Abstract

Prothèse de cotyle de hanche sans ciment avec un ancrage expansif qui est incluse dans l'os, caractérisée en ce qu'elle se compose d'un élément de forme externe cylindro-sphérique à fond plat (1) présentant des fentes (2) disposées périphériquement et se terminant chacune par un évasement cylindrique afin de définir entre les fentes des segments des sphères (4) qui sont percés de trous (5) en nombre variable à des hauteurs différentes pour le passage de vis utilisées en ostéosynthèse humaine, en ce que le bord inférieur circulaire (1a) de l'élément (1) est bordé extérieurement d'empreintes (7) en forme de pointes de diamant et d'encoches en creux (8) tandis que la partie supérieure (1b) de l'élément cylindro-sphérique (1) est percé d'un trou (10) de grand diamètre puis en ce qu'au moins un plot extensif (6) est positionné sur une tangente de l'élément cylindrosphérique perdiculairement au bord inférieur (1a) de cette prothèse qui est destiné à recevoir intérieurement un élément cylindro conique (20) appelé "insert".

Description

La présente invention a pour objet une prothèse de cotyle de hanche sans ciment avec un ancrage expansif et un blocage isoélastique pouvant ou non comporter un revetement hydroxylapatite.
On a constaté que l'os était un materiau vivant, souple et solide, ce qui fait que dans la cavité cotyloïdienne fémorale, pendant l'activité humaine, des forces étaient transmises lors des mouvements et réparties dans des zones avec des valeurs différentes suivant la position de l'individu et l'activité qu'il développe momentanément.
On peut donc soutenir qu'un cotyle artificiel subit des forces différentes par rapport à un cotyle naturel car, jusqu'à présent, l'élément prothétique était monobloc et rigide. L'os vivant transmettait donc à l'élément prothétique des forces selon le processus habituel mais l'élément artificiel ne pouvait pas répondre à l'os par son manque de souplesse.
On a donc constaté suivant les prothèses employées de nombreux inconvénients survenant après un certain laps de temps tels que descellement, bris, désancrage de cette prothèse puis, par suite, des interventions chirurgicales lourdes de conséquences.
Finalement, il est à remarquer que le problème de changement de pièce résultant de l'usure normale était jusqu'à présent nul ou très mal abordé.
L'étude presente est donc dirigée de façon à obtenir une prothese implantable facilement, résistante, suffisamment souple et répondant aux forces naturelles.
De plus. cette prothèse suit les mouvements habituels de l'os et peut etre facilement révisable en cas d'usure.
Les éléments proposés par l'invention sont donc associes de la façon suivante
a) éléments mécaniques
b) éléments isoélastiques
c) éléments bio-compatibles avec le corps humain pour un élément prothétique en deux parties, à savoir
1 - un élément cylindro-sphérique creux en métal ou autre matériau, compatible avec le corps humain.
2 - un élément conique creux s'insérant à force dans l'élément cylindro-sphérique. Cet élément conique creux appele généralement "insert" est réalisé en matière plastique à haute densité ou autre matériau de module compressible important et compatible avec le corps humain.
Conformément à l'invention, la prothese de cotyle de hanche sans ciment avec un ancrage expansif qui est incluse dans l'os, est caractérisée en ce qu'elle se compose d'un élément de forme externe cylindro-sphérique à fond plat présentant des fentes disposées périphériquement et se terminant chacune par un évasement cylindrique afin de definir entre les fentes des segments des sphères qui sont percés de trous en nombre variable à des hauteurs différentes pour le passage de vis utilisées en ostéosynthèse humaine, en ce que le bord inférieur circulaire de l'élément est bordé extérieurement d'empreintes en forme de pointes de diamant et d'encoches en creux tandis que la partie supérieure de l'élément cylindro-sphérique est percé d'un trou de grand diametre puis en ce qu'au moins un plot extensif est positionne sur une tangente de 1 'élément cylindro-sphérique perdiculairement au bord inférieur de cette prothese qui est destinée à recevoir intérieurement un élément cylindro-conique appel "insert" évidé intériourcmont tandis que la partie inférieure de l'insert est munie d'un prolongement angle présentant des te tons correspondant aux encoches en creux de la calotte.
Diverses autres caractéristiques de l'invention ressortent d'ailleurs de la description détaillée qui suit.
Une forme de réalisation de l'objet de l'invention est représentée, à titre d'exemple non limitatif, aux dessins annexés.
La fig. 1 est une élévation montrant une prothèse vue de l'extérieur sans la pièce de liaison.
La fig. 2 est une coupe verticale de la prothèse.
La fig. 3 est une vue en plan de la prothèse.
La fig. 4 est une coupe-élevation à plus grande echelle de l'organe de fixation de la prothèse (plot extensif).
La fig. 5 est une élévation extérieure de la pièce de liaison avec la tete fémorale (appelée insert).
La fig. 6 est un plan correspondant à la fig. 5 vu de dessus.
La fig. 7 est un schema montrant la mise en place d'une prothèse conforme a l'invention dans le cotyle de hanche d'un bassin humain.
