a través de una abertura pequeña en el
hueso fracturado o debilitado, y luego se amplia dentro del hueso.
Este procedimiento comprime el hueso canceloso, y liberablemente
mueve el hueso fracturado a su orientación previa a la fractura, la
creación de una cavidad dentro del hueso que puede estar lleno de un
material ajustable como el cemento o cualquier número de sustitutos
óseos sintéticos. En efecto, el procedimiento "coloca" el hueso
en o cerca de la posición previa a la fractura y crea un
"enyesado" interno, que evita que el hueso se siga fracturando
y/o colapsando. Desde luego, este procedimiento es apropiado para
ser utilizado en otros huesos.
Aunque la Cifoplastia puede restaurar los huesos
a una condición previa a la fractura y el relleno óseo inyectado
tiene menos probabilidad de fugarse del cuerpo vertebral durante un
procedimiento de Cifoplastia, la Cifoplastia requiere un mayor
número de instrumentos quirúrgicos que un procedimiento de
Vertebroplastia, con un mayor costo. Además, los instrumentos de la
Cifoplastia son típicamente de mayor diámetro que los instrumentos
de la Vertebroplastia y, por tanto, requieren incisiones más grandes
y son por lo general más invasivos.
La WO 97/28840 describe un expansor tisular
prostático que está compuesto por un catéter flexible, un balón de
expansión tisular, un balón inflable de Foley, un lumen de orina y
dos lumen para inflar el balón.
La WO 00/09024 representa la técnica anterior
más cercana y muestra un sistema para colocar material en la base,
con una estructura expansible. La figuras 1-30 y su
descripción del documento WC 01/76492 son de avanzada en
correspondencia con el Art. 54(3) EPC.
La presente invención proporciona un
catéter-balón de acuerdo con la reivindicación
1.
La presente invención erradica muchos de los
problemas y desventajas asociados a las estrategias y diseños
actuales en los procedimientos médicos para reparar, reforzar y/o
tratar un hueso debilitado, enfermo y/o fracturado. En este
documento se describen procedimientos de vertebroplastia mejorados y
los instrumentos quirúrgicos que facilitan tales procedimientos.
En uno de dichos procedimientos, un dispositivo
de inserción, preferentemente un conjunto de aguja espinal calibre
once, se inserta en un cuerpo vertebral proyectado empleando la
fluoroscopía de rayos x para observar la colocación de la aguja. Un
dispositivo que forma cavidades se inserta a través de la aguja en
el cuerpo vertebral. El dispositivo que forma cavidades comprime
liberablemente el hueso canceloso cerca de la punta distal de la
aguja - - formando una pequeña cavidad dentro del hueso. Se
retira el dispositivo que forma cavidades y se introduce el cemento
a través de la aguja espinal. Relleno óseo tal como el cemento óseo
mezclado con un agente fluoroscópico se inyecta en la cavidad
empleando la fluoroscopia de rayos x para observar el flujo, del
relleno óseo dentro del cuerpo vertebral. La introducción del
relleno óseo se detiene cuando se alcanza la cantidad de relleno
deseada, los segmentos fracturados del cuerpo vertebral se aproximan
y/o retornan a su posición previa a la fractura, o la fuga del
relleno óseo es inminente. Debido a que se crea una cavidad dentro
del cuerpo vertebral antes de la introducción del relleno óseo,
pueden emplearse presiones de inyección muy bajas, reduciendo
significativamente la posibilidad de fuga del cemento. Además, la
creación de vías de flujo deseados permite un mayor control en la
colocación del material de relleno óseo dentro del cuerpo
vertebral.

En otro procedimiento se inserta un dispositivo
de inserción, como un conjunto de aguja espinal de calibre 11
comercialmente disponible, a través de una región ósea cortical y en
una región ósea cancelosa de un hueso proyectado empleando la
fluoroscopía de rayos x para observar la colocación de la aguja. Un
primer material, como un relleno óseo, se introduce en la región
ósea cancelosa a través del dispositivo de inserción. Luego se
inserta una estructura expansible a través del dispositivo de
inserción y se amplía en el hueso, comprimiendo el primer material
y/o el hueso canceloso, creando así una cavidad y/o una región de
barrera del hueso canceloso comprimido que rodea sustancialmente la
cavidad. Un segundo material, que puede ser del mismo material que
el primero, se introduce entonces a través del dispositivo de
inserción en el hueso. Si se desea, el primer material puede
comprender una fuerzan material con una fortaleza suficiente para
sostener la cavidad durante el procedimiento quirúrgico, evitando
así el colapso de la cavidad con la contracción y extraída de la
estructura expansible. Ese primer material podría incluir, pero no
se limita a cemento óseo, material de injerto óseo o stent metálico
y no metálico.
Los procedimientos y métodos descritos en este
documento pueden realizarse en una vértebra comprimida y/o
fracturada para fortalecer la vértebra, recolocarla, al menos
parcialmente, en su posición previa a la fractura, evitar que la
vértebra se siga fracturando o colapsando, y/o aliviar el dolor
asociado a las fracturas y compresiones espinales.
El catéter-balón de la presente
invención puede insertarse mediante un dispositivo de inserción, tal
como un conjunto de agujas de calibre once, en un hueso, tal como un
cuerpo vertebral, con el extremo distal del catéter que se extiende
más allá de la aguja hasta una longitud determinada por el médico.
Cuando se llena el catéter con un medio de inflación, la parte del
catéter que se extiende más allá de la aguja, se expande hacia
afuera, comprimiendo el hueso canceloso y formando una cavidad
deseada dentro del cuerpo vertebral.
Otros objetivos, ventajas y realizaciones de la
invención son presentados en parte en la siguiente descripción, y en
parte, serán obvios en esta descripción, o pueden aprenderse en la
práctica de la invención.

