LAS COMPOSICIONES PUEDEN ADMINISTRARSE MEDIANTE LA UTILIZACION DE UN PARCHE TRANSDERMICO. EL PARCHE ADMINISTRARA A LAS MUJERES NORGESTIMATO 17 - DESACETILO SOLO O EN COMBINACION CON UN ESTROGENO COMO ETINILESTRADIOL.
Description
Parche transdérmico para la administración de 17-deacetil norgestimato, en combinación con un estrógeno.
La presente invención se refiere a la administración transdérmica de fármacos. Más concretamente, se refiere a unos parches para la administración transdérmica de 17-desacetil-norgestimato, combinado con un estrógeno, en concreto etinil-estradiol.
Combinaciones de norgestimato (Ngm) y etinil-estradiol (EE) se administran a las mujeres por vía oral como anticonceptivos. Bringer J., Am. J. Obstet. Gynecol. (1992) 166: 1969-77. McGuire, J.C. et al., Am. J. Obstet. Gynecol. (1990) 163: 2127-2131 sugiere que el Ngm administrado vía oral se metaboliza a 17-desacetil-norgestimato (17-d-Ngm), 3-ceto-norgestimato y levonorgestrel (Lng), y que estos metabolitos podrían contribuir a la respuesta farmacológica al fármaco cuando se administra vía oral.
Chien y col., en la patente estadounidense número 4.906.169, describen parches transdérmicos para la liberación conjunta de estrógenos y progestinas a mujeres con fines anticonceptivos. Mencionan el EE como uno de los estrógenos que se podrían administrar desde el parche, y Ngm y Lng como posibles progestinas que se podrían administrar.
Los solicitantes desconocen cualquier técnica que describa una administración vía transdérmica o por otras vías de administración de 17-d-Ngm, sólo o combinado con un estrógeno.
La presente invención proporciona un parche transdermico para evitar la ovulación en una mujer, según lo definido en la reivindicación 1, adaptado para administrar una cantidad inhibidora de ovulación de 17- desacetil-norgestimato. Una cantidad eficaz puede ser de 150 a 350 Ig/día de 17-desacetil-norgestimato, preferiblemente de 175 a 300 Ig/día. En una forma de presentación preferida, el parche esta adaptado para administrar conjuntamente una cantidad inhibidora de la ovulación de un estrógeno como el etinil-estradiol. Una cantidad eficaz es de 150 a 350 Ig/día de 17-desacetil-norgestimato, preferiblemente de 175 a 300 Ig/día, yde 10 a 35 Ig/día de etinil-estradiol.
Los parches transdérmicos de la invención confieren anticoncepción a las mujeres. También están adaptados para un tratamiento de sustitución hormonal.
Por consiguiente, la presente invención también proporciona un parche transdermico para administrar un tratamiento de sustitución hormonal a una mujer, según lo definido en la reivindicación 5.
Los parches se han concebido para administrar a la piel 17-d-Ngm y un estrógeno de forma continuada durante un periodo prolongado, típicamente de 1 a 7 días y preferiblemente de 7 días.
Cuando los parches se llevan como anticonceptivos, normalmente se colocará un parche sobre la piel el quinto día del ciclo menstrual, y se reemplazará según sea necesario hasta que se hayan cumplido los 21 das de uso. Por ejemplo, en el caso de un parche para 7 días, se requerirán tres parches para administrar el fármaco o los fármacos durante el periodo de 21 días. Si se desea, es posible utilizar un parche placebo de aquí en adelante hasta el quinto día del siguiente ciclo menstrual. Esta pauta se repite en cada ciclo menstrual.
Tanto el 17-d-Ngm como los estrógenos inhiben la ovulación, aunque por vías metabólicas diferentes. 17-d-Ngm inhibe la liberación de la hormona luteinizante (LH), mientras que el efecto principal del estrógeno es inhibir la secreción de la hormona estimulante del folículo (FSH). Por consiguiente, cuando se administra una combinación de 17-d-Ngm y estrógeno mediante un parche según la invención, se evita la ovulación por inhibición del estímulo de la ovulación y/o por inhibición del crecimiento de los folículos. La administración de 17-d-Ngm se cree ventajosa con respecto del compuesto original (Ngm) o el resto de sus metabolitos, puesto que 17-d-Ngm inhibe poco o nada la actividad andrógena.
La dosis efectiva de 17-d-Ngm para la inhibición de la ovulación está normalmente comprendida en el intervalo de 150 a 350 Ig/día, más preferiblemente entre 175 y 300 Ig/día y mejor aún entre 175 y 250 Ig/día. Típicamente, los parches propuestos en la presente invención tendrán una superficie basal (el área de intercambio por difusión con la piel) de entre 10 y 50 cm2. La dosis de estrógeno eficaz para la inhibición de la ovulación dependerá del estrógeno concreto que se coadministre. Por ejemplo, cuando el estrógeno sea el etinil-estradiol, la dosis será normalmente de 10 Ig/día como mínimo, preferiblemente de 10 a 35 Ig/día, y más preferiblemente de aproximadamente 20 Ig/día. Los parches contendrán cantidades suficientes de 17-d-Ngm y de estrógeno para proporcionar las dosis diarias necesarias para el tiempo de uso previsto para el parche. Típicamente, estas dosis son de 20 Ig/día a 200 Ig/día de etinil-estradiol, y preferiblemente de 30 Ig/día a 150 Ig/día.
Los parches de la presente invención tienen estructura laminada matricial o de tipo monolítico. Estos parches transdérmicos son muy conocidos en el ámbito de las técnicas transdérmicas. Comprenden una capa matricial, que contiene el fármaco o los fármacos mezclados con un adhesivo sensible a la presión, y una capa de soporte. La matriz sirve como reservorio del fármaco y como medio mediante el cual el parche se pega a la piel. Antes de su uso, el parche incluye también una capa formada por una cubierta de liberación impermeable.
La capa de soporte es impermeable al fármaco y al resto de los componentes de la matriz y delimita la superficie superior del parche. Puede estar compuesta por una capa única de película de polímero, o ser un laminado de una o mas capas de polímero y una hoja de metal. Ejemplos de polímeros adecuados para el uso como películas de soporte son: el cloruro de polivinilo, el cloruro de polivinilideno, las poliolefinas -como los copolimeros de acetato de etilen-vinilo-, el polietileno y el polipropileno, el poliuretano y los poliésteres como el tereftalato de polietileno.
El adhesivo de la matriz, sensible a la presión, es una silicona o poliisobutileno (PIB). Esos adhesivos son muy conocidos en el campo de las técnicas transdérmicas. Véase, por ejemplo, el "Manual de Tecnologías de Adhesivos Sensibles a la Presión" (Handbook of Pressure Sensitive Adhesive Technology), 2a edicion (1989), Van Nostrand, Reinhold.