A la fig. 1, on a représenté une forme de réalisation de prothèse de cotyle de hanche selon l'invention qui se compose d'un élément de forme externe cylindro-sphérique à fond plat 1 présentant des fentes 2 disposées périphériquement se terminant chacune par un évasement cylindrique 3, ces fentes 2 en nombre variable permettant la création de segments de sphère 4 ayant une isoélasticite.
Il est prévu également sur cette calotte cylindrosphérique. des trous 5 en nombre variable permettant. à des hauteurs différentes. sur la sphère, le passage de vis de type classique de l'ostéosynthèse humaine. Ces trous 5 sont en nombre variable et leur diamètre est différent suivant les cas.
Comme on peut s'en rendre compte à la fig. 1, il est prévu au moins un plot extensif 6 positionné sur une tangente et perpendiculaire au bord inférieur la de la prothèse cylindro-sphérique I.
I1 est a remarquer que le bord circulaire périphérique inférieur 1a est borde entièrement de séries d'empreintes 7 en forme de "pointes de diamant" et encoches en creux 8. De plus, la partie supérieure lb de la prothèse 1 est percée d'un trou 10 de grand diamètre.
Comme on peut s'en rendre compte, il est possible de revetir l'extérieur de la prothèse d'une couche de métal incorporant un traitement R.A.P. (hydroxylapatite) ou autre traitement de surface analogue.
De meme comme on peut le voir à la fig. 2, la forme de la cavité intérieure 12 est cylindro-conique avec une petite gorge 13 dans la partie inférieure cylindrique 12a.
Comme on peut s'en rendre compte aux fig. 1, 2, 4, chaque plot extensif destiné à maintenir la calotte I formant la prothèse cylindro-sphérique est constitué par un tube extérieur 6 disposé tangantiellement à la calotte 1 et presentant sur une partie ou sur la totalité de sa périphérie des fentes 6o verticales.
Une vis 15 à tette 16 se visse dans le fond supérieur 6b du plot 6 tout en étant arreté à la hauteur de la tete 16 par un bourrelet circulaire 17, ce qui provoque au moment où la vis tourne dans le fond supérieur 6, une traction du haut vers le bas du plot extensif 6 venant bloquer ainsi la calette dans le cotyle de hanche puisque les evasements 6a maintiennent fermement dans les trous percés par les praticiens dans l'os cette calotte 1. Far mesure de sécurité, il est possible, comme cela est représente å la fig. 3, d'avoir deux plots extensifs 6 disposés par exemple à 60 l'un de l'autre (voir fig. 3).
Comme on peut le voir à la fig. 7, le praticien, après avoir dégagé la zone du bassin considéré préparé dans le cotyle de hanche, l'emplacement de la calotte 1, fixe cette dernière, comme expliqué ci-dessus, à l'aide d'une part, du ou des plots extensifs 6 et/ou d'autre part, des vis de type classique de l'ostéosynthèse humaine à travers les trous 5.
Comme il a éte explique, la déformation du ou des plots extensifs 6 assure un blocage mécanique dans les trous forés dans l'os, mais il est possible également de prévoir le depbt de I'élément cylindro-sphérique ou calotte 1 par un dévissage des plots 6 car les évasements 6 reprennent leur position premiere en permettant ainsi l'extraction aisée des plots 6. Ces derniers assurent d'ailleurs non seulement un ancrage primaire stable mais aussi une action anti rotation. La pose des vis d'ostéosynthèse assure une stabilisation latérale dans le cas d'une qualité d'os médiocre et la partie cylindrique à pointes de diamant 7 assure un blocage bio-mécanlque (press-fit) dans la cavité cotyloîdienne humaine.
Comme on peut le voir à la fig. 5, le praticien place dans la calotte 1 l'élément cylindro conique 20, appelé insert, en matière plastique à haute densité compatible avec le corps humain ou en un autre matériau bio compatible et selon les normes en vigueur. Cet insert est à fond plat ou angulé (plusieurs angulations sont possibles suivant les cas de l'anatomio du sujet). cette angulation permet d'obtenir une surface supplémentaire a la cavité sphérique pr?ur la tete de la prothèse fémorale.
On remarque que lteléoent cylindro-conique 20 comporte un toit 21 de forme bombée avec un décrochement 22 pour le passage dans le trou 10 de la partie métallique formée par la calotte 1.
La surface bombée ou toit 21 est revêtue d'un fin treillis métallique ou de metal poreux afin que l'os naturel puisse la réhabiter.
Il y a lieu de signaler des gorges circulaires 23 sur le pourtour externe de l'élément 20 permettant le logement des tètes de vis maintenant la calotte 1.
Finalement on doit mentionner, dans le cas de la fig. 1, un prolongement 20a angule dont la partie supérieure 20b comporte des tétons 25 correspondent aux encoches en creux 8 de la calotte 1, cette partie supérieure 20b formant bourrelet sur le pourtour de l'élément angulé 20a assurant un blocage mécanique anti recul et limite ainsi également la rotation des pièces les unes par rapport aux autres en donnant d'une manière simple la position angulaire désirée entre la calotte et l'élément cylindro-conique 20.
Pour finir, on remarque dans la partie angulée 20a un canal 26 qui contient une tige métallique 27 destinee à servir de repere radiologique.
L'evidement inférieur 30 et l'élément cylindroconique 20 de forme sphérique constituent la cavité permettant l'adaptation des tètes fémorales existant sur le marché.

