Abstract
LA INVENCION SE REFIERE A UNA PROTESIS ENDOVASCULAR CON FLEXIBILIDAD AXIAL. EN UNA REALIZACION PREFERIDA DE LA INVENCION, DICHA PROTESIS PRESENTA UN EJE LONGITUDINAL Y COMPRENDE UNA PLURALIDAD DE BANDAS DISPUESTAS LONGITUDINALMENTE, DE FORMA QUE CADA BANDA DEFINE UNA ONDULACION GENERALMENTE CONTINUA, A LO LARGO DE UN SEGMENTO DE LINEA PARALELO AL EJE LONGITUDINAL. UNA PLURALIDAD DE UNIONES MANTIENE LAS BANDAS EN UNA ESTRUTURA TUBULAR. Description
Stent flexible axialmente.
La presente invención se refiere a un stent que
presenta flexibilidad axial y resiliencia en su forma expandida.
Un stent se usa habitualmente como una estructura
tubular que se introduce en el lumen de un conducto para reducir una
obstrucción. Frecuentemente, los stents se insertan en el lumen con
forma no expandida y a continuación se expanden de forma autónoma
(Tihon y col. (1994), patente estadounidense
US-5356423) o con la ayuda de un segundo dispositivo
in situ. Un procedimiento habitual de expansión se produce
mediante el uso de un balón de angioplastia colocado en un catéter
que se infla en el interior del vaso o pasaje corporal estenosado
para cortar e interrumpir las obstrucciones asociadas con los
componentes de la pared de los vasos y para obtener un lumen
ampliado. En ausencia de un stent, puede producirse reestenosis como
resultado del retroceso elástico de la lesión estenótica. A pesar de
que se presentaron una gran cantidad de diseños de stent, dichos
diseños han sufrido una gran cantidad de limitaciones. Entre estas
se incluyen las restricciones en la dimensión del stent (Cardon y
col. 1995, patente estadounidense US-5383892).
Cardon et al describe un stent que presenta extremos rígidos
(8 mm) una parte intermedia flexible de 7-21 mm.
Dicho dispositivo está formado por múltiples partes y su
flexibilidad no es continua a lo largo del eje longitudinal. Otro
diseño de stent que presenta segmentos rígidos y segmentos
flexibles, se describe en la patente estadounidense
US-5449373 (1995) de Pinchasik y col.
En el documento
WO-A-9603092, publicado tras la
fecha de prioridad de la presente solicitud, se describe un stent
que comprende un tubo que presenta una forma modelada que posee un
primer y segundo diseño serpenteante con ejes en una primera y
segunda dirección.
Otro stent (documento
EP-A-0734698, publicado tras la
fecha de prioridad de la presente solicitud) presenta un eje
longitudinal y proporciona flexibilidad longitudinal. Comprende una
pluralidad de bandas dispuestas longitudinalmente, en el que cada
banda define una onda genéricamente continua que presenta una
frecuencia espacial a lo largo de un segmento de línea paralelo al
eje longitudinal, y una pluralidad de enlaces para mantener las
bandas en una estructura tubular.
Otros stents se describen como flexibles
longitudinalmente (Lau y col. (1995), patente estadounidense
US-5421955 y también la solicitud
PE-540290-A2, A3) pero consisten en
una pluralidad de elementos cilíndricos conectados por miembros
flexibles. Dicho diseño presenta al menos una desventaja importante,
por ejemplo, según dicho diseño, se producen bordes salientes cuando
se dobla el stent alrededor de una curva, lo que supone la
posibilidad de retención involuntaria del stent sobre la placa
depositada sobre las paredes arteriales. Esto puede provocar que el
stent embolice o se descoloque de su posición y también puede
provocar daños a la alineación interior de los vasos sanos (véase la
Figura 1(a) a continuación).
Por tanto, los stents conocidos en la materia,
que pueden expandirse por angioplastia de balón, comprometen, por lo
general, la flexibilidad axial para permitir la expansión y
proporcionar integridad estructural total.
La presente invención soluciona algunos de los
defectos percibidos de los stents de la técnica anterior al
proporcionar un stent con flexibilidad axial. En una forma de
realización preferida, el stent presenta un primer extremo y un
segundo extremo con una sección intermedia entre los dos extremos.