El catéter-balón de la presente
invención puede emplearse en métodos quirúrgicos para la reparación,
el reforzamiento y/o tratamiento del hueso debilitado, enfermo y/o
fracturado.
La figura 1 representa una espina dorsal humana
1 típica, en la que ha ocurrido una fractura por compresión 10 en
una vértebra lumbar 100. Como se muestra mejor en la figura 3, la
vértebra 100 se ha fracturado, con los discos superior e inferior
103 y 104 oprimiendo por lo general hacia la pared anterior 10 de la
vértebra 100 y lejos de su orientación normalmente paralela, previa
a la fractura (indicada en general como líneas paralelas 90).
La figura 4 representa una vista coronal (desde
arriba) de la vértebra de la figura 3. La vértebra 100 incluye un
cuerpo vertebral 105, que se extiende en el lado anterior (es decir,
frente o tórax) de la vértebra 100. El cuerpo vertebral 105 tiene
aproximadamente la forma de un disco oval, con una pared anterior 10
y una pared posterior 261. La geometría del cuerpo vertebral 105 es
generalmente simétrica. El cuerpo vertebral 105 incluye un exterior
formado del hueso cortical compacto 110. El hueso cortical 110
incluye un volumen interior del hueso canceloso reticulado, o
esponjoso 115 (también llamado hueso medular o hueso
trabecular).
El canal espinal 150 está ubicado en el lado
posterior (es decir, atrás) de cada vértebra 100. La médula espinal
151 pasa por el canal espinal 150. Un arco vertebral 135 rodea el
canal espinal 150. Los pedículos izquierdo y derecho 120 del arco
vertebral 135 unen al cuerpo vertebral 105. El proceso espinoso 130
se extiende desde el arco vertebral posterior 135, como lo hace el
proceso transversal de izquierda a derecha 125 y los procesos
mamilares 126.
La figura 2 representa a un paciente 50
preparado para los métodos descritos en este documento. Estos
procedimientos pueden ser realizados en un paciente ambulatorio o en
pacientes hospitalizados por un personal médico profesional
correctamente adiestrado y capacitado para realizar los
procedimientos mostrados. Deseablemente, se le pondrá al paciente
anestesia general o local mientras dure el procedimiento
quirúrgico.
Uno de estos métodos quirúrgicos comprende la
inserción de un dispositivo de inserción 350 (vea la figura 5A) de
manera percutánea en el hueso, tal como un cuerpo vertebral
fracturado 105 hasta, preferentemente, un área seleccionada de la
espalda, representada como 60 en la figura 2. El dispositivo de
inserción 350 puede ser de tipo y tamaño de instrumento hueco,
preferentemente que tenga un extremo puntiagudo. El dispositivo de
inserción 350 puede comprender una aguja hueca de aproximadamente
2,3 mm de diámetro (calibre once). Se prefiere una aguja de 2,3 mm
(calibre once) para el procedimiento porque incorpora un lumen hueco
de tamaño suficiente para permitir el paso de diversos instrumentos
y materiales, no obstante, el tamaño total de la aguja es lo
suficientemente pequeño para reducir al mínimo el daño óseo y
tisular en el paciente. Sin embargo, debe comprenderse, que podrían
emplearse varios conjuntos de agujas de otros tamaños, incluidas las
agujas de 1,63 a 4,12 mm (calibre 6 al 14) de diámetro, con
resultados diferentes. Además, podrían emplearse con resultados
diferentes otros instrumentos de acceso, como los descritos en las
Patentes Norteamericanas números 4969,888, 5,108,404, 5827289,
5972,015, 6.048346 y 6.066.154.
El dispositivo de inserción 350 está compuesto
preferentemente de un material fuerte, no reactivo y de tipo médico
como el acero quirúrgico. Si se desea, al dispositivo de inserción
350 se le agrega un conjunto de manipulación que está compuesto por
un material no reactivo y de tipo médico que incluye, pero no se
limita a plásticos, cerámicas, metales, compuestos,
acrilonitrilo-butadieno-estireno
(ABS), polietileno, polipropileno, poliuretano, Teflon®, o acero
quirúrgico. La figura. 5A representa un conjunto de aguja
comercialmente disponible.
Como se muestra en la figura 5A, puede
insertarse un dispositivo de inserción 350, como una aguja de
biopsia de 2,3 mm de diámetro (calibre once) (comercialmente
disponible de Becton Dickinson y Co de Franklin Lakes, NJ) a través
de los tejidos blandos de la espalda y en el cuerpo vertebral 105.
En general, el enfoque para tal procedimiento será transpedicular,
aunque podrían emplearse otros enfoques, que incluyen los enfoques
lateral, extrapedicular y/o anterior, según el nivel tratado y/o las
características anatómicas intermedias bien conocidas por los
especialistas en la técnica. El dispositivo 350 puede comprender un
cuerpo de aguja 348 y un estilete 349, como es bien conocido en la
técnica. Durante la inserción del dispositivo 350, es aconsejable
observar la ubicación del dispositivo 350 empleando el equipo de
visualización como Rayos X en tiempo real, equipo de escaneo CT 70
(vea la figura 2), MRI, o cualquier otro equipo de monitoreo
normalmente usado por los especialistas en la técnica, incluidos los
equipos de orientación y cartografía asistidos por computadora como
los sistemas comercialmente disponibles de BrainLab Corporation o
General Electric Corporation.
El extremo distal 351 del dispositivo de
inserción 350 puede colocarse en el cuerpo vertebral 105,
preferentemente en un lugar hacia el lado posterior de, el cuerpo
vertebral 105. Si se desea, podría colocarse el extremo distal 351
en diversas ubicaciones en todo el cuerpo vertebral 105, incluido el
lado anterior. Una vez colocado, puede extraerse el estilete 349 del
dispositivo de inserción 350, vea la figura 5B y puede insertarse un
dispositivo que forma cavidades 200 a través del eje 348 y en el
cuerpo vertebral 105. Vea la figura 5C. El dispositivo que forma
cavidades 200, que está compuesto deseablemente de un material
biológicamente compatible y aceptable desde el punto médico, es
capaz de desplazar el hueso canceloso. Una vez que el dispositivo
que forma cavidades se coloca dentro del cuerpo vertebral 105, se
utiliza para desplazar el hueso canceloso 115, creando una cavidad
170. Vea la figura. 5F.
