Description
- Die Erfindung betrifft die Verwendung von Cepharanthin zur Herstellung einer pharmazeutischen zusammensetzung für die Behandlung von AIDS.
- Das erworbene Immunschwächesyndrom (AIDS) ist eine ernste Immunschwächeerkrankung, die durch Infektion mit dem AIDS- Virus, d. h. dem humanen Immunschwächevirus (HIV) verursacht wird. Aufgrund des sehr hohen Risikos eines tödlichen Ausganges besteht ein dringendes soziales Bedürfnis für Gegenmaßnahmen, jedoch wurde bislang noch keine wirksame therapeutische Modalität oder Impfstoff entwickelt. Das einzig bekannte Anti-AIDS-Virusmittel, das von der Food and Drug Administration of the United States zugelassen wurde, ist Azidothymidin (AZT):Jedoch hat dieses Arzneimittel nicht nur eine hohe Toxizität es zeigt auch den Nachteil, daß wiederholte Gaben über 1 Jahr zur Entwicklung von Resistenz gegen AZT beim AIDS-Virus führen. Daher besteht das starke Bedürfnis nach der Entwicklung eines geeigneteren therapeutischen Mittels (Science 249, 1533-1543 (1990)).
- Die Erfinder haben intensive Forschungen unternommen, um die obigen Probleme zu lösen und gefunden, daß Cepharanthin als Wirkstoff in einer Anti-AIDS-Viruszusammensetzung nützlich ist. Die Erfindung beruht auf dieser Erkenntnis.
- Daher stellt die Erfindung die Verwendung von Cepharanthin zur Herstellung einer pharmazeutischen Zusammensetzung für die Behandlung von AIDS, zur Verhinderung des Ausbruchs von AIDS und zur Inhibierung der Vermehrung von AIDS-Virus im menschlichen Körper zur Verfügung.
- Cepharanthin ist ein Pflanzenalkaloid dargestellt durch die Folgende Formel (1) (Merck Index 11, 306, 1981 Cepharanthine (1989); US-Patente 2 206 407 und 2 248 241; und japanische Patente 120 483, 128 533 und 141 292).
- Cepharanthin hat eine antiallergische, immunmodulierende, Anti-Blutplättchen-aggregierende, Anti-Krebs- Arzneimittelrersistenz-inhibitorische und andere Aktivitäten (Journal of Ethonopharmacology 20, 107-120 (1987); Cancer Research 48, 1307-1311 (1988); und Biochemical Pharmacology 39, 1255-1259 (1990)) und wird klinisch weit verbreitet als sicheres Arzneimittel mit einem Minimum an Nebenwirkungen bei der Behandlung von Leukocytopenie, Alopecia areata, exudative Otitis media, Viper bites, usw. eingesetzt. Es war bislang jedoch nicht bekannt, daß Cepharanthin eine Anti-AIDS- Virusaktivität besitzt.
- Cepharanthin, das erfindungsgemäß verwendet wird, kann aus Pflanzen wie Stephania cepharantha, Stephania sasakii, usw. von der Familie Menispermaceae extrahiert werden. Jeder Vakuum-konzentrierte Cepharanthinextrakt, das bei Alkalisierung einer sauren Lösung des Extrakts gebildete Präzipitat, die heraus abgetrennte Alkaloidfraktion und durch übliche Isolierung und Reinigung erhältlichen Kristalle können in die erfindungsgemäße Verwendung eingesetzt werden. Pharmazeutisch verträgliche Salze von Cepharanthin können ebenfalls für die erfindungsgemäßen Zwecke eingesetzt werden. Solche Salze umfassen Säureadditionssalze mit Säuren, wie Salzsäure und Schwefelsäure, usw.
- Diese verschiedenen Cepharanthin-haltigen Zusammensetzungen können nach konventionellen Verfahren hergestellt werden (siehe US-Patente 2 206 407 und 2 248 241; japanische Patente 120 483, 128 533 und 141 292; und Yakugaku Zassi 89, 1678- 1681 (1969), usw.). B. kann Cepharanthin mit einem organisches Lösungsmittel, wie Methanol, Ethanol, Aceton, Ether und Benzol, oder mit einer wäßrigen Lösung einer Säure, wie Salzsäure, Schwefelsäure, Essigsäure und Zitronensäure extrahiert werden. Die mit dem organischen Lösungsmittel extrahierte Cepharanthin-haltige Lösung kann unter Atmosphärendruck oder vermindertem Druck konzentriert werden. Die saure Lösung des so konzentrierten Cepharanthinextrakts oder der obige saure Extrakt kann mit Natriumhydroxid oder Ammoniak, usw. alkalisch gemacht werden und dann mit einem organischen Lösungsmittel, welches sich nicht mit Wasser mischt, wie Ether, Benzol, Hexan und Chloroform unter Erhalt der Alkaloidfraktion extrahiert werden.