El stent también presenta un eje longitudinal y comprende una
pluralidad de bandas dispuestas longitudinalmente, en el que cada
banda define una onda genéricamente continua a lo largo de un
segmento de línea paralelo al eje longitudinal. Una pluralidad de
enlaces mantiene las bandas en una estructura tubular, en la que
cada banda del stent dispuesta longitudinalmente está conectada en
una pluralidad de ubicaciones periódicas, mediante una unión
circunferencial corto, a una banda adyacente. La frecuencia espacial
de la onda asociada con cada una de las bandas se reduce en una
primera región de extremo que se encuentra próxima al primer extremo
y en una segunda región de extremo que se encuentra próxima al
segundo extremo, en comparación con la frecuencia espacial de la
onda en la sección intermedia. En una forma de realización de la
invención, la onda asociada con cada una de las bandas presenta,
aproximadamente, la misma frecuencia espacial fundamental en la
sección intermedia, y las bandas se disponen de tal manera que las
ondas asociadas con ellas se alinean espacialmente de forma que se
encuentran genéricamente en fase entre sí.
En concreto, cada una de un primer grupo de
posiciones axiales comunes, existe una unión circunferencial entre
cada conjunto de entre un primer conjunto de pares adyacentes de
bandas.
En cada una de un segundo grupo de posiciones
axiales comunes, existe una unión circunferencia entre cada una de
un segundo grupo de filas adyacentes de bandas, en el que, a lo
largo del eje longitudinal, una posición axial común se produce
alternativamente en el primer grupo y en el segundo grupo, y el
primer y el segundo conjunto se seleccionan de tal forma que una
banda dada se une con una banda adyacente en solamente una del
primer y segundo grupo de posiciones axiales comunes.
En otra forma de realización de la invención, la
frecuencia espacial de las bandas en la primera y segunda región de
extremo se reduce el 20% en comparación con la frecuencia espacial
de las bandas en la sección intermedia. La primera región de extremo
puede situarse entre el primer extremo y un conjunto de enlaces
circunferenciales que se encuentran más próxima al primer extremo y
la segunda región de extremo se encuentra más próxima al segundo
extremo. Las anchuras de las secciones correspondientes de las
bandas en dichas regiones de extremo, medidas en una dirección
circunferencial, son mayores en la primera y segunda región de
extremo que en la sección intermedia. Cada banda incluye un término
en cada uno de los extremos primero y segundo y los pares adyacentes
de bandas se unen en sus términos para formar un bucle cerrado.

descripción detallada, teniendo en cuenta los dibujos adjuntos, en

Las Fig. 1 (a) y 1 (b) son vistas laterales de
un stent que presenta bandas dispuestas circunferencialmente, en las
que el stent se encuentra, respectivamente, en posición doblada y
desdoblada, y en esta última se muestran bordes salientes.
Las Fig. 1 (c) y 1 (d) son vistas laterales de
un stent flexible axialmente de conformidad con la presente
invención, en las que el stent se encuentra, respectivamente, en
posición doblada y desdoblada, y en esta última se muestra una
ausencia de bordes salientes.
La Fig. 2 es una vista lateral de una sección del
stent de las Fig. 1 (c) y 1 (d) que muestra las bandas
longitudinales, los espacios y las medidas radiales interiores de
las dobleces en las bandas, estando expresadas en pulgadas.
Las Fig. 3 (a) y 3 (b) muestran una sección del
stent de la Fig. 2 con dos bandas entre dos enlaces
circunferenciales (a) antes de su expansión en el estado sin
deformación; y (b) tras la expansión, en el estado deformado.
La Fig. 4 es una vista a lo largo de la longitud
de una pieza de stent cilíndrico (los extremos no se muestran) antes
de la expansión que muestra la superficie exterior del cilindro del
stent y el modelo de bandas característico.
La Fig. 5 es una vista isométrica de un área de
deflexión en la que el stent de la Fig. 2 se amplía a un diámetro
mayor de 5 mm.
La Fig. 6 muestra un diseño bidimensional del
stent de la Fig. 4 para formar un cilindro de forma que el borde
"A" se junte con el borde "B", y que ilustra la acción
similar a un resorte realizada en las direcciones circunferencial y
longitudinal.
La Fig. 7 muestra un diseño bidimensional del
stent. Los extremos se modifican de forma que la longitud (L_{A})
sea aproximadamente el 20% menor que la longitud (L_{B}) y la
anchura de la banda A sea mayor que la anchura de la banda B.