El dispositivo que forma cavidades mostrado en
las figuras 9 y 10 comprende un catéter-balón 201.
El catéter-balón 201 se extiende liberablemente a
través de al menos el 20% del cuerpo vertebral, pero podría
extenderse más o menos, en dependencia del tamaño deseado de la
cavidad a producir. A medida que el catéter-balón
201 es ampliado, el hueso canceloso se desplaza generalmente hacia
afuera de la cavidad 170 de forma controlada, formando
liberablemente una región de hueso comprimido 172 alrededor de una
parte sustancial de la periferia exterior de la cavidad 170.
El catéter-balón 201, que se
describirá a continuación con más detalle, es medido o doblado para
que quepa a través del orificio interior del eje 348 y dentro de un
cuerpo vertebral 105. Una vez colocado en una posición deseada
dentro del cuerpo vertebral 105, el catéter-balón
190 se llena con un medio presurizado de relleno 275 apropiado para
el uso en las aplicaciones médicas que incluye pero no se limita a
aire, nitrógeno, solución salina o agua. Vea las figuras 5D y 11.
El medio de relleno 275 es preferentemente un líquido radiopaco
(como el líquido de Conray® disponible comercialmente en
Mallinkrodt Inc., de St. Louis) MO), que le permite al médico
visualizar el catéter 190 durante la inflación. Si se desea, podrían
usarse formas alternativas para ampliar el catéter, incluidos
expansores mecánicos, gatos, resortes de expansión y/o agentes de
expansión/espumosos, con resultados diferentes.
El catéter 201 puede expandirse a cualquiera
volumen apropiado que cree una cavidad 170 dentro del cuerpo
vertebral 105. Preferentemente, el catéter 201 se amplía hasta un
volumen de al menos 0,20 cc, pero podría expandirse a tamaños
significativamente mayores, como 1, 2, 4, 6 ó 8 cc, en dependencia
de la calidad y densidad ósea. Después de creada la cavidad, se
desinfla el catéter 20l (vea la figura 5E) y se separa del cuerpo
vertebral 105 y del eje 348 (vea la figura 5E). El relleno óseo 180
se introduce a través del eje 348 y en el cuerpo vertebral 105
empleando cualquier tipo de émbolo, émbolo extrusor y/o de línea de
suministro 349 compatible con el cuerpo de la aguja 348. Una vez
que se complete la inyección del relleno óseo, puede extraerse el
eje 348.
Si se desea, una parte del
catéter-balón 201 podría dejarse momentánea o
permanentemente dentro un cuerpo vertebral 105. Por ejemplo, después
de formarse la cavidad y extraerse el medio de inflación, podría
rellenarse la sección ampliada desinflada del
catéter-balón 201 con relleno óseo 180 y dejarla
dentro del cuerpo vertebral 105. Alternativamente, el medio de
inflación 275 podría comprender el relleno óseo 180. Después que el
catéter-balón 201 sea llenado con un medio de
inflación tal, al menos una parte del catéter 201 podría dejarse
permanentemente dentro de la cavidad 170. Alternativamente, el
catéter 201 que está concebido para permanecer dentro de la cavidad
170 podría comprender un material bioabsorbible y/o un material de
tela/malla como la estructura expansible.
Al crear la cavidad 170, la inflación del
catéter 201 provoca que el material expansible 210 presione contra
el hueso canceloso que puede formar una región ósea comprimida o
"concha" 172 a lo largo de gran parte de la periferia de la
cavidad 170. Deseablemente esta concha 172 impedirá o evitará que el
relleno óseo 180 salga de la cavidad 170, inhibiendo así la
extravasación del relleno óseo y/o facilitando la presurización del
relleno óseo 180, si se desea, dentro de la cavidad. A medida que
aumenta la presión en la cavidad 170, las paredes de la cavidad 170
liberablemente serán forzadas aún más hacia el exterior por el
relleno óseo 180, comprimiendo el hueso canceloso adicional dentro
del cuerpo vertebral 105 y/o aumentando el tamaño de la cavidad 170.
Si se dispone de suficiente presión y puede mantenerse la integridad
de la concha 172 sin pérdidas significativas de relleno óseo 180, se
puede desarrollar una presión capaz de desplazar el hueso cortical
fracturado.
Después de formada la cavidad, se introduce una
cantidad de un material, tal como un relleno óseo 180, a través del
eje 348 en el cuerpo vertebral 105 sometido a baja presión. La
cantidad de relleno óseo será deseablemente mayor que el volumen de
la cavidad 170, sin embargo, puede introducirse menos relleno óseo
con resultados variables. Una vez que la cavidad 170 es llenada
sustancialmente, la introducción continuada del relleno óseo 180
presionará el relleno óseo 180 en la cavidad 170 de manera tal que
el aumento de presión provocará que al menos una parte de las
paredes de la cavidad se mueva hacia afuera, aumentando así la
cavidad 170 y comprimiendo más el hueso canceloso y/o desplazando
el hueso cortical. Deseablemente, la introducción del relleno óseo
180 seguirá hasta que parezca inminente la fuga de relleno óseo del
cuerpo vertebral, el hueso cortical haya recuperado su posición
previa a la fractura y/o el profesional determine que se ha
inyectado suficiente relleno óseo 180 en el hueso. Si se desea, el
médico puede utilizar el dispositivo que forma cavidades para crear
cavidades adicionales para el relleno óseo, o puede extraerse el eje
348 del cuerpo vertebral para completar el procedimiento.
El relleno óseo 180 podría ser cualquier
material de relleno empleado en cirugía, incluyendo, pero
limitándose a, tejido aloinjerto o autoinjerto, hidroxiapatita,
resina epoxi, cemento óseo PMMA, o hueso sintético sustituto de
Osteoset® de Wright Medical
Technologies, yeso de París de tipo médico, cemento Skeletal Repair System (SRS®) de Norian Corporation, o Collagraft de Zimmer. A medida que se introduce el relleno óseo 180 en el cuerpo vertebral 105, la introducción es monitoreada deseablemente por la fluoroscopia de rayo X, o por otro dispositivo o método de monitoreo apropiado, para asegurar que el relleno óseo 180 no fluya fuera del cuerpo vertebral 105. Para facilitar la visualización, el relleno óseo 180 puede mezclarse con un agente fluoroscópico, como sulfato de bario radiopaco.

