Abstract

Juego para la realización de un implante como sustitución de una cavidad cotiloidea del hueso coxal destruida, que presenta - una cavidad cotiloidea (1) que puede colocarse en el acetábulo con al menos un lado exterior que porta por tramos una estructura reticular (9) tridimensional de mallas abiertas, que presenta al menos tres taladros (2) que atraviesan su pared, caracterizado porque los taladros están cercados en el lado exterior de la cavidad cotiloidea (1) por una tubuladura esférica (3), respectivamente, y que está realizada con una zona saliente hacia un lado, a diferencia de una cavidad en forma de calota idealizada, de tal forma que, en una vista en planta desde arriba, presenta un contorno fundamentalmente ovalado y, en alzado lateral, semiovalado, - al menos un elemento de fijación (4) alargado, que puede ser acoplado a la cavidad cotiloidea (1) en la zona de los al menos tres taladros (2) y que está realizado de tal forma que el extremo orientado hacia el lado exterior de la cavidad cotiloidea (1) está conformado cóncavamente, de modo que forma con la tubuladura esférica (3) que cerca los al menos tres taladros (2) una articulación esférica que permite el giro del elemento de fijación (4), - al menos un perno roscado (6), que puede colocarse a través del al menos un taladro (2) y que puede enroscarse en el al menos un elemento de fijación (4) de tal forma que el elemento de fijación (4) puede ser inmovilizado para impedir que siga girando.

Description

Juego para la realización de un implante como sustitución de una cavidad cotiloidea del hueso coxal destruida.
La presente invención se refiere a un juego para la realización de un implante como sustitución de una cavidad cotiloidea del hueso coxal destruida.
Se conoce este tipo de juegos. A título de ejemplo se remite a un juego como está descrito en el documento DE 39 18 970 C, que presenta las características del preámbulo de la reivindicación 1.
Este juego está formado por una cavidad cotiloidea en forma de calota que puede introducirse a presión en una cavidad cotiloidea del hueso coxal y, respectivamente, al menos un primer y segundo elemento de fijación para arriostrar la cavidad cotiloidea en el hueso coxal. Un primer elemento de fijación puede insertarse pasando por un orificio en la cavidad cotiloidea y puede guiarse allí de forma desplazable, estando orientado radialmente hacia fuera. En la superficie exterior de la cavidad cotiloidea está previsto un listón de montaje envuelto por la estructura metálica de células abiertas de la cavidad cotiloidea con al menos dos soportes de fijación, en los que puede insertarse a elección uno o varios segundos elementos de fijación.
Existen lesiones en la zona pélvica en cuyo caso ya no puede usarse el juego mencionado, puesto que las zonas óseas dañadas son tan grandes que los elementos de fijación mencionados ya no pueden cubrir los defectos. El implante fabricado con el juego conocido ya no es apto para una fijación primaria en las indicaciones de este tipo, puesto que las superficies de contacto entre el hueso pélvico y el implante son demasiado pequeñas, para que pueda crecer suficiente hueso en la estructura superficial de la cavidad cotiloidea. La fijación primaria insuficiente conlleva prácticamente la fuerza la fijación secundaria insuficiente por una falta de integración de suficiente material óseo en la superficie del implante.
Con estos antecedentes, la presente invención tiene el objetivo de especificar un juego para la fabricación de un implante apto para la fijación secundaria como sustitución de una cavidad cotiloidea del hueso coxal destruida, que permita la fabricación de un implante de cavidad cotiloidea pélvica que pueda implantarse de forma estable mediante fijación secundaria, incluso en caso de destrucciones muy graves.
Este objetivo se consigue mediante el juego con las características de la reivindicación 1. Configuraciones ventajosas están indicadas en las reivindicaciones subordinadas.
Según esto está previsto que el juego mencionado presente lo siguiente:
-
una cavidad cotiloidea que puede colocarse en el acetábulo con al menos un lado exterior que porta por tramos una estructura reticular tridimensional de mallas abiertas, que presenta al menos tres taladros que atraviesan su pared, que están cercados en el lado exterior de la cavidad cotiloidea por una tubuladura esférica, respectivamente, y que está realizada con una zona saliente hacia un lado, a diferencia de una cavidad en forma de calota idealizada, de tal forma que, en una vista en planta desde arriba, presente un contorno fundamentalmente ovalado y, en alzado lateral, semiovalado,