- Die Anti-AIDS-Viruszusammensetzung kann Cepharanthin oder ein pharmazeutisch verträgliches Salz davon als solche oder ein Extrakt, der Cepharanthin oder eine Cepharantin-haltige Alkaloidfraktion enthält sein, wird jedoch im allgemeinen mit einer allgemein üblichen Menge an üblichen Exzipienten, Binder, Gleitmittel, Lösungsmittel und/oder Stabilisatoren formuliert in solchen Dosierungsformen wie Tabletten, Pulvern, Granula, Kapseln und Injektionen, usw. bereitgestellt.
- Der oben erwähnte Exzipient umfaßt u. a. Stärke, Lactose, Methylcellulose, kristalline Cellulose, synthetisches Aluminiumsilikat usw. Der Binder umfaßt u. a. Hydroxypropylcellulose und Polyvinylpyrrolidon. Das Gleitmittel umfaßt u. a. Talk, Magnesiumstearat, Kalziumstearat usw.
- Die Dosierung der Anti-AIDS-Viruszusammensetzung gemäß der Erfindung ist unterschiedlich je nach dem Alter, Körpergewicht, Zustand, usw. des Patienten. Im allgemeinen ist die Dosis von ca. 10 bis 1000 ml/Tag, vorzugsweise ca. 30 bis 300 mg/Tag bezogen auf den Wirkstoff bei der oralen Verabreichung, und ca. von 10 bis 300 mg/Tag, vorzugsweise ca. 50 bis 200 mg/Tag bezogen auf den Wirkstoff bei einer intravenösen Verabreichung. Eine solche Dosis wird als Einzeldosis oder in zwei oder drei aufgeteilten Dosen bei der oralen Verabreichung gegeben, und im allgemeinen in einer Einzeldosis bei der intravenösen Verabreichung gegeben.
- Cepharanthin hat eine inhibitorische Wirkung auf die Produktion des AIDS-Virus. D. h. Cepharanthin ist in der Lage die Vermehrung des AIDS-Virus zu inhibieren. Daher ist Cepharanthin nützlich als Anti-AIDS-Viruszusammensetzung zur Behandlung von AIDS oder für die Verhinderung des Ausbruchs von AIDS.
- Die folgenden Beispiele dienen zur weiteren Erläuterung der Erfindung, sind jedoch in keiner Weise limitierend.
- Cepharanthinhydrochlorid (500 mg), Lactose (3,0 g), Maisstärke (1,28 g), Hydroxypropylcellulose (200 mg( und Magnesiumstearat (20 mg) werden gut miteinander vermischt, granuliert und in Tabletten, enthaltende 100 mg Wirkstoff, komprimiert.
- Die Alkaloidfraktion von Stephania sasakii (500 mg), Lactose (2,5 g), Kartoffelstärke (1,78 g), kristalline Cellulose (240 mg) und Kalziumstearat (10 mg) werden gut miteinander gemischt, und die Mischung wird in eine Gelatinekapsel unter Erhalt einer Kapsel, enthaltend 10 mg Wirkstoff, gefüllt.
- Die Alkaloidfraktion von Stephania cepharantha (500 mg) wird in verdünnter Salzsäure gelöst, mit destilliertem Wasser für die Injektion aufgefüllt und mit Natriumchlorid auf ein Gesamtvolumen von 100 ml isotonisiert. Diese Lösung wird durch ein 0,2 µm Membranfilter filtriert und in 10 ml Ampullen aufgeteilt, die dann versiegelt und unter Erhalt einer Injektion hitzesterilisiert werden.
- Das erste Anzeichen dafür, daß Cepharanthin eine Wirkung auf die AIDS-Virusinfektion hat, wurde durch Experimente geliefert, worin CEM-CDC-Zellen, die mit einer hohen Multiplizität von HIV-l infiziert waren, vor der Cytolyse geschützt waren. D. h. CEM-CDC-Zellen (5 x 10&sup5; Zellen/ml) starben in der Kultur 3 Tage nach der Infektion mit HIV-1 ab (10&sup8; TCID&sub5;&sub0;/ml; TCID: "tissue culture mean infektous dose" = mittlere infektiöse Dosis in der Gewebekultur)) überlebten jedoch eine Infektion, falls die Kulturen in, Cepharanthinhaltigem Medium gehalten wurden. Die Anti-AIDS-Viruswirkungen von Cepharanthin wurden in HIV-l-infizierten CEM-CDC-Zellen wie folgt bestätigt.