Los adhesivos de poliisobutileno son mezclas de PIB de alto peso molecular (APM) y PIB de bajo peso molecular (BPM). Esas mezclas ya se han descrito, por ejemplo en PCT/US91/02516. El peso molecular del PIB de APM suele estar comprendido en el intervalo de 700.000 a 2.000.000 Da, mientras que el del PIB de BPM normalmente está dentro del intervalo de 35.000 a 60.000 Da. Los pesos moleculares mencionados aquí son medias ponderadas. La relación de peso del PIB de APM y el PIB de BPM de los adhesivos normalmente varía entre 1:1 y 1:10. El adhesivo de PIB también incluirá normalmente un adhesivo, como el aceite de polibuteno, y resinas alifáticas de temperatura de transición vítrea (Tg) alta y peso molecular bajo, como las resinas ESCOREZ™ de Exxon Chemical disponibles en el mercado. Los polímeros de poliisobutileno son comercialmente disponibles bajo la marca comercial VISTANEX™ de Exxon Chemical.
Los adhesivos de silicona que se pueden usar para formar la matriz son típicamente dimetil-polisiloxanos o dimetil-difenil-polisiloxanos de peso molecular alto. Las formulaciones de los adhesivos de silicona que son útiles para los parches transdérmicos se describen en las patentes estadounidenses número 5.232.702, 4.906.169 y 4.951.622.
Los estrógenos que se pueden combinar con el 17-d-Ngm en la matriz son: el 17-ß-estradiol y sus ésteres como el valerato de estradiol, el cipionato de estradiol, el acetato de estradiol, el benzoato de estradiol y EE. EE es un estrógeno preferido para usar en combinación con el 17-d-Ngm. Las combinaciones de EE y 17-d-Ngm pueden afectar favorablemente parámetros metabólicos como el aumento de las lipoproteínas séricas de densidad alta y la reducción de la relación entre las lipoproteínas de densidad baja y las lipoproteínas de densidad alta del suero.
Además del adhesivo sensible a la presión, el 17-d-Ngm y el estrógeno opcional, la matriz contendrá normalmente cantidades suficientes de potenciadores de la permeabilidad, para aumentar la permeabilidad del 17-d-Ngm y el estrógeno a través de la piel y proporcionar flujos comprendidos en los intervalos mencionados mas arriba. En las patentes estadounidenses número 5.059.426, 4.973.468, 4.906.463 y 4.906.169 se describen ejemplos de potenciadores de la permeabilidad que se pueden incluir en la matriz, entre los cuales se encuentran, el éster del ácido láctico con alcohol alifático de C12 a C18, el lactato de laurilo, el ácido oleico o el monolaurato de propilen-glicol (PGML), aunque no se limitan a ellos. La cantidad de potenciador de la permeabilidad incluida dependerá del potenciador o los potenciadores particulares que se empleen. En la mayoría de los casos constituirá el 1 al 20 % en peso de la matriz.
La matriz puede contener otros aditivos dependiendo del adhesivo concreto que se utilice. Por ejemplo, es posible incluir materiales como la polivinil-pirrolidona (PVP) que inhiben la cristalización del fármaco, agentes higroscópicos que mejoran la duración, o aditivos que mejoran las propiedades físicas (por ejemplo, el flujo en frio) o de adhesión (por ejemplo, la adhesividad, la fuerza de cohesión) de la matriz.
Los parches descritos mas arriba también son útiles para proporcionar un tratamiento de sustitución hormonal. Cuando se usan para proporcionar un tratamiento de sustitución hormonal, la matriz se prepara como para proporcionar una cantidad de 17-d-Ngm y estrógeno efectiva para el propósito al que esta destinada. Típicamente, la matriz y, por lo tanto, el parche se preparan para proporcionar de 150 a 350 Ig/día de 17-d-Ngm y preferiblemente de 175 a 300 Ig/día, coadministrados con 5 a 45 Ig/día de etinil-estradiol y preferiblemente con 10 a 35 Ig/día. En una realización alternativa, el parche administrara de 200 a 350 Ig/día de 17-d-Ngm y preferiblemente de 175 a 300 Ig/día, coadministrados con aproximadamente 20 a aproximadamente 175 Ig/día de 17-ß-estradiol, preferiblemente con 30 a 150 Ig/día. El parche se aplica durante 7 días y se reemplaza por un parche nuevo (para 7 días) hasta cumplir la duración del tratamiento.
Los parches de la invención pueden fabricarse usando procedimientos conocidos de las técnicas de parches transdérmicos. El procedimiento suele implicar generalmente la formulación de la matriz (es decir, el mezclado del adhesivo, el fármaco o los fármacos, el potenciador de la permeabilidad y los aditivos si existiesen), el vertido de la matriz sobre la capa de soporte o la capa de la cubierta de liberación, la eliminación del solvente de la matriz, y la aplicación de la capa de soporte o de la capa de la cubierta de liberación según sea el caso. Como es evidente para los especialistas en la materia, la composición de la matriz, que contiene una cantidad eficaz de fármaco dispersa, puede incorporarse a varias estructuras transdérmicas y, por consiguiente, los solicitantes no están limitados a las estructuras ejemplificadas en los párrafos siguientes.
Los siguientes ejemplos ilustran otro tanto la invención. Estos ejemplos no tienen el propósito de limitar de ninguna manera la invención. A no ser que se indique lo contrario, los porcentajes enunciados son en peso.
Ejemplo 1
La silicona 4202 es un adhesivo dimetil-polisiloxano de Dow Corning. Se mezclo con 17-d-Ngm, EE, PVP al 7% (K30
de BASF, disuelto en n-propanol) y varios potenciadores. Estas mezclas se vertieron formando una capa de 100
5 micrómetros de espesor (húmedo) sobre un soporte de poliéster 3M 1022 y se secaron. Se realizaron pruebas de flujo
sobre la piel de las estructuras resultantes según el procedimiento descrito en la columna 7 de la patente
estadounidense 5.252.334. Se utilizo HPLC para determinar el 17- d-Ngm y el EE. Se uso un sistema Perkin Elmer de
HPLC con detector de ordenación de jodos ajustado a 245 nm para el 17-d-Ngm y a 215 nm para el EE,
respectivamente. La fase móvil fue 55 % de agua, 45 % de acetonitrilo a un flujo de 1,0 ml/min. El tiempo de retención 10 fue 4,5 minutos para el 17-d-Ngm y 3,0 minutos para el EE, respectivamente Los detalles de las formulaciones y de los
estudios de flujo se describen en la tabla 1 incluida a continuación.