óseo.
Como se muestra en las figuras 5G a la 5L, un
primer relleno óseo 180 se introduce en la cavidad 170, la cantidad
del primer relleno óseo 180 debe ser deseablemente menor que o
aproximadamente igual al volumen de la cavidad 170. Por ejemplo, si
el catéter-balón 201 utilizado para crear la cavidad
170 se infló con 1.0 cc del fluido de inflación, luego se inyectará
inicialmente menos de o aproximadamente 1,0 cc de relleno óseo 180
en la cavidad 170. Por supuesto, si se desea, podría inyectarse en
la cavidad una cantidad del primer relleno óseo 180 mayor que el
volumen de la cavidad. Luego se vuelve a colocar el eje 348 dentro
del cuerpo vertebral 105, vea la figura 5H, con el extremo distal

segunda cantidad de relleno óseo 182 seguirá hasta que parezca
inminente la fuga de relleno óseo del cuerpo vertebral, el hueso
cortical haya recuperado su posición previa a la fractura y/o el
profesional determine que se ha inyectado suficiente relleno óseo
180 en el hueso. Si se desea, el médico pudiera volver a insertar un
catéter 200 para crear una cavidad adicional, o puede extraerse el
eje 348 para completar el procedimiento.
Alternativamente, como se muestra en las figuras
8A a la 8C, puede introducirse un primer material, tal como un
relleno óseo 180, en la cavidad 170 y posteriormente se inserta un
dispositivo que forma cavidades 200 en el hueso. Luego se amplía el
dispositivo que forma cavidades 200 y liberablemente se comprime el
relleno óseo 180 contra las paredes de la cavidad, sellando
cualquier grieta y/o pasos venosos importantes a través de los
cuales fluirá el cemento. Alternativamente, un profesional puede
esperar a que el primer relleno óseo se endurezca parcial o
totalmente antes de retirar el dispositivo que forma cavidades y/o
antes de introducir un segundo material, tal como un relleno óseo.
Posteriormente el segundo material (no mostrado) puede inyectarse
en el cuerpo vertebral sin temor a que se fugue. Si se desea; este
método podría utilizarse siempre que parezca inminente la fuga de
cemento y puede repetirse múltiples veces hasta que el profesional
determine que se ha inyectado suficiente relleno óseo 180 en el
hueso. Además, el profesional podría repetir este procedimiento
hasta que el hueso cortical haya recuperado su posición previa a la
fractura. Alternativamente, el profesional podría utilizar un
dispositivo que forma cavidades antes de la introducción del primer
relleno óseo, y luego introducir el primer relleno óseo en la
cavidad, siguiendo posteriormente alguno de los métodos
descritos.
El primer relleno óseo comprenderá deseablemente
un material que pueda introducirse en la cavidad, pero que resistirá
la extravasación fuera de la cavidad y/o el cuerpo vertebral cuando
se inyecte el segundo relleno óseo en la cavidad. El primero y
segundo rellenos óseos pueden estar compuestos por cemento óseo, con
el primer cemento óseo siendo más resistente a la extravasación que
el segundo cemento óseo. Por ejemplo, los ingredientes del primer
cemento óseo podrían ser elaborados específicamente de manera tal
que el primer cemento óseo cure más rápido que el segundo cemento
óseo. Alternativamente, el primer cemento óseo podría prepararse y/o
introducirse en el cuerpo vertebral antes que el segundo cemento
óseo, permitiendo que el primer cemento óseo se cure parcial o
totalmente antes que el segundo cemento óseo. Alternativamente, la
curación y/o el endurecimiento del primer cemento óseo podría
acelerarse (al aplicar calor, por ejemplo) o podría retrasarse la
curación y/o el endurecimiento del segundo cemento óseo (mediante
enfriamiento, por ejemplo). Alternativamente, el primero y segundo
rellenos óseos están compuestos por cemento óseo, con el primer
cemento óseo siendo deseablemente más viscoso que el segundo
cemento óseo. El primer relleno óseo puede comprender
alternativamente una estructura expansible, como un
stent.