al menos un elemento de fijación alargado, que puede ser acoplado a la cavidad cotiloidea en la zona de los al menos tres taladros y que está realizado de tal forma que el extremo orientado hacia el lado exterior de la cavidad cotiloidea está conformado cóncavamente, de modo que forme con la tubuladura esférica que cerca los al menos tres taladros una articulación esférica que permita el giro del elemento de fijación,

al menos un perno roscado, que puede colocarse a través del al menos un taladro y que puede enroscarse en el al menos un elemento de fijación de tal forma que el elemento de fijación pueda ser inmovilizado para impedir que siga girando.
Según la invención, el juego básico está formado por tres componentes fundamentales. El primer componente es la cavidad cotiloidea realizada de forma ovalada, cuya configuración permite ofrecer mejores coberturas en el caso de un acetábulo prácticamente del todo destruido. En el caso de una cavidad cotiloidea puramente en forma de calota, prácticamente no podría conseguirse un arrastre de forma deseado con el acetábulo. La cavidad cotiloidea porta en su lado exterior una estructura reticular tridimensional de mallas abiertas, en la que crece material óseo de forma conocida después de una estimulación, consiguiéndose de esta forma la fijación secundaria. El cercado de los al menos tres taladros que atraviesan la pared de la cavidad cotiloidea mediante una tubuladura esférica, respectivamente, debe verse en combinación con el segundo componente fundamental, es decir, el al menos un elemento de fijación alargado. Gracias a la formación de una articulación esférica entre la tubuladura esférica y el extremo realizado de forma cóncava del elemento de fijación es posible que el elemento de fijación pueda entrar en posiciones de grandes intervalos de ángulos de giro respecto a la cavidad cotiloidea. El elemento de fijación se fija mediante un perno roscado, que se coloca a través del taladro correspondiente en la cavidad cotiloidea y que se enrosca en el elemento de fijación.
La implantación del implante en la operación se realiza de la siguiente manera:
El acetábulo destruido se limpia, dado el caso, algo más mediante cinceles y la cavidad cotiloidea se inserta de tal forma en el hueco que se consiga el máximo arrastre de forma posible entre el hueso coxal y el lado exterior de la cavidad cotiloidea.
A continuación, se fresa un taladro para insertar el elemento de fijación en regiones adecuadas del hueso coxal, es decir, por ejemplo hacia el hueso sacro o el ilion, lo cual parece siempre más favorable para una colocación, realizándose el taladro concretamente a través del interior de la cavidad cotiloidea, que sirve en este caso, por así decirlo, de plantilla para taladrar. En el concepto según la invención, todos los huecos fresados para el alojamiento del elemento de fijación están orientados en la dirección craneal, puesto que la fijación está orientada expresamente en primer lugar a fuerzas de tracción bajo carga. A continuación, el operador retira la cavidad cotiloidea del acetábulo restante e inserta uno o varios elementos de fijación en uno o más huecos fresados, previamente limpiados. Acto seguido, el operador vuelve a insertar la cavidad cotiloidea en el acetábulo restante y ejerce presión sobre la misma hasta que los elementos de fijación vuelvan a ser visibles a través de los taladros correspondientes en la pared de la cavidad cotiloidea o hasta que se puedan palpar mediante una ayuda de enhebrado, por ejemplo en forma de un alambre. Si esto es el caso, el operador coloca un perno roscado por elemento de fijación a través de los taladros correspondientes y enrosca el perno roscado en el elemento de fijación, de tal forma que puedan transmitirse fuerzas de tracción a la cavidad cotiloidea.
La conformación de la tubuladura esférica en el lado exterior de la cavidad cotiloidea junto con la realización cóncava del extremo de acoplamiento del elemento de fijación permite que las dos partes encuentren automáticamente su posición óptima, sin esfuerzos, una respecto a la otra. La unión duradera entre la cavidad cotiloidea y el elemento de fijación no se realiza hasta el enroscado del perno roscado, haciéndose esto de tal forma que puedan transmitirse fuerzas y momentos. El implante realizado de esta manera in situ depende de la fijación secundaria, es decir, de la integración de material óseo en las superficies exteriores de las partes del implante. Esto significa que la pelvis así tratada no puede cargarse inmediatamente después de la operación, sino que deben pasar primero algunas semanas. No obstante, esto supone para los pacientes tratados aún un aumento considerable de calidad de vida en comparación con el estado preoperatorio.
Cuando parece conveniente y posible, pueden estar previstos elementos de fijación adicionales, que pueden colocarse a través de los taladros restantes en la cavidad cotiloidea. Estos elementos de fijación pueden ser preferiblemente tornillos para hueso. No obstante, éstos no sirven para una eventual fijación primaria del implante en el acetábulo restante sino, por el contrario, para el arriostrado de la cavidad cotiloidea in situ, para estimular el crecimiento del hueso y, por lo tanto, la integración en los lados exteriores de las partes del implante.
Los al menos tres taladros están realizados preferiblemente de tal forma que se ensanchen cónicamente hacia el interior de la cavidad cotiloidea. En este caso, la cabeza del perno roscado está realizada preferiblemente como cabeza avellanada. Esta combinación permite girar el perno roscado de un lado a otro en el taladro en la cavidad cotiloidea.
El elemento de fijación está formado preferiblemente por un cuerpo de armadura hueco, de pared fina, con un número de perforaciones tal en su pared exterior que la relación de superficies totales de las perforaciones respecto a su superficie total sea al menos de 1:2. Aquí no solamente es importante la integración de material óseo en el interior del elemento de fijación para la fijación secundaria del conjunto del implante, sino también la realización elástica de la pared del elemento de fijación, como se manifiesta con el concepto de pared fina. De esta forma, todo el sistema se vuelve más flexible y se aumenta su tolerabilidad. La desviación elástica de la pared del elemento de fijación es del orden micrométrico.
La invención se explicará más detalladamente con ayuda de un ejemplo de realización. Aquí muestran:

A continuación, las piezas idénticas están provistas del mismo signo de referencia.
La figura 1 ofrece una primera vista de conjunto. Puede verse la cavidad cotiloidea 1 con su lado exterior recubierto en gran medida con una estructura reticular tridimensional de mallas abiertas. A diferencia de una cavidad en forma de calota idealizada, la cavidad cotiloidea ovalada está realizada de forma saliente hacia un lado, en la figura 1, hacia arriba. La pared de la cavidad cotiloidea 1 presenta seis taladros 2. Los taladros 2 están cercados en el lado exterior de la cavidad cotiloidea mediante una tubuladura esférica o un segmento esférico 3, respectivamente. La tubuladura esférica 3 debe verse en combinación con el elemento de fijación 4. Éste presenta en su extremo de acoplamiento una forma cóncava 5 (figura 3), que actúa junto con la tubuladura esférica 3 a modo de una articulación esférica. Esto permite al elemento de fijación entrar en una posición girada respecto a la cavidad cotiloidea 1, sin perder el arrastre de forma entre la cavidad cotiloidea 1 y el elemento de fijación 4.
La cabeza 7 del perno roscado 6 está realizada en este caso como cabeza avellanada y actúa en combinación con los taladros 2 que se estrechan hacia el interior de la cavidad cotiloidea en el sentido de una suspensión casi cardán del elemento de fijación 4 en la cavidad cotiloidea 1.
Es posible acoplar más de un elemento de fijación 4 a la cavidad cotiloidea 1, según el cuadro clínico y los huesos aún existentes, cargables de la pelvis. En los taladros 2 restantes pueden insertarse tornillos para hueso (no representados), sobre todo en la dirección caudal, y enroscarse en el hueso restante para ejercer presión sobre la cavidad cotiloidea en el acetábulo restante, lo cual conduce a su vez a la estimulación del material óseo adyacente, lo cual favorece la integración de partículas óseas en la estructura reticular tridimensional 9, reduciendo la fase de convalecencia.