Las mejoras que proporcionan las formas de
realización de la presente invención incluyen (a) una flexibilidad
mejorada en dos planos del stent no expandido a la vez que se
mantiene la resistencia radial y un elevado porcentaje de área
abierta tras la expansión; (b) presión homogénea sobre el stent en
expansión que garantiza el contacto consistente y continuado del
stent expandido contra la pared arterial; (c) evitar las partes
salientes durante el doblado; (d) eliminación de las restricciones
existentes sobre la longitud máxima del stent; y reducción de
cualquier efecto de acortamiento durante la expansión del stent.
En una forma de realización preferida de la
invención, se proporciona un stent cilíndrico expansible que
presenta una estructura fenestrada para su colocación en un vaso
sanguíneo, conducto o lumen para mantener el vaso, conducto o lumen
abiertos, más en particular, para proteger un segmento de la arteria
de reestenosis tras angioplastia. El stent puede expandirse
circunferencialmente y mantenerse en una configuración expandida que
sea rígida circunferencialmente. El stent es flexible axialmente y
al doblarse en una banda, el stent evita cualquier parte componente
saliente exteriormente. La Figura 1 muestra lo que ocurre con un
stent, de un diseño similar a una forma de realización de la
presente, pero que en cambio usa un conjunto de bandas dispuestas
circunferencialmente, al doblarse de tal manera que es probable que
se encuentre en el interior de un lumen corporal. Un stent con una
disposición circunferencial de las bandas (1) muestra un efecto
análogo a un conjunto de vagones de tren sobre una vía. A medida que
la fila de vagones de tren avanza alrededor de la doblez, la esquina
de cada vagón que avanza alrededor de la doblez tras el acoplamiento
sobresale del contorno de la vía. De forma similar, las bandas
circunferenciales de serpentina presentan salientes (2) sobre la
superficie del stent como las dobleces del stent. Por el contrario,
el diseño novedoso de la forma de realización mostrada en las Fig. 1
(c) y 1 (d) y la Figura 7 en las que las bandas (3) son flexibles
axialmente y están dispuestas a lo largo del eje longitudinal,
evitar el efecto de vagón cubierto al doblar el stent, por lo que
las bandas dobladas (4) no sobresalen del perfil de la curva del
stent. Además, cualquier ensanchamiento en los extremos del stent
que pueda ocurrir con un stent que presenta una estructura uniforme
se elimina esencialmente al introducir una modificación en los
extremos del stent. Dicha modificación comprende la reducción de la
frecuencia espacial y el aumento de la anchura de las bandas
correspondientes en una dirección circunferencial (L_{A} y A) en
comparación con la de la sección intermedia (L_{B} y B). En una
forma de realización de la presente invención, la frecuencia
espacial L_{A} puede reducirse 0-50% respecto a
L_{B}, y la anchura A puede aumentarse en el intervalo de
0-150% respecto a B. Otras modificaciones en los
extremos del stent puede incluir el aumento del grosor de la pared
del stent y pulido eléctrico selectivo. Dichas modificaciones
protegen la arteria y cualquier placa de la abrasión que puede
producirse en los extremos del stent durante la inserción del stent.
La modificación también puede proporcionar una mayor densidad a los
rayos X en los extremos del stent. Por consiguiente, resulta posible
colocar el stent con más precisión una vez se encuentra en su lugar
en el cuerpo.
La forma de realización que se muestra en las
Fig. 2 y 6 presenta la única ventaja de poseer "resortes"
eficaces tanto en la dirección circunferencial como longitudinal,
que se muestran, respectivamente, como elementos (5) y (6). Dichos
resortes proporcionan al stent la flexibilidad necesaria para
circular por vasos corporales con fricción reducida y expandirse en
el lugar seleccionado de forma que proporciona las dimensiones
expandidas finales necesarias sin fuerza excesiva mientras conserva
la resiliencia estructural de la estructura expandida.
Como se muestra en las Fig. 2, 4 y 6, cada banda
longitudinal ondula a través de aproximadamente dos ciclos antes de
formarse una unión circunferencial a una banda adyacente. Antes de
la expansión, la onda asociada con cada una de las bandas puede
presentar, aproximadamente, la misma frecuencia espacial
fundamental, y las bandas se disponen de tal forma que las ondas
asociadas con ellas se alinean espacialmente, de forma que se
encuentran genéricamente en fase entre sí, como se muestra en la
Fig. 6.