Claims (4)

REVNDTCATIONS
1 - Prothèse de cotyle de hanche sans ciment avec un ancrage expansif qui est incluse dans l'os, caractérisée en ce qu'elle se compose d'un élément de forme externe cylindro-sphérique à fond plat (1) présentant des fentes (2) disposées périphériquement et se terminant chacune par un evasement cylindrique afin de définir entre les fentes des segments des sphères (4) qui sont percés de trous (5) en nombre variable a des hauteurs différentes pour le passage de vis utilises en ostéosynthèse humaine, en ce que le bord inferieur circulaire (la) de l'élément (1) est bordé extérieurement d'empreintes (7) en forme de pointes de diamant et d'encoches en creux (8) tandis que la partie superieure (lb) de ltele- ment cylindro-spherique (1) est percé d'un trou (10) de grand diamètre puis en ce qu'au moins un plot extensif (6) est positionné sur une tangente de l'élément cylindro-sphérique perdiculairement au bord inférieur (la) de cette prothèse qui est destinee à recevoir intérieurement un élément cylindro-conique (20) appele "insert" évidé intérieurement (en 30) tandis que la partie inférieur de l'insert (20) est munie d'un prolongement (20a) angule présentant des tetons (25) correspondant aux encoches en creux (8) de la calotte (1).
2 - Prothèse suivant la revendication 1, caractérisée en ce que chaque plot extensif (6) de forme cylindrique présente sur une partie ou sur la totalité de sa peripherie des fentes (6a) verticales permettant une déformation du plot (6) sous l'action d'une vis (15) à tète (16). la texte (15) se vissant dans le fond supérieur (6b) percé et fileté du plot extensif (6).
3 - Prothèse suivant l'une des revendications précédentes, caractérisee en ce que l élément cylindro conique (20) est formé à sa partie supérieure par un toit (21), realisé à l'aide d'un fin treillis metallique ou en metal poreux.
4 - Prothèse suivant la revendication 3, caractérisée en ce que l'élément cylindro-conique (20) présente, dans sa partie moyenne, des gorges circulaires (23) paralleles permettant le logement des vis (5) maintenant dans l'os la calotte (1).