El primer relleno óseo puede comprender un
material más viscoso que el segundo relleno óseo, el primero y
segundo rellenos óseos compuestos por diferentes materiales.
Alternativamente, el primer relleno óseo puede comprender un
material que sea más resistente a la extravasación en el hueso
canceloso que el segundo relleno óseo. Alternativamente, el primer
relleno óseo puede comprender un material que tiene partículas
generalmente más grandes que las partículas del segundo relleno
óseo. En una opción adicional, las partículas del primer relleno
óseo generalmente pueden ser más grandes que el tamaño promedio del
poro dentro del hueso canceloso. Aún más, el primer relleno óseo
puede comprender un material ajustable, como un material plástico de
dos partes u otro biomaterial curable.
Como se muestra en el método representado por
las figuras 16A a la 16D, inicialmente puede introducirse un primer
material, como un relleno óseo 180, en el hueso canceloso 115 de un
hueso humano, tal como un cuerpo vertebral 105. Posteriormente, se
inserta una estructura expansible 210, tal como la que se encuentra
en el extremo distal de un catéter-balón 200, en el
cuerpo vertebral 105. Luego se amplia la estructura expansible 210,
lo que desplaza el relleno óseo 180 y/o el hueso canceloso 115,
creando una cavidad 170 dentro del cuerpo vertebral 105. La
expansión de la estructura expansible 210 puede forzar más al
relleno óseo 180 hacia dentro del hueso canceloso 115, y/o
comprimir más el hueso canceloso. Para reducir al mínimo la fuga del
relleno óseo 180, el relleno óseo se deja endurecer completa o
parcialmente antes de expandir la estructura expansible 210.
Alternativamente, la estructura expansible 210 puede expandirse y el
relleno óseo 180 se dejará endurecer completa o parcialmente
alrededor de la estructura expansible 210. En cualquiera de los
casos, puede introducirse en la cavidad 170 un segundo material,
opcionalmente relleno óseo adicional. El segundo material puede ser
un material que soporte el hueso en una posición de reposo. Este
método puede utilizarse siempre que parezca inminente la fuga de
cemento y puede repetirse múltiples veces hasta que el profesional
determine que se han introducido suficientes cantidades y variedades
de material en el hueso. Alternativamente, el profesional podría
detener la introducción del material de relleno cuando el hueso
cortical recupere o se aproxime a su posición previa a la
factura.
Al crear cavidades y/o las vías de flujo
preferidos dentro del hueso canceloso, no es necesaria la inyección
de presiones extremadamente altas del relleno óseo en el hueso
canceloso. Si se desea, el relleno óseo podría inyectarse en el
hueso a o aproximadamente a presión atmosférica y/o ambiental, o a
presiones menores de aproximadamente 400 libras por pulgada
cuadrada, empleando sistemas de emisión de rellenos óseos como el
descrito en la publicación PCT número WO 00/09024. Así, rellenos
óseos más viscosos (como, por ejemplo, cemento óseo más espeso)
pueden inyectarse dentro del hueso a bajas presiones (como, por
ejemplo, sacar el dispositivo de emisión a una presión de emisión a
o próxima a la presión ambiental o atmosférica), reduciendo las
oportunidades de fuga de cemento y/o la extravasación fuera del
hueso.
En las figuras 9, 10, y 11 se muestra un
dispositivo que forma cavidades comparativo que comprende un
catéter-balón 201. El catéter comprende un tubo
hueco 205, que está compuesto deseablemente por un material de tipo
médico tal como plástico o acero inoxidable. El extremo distal 206
del tubo hueco 205 se rodea de un material expansible 210 compuesto
por un material flexible como el utilizado normalmente para los
catéteres-balón que incluye, pero no se limita a
materiales metálicos, plásticos compuestos, polietileno, mylar, goma
o poliuretano. Una o más aberturas 250 están dispuestas en el tubo
205 cerca del extremo distal 206, permitiendo liberablemente la
comunicación fluida entre el interior del hueco del tubo 205 y el
lumen formado entre el tubo 205 y la estructura expansible 210. Un
accesorio 220, que tiene uno o más puertos de inflación 222, 224, se
asegura al extremo proximal 207 del tubo 205. Una vez que el catéter
201 esté en la posición deseada dentro del cuerpo vertebral 105, se
introduce un medio de inflación 275 dentro del accesorio 220 a
través del puerto de inflación 222, donde este viaja a través del
accesorio 220, a través del tubo hueco 205, a través de la(s)
abertura(s) 250 y hacia el lumen 274 entre, la estructura
expansible 210 y el tubo hueco 205. A medida que continúa la
inyección del medio de inflación 275, la presión del medio de
inflación 275 fuerza a la estructura expansible 210 hacia afuera
del tubo hueco 205, inflándolo hacia afuera y de esa forma comprime
el hueso canceloso 115 y forma una cavidad 170. Una vez alcanzado el
tamaño deseado de la cavidad, el medio de inflación 275 se retira

cavidad 170 y se puede extraer el catéter 200.
Por ejemplo, un catéter-balón
201 apropiado para ser utilizarlo con aguja de un diámetro de 2,3 mm
(calibre 11), podría comprender un tubo hipodérmico hueco de acero
inoxidable 205, que tiene un diámetro exterior de 0,89 mm (0,035
pulgadas) y una longitud de 27,3 cm (10,75 pulgadas). Una o más
aberturas 250 se forman aproximadamente a 0,65 cm (0,25 pulgadas)
del extremo distal del tubo 205. El extremo distal 206 del tubo
hueco 205 puede ser cerrado herméticamente empleando cualquier medio
bien conocido en la técnica, incluyendo adhesivo.
El tubo hueco 205 puede estar rodeado
principalmente por una estructura expansible 210 que comprende un
tubo extruído de plástico. El tubo plástico puede tener un diámetro
interior de 1,17 mm (0,046 pulgadas), un diámetro exterior de 2, 08
mm (0,082 pulgadas) y una longitud de 24,1 cm (pulgadas 9½). El
extremo distal del tubo plástico se une al extremo distal 206 del
tubo hueco 205 por medios bien conocidos en la técnica, tal como un
adhesivo apropiado. Alternativamente, el tubo plástico pueda
sellarse con calor cerca del extremo distal 206 del tubo hueco 205
con medios bien conocidos en la técnica. Una pieza con 1,9 cm (¾
pulgada) de longitud de un tubo termoencogible 215, que tiene un
diámetro exterior de 0,18 cm (3/16 pulgadas), puede asegurarse
alrededor del extremo de la tubería plástica. El extremo proximal de
la tubería hueca 205 puede insertarse en el accesorio 220 y en el
tubo termoencogible 215 se une liberablemente al accesorio 220

accesorio 220, que puede ser un accesorio Luer T, comercialmente
disponible de numerosos proveedores de piezas, puede hacerse de