Las bandas alineadas sobre el eje longitudinal
están conectadas en una pluralidad de ubicaciones periódicas,
mediante una unión circunferencial corto a una banda adyacente.
Considérese una primera posición axial común como muestra la línea
X-X en las Fig. 4 y 6. En ellas, un par adyacente de
bandas se une mediante el enlace circunferencial 7. De igual forma,
otros pares de bandas se unen también en dicha posición axial común.
En una segunda posición axial común, mostrada en la Fig. 6 mediante
la línea Y-Y, un par adyacente de bandas se une
mediante el enlace circunferencial 8. Sin embargo, cualquier par de
bandas dado que se una en X-X no se une en
Y-Y y viceversa. El modelo de enlaces
X-X se repite en la posición axial común
Z-Z. Por consiguiente, existen por lo general dos
grupos de posiciones axiales comunes. En cada una de las posiciones
axiales de cualquier grupo existen enlaces entre los mismos pares de
bandas adyacentes, y los grupos alternan a lo largo del eje
longitudinal de la forma de realización. De este modo, se
proporcionan el resorte circunferencial 5 y el resorte longitudinal
6.
Una característica del suceso de expansión
consiste en que el diseño del espacio abierto en el stent de la
forma de realización de la Fig. 2 antes de la expansión es diferente
al diseño del stent tras la expansión. En particular, en una forma
de realización preferida, el diseño del espacio abierto en el stent
antes de la expansión es serpentina, mientras que tras la expansión,
el diseño se aproxima a la forma de diamante (3a, 3b). En las formas
de realización de la presente invención, la expansión puede
obtenerse mediante la presión de un balón en expansión u otros
medios mecánicos.
Durante la expansión, como se muestra en la Fig.
3, las bandas con forma de onda tienden a enderezarse. Cuando las
bandas se enderezan, se vuelven más rígidas y, por tanto, resisten
fuerzas relativamente elevadas. La Fig. 3 muestra cómo la expansión
radial del stent hace que la ventana se abra en forma de diamante, y
la carga máxima se ejerza sobre los vértices del diamante a lo largo
del eje longitudinal. Tras realizar análisis de elementos finitos,
que incluyeron estudios sobre cargas, sobre el stent, se observó que
la carga máxima se registraba en las bandas y los enlaces y se
encontraba por debajo del máximo considerado necesario para mantener
la integridad estructural.
La optimización de la carga del stent se obtiene
al crear un radio de viraje lo mayor posible en la onda asociada a
cada banda en el stent no expandido. Esto se realiza mientras se
conserva una cantidad suficiente de bandas y enlaces para mantener
la integridad estructural del stent tras la expansión. En una forma
de realización de la invención, la carga puede ser inferior a 0,57
para el acero inoxidable 316L. La presión de la expansión puede ser
1,0-7,0 atmósferas. La cantidad de bandas y la
frecuencia espacial de la onda a lo largo del eje longitudinal
también afecta a la cantidad de enlaces circunferenciales. Los
enlaces circunferenciales contribuyen a la integridad estructural
durante la aplicación de la fuerza radial usada en expansión del
stent y en el mantenimiento de la forma expandida. Mientras no se
limite a un conjunto único de parámetros, un ejemplo de un stent de
la invención que presenta un eje longitudinal y proporciona
flexibilidad axial del tipo mostrado en la Fig. 6, puede incluir un
stent que presenta un diámetro expandido de 4 mm y una longitud de
30 mm que puede presentar, por ejemplo, aproximadamente
8-12 filas, más en particular, 10 filas y
aproximadamente 6-10 ranuras, más en particular, 8
ranuras (en la Figura 6 se muestra una ranura extendida entre X y
Z), con una amplitud de onda de aproximadamente
1/4-1/10 de la longitud de una ranura, más en
particular, 1/8 de la longitud de una ranura.
El stent puede estar fabricado a partir de
diversos procedimientos. Por ejemplo, el stent puede fabricarse a
partir de un hueco o un tubo de acero inoxidable conformado que
puede recortarse mediante láser, fresado por descarga eléctrica
(EDM), corrosión química u otros medios. El stent se inserta en el
cuerpo y se coloca en el lugar deseado de forma no expandida. En una
forma de realización preferida, la expansión del stent se realiza en
un vaso sanguíneo mediante un catéter de balón, en el que el
diámetro final del stent representa una función del diámetro del
catéter de balón usado.
