Description

Die Erfindung betrifft eine Hüftpfanne für das Acetabulum mit den Merkmalen des Patentanspruchs 1.
Eine derartige Hüftpfanne ist in der EP 0 121 002 A1 beschrieben. Das sägezahnförmige Profil besteht aus Lappen, die durch Einschnitte an Verankerungsrippen gebildet sind. Dadurch bestehen zwischen in Längs­ richtung des Verankerungszapfens benachbarten Lappen Abstände. Eine spezielle Gestaltung der Lappen ist nicht beschrieben.
Eine ähnliche Hüftpfanne ist in der DE 33 41 724 C1 beschrieben. Der dortige Verankerungszapfen ist zylindrisch gestaltet und weist für das Einwachsen von Gewebe an seinem Umfang Fällen auf. Um den Zapfen in das Acetabulum einzutreiben, wird in dieser eine Bohrung gefertigt, deren Durchmesser dem Durchmesser des Verankerungszapfens entspricht. Es läßt sich damit nur beschränkt ein fester Sitz des Verankerungszapfens erreichen. Außerdem wird mit nur einem einzigen solchen Verankerungszapfen ein drehfester Sitz nicht erreicht.
Aufgabe der Erfindung ist es, durch eine besondere Gestalt eines Zapfens die Verankerung des Pfannenkörpers in das Acetabulum zu verbessern.
Erfindungsgemäß ist obige Aufgabe durch die Merkmale des kennzeichnenden Teils des Anspruchs 1 gelöst.
Durch die konische Grundform ist erreicht, daß beim Eintreiben des Verankerungszapfens in die am Acetabulum vorbereitete Bohrung in dieser befindliches oder in sie eingebrachtes Material, wie Spongiosa, Knochenspäne und/oder Knochenmehl, verdichtet wird und zwar nicht nur am Grund der Bohrung, sondern vorzugsweise an den Seiten, also zwischen der Umfangsfläche des Verankerungszapfens und der Bohrung. Dadurch wird ein primär fester Sitz gewährleistet.
Das als Widerhaken wirkende sägezahnförmige Profil bildet Hinterschneidungen, in die das genannte Material beim Eintreiben in die Bohrung des Acetabulums eintritt, was beim Einwachsen des Verankerungszapfens zu einem sekundär sicheren Sitz führt. Das sägezahnförmige Profil nimmt in seinen Hinterschneidungen Material auf, das beim Eintreiben des Verankerungszapfens in der Bohrung komprimiert wird und aufgrund der konischen Grundform die Tendenz hat, nach außen, in Richtung des Pfannenkörpers zu wandern.
Ein besonders fester Sitz des Verankerungszapfens in der Bohrung des Acetabulums wird dadurch erreicht, daß der Durchmesser der konischen Außenkontur in seinem größeren Durchmesserbereich größer ist als der Durchmesser der im Acetabulum vorbereiteten Bohrung.
Das sägezahnartige Profil ist am Umfang des Verankerungszapfens durch Einbuchtungen unterbrochen, so daß mehrere, am Umfang des Verankerungszapfens verteilte, in Längsrichtung des Verankerungszapfens verlaufende Reihen von Sägezähnen und zwischen diesen Reihen in Längsrichtung des Verankerungszapfens verlaufende Rinnen bestehen. Beim Eintreiben des am Pfannenkörper festliegenden Verankerungszapfens in die Bohrung des Acetabulums schneiden die Sägezähne in Eintreibrichtung zunehmend in den Knochen (Acetabulum) ein, was einerseits das Eintreiben erleichtert und andererseits zu einem auch gegen Drehung um die Längsachse des Verankerungszapfens festen Sitz führt. Die Einbuchtungen bzw. Rinnen können zahnfrei sein. Sie unterstützen das Eindrücken von in der Bohrung befindlichen Material in die Hinterschneidungen der Sägezähne. Außerdem bilden die Rinnen Raum für das nach dem Einwachsen einen sekundär festen Sitz gewährleistende Material. Die vergleichsweise große Oberfläche der Rinnen unterstützt dies.
Der Grund der Einbuchtungen liegt in Längsrichtung des Verankerungszapfens auf einer Innenkontur, die sich zum Pfannenkörper hin konisch erweitert, wobei der Winkel dieser konischen Erweiterung der Innenkontur kleiner ist als der Winkel der konischen Erweiterung der Außenkontur.