Alternativamente, como se muestra en la figura 12, la estructura
expansible 210 podría ser significativamente más corta que el tubo
hueco 205 y estar unida en su extremo distal 206 y en su extremo
proximal 209 al tubo hueco 205.
El tubo hueco 205 y una o más aberturas 250
facilitan la extracción del medio de inflación del catéter durante
los procedimientos descritos. Cuando un catéter se desinfla,
normalmente la estructura expansible 210 colapsa contra el tubo
205, que a menudo puede cerrar herméticamente el lumen (a falta de
al menos una vía de extracción secundaria) e impide que se siga
extrayendo el medio de inflación desde la estructura expandida 210
de un catéter. Sin embargo, una o más aberturas 250 cercanas al
extremo distal del tubo 205 pueden permitir que el medio de
inflación 275 sea extraído a través del tubo hipodérmico hueco 205,
desinflando más la estructura expansible 210. Las fuertes paredes
del tubo hipodérmico hueco 205 resisten el colapso al vacío que

desinflado del catéter en unos pocos segundos.
Al inflar el catéter 201, el medio de inflación
275 tratará típicamente de llenar todo el lumen entre la estructura
expansible 210 y el tubo hueco 205, expandiendo así el catéter 201 a
lo largo de toda la estructura expansible 210. Sin embargo, debido
a que gran parte del catéter 201 se encuentra dentro del lumen del
eje 348, con el extremo distal 206 del catéter 201 extendido en el
cuerpo vertebral 105, el eje 348 limitará liberablemente la
expansión de la estructura expansible 210, motivando que la
estructura expansible 210 se expanda principalmente hasta el
extremo distal 206 del catéter 200. Deseablemente, la posterior
inserción o extracción del catéter 201 alterará la cantidad de
estructura expansible 210 que se extiende desde el extremo distal
del eje 348, aumentando o disminuyendo así la longitud de la
estructura expansible 210 que es libre de expandirse dentro del
cuerpo vertebral 105. Al elegir la cantidad de catéter 201 a
insertar en el cuerpo vertebral 105, el profesional puede alterar
la longitud de la estructura expansible y en última instancia el
tamaño de la cavidad 170 creada por el catéter 201, durante el
procedimiento quirúrgico. Por consiguiente, las realizaciones
descritas pueden obviar y/o reducir la necesidad de catéteres
múltiples de diferentes longitudes. Si se desea, se pueden colocar
marcas 269 (vea la figura 9) a lo largo de la sección proximal del
catéter que se corresponden con la longitud del catéter 201 que se
extiende desde el eje 348, permitiendo al profesional medir el
tamaño de la estructura expansible 210 del catéter 200 dentro del
cuerpo vertebral 105. De igual manera, como se describe más
adelante, el dispositivo que forma cavidades 201 podría incorporar
las marcas correspondientes a la longitud de las cerdas 425 que se
extienden más allá de la punta del eje 348.
Como se muestra en la figura 13, la longitud de
una sección expansible 211 del catéter puede limitarse aún más al
asegurar y/o adherir la estructura expansible 210 a un lugar
secundario 214 a lo largo del tubo hueco 205, limitando así la
expansión más allá del lugar secundario 214. Por ejemplo, si una
longitud máxima deseada de la sección expansible 211 fuera de 7,6 cm
(3 pulgadas), entonces la estructura expansible 210 podría
asegurarse al tubo hueco 205 en un lugar secundario 214
aproximadamente de 7,6 cm (tres pulgadas) desde el extremo distal
206 del tubo hueco 205. Deseablemente, esta disposición permitiría a
un profesional elegir una longitud expandida de la sección
expansible 211 de hasta 7,6 cm (tres pulgadas), mientras limita y/o
previene la expansión de la sección restante 203 del catéter 201.
Esta disposición también puede evitar la expansión no deseada de la
parte 202 del catéter que se extiende fuera del extremo proximal 191
del cuerpo del eje 348 (vea la figura 5C).
Como se señaló anteriormente, deseablemente la
estructura expansible se asegura al extremo distal del tubo hueco,
lo que facilitará la recuperación de fragmentos de la estructura
expansible 210 si la estructura expansible 210 es desgarrada o
dañada, tal como por una rasgadura radial completa. Dado que el tubo
hueco 205 permanecerá liberablemente adjuntado a los fragmentos (no
mostrados) de la estructura expansible 210, estos fragmentos podrán
retirarse del cuerpo vertebral 105 con el tubo hueco 205. Además,
deseablemente el anexo distal evitará y/o reducirá la expansión
significativa de la estructura expansible 210 a lo largo de los ejes
longitudinales del tubo hueco 205.
La figura 17 representa otro dispositivo que
forma cavidades 300. El tubo hueco 205 se extiende a través del
accesorio 220, tal como un accesorio en forma de T, y se asegura a
una tapa 310. El tubo hueco 205 es preferiblemente capaz de rotar
en relación con el accesorio 220. Si se desea, un sello (no
mostrado), tal como un anillo de silicona o de Teflon®, puede
incorporarse al accesorio proximal 222 para limitar y/o prevenir que
la fuga del medio de inflación pase el tubo hueco 205.
Al usarse, un dispositivo que forma cavidades
300 comprime el hueso canceloso y/o forma una cavidad de manera
similar a las realizaciones anteriormente descritas. Sin embargo,
una vez formada la cavidad y extraído el dispositivo 300 si se
desea, puede rotarse la tapa 310, torciendo el material expansible
210 en relación con el accesorio 220 y girando la estructura
expansible 210 contra el tubo hueco 205, liberablemente reduciendo
al mínimo el diámetro general exterior de la parte expansible del
dispositivo 300. Luego el dispositivo 300 puede retirarse
fácilmente a través del eje 348. Aun en los casos en que la
estructura expansible 210 se haya deformado plásticamente, o se ha
quebrado de alguna manera, la estructura expansible 210 puede
enrollarse alrededor del tubo hueco 205 para facilitar la
extracción y/o inserción. Alternativamente, el tubo hueco 205 puede
desplazarse en relación con el eje longitudinal del accesorio 220,
lo que extiende y/o contrae la estructura expansible 210 contra el
tubo hueco 205.
Las figuras 6A y 6B representan un dispositivo
que forma cavidades 410. El dispositivo que forma cavidades 410
comprende un eje 420 que se ajusta deseablemente para que pase a
través del eje 348 de un dispositivo de inserción 350. Un conjunto
de agarraderas 415, que facilita la manipulación del dispositivo que
forma cavidades 410, se asegura al extremo proximal 412 del eje
420. Uno o más alambres o "cerdas" 425 se aseguran al extremo
distal 423 del eje 420. Las cerdas 425 pueden asegurarse al eje 420
mediante soldeo, soldadura, adhesivos u otros medios de seguridad
bien conocidos en la técnica. Alternativamente, la(s)
cerda(s) 425 puede(n) formar parte integral del eje
420, o pueden grabarse a partir de un eje empleando un rayo láser u
otros medios bien conocidos en la técnica. Las cerdas y el eje