Ejemplo 2
15 Se realizaron estudios de comparación con parches de 17-d-Ngm y EE y adhesivo de silicona, empleando dos tipos de PVP: una soluble, de bajo peso molecular denominada PVP-K30 de BASF, y otra insoluble reticulada y micronizada denominada PVP-CLM de BASF.
La PVP-K30 se disolvió en etanol puro. Se prepararon mezclas de 17-d-Ngm, EE y PVP-K30, después de lo cual se agregaron silicona 4202 y laurato de metilo. La mezcla se homogeneizó durante toda la noche, y luego se vertió sobre
20 un soporte 3M 1022 hasta formar un espesor de 0,38 mm (húmedo) y se secó a 70° C durante 40 minutos.
La PVP-CLM está disponible como sólido micronizado. Se mezclaron la silicona 4202 y la PVP-CLM y luego se agregaron el laurato de metilo, el EE, el 17-d-Ngm y el etanol. La mezcla se homogeneizó durante toda la noche, y luego se vertió sobre un soporte 3M 1022 y se secó según lo descrito mas arriba.
Como en el ejemplo 1, se realizaron estudios de flujo sobre la piel con las estructuras descritas arriba. Los detalles de
25 estas formulaciones y los resultados de las pruebas de flujo en la piel se describen en la tabla 2 presentada a continuación.