In Weiterbildung der Erfindung ist am Verankerungszapfen zwischen dem Pfannenkörper und dem sägezahnförmigen Profil ein Bund ausgebildet, dessen Durchmesser kleiner als der ihm nächstliegende Durchmesser der Außenkontur. In Richtung des Bundes beim Eintreiben des Verankerungszapfens aus der Bohrung ausgeschobenes Material wird im Bereich des Bundes aufgenommen bzw. verdichtet. Der Bund bildet dabei einen Abschluß der Bohrung und unterstützt den primär und sekundär festen Sitz des Zapfens im Knochen (Acetabulum). Der Bund sitzt im Preßsitz in der Corticalis.
Weitere vorteilhafte Ausgestaltungen der Erfindung ergeben sich aus den Unteransprüchen und der folgenden Beschreibung eines Ausführungsbeispiels. In der Zeichnung zeigen:

An einem in ein Acetabulum (A) einsetzbaren Pfannenkörper (1) ist in einem geeigneten Winkel ein Verankerungszapfen (2) befestigt. Der Verankerungszapfen (2) ist hierbei mit einem zylindrischen Endbereich (3) in eine Bohrung (4) des Pfannenkörpers (1) eingepreßt.
Der Pfannenkörper (1) weist zusätzlich wenigstens eine Durchbrechung (5) auf, in die eine Sicherungsschraube (6) in verschiedenen Winkeln einsteckbar ist. Die Sicherungsschraube (6) wird in das Acetabulum (A) eingeschraubt, wenn der Pfannenkörper (1) mit seinem Verankerungszapfen (2) in einer mittels einer Lehre am Acetabulum (A) vorbereiteten Bohrung (B) des Durchmessers (D) eingetrieben ist.
Das Acetabulum (A) ist zur Aufnahme eines Einsatzteils (1') bestimmt, welches zur Aufnahme eines im wesentlichen kugelförmigen Kopfes der Hüftgelenksprothese ausgelegt ist.
Der Verankerungszapfen (2) hat in seinem Bereich, mit dem er in die den Durchmesser (D) aufweisende Bohrung (B) des Acetabulums (A) einzutreiben ist, eine sich zum Pfannenkörper (1) hin konisch erweiternde Außenkontur (7). Er weist ein sägezahnförmiges Profil auf, das im Beispielsfalle von sechs Reihen (vgl. Fig. 3, 4) von jeweils vier Sägezähnen (8, 9, 10, 11) gebildet ist (vgl. Fig. 2). Die abgeflachten Spitzen (8', 9', 10', 11') der Sägezähne (8 bis 11) liegen auf der konischen Außenkontur (7). Die steilen Flanken (8", 9", 10", 11") der Sägezähne (8 bis 11) sind jeweils dem Pfannenkörper (1) zugewandt.
Zwischen den Sägezahnreihen bestehen ausgefräste Einbuchtungen, die Rinnen (12) bilden. Die Rinnen (12) sind im Querschnitt des Verankerungszapfens (2) gesehen (vgl. Fig. 3, 4) konkav gewölbt. Die Gründe (13) der Rinnen (12) liegen auf einer Innenkontur (14), die sich zum Pfannenkörper (1) hin konisch erweitert. Der Erweiterungswinkel (a) der konischen Außenkontur (7) ist größer als der konische Erweiterungswinkel (b) der Innenkontur (14). Der Winkel (a) beträgt beispielsweise etwa 20°. Der Winkel (b) beträgt beispielsweise etwa 9°.
Die Sägezähne (8 bis 11) jeder Sägezahnreihe fluchten in Richtung der Längsachse (15) der Verankerungszapfens (2). Die Breite (c) der Rücken (16) der Sägezähne (8 bis 11) nimmt in Richtung auf den Pfannenkörper (1) zu, was einerseits das Eintreiben des Verankerungszapfen (2) in die Bohrung (B) erleichtert und andererseits einen festen Sitz unterstützt.
Die Länge (l) der Sägezähne (8 bis 11) vergrößert sich zum Pfannenkörper (1) hin (vgl. Fig. 2).