- Der NIT-Stamm von HIV-1, der aus einem Patienten mit AIDS in New York1) isoliert worden war, wurde als Virus verwendet. Die CD4-poisitive humane T-Zellinie, CEM², die von den Centers for Disease Controll erhalten wurde, bezeichnet als CEM-CDC-Zelle, wurde als Wirtszelle verwendet. Das Virus wurde in CEM-CDC-Zellen vermehrt und mit 0,8-µm-Nalgen-Filter (Millipore) vor Verwendung filtriert.
- Zielzellen (CEM-CDC; 1 x 10&sup6; Zellen) wurden mit HIV-1 (2 x 10&sup5; TCID&sub5;&sub0;) unter 5 % CO&sub2; 3 bis 4 Stunden bei 37ºC inkubiert. Die infizierten Zellen wurden intensiv (3 bis 5mal) gewaschen und im Medium [RPMI-1640 Gewebekulturmedium supplementiert mit 5 % (Vol/Vol) Hitze-inaktiviertem fetalem Kalbsserum, 100 E/ml Penicillin G und 100 µg/ml Streptomycin] enthaltend verschiedene Cepharanthinkonzentrationen gehalten. Zur Kontrolle wurden CEM-CDC-Zellen im Medium allein gehalten. In allen Experimenten wurden die infizierten Zellen (2 x 10&sup5; Zellen pro 1,5 ml) in eine 24-Well-Kulturplatte verteilt und unter 5 % CO&sub2; bei 37ºC 5 Tage inkubiert. Nach der Inkubation wurden die Kulturüberstände, enthaltend das HIV-1-Haupt-Gag- oder Core-Protein, p24, gesammelt und durch 0,45-µm-Filter filtriert. Die Konzentration von HIV-1-p24- Antigen im filtrierten zellfreien Überstand wurde mit einem Enzym-Immuntest spezifisch für HIV-1 gemessen (Abbot Laboratory, N. Chicago, IL, USA)3) der von der US Food and Drug Administration für Forschungszwecke zugelassen ist. Man ließ nämlich verschiedene Verdünnungen der Testüberstände mit Anti-p24-Antikörper-beschichteten Perlen inkubieren. Verdünnungen bekannter p24-Standards (0 bis 1000 E p24/ml) vermischt mit den Perlen, wurden als Kontrolle eingesetzt.
- Nach intensivem Waschen ließ man die Perlen mit einem p24- spezifischen Kaninchen-Antikörper reagieren. Nach dem Waschen ließ man die Perlen mit Meerrettich-Peroxidase-kunjugiertem Ziegen-Anti-Kaninchen-IGG regieren, entwickelte mit o- Phenylendiamin und der Absorptionswert bei 492 nm wurde abgelesen. Die Konzentration von p24 (E/ml) wurde von der Standardkurve, abgeleitet von Verdünnungen des bekannten Standards durch einen handelsüblich erhältlichen p24- Abfangtest (Coulter HIV Antigen Assay kit; Coulter Corporation, Hialeah, FL, USA)4), 5) unter Verwendung eines ELISA-Tests abgeleitet.

- Der Prozentsatz der Inhibierung der HIV-1-Vermehrung wird durch [1-(T/C)] x 100 angegeben, worin T die Menge von p24- Antigen im Kulturüberstand, der mit den Mitteln behandelt wurde ist, und C ist die Menge an p24-Antigen im Überstand der ohne Mittel behandelten Kultur. Die Ergebnisse sind in Tabelle 1 gezeigt.
- Die Lebensfähigkeit von Zellen in der Kultur wurde Trypanblau-Ausschlußzählung bestimmt und berechnet als:
- Lebensfähigkeit der Zellen in % = (V/T) x 100, worin
- V = Anzahl der lebensfähigen Zellen und

- Wie in Tabelle 1 gezeigt, enthielten die Überstände von CEM- CDC-Zellkulturen, die in Cepharanthin-haltigem Medium gehalten wurden, nach Infektion mit HIV-1 eine geringere Konzentration von p24-Antigen im Vergleich zu CEM-CDC- infizierten Zellenkulturen, die im Medium allein gehalten wurden. Aus diesen Ergebnissen geht hervor, daß Cepharanthin die HIV-1-Produktion inhibierte. Der Prozentsatz der Inhibierung der HIV-1-Produktion stieg mit der Konzentration des zur Behandlung der Kulturen verwendeten Cepharanthins an.
- Darüber hinaus hatte Cepharanthin bei den verwendeten Konzentrationen keinen toxischen Effekt auf die CEM-CDC- Zellen, wie in Tabelle 2 gezeigt.

Ausbruchs von AIDS.

pharmazeutischen Zusammensetzung zur Inhibierung der
Vermehrung von AIDS-Virus im menschlichen Körper.