348, pero pueden expandirse o "extenderse" al salir del eje
348. Si se desea, las cerdas pueden ser rectas o curvas, para
facilitar su paso por el hueso canceloso 115. Además, si se desea,
una o más de las cerdas 425 pueden ser huecas, permitiendo a un
profesional tomar una muestra de biopsia del hueso canceloso durante
la inserción del dispositivo 410.
Como se muestra en la figura 7, el dispositivo
que forma cavidades 410 puede insertarse liberablemente a través de
un eje 348 colocado en un hueso proyectado, tal como un cuerpo
vertebral 105. A medida que las cerdas 425 entran al hueso canceloso
115, las cerdas 425 desplazarán liberablemente al hueso 115 y
crearán una o más cavidades 426 o las vías de flujo preferidas en
el cuerpo vertebral. Si se desea, un profesional puede extraer las

óseo (no mostrado), a través del eje 348. El relleno óseo
liberablemente se desplazará inicialmente a través de las cavidades

interrumpir la introducción del relleno óseo y crear cavidades
adicionales al volver a insertar el dispositivo que forma cavidades
410. Además, en caso de que ocurra o que sea inminente una fuga de
relleno óseo, un profesional puede interrumpir la inyección de
relleno óseo, crear cavidad(es) adicionales como se describió
anteriormente, esperar a que el relleno óseo introducido/fugado se
endurezca lo suficiente para resistir la extravasación adicional y
luego seguir la introducción del relleno óseo. Como se describió
anteriormente, el relleno óseo podría comprender muchos materiales
diferentes, o combinaciones de materiales, con diferentes
resultados.
La figura 14 representa otro dispositivo que
forma cavidades 500. El dispositivo que forma cavidades 500
comprende un eje 520 que se ajusta para pasar a través del eje 348
de un dispositivo de inserción 350. Un conjunto de agarraderas 515,
que facilita la manipulación del dispositivo que forma cavidades
500, se asegura al extremo proximal 512 del eje 520. El eje 520 del
dispositivo que forma cavidades 500 es deseablemente más largo que
el eje 348 del dispositivo de inserción 350. El extremo distal 525
del eje 520 puede ser biselado (no mostrado) para facilitar el paso
a través del hueso canceloso 115, o puede redondearse o aplanarse
para reducir al mínimo las oportunidades de que penetre la pared
anterior 10 del cuerpo vertebral 105. Además, si se desea, el
extremo distal 525 del eje 520 podría ser hueco (no mostrado),
permitiendo al profesional tomar una muestra de biopsia del hueso

La figura 15 representa un dispositivo adicional
que forma cavidades 600 que comprende un eje 620 que se ajusta para
pasar a través del eje 348 de un dispositivo de inserción 350. Un
conjunto de agarraderas 615, que facilita la manipulación del
dispositivo que forma cavidades 600, se asegura al extremo proximal
612 del eje 620. El eje 620 es deseablemente más largo que el eje
348 del dispositivo de inserción 350. El extremo distal 625 del eje

la manipulación del dispositivo que forma cavidades 600a, se asegura
al extremo proximal 612a del eje 620a. El eje 620a es deseablemente
más largo que el eje 348 del dispositivo de inserción 350. El
extremo distal 625a del eje 620a puede redondearse o biselarse para
facilitar el paso a través del hueso canceloso 115, o puede
aplanarse para reducir al mínimo las oportunidades de que penetre la
pared anterior 10 del cuerpo vertebral 105.
Una abertura o ventana 700 se forma
deseablemente en el eje 620a. Como se muestra en las figuras 19 y
20, una estructura expansible 710 es colocada al menos parcialmente
dentro del eje 620a, deseablemente en una posición adyacente a la
ventana 700. Con la introducción del líquido de inflación a través
de un lumen que se extiende a través del eje 620a, la estructura
expansible 710 se expande y al menos una parte de la estructura
expansible 710 se extenderá fuera del eje 620a por la ventana 700.
Deseablemente, a medida que la estructura continúa expandiéndose, la
estructura expansible 710 "crecerá" (P1 a P2 a P3 en la figura
20) a través de la ventana 700, de ese modo se compacta el hueso
canceloso, se crea una cavidad y/o se desplaza un hueso cortical.
Con la contracción de la estructura expansible 710, se extraerá la
mayor parte de la estructura expansible 710 deseablemente hacia el
eje 620a para retirar la herramienta del cuerpo vertebral. En una
realización, al menos una parte del material que comprende la
estructura expansible 710 se deformará plásticamente a medida que se
expanda.
La estructura expansible 710 puede estar
compuesta de un material flexible común en las aplicaciones de los
dispositivos médicos, que incluye, pero no se limita a, plásticos,
polietileno, mylar, caucho, nailon, poliuretano, metales,
materiales de cerámica o compuestos. Deseablemente, el eje 620a
comprenderá un material que es más resistente a la expansión que el
material de la estructura expansible 710, que incluye, pero no se
limita a acero inoxidable, cerámica, material compuesto y/o
plásticos rígidos. En una representación alternativa, puede
emplearse materiales similares para la estructura expansible 710 y
el eje 620a, pero de diferente espesor y/o en diferentes cantidades,
introduciendo así la estructura expansible para que sea más propensa
a la expansión que el material del eje 620a. La estructura
expansible 710 puede unirse directamente al eje 620a por diversos
medios bien conocidos en la técnica, incluyendo, pero no limitándose
a medios tales como soldadura, fusión, engomado o similares. En las
realizaciones alternativas, la estructura expansible puede
asegurarse dentro o fuera del eje 620a, o una combinación de
estos.
Como se señaló anteriormente, cualquiera de los
dispositivos que forman cavidades 500, 600 y 600a pueden insertarse
a través de un eje 348 colocado en un hueso seleccionado, como un
cuerpo vertebral 105. A medida que el (los) dispositivo(s)
se introduce(n) en el hueso canceloso 115, desplazarán
liberablemente al hueso 115 y crearán una o más cavidades en el
cuerpo vertebral. Si se desea, el médico puede retirar el (los)
dispositivo(s) de vuelta hacia el eje 348 y reinsertar si es
necesario para crear la(s) cavidad(es)
deseada(s) en el hueso canceloso 115.
En la realización de un dispositivo que forma
cavidades de las figuras 18-20, el dispositivo que
forma cavidades 600a puede utilizarse sin el dispositivo de
inserción asociado. En tal caso, el dispositivo que forma cavidades
incorporará una punta distal afilada capaz de penetrar en los
tejidos blandos y en el hueso cortical/canceloso del cuerpo
vertebral. Si se desea, la punta distal puede ser hueca o de una
construcción sólida. De igual manera, la ventana puede extenderse
alrededor de una mayor o menor parte de la periferia del eje 620a,
en dependencia del tamaño y la configuración de la estructura
expansible y la fuerza deseada del dispositivo que forma
cavidades.
Al crear una o más cavidades dentro del hueso
canceloso 115, los dispositivos que forman cavidades de la presente
invención crean deseablemente vías de flujo preferidas para el
relleno óseo 180. Además, deseablemente los dispositivos que forman
cavidades también pueden cerrar y/o bloquear otras vías de flujo
natural fuera de la cavidad, como las venas y/o las grietas en el
hueso canceloso. Además, los métodos y dispositivos descritos en
este documento pueden emplearse para manipular el relleno óseo ya
introducido en el hueso. Por lo tanto, la presente invención reduce
las oportunidades de fuga del cemento fuera del cuerpo vertebral y/o
mejora la distribución del relleno óseo en todas las partes
significativas del cuerpo vertebral. Además, la creación de las
cavidades y vías de flujo deseadas descritas en la presente
invención permiten la colocación del biomaterial con más seguridad,
bajo mayor control y a presiones infe-
riores.