Ejemplo 3
Se prepararon soluciones de PIB disolviendo VISTANEX L100, VISTANEX LM-MS-LC y polibuteno (Indopol H1900)
5 en hexano. Se prepararon suspensiones de PVP-CLM, 17-d-Ngm, EE y varios potenciadores en etanol / acetato etílico. Se agregó la solución de PIB a las suspensiones de fármaco y las mezclas resultantes se homogeneizaron minuciosamente. Luego se vertieron las mezclas formando una capa de 0,25 mm de espesor (húmedo) sobre las cubiertas de liberación y se secaron a 70° C durante 40 minutos. Se cubrieron las subestructuras con una lámina de soporte Saranex 2015. Como en el ejemplo 1, se realizaron estudios de flujo sobre la piel con estas estructuras. Los
10 detalles de estas estructuras y los resultados de los estudios de flujo sobre la piel se describen en la tabla 3 incluida más abajo.

Se preparan una composición y una matriz adecuadas para el tratamiento de sustitución hormonal de la siguiente forma: Se disuelven 2% de 17-ß-estradiol, 2% de 17-desacetil-norgetimato, 20% de PVP-CLM y 76% de adhesivo PIB (1:5:2,5:1,5 Vistanex L100:Vistanex LM-MS-LC:Polibuteno:Zonester 85FG) en una combinación de hexano, acetato etílico y etanol. Se vierte sobre una cubierta de liberación de poliester y se seca a 70°C durante 45 minutos. Se aplica una capa de soporte de poliester antes del estudio de flujo. Dicho estudio se realiza según lo descrito en el ejemplo 1.