Al contrario que los stents de la técnica
anterior, el stent de la invención puede realizarse con cualquier
longitud deseada, más preferiblemente, con una longitud nominal de
30 mm que puede extenderse o reducirse mediante incrementos, por
ejemplo, incrementos de 1,9 mm.
Deberá tenerse en cuenta que un stent de
conformidad con la presente invención puede realizarse en un
material con memoria de forma, que incluye, por ejemplo, una
aleación adecuada de níquel y titanio o de acero inoxidable. En
dicha forma de realización, una vez formado el stent, puede
comprimirse de forma que ocupe un espacio lo suficientemente
reducido que permita su inserción en un vaso sanguíneo u otro tejido
mediante medios de inserción, en el que los medios de inserción
incluyen un catéter adecuado o una barra flexible. Al emerger del
catéter, el stent puede configurarse para expandirse en la
configuración deseada, en la que la expansión es automática o se
desencadena mediante un cambio en la presión, en la temperatura o
por estimulación eléctrica.
Una forma de realización del stent mejorado
resulta de utilidad no solamente en los vasos sanguíneos descritos
anteriormente, sino también en cualquier sistema tubular del cuerpo,
como las vías biliares, el sistema urinario, el tubo digestivo y los
tubos del sistema reproductivo, tanto masculino como femenino.
Claims (9)
1. Un stent que presenta un primer y segundo
extremo con una sección intermedia entre los mismos, presentando el
stent también un eje longitudinal y que proporciona flexibilidad
axial, que comprende:
una pluralidad de bandas (5) dispuestas
longitudinalmente, en el que cada banda define una onda
genéricamente continua, presentando una frecuencia espacial a lo
largo de un segmento de línea paralelo al eje longitudinal; y
una pluralidad de enlaces (8) para mantener las
bandas en una estructura tubular, en el que cada banda se conecta,
en una pluralidad de ubicaciones periódicas, mediante una unión
circunferencial a una banda adyacente,
caracterizado porque:
la frecuencia espacial de la onda asociada a cada
una de las bandas (5) se reduce en una primera región de extremo que
se encuentra próxima al primer extremo y en una segunda región de
extremo que se encuentra próxima al segundo extremo, en comparación
con la frecuencia espacial de la onda en la sección intermedia.
2. Un stent según la reivindicación 1, en el que
la onda asociada a cada una de las bandas presenta, aproximadamente,
la misma frecuencia fundamental para la sección intermedia.
3. Un stent según la reivindicación 2, en el que
las bandas se disponen de tal forma que las ondas asociadas a ellas
se alinean espacialmente de forma que se encuentran genéricamente en
fase entre sí.
4. Un stent según la reivindicación 3, en el que,
en cada una de un primer grupo de posiciones axiales comunes existe
una unión circunferencial entre cada conjunto de entre un primer
conjunto de pares adyacentes de bandas.
5. Un stent según la reivindicación 4, en el que,
en cada grupo de entre un segundo grupo de posiciones axiales
comunes existe una unión circunferencial entre cada conjunto de
entre un segundo conjunto de filas adyacentes de bandas, en el que,
a lo largo del eje longitudinal, se encuentra una posición axial
común alternativamente en el primer grupo y en el segundo grupo, y
el primer y segundo conjunto se seleccionan de forma que una banda
dada se une a una banda adyacente en solamente uno de entre el
primer y segundo grupo de posiciones axiales comunes.
6. Un stent según la reivindicación 1, en el que
la frecuencia espacial se reduce hasta aproximadamente el 20% en
comparación con la frecuencia espacial de la onda en la sección
intermedia.
7. Un stent según las reivindicación 1 ó 6, en el
que la primera región de extremo se encuentra entre el primer
extremo y un conjunto de uniones circunferenciales que se encuentran
más próxima al primer extremo y la segunda región de extremo se
encuentra entre el segundo extremo y un conjunto de uniones
circunferenciales que se encuentran más próximas al segundo
extremo.
8. Un stent según las reivindicaciones 1 ó 7, en
el que las anchuras de las secciones correspondientes de las bandas,
medidas en dirección circunferencial, son mayores en la primera y
segunda región de extremo que en la sección intermedia.
9. Un stent según la reivindicación 1, en el que
cada banda (5) incluye un término en cada uno del primer y segundo
extremo y los pares adyacentes de bandas se unen en sus finales para
formar un bucle cerrado.