Además de los usos específicos descritos
anteriormente, los métodos y dispositivos que forman cavidades
descritos en este documento también serían adecuados para usarlos
en el tratamiento y/o reforzamiento de los huesos debilitados,
enfermos y/o fracturados y otros órganos en diversas ubicaciones en
todo el cuerpo. Por ejemplo, los dispositivos y métodos descritos
podrían emplearse para distribuir materiales de refuerzo y/o
medicamentos, tales como fármacos contra el cáncer, células óseas
sustitutas, colágeno, matriz ósea, calcio desmineralizado y otros
materiales/medicamentos, directamente a un hueso fracturado,
debilitado y/o enfermo, aumentando así la eficacia de los
materiales, reforzando el hueso y/o acelerando la cura. Además, la
inyección de tales materiales en un hueso dentro de un cuerpo
podría permitir que el medicamento/material emigre y/o se transporte
a otros huesos y/u órganos en el cuerpo, mejorando así la calidad
de los huesos y/o de los otros órganos no inyectados directamente
con los materiales y/o medicamentos.
Otras realizaciones y usos de la invención serán
evidentes para los especialistas en la técnica a partir de la
consideración de la especificación y la práctica de la invención
descrita en este documento. La especificación y los ejemplos deben
considerarse ejemplares sólo con el alcance real de la invención
indicada por las siguientes reivindicaciones. Como podrán entender
fácilmente los especialistas en la técnica, se pueden hacer con
facilidad variaciones y modificaciones de cada una de las
realizaciones descritas dentro del alcance de esta invención como se
define en las siguientes reivindicaciones.
Claims (11)
1. Un catéter-balón (600a) que
comprende:
- un tubo (620a) que comprende un extremo proximal (612a)
- un extremo distal cerrado (625a);
- un lumen dentro del tubo (620a) para transportar un medio de inflación;
- una abertura (700) formada en una pared lateral del tubo (620a);
- un accesorio (615a) en o cerca del extremo proximal (612a) del tubo (620a) que se adapta para acoplarse liberablemente a una fuente del medio de inflación, el accesorio tiene uno o más puertos de inflación (222, 224) que se comunican con el lumen; y una estructura expansible (710) que es llevada y colocada al menos parcialmente dentro del tubo (620a) que se comunica con el lumen, al menos una parte de la estructura expansible se expande a través de la abertura como respuesta a la introducción del medio de inflación a través de uno o más puertos de inflación (222, 224) y el lumen.
2. El catéter-balón (600a) de la
reivindicación 1, en el que la estructura expansible (710) es de
poliuretano.
3. El catéter-balón (600a) de la
reivindicación 1, en el que la estructura expansible (710) está
unida al lumen del eje.
4. El catéter-balón (600a) de la
reivindicación 3, en que la estructura expansible está unida por
soldadura.
5. El catéter-balón (600a) de la
reivindicación 3, en el que la estructura expansible está unida por
fusión.
6. El catéter-balón (600a) de la
reivindicación 3, en el que la estructura expansible está unida por
pegamento.
7. El catéter-balón (600a) de la
reivindicación 1, en el que el extremo distal (625a) es
redondeado.
8. El catéter-balón (600a) de la
reivindicación 1, en el que el extremo distal (652a) es
aplanado.
9. El catéter-balón (600a) de la
reivindicación 1, en el que el extremo distal (625a) es
puntiagudo.
10. El catéter-balón (600a) de
la reivindicación 1, que además comprende un dispositivo de
inserción (350) ajustado y configurado para permitir el paso
percutáneo del tubo (620a).
11. El catéter-balón (600a) de
la reivindicación 10, en el que el dispositivo de inserción (350)
es una