10 Parches transdérmicos, con una matriz compuesta de adhesivo PIB, PVP=CLM, lactato de laurilo o lactato de miristilo, 17-d-Ngm y EE, se prepararon del siguiente modo:
Se disolvieron el 17-d-Ngm y el EE en acetato etílico y se agregaron a esa solución el PVP-CL y el lactato de laurilo (obtenido de ISP VanDYK de Belleville, Nueva Jersey.). Se agregó a las soluciones esteroides una solución de adhesivo PIB (1:5:4 Vistanex L100:Vistanex-LM-MS-LC:Indopol H1900) en hexano y se mezcló vigorosamente. Se 15 agregó lentamente a las mezclas finales el N-propanol al 10 % del peso del PVP-CLM. Las mezclas se vertieron sobre una cubierta de liberación y se secaron en un horno a 70°C durante 40 minutos. La matriz seca pesaba 7,5 mg/cm2. La subestructura cubierta de liberación - matriz se cubrió con una lámina protectora de poliester (Scotchpak 1012). Otra subestructura se cubrió con una lámina de poliester no tejida (Remay 2250). La cubierta de liberación se separó del ensamblaje protector y se aplicó sobre el ensamblaje de capa no tejida para dar un compuesto de 5 capas: protección,
20 matriz adhesiva, no tejida, matriz adhesiva y cubierta de liberación.
Se realizaron estudios de flujo sobre la piel con estos parches según lo descrito en el ejemplo 1. Los detalles de los parches y los resultados de los estudios de flujo se describen en la tabla 5 que se incluye a continuación.

Claims (12)
- REIVINDICACIONES1. Un parche transdérmico para evitar la ovulación de la mujer, el cual comprende:
- (a)
- una capa de soporte; y
- (b)
- una capa matricial subyacente a la capa de soporte, la capa matricial que comprende una mezcla de
5 17-desacetil-norgestimato, un estrógeno, un potenciador de la permeabilidad y un adhesivo sensible a la presión, y está adaptada para tener un intercambio por difusión con la piel de la mujer y para administrar a la mujer una cantidad inhibidora de la ovulación de 17-desacetil-norgestimato y de estrógeno, en donde el adhesivo sensible a la presión es un adhesivo de silicona o un adhesivo de poliisobutileno. - 2. El parche de la reivindicación 1, en el cual la mencionada cantidad inhibitoria de la ovulación de 10 17-desacetil-norgestimato es de 150 a 350 Ig/día.

- El parche de la reivindicación 2, en el cual la mencionada cantidad inhibitoria de la ovulación de 17-desacetil-norgestimato es de 175 a 300 Ig/día.

- El parche de cualquiera de las reivindicaciones 1 a 3, en el cual la mezcla comprende además una polivinilopirrolidona.
15 5. Un parche transdérmico para proporcionar tratamiento de sustitución hormonal a una mujer que comprende:- (a)
- una capa de soporte; y
- (b)
- una capa matricial subyacente a la capa de soporte, la capa matricial que comprende una mezcla de 17-desacetil-norgestimato, un estrógeno, un potenciador de la permeabilidad y un adhesivo sensible a la presión, y está adaptada para tener un intercambio por difusión con la piel de la mujer y para coadministrar a la
20 mujer una cantidad terapéutica de 17-desacetil-norgestimato y de estrógeno, en donde el adhesivo sensible a la presión es un adhesivo de silicona o un adhesivo de poliisobutileno. -
- 6.
- El parche de la reivindicación 1 o la reivindicación 5, en el cual el estrógeno es un estradiol.

- El parche de la reivindicación 6, en el cual el estradiol es etinil-estradiol.

- 8.
- El parche de la reivindicación 7, en el cual la mencionada cantidad es de 175 a 300 Ig/día de 25 17-desacetil-norgestimato y de 10 a 35 Ig/día de etinil-estradiol.

- El parche de la reivindicación 6, en el cual el estradiol es 17-ß-estradiol.

- El parche de la reivindicación 9, en el cual la mencionada cantidad es de 175 a 300 Ig/día de 17-desacetil-norgestimato y de 30 a 150 Ig/día de 17-ß-estradiol.

- 11.
- El parche de una cualquiera de las reivindicaciones precedentes, en el cual el adhesivo sensible a la presión 30 es un poliisobutileno.

- 12.
- El parche de una cualquiera de las reivindicaciones 1 a 11, en el cual el potenciador de la permeabilidad a la piel es un éster lactato de un alcohol alifático de C12 a C18, ácido oleico y PGML.

- 13.
- El parche de la reivindicación 12, en la cual el potenciador de la permeabilidad a la piel es el lactato de laurilo.