Der Verankerungszapfen (2) läuft an seinem dem Pfannenkörper (1) abgewandten Ende (17) im Vergleich zur konischen Außenkontur (7) spitz zu, was das Einsetzen des Verankerungszapfens (2) in die Bohrung (B) des Acetabulums (A) erleichtert. Im Bereich des äußersten Sägezahnes (11) oder des aus diesem übergehenden Ende (17) entspricht der Durchmesser (d1) (vgl. Fig. 2, 4) dem Durchmesser (D) der Bohrung (B) des Acetabulums (A). Dieser Durchmesser beträgt etwa 4 mm. Aufgrund der konischen Außenkontur (7) nimmt der Durchmesser des Verankerungszapfens (2) bei den Rücken (16) der Sägezähne (8) den Wert (d2) an. Der Durchmesser (d2) (vgl. Fig. 3) ist deutlich größer als der Durchmesser (D) der Bohrung (B). Er beträgt etwa 6,5 mm.
Wie Fig. 3 zeigt, ist wenigstens im Bereich der Sägezähne (8) die von den Rinnen (12) gebildete Oberfläche größer als die auf dem Durchmesser (d2) liegende Fläche. Die Rinnen (12) bilden also nicht nur einen Aufnahmeraum für in die Bohrung (B) eingefülltes und aus dieser beim Eintreiben des Verankerungszapfens (2) tendenziell ausgetriebenes Material, sondern auch eine große Oberfläche für dessen Einwachsen.
Zwischen dem in die Bohrung (4) des Pfannenkörpers (1) eingesetzten zylindrischen Endbereich (3) und dem Sägezahn (8) ist am Verankerungszapfen (2) ein durchgehend umlaufender Bund (18) ausgebildet, der mit einer Schräge (19) in den zylindrischen Endbereich (3) und mit einer Schräge (20) in den Grund der steilen Flanke (8") des ihm nächsten Sägezahns (8) übergeht. Der Durchmesser (d3) des Bundes (18) ist kleiner als der Durchmesser (d2), auf dem die Spitzen (8') liegen. Der Durchmesser (d3) ist größer als der ihm nächstliegende Durchmesser (d4) der Innenkontur (14). Der Bund (18) bildet im in die Bohrung (b) des Acetabulums (A) eingetriebenen Zustand des Verankerungszapfens (2) einen verdichtenden Abschluß und sitzt in der Corticalis des Acetabulums (A) im Preßsitz fest.
Bei der Implantation des den Verankerungszapfen (2) tragenden Pfannenkörpers (1) in das Acetabulum (A) wird etwa folgendermaßen vorgegangen:
In die Acetabulum (H) wird zunächst mittels einer nicht näher dargestellten Lehre die Bohrung (B) mit dem Durchmesser (D) gesetzt. Nach Verfüllen der Bohrung (B) mit Spongiosa und Knochenmaterial wird der vom Pfannenkörper (1) getragene Verankerungszapfen (2) in die Bohrung (B) eingetrieben. Das im Vergleich zur konischen Außenkontur (7) spitz zulaufende und gegebenenfalls abgerundete Ende (17) erleichtert das Einführen des Verankerungszapfens (2) in die Bohrung (B).
Beim Eintreiben des vom Pfannenkörper (1) getragenen Verankerungszapfens (2) in die Bohrung (B) schneiden die Sägezähne (11, 10, 9, 8) zunehmend in das Acetabulum (A) in der Umgebung deren Bohrung (B). In den Rinnen (12) entgegen der Eintreibrichtung in Richtung des Pfannenkörpers (1) ausgetriebenes Material wird in die von den steilen Flanken (8" bis 11") gebildeten Hinterschneidungen gepreßt. Beim Abschluß der Implantation sitzt dann der Bund (18) in der Corticalis des Acetabulums (A) fest, wobei er in seiner Umgebung restliches, durch die Rinnen (12) ausgetriebenes Material verdichtet.
Nach diesem beschriebenen Vorgang läßt sich die Sicherungsschraube (6) in das Acetabulum (A) einschrauben, die eine zusätzliche Sicherheit gegen Verdrehen und Lockern des Pfannenkörpers (1) im Acetabulum (A) ist. Anschließend wird dann in dem Pfannenkörper (1) das genannte Einsatzteil eingesetzt, das der Aufnahme des Kugelkopfes der Hüftpfanne dient. Es ist auch möglich, den Pfannenkörper (1) so zu gestalten, daß er direkt den Kugelkopf der Hüftgelenkprothese aufnimmt.
Bei der beschriebenen Ausführung genügt ein einziger Verankerungszapfen am Pfannenkörper (1) und somit eine Bohrung im Acetabulum (A), weil der beschriebene Verankerungszapfen den Pfannenkörper (1) sowohl gegen Verdrehen als auch gegen axiales Lockern im Acetabulum (A) sichert. Es könnten jedoch im Bedarfsfall auch zwei Verankerungszapfen an dem Pfannenkörper (1) befestigt sein.

Claims (12)

1. Hüftpfanne für ein Acetabulum mit einem Pfannenkörper, der wenigstens einen in das Acetabulum eintreibbaren Verankerungs­ zapfen trägt, wobei der Verankerungszapfen eine sich zum Pfannenkörper konisch erweiternde Außenkontur aufweist und an dem Verankerungszapfen ein zum Pfannenkörper hin verlaufendes sägezahnförmiges Profil mit Sägezahnreihen und Rinnen zwischen diesen ausgebildet ist, dadurch gekennzeichnet, dass die Gründe (13) der Rinnen (12) in Längsachsrichtung (15) des Verankerungszapfens (2) auf einer Innenkontur (14) liegen, die sich zum Pfannenkörper (1) konisch erweitert, wobei der Winkel (b) der konischen Erweiterung der Innenkontur (14) kleiner als der Winkel (a) der sich konisch erweiternden Außenkontur (7) ist, und dass die Längen (l) der Sägezähne (11, 10, 9, 8) zum Pfannenkörper (1) hin zunehmen und dass der Verankerungszapfen (2) an seinem dem Pfannenkörper (1) abgewandten Ende (17) im Vergleich zur konischen Außenkontur (7) spitz zuläuft.
2. Hüftpfanne nach Anspruch 1, dadurch gekennzeichnet, dass die Spitzen (8' bis 11') des sägezahnförmigen Profils (8 bis 11) auf der konischen Außenkontur (7) liegen.
3. Hüftpfanne nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass das sägezahnförmige Profil in Längsrichtung des Verankerungs­ zapfens (2) mehrere Sägezähne (8 bis 11) bildet.
4. Hüftpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Rinnen (12) im Querschnitt des Verankerungszapfens (2) konkav gewölbt sind.
5. Hüftpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Winkel (a) der konischen Erweiterung der Außenkontur (7) etwa 20° beträgt.
6. Hüftpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Winkel (b) der konischen Erweiterung der Innenkontur (14) etwa 9° beträgt.
7. Hüftpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Breiten (c) der Rücken (16) der Sägezähne (11, 10, 9, 8) in den Sägezahnreihen zum Pfannenkörper (1) hin zunehmen.
8. Hüftpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Längen (l) der Sägezähne (11, 10, 9, 8) zum Pfannenkörper (1) hin zunehmen.
9. Hüftpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Verankerungszapfen (2) im Bereich seines dem Pfannen­ körper (1) abgewandten Endes (17) einen Durchmesser (d1) aufweist, der gleich dem Durchmesser (D) einer in das Acetabulum (A) zum Einsetzen des Verankerungszapfens (2) vorbereiteten Bohrung (B) ist.
10. Hüftpfanne nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass am Verankerungszapfen (2) zwischen dem Pfannenkörper (1) und dem sägezahnförmigen Profil (8 bis 11) ein Bund (18) ausgebildet ist, dessen Durchmesser (d3) kleiner als der ihm nächst­ liegende Durchmesser der Außenkontur (7) ist.
11. Hüftpfanne nach Anspruch 10, dadurch gekennzeichnet, dass der Durchmesser (d3) des Bundes (18) größer ist als der ihm nächstliegende Durchmesser (d4) der Innenkontur (14).
12. Hüftpfanne nach Anspruch 10 oder 11, dadurch gekennzeichnet, dass der Bund (18) über Schrägen (19, 20) einerseits zum Pfannen­ körper (1) und andererseits zum sägezahnförmigen Profil (8 bis 11) übergeht.
